Abstract:Statement of problem. Monolithic zirconia has excellent mechanical and biologic properties. However, evidence of the clinical properties of implant-supported monolithic zirconia prostheses is limited.Purpose. The purpose of this retrospective clinical study was to compare the peri-implant marginal bone changes of metal-ceramic and monolithic zirconia single crowns in the posterior region after prosthetic loading.Material and methods. A total of 224 participants treated with 327 implants restored with either metal-ceramic or monolithic zirconia single crowns in the posterior region between 2012 and 2016 were included in this study. Clinical outcomes, including the plaque index, peri-implant probing depth, and bleeding on probing, were recorded, and the marginal bone level was recorded by using the panoramic radiographs obtained at implant placement, second-stage surgery, and the most recent follow-up visit. The included parameters were analyzed with the nonparametric Mann-Whitney tests (a=.05).Results. The mean follow-up time was 30.4 months, and the cumulative survival rate of implants was 100% and that of the prostheses was 99.1%. The plaque index was 0.46 in the metal -ceramic group, which was significantly higher (P<.05) than 0.37 in the monolithic zirconia group. However, no significant differences (P>.05) were observed in peri-implant probing depth and bleeding on probing between the 2 groups. The marginal bone level at implant placement, second-stage surgery, and the most recent follow-up visit was above the implant platform in both the metal-ceramic and monolithic zirconia groups. The marginal bone changes of the metal-ceramic group was 0.31 mm in the healing period and 0.38 mm in the prosthetic loading period, while in the monolithic zirconia group, it was 0.25 mm in the healing period and 0.43 mm in the prosthetic loading period; no significant differences (P>.05) were observed between the 2 groups.Conclusions. The peri-implant marginal bone level change was comparable after prosthetic loading for metal-ceramic and monolithic zirconia single crowns, although monolithic zirconia was associated with reduced plaque. Outcome of Teeth Restored with CAD/CAM Zirconium Dioxide Post-Cores: a Retrospective Study with a Follow-Up Period of 3–6 Years

Abstract:Abstract Statement of problem Computer aided design/computer aided manufacturing (CAD/CAM) zirconia post-cores is one of the options of post crown restoration materials due to their esthetic properties and superior mechanical strength. However, the clinical effect on aesthetics and strength properties is unclear due to the lack of results of their long-term follow-up. Purpose This retrospective clinical study aims to analyze the survival rate, clinical manifestations, and failure factors after CAD/CAM zirconia post-core restoration. Material and methods Clinical and radiographic examinations were performed on 342 patients with 400 teeth for 3–6 years postsurgical follow-up examination. The patients were all received CAD/CAM zirconia post-cores and all-ceramic crowns at the Department of Prosthodontics in the public hospital. The retrospective outcomes were conducted after zirconia post restoration, including survival rate by Kaplan–Meier analysis and findings of manifestations and failure factors. The effects of gender and dental position on survival rate were analyzed by Cox–Mantel Test. Results This study retrospectively evaluated 261 teeth from 229 patients with a 35% drop-out rate. The survival rate was 96.0%, and the success rate was 92.4%. According to the tooth position classification, the survival rate was 100% for 101 anterior teeth, 95.4% for 69 premolars, and 88.3% for 91 molars. According to gender, the survival rate of the male group was 92.3%, while that of the female group was 98.0%, with a significant difference (P < 0.01). The complications included crown fracture (1.9%) periapical inflammation (1.9%), crown debonding (1.1%), percussion abnormal (1.9%) and root fracture (0.8%). Conclusions Within the limitations of this retrospective study, it can be concluded that CAD/CAM zirconia post-cores are clinically promising. Compared with the posterior teeth, CAD/CAM zirconia post-cores are more suitable for anterior teeth.
[Effect of Customized Zirconia Abutment on Peri-Implant Tissue: a One-Year Prospective Study].

