Abstract:High precision radiotherapy makes it important to research set-up errors in radiotherapy. This paper reviews the definition, sources and presentation of set-up errors, summarizes the reported research data and gives the recommendations of procedures to quantify, report and reduce patient set-up errors.

Abstract:Objective: To investigate the influencing factors of positioning errors in X-ray stereotactic radiotherapy for different region tumors,and discuss the quantity control methods for the practice process of precision radiotherapy positioning.Methods: From October 2011 to December 2012,63 patients underwent the Varian TrueBeam1103 linear accelerator,The selected patients were all treated with stereotactic radiotherapy,including 20 cases metastatic brain tumors、32 cases lung cancers and 11 cases liver cancers.All the patients were scanned by kilovoltage cone-beam computed tomography before the everytime radiotherapy.The acquired CBCT images were co-registered with planning CT and setup errors of iso-center positions on the axes of X,Y and Z were obtained for statistical analyses.Results: The systemic±random errors on lateral(x),vertical(y)and longitudinal(z)directions were x(0.82±0.83) mm,y(0.79±0.73) mm,z(0.88±0.81) mm for brain tumors;x(1.81±1.72) mm,y(1.08±1.02) mm,z(2.16±1.88) mm for the lung cancers;x(1.64±1.79) mm,y(1.10±1.12) mm,z(2.89±2.42) mm for the liver cancers,respectively.Conclusions: The replacement errors always occurs in the process of radiotherapy for different region tumors.The position reproducibilities of brain tumors were markedly better than those of different region tumors.Only by thorough understanding of the causes of errors can we adopt the specific measures to reduce it.Choosing correct body immobilization technology and CBCT verification of the target point for intensity modulated radiation therapy has an important role in the precise implementation.
The Impact of Set-Up Errors on Dose Distribution During the Image-Guided Intensity-Modulated Radiotherapy in Elderly Patients with Prostate Cancer

Abstract:Objective To analyze the impact of set-up errors on planning target volume (PTV) for tumors and radiation dose distributions,then to investigate the necessity of on-line set-up error correction for prostate cancer during IMRT.Methods Cone-beam CT (CBCT) scanning images in 16 patients with prostate cancer were acquired with a total of 214 times during IMRT.The CBCT images and the original planning CT images were matched to acquire the translational errors of the x,y and z axis.The dose distributions were recalculated based on the data of each set-up error.The PTV,radiation dose distribution in organs at risk including bladder and rectum,and dose volume parameters were calculated in the re-planning,and then compared with the original planning by paired t-test.Results The PTV 95％ dose volume (D95) and PTV minimal dose volume (Dmin) were lower in the re-planning than in the original planning after 214 times set-up (7613.73 cGy vs.7764.25 cGy,80.2％ vs.86.8％,t=2.494 and 5.536,P=0.025 or ＜0.001),but it could be acceptable.Bladder received 50 Gy dose volume (V50),30％ bladder volume dose (D30),rectume received 50 Gy dose volume (V50) and 25％ rectume volume dose (D25) in the re-planning had no statistical differences with those in the original planning.Conclusions The set-up errors lead to the decrease of PTV D95 but be acceptable.The radiation dose received by bladder and rectum has no obvious changes in the re-planning as compared with the original planning.The on-line set-up error correction for prostate cancer may improve the treatment accuracy during IMRT.
EPID for Set-up Errors Analysis and Quality Assurance of Radiotherapy in Head and Neck Cancer

