Abstract:OBJECTIVE:This study aims to evaluate the shaping capability of Reciproc, WaveOne, Mtwo, and ProTaper instruments in simulated root canals. METHODS:A total of 40 simulated resin blocks were divided randomly into four groups. Each group was prepared with Reciproc (Group 1), WaveOne (Group 2), Mtwo (Group 3), and ProTaper (Group 4). The preparation time and reduction in working length after preparation were measured. Pre- and post-operative images were obtained with a scanner and superimposed through Photoshop. The changes in canal curvature and material removal from the inner and outer canal walls at 10 points beginning at 1 mm from the end point of the canal were measured with Image J. Centering capability was determined accordingly. Data were analyzed through one-way ANOVA, SNK, and Kruskal-Wallis at a significance level of P < 0.05. RESULTS:The preparation time of Group 2 was (53.7 ± 6.7) s, whereas those of Groups 1, 3, and 4 were (86.9 ± 8.1) s, (112.2 ± 8.2) s, and (177.9 ± 11.2) s, respectively; the difference was found to be significant (P < 0.05). The reductions in working length among the four groups after preparation were not significantly different (P > 0.05). The canal curvature for Groups 1 to 4 were 2.671° ± 0.637°, 2.667° ± 0.450°, 3.664° ± 0.870°, and 3.797° ± 0.601°, respectively. The changes for Groups 1 and 2 were significantly smaller than those for Groups 3 and 4. At the 3 mm point, the transportation of Group 1 was (-0.016 ± 0.094) mm, which was significantly less than that of the other instruments (P<0.05). At the 4 mm and 5 mm points, the trans- portation values of Group 2 were (-0.080 ± 0.104) mm and (-0.312 ± 0.088) mm, which were significantly less than that of Group 1 [(-0.243 ± 0.099) mm, (-0.404 ± 0.064) mm, P < 0.05]. CONCLUSION:Reciproc and WaveOne can complete preparation faster and can maintain the original canal curvature better than Mtwo and ProTaper. Reciproc exhibits superior centering capability in the apical part of the canal, whereas WaveOne exhibits superior centering capability in the middle part of the canal.
COMPARATIVE ANALYSIS OF APICAL EXTRUSION OF DEBRIS AND SHAPING ABILITY IN CURVED ROOT CANALS USING FOUR DIFFERENT NITI INSTRUMENTS

Abstract:Introduction: The aim of this study was to evaluate the apical extrusion of debris, apical transportation (AT) and centering ratio (CR) after root canal preparation using Reciproc, ProTaper, ProTaper Next (PTN) and Revo-S. Materials and methods: Sixty extracted human mandibular premolars with a curvature angle of 25 degrees and the same length were randomly divided into four groups (n=15). All root canals were shaped to size of 25: ProTaper Next X2 (Dentsply Maillefer), Reciproc (R25; VDW, Munich, Germany), Revo-S SU (25#, .06; MicroMega, Besancon, France) and ProTaper F2 (25#, .08; Dentsply Maillefer, Ballaigues, Switzerland). All roots were sectioned perpendicularly at 1, 3, 5 and 7 mm from the apex and were scanned by a micro-CT before and after the preparation. During this process, collect the debris of each tooth and weigh it three times consecutively. The amount of apical debris extrusion, AT and CR of teeth were recorded and compared between the groups by using the Kruskal-Wallis test. Results: In terms of extruded debris, all specimens showed apical debris extrusion, and no statistically significant difference was obtained among them (P>.05). In terms of transportation, there was no significant difference among the four groups (P >0.05); in terms of centering ratio, PTN and Revo-S had significantly higher (closer to C=1) centreing ratios than Reciproc and ProTaper. There was a significant difference between PTN and ProTaper (P<0.05), PTN and Reciproc (P<0.05), ProTaper and Revo-S (P<0.05) at 3 mm and 5 mm from the apical surface, but there was no significant difference in the mean centering ratio at 1 mm or 7 mm from the apical surface among the four groups. Conclusion: ProTaper Next and Revo-S prepared a more centered root canal shapes. All instruments caused extrusion of debris through the apex.
Shaping ability of R-phase nickel-titanium rotary instruments in simulated S-shaped canals

