Peri-implant Marginal Bone Changes with Implant-Supported Metal-Ceramic or Monolithic Zirconia Single Crowns: A Retrospective Clinical Study of 1 to 5 Years.

Abstract:Statement of problem. Monolithic zirconia has excellent mechanical and biologic properties. However, evidence of the clinical properties of implant-supported monolithic zirconia prostheses is limited.Purpose. The purpose of this retrospective clinical study was to compare the peri-implant marginal bone changes of metal-ceramic and monolithic zirconia single crowns in the posterior region after prosthetic loading.Material and methods. A total of 224 participants treated with 327 implants restored with either metal-ceramic or monolithic zirconia single crowns in the posterior region between 2012 and 2016 were included in this study. Clinical outcomes, including the plaque index, peri-implant probing depth, and bleeding on probing, were recorded, and the marginal bone level was recorded by using the panoramic radiographs obtained at implant placement, second-stage surgery, and the most recent follow-up visit. The included parameters were analyzed with the nonparametric Mann-Whitney tests (a=.05).Results. The mean follow-up time was 30.4 months, and the cumulative survival rate of implants was 100% and that of the prostheses was 99.1%. The plaque index was 0.46 in the metal -ceramic group, which was significantly higher (P<.05) than 0.37 in the monolithic zirconia group. However, no significant differences (P>.05) were observed in peri-implant probing depth and bleeding on probing between the 2 groups. The marginal bone level at implant placement, second-stage surgery, and the most recent follow-up visit was above the implant platform in both the metal-ceramic and monolithic zirconia groups. The marginal bone changes of the metal-ceramic group was 0.31 mm in the healing period and 0.38 mm in the prosthetic loading period, while in the monolithic zirconia group, it was 0.25 mm in the healing period and 0.43 mm in the prosthetic loading period; no significant differences (P>.05) were observed between the 2 groups.Conclusions. The peri-implant marginal bone level change was comparable after prosthetic loading for metal-ceramic and monolithic zirconia single crowns, although monolithic zirconia was associated with reduced plaque. Papilla‐Crown Height Dimensions around Zirconium Dioxide Implants in the Esthetic Area: A 3‐Year Follow‐Up Study

Abstract:PURPOSE Soft tissue interactions with ceramic dental implants have previously been shown to have favorable esthetic outcomes. This study aimed to evaluate the papilla-crown proportion around zirconia implants in a 3-year follow-up study and the correlation between the gingival biotype and changes in papillary height. MATERIALS AND METHODS This was a prospective study of 39 patients with 40 single-gap implants (Straumann PURE Ceramic ZLA Implant). The papilla-crown proportion was assessed after 3 months, 1 year, and 3 years. In addition, correlations between the peri-implant biotypes and changes in papillary heights were evaluated. RESULTS The papilla-crown proportion improved from 35.5% after 3 months to 41.7% after 3 years. The gingival biotype was correlated very weakly to papilla height alterations. Significant papillary fill was observed in the interdental space between 3 months and 3 years (p < 0.001). CONCLUSIONS An ideal papilla-crown proportion of 40% around single implants was observed after 3 years. A thin or thick gingival biotype showed a very weak correlation with soft tissue alterations.

Peri-implant Crestal Bone Changes Around Zirconia Implants in Periodontally Healthy and Compromised Patients.

Abstract:PURPOSE No consensus regarding the efficacy of zirconia implants in maintaining peri-implant hard and soft tissue health has yet been obtained. The aim of this retrospective follow-up study was to gain knowledge about peri-implant bone behavior and about implant survival and success after treatment with zirconia dental implants in patients with normal and compromised soft and hard tissue conditions. MATERIALS AND METHODS This follow-up study involved 86 patients with 123 zirconia implants (Straumann PURE Ceramic Implant) that were radiographically investigated directly after implant placement (day 0), 3 months after placement, and 1 year after the definitive implant crown placement. The clinical assessment was done at the 1-year postloading appointment and also included the modified Plaque Index, modified Sulcus Bleeding Index, and sulcus pocket depths. Eighteen patients with periodontally compromised conditions were compared to 68 patients with healthy periodontal conditions. RESULTS The survival rate was 100% and success rate was 94.5%, with no differences between the two groups. The alveolar crest around the ceramic implants showed no significant difference between day 0 and 1 year postloading for both groups (P > .05). There was also no significant difference at 1 year postloading between the groups in the distance from the implant shoulder to the peri-implant bone crest (P = .67) or in pocket depth (P = .07). CONCLUSION No significant peri-implant bone loss was observed in the first year. The survival and success rates showed no differences between the periodontally healthy and periodontally compromised groups; however, only a limited number of patients with periodontally compromised conditions were included in this study.