Abstract:Objective: This study was to investigate the set-up error using electronic portal imaging device (EPID) for intensity modulated radiation ( IMRT), assess of the CTV-to-PTV margins, analyze the factors on causing the error periodically, and to improve the quality control level. Methods: From October 2007 to January 2008, twelve, eleven, seventeen, and sixteen patients with head and neck carcinoma were enrolled in this study. The patients were immobilized with the head/neck shoulder immobilization system on margins. The set-up errors were analyzed by comparing the films of digitally reconstructed radiographs (DRR) with EPID. Electronic portal imaging was performed for twice each week before each treatment at the position AP (gantry angle 0°) and LT (gantry angle 90°), respectively. CTV-to-PTV margins were calculated by the equation 2.0Σ+0.7σ (Σ is the systematic error, and σ is the random error) in the end of each month, the factors which might be the causes were analyzed and some quality control methods were proposed. Results: The theoretical CTV-to-PTV margins calculating by the equation was declined gradually during the previous three months and stable in the fourth month. Conclusions: This study supplied theoretical evidences and validated CTV-to-PTV margins for our department of radiotherapy by analyzing the set-up errors. This strategy may diminish the unnecessary irradiation fields and enhance the treatment outcome through the technique improvement.
Impact of treatment set-up errors on accuracy of post-operation radiotherapy in patients with breast cancer

Abstract:Background and purpose:The precise radiotherapy in patients with breast cancer consists of multiple procedures,such as simulation,treatment planning and delivery.We assessed the impact of treatment set-up errors on accuracy of post-operative radiotherapy(RT) in patients with breast cancer receiving tangential RT.Methods:Between November 2007 and August 2008,a total of 50 patients were enrolled an institutional review board-approved study.All patients received tangential RT for either right or left breast cancer.Read-out numbers of X,Y,Z directions shown on linear accelerator were recorded according to the sequence of before and after medial tangential fi eld and after lateral tangential fi eld.The impact of treatment set-up errors on postoperative radiotherapy(RT) in different subgroup patients was analyzed using stata 7.0 statistics software.Results:The mean set-up errors on Z,Y,X direction were 2.55,3.96 and 0.58 cm,respectively.The largest set-up errors in all three directions were observed in the first week during treatment and the largest movement was observed in Z direction.Conclusion:The movements after medial tangential fi eld treatment are much more than those after lateral treatment during RT.The movements in X direction are substantially less than Y direction or Z direction intrafraction.As interfraction differences have been observed,the movements in the beginning two weeks are more than other treatment period.

Abstract:Accurate information on set-up error during radiotherapy is essential for determining the optimal number of treatments in hypofractionated radiotherapy for prostate cancer. This necessitates careful control by the radiotherapy staff to assess the patient's condition. This study aimed to develop an evaluation method of the temporal trends in a patient's specific prostate movement during treatment using image matching and margin values. This study included 65 patients who underwent prostate volumetric modulated arc therapy (mean treatment time, 87.2 s). Set-up errors were assessed using bone, inter-, and intra-fraction marker matching across 39 fractions. The set-up margin was determined by dividing the four periods into 39 fractions using Stroom's formula and correlation coefficient. The intra-fraction set-up error was biased in the anterior-superior (AS) direction during treatment. The temporal trend of set-up errors during radiotherapy slightly increased based on bone matching and inter-fraction marker matching, with a 1.6-mm difference in the set-up margin fractions 11 to 20. The correlation coefficient of the mean prostate movement during treatment significantly decreased in the superior-inferior direction, while remaining high in the left-right and anterior-posterior directions. Image matching contributed significantly to the improvement of set-up errors; however, careful attention is needed for prostate movement in the AS direction, particularly during short treatment times. Understanding the trend of set-up errors during the treatment period is essential in numerical information sharing on patient condition and evaluating the margins for tailored hypo-fractionated radiotherapy, considering the facility's image-guided radiation therapy technology.