Abstract:Objective: To observe the changes of peri-implant tissue around the individualized abutment that was grinded from zirconia provisional crown in one year. Methods: In this research, a prosthodontic-driven virtual implant planning and immediate provisionalization were conducted in computer assisted design software. And computer-aided design/computer-aided manufacturing (CAD/CAM) techniques were used to fabricate the zirconia provisional crown and surgical guide template before surgery. The implant was accurately placed with the surgical guide, and the zirconia provisional crown was immediately delivered after surgery. Three months later, the implant osseointegration was completed, and zirconia provisional crown was prepared intraorally to generate customized zirconia abutment for final prosthesis. The study included 30 patients with single anterior tooth loss, including 18 males and 12 females, aged from 26 to 50 years old, and the mean age was (36.2±6.1) years old. The patients were from the Center of Oral Implantology, The First Affiliated Hospital of Zhejiang University Medical College from January 2017 to February 2018. After cementation of the final prosthesis, the cases were followed up at 6 and 12 months time intervals. Implant survival rate, probing depth, bleeding on probing, marginal bone level loss and papilla index score (PIS) were recorded in every appointment. Results: The survival rate of 30 implants was 100%, and the probing depths were less than 5 mm. The bone resorption at 6 and 12 months follow-up after the final delivery was 0 (0, 0) mm and 0 (-0.2, 0) mm, respectively, and the difference was not statistically significant (P>0.05). The PIS was 3.0 (2.0, 4.0), 3.0 (2.8, 4.0) and 3.0 (3.0, 4.0) on the final delivery, 6 and 12 months after final delivery, respectively. Conclusions: Marginal bone level and bone loss were stable with this new implant clinical protocol at the one-year follow-up.
The Effect of Initial Biologic Width on Marginal Bone Loss: A Retrospective Study.

Abstract:Purpose: To evaluate the short-term effect of dental implant placement, mucosa thickness, and their combined effects (initial biologic width) on marginal bone loss. Materials and Methods: This was a retrospective study on patients who received implant surgery in the posterior region without bone augmentation surgery between 2012 and 2016, and implants had been loaded for more than 12 months. Each patient received radiographic examination before and after implant surgery, before the stage-two surgery, and during the 1-to 5-year follow-up. The thickness of mucosa, depth of dental implant placement, and crestal bone loss were evaluated on digital radiographs. The interaction was discussed by defining the combination of initial mucosal thickness and implantation depth as the initial biologic width. The implants were divided into four study groups based on the quartile of the initial biologic width. Results: This study included 266 patients (94 male and 172 female, 22 to 85 years of age, mean age: 51.43 years), with 413 dental implants placed including 239 Straumann implants and 174 Ankylos implants. The average follow-up was 21.50 months. After 1 to 5 years, the median crestal bone loss around implants was 0.35 mm (0.30 mm for Straumann BL and 0.40 mm for Ankylos). The implants were divided into four groups: group A (<= 2.85 mm), group B (2.85 to 3.40 mm), group C (3.40 to 3.97 mm), and group D ( 3.97 mm). Group B showed significantly less crestal bone loss than group A (0.38 mm vs 0.25 mm; P < .05) and group C (0.25 mm vs 0.40 mm; P < .05) during the follow-up. Significantly more crestal bone loss around implants was observed in the thin mucosa group than in the thick mucosa group (0.50 mm vs 0.30 mm; P < .001), while implants placed beneath the bone level displayed a significantly higher amount of marginal bone loss than implants placed even with the bone crest (0.50 mm vs 0.10 mm; P < .001). Conclusion: The initial biologic width has an effect on crestal bone loss. When the initial biologic width was between 2.85 and 3.40 mm, the marginal bone loss was lowest. Based on radiographic evaluation, implants placed in thick gingiva and even with the bone level showed less alveolar marginal bone loss compared with implants placed in thin gingiva and below the crestal bone level. Abstract:ObjectiveThe primary aim of this study was to determine the effect of implant-supported porcelain-fused-to-metal (PFM) and indirect-composite-resin (ICR) fixed dental prostheses on peri-implant marginal bone resorption (MBR) in custom-made anatomic modified zirconia dental implants.MethodsA prospective randomized controlled clinical trial was conducted. Participants with premolars indicated for dental extractions were recruited into this study to receive a single-unit implant-supported fixed dental prosthesis. Modified anatomic zirconia implants with thorny-retentive surfaces were placed and loaded randomly after 3 months with either PFM or ICR crowns. Participants were recalled after 12 and 18 months for radiographic evaluation of peri-implant MBR. Implants survival was also reported.Results18 out of 20 zirconia implants were included in all study phases. 18-month survival rate was 90%. After 12 months of implant placement, the mean MBR values were 0.53 (±0.21) mm and 0.60 (±0.14) mm in the ICR group compared to 0.67 (±0.16) mm and 0.61 (±0.27) mm in the PFM group. In the 18-month follow-up, the mean MBR values were 0.61 (±0.27) and 0.67 (±0.16) mm in the ICR group compared to 0.77 (±0.29) and 0.77 (±0.27) mm in the PFM group. No significant differences were found in MBR mean values between study groups at 12- and 18-month follow-up points.ConclusionThis study showed that PFM and ICR crowns were viable zirconia-implant-supported restorations with no preference regarding MBR after 18 months. Nevertheless, long-term evaluations are warranted.
Peri-implant Crestal Bone Changes Around Zirconia Implants in Periodontally Healthy and Compromised Patients.

