Abstract:Objective:To evaluate the clinical value of retroperitoneal laparoscopic radical nephrectomy.Methods: 247 patients were confirmed with renal carcinoma by B ultrasonic examination,IVU plus KUB and(or) retrograde urography.148 cases were right renal cell carcinoma,99 cases were left renal cell carcinoma.TNM stage:T _1N_0M_0 188 cases,T_2N_0M_0 59 cases.All cases were underwent transabdominal or retroperitoneal laparoscopic radical nephrectomy.The operation time,blood loss,postoperative drainage volume,intestinal function recovery time and postoperative length of stay were observed.Results:The operating time was from 80 to 150 minutes( mean 120 minutes),the blood loss was from 100 to 300 ml(mean 200 ml),the postoperative drainage volume was from 100 to 300 ml(mean 200 ml).Removed drainage tube in 3-5 days.Postoperative intestinal function recovery time was 2-3 days with transabdominal approach,was 1-2 days with retroperitoneal approach.The postoperative hospital stay was from 7 to 9 days(mean 8 days).There was no significant difference in transabdominal and retroperitoneal approach.All cases were clear cell carcinoma by pathological reports.The postoperative follow-up time was from 3 to 36 months(mean 15 months).There is no recurrence and metastasis in tumor.Conclusions:The transabdominal or retroperitoneal laparoscopic radical nephrectomy has the merits of mini-invasion,less bleeding and rapid recovery.It is significantly better than open surgery.Transabdominal approach is with good vision,and retroperitoneal approach is with non intrusive.

Abstract:Objective To evaluate the clinical effect of retroperitoneal laparoscopic nephron-sparing surgery.Methods From March 2007 to May 2008,15 patients with local renal tumors underwent retroperitoneal laparoscopic partial nephrectomy by a single surgeon.Hilar control was achieved with bulldog clamps.Hook-cautery is used to circumscribe the tumor for resection by scoring the renal capsule circumferentially with a 2-10 mm margin around the tumor.Specimens are randomly extracted through tumor edges and immediately given to pathological frozen sections.The operative time,blood loss,postoperative stay,complications and the time of complete convalescence were recorded.Results Mean operative time was 110 minutes and mean time of hilar control was 26 minutes.Mean blood loss was 200ml.Mean postoperative stay was 9 days.Final pathological evaluation revealed negative positive margins in these cases.During 2-14 months follow-up there was no local recurrence.Conclusions Retroperitoneal laparoscopic partial nephrectomy for small renal tumors can be performed safely and effectively.Extended follow-up period is required to determine its long-term functional and oncological outcomes.
Experience and Value of Retroperitoneal Laparoscopic Nephron-Sparing Surgery:Report of 35 Cases

Abstract:Objective To investigate the experience and value of retroperitoneal laparoscopic nephron-sparing excision.Methods Between September 2005 and May 2009,35 cases underwent the retroperitoneal laparoscopic nephron-sparing surgery.Results All procedures were technically successful.The mean operating time was 85 min,and mean ischemic time was 25 min.The mean estimated blood loss was 240 mL.The mean hospital stay after surgery was 9d.No intraoperative complications occurred.No local recurrence or trocar site metastasis was observed during a mean follow-up period of 1-30 months.Conclusion Retroperitoneal laparoscopic nephron-sparing surgery represents a feasible option for the patients with localized small renal tumors.It offers a new effective and minimally invasive treatment for selected patients.
Retroperitoneal laparoscopic radical nephrectomy: Report of 126 cases