Abstract:Objective To measure the set-up errors of patients with head and neck (H&N) cancer during the image guided intensity-modulated radiotherapy (IMRT) treatment and analyze the impact of setup errors on dose distribution ; then to further investigate the necessity of adjustment online for H&N cancer during IMRT treatment.Methods Cone-beam CT (CBCT) scanning of thirty patients with H&N cancer were acquired by once weekly with a total of 6 times during IMRT treatment.The CBCT images and the original planning CT images were matched by the bony structure and worked out the translational errors of the x,y,z axis,as well as rotational errors.The dose distributions were recalculated based on the data of each setup error.The dose of planning target volume (PTV) and organs at risk were calculated in the replanning,and than compared with the original plan by paired t-test.Results The mean value of x,y,z axis translational set-up errors were ( 1.06 ± 0.95 ) mm,( 0.95 ± 0.77 ) mm and ( 1.31 ± 1.07 ) mm,respectively.The rotational error of x,y,z axis were ( 1.04 ±0.791 ),( 1.06 ±0.89) and (0.81 ±0.61 ),respectively.PTV 95％ volume dose ( D95 ) and PTV minimal dose of replanning for 6 times set-up were lower than original plan (6526.6 cGy:6630.3 cGy,t =3.98,P =0.000 and 5632.6 cGy:5792.5 cGy,t =- 2.89,P =0.007).Brain stem received 45 Gydose volume ( V45 ) and 1％ brain stem volume dose ( D01 )were higher than original plan ( 3.54％:2.75％,t =3.84,P =0.001 and 5129.7 cGy:4919.3 cGy,t =4.36,P =0.000).Conclusions The set-up errors led to the dose of PTV D95 obviously insufficient and significantly increased V45,D01 of the brainstem.So,adjustment online is necessary for H&N cancer during IMRT treatment.

Abstract:Objective To analyze the set-up errors in the left-right( LR, X axis) , superoinferior ( SI, Y axis) and antero-posterior (AP, Z axis) directions and the rotations during radiotherapy of prostate cancer in the supine position. Methods Twenty-five prostate cancer patients who received radiotherapy from October 2011 to June 2013 were analyzed. All the patients were treated with intensity-modulated radiotherapy ( IMRT) in the supine position after external immobilization using thermoplastic shell. Measurements for set-up errors and rotations in X, Y, Z directions were collected from daily image-guided radiotherapy( IGRT, bony match) pre-treatment. The set-up errors and rotations were analyzed. Results Each patient received 9 fractions of measurement with a total of 225 fractions. Mean set-up errors were (0. 19±0. 18)cm, (0. 36±0. 30)cm, and(0. 21±0. 16)cm in the X, Y and Z axes, respectively. The set-up errors of≥5 mm was 5. 8%, 24. 3%, and 8. 0% of all measured fractions in the X, Y and Z axes, respectively. Mean rota-tions were (1. 07±1. 03)°, (0. 82±0. 66)°, and (0. 79±0. 68)°in the axis, vertical, and lateral directions, respectively. After com-paring the first 5 and the following 4 set-up and rotation errors, there were no significant differences( P>0. 05) . Conclusion The set-up errors is larger in the Y than in the X and Z axes during radiotherapy of prostate cancer in the supine position. The rotation errors are much smaller and can be omitted.
Research on the correlation of setup errors for head and neck cancer patients with radiotherapy based on image guiding

Abstract:Objective:To explore the correlation of six-dimensional set-up errors in head and neck tumors radiotherapy to provide the basis to improve the clinical setup accuracy.Methods: 30 patients with nasopharyngeal carcinoma who had been treated by Tomotherapy HD System were selected. The correlation of set-up errors were retrospectively analyzed in six-dimensional (Left-Right direction: X, inferior-superior direction: Y, anterior-posterior direction: Z, left-right rotation: roll, anterior-posterior rotation: pitch, counterclockwise or clockwise rotation: yaw) using the IGRT data.Results:X had no correlation with Y and Z and Y had significant weak negative correlation with Z on three-dimensional directions. The roll and pitch had significant weak negative correlation on three-rotation directions. On the three-dimensional and three-rotation directions, X and roll had significant weak negative correlation, X and yaw had significant moderate positive correlation, Y had significant weak negative correlation with pitch and yaw, and Z and pitch had significantly strong negative correlation. Conclusion: More attention have been paid on the correlation of different directions, especially, the accuracy between Z and pitch or/and between Y and Z in the set-up process.
Evaluation of Set-up Errors for 493 Patients by Simulator

