Abstract:Objective To evaluate the clinical effect of retroperitoneal laparoscopic nephron-sparing surgery.Methods From March 2007 to May 2008,15 patients with local renal tumors underwent retroperitoneal laparoscopic partial nephrectomy by a single surgeon.Hilar control was achieved with bulldog clamps.Hook-cautery is used to circumscribe the tumor for resection by scoring the renal capsule circumferentially with a 2-10 mm margin around the tumor.Specimens are randomly extracted through tumor edges and immediately given to pathological frozen sections.The operative time,blood loss,postoperative stay,complications and the time of complete convalescence were recorded.Results Mean operative time was 110 minutes and mean time of hilar control was 26 minutes.Mean blood loss was 200ml.Mean postoperative stay was 9 days.Final pathological evaluation revealed negative positive margins in these cases.During 2-14 months follow-up there was no local recurrence.Conclusions Retroperitoneal laparoscopic partial nephrectomy for small renal tumors can be performed safely and effectively.Extended follow-up period is required to determine its long-term functional and oncological outcomes.

Abstract:Objective To report initial experience and the indications of retroperitoneal laparoscop- ic partial nephrectomy.Methods From December 2001 to October 2005,23 patients (including 14 cases of renal cell carcinoma,5 of hamartoma and 4 of duplex kidney) underwent retroperitoneal laparoscopic par- tial nephrectomy.One of the patients had solitary kidney.Results All the operations were successful ex- cept for only 1 requiring conversion to laparoscopic nephrectomy because of bleeding resulting from blocking anterior branch of renal artery.The mean operative time was 121 min (60-240 min),and mean ischemic time was 32 min (20-55 min).The estimated bleeding volume ranged from 100 ml to 300 ml,and no pa- tient needed blood transfusion.Pathology showed negative surgical margins in 14 cases of renal cell carcinoma and confirmed the diagnosis of hamartoma in 5 cases.One patient with duplex kidney required open partial nephrectomy because of renal cystic mass resulting from incomplete resection.Conclusions Retroperito- neal laparoscopic partial nephrectomy offers a new effective and minimally invasive treatment for selected pa- tients with renal mass.The long-term effects of the procedure need further investigation.

Abstract:Objectives To evaluate the feasibility and the clinical value of retroperitoneal laparoscopic partial nephreetomy.Methods Thirty-nine patients with renal tumor were treated with retroperitoneal laparoscopic surgery.The tumor size was 2.5 ～4.6cm(mean was 3.8 cm)in diameter.During operation,renal circulation was temporarily interrupted in all patients.The operative time,intraoperative renal interrupted time,blood loss,postoperative complications and recurrence were observed and evaluated.Results Thirty-eight operations were successfully accomplished,the other 1 case was converted to open surgery.The intraoperative renal interrupted time was 21 ～36min(mean was 26min),The operative time was 70 ～ 145min(mean was 102min).And the intraoperative blood loss was60 ～900ml(mean was 105ml).Pathological examination showed the renal cell carcinomar 32 cases and the hamartoma 7 cases.The postoperative complication have one case leakage of urine,placed double J tube cured.Postoperative hospitalization time was 7 ～ 15d(mean was 10d).No tumor recurrence was found during a follow-up of 3 ～ 36 months.Conclusions Retroperitoneal laparoscopie partial nephreetomy is worthy to be extended with advantages of mini-invasion and rapid recovery.May replace open surgery,become frist choice therapeutic method for renal limitations tumor.

Abstract:Objective:To evaluate the technical feasibility and clinical efficacy of laparoscopic partial nephrectomy(LPN).Methods:Between March 2005 and June 2011,Retroperitoneal laparoscopic partial nephrectomy was performed on 205 patients with exophytic tumors(145 of renal carcinoma and 60 of renal harmatoma confirmed with pathology).Mean age was 52-year(range:32-75-year).In all cases hilar were controlled by clamping the renal artery alone with endoscopic bulldog clamps within 30 minutes.Renal masses were excised with cold endoscissors.Hemostasis and repair of the collecting system were achieved by suture with 2-0 VICRYL.Results;Median tumor size was 2.8 cm(range:1-5 cm).All of 205 operations were successfully performed,with a mean surgical time of 80 minutes(range 60 min to 150 min),and blood loss of 200 ml(range 100 to 800 ml),clamping time 20 min(range 9 to 30 min).Pathologic diagnosis:145 renal-cell carcinoma;60 hamartoma,all surgical margins were negative.1 case suffered from bleeding at the operation night,and 2 cases bled two days later.For the 3 cases,we achieved a satisfy result by intervention embolization.Another 3 cases suffered from pseudoaneurysm 2 weeks after operation.we applied the intervention embolization and patients were cured.There were no patients suffering urinary fistula.The median follow-up for 205 cases is 36 months with a range between 6 to 56 month and no recurrence or metastasis were noted during our observation.Conclusions:Retroperitoneal laparoscopic partial nephrectomy has been proved to have distinct advantages of minor trauma and quick recovery compared with open partial nephrectomy.But the indication of operation should be strictly mastered.

