Abstract:Aim This study measured the two-dimensional size of soft and hard tissues and analysed some relevance between them. Material and methods Ninety-six maxillary anterior implants with a follow-up time 1-7 years postoperatively were evaluated. We superimposed the CBCT data of 72 patients with the optical scan data, followed by the measurements of the thickness and the height of soft and bone tissues. The shoulder of the implant was the reference point for all vertical measurements. Results At the level of implant shoulder, the mid-buccal mean thickness of soft tissue was 1.83 +/- 0.76 mm, and on the palatal side, it was 4.00 +/- 1.22 mm. The mean height of mid-buccal and mid-palatal gingiva was 4.16 +/- 1.07 mm and 4.27 +/- 1.07 mm. The buccal and palatal vertical bone wall peak was 0.41 +/- 0.73 mm and 0.22 +/- 0.57 mm coronal to the implant shoulder, and the marginal bone loss was -0.28 +/- 0.76 mm and -0.84 +/- 1.41 mm. Correlation analysis showed that the soft tissue thickness was negatively correlated with the bone thickness at 0 mm on the buccal side and at 0/2/4 mm on the palatal side apical towards from implant shoulder. The gingival height was significantly positively correlated with bone peak height and marginal bone height. Conclusion Soft and hard tissues were basically negatively correlated in the horizontal direction. Sites with little marginal bone loss or high bone peak positively correlated with higher soft tissue levels.

Abstract:Statement of problem. Monolithic zirconia has excellent mechanical and biologic properties. However, evidence of the clinical properties of implant-supported monolithic zirconia prostheses is limited.Purpose. The purpose of this retrospective clinical study was to compare the peri-implant marginal bone changes of metal-ceramic and monolithic zirconia single crowns in the posterior region after prosthetic loading.Material and methods. A total of 224 participants treated with 327 implants restored with either metal-ceramic or monolithic zirconia single crowns in the posterior region between 2012 and 2016 were included in this study. Clinical outcomes, including the plaque index, peri-implant probing depth, and bleeding on probing, were recorded, and the marginal bone level was recorded by using the panoramic radiographs obtained at implant placement, second-stage surgery, and the most recent follow-up visit. The included parameters were analyzed with the nonparametric Mann-Whitney tests (a=.05).Results. The mean follow-up time was 30.4 months, and the cumulative survival rate of implants was 100% and that of the prostheses was 99.1%. The plaque index was 0.46 in the metal -ceramic group, which was significantly higher (P<.05) than 0.37 in the monolithic zirconia group. However, no significant differences (P>.05) were observed in peri-implant probing depth and bleeding on probing between the 2 groups. The marginal bone level at implant placement, second-stage surgery, and the most recent follow-up visit was above the implant platform in both the metal-ceramic and monolithic zirconia groups. The marginal bone changes of the metal-ceramic group was 0.31 mm in the healing period and 0.38 mm in the prosthetic loading period, while in the monolithic zirconia group, it was 0.25 mm in the healing period and 0.43 mm in the prosthetic loading period; no significant differences (P>.05) were observed between the 2 groups.Conclusions. The peri-implant marginal bone level change was comparable after prosthetic loading for metal-ceramic and monolithic zirconia single crowns, although monolithic zirconia was associated with reduced plaque. Papilla‐Crown Height Dimensions around Zirconium Dioxide Implants in the Esthetic Area: A 3‐Year Follow‐Up Study

Abstract:PURPOSE Soft tissue interactions with ceramic dental implants have previously been shown to have favorable esthetic outcomes. This study aimed to evaluate the papilla-crown proportion around zirconia implants in a 3-year follow-up study and the correlation between the gingival biotype and changes in papillary height. MATERIALS AND METHODS This was a prospective study of 39 patients with 40 single-gap implants (Straumann PURE Ceramic ZLA Implant). The papilla-crown proportion was assessed after 3 months, 1 year, and 3 years. In addition, correlations between the peri-implant biotypes and changes in papillary heights were evaluated. RESULTS The papilla-crown proportion improved from 35.5% after 3 months to 41.7% after 3 years. The gingival biotype was correlated very weakly to papilla height alterations. Significant papillary fill was observed in the interdental space between 3 months and 3 years (p < 0.001). CONCLUSIONS An ideal papilla-crown proportion of 40% around single implants was observed after 3 years. A thin or thick gingival biotype showed a very weak correlation with soft tissue alterations.