Abstract:Objective: To evaluate set-up error by Varian Acuity simulator.Methods: From October 2009 to December 2010,493 patients were included in this study,including 167 receiving head and neck irradiation,210 thoracic irradiation and 116 abdominal irradiation.All patients were immobilized with fixation devices.DRRs of each patient were compared with portal images from simulator and set-up errors were recorded and analyzed.The shift margin from CTV or ITV to PTV was calculated.Results: In head and neck region,the set-up errors in Right-Left,Cranial-Caudal and Anterior-Posterior directions were(0.13±0.17)cm、(0.21±0.27) cm and(0.21±0.23)cm.In thoracic region,the set-up errors in R-L,C-C and A-P directions were(0.24±0.27)cm、(0.24±0.32) cm and(0.27±0.29)cm.In abdominal region,the set-up errors in R-L,C-C and A-P directions were(0.22±0.29)cm、(0.25±0.29)cm and(0.19±0.29)cm.And the margins from CTV or ITV to PTV were given.Conclusions: Set-up errors and margins from CTV or ITV to PTV can be obtained by simulator.
A Clinical Evaluation of Setup Errors in IMRT Using Cone Beam CT for Head and Neck Cancer

Abstract:[Purpose]To investigate the linear and rotational setup errors in intensity modulated radiation therapy (IMRT) using kV cone beam CT (CBCT) on IGRT for head and neck cancer. [Methods] Twelve patients with head and neck carcinoma were treated by Elekta Synergy IGRT with a total of 217 times.X-ray volumetric images(XVI) acquired pre-,intra-(after correcting the setup error),and post-radiation,were compared with planned CT images to get informations of linear and rotational setup errors of X-axis,Y-axis,Z-axis.[Results] A total of 217 CBCT scans were applied for 12 patients pre-radiation.The systemic±random errors on linear X,Y,Z directions were (-0.04±2.63)mm,(0.07±1.69)mm,(-1.15±1.33)mm,on rotational X,Y,Z directions were (0.10°±1.14°)、(0.16°±1.41°)、(-0.06°±1.22°)respectively. After correction of all the setup errors,both the systematic errors and random errors were obviously less than those before correction.The difference of setup errors between intra-radiation (after correcting the setup error) and post-radiation was not significant. [Conclusion] Real-time correction of setup error in patients with head and neck cancer by CBCT can reduce the setup error and decrease planning target volume (PTV) extending. And the setup error in fractionated radiation for head and neck cancer is small.

Abstract:Objective To analyze the source of set-up error in electronic portal imaging device(EPID), and decrease the errors caused by poor radiation therapy and study the quality assurance measures for course of treatment. Method From October 2014 to October 2015, 50 patients with head tumor and 50 patients with body tumor were enrolled, which included 68 males and 32 females, aged 35-76 years old with mean age of 53 years old. All the patients were treated by conventional position-ing and intensity modulate radiotherapy(IMRT), and positioned once a week, respectively. The set-up errors in direction of head and foot(Z axis), left and right(X axis), and vertical(Y axis) were measured. Once the errors exceed 5 mm in head tumor group and 10 mm in body tumor group, the causes were analyzed by physicist and physician. Result For each patient there were about 5-8 times EPID validation in the whole process of radiotherapy. The error of set-up below 5 mm in head tumor group was 98.2%and the error of set-up below 10 mm in body tumor group was 90.0 %. For patients with head tumor, the error of set-up in X axis, Y axis and Z axis were below 5 mm, the difference was no statistically significant(t=1.65, P>0.05). For body tumor patients, the error of set-up in X axis, Y axis and Z axis were below 5 mm, the difference was no statistically significant (t=1.43, P>0.05). There was significant difference between head and body in set-up of Z axis by breathing movement(P<0.05), which showed good stability in radiotherapy of head tumor patients. Conclusion The study demonstrated that the source of set-up error are complicated during the clinical operation of radiation therapy, the EPID set-up verification could reduce the target error and improve the local control rate and safety of tumor target area. In the whole procedure, the analysis of set-up error of EPID is particularly important.
Set-up errors in three-dimensional conformal/intensity-modulated radiation therapy measured with repeat CT scans for reducing taget volume