Abstract:Objective To evaluate the shaping ability of TF,Endo Wave and K3 instruments in simulated Sshaped root canals. Methods A total of 120 simulated S-shaped resin blocks were divided randomly into three groups( n= 40),each group was prepared with each instrument,respectively. Preparation time was recorded. Pre-and postoperative images were obtained by a digital microscope and superimposed using Photoshop. The change of curvatures,and material removal from the inner and outer canal walls at 10 points beginning from 0 mm at the end point of the canal were measured using Image J. Centering ability was determined,accordingly. The data were analyzed using the One-Way ANOVA at a significance level of P 0. 05. Results Preparation time for TF was 14. 00 ± 4. 56 s,significantly faster than that for K3 and Endo Wave,while the changes of curvatures were not significantly different among the three instruments. At the apical curve,the removal of resin from the inner wall by TF was significantly less than Endo Wave and K3( P 0. 05). At the coronal curve,the material removal from the outer wall of TF were significantly less than K3( P 0. 05). At the straight part,TF removed significantly less material from the inner wall than Endo Wave and K3( P 0. 05). At points from 5 mm to 9 mm,the transportation of TF were significantly less than Endo Wave( P 0. 05),while that of TF was not significantly different with that of K3( P 0. 05),except for the 6 mm point( P 0. 05). Conclusion TF prepares S-shaped resin canals efficiently,with less aberrations and transportations than Endo Wave and K3.

Abstract:OBJECTIVE To compare the shaping ability of ProGlider, HyFlex EDM Glidepath File and RaCe ISO 10 with that of PathFile and stainless steel K-file hand instruments in simulated root canals. METHODS A total of 60 simulated resin blocks were divided randomly into five groups (n=12), each group prepared with each instrument, respectively. Preparation time was recorded. Pre- and post-operative images were obtained using a scanner and superimposed using Photoshop. The material removal from the inner and outer canal walls beginning from 0 mm at the end point of the canal were measured using ImageJ. Centering ability was determined, accordingly. The data were analyzed using the One-Way ANOVA at a significance level of P<0.05. RESULTS Preparation time for ProGlider was (61.018±5.020) s, significantly the fastest (P < 0.05), that for SS K-file (111.100±6.987) s, the slowest (P<0.05), and the differences among the other 3 groups were not significant (P>0.05). The removal of resin from the outer wall at the apical portion was significantly lower for HyFlex EDM Glidepath File than for ProGlider (P<0.05), both of them were significantly lower than the other 3 groups (P<0.05). For the removal from the inner wall at the curvature portion, ProGlider was the least whilst SS K-file the most (P<0.05). At the straight portion, RaCe ISO 10 removed the most not only from the inner but also the outer wall (P<0.05), HyFlex EDM Glidepath File the second most, and the other 3 groups among the same levels. As far as the centering ability was concerned, at the apical portion, HyFlex EDM Glidepath File was the best, ProGlider the second best, no significant difference identified among the other 3 groups. At the curvature portion, ProGlider and HyFlex EDM Glidepath File were the best, PathFile the second, RaCe ISO 10 the third, the worst was the SS K-file. At the straight portion, the difference amongt the 5 groups was not significant. CONCLUSION The shaping abilities of ProGlider, HyFlex EDM Glidepath File and RaCe ISO 10 were better or comparable to that of PathFile, while the preparation time for ProGlider reduced significantly, and HyFlex EDM Glidepath File and RaCe ISO 10 removed significantly more resin at the straight portion of the canal.

Abstract:OBJECTIVE:To evaluate shaping ability of nickel-titanium rotary instruments (ProTaper) in curved root canals.METHODS:The computer assistant measure system of simulated root canal was used for quantitative analysis of root shape of eight simulated root canals during ProTaper shaping process. Blue ink was injected into the simulated root canals before instrumentation, when F2 reached work length(WL) (F2) and when F3 reached WL(F3). Orifices and apexes were sealed with adhesive tapes. Root canal images were gained by Mustek BP4800TA scanner. The images before and after preparation were turned into Di-numerical images and fused. The changes of central line positions and curvatures were measured in quantity through this system. Data of seven preestablished point from orifice to apex were recorded and analysed by SPSS 10.0 soft ware.RESULTS:ProTaper system reduced the angles of simulated canals. Selecting F2 or F3 as the master apical file (MAF) had no influence on the changing rate of the root angles. ProTaper had excess cutting on the outer wall of orifice and the inner wall of curve part, but the displacement of apex was minimal (only 0.021 mm).CONCLUSION:ProTaper has preferable shaping ability. To prepare very difficult curved canals, the master apical file (MAF) could be F3. Attention should be taken when preparing some curved canals with hollows in their inner walls, in order to avoid the lateral perforation.
Root canal preparation efficiency of protaper vs. combined technique in simulated resin root canal