Abstract:Purpose: To evaluate the short-term effect of dental implant placement, mucosa thickness, and their combined effects (initial biologic width) on marginal bone loss. Materials and Methods: This was a retrospective study on patients who received implant surgery in the posterior region without bone augmentation surgery between 2012 and 2016, and implants had been loaded for more than 12 months. Each patient received radiographic examination before and after implant surgery, before the stage-two surgery, and during the 1-to 5-year follow-up. The thickness of mucosa, depth of dental implant placement, and crestal bone loss were evaluated on digital radiographs. The interaction was discussed by defining the combination of initial mucosal thickness and implantation depth as the initial biologic width. The implants were divided into four study groups based on the quartile of the initial biologic width. Results: This study included 266 patients (94 male and 172 female, 22 to 85 years of age, mean age: 51.43 years), with 413 dental implants placed including 239 Straumann implants and 174 Ankylos implants. The average follow-up was 21.50 months. After 1 to 5 years, the median crestal bone loss around implants was 0.35 mm (0.30 mm for Straumann BL and 0.40 mm for Ankylos). The implants were divided into four groups: group A (<= 2.85 mm), group B (2.85 to 3.40 mm), group C (3.40 to 3.97 mm), and group D ( 3.97 mm). Group B showed significantly less crestal bone loss than group A (0.38 mm vs 0.25 mm; P < .05) and group C (0.25 mm vs 0.40 mm; P < .05) during the follow-up. Significantly more crestal bone loss around implants was observed in the thin mucosa group than in the thick mucosa group (0.50 mm vs 0.30 mm; P < .001), while implants placed beneath the bone level displayed a significantly higher amount of marginal bone loss than implants placed even with the bone crest (0.50 mm vs 0.10 mm; P < .001). Conclusion: The initial biologic width has an effect on crestal bone loss. When the initial biologic width was between 2.85 and 3.40 mm, the marginal bone loss was lowest. Based on radiographic evaluation, implants placed in thick gingiva and even with the bone level showed less alveolar marginal bone loss compared with implants placed in thin gingiva and below the crestal bone level. Abstract:PURPOSE No consensus regarding the efficacy of zirconia implants in maintaining peri-implant hard and soft tissue health has yet been obtained. The aim of this retrospective follow-up study was to gain knowledge about peri-implant bone behavior and about implant survival and success after treatment with zirconia dental implants in patients with normal and compromised soft and hard tissue conditions. MATERIALS AND METHODS This follow-up study involved 86 patients with 123 zirconia implants (Straumann PURE Ceramic Implant) that were radiographically investigated directly after implant placement (day 0), 3 months after placement, and 1 year after the definitive implant crown placement. The clinical assessment was done at the 1-year postloading appointment and also included the modified Plaque Index, modified Sulcus Bleeding Index, and sulcus pocket depths. Eighteen patients with periodontally compromised conditions were compared to 68 patients with healthy periodontal conditions. RESULTS The survival rate was 100% and success rate was 94.5%, with no differences between the two groups. The alveolar crest around the ceramic implants showed no significant difference between day 0 and 1 year postloading for both groups (P > .05). There was also no significant difference at 1 year postloading between the groups in the distance from the implant shoulder to the peri-implant bone crest (P = .67) or in pocket depth (P = .07). CONCLUSION No significant peri-implant bone loss was observed in the first year. The survival and success rates showed no differences between the periodontally healthy and periodontally compromised groups; however, only a limited number of patients with periodontally compromised conditions were included in this study.