Abstract:Objective To investigate the experience and value of retroperitoneal laparoscopic nephron-sparing excision.Methods Between September 2005 and May 2009,35 cases underwent the retroperitoneal laparoscopic nephron-sparing surgery.Results All procedures were technically successful.The mean operating time was 85 min,and mean ischemic time was 25 min.The mean estimated blood loss was 240 mL.The mean hospital stay after surgery was 9d.No intraoperative complications occurred.No local recurrence or trocar site metastasis was observed during a mean follow-up period of 1-30 months.Conclusion Retroperitoneal laparoscopic nephron-sparing surgery represents a feasible option for the patients with localized small renal tumors.It offers a new effective and minimally invasive treatment for selected patients.

Abstract:Objective To explore the clinical value and efficacy of retroperitoneal laparoscopicpartial nephrectomy.Methods From July 2013 to November 2017,37 patients with renal tumors were received retroperitoneal laparoscopic partial nephrectomy in our hospital,the lesions demonstrated in renal clear cell carcinoma(23 lesions in T1 a stage,4 lesions in T1 b stage),hamartoma(9 lesions),chromophobe carcinoma(1 lesion in T1 a stage).Retroperitoneal space conventional was prepared,the Gerota fascia was opened,the renal artery trunk was found,arteries with bull-dog clamp were blocked,mass and the normal tissue which was 0.5 cm away from the tumor margin were resected and wound surface with Hem-o-lock fixed was sutured.Results Laparoscopic partial nephrectomy (LPN) was performed successfully in 36 cases,1 case transited to open surgery due to the notcompletely blocked renal artery,2 cases did not have renal artery blocked.Statistically,the average operation time was 95 min (65-225 min) and blood loss was 125 mL (20-1000 mL),clamping time 24 min (10-35 min)during the operation.One case had postoperation secondary bleeding and took selective renal artery embolization.The mean hospital stage was 14.6 days(11-27 days).These cases were followed up for 3 to 27 months,none of them had long-term complications and recurrency.Conclusions Retroperitoneal laparoscopic partial nephrectomy has several advantages of safety,efficiency,minimally invasion and rapid recovery,which should be promoted.

Abstract:Objective:To investigate the techniques and clinical value of laparoscopic partial nephrectomy by a retroperitoneal route.Methods:Data of 5 cases of renal tumor with laparoscopic surgeries, including 3 cases of renal cell carcinoma, 2 cases of renal hamartoma. The diameter of tumors ranged from 1.5 ～ 4.0 cm. All the surgeries were perfomed by the retroperitoneal route: ①Exposing the tumor and renal artery; ②Dragging the renal artery with a silica tube to block it temporarily if necessary; ③Resecting the tumor with 1 cm normal renal tissue by electro-hook; ④Suturing the resection surface with fibrin and absorbable hemostatic gauze.Results:All cases finished successfully. The operating time ranged from 150 to 210 mins. The blood loss during the operations was 80 to 350 ml.A full diet and bed activities were achieved by the first or second postoperative day, and hospital activities were achieved by the first to forth postoperative day. The hospital stay was 5 to 9 days, average 7 days.Conclusions:The laparoscopic partial nephrectomy by a retroperitoneal route is a minimally invasive, safe and effective therapy and it's considered the first choice for the cases with exophytic growth tumors under 4 cm.