Abstract:PURPOSE No consensus regarding the efficacy of zirconia implants in maintaining peri-implant hard and soft tissue health has yet been obtained. The aim of this retrospective follow-up study was to gain knowledge about peri-implant bone behavior and about implant survival and success after treatment with zirconia dental implants in patients with normal and compromised soft and hard tissue conditions. MATERIALS AND METHODS This follow-up study involved 86 patients with 123 zirconia implants (Straumann PURE Ceramic Implant) that were radiographically investigated directly after implant placement (day 0), 3 months after placement, and 1 year after the definitive implant crown placement. The clinical assessment was done at the 1-year postloading appointment and also included the modified Plaque Index, modified Sulcus Bleeding Index, and sulcus pocket depths. Eighteen patients with periodontally compromised conditions were compared to 68 patients with healthy periodontal conditions. RESULTS The survival rate was 100% and success rate was 94.5%, with no differences between the two groups. The alveolar crest around the ceramic implants showed no significant difference between day 0 and 1 year postloading for both groups (P > .05). There was also no significant difference at 1 year postloading between the groups in the distance from the implant shoulder to the peri-implant bone crest (P = .67) or in pocket depth (P = .07). CONCLUSION No significant peri-implant bone loss was observed in the first year. The survival and success rates showed no differences between the periodontally healthy and periodontally compromised groups; however, only a limited number of patients with periodontally compromised conditions were included in this study.

Abstract:BACKGROUND: Dental implants made of zirconia ceramics experienced a renaissance in the past years. However, there is still a lack in reliable long-term clinical data.PURPOSE: Aim of the present prospective clinical study was to analyze the dental implants made from zirconia, regarding their clinical performance compared to natural teeth (control).MATERIALS AND METHODS: Eighty-three zirconia implants in 28 adults were analyzed after 7.8 years of integration. Approximal plaque index (API), sulcus bleeding index (SBI), probing pocket depth (PPD), probing attachment level (PAL), and creeping or recession (CR/REC) of the mucosa/gingiva were analyzed. Changes in the marginal bone level (MBL) have been determined by comparing radiographs recorded immediately after implant placement and at the follow-up investigation. Furthermore, the pink esthetic score (PES), Periotest values (PTV), microbial colonization of the implant/dental sulcus fluid, and patient satisfaction were assessed.RESULTS: The survival rate of zirconia implants was 100%, however one implant presented a profound peri-implantitis, resistant to therapies. No statistical significance was observed between implants and teeth regarding SBI and PAL. Statistical significance was detected regarding API and CR/REC with significantly less plaque accumulation and recession in the study group. Significantly higher values for PPD were obvious in the zirconia implant group. PES of 9 ± 2.7 indicate a rather average esthetic result. Analysis of MBL revealed a mean peri-implant bone resorption of 1.2 mm. Microbial analysis revealed a nonstatistically significant higher colonization of bacteria in the study group, whereas single bacteria species were detected in statistically significant higher amounts.CONCLUSION: The investigated one-piece zirconia dental implants presented favorable long-term clinical results, comparable to natural teeth (SBI and PAL), and, regarding adhesion of plaque and creeping attachment (CR/REC), even superior. The microbial investigation revealed a shift in peri-implant colonization around zirconia implants over years, mainly without clinical effect on peri-implantitis.