Abstract:Objective To evaluate the efficacy of retroperitoneal laparoscopic radical nephrectomy.Methods A total of 126 patients underwent retroperitoneal laparoscopic radical nephrectomy from November 2002 to June 2006.The operation was performed through 3 lumbar ports.The renal artery and veins were blocked with the Hem-o-lok and then severed;the ureter was severed near the iliac blood vessel.Results A conversion to open surgery was required in 2 patients because of bleeding and in 1 patient because of severe adhesion.The mean operation time was 115 min(range,60~255 min) and the mean blood loss was 112ml(range,20~1000 ml).Blood transfusion was required in 2 patients.Complications occurred in 7 patients(spermatic vein injury in 1 patient,accessory renal artery injury in 1 patient,inferior vena cava injury in 1 patient,hemorrhage of renal artery stump in 1 patient,renal vein injury in 2 patients,and pancreatic tail injury in 1 patient).The patient with inferior vena cava injury was followed for 2 years and no abnormal findings were found.The patient with pancreatic tail leakage was followed for 1 year and no pancreatitis or liquid in the renal fossa was detected.The remaining 5 patients with complications presented no abnormal findings.The mean postoperative hospital stay was 5 days(range,4~8 days).Pathological results included renal clear cell carcinoma in 117 patients,cystic renal cell carcinoma in 3 patients,renal pelvis transitional cell carcinoma in 2 patients,chromophobe carcinoma in 1 patient,hamartoma in 2 patients,and oxyphil cell tumor in 1 patient.One patient developed a liver metastasis 6 months after operation,and died 1 year after operation.The rest of 125 patients were followed for a mean of 27 months(range,1~43 months),and survived without renal fossa or incision metastasis,or distant recurrence.Conclusions Retroperitoneal laparoscopic radical nephrectomy is a safe,reliable,and effective technique for renal tumors.
Technical Improvements in Open Radical Nephrectomy with Renal Carcinoma

Abstract:Objective:To evaluate the effect of technical improvements of open radical nephrectomy with renal carcinoma.Methods:Technical improvement included nephrectomy with choosing lumbar incision and expose the nephridium, cut open the stretch board between lateral conal fascia and lumbar quadrate muscle fascia along the outer margin of quadratus lumborum. According to the sequence of post-upper-fore, dissociate anatomically anterior and posterior diastema of perirenal fascia. Isolate the vessel of kidney, and successively ligture renal arteries and veins, then remove kidney and tumor. Results:310 patients who account for 91% in the synchronization received this kind approach. All tumor diameter was less than 10 cm, stage Ⅰaccount 69%, stage Ⅱ30%, stage Ⅲ1%, stage Ⅳ 3%. The mean time of operation was 90 minutes, and the mean bleeding quantity was 50 ml. 5 patients had complications, among them, two cases was injury of pleura, one was injury of vein cava and bleeding, the other two cases emerged shock due to chronic bleeding during 4-6 hours after operation. 281 patients (91%) had no recarrence in the nephridial pit after 1-36 months follow-up.Conclusions:Anatomical and sharp radical nephrectomy through lumbar approach is an effective and safe radical method for renal cancer.

Abstract:Objective:To analyze the relationship between laparoscopic characteristics of upper urinary tract anatomy and clinical effects of retroperitoneal laparoscopic radical nephrectomy in order to explore surgical skills of retroperitoneal laparoscopic radical nephrectomy.Methods:Sixty-three cases with localized renal carcinoma were enrolled in this study.Among them,tumor of 35 cases were in the left kidney and 28 cases were in the right kidney.The maximum diameter of tumor were 1.5cm to 8.2cm with an average diameter 3.6cm.All patients underwent retroperitoneal laparoscopic radical nephrectomy according to characteristics of upper urinary tract anatomy.Routinely,retroperitoneal space was created and laparoscopic dissociation of dorsomedial kidney was performed in bloodless plane.After ureter was dissected,renal pedicle was dissocaited and Hem-O-lock ligation of renal vessels was done before clipping the vessels.Then,fully free kidney was found when dissociation around the renal anterior diastema was finished.Results:All the operations were successful with no case transferred to open operation.No case had blood transfusion and severe complications during surgery.The mean operative time was 48min(range from 35 to 90min).The mean intraoperative blood loss was 40ml(range from 20 to 150ml).All patients discharged from hospital 3 to 7 days after surgery.Follow-up was carried out for 5 to 34 months in 62 patients.Only one case suffered from retroperitoneal lymph node metastasis.Others had no local recurrence or distant metastasis by ultrasound and chest X-ray inspection.Conclusions:Dissociation of perirenal space in bloodless plane and related surgical skills of handling renal pedicle according to laparoscopic characteristics of upper urinary tract anatomy can reduce intraoperative blood loss and complications.

Abstract:Objective To evaluate the clinical effective of radical retropubic prostatectomy and study the methods of decreasing its postoperative complications. Methods Clinical data of 53 cases who had undergone radical retropubic prostatectomy from June 2000 to April 2006 were retrospectively analyzed.neurovascular bundles were preserved during the prostatectomy in 9 of the 53 cases. Results Mean operating time was 140 minutes,and the average blood loss was 700ml.There were no operative deaths.3 patients were complicated with stress incontinence and no patients were with totally incontinence. Conclusion Radical retropubic prostatectomy is a safe and effective method for treating prostatic carcinoma,and the nerve-sparing operation can preserve the erectile function of the patients satisfactorily.

