Abstract:Objective:With the accumulated experience from the quality assurance and quality control work that our institute has done since the beginning of IGRT technology,the major quality control and monitoring programs and realization methods in IGRT system are analyzed and discussed,to ensure the safe,efficient and stable operation.Methods:The image low contrast visibility, spatial resolution,uniformity and the geometry accuracy of the imaging system were detected,the registration accuracy of the system was tested,the isocenter of the six-degree couch was calibrated.Results:The image low contrast visibility was below 2%,the spatial resolution was no less than 7lp/ cm,the deviation of the pixel value which indicates the image uniformity was below 2%,the geometry accuracy which represents the degree of image distortion was below 1.04mm in X,Y,Z directions, respectively.When the XVI system rotates with the gantry,the deviation of rotation isocenter due to gravity was below 4pixel;the deviation between the KV system and MV system was below 0.5mm in X, Y,Z directions,respectively.The deviation between the six-degree couch and the isocenter was within 0.2mm,0.2°.Conclusion:The corresponding quality control programs and methods could ensure the operational safety and stability,and helped to minimizing the setup errors during the patients' radiotherapy.
Analysis of verification results of intensity-modulated radiation therapy for 2887 cases and its effect in quality control

Abstract:Objective: To analyze the reason that verification plan could not be approved and its effect in the quality control according to the verification result of intensity-modulated radiation therapy (IMRT). Methods:The intensity modulated plans of 2887 patients pre-radiotherapy were carried out verification analysis. These non-approved intensity modulated plans were carried out classified statistic as type of error, and the reasons were counter calculated according to statistical discrepancy and quality control results of accelerator. Results: 2838 cases of the 2887 cases were passed, and the qualified rate was 98.30%. Under the quality control of same standard, the qualified rates of one accelerator pre and post resurveying data were 96.92% and 99.24%, respectively. And the amount of non-approved plans, which reason was that the optimal result of single treatment planning system (TPS) was poor, were 16.33% in all of non-approved intensity-modulated plans. Conclusions: Based on ensuring the consistency of calculated dose of TPS and exposure dose of patients, the verification of IMRT pre-radiotherapy can more contribute to the comprehensive quality control for accelerator and radiotherapy process.
National Survey of Patient Specific IMRT Quality Assurance in China

Abstract:BACKGROUND:To analyze and present the China's national survey on patient-specific IMRT quality assurance (QA).METHODS:A national survey was conducted in all radiotherapy centers in China to collect comprehensive information on status of IMRT QA practice, including machine, technique, equipment, issues and suggestions.RESULTS:Four hundred and three centers responded to this survey, accounting for 56.92% of all the centers implementing IMRT in China. The total number of medical physicists and the total number of patients treated with IMRT annually in these centers was 1599 and 305,000 respectively. All centers implemented measurement-based verification. Point dose verification and 2D dose verification was implemented in 331 and 399 centers, respectively. Three hundred forty-eight centers had 2D arrays, and 52 centers had detector devices designed to measure VMAT beams. EPID and film were used in 78 and 70 centers, respectively. Seventeen and 20 centers used log file and 3D DVH analysis, respectively. One hundred sixty-eight centers performed measurement-based verification not for each patient based on different selection criteria. The techniques and methods varied significantly in both point dose and dose distribution verification, from evaluation metrics, criteria, tolerance limit, and steps to check failed IMRT QA plans. Major issues identified in this survey were the limited resources of physicists, QA devices, and linacs.CONCLUSIONS:IMRT QA was implemented in all the surveyed centers. The practice of IMRT QA varied significantly between centers. An increase in personnel, QA devices and linacs is highly desired. National standard, guideline, regulation and training programs are urgently needed in China for consistent and effective implementation of IMRT QA.

Abstract:Intensity-Modulated Radiation Therapy (IMRT) mathematically forms a large-scale optimization problem. The development of an IMRT plan is computationally expensive resulting in time-consuming, inefficient, and difficult to develop high-quality IMRT plans. By combining prior knowledge with proposed novel measures derived from both Overlap Volume Histogram (OVH) descriptors and Dose Volume Histograms (DVHs), a novel quality control method for IMRT planning is proposed to assure the high quality of IMRT plan. Clinical approved nasopharyngeal IMRT plans were employed for the experiments, where the reference plan is firstly retrieved from IMRT plan database for each query case by using measures derived from both OVH descriptors and DVHs. Then the DVHs of the reference plan are served as additional goals for the IMRT plan re-optimization. The experimental results show that the proposed method can effectively pick out those IMRT plans, whose quality could be improved substantially (i.e. the doses of their Clinical Targets Volume (CTV) could be effectively increased) and the dose of their Organs at Risk (OARs) could be reduced after the IMRT plan has being re-optimized. In conclusion, the proposed methods can effectively guarantee the high quality of the IMRT planning.