Abstract:ObjectiveThe primary aim of this study was to determine the effect of implant-supported porcelain-fused-to-metal (PFM) and indirect-composite-resin (ICR) fixed dental prostheses on peri-implant marginal bone resorption (MBR) in custom-made anatomic modified zirconia dental implants.MethodsA prospective randomized controlled clinical trial was conducted. Participants with premolars indicated for dental extractions were recruited into this study to receive a single-unit implant-supported fixed dental prosthesis. Modified anatomic zirconia implants with thorny-retentive surfaces were placed and loaded randomly after 3 months with either PFM or ICR crowns. Participants were recalled after 12 and 18 months for radiographic evaluation of peri-implant MBR. Implants survival was also reported.Results18 out of 20 zirconia implants were included in all study phases. 18-month survival rate was 90%. After 12 months of implant placement, the mean MBR values were 0.53 (±0.21) mm and 0.60 (±0.14) mm in the ICR group compared to 0.67 (±0.16) mm and 0.61 (±0.27) mm in the PFM group. In the 18-month follow-up, the mean MBR values were 0.61 (±0.27) and 0.67 (±0.16) mm in the ICR group compared to 0.77 (±0.29) and 0.77 (±0.27) mm in the PFM group. No significant differences were found in MBR mean values between study groups at 12- and 18-month follow-up points.ConclusionThis study showed that PFM and ICR crowns were viable zirconia-implant-supported restorations with no preference regarding MBR after 18 months. Nevertheless, long-term evaluations are warranted.
Peri-implant and esthetic outcomes of cemented and screw-retained crowns using zirconia abutments in single implant-supported restorations-A systematic review and meta-analysis

Abstract:Objectives: To evaluate the peri-implant tissue changes and esthetic outcomes of cemented and screw-retained crowns of single-tooth implants in the esthetic zone using zirconia abutments. Material and methods: An electronic search was performed on nine databases. The risk-of-bias was assessed by the revised Cochrane risk-of-bias tool for randomized (RoB 2) and non-randomized (ROBINS-I) clinical trials. Marginal bone level change, soft tissue thickness, bleeding on probing, probing depth, survival rates of implants and crowns, complications, plaque and papilla indexes, and pink esthetic score data were extracted and analyzed. The certainty of evidence was accessed through the GRADE approach. Results: Nine records were included and 7 were used in the meta-analyses. Screw-retained crowns presented greater marginal bone level change (MD -0.04 [-0.08, -0.00] p = 0.04, I2 = 0%) compared to cemented crowns up to 1-year. At 3 and 4 years no significant differences (p > 0.05) were observed. Soft tissue thickness did not differ between groups (p > 0.05). The bleeding on probing was higher in cemented group than in screw-retained crowns at 1-year (MD 0.17 [0.08, 0.27] p = 0.0005, I2 = 0%), at medium-term periods (3 and 4 years) no statistically significant differences (p > 0.05) were observed for this outcome. Probing depth, survival rates of implants and crowns, complications, and plaque index, as well as esthetic analysis using the papilla index and pink esthetic score did not differ statistically (p > 0.05) between both retention systems at short and medium-term periods. Conclusion: The connection system considering zirconia abutments presented no influence on peri-implant parameters and esthetics evaluation for medium-term periods (3 and 4 years).
Long-Term Peri-Implant Health and Papilla Formation at Healed Sites by Chairside Provisionalization of Single-Tooth Implants: A Prospective Case Series