Abstract:Objective To study root canal preparation efficiency of Combined Technique vs.ProTaper hand NiTi file.Methods Sixty simulated resin curved root canals were randomly divided into two groups,combined canal preparation technique group(group C)and Protaper hand NiTi file group(group P).Group C were first prepared by stainless steel hand K file,and then followed by ProTaper F2 file.Incidence of instrument fracture was investigated.Preparation time and the costs of two groups were recorded and compared.All these parameters were used to evaluate efficiency of preparation.Results All stimulated root canals were prepared continuous taper root canal in the form of big crownal access and intact apical foramen.Group C had no separation of instrument,while the rate of instrument fracture in group P was 10 percent.The average preparation time of group C was 6.37 min,and that of group P was 11.32 min(P<0.05).The costs of instruments used in group P were about 3 times higher than those in group C.Conclusion Combined technique(group C)showed great shaping ability.It can prepare root canals effectively,safely and with little costs,so it is worth wide application.

Abstract:The purpose of this study was to evaluate four instruments with different working motion for preparation of a C-shaped single canal wall using the same artificial plastic models reproduced from a human tooth. One tooth with root canal morphology C1 (the shape is an uninterrupted “C” with no separation or division) was selected among three-dimensional micro-computed tomography (micro-CT) imaging data of extracted human teeth. Imaging data were then converted into STL form data, and twenty-four C-shaped root canal model blocks were manufactured using this STL form data. These blocks were randomly divided into four groups of six blocks each and instrumented as follows: stainless steel K-files (SSK), Self-Adjusting File (SAF), ProTaper NEXT (PTN) and RECIPROC (REC). Micro-CT images taken before and after canal preparation were superimposed, and instrumented canal area, percentage of instrumented canal area, part of instrumented canal area, volume of instrumented canal and time taken for instrumentation were evaluated for each group. The greatest instrumented canal area, percentage of instrumented canal area and volume of instrumented canal were as follows (in decreasing order): SSK > SAF > PTN > REC (P < 0.05). The longest time taken for instrumentation was as follows (in decreasing order): SAF > SSK > PTN > REC (P < 0.05). The conscious shaping of SSK and the lattice structure of SAF were instrumented all root canal walls equally. PTN and REC required less time taken for instrumentation, but showed unequal instrumentation.

Comparison of Dentinal Defects Formation in Straight, Moderate and Severely Curved Canals by Three Distinctive Nickel Titanium Instruments: An in vitro Study

Abstract:Statement of the problem: It is stated that engine-driven instruments might cause dentinal defects during root canal preparation. These defects might spread and progress into greater fractures or vertical root fracture. Purpose: This study aimed to compare the incidence of dentinal defects that might arise all through preparation of root canals, using One Shape, RaCe, and WaveOne systems in canals with a curvature (0-20°) and (20-40°). Materials and method: In this in vitro study, 150 mandibular first molars were enrolled. Based on the degree of curvature in the mesial roots, the samples were divided into two groups (n=75) of straight and moderately curved canals (0-20º), and severely curved canals (20-40°). Then each group was randomly divided into four sub-groups. In subgroups 1 to 3 from each group, canals were prepared using WaveOne, One Shape, and RaCe. Then all roots were sectioned at 3, 6 and 9-mm distances from the apex. The slices were evaluated using stereomicroscope at 12× magnification. The data were analyzed using the Chi-square and Fisher's exact tests and the level of significance was set at 0.05. Results: Fracture and other defects were not found in the control groups. In canals with curvature (0-20°), WaveOne caused the maximum dentinal defects and RaCe produced the least. Moreover, in canals with curvature (20-40°), One Shape caused the maximum dentinal defects while WaveOne and RaCe caused equal dentinal defects approximately. Conclusion: There was a statistically significant relationship between the performance of RaCe and One Shape in canals with curvature (0-20º) and (20-40º), (pValue< 0.05) while no statistically significant difference was observed in connection to the performance of WaveOne (p> 0.05).