Abstract:Objective To study the set-up errors of thermoplastic mask used for the immobilization of patients treated by three-dimensional conformal/intensity- modulated radiation therapy measured with repeat CT scans in order to reduce target volume. Methods Seventy-seven patients underwent CT scans before and during treatment, and the second CT scan used to reduce target volume was as same as the first scan in the position of patients, immobilization, scanning parameters and scopes. The same anatomical bony landmarks were selected to establish the origin axis, and were measured the shift of left, middle, right marks in X, Y and Z direction, respectively. During the treatment, the absolute values of the shift of three marks was set-up errors. Results Patients in the study had no consistency of the shift of left, middle and right marks in CT scans. Set-up errors in head, head and neck and shoulder positions were small, but they were large in thoraci, abdominal and pelvic positions, and both were larger than 3 mm. The margins from clinical target volume (CTV) to planning target volume (PTV) was 3-8 mm in head, head and neck and shoulder positions and they were 6-16 mm in thoraci, abdominal and pelvic positions. Conclusions Even if the laser landmarks matched very precise, an offset was still emerged comparing to the bony anatomy. Radiation therapy must be under sufficient quality control measures to ensure treatment accuracy, while it must consider carefully to delive the intensity-modulated radiation therapy in the thoraci, abdominal and pelvic tumor. Imageguided radiation therapy (IGRT) may be one of the solutions.
Application of a New Method for Patient Positioning Based on Optical Surface Monitoring Technology to Head Radiotherapy

Abstract:Objective:To develop a new method to set up patients using optical surface monitoring system and to compare it with the conventional method in head radiotherapy.Methods:A total of 358 set-ups (130 with the conventional method and 228 with the new method ), which were from 99 head tumor patients in Beijing Cancer Hospital treated between May 2018 to April 2019, obtained by using Image Guided Radiotherapy were retrospectively analyzed. The distributions of set-up errors, the number of abnormal positions, and the set-up time were compared to evaluate the potential advantages of the new method .Results:The mean (± standard deviation) absolute values of setup errors of the new method were (0.07±0.07) , (0.08±0.06) and (0.06±0.06) cm for the vertical, lateral, and longitudinal, (0.53±0.41)°, (0.59±0.44)° and (0.59±0.46)° for the rotation, pitch and roll, respectively. In the new method , the setup accuracy was improved( t=3.24-6.10, P<0.001)and the number of abnormal positions was greatly reduced(χ 2=60.66, P<0.001). Compared with the conventional method, the patient setup time was slightly reduced by the new method , but the difference was not statistically significant ( P>0.05). Conclusions:The new high-precision method to set up patients using optical surface monitoring system improves the accuracy of patients′ position, decreases the corrections applied by 6DoF couch, reduces the probability of abnormal positions, and suggests the potential benefit in head radiotherapy.
Effects of error on dose of target region and organs at risk in treating nasopharynx cancer with intensity modulated radiation therapy