Abstract:PURPOSE: Several procedures are performed to achieve optimal esthetic results in single-tooth implants. However, there is discordance regarding the potential benefit and risks of immediate implant loading/provisionalization. The aim of this prospective case series is to investigate the effect of immediate provisionalization of single-tooth implants at healed sites for periimplant soft-tissue conditions, focusing on papilla formation around single implants.MATERIALS AND METHODS: Twelve patients received a total of 12 implants in the incisor, canine or premolar region of the upper and lower jaw at healed sites with immediate chair-side provisionalization. Four months later, the temporary crown was replaced by the permanent crown. After 40±13.1 months, clinical follow-up was conducted, assessing Probing pocket depth (PPD); Bleeding on Probing(BoP); Mucosal recession (MR) and Width of Keratinized Mucosa (KM). Papilla index (PI) was determined immediately after implant placement (baseline), before removing the temporary crown (t1), 4 weeks after insertion of the definitive crown (t2) and at the final follow-up examination (t3) to evaluate papilla formation and its change over time.RESULTS: None of the implants were lost. The mean PPD was 2.5±0.39 mm, BoP of 25% and 3.5 mm of KM were observed at the final follow-up. No implants showed MR. PI increased in all patients from 1.5±0.45 at baseline to 2.4±0.56 at t1, 2.6±0.47 at t2 and 3.02.6±0 at t3. The increase in PI between t0 and each individual timepoint from t1-t3 showed statistical significance.CONCLUSION: The present results indicate the suitability and benefit of immediate provisionalization to achieve favorable peri-implant soft-tissue conditions and papilla formation.

Abstract:Objective: To observe the changes of peri-implant tissue around the individualized abutment that was grinded from zirconia provisional crown in one year. Methods: In this research, a prosthodontic-driven virtual implant planning and immediate provisionalization were conducted in computer assisted design software. And computer-aided design/computer-aided manufacturing (CAD/CAM) techniques were used to fabricate the zirconia provisional crown and surgical guide template before surgery. The implant was accurately placed with the surgical guide, and the zirconia provisional crown was immediately delivered after surgery. Three months later, the implant osseointegration was completed, and zirconia provisional crown was prepared intraorally to generate customized zirconia abutment for final prosthesis. The study included 30 patients with single anterior tooth loss, including 18 males and 12 females, aged from 26 to 50 years old, and the mean age was (36.2±6.1) years old. The patients were from the Center of Oral Implantology, The First Affiliated Hospital of Zhejiang University Medical College from January 2017 to February 2018. After cementation of the final prosthesis, the cases were followed up at 6 and 12 months time intervals. Implant survival rate, probing depth, bleeding on probing, marginal bone level loss and papilla index score (PIS) were recorded in every appointment. Results: The survival rate of 30 implants was 100%, and the probing depths were less than 5 mm. The bone resorption at 6 and 12 months follow-up after the final delivery was 0 (0, 0) mm and 0 (-0.2, 0) mm, respectively, and the difference was not statistically significant (P>0.05). The PIS was 3.0 (2.0, 4.0), 3.0 (2.8, 4.0) and 3.0 (3.0, 4.0) on the final delivery, 6 and 12 months after final delivery, respectively. Conclusions: Marginal bone level and bone loss were stable with this new implant clinical protocol at the one-year follow-up.

Abstract:Aim: To explore the features of and potential landmarks that predict crestal ridge remodeling after immediate implant placement and provisionalization (IIPP) with thin buccal plate phenotype in the anterior maxilla. Materials and methods: Patients requiring IIPP of single maxillary incisor with thin buccal bone plate (<1 mm) were recruited. Cone-beam computed tomography (CBCT), performed before and 6 months after the intervention, were three-dimensionally superimposed. A virtual line connecting the bony emergence points of adjacent teeth was drawn as a reference. The facial-coronal alveolar crest of the extraction socket and healed ridge were located in mesial, middle and distal cross-sectional planes, and linear ridge reductions were measured. The correlation between ridge reduction and local factors was analyzed. Results: Fifty patients were included. From the extraction socket to healed ridge, the alveolar crest underwent linear bone reduction of 1.50 +/- 0.50, 1.54 +/- 0.66 and 1.65 +/- 0.73 mm in the mesial, middle, and distal regions in an apical-palatal direction. The middle region had significantly higher horizontal (facial-palatal) but lower vertical (coronal-apical) bone reduction than the mesial and distal areas (P < 0.01). The facial-coronal ridge crest of the healed ridge located close to the reference line. Ridge reduction positively correlated with the distance between the initial facial-coronal crest of the extraction socket and the reference line (P < 0.01). Conclusions: Alveolar crest of the socket lost its curvature and tended to attain a flat profile after IIPP due to inconsistent ridge reduction in middle, mesial and distal areas. The reference line may be a potential landmark for predicting the ridge crest after remodeling.
The effect of BioHPP versus Zirconia CAD/CAM-fabricated fixed-detachable prosthesis rehabilitating single maxillary arches on the peri-implant bone level changes