Abstract:Objective:To investigate the surgical technique and clinical effect of retroperitoneal laparoscopic nephrectomy.Methods:From June 2003 to June 2008,a total of 54 patients have undergone retroperitoneal laparoscopic nephrectomies in our department,with 14 cases undergoing radical nephrectomies,2 cases undergoing radical nephroureterectomies and 38 cases undergoing simple nephrectomies.The tumors were averagely(3.8±1.2)cm(range 2.0 to 6.0cm)in diameter,with 9 cases at T1aN0M0 stage,5 cases at T1bN0M0 stage.Among the 38 simple nephrectomy cases,12 cases were kidney tuberculosis,22 cases were hydronephrosis,and 4 cases were renal arterial stenosis.All the kidneys were confirmed to have no function by radiological renal pictures.The operating time,intraoperative blood loss volume,and the time of hospitalization were recorded.Results:Fifty-three cases were completed successfully.Only one case was converted to open operation because of severe conglutination.The operating time was 65-185min,mean(93.5±28.0)min,and the blood loss was 50-1 200ml,mean(156±64)ml.The average time of hospitalization was(8.2±4.0)days.All patients survived during a follow-up of 3-60 months.Conclusions:Retroperitoneal laparoscopic nephrectomy,with minimal invasion,less blood loss and quicker postoperative recovery,is a practical surgical procedure.

Abstract:Objective To assess the surgical methods and clinical effect of retroperitoneal laparoscopic nephrectomy.Methods A total of 35 patients(20 males,15 females),age 25-78y,mean(45.6±9.5)y underwent retroperitoneal laparoscopic nephrectomy during Jan.2005 to Dec.2008.Of them 11 received radical nephrectomy,23 simple nephrectomy,and one radical nephrectomy with carcinoma of renal pelvis.Results Operations were carried on successfully.None of the 35 cases required open surgery.The operation time was 45-305min,[mean(72.3±24.0)min],and blood lose was 50-500mL [(mean(102±56)mL].The postoperative hospital stay was(7.2±3.1)d.During the following-up of 3-51 months,all these 35 patients survived.Conclusion Laparoscopic nephrectomy is safe and effective with less trauma,less bleeding and faster postoperative recovery.

Abstract:Objective To report initial experience and the indications of retroperitoneal laparoscop- ic partial nephrectomy.Methods From December 2001 to October 2005,23 patients (including 14 cases of renal cell carcinoma,5 of hamartoma and 4 of duplex kidney) underwent retroperitoneal laparoscopic par- tial nephrectomy.One of the patients had solitary kidney.Results All the operations were successful ex- cept for only 1 requiring conversion to laparoscopic nephrectomy because of bleeding resulting from blocking anterior branch of renal artery.The mean operative time was 121 min (60-240 min),and mean ischemic time was 32 min (20-55 min).The estimated bleeding volume ranged from 100 ml to 300 ml,and no pa- tient needed blood transfusion.Pathology showed negative surgical margins in 14 cases of renal cell carcinoma and confirmed the diagnosis of hamartoma in 5 cases.One patient with duplex kidney required open partial nephrectomy because of renal cystic mass resulting from incomplete resection.Conclusions Retroperito- neal laparoscopic partial nephrectomy offers a new effective and minimally invasive treatment for selected pa- tients with renal mass.The long-term effects of the procedure need further investigation.
[Retrospective study of laparoscopic peritoneal radical nephrectomy]

