Effect Comparison of Cardiac Dual-and Tri-chamber Pacmakers in the Prevention of Paroxysmal Atrial Tachyarrhythmias in Aging

Abstract:In order to compare the effect of cardiac dual and tri-chamber pacemaker in the prevention of paroxysmal atrial tachyarrhythmias (PAT) twenty-one aged patients, with pacemaker indication and characteristics of age ≥60 years, frequent PAT, no overt organic heart disease and refractory to antiarrhythmic agents were randomized into two groups. A dual chamber (right atrium and ventriculum) or a tri-chamber pacemaker (two atria and right ventriculum) was implanted into two groups respectively. They were followed up in 3, 6 and 12 months after discharge. Following parameters were compared: ①average frequencies of PAT per week and atrial premature beat per 24 hour, ②chronic pacemaker parameters and complications, ③side effects of antiarrhythmic agents. Result: All pacemakers were implanted successfully by one procedure without complication. Hospitalization was 9.6±2.3 days. During one-year follow-up, the pacemakers worked normally and no side effect of antiarrhythmic agents was found. In dual-chamber group, the frequencies of PAT and atrial premature beat were not significantly changed ( P 0.05). In tri-chamber group, above parameters were not changed in the first three months ( P 0.05). But in the 6 and 12 months, they were decreased significantly in comparison with both before operation and dual-chamber group ( P 0.05). Conclusion:Tri-chamber pacing could significantly decreases the frequency of PAT and the combination of antiarrhythmic agents in comparison with dual-chamber pacing.