Abstract:Objective: To measure setup error of head and neck neoplasm in radiotherapy and discuss over effects of error on physical dose acting on target region and organs at risk of nasopharynx cancer (NPC) patients treated with intensity modulated radiation therapy (IMRT). Methods: A total of 152 patients who developed head and neck neoplasm and received IMRT were randomly selected. Through comparing digital portal image and digital reconstruction image, we measured setup error, calculated expanding margin from clinical target volume (CTV) to planning target volume (PTV) and analyzed whether there was rules between setup error and treatment time. Additionally, 20 cases of NPC were selected. Three-dimensional error was simulated in planning system. Dose distribution was recalculated and a series of dose parameters of target volume and OAR were analyzed. Results: Setup error in left-right, head-feet and ventral-dorsal direction was (-0.62±1.46) mm, (-0.41±1.54) mm and (-0.31±1.67) mm respectively. Regarding limit value, the maximum and minimum value in left-right direction, head-feet direction and ventral-dorsal direction was 2.70 mm and -6.00 mm; 3.00 mm and -5.00 mm, 5.00 mm and -7.50 mm. Expanding margin from CTV to PTV was 2.26 mm, 1.88 mm and 1.97 mm in left-right direction, head-feet direction and ventral-dorsal direction. Conclusion: During IMRT, only when setup error is controlled below 3 mm can sharply reduce the damage caused by radiation to normal tissue; therefore, quality security and control of electronic portal imaging device need (EPID) to be improved.
Positioning Errors Assessed with Kv Cone-Beam CT for Image-Guided Prostate Radiotherapy

Abstract:Objective To assess set-up errors measured with kilovoltage cone-beam CT (KV-CBCT), and the impact of online corrections on margins required to account for set-up variability during IMRT for patients with prostate cancer. Methods Seven patients with prostate cancer undergoing IMRT were enrolled onto the study. The KV-CBCT scans were acquired at least twice weekly. After initial set-up using the skin marks, a CBCT scan was acquired and registered with the planning CT to determine the setup errors using an auto grey-scale registration software. Corrections would be made by moving the table if the setup errors were considered clinically significant ( i. e. , ＞ 2 mm). A second CBCT scan was acquired immediately after the corrections to evaluate the residual error. PTV margins were derived to account for the measured set-up errors and residual errors determined for this group of patients. Results 197 KV-CBCT images in total were acquired. The random and systematic positioning errors and calculated PTV margins without correction in mm were:a) Lateral 3. 1,2. 1,9. 3;b) Longitudinal 1.5, 1.8, 5. 1 ;c) Vertical 4. 2,3.7, 13.0. The random and systematic positioning errors and calculated PTV margin with correction in mm were:a) Lateral 1.1,0. 9, 3.4;b) Longitudinal 0. 7, 1.1, 2. 5;c) Vertical 1.1, 1.3, 3.7. Conclusions With the guidance of online KV-CBCT, set-up errors could be reduced significantly for patients with prostate cancer receiving IMRT. The margin required after online CBCT correction for the patients enrolled in the study would be appoximatively 3-4 mm.
Effects of the Setup Error on the Physics Dose for Nasopharyngeal Carcinoma Undergoing the Intensity Modulated Radiation Therapy

Abstract:Objective: To measure the setup error undergoing the radiation therapy for head-neck neoplasms,and to analyze the effect of the setup error on the physics dose of targets and peripheral organs at risk(OAR)for nasopharyngeal carcinoma(NPC)undergoing the intensity modulated radiation therapy (IMRT). Methods: Seventy-six patients with head-neck neoplasms were choosed at random,measured the setup errors through comparing their digital portal films with their digitally reconstructed radiographs(DRR). Other ten NPC patients in hospital were choosed,simulated the patients' three-dimensional errors in actual therapy on the treat planning system (TPS),and recalculated their dose distribution. We analyzed a series of relative targets and OAR dose parameters,obtained the impact of the setup errors on the physics dose. Results: The setup errors were (-0.62±1.46) mm,(-0.41±1.54) mm and(-0.31±1.67) mm in left-right,cranial-caudal and ventral-dorsal directions respectively. If the setup errors exceed 3 mm,they have statistics signification (P0.05) for GTV in low dose,few parameters in CTV,and spinal cord and brainstem in ventral-dorsal direction. Conclusions: For the patients with NPC undergoing the IMRT,the setup errors should be controlled within 3 mm. So in the daily work,a series of strict QA and QC work through EPID(electronic portal imaging device) is very important.
Workflow and error analyses of patient setup based on open-face mask immobilization combined with AlignRT for head tumor radiotherapy