Abstract:Objective: To analyse the results of laparoscopic radical nephrectomy in terms of feasibility, safety, morbidity and tumour control. Material and methods: Between August 1993 and July 1998, 29 laparoscopic retroperitoneal radical nephrectomies were performed in 17 men and 12 women for renal tumours less than or equal to 90 mm in diameter. The working space was created by finger dissection without using a dilatation balloon. Four additional trocars were inserted under digital control. The renal pedicle was dissected first: the renal artery was clipped and the renal vein was sectioned by EndoGIA. After sectioning the ureter, the kidney was dissected with the perirenal fat and Gerota's fascia. The operative specimen was extracted in an impermeable endoscopy bag, without division, by enlarging the first trocar incision to 50 mm in order to avoid any parietal contact. Results: The mean age of the patients was 61.3 years (range: 40 to 78 years). Radical nephrectomy was performed on 13 right kidneys and 16 left kidneys, with mean dimensions of 112.9 mm (range: 85 to 150 mm). The mean tumour diameter was 40.2 mm (range: 20 to 90 mm). The mean operating time was 145 min (range: 80 to 330 min) with a mean blood loss of 100 cc (range: 0 to 1000 cc). A mean daily dose of 2.8 g (range: 1 to 5 g) of parenteral paracetamol and a mean daily dose of 1.8 mg (range: 0 to 10 mg) of morphine hydrochloride were necessary postoperatively. The mean hospital stay was 4.8 days (range: 1 to 11 days). One intraoperative haemorrhage required open conversion and one colon injury was treated by temporary colostomy. One local recurrence with hepatic metastasis occurred 9 months after the operation in a patient operated for a Furhman grade II stage pT3 tumour with negative resection margins. Conclusion: Laparoscopic radical nephrectomy is currently under evaluation. This technique is feasible, and respects the principles of cancer surgery: primary control of renal vessels, en bloc extraction of the kidney with Gerota's fascia.
Clinical Efficacy Evaluation of Retroperitoneal and Transperitoneal Laparoscopic Radical Nephrectomy

Abstract:【Objective】To evaluate the clinical efficacy of retroperitoneal and transperitoneal laparoscopic radical nephrectomy.【Methods】 Between August 2003 and April 2007, 55 patients with renal cell carcinoma underwent retroperitoneal (29 patients, group A) and transperitoneal (26 patients, group B) laparoscopic radical nephrectomy. The size of renal neoplasms was 2.0～8.5 (5.3±2.3) cm in group A and 1.8～8.7 (5.2±2.5) cm in group B respectively. The preoperative clinical stage was T1-2N0M0 in two groups. Tumor size, operating time, blood loss, time to control renal artery and vein, specimen weight, time to ambulation and the postoperative hospital stay were compared between group A and group B respectively. 【Results】The operations of group A were all successful, In group B, 1 case was failed and conversed to open surgery. Tumor size [(5.3±2.3) cm versus (5.2±2.5) cm], blood loss [(132±65) mL versus (136±59) mL], specimen weight [(598±345) g versus (585±337) g], time to ambulation [(28±10) h versus (29±11) h], the postoperative hospital stay [(5.5±1.6) d versus (5.4±1.7) d] were not significantly different (P 0.05) between two groups. While in group A, operating time [(159±55) min versus (215±70)min] and time to control renal artery and vein [(35±22) min versus (90±41) min] were significantly reduced than those of group B (P 0.01). 【Conclusions】 Laparoscopic radical nephrectomy can be performed efficiently and effectively with the retroperitoneal and transperi-toneal approach. While renal hilar control and total operative time may be quicker with retroperitoneal laparoscopic radical nephrectomy.

Abstract:Objective To introduce the technical improvements in retroperitoneal laparoscopic radical nephrectomy and discuss their roles in decreasing the perioperative complication. Methods Sixty-one patients with localized renal cell carcinoma were treated with retroperitoneal laparoscopic radical nephrectomy from December 2006 to March 2008. Pre-operative CT scan was performed to evaluate the renal vessel status. Thirty degree laparoscope was used together with self-made balloon dilator to establish the retroperitoneal space. Harmonic scalpel was used in dissection and removal of the extra-peritoneal fat. Cautions were taken during the separation of peritoneum. The renal artery and vein were separated and ligated with Hem-o-lok clips. Complications such as vessel split and intes-tinal injury were recorded. Operation time, estimated blood loss, drainage volume, and length of hos-pital stay were analyzed as well. Results The operation time was 50-135 min, estimated blood loss during operation was 20-170 ml, post-operative drainage volume was 20-210 ml, mean post-opera-tive length of hospital stay was 5 d. Two complications occurred during the operation: one was a vena eava injury, and another was a minor duodenum wall perforation. Conclusions Retroperitoneal lapa-roscopic radical nephrectomy is an effective and safe surgical option for localized renal cell carcinoma. Improvements in this surgical technique bring more safety and convenience and could decrease perio-perative complications relatively.
Application of Retroperitoneal Laparoscopic Technique in Nephrectomy