Abstract:Objective:To explore the procedures and clinical outcomes of retroperitoneal partial nephrectomy for early malignant renal tumors.Methods: A total of 21 patients(19 men and 2 women,with a mean age of 49 years) with early malignant renal tumors were subjected to retroperitoneal partial nephrectomy using ultrasound knife and electron microscopy.The 21 patients included 17 with clear cell renal carcinoma,3 with granule cell renal carcinoma,and 1 with oxyphil cell renal carcinoma.The mean diameter of the tumors was(2.8±0.8) cm.Results: All the 21 cases underwent successful retroperitoneal partial nephrectomy without conversion to open operation.Mean surgical time was(105±15) min and mean blood loss was(120±22) ml. Four patients had a mean blood transfusion of 400 ml.One patient had urine leakage,with drainage volume of 200-300 ml;the drainage was stopped 15 days after operation when the drainage was less than 20 ml.The mean postoperative hospital stay was(9±2) days(7-17 days).No patients had local recurrence during a mean follow up of(20±4) months.Conclusion: Laparoscopic partial nephrectomy is feasible and safe.

Abstract:Objective To explore the surgical skills of retroperitoneoscopic partial nephrectomy.Methods From January 2012 to June 2015,60 patients with renal tumors received retroperitoneoscopic partial nephrectomy in our hospital.Agnail suture and high selective occlusion of renal artery branches were used during surgeries.Clinical outcome was evaluated during follow up.Results The operations were successfully completed in 60 patients.The average operation time,intraoperative blood loss and hospital stay were(67.6±15.5)min,(76.6±22.6)mL and(7.5±3.6)d,respectively.The renal artery was occluded in 54 patients and the average occlusion time was(27.5±8.5)min.The follow up ranged from 1month to 42 months.No recurrence was found.Postoperative pathological diagnosis included 30 cases of renal clear cell carcinoma,4cases of papillary renal cell carcinoma,1cases of teratoma,3cases of oncocytic adenoma,16 cases of renal angiomyolipoma,3cases of multilocular cystic renal cell carcinoma and 3cases of spindle cell carcinoma.Conclusion Retroperitoneoscopic partial nephrectomy is an effective and safe procedure.Computed tomographic angiography(CTA),familiarity of anatomic structure,agnail suture and selective occlusion of renal artery branches can improve clinical efficacy and safety of retroperitoneoscopic partial nephrectomy.

Abstract:[Objective] To evaluate the safety and efficacy of retroperitoneoscopic radical nephrectomy. [Methods] 5 patients were positioned in a lateral decubitus position with a four-trocar technique. After reflection of colon and identification of the renal hilum, the renal artery and vein were dissected proximally. The ureter was transected as distal as possible. Following the Gerota's fascia, the kidney was fully mobilized. Then a complete regional lymphadenectomy was performed. Subsequently the kidney together with the tumor was removed through an abdominal incision. [Results] The mean (range) duration of surgery was 110(75~250) minutes; the blood loss was 50~300 mL, No major complications were noted in these patients. [Conclusions] Retroperitoneoscopic laparoscopic radical nephrectomy is safe and effective. Removal of the kidney outside Gerota′s fascia is still the most important aspect of laparoscopic radical nephrectomy, and it can ultimately prevent seeding.

Abstract:Partial nephrectomy has become the standard treatment for stage T1a renal cell carcinoma.Indications of laparoscopic partial nephrectomy are identical to those of open surgery.In China,retroperitoneal approach is widely used in laparoscopic partial nephrectomy.On the basis of traditional laparoscopic partial nephrectomy,we summarized the operation proceduresin six steps with our own experience.In thisarticle,we will briefly review the clinical application of laparoscopic partial nephrectomy and introduce the detailed procedures and key points of six-step retroperitoneal laparoscopic partial nephrectomy.

Abstract:目的探索后腹腔镜下肾部分切除术的应用范围和经验. 方法 2001年12月至2005年10月,对23例患者施行后腹腔镜下肾部分切除术,其中肾细胞癌14例、错构瘤5例、重复肾4例,孤立肾1例. 结果 22例手术顺利完成,1例肾肿瘤因仅阻断肾动脉前支时出血而行腹腔镜肾切除,手术时间60～240 min,平均121 min.肾动脉阻断时间20～55 min,平均32 min.术中出血量100～300 ml,均未输血.病理报告肾细胞癌14例,切缘均阴性;错构瘤5例.1例重复肾因切除不彻底,术后发现肾上极囊性肿块而再次开放手术行肾部分切除. 结论后腹腔镜下肾部分切除术对选择性的肾脏病变是一种有效和微创的治疗方法,远期效果有待进一步观察。
Application of Retroperitoneal Laparoscopic Technique in Nephrectomy