Abstract:Objective:To propose a markless patient setup workflow based on the optical surface monitoring system (AlignRT) and open-face mask immobilization for whole-course head tumor radiotherapy, assess the setup time and repositioning frequency of the proposed workflow, and conduct a comparative analysis of the differences, correlation, and consistency of the setup errors of the AlignRT and cone beam CT (CBCT) systems.Methods:A retrospective analysis was conducted for the data on the errors of 132 fractionated setup based on open-face mask immobilization of 33 head tumor patients. AlignRT-guided markless patient setup workflow was applied throughout the radiotherapy. Meanwhile, the body structures automatically generated by the treatment planning system were used as body references. The 6-degree-of-freedom (6DoF) setup errors (lateral, vertical, longitudinal, rotation, pitch, roll, and yaw directions), setup time, and repositioning frequency of the AlignRT and CBCT systems were recorded and analyzed. The Wilcoxon and Spearman analyses were used to statistically assess the differences and correlation of the setup errors of the two systems. Moreover, the Bland-Altman analysis was employed to evaluate the consistency of the two systems.Results:The 6DoF setup errors of CBCT were within the clinical tolerance (linear motions: -0.30 to 0.30 cm; rotational motions: -2.0° to 2.0°). The setup time and repositioning frequency of CBCT were (98 ± 31) s and 1.51% (2/132), respectively. There was no significant difference in setup errors between the two systems except those in x-axis ( Z = -3.11, P= 0.002), y-axis ( Z = -7.40, P<0.001), and Pitch ( Z= -4.48, P<0.001). There was a significant positive correlation between the setup errors along lateral ( rs = 0.47, P<0.001) and vertical ( rs = 0.29, P = 0.001) directions, rotation (Rtn; rs = 0.47, P<0.001), pitch (Pitch; rs = 0.28, P = 0.001) and roll (Roll; rs = 0.45, P<0.001) of the two systems. The 95% limits of agreement (95% LoA) of 6DoF setup errors were -0.12 to 0.09 cm, -0.07 to 0.17 cm, -0.19 to 0.20 cm, -1.0° to 0.9 °, -1.0° to 1.5°, and -0.9° to 1.0°, respectively. The 95% confidence interval (95% CI) of 95% LoA was -0.14 to 0.11 cm, -0.09 to 0.19 cm, -0.23 to 0.23 cm, -1.2° to 1.1°, -1.2° to 1.7°, and-1.0° to 1.1°, respectively, all of which were within the permissible error ranges. The 6DoF setup error difference of 3.41% (27/792< 5%) was beyond the 95% LoA. The maximum absolute differences of 6DoF setup errors within the 95% LoA were 0.12, 0.16, 0.19 cm, 0.9°, 1.5°, and 1.0°, respectively. Conclusions:The proposed markless setup workflow based on AlignRT combined with open-face mask immobilization for whole-course head tumor radiotherapy exhibits reasonable agreement and consistency with the patient setup using CBCT, with acceptable clinical efficiency. It can be applied to the first radiotherapy and the real-time monitoring of therapy to improve the safety and thus is of value in clinical applications.
Companison of Dose Distribution and Setup Error of Two Different Positions and Immobilization Techniques in Breast Cancer Radiotherapy

