Abstract:Hysteresis is already featured in most of single and double chamber pacemakers.The aim is to support the intrinsic rhythm of the heart.In the standard hysteresis the basic interval veil be lengthened after detection of intrinsic activity: the so-called hysteresis interval (HI).If no sense event occurs within the HI, a pacing pulse is emitted and the basic interval is reverted.To maintain and to promote the spontaneous rhythm, two new algorithms are now available: Repetitive and Scan Hysteresis.The Repetitive Hysteresis(RH) helps to maintain the spontaneous rhythm: following a sensed intrinsic event, the hysteresis interval is than repeated for a programmable number of cycles.The Scan Hysteresis (SH) promotes the spontaneous rhythm dudng phases of continuous stimulation: the pacemaker veil reduce the stimulation rate to the hysteresis rate after every 180 consecutive pace events during a programmable number of cycles.The aim of our study was to evaluate the promotion of the spontaneous sinus activity in DDD pacemakers by programming the different hysteresis settings; conventional hysteresis, repotithve and scan hysteresis -during 3 months periods within the same patient.Methods: A dual chamber pacemaker Biotronik Actms DR. was implanted in 25 patients.With time intervals of 3 months, beginning after hospital discharge all patients are programmed as followed;-Pre study follow-up: dudng the first 3 months with lower rate 70, hysteresis -20, repetitive and scan hysteresis OFF -Follow-up 1: dudng the second 3 months with lower rate 70, hysteresis -20, recetitive 3 and scan hysteresis 3 -Follow-up 2: during the third 3 months with lower rate 70, hysteresis -20, repetitive 3 and scan hysteresis OFF.Follow-up 3: in the last 3 months period lower rate 70, hysteresis -20, repetitive OFF and scan hysteresis 3.After every follow up period counter data were collected from the pacemaker event counter.The percentage atrial sensed events at follow up 1 (3 months period with conventional hysteresis) served as the control indicator.Results: From 11 patients (3 women, 8 men, mean age 75 +/-5 years) follow up data are available for Follow up 1 and 2 a.At Follow Up 2 after adding the Repetitive Hysteresis and the Scan Hysteresis of 3 cycles, during 3 months, the atrial sensed events increased with 45,8 % compared to the atrial sensing after a 3 months period with Standard Hysteresis.Conclusion: Our preliminary results with the repetitive and automatic hysteresis search scans disclosed a satisfactorily working algorithm with effective promotion of the spontaneous rhythm.