Abstract:Purpose: The objective of this study was to compare the use of BioHPP and zirconia CAD/CAM fabricated materials inimplant supported maxillary fixed-detachable prosthesis opposing dentate mandibular arch, where peri-implants bone levelchanges were evaluated for two years.Materials and method: Ten patients with edentulous single maxilla opposing dentate mandibular arch were selected toshare in this study. For all patients, proper planning for implant placement was made where the participants were randomlyallocated into two groups five patients in each group. Six implants were inserted guided by the sterolithographic surgical guidefollowing flapless surgical approach. Immediate loading protocol was followed, where the milled acrylic temporary prosthesis,which was planned for each patient according to the computer software, was screwed to the implants. After 4 months, thefinal restoration was placed according to the following grouping. In group I, patients were rehabilitated with fixed-detachablezirconium restoration while in group II patients were rehabilitated with fixed-detachable BioHpp Peek restoration. Peri-implantmarginal bone changes were evaluated at loading time, after 6 month, 1 and 2 years follow up visits. Data collected weretabulated and statistically analyzed.Results: In intragroup comparisons, peri implant bone loss was significant. Where in group I, the mean value of peri–implantbone height change measures from implants insertion to 6 month was 0.35 mm, from insertion to 12 months was 0.62 mm whilefrom insertion to 24 months follow up was found to be 0.96 mm. While in group II at same time intervals were 0.39 mm, 0.69mm and 1.23 mm respectively. On the other hand in intergroup comparison, to compare the mean amount of peri-implant boneheight changes in the two studied groups during the follow up intervals, student t test was performed and showed that group IIhad a non significant higher bone loss.Conclusion: Based on these results and within the limitation of this study. Both Zirconia and BioHpp when used as frameworkmaterial for fixed-detachable maxillary single prosthesis had successful results on the supporting implants. However, BioHppfixed-detachable prosthesis had higher effect on the peri-implant marginal bone loss but it was not of statistical significance.

Abstract:The objective of this study was to clinically and radiologically evaluate the performance of a short (8 mm), 1-piece, zirconia implant after an observation period of 1 year in function. A total of 47 patients with 1 missing tooth in the position of a premolar or molar were recruited. Short (8 mm), 1-piece, zirconia implants were placed and loaded after a healing period of 2 to 4 months with monolithic crowns made of 3 different materials. Implants were followed up for one year and clinically and radiologically assessed. A total of 46 implants were placed. One was excluded since no primary stability was achieved at implant placement. At the 1-year follow-up, mean marginal bone loss 1 year after loading was 0.05 ± 0.47 mm. None of the implants showed marginal bone loss greater than 1 mm or clinical signs of peri-implantitis. A total of 2 implants were lost during the healing phase and another after loading, resulting in a survival rate of 93% after 1 year. All lost implants showed a sudden increased mobility with no previous signs of marginal bone loss or peri-implant infection. The short, 8 mm, zirconia implants showed stable marginal bone levels over the short observation period of 1 year. Although they revealed slightly lower survival rates, they can be suggested for the use in sites with reduced vertical bone. Scientific data are very limited, and long-term data are not yet available, and therefore, they are needed.