[123I-OIH renoscintigraphy in children with chronic disorder of glomerulus].

Abstract:To evaluate the usefulness of 123I-orthoiodohippurate (OIH) renography, 31 patients with chronic disorder of glomerulus, from 5 years to 15 years old, were studied. In chronic glomerulonephritis and nephrotic syndrome with resistance to steroid therapy, Tmax and T 1/2 of the time-activity curve were statistically prolonged. Most of the renograms (time-activity curves) of the patients showed the hypofunction pattern. In most of the patients, other laboratory data, including BUN, PSP test and creatine clearance, didn't show abnormal values. 123I-OIH scan was considered to be a sensitive and useful method to evaluate the renal function in pediatric patients with chronic glomerulonephritis.

Abstract:We sought to determine the effects of longitudinal loading (artificial gravity) on skeletal muscle protein kinetics in 15 healthy young males after 21 days of 6 degrees head-down tilt bed rest [experimental treatment (Exp) group: n = 8, 31 +/- 1 yr; control (Con) group; n = 7, 28 +/- 1 yr, means +/- SE]. On days 1 and 21 of bed rest, postabsorptive venous blood samples and muscle biopsies (vastus lateralis and soleus) were obtained during a 1-h pulse bolus infusion protocol (0 min, l-[ring-(13)C(6)]phenylalanine, 35 mumol/kg; 30 min, l-[ring-(15)N]phenylalanine, 35 mumol/kg). Outcome measures included mixed muscle fractional synthesis (FSR) and breakdown rates (FBR). The Exp group experienced 1 h of longitudinal loading (2.5G at the feet) via a short-radius centrifuge during each day of bed rest. Mixed muscle FSR in the Con group was reduced by 48.5% (day 1, 0.081 +/- 0.000%/h vs. day 21, 0.042 +/- 0.000%/h; P = 0.001) in vastus lateralis after 21 days of bed rest, whereas the Exp group maintained their rate of protein synthesis. A similar but nonsignificant change in FSR was noted for the soleus muscle (Exp, -7%; Con, -22%). No changes in muscle protein breakdown were observed. In conclusion, 1 h of daily exposure to artificial gravity maintained the rate of protein synthesis of the vastus lateralis and may represent an effective adjunct countermeasure to combat the loss of muscle mass and functional during extended spaceflight.
Repeat conisation or HPV test? What should be done if histology of the primary conisation requires a second conisation?