Abstract:Introduction There is evidence that the apico‐coronal implant position and the mucosal phenotype can affect the extent of peri‐implant bone loss. This clinical trial analyzes the bone remodeling and marginal bone loss that occur around conical‐connection implants placed equicrestally and subcrestally, assessing the effect of the peri‐implant soft‐tissue phenotype. Methods Fifty‐one patients received 56 implants of distinct diameters (3.5 mm Ø n = 6; 4.3 mm Ø n = 41; 5 mm Ø n = 9) in the posterior part of the maxilla or mandible. The implants were placed equicrestally, 1 mm subcrestally and >1 mm subcrestally, depending on the initial supracrestal tissue height (STH). After 3 months of non‐submerged healing, single metal‐ceramic screw‐retained implant‐supported crowns were placed. Longitudinal measurements of STH, mucosal thickness and keratinized mucosa width (KMW) were made at the time of implant placement (T0), crown placement (T1), and after 3 (T2) and 6 months (T3) of prosthetic loading. At each of these points, a radiographic evaluation of bone remodeling and marginal bone loss was also performed. Results STH was significantly greater for implants placed >1 mm subcrestally than for those placed 1 mm subcrestally. After 12 months of follow‐up, a very significant (p 1 mm subcrestally, respectively. After the multiple linear regression, marginal bone loss was found to depend primarily on KMW (β = −0.43), while also being affected by STH (β = 0.32) and implant diameter (β = −0.28). Conclusions Marginal bone loss may be influenced by the position with respect to the bone crest, as well as the KMW, STH, and implant diameter. However, more well‐controlled studies are needed to verify these above‐mentioned findings with different implant designs and connections.

Papilla Alteration in Different Gingival Biotypes at Single Implant-Supported Restoration in Anterior Maxilla

Abstract:PURPOSE:To observe the alteration of the interproximal papilla height after single implant restoration in the anterior maxilla and to assess the influence of gingival biotypes on papilla regeneration.METHODS:Thirty patients with single-tooth implant-supported restorations in the anterior maxilla were assigned to two groups according to labial mucosa thickness. Group A: labial mucosal thickness≥1.5 mm and group B: labial mucosal thickness<1.5 mm. The presence of interproximal papilla as well as the distance between papilla apex and contact point were assessed at crown attachment (baseline) and after 6 months (follow-up).Statistical analysis was performed using SPSS19.0 software package.RESULTS:The 6-month survival rate of implants was 100%. The mean height of interproximal papilla at baseline and follow-up was (1.64±0.97) mm and (3.10±1.39) mm, respectively. A significant improvement in papilla height was found between baseline and follow-up(P<0.05). In group A, the mean distance from papilla apex to the contact point at mesial and distal aspects was (0.15±0.24) mm and (0.23±0.33) mm, respectively, and the presence of interproximal papilla were (97±5)% and (94±8)%, respectively. In group B, the mean distance from papilla apex to the contact point at mesial and distal aspects was (1.94±0.40) mm and (2.10±0.34) mm, respectively, and the presence of interproximal papilla was (53±9)% and (43±14)% respectively. Statistically significant difference was found between the two groups(P<0.05).CONCLUSIONS:The interproximal papilla height around single implant restorations could improve significantly after 6-month follow-up. The interproximal papilla height around single-tooth implants in the anterior maxilla was influenced by gingival biotype. The regenerative potential of thick-gingival biotype was better than that of thin-gingival biotype.

Abstract:OBJECTIVE To analyze and find factors affecting the height of adjacent gingival papilla of maxillary central incisor single implant in human in order to promote the esthetic result of dental implants. METHODS Sixteen maxillary central incisor single implants of 16 patients and 30 sites of adjacent tissues and prosthesis were evaluated. Data on the height of adjacent gingival papilla of dental implant and relative factors were obtained from clinical parameters, photographs, research models, and X radiographs. RESULTS The mean and standard deviation of adjacent gingival papilla height of maxillary central incisor single implant were (4.01 +/- 1.85) mm. Statistical analysis revealed that factors affecting the height of adjacent gingival papilla of implant were as following from strong to weak: vertical distance between contact point and gingival margin, vertical distance between proximal bone level of natural tooth and implant abutment, vertical distance between proximal bone level of natural tooth and contact point, proximal protruding degree of crown, horizontal distance between abutment and root, length ratio of the crown and fixture, vertical distance between proximal bone level of implant and abutment, lip-side protruding degree of crown, probing depth. CONCLUSIONS Multiple factors affected the height of adjacent gingival papilla of maxillary central incisor single implant in human.
[Effect of Customized Zirconia Abutment on Peri-Implant Tissue: a One-Year Prospective Study].