Papilla Alteration in Different Gingival Biotypes at Single Implant-Supported Restoration in Anterior Maxilla

Abstract:PURPOSE:To observe the alteration of the interproximal papilla height after single implant restoration in the anterior maxilla and to assess the influence of gingival biotypes on papilla regeneration.METHODS:Thirty patients with single-tooth implant-supported restorations in the anterior maxilla were assigned to two groups according to labial mucosa thickness. Group A: labial mucosal thickness≥1.5 mm and group B: labial mucosal thickness<1.5 mm. The presence of interproximal papilla as well as the distance between papilla apex and contact point were assessed at crown attachment (baseline) and after 6 months (follow-up).Statistical analysis was performed using SPSS19.0 software package.RESULTS:The 6-month survival rate of implants was 100%. The mean height of interproximal papilla at baseline and follow-up was (1.64±0.97) mm and (3.10±1.39) mm, respectively. A significant improvement in papilla height was found between baseline and follow-up(P<0.05). In group A, the mean distance from papilla apex to the contact point at mesial and distal aspects was (0.15±0.24) mm and (0.23±0.33) mm, respectively, and the presence of interproximal papilla were (97±5)% and (94±8)%, respectively. In group B, the mean distance from papilla apex to the contact point at mesial and distal aspects was (1.94±0.40) mm and (2.10±0.34) mm, respectively, and the presence of interproximal papilla was (53±9)% and (43±14)% respectively. Statistically significant difference was found between the two groups(P<0.05).CONCLUSIONS:The interproximal papilla height around single implant restorations could improve significantly after 6-month follow-up. The interproximal papilla height around single-tooth implants in the anterior maxilla was influenced by gingival biotype. The regenerative potential of thick-gingival biotype was better than that of thin-gingival biotype.

Abstract:Design This study is a monocentre cohort study with a primary aim to assess the five-year survival of alumina-toughened zirconia (ATZ) implants (Metoxit AG, Thayngen, Switzerland). Secondary aims were to evaluate peri-implant marginal bone loss (MBL), soft tissue health and patient-reported outcome measures (PROMs). The implants were placed into healed ridges and, following a period of temporisation, were restored with either IPS e.max CAD LT or IPS e.max ZirCAD veneered with IPS e.max ZirPress (Ivoclar Vivadent, Schaan, Liechtenstein). Patients were followed up at prosthetic delivery, one-year and five-years post-implant placement. The clinical parameters recorded were clinical attachment levels (CAL), probing depth (PD), gingival recession (GR), modified bleeding index (mBI) and modified plaque index (mPI). MBL was evaluated using standardised radiographs. PROMs were used to assess patients' satisfaction utilising a visual analogue scale (VAS) to rate function, aesthetics, sense, speech and self-esteem. The null hypothesis assumed there was no difference in implant survival between one and five years post-operatively. This study was approved by the ethics committee at the University of Freigburg, Germany (No. 354/07).

However, there is still a lack in reliable long-term clinical data.PURPOSE: Aim of the present prospective clinical study was to analyze the dental implants made from zirconia, regarding their clinical performance compared to natural teeth (control).MATERIALS AND METHODS: Eighty-three zirconia implants in 28 adults were analyzed after 7.8 years of integration. Approximal plaque index (API), sulcus bleeding index (SBI), probing pocket depth (PPD), probing attachment level (PAL), and creeping or recession (CR/REC) of the mucosa/gingiva were analyzed. Changes in the marginal bone level (MBL) have been determined by comparing radiographs recorded immediately after implant placement and at the follow-up investigation. Furthermore, the pink esthetic score (PES), Periotest values (PTV), microbial colonization of the implant/dental sulcus fluid, and patient satisfaction were assessed.RESULTS: The survival rate of zirconia implants was 100%, however one implant presented a profound peri-implantitis, resistant to therapies. No statistical significance was observed between implants and teeth regarding SBI and PAL. Statistical significance was detected regarding API and CR/REC with significantly less plaque accumulation and recession in the study group. Significantly higher values for PPD were obvious in the zirconia implant group. PES of 9 ± 2.7 indicate a rather average esthetic result. Analysis of MBL revealed a mean peri-implant bone resorption of 1.2 mm. Microbial analysis revealed a nonstatistically significant higher colonization of bacteria in the study group, whereas single bacteria species were detected in statistically significant higher amounts.CONCLUSION: The investigated one-piece zirconia dental implants presented favorable long-term clinical results, comparable to natural teeth (SBI and PAL), and, regarding adhesion of plaque and creeping attachment (CR/REC), even superior. The microbial investigation revealed a shift in peri-implant colonization around zirconia implants over years, mainly without clinical effect on peri-implantitis.