Abstract:OBJECTIVE In our retrospective study we focused on the sensitivity of HPV DNA testing towards reducing the number of repeat (re)conisations. Is the second HPV test (pre repeat conisation) an appropriate method to reduce the number of interventions in histologically positive cases? STUDY 438 cervical conisations--loop electrosurgical excision procedure (LEEP)--were performed between March 2008 and August 2010 at our Gynaecology Department. Samples for high-risk HPV testing (Genoid, Hungary) were taken from the surface of the cervix and from the cervical canal before the LEEP procedure, and histopathological examinations were performed. Margin positivity was the indication for re-conisation (re-LEEP). RESULTS 119 (27.2%) out of 438 cases were re-conisations. In cases of histologically proven residual dysplasia (29 of 119) high-risk HPV infection was also detected by HPV testing. In 90 cases of 119 residual dysplasia was not seen by histological examination. In this high-risk group HPV infection had not been detected in 77 cases (85.5%) by the time the second HPV test was performed. HPV tests for high-risk types were positive only in 13 of 90 (14.5%) without residual dysplasia. Furthermore the same HPV type was detected only in three cases taken before the first and second conisation procedure. CONCLUSION Pre re-conisation HPV testing might be useful in reducing the number of re-conisations where the high-risk HPV test is either negative or does not confirm the previously proven HPV type.
[Influence of gingival biotype width on the health of peri-implant bone and soft tissues and the aesthetic outcome of the gingival papilla for single maxillary posterior implants]

Abstract:Purpose: To explore the influence of gingival biotype and width of keratinized gingiva on peri-implant bone tissue, soft tissue health, and esthetic outcome of the papilla surrounding single posterior maxillary implants. Methods: Seventy-eight patients who underwent single posterior maxillary implant surgery from May 2019 to September 2022 were selected, involving the placement of 78 implants. Based on periodontal probing outcomes one month post-restoration, the patients were divided into thin gingival biotype group(n=32) and thick gingival biotype group(n=46). Comparisons were made six months after implant restoration regarding buccal keratinized mucosa width(KMW), peri-implant bone tissue [implant bone loss(IBL)], soft tissue health [modified plaque index (mPLI), modified bleeding index for implants (mBLI), probing pocket depth (PPD)], and esthetic effect of the papilla [papilla index score (PIS), food impaction, gingival margin color satisfaction index (GMCS)]. Statistical analysis was performed with SPSS 27.0 software package. Results: The thick gingival biotype group showed significantly greater keratinized gingival width compared to the thin gingival biotype group (P＜0.05). Spearman correlation analysis revealed a positive correlation between gingival biotype and keratinized gingival width(r=-0.416, P=0.000). For peri-implant bone tissue, bone loss in the thin gingival biotype group was significantly higher than that in the thick gingival biotype group. In soft tissue health, the probing pocket depth for implants in the thin gingival biotype group was significantly less than that in the thick gingival biotype group. In terms of esthetic effect of the papilla, PES score in the thin gingival biotype group was significantly lower than in the thick gingival biotype group(P＜0.05). Pearson correlation analysis showed a negative correlation between gingival biotype and papilla index score, GMCS, bleeding on probing, and PPD, but a positive correlation with food impaction, bone loss and mPLI(P＜0.05). The width of keratinized gingiva was positively correlated with papilla index score, GMCS, bleeding on probing and PPD, but negatively correlated with food impaction, bone loss and mPLI(P＜0.05). There was significantly difference between thin and thick gingival biotype groups for KMW >2 mm(P＜0.05). A significant difference was showed in thick gingival biotype group when KMW ≤2 mm and ＞2 mm(P＜0.05). Conclusions: Gingival biotype and keratinized mucosa width significantly influence peri-implant bone and soft tissue health as well as esthetic outcome of the papilla around single posterior maxillary implants, offering guidance for predicting the long-term success and esthetic outcomes of implants.

Abstract:Highlights • An overview of developments, challenges and strategies related to soft tissue integration around zirconia abutments. Stable soft tissue integration around the implant abutment attenuates pathogen penetration, protects underlying bone tissue, prevents peri-implantitis and is essential in maintaining long-term implant stability. The desire for "metal free" and "aesthetic restoration" has favored zirconia over titanium abutments, especially for implant restorations in the anterior region and for patients with thin gingival biotype. Soft tissue attachment to the zirconia abutment surface remains a challenge. A comprehensive review of advances in zirconia surface treatment (micro-design) and structural design (macro-design) affecting soft tissue attachment is presented and strategies and research directions are discussed. Soft tissue models for abutment research are described. Guidelines for development of zirconia abutment surfaces that promote soft tissue integration and evidence-based references to inform clinical choice of abutment structure and postoperative maintenance are presented. Effects and underlying mechanism of micro-nano-structured zirconia surfaces on biological behaviors of human gingival fibroblasts under inflammatory conditions