Abstract:Background and Objectives: The aim of the present study is to compare the efficacy of three root canal preparation systems in the shaping of 3D-printed root canal replicas of single rooted teeth. Materials and Methods: Sixty 3D-printed root canal replicas were produced and divided into three groups, each consisting of twenty samples. Each group was shaped with a different instrument: Reciproc Blue R25/08 (VDW GmbH, Munich, Gemany), WaveOne Gold Primary 25/07 (Dentsply Sirona, Ballaigues, Switzerland), and ProTaper Gold F2 25/08 (Denstply Sirona). To ensure the reproducibility of pre- and post-operative CBCT images of the root canals, the endodontic printed replicas were placed in a mould of silicon impression material. A cone-beam computed tomography (CBCT) software was used to compare pre- and post-instrumentation images collected at three levels of the root canal length: 3, 6, and 9 mm from the apical foramen. The amount of transportation, centring ability, and curvature angle after shaping were evaluated for each system. The results were statistically analysed and compared using one-way analysis of variance (ANOVA). Results: Regarding the transportation of the root canal after shaping, significant differences between groups at 3 mm (p = 0.010721) and 6 mm (p = 0.000046) were recorded in the mesio-distal direction, while in the bucco-lingual significant differences were only observed at 6 mm (p = 0.000554). Reciproc Blue removed more dentin from the mesial and buccal wall of the root canal. When evaluating the centring ability of the three systems, significant differences were observed between the groups at the level of 9 mm (p = 0.037258) in the mesio-distal direction, and at the level of 6 mm (p = 0.038197) in the bucco-lingual direction. Significant differences of the canal curvature angle after shaping were also observed between groups (p = 0.000001). Reciproc Blue straightened the curvature the most, while ProTaper Gold the least. Conclusions: All systems produced minor root canal transportation. No instrument was able to achieve a perfect centring preparation of the root canal. All systems produced a small degree of root canal straightening.
Comparison of Stress Distributions in Dental Roots after Root Canal Preparation with 3 Different Instruments

Abstract:AIM: To investigate the effects of root canal preparation with 3 different instruments on stress distributions in dental roots. METHODS: Three-dimensional finite element models of human first mandibular molar with well prepared root canals by K-type file, ProTaper or Profile were simulated on the basis of the diameters and tapers of the instruments used. A vertical load was applied on the surface of the occlusal restoration, then stress distributions in dental roots were calculated. RESULTS: Stress distributions in dental roots with root canal preparation by K-type file, ProTaper or Profile were slightly different. ProTaper group showed the highest stress level, while K-type file exhibited the lowest. CONCLUSION: Though the stress level in dental roots of ProTaper group is higher than that of Profile or 35#K file, the difference is small. This suggests that root canal preparation with the above tested 3 root canal instruments does not have obvious effects on the whole stress level and strength of the dental roots.

Abstract:INTRODUCTION We present our experience with limb salvage emphasising the use of vascularised bone grafts in combination with autoclaved tumour bone for limb salvage in osteosarcoma. MATERIALS AND METHODS Thirty-six cases with extremity osteosarcoma, all of which were managed at a single institution between 1980 and 1999, were reviewed at a mean follow-up of 9 years (range, 10 months to 200 months). There were 26 patients who underwent limb salvage surgery and 10 patients who had amputation. RESULTS In the limb salvage group, 12 bone defects were bridged using vascularised bone grafts alone while autoclaved tumour bone was used for reconstruction in the remaining 14 patients. In these 14, there were 10 with autoclaved tumour bone and vascularised bone, 2 with autoclaved bone and non-vascularised bone and 2 with autoclaved bone alone. The joint was preserved in 13 of the 26 reconstructions. Of these, 11 involved preservation of the knee joint contributing to a quantum gain in function with 84% good and excellent results. The actuarial survival was 64% at 2 years and 48% at 5 years, with no significant difference between ablation and limb salvage. CONCLUSION The use of the autoclaved bone provided yet another effective alternative for bridging large defects following resection of high grade tumours.
A comparative study of the effectiveness of instrumentation techniques in preparation of the fine curved carals of posterior teeth