Abstract:Objective:To explore the clinical application value of retroperitoneal laparoscopic nephrectomy.Methods:17 cases of retroperitoneal laparoscopic nephrectomy from May 2006 to October 2009 were retrospectively discussed.Among them,11 cases were male and 6 cases were female and their average age was 62 years old.The resections were in the left for 10 cases and in the right for 7 cases,which were completed by retroperitoneal laparoscope.Results:Complete resections of the ill kidney were performed in 17 cases,among which a conversion to open surgery was needed in 2 cases.The surgeries lasted 110 min to 210 min and the average time was 100 min.The blood loss during operation was 50ml to 150ml and the average was 100ml without blood transfusion during all surgeries.Peritoneal damage was in 2 cases,but no pneumothorax.Hemorrhage of puncture hole after operation was in 1 case,which was healed by processing without wound infection.The median length of hospital stay was 7-8d.There was no dead case after operation.With all patients followed up for 5-24 months,no further complication was found.Conclusion:Retroperitoneal laparoscopic surgery for nephrectomy could be fast for blood vessel exposing with clear layer of anatomy.It has the advantages of less trauma,less hemorrhage and rapid recovery,so it is worth expanding clinically.

Abstract:Objective:The retroperitoneoscopic approach for renal cell carcinoma is quickly evolving.We report our experience with retroperitoneoscopic radical nephrectomy and compare it to a contemporary cohort of patients who underwent open radical nephrectomy with tumor mass larger than 4 cm.Methods:From January of 2002 to July of 2006,36 patients with such tumor masses underwent retroperitoneoscopic radical nephrectomies for renal cell carcinoma.Clinical data is reviewed and compared to a contemporary group of 46 patients who underwent open radical nephrectomy.Results:For patients with tumor mass 4-10 cm,the retroperitoneoscopic group had significantly reduced estimated blood loss(210±30 versus 410±100 ml,P<0.01),quicker recovery of intestinal function(3.9±1 versus 7.4±2 days,P<0.01)and shorter postoperative hospital stay(7±1.3 versus 8.5±1.8 days,P<0.01).The operating time in the retroperitoneoscopic group is similar with the open group(172±35 versus 165±40 mins,P>0.01).The average followup was 27 months(range 9 to 50)for all patients.By the time of the last followup,there have been no cases of tumor seeding in both trocar and open incision.Conclusions:Retroperitoneoscopic radical nephrectomy,although technically demanding,is a valuable alternative for managing localized renal tumors 4 to 10 cm in size.It affords patients an improved postoperative course,a quicker recovery and a similar efficacy to compare with open procedure within 3-year follow-up.

Abstract:Objective To evaluate the safety and efficacy of the anatomical retroperitoneoscopic nephrectomy(RSN)and standardize the procedure of RSN. Methods The retrospective analysis was performed on 405 consecutive patients underwent anatomical RSN in Our institute from January 2002 to June 2008.There were 232 male and 173 female patients with the average age of(57.2±14.2)years,among whom there were 228 renal cell carcinoma patients accepted RSU,96 and 49 renal pelvic carcinoma and ureteral carcinoma cases accepted retroperitoneoscopic ureteronephrectomy (RSUN) and 32 cases accepted simple RSN due to loss of renal function caused by benign renal discsses.The tadical RSN was performed by dissecting outside Gerota's fascia and in the latent cavities between this fascia and lateral conal fascia in the dorsal side and between this fascia and prerenal fusion fascia in the ventral side,whereas the simple RSN was done inside Gerota's fascia by making direct incision on it and dissecting between this fascia and perirenal adipose tissue.Kidneys and perirenal adipose tissue were completely removed by dissection along several avascular planes around the kidney under the amplified view of laparoscopy. The software SPSS 12.0 was used for the statistical analysis of all data. Results The mean operative time was (132±48)min for radical and simple RSN and (245 ± 62)min for radical RSUN, which included the time for position change and second skin preparation. The medium estimated blood loss was 100 ml(10-2500 ml) and the average drainage volume was 150 ml (0-1152 ml) postoperatively. 15 cases (3. 70%) required blood transfusion with the median volume of 400ml (400-1650 ml). Four cases (0. 99%) were converted to open surgery due to severe adhesion (2 cases), difficult exposure of renal helium (1 case) and severe bleeding (1 case).The mean drainage time was (3. 9±1.8)d, the mean time to first oral intake was (2.7±1.2)d and the mean postoperative hospital stay was (8.6±3. 8)d. Conclusion The anatomical RSN is safe and effective and should be the standard surgical procedure for laparoscopic nephrectomy.