Abstract:Zirconia is one of the most commonly used materials for abutments of dental implants, especially in the anterior region. Soft tissue integration to the zirconia abutment surface remains a challenge. Peri-implant soft tissue integration serves as a physiological barrier, attenuating pathogen penetration and preventing peri-implant disease. The surface microstructure of zirconia has significant effects on the biological behaviors of human gingival fibroblasts (HGFs), but the effects under inflammatory conditions are still unclear. In this study, we established two micro-nano structures on zirconia surfaces using a femtosecond laser, including microgrooves with widths of 30 μm (G3) and 60 μm (G6) and depths of 5 μm, and nanoparticles inside the microgrooves. Polished surfaces were used as controls. HGFs were seeded onto the three groups of zirconia specimens and stimulated with lipopolysaccharide. The HGFs on micro-nano-structured zirconia surfaces exhibited lower inflammatory responses and higher cell adhesion, proliferation, and migration under inflammatory conditions compared with the polished surfaces. Additionally, the G3 group exhibited lower inflammatory responses and higher cell adhesion and migration than the G6 group. The micro-nano-structured zirconia surface exhibited decreased neutrophil infiltration and increased M2-type macrophage polarization in vivo. To explore the molecular mechanism, RNA sequencing and gene silencing were utilized, which revealed two critical target genes regulated by the G3 group. Overall, we proposed an innovative micro-nano-structured zirconia surface that reduced the in vitro and in vivo inflammatory responses and promoted HGF adhesion, migration, and proliferation under inflammatory conditions, in which TRAFD1 and NLRC5 were the underlying key genes. In this study, we developed an innovative micro-nano-structured zirconia surface using a femtosecond laser, which reduces the in vitro and in vivo pro-inflammatory responses and promotes HGFs adhesion, migration, and proliferation under inflammatory conditions compared with the polished zirconia surface. The potential underlying mechanism was also investigated. This work has provided some theoretical basis for the micro-nano-structured zirconia surface in potentially reducing the inflammation and enhancing peri-implant soft-tissue integration under inflammatory conditions.

Abstract:Objective:To compare the effects of periodontal tissue between gingival retraction or not before the ce?mentation of zirconia ceramic crowns on anterior teeth. Methods:Follow up and measure the meliorated plaque index (mPLI),meliorated sulcus bleecling index(mSBI) and probing depths of two groups that each group with 50 patients,who were division by gingival retraction or not before the cementation of zirconia ceramic crown. Results:Reexamined all the pa?tients after cementation in 1,3 and 6 months. There were no statistically significant difference between two groups in the mPLI and probing depths after 1 month and 3 months respectively. But there was statistically significant difference after 6 months( P<0. 05). There was no statistically significant difference between two groups in the mSBI after 1 month,but there were statis?tically significant difference after 3 and 6 months,respectively(P<0. 05). Conclusion:The technology of gingival retraction before the cementation of zirconia ceramic crowns on anterior teeth,which can prevent periondontal disease,and release the damage of periondontal tissue.