Abstract:Objective: To observe the changes of peri-implant tissue around the individualized abutment that was grinded from zirconia provisional crown in one year. Methods: In this research, a prosthodontic-driven virtual implant planning and immediate provisionalization were conducted in computer assisted design software. And computer-aided design/computer-aided manufacturing (CAD/CAM) techniques were used to fabricate the zirconia provisional crown and surgical guide template before surgery. The implant was accurately placed with the surgical guide, and the zirconia provisional crown was immediately delivered after surgery. Three months later, the implant osseointegration was completed, and zirconia provisional crown was prepared intraorally to generate customized zirconia abutment for final prosthesis. The study included 30 patients with single anterior tooth loss, including 18 males and 12 females, aged from 26 to 50 years old, and the mean age was (36.2±6.1) years old. The patients were from the Center of Oral Implantology, The First Affiliated Hospital of Zhejiang University Medical College from January 2017 to February 2018. After cementation of the final prosthesis, the cases were followed up at 6 and 12 months time intervals. Implant survival rate, probing depth, bleeding on probing, marginal bone level loss and papilla index score (PIS) were recorded in every appointment. Results: The survival rate of 30 implants was 100%, and the probing depths were less than 5 mm. The bone resorption at 6 and 12 months follow-up after the final delivery was 0 (0, 0) mm and 0 (-0.2, 0) mm, respectively, and the difference was not statistically significant (P>0.05). The PIS was 3.0 (2.0, 4.0), 3.0 (2.8, 4.0) and 3.0 (3.0, 4.0) on the final delivery, 6 and 12 months after final delivery, respectively. Conclusions: Marginal bone level and bone loss were stable with this new implant clinical protocol at the one-year follow-up.

Interimplant papilla reconstruction: assessment of soft tissue changes and results of 12 consecutive cases

Abstract:Background: Loss of soft tissue between adjacent implants is a frequent occurrence. Reconstruction of the papilla between adjacent implants is often necessary where there are esthetic demands. The purpose of this article is to describe a fixed and reproducible reference line which can be used to measure changes in the height of the interimplant papillae. A surgical technique carried out at the time of the abutment connection surgery is also described to show the changes that may occur over a 12-month period. Assessment of changes in the soft tissue dimension is shown following a modified flap technique at the time of abutment connection. Methods: With the aid of clinical photographs taken preoperatively, a reference line can be drawn between the highest point of the gingival margins on teeth distal to the intended site of soft tissue surgery. This reference line can be reproduced on clinical photographs postoperatively. Using this reference line, the changes in soft tissue height using a modified flap technique in 12 consecutive patients are presented. Assessment of the vertical height of the interimplant papillae before and after treatment is described. Results: Four patients showed an increase in the height of the papillae when reviewed 12 months following abutment connection. One patient showed no change in the height of the papillae, and 7 patients showed a loss in papillary height. There was an average loss of 0.5 mm in height of the interimplant soft tissue. Conclusions: Use of a fixed reference line to assess the outcome of treatment is necessary when evaluating vertical changes in soft tissue. This reference line is used to assess a soft tissue flap technique carried out at the time of implant exposure.

Abstract:Highlights • An overview of developments, challenges and strategies related to soft tissue integration around zirconia abutments. Stable soft tissue integration around the implant abutment attenuates pathogen penetration, protects underlying bone tissue, prevents peri-implantitis and is essential in maintaining long-term implant stability. The desire for "metal free" and "aesthetic restoration" has favored zirconia over titanium abutments, especially for implant restorations in the anterior region and for patients with thin gingival biotype. Soft tissue attachment to the zirconia abutment surface remains a challenge. A comprehensive review of advances in zirconia surface treatment (micro-design) and structural design (macro-design) affecting soft tissue attachment is presented and strategies and research directions are discussed. Soft tissue models for abutment research are described. Guidelines for development of zirconia abutment surfaces that promote soft tissue integration and evidence-based references to inform clinical choice of abutment structure and postoperative maintenance are presented. JA Model allows to describe both the hysteresis and the dynamical features of charging and discharging cycles in a lithium battery. The identification of the model is obtained by using a neural network technique developed for magnetic systems. The model is validated on some experimental tests on commercial batteries.