Abstract:Objective:To explore the clinical application value of retroperitoneal laparoscopic nephrectomy.Methods:17 cases of retroperitoneal laparoscopic nephrectomy from May 2006 to October 2009 were retrospectively discussed.Among them,11 cases were male and 6 cases were female and their average age was 62 years old.The resections were in the left for 10 cases and in the right for 7 cases,which were completed by retroperitoneal laparoscope.Results:Complete resections of the ill kidney were performed in 17 cases,among which a conversion to open surgery was needed in 2 cases.The surgeries lasted 110 min to 210 min and the average time was 100 min.The blood loss during operation was 50ml to 150ml and the average was 100ml without blood transfusion during all surgeries.Peritoneal damage was in 2 cases,but no pneumothorax.Hemorrhage of puncture hole after operation was in 1 case,which was healed by processing without wound infection.The median length of hospital stay was 7-8d.There was no dead case after operation.With all patients followed up for 5-24 months,no further complication was found.Conclusion:Retroperitoneal laparoscopic surgery for nephrectomy could be fast for blood vessel exposing with clear layer of anatomy.It has the advantages of less trauma,less hemorrhage and rapid recovery,so it is worth expanding clinically.
The Application of “renal Pedicle Rotation” Method in Retroperitoneal Laparoscopic Partial Nephrectomy for Renal Ventral Tumors

Abstract:Objectives: To present preliminary experience of renal pedicle rotation method in retroperitoneal laparoscopic partial nephrectomy (RLPN) for renal ventral tumors. Methods: A retrospective analysis of renal surgery database about patients undergoing RLPN was conducted. We identified 14 consecutive cases of all the surgeries from December 2013 to October 2014 with renal ventral tumors. The 14 surgeries were all in the application of renal pedicle rotation method, an intraoperative skill in which surgeons rotates the kidney on renal pedicle axis after separating renal artery and vein for a better tumor exposure. Results: A total of 14 patients underwent RLPN. All surgeries were successful without conversion to radical nephrectomy or open partial nephrectomy. The mean operative time was 171.147.2min (range 83-246), the mean warm ischemia time was 15.9 +/- 9.8min (range 7-35) and the mean estimated blood loss was 89.3 +/- 102.2mL (range 10-300). Mean postoperative hospital stay was 6.2 +/- 1.9 days (range 4-10) and mean retroperitoneal drainage was 4.2 +/- 1.4 days (range 2-8). No intraoperative complications occurred. Postoperative complications developed in four patients. All patients had negative margins and pathological examination revealed 10 (71.4%) clear cell carcinomas, two (14.2%) epithelial renal angiomyolipoma, one (7.1%) papillary carcinoma and one (7.1%) nephrotuberculosis. The review of the early oncologic outcomes revealed no local recurrence and distant metastasis. Conclusion: The renal pedicle rotation method might be a feasible and safe procedure to get a better tumor exposure for incision and suture in renal ventral tumors RLPN surgeries with acceptable postoperative outcomes.
Laparoscopic partial nephrectomy: comparison of transperitoneal and retroperitoneal approaches

Abstract:Purpose: Laparoscopic partial nephrectomy has become an effective alternative for small renal tumors. Previous reports include transperitoneal and retroperitoneal approaches but to our knowledge the indications for when to apply the techniques have not been defined. We report our experience with comparing the 2 techniques. Materials and methods: A retrospective review of 51 laparoscopic partial nephrectomies was performed. Patients were analyzed based on the surgical approach, operative parameters and postoperative recovery. The 2 approaches used similar operative techniques to control parenchymal bleeding and collecting system entry. Results: There were 32 retroperitoneal and 19 transperitoneal partial nephrectomies. Mean operative time (3.5 vs 5.4 hours, p = 0.000001) and blood loss (192 vs 403 cc, p = 0.002) was significantly less for the retroperitoneal approach. Renal vessel clamping was performed in 81% of retroperitoneal and 63% of transperitoneal operations. Warm ischemia time was not significantly different between the groups. Patients undergoing the retroperitoneal approach had a statistically significant decrease in time to tolerating a regular diet (1.2 vs 1.7 days, p = 0.02), catheter removal (1.4 vs 2.5 days, p = 0.004) and discharge home (2.3 vs 3.6 days, p = 0.0008). Conclusions: Based on tumor location as the selection criteria the retroperitoneal approach was associated with shorter operative time, less blood loss, more rapid return of bowel function and shorter hospitalization compared with those in patients selected for the transperitoneal technique. Based on our experience we believe that the decision on the approach should be based on the tumor location on the kidney surface. For polar or posterolateral masses the retroperitoneal approach is preferred. The transperitoneal approach is best suited to anterior and medial lesions.