Abstract:OBJECTIVE: Limited clinical evidence has been reported in dental literature regarding zirconia dental implants. The aim of the present retrospective consecutive case series was to evaluate the clinical outcome of zirconia dental implants with 1 to 5 years of follow-up.CLINICAL CONSIDERATIONS: The clinical outcome of 24 implants in 12 consecutive patients (male 5, female 7) with a mean age of 55 years (range 27-86) is reported. Radiographic crestal bone level was assessed by the use of "ImageJ" software program. Gingival and plaque indices were recorded at baseline and latest follow-up.CONCLUSION: The overall success rate of zirconia implants was 92%. Within the limitations of the present clinical evaluation, zirconia implants provided excellent clinical results and esthetic outcomes. A mean periimplant bone loss of 0.3 mm was measured in 33.3% of the implants and 66.7% were not affected by radiographic detectable periimplant bone loss. Two implants in two patients failed. Low gingival and plaque indices were predominant values for both interim and final restorations. The apparent less affinity to plaque accumulation may favor soft tissue health around zirconia dental implants and decrease the risk of inflammation or infection. Zirconia dental implant merits further investigation.CLINICAL SIGNIFICANCE: Zirconia dental implants are emerging as an option in clinical practice of implant dentistry providing stable clinical results and esthetic outcomes.

Abstract:Prevention of bacterial inflammation around dental implants (peri-implantitis) is one of the keys to success of the implantation and can be achieved by securing the gingival tissue-abutment interface preventing penetration of bacteria. Modern dental practice has adopted zirconia abutments in place of titanium, but the adhesion of gingival tissue to zirconia is inferior to titanium. The aim of this study was to assess and improve the adhesion of mucosal tissues to zirconia posts using sol-gel derived TiO2 coating following dynamic mechanical testing. The posts were cultivated with porcine bone-gingival tissue specimens in vitro for 7 and 14 days and then subjected to dynamic mechanical analysis simulating physiological loading at 1 Hz up to 50 μm amplitude. In parallel in silico analysis of stresses and strains have been made simulating “the worst case” when the fixture fails in osseointegration while the abutment still holds. Results show treatment of zirconia can lead to double interface stiffness (static shear stiffness values from 5–10 to 17–23 kPa and dynamic from 20–50 to 60–125 kPa), invariant viscostiffness (from 5–35 to 45–90 kPa·sα) and material memory values (increased from 0.06–0.10 to 0.17–0.25), which is beneficial in preventing bacterial contamination in dental implants. This suggests TiO2-coated zirconia abutments may have a significant clinical benefit for prevention of the bacterial contamination.

Abstract:Statement of problem: Despite the broad clinical application of zirconia for fixed implant-supported prostheses, evidence of long-term performance is sparse. Purpose: The purpose of this retrospective study was to evaluate the long-term clinical and radiographic outcomes of zirconia-based partial and complete screw-retained implant-supported zirconia fixed dental prostheses (ISZFDPs). Material and methods: Records of patients treated with dental implants and ISZFDPs between December 2004 and June 2017 were screened. Eligible study participants, according to inclusion criteria, were contacted and invited to undergo clinical and radiographic examinations. Outcomes were evaluated as implant and prosthetic survival rates, prosthetic success rate, complications, marginal bone level (MBL) change, and soft tissue condition. Along with the effects of zirconia prosthesis type and level, the effects of implant type and connection, type of loading, and follow-up on MBL were tested with a generalized linear effects model (GLEM) (α=.05). Results: A total of 118 patients were identified, of whom 20 (16.9%) were not available for clinical examination for various reasons. Ninety-eight participants (mean age 60.7 ±11.7 years) with 337 implants were included, of which 176 (52.2%) had been immediately loaded. A total of 111 ISZFDPs (96 zirconia connection and 15 titanium base) were investigated: 24 complete ISZFDPs with a zirconia connection (12.9 ±0.97 dental units, minimum 12, maximum 14), 72 partial with a zirconia connection (3.11 ±1.12, minimum 2, maximum 7), 15 partial with a titanium base (3.62 ±1.02, minimum 2, maximum 5). Forty ISZFDPs had been in function for ≥10 years (36%), 38 for 5 to 9 years (34.2%), and 33 for 2 to 4 years (22.8%). The mean follow-up time was 7.2 ±3.4 years. No zirconia fractures were identified. Two implants and 2 ISZFDPs failed, with chipping being the most common complication (13.5%). The implant survival rate was 99.4%, and the prosthetic survival rate was 98.2%. The cumulative prosthetic success rate was 91.9%. MBL change was -0.18 ±0.59 mm. Thirteen implants were treated for peri-implantitis (3.8%), and 9 for mucositis (2.7%), but presented healthy peri-implant soft tissues at the follow-up examination. A significant difference was found between the implant-level and abutment-level prostheses (P=.013), with less marginal bone loss observed in ISZFDPs delivered at the implant level. Conclusions: Zirconia-based screw-retained implant-supported prosthesis can be considered a reliable long-term treatment option for partial and complete edentulism. No zirconia fractures were experienced. Stable bone levels and low peri-implantitis rates were reported regardless of the ISZFDP type and level, implant type and connection, and type of loading.
Zirconia implant abutments supporting single all-ceramic crowns in anterior and premolar regions: A six-year retrospective study