Abstract:PURPOSE: Several procedures are performed to achieve optimal esthetic results in single-tooth implants. However, there is discordance regarding the potential benefit and risks of immediate implant loading/provisionalization. The aim of this prospective case series is to investigate the effect of immediate provisionalization of single-tooth implants at healed sites for periimplant soft-tissue conditions, focusing on papilla formation around single implants.MATERIALS AND METHODS: Twelve patients received a total of 12 implants in the incisor, canine or premolar region of the upper and lower jaw at healed sites with immediate chair-side provisionalization. Four months later, the temporary crown was replaced by the permanent crown. After 40±13.1 months, clinical follow-up was conducted, assessing Probing pocket depth (PPD); Bleeding on Probing(BoP); Mucosal recession (MR) and Width of Keratinized Mucosa (KM). Papilla index (PI) was determined immediately after implant placement (baseline), before removing the temporary crown (t1), 4 weeks after insertion of the definitive crown (t2) and at the final follow-up examination (t3) to evaluate papilla formation and its change over time.RESULTS: None of the implants were lost. The mean PPD was 2.5±0.39 mm, BoP of 25% and 3.5 mm of KM were observed at the final follow-up. No implants showed MR. PI increased in all patients from 1.5±0.45 at baseline to 2.4±0.56 at t1, 2.6±0.47 at t2 and 3.02.6±0 at t3. The increase in PI between t0 and each individual timepoint from t1-t3 showed statistical significance.CONCLUSION: The present results indicate the suitability and benefit of immediate provisionalization to achieve favorable peri-implant soft-tissue conditions and papilla formation.

Abstract:OBJECTIVES: To evaluate whether zirconia implants demonstrate differences in hard and soft tissue integration compared to titanium implants in preclinical studies.MATERIAL AND METHODS: In March 2017, electronic (MEDLINE, EMBASE) and hand search was performed to identify preclinical studies comparing zirconia and titanium implants. Primary outcomes were bone-to-implant contact (BIC) and removal torque out (RTQ), respectively, push-in (PI) measurements. Secondary outcomes included biologic width (BW) dimensions.RESULTS: A total of 37 studies were included for data extraction after screening of 91 from 1,231 selected titles. Thirty-seven experimental studies using six different species were identified. The follow-up periods ranged between 0.4 and 56 weeks. For titanium, mean values of 59.1% (95% CI: 53.3 - 64.8), 102.6 Ncm (95% CI: 81.5 - 123.6), and 25.1 N (95% CI: 20.2 - 30.0) for BIC, RTQ, and PI were estimated, respectively. The mean values for zirconia were 55.9% (95% CI: 51.6 - 60.1), 71.5 Ncm (95% CI: 51.1 - 91.9), and 22.0 N (95% CI: 13.2 - 30.7) for corresponding parameters. Confounding factors such as animal species, implant material, loading protocol, and study or loading duration significantly influenced the outcomes. Similar qualitative soft tissue integration was reported for zirconia and titanium implants. However, faster maturation processes of epithelial and connective tissues around zirconia implants were assumed. Quantitatively, similar BW dimensions were evaluated for titanium (3.5 mm; 95% CI: 2.9 - 4.2) and zirconia (3.2 mm; 95% CI: 2.7 - 3.7), whereas the loading protocol significantly influenced the outcomes.CONCLUSIONS: Zirconia and titanium implants demonstrate a similar soft and hard tissue integration capacity. However, titanium tended to show a faster initial osseointegration process compared to zirconia. Importantly, not only material characteristics but predominantly animal species and study protocols can significantly influence the outcomes.