Abstract:Objective To discuss the safety and efficacy of retroperitoneal laparoscopic partial nephrectomy in the treatment of unilateral multifocal renal tumors.Methods The data of 12 patients with unilateral multifocal renal tumors managed with retroperitoneal laparoscopic partial nephrectomy between 2009 and 2012 were reviewed retrospectively.The 12 patients had a mean age of 42 years (28 to 62) and body mass index of 23.5 kg/m2 (18.6 to 29.3),including 4 males and 8 females.Four cases were in the left side,7 cases were in the right side,and 1 case had bilateral renal tumors.The diameter of renal tumors ranged from 0.5 cm to 4.7 cm,and all tumors were projected on the surface of kidney.Nine cases were diagnosed as renal cell carcinoma,and 3 cases were diagnosed as angiomyolipoma before operation.All patients underwent retroperitoneal laparoscopic partial nephrectomy under general anesthesia,while 1 patient with bilateral renal tumors experienced radical nephrectomy of the contralateral renal tumor 3 months after the first operation.Results All the oprations were successful without conversion to open surgery.The mean renal artery cut off time,operative time,estimated blood loss and hospital stay were 36 min (26-60),126 min (75-184),142 ml (50-300) and 5 d (3-9),respectively.No complication occurred and 1-week postoperative serum creatinine was (78.1 ±8.1)μmol/L.Histopathological study revealed 9 cases of renal cell carcinomaand 3 cases of angiomyolipoma.During the follow-up for 33 months (13-51),1 patient with yon Hippel-Lindau syndrome was operated with ipsilateral radical nephrectomy for a local recurrence while the remains had no local recurrence or metastasis.Conclusions For localized and exogenous unilateral multifocal renal tumors,retroperitoneal laparoscopic partial nephrectomy is feasible and safe.Those who have hereditary and/or malignant renal tumors should be monitored closely.
Trans-retro-peritoneal Laparoscopic Partial Nephrectomy for Posterior Hilar Tumor: Technical Feasibility and Preliminary Results.

Abstract:Background:Urologists still encounter challenges when it comes to the surgical management of tumors located on the posterior lip and posterior renal hilar region. We propose a trans-retro-peritoneal (TRP) technique to address the difficulties associated with posterior hilar tumors during retroperitoneal laparoscopic partial nephrectomy (LPN). Its efficacy was evaluated in a retrospective case-control study.Methods:The patients with posterior hilar tumors (≤7 cm) that underwent retroperitoneal LPN were included. The TRP technique allowed the posterior hilar tumor completely visible by incising the ventral peritoneum and rotating kidney ventrally during retroperitoneal LPN, which was applied in 36 cases, while the conventional retroperitoneal LPN was performed in 22 cases. Perioperative data were analyzed to evaluate the efficacy of TRP-LPN.Results:In TRP-LPN group, the TRP technique was successfully performed in all the patients without converting to open surgery or radical nephrectomy. The warm ischemia time was significantly shorter in TRP-LPN group than conventional LPN group (20.3 vs. 28.5 min, P<0.001). Furthermore, the mean estimated blood loss in TRR-LPN group was significantly less than that in conventional LPN group (86.5 vs. 90.9 mL, P<0.05). The mean operation time and recovery time of gastrointestinal function were similar between two groups. No severe complications occurred, and no positive surgical margin was found. The rate of Trifecta achievement was 50.0% (18/36) and 31.8% (7/22) respectively for TRP-LPN and conventional LPN (P=0.175). After mean follow-up of 21 months, no recurrence or metastasis occurred in all cases.Conclusions:Our findings, as demonstrated by the Trifecta outcomes, support the feasibility and efficacy of TRP-LPN in managing posterior renal hilar tumors. This approach may be considered as an efficient option for surgical management of such tumors.