Abstract:Background: Clinical studies regarding zirconia implant abutments reported good survival rates in the short-term observation period. The purpose of this study was to assess the six-year clinical performance of zirconia abutments supporting all-ceramic crowns in anterior and premolar regions. Methods: The patients received zirconia implant abutments to support all-ceramic crowns in Chang-Gung Medical Center during the period August 2010 to August 2011 were enrolled. In the following six years of observation period after the implant-crown had finished, the clinical parameters of all of the included patients were registered on a special form. The records regarding the following variables: age, gender, implant location, the condition of edentulous site before implant placement, esthetic performance at baseline, presence or absence of technical complications, and biological outcomes were registered and scrutinized for evaluation. Results: Out of the 32 zirconia implant abutments and 32 all-ceramic crowns that were followed for six years. Neither abutments nor crowns were lost, yielding 100% survival rates for both zirconia abutments and crowns. The esthetic outcomes were excellent except that a score of 2 was given to two restorations. With regard to technical complications, there was one instance of abutment screw loosening, two cases of veneering ceramic chipping, one restoration with occlusal roughness, and three instances of crowns loosening. Overall, the success rates were 96.8% and 81.2% for abutments and crowns respectively. In biological performance, only 1 implant was classified in group II (satisfactory survival) in the Misch classification, while all the others were classified in group I (excellent). Conclusions: Zirconia abutments supporting all-ceramic crowns demonstrated high survival rate, good biological and esthetic results. While some technical complications were frequently observed, the complication-free rates were 96.8% for abutments and 81.2% for crowns in the medium-term observation period.

Abstract:BACKGROUND:To evaluate the clinical outcome and changes in posterior buccal soft tissue following implant restoration in groups with and without a free gingival graft (FGG) before implant placement. METHODS:Twenty-six individuals who required implant restoration and displayed lack of keratinized mucosa (KM) were recruited and assigned to the FGG group (with FGG before implant placement) or Control group (without FGG before implant placement) randomly. A screw-retained conventional implant restoration was performed for each patient. Peri-implant soft tissue was captured by an intraoral scanner and analyzed by an image processing software. Clinical parameters (plaque index, gingival index, probing depth, and bleeding on probing) were assessed at baseline and 1, 3, 6, and 12 months. Buccal soft tissue changes (mucosal margin, soft tissue thickness, and width of keratinized mucosa) on the buccal side of implant site were assessed at 1, 3, 6, and 12 months. Two-way ANOVA and Bonferroni test were used to analyze significant difference between groups at each time point (α = 0.05). RESULTS:The clinical parameters were lower in the FGG group than that in the Control group, although there were no significant differences between the two groups (P > 0.05). Peri-implant soft tissue collapsed and the changes (mucosal margin and soft tissue thickness) were significantly greater in the Control group than the FGG group (P < 0.05). Width of KM was larger in the FGG group than the Control group, although there was no significant difference between the two groups (P > 0.05). CONCLUSIONS:Minimal peri-implant soft tissue changes occurred in two groups. Performing FGG before implant placement is a viable procedure to maintain peri-implant soft tissue but might not affect peri-implant health during 12 months follow-up. However, small sample size must be considered.