Abstract:Objective To compare the dose distribution and setup error of angled breast board position ( ABB) with plain breast board position ( PBB) in breast cancer radiotherapy. Methods Twenty consecutive postoperative left breast cancer patients in the First Affiliated Hospital, Zhejiang University were enrolled from March 2017 to January 2018. All cases were assigned into the ABB and PBB groups according to positions and there were 10 cases in each groups respectively. The target volume, heart and lung structures were defined on the CT images of the localized scan. The plan was designed using the field in field ( FIF) technique to compare the dosimetric parameters of the PTV, lung, and heart treatments, and the setup errors for the two different positions. Results The ipsilateral lung V20[ABB:(11.2 ±3.2)％, PBB:(15.9 ±5.3)％, t= -2.47,P< 0.05], and V30[ABB:(9.8 ±1.5)％, PBB:(12.9 ±2.2)％, t= -4.46,P<0.05] were both statistically significant for the two different position and immobilization. Heart dose V25[ABB: (1.9 ± 0.2)％, PBB: (2.8 ± 0.4)％, t = -8.28, P <0.05], V30 [ABB:(1.8 ±0.1)％, PBB: (2.7 ± 0.3)％, t = -8.34,P < 0.05], and Dmean of heart [ABB: (3.0 ±0.5)Gy, PBB:(5.3 ± 1.2)Gy, t=5.58,P<0.05] were all statistically significant for the two different positions. The translational errors of ABB and PBB on LR, SI, and AP were ( 3.23 ± 2.63 ) , ( 5.42 ± 3.22), (4.58 ± 2.30) mm, and (2.35 ± 1.22), (2.17 ± 1.29), (2.27 ± 1.58) mm, respectively. The rotation errors of pitch(θ), yaw(Φ) and roll(ψ) for ABB and PBB were (1.60 ± 0.56)°, (3.40 ± 1.65)°, (2.50 ± 1.72)°, and (1.37 ± 0.43)°, (1.79 ± 0.71)°, (2.06 ± 0.63)°, respectively. Meanwhile, the in-and out-SI, anterior and posterior AP, yaw rotation error (Φ) were also statistically significant(t=3.06, 2.80, 3.33,P<0.05). Conclusions There is no statistically significant difference in the tumor target between the two position and immobilization techniques. However, the ABB is better than the PBB in normal tissue sparing while the setup accuracy of PBB is better than the ABB.
Setup Errors Analysis in Iterative Kv CBCT: A Clinical Study of Cervical Cancer Treated with Volumetric Modulated Arc Therapy.

Abstract:Objective This study aims to analyze setup errors in pelvic Volumetric Modulated Arc Therapy (VMAT) for patients with non-surgical primary cervical cancer, utilizing the onboard iterative kV cone beam CT (iCBCT) imaging system on the Varian Halcyon 2.0 ring gantry structure accelerator to enhance radiotherapy precision. Method We selected 132 cervical cancer patients who underwent VMAT with daily iCBCT imaging guidance. Before each treatment session, a registration method based on the bony structure was employed to acquire iCBCT images with the corresponding planning CT images. Following verification and adjustment of image registration results along the three axes (but not rotational), setup errors in the lateral (X-axis), longitudinal (Y-axis), and vertical (Z-axis) directions were recorded for each patient. Subsequently, we analyzed 3642 iCBCT image setup errors. Results The mean setup errors for the X, Y, and Z axes were 4.50 +/- 3.79 mm, 6.08 +/- 6.30 mm, and 1.48 +/- 2.23 mm, respectively. Before correction with iCBCT, setup margins based on the Van Herk formula for the X, Y, and Z axes were 6.28, 12.52, and 3.26 mm, respectively. In individuals aged 60 years and older, setup errors in the X and Y axes were significantly larger than those in the younger group (p < 0.05). Additionally, there is no significant linear correlation between setup errors and treatment fraction numbers. Conclusion Data analysis underscores the importance of precise Y-axis setup for cervical cancer patients undergoing VMAT. Radiotherapy centers without daily iCBCT should appropriately extend the planning target volume (PTV) along the Y-axis for cervical cancer patients receiving pelvic VMAT. Elderly patients exhibit significantly larger setup errors compared to younger counterparts. In conclusion, iCBCT-guided radiotherapy is recommended for cervical cancer patients undergoing VMAT to improve setup precision.