Abstract:Objective The aim of this study was to compare the effectiveness of three instrumentation techniques in preparation of the fine curved canals of posterior teeth.Methods Forty-five mesiobuccal root canals in fleshly extracted human molar teeth were instrumented by using the step-back technique,the step-down technique or the passive step-back technique.Time of preparation of each canal was taken down.After cracking the roots longitudinally,the canal walls were observed under a scanning electron microscope,and the coronal,middle and apical thirds of each canal were photographed at the 100 and 1000 times magnification.Each canal was qualitatively evaluated and the groups were compared for the removal of debris and smear layer,both overall and at each level(apical,middle,and coronal).Results There were no statistically significant differences between the techniques either overall or within any of the regions(P > 0.05).When comoparing regions(regardless of technique),the apical portion of the canal was less clean than the middle or coronal portions(P < 0.05).Preparation time in these three techniques showed no statistical significance(P > 0.05).Conclusions Efficacy differed little among the techniques.None of the three techniques removed smear layer completely and all left debris,especially within the apical third of the canals.The passive step-back technique was superior to the step-back and the step-down techniques when it was related to the cleaning efficiency,but it took a longer time for preparation and was so exhausting.The step-back technique can be used more easily within a short time.

Abstract:In summary, we have optimized the operation current to obtain stable endurance and retention properties in Ti/HfOx based bipolar RRAM for future low power nonvolatile memory applications. Furthermore, the 1st RESET resistance pinning effect after the FORMING process at lower current is greatly improved by selecting the Ti/HfOx thickness ratio in TiN/Ti/HfOx/TiN 1T1R RRAM. Thus, in order to achieve the reliable switching at low operation current, the design of the memory element is crucial.

Abstract:Aim: The aim of this study was to evaluate the root canal preparation abilities of two nickel–titanium (NiTi) instrument systems, namely WaveOne (WO) and ProTaper Next (PTN), using cone-beam computed tomography (CBCT), on human extracted teeth. Materials and Methods: Eighty mesiobuccal root canals of the maxillary and mandibular first molars with a curvature of 10°–39° were chosen. CBCT images were captured before instrumentation using special silicone molds to ensure that the teeth are positioned at the same place before and after instrumentation. The root canals were divided into two groups, namely Group 1 and Group 2 (40 root canals in each group with similar root canal curvatures). Group 1 and Group 2 were prepared using WO and PTN, respectively. Teeth were inserted again into the previous silicone molds at the right positions. Postinstrumentation CBCT images were captured with the same parameters. Pre- and postinstrumentation CBCT images were analyzed using Invivo 5 software provided by the CBCT machine manufacturer. Results: There was no difference between the two groups of root canals instrumented by two NiTi instrument systems in root canal transportation, centering ratio, straightening, and volume changes ( P > 0.05). Conclusions: Both NiTi instrument systems can be used effectively and safely in root canal instrumentation.

In clinical practice, commonly selected instruments include multifile NiTi systems, such as M3-Pro PLUS (M3-PRO), Orodeka Plex 2.0 (ODP), Rotate (ROT), and Protaper Gold (PTG), as well as single-file NiTi systems, such as M3-L Platinum 2019 (M3L), Waveone Gold (WOG), and Reciproc Blue (RCB). This study aimed to comprehensively evaluate the differences in the apical extrusion of debris and centering ability of the above NiTi files.

Abstract:Objective: To compare the reduction of residual dentine thickness of two different post preparation methods on the mandibular second molars with a C-shaped root canal configuration. Methods: A total of 26 extracted right mandibular second molars with a C-shaped root canal configuration were selected and paired based on similar canal morphology. Each of the paired teeth was randomly allocated to the heat and ultrasonic instruments group (HU group) or Peeso Reamer (Mani, Utsonomiya, Japan) group (PR group) (n = 13) and received post preparation with different instruments after the same endodontic treatment. The reduction of residual dentine thickness and the minimal remaining dentine thickness at the apical sections at 4 or 7 mm below the cementoenamel junction (CEJ) were recorded. The data were analysed using an independent samples t test (alpha = 0.05). Results: The reduction of residual dentine thickness for the HU group was less than that for the PR group in the two sections. Moreover, at the section 7 mm below the CEJ, the teeth reduction of the distolingual wall in the HU group (0.022 +/- 0.007 mm) was significantly lower than that in the PR group (0.101 +/- 0.013 mm) (P < 0.01). Conclusion: Using heat and ultrasonic instruments to perform post preparation could follow the original canal configuration to save more tooth structure in the remaining root canal wall, minimise the reduction of residual dentine thickness and decrease the incidence of root canal perforation.