Abstract:Machine- and patient-specific quality assurance (QA) is essential to ensure the safety and accuracy of radiotherapy. QA methods have become complex, especially in high-precision radiotherapy such as intensity-modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT), and various recommendations have been reported by AAPM Task Groups. With the widespread use of IMRT and VMAT, there is an emerging demand for increased operational efficiency. Artificial intelligence (AI) technology is quickly growing in various fields owing to advancements in computers and technology. In the radiotherapy treatment process, AI has led to the development of various techniques for automated segmentation and planning, thereby significantly enhancing treatment efficiency. Many new applications using AI have been reported for machine- and patient-specific QA, such as predicting machine beam data or gamma passing rates for IMRT or VMAT plans. Additionally, these applied technologies are being developed for multicenter studies. In the current review article, AI application techniques in machine- and patient-specific QA have been organized and future directions are discussed. This review presents the learning process and the latest knowledge on machine- and patient-specific QA. Moreover, it contributes to the understanding of the current status and discusses the future directions of machine- and patient-specific QA.
Step and Shoot Planning of Intensity-Modulated Radiotherapy and 30 Patients Dosimetric Verification Analysis

Abstract:Objective To discuss the process of step and shoot intensity-modulated radiotherapy, dosimetric verification and quality control. MethodsThirty patients were treated with step and shoot intensity-modulated radiotherapy(IMRT) technique from May 2003 to May 2004, including nasopharynx cancer, rectum cancer, lung cancer and so on. The plans were made in Nuclear PLATO planning treatment system. The doses of the interesting points and isodose distributions of the interesting planes in phantom were verified in Simens Primus accelerator. Results The error between the measured and calculated in the interesting points was less than 5%. Isodose distribution and fluent map by the film were consistent with those predicted by the planning. Conclusion The dosimetric verification of IMRT is a reliable measure in the course of radiotherapy.

Abstract:The increasing complexity of modern radiation therapy planning and delivery challenges traditional prescriptive quality management (QM) methods, such as many of those included in guidelines published by organizations such as the AAPM, ASTRO, ACR, ESTRO, and IAEA. These prescriptive guidelines have traditionally focused on monitoring all aspects of the functional performance of radiotherapy (RT) equipment by comparing parameters against tolerances set at strict but achievable values. Many errors that occur in radiation oncology are not due to failures in devices and software; rather they are failures in workflow and process. A systematic understanding of the likelihood and clinical impact of possible failures throughout a course of radiotherapy is needed to direct limit QM resources efficiently to produce maximum safety and quality of patient care. Task Group 100 of the AAPM has taken a broad view of these issues and has developed a framework for designing QM activities, based on estimates of the probability of identified failures and their clinical outcome through the RT planning and delivery process. The Task Group has chosen a specific radiotherapy process required for "intensity modulated radiation therapy (IMRT)" as a case study. The goal of this work is to apply modern risk-based analysis techniques to this complex RT process in order to demonstrate to the RT community that such techniques may help identify more effective and efficient ways to enhance the safety and quality of our treatment processes. The task group generated by consensus an example quality management program strategy for the IMRT process performed at the institution of one of the authors. This report describes the methodology and nomenclature developed, presents the process maps, FMEAs, fault trees, and QM programs developed, and makes suggestions on how this information could be used in the clinic. The development and implementation of risk-assessment techniques will make radiation therapy safer and more efficient.

Abstract:Abstract Background Intensity-modulated radiation therapy (IMRT) and volume-modulated arc therapy (VMAT) are rather complex treatment techniques and require patient-specific quality assurance procedures. Electronic portal imaging devices (EPID) are increasingly used in the verification of radiation therapy (RT). This work aims to develop a novel model to predict the EPID transmission image (TI) with fluence maps from the RT plan. The predicted TI is compared with the measured TI for in vivo treatment verification. Methods The fluence map was extracted from the RT plan and corrections of penumbra, response, global field output, attenuation, and scatter were applied before the TI was calculated. The parameters used in the model were calculated separately for central axis and off-axis points using a series of EPID measurement data. Our model was evaluated using a CIRS thorax phantom and 20 clinical plans (10 IMRT and 10 VMAT) optimized for head and neck, breast, and rectum treatments. Results Comparisons of the predicted and measured images were carried out using a global gamma analysis of 3%/2 mm (10% threshold) to validate the accuracy of the model. The gamma pass rates for IMRT and VMAT were greater than 97.2% and 94.5% at 3%/2 mm, respectively. Conclusion We have developed an accurate and straightforward EPID-based quality assurance model that can potentially be used for in vivo treatment verification of the IMRT and VMAT delivery.

Abstract:Objective To develop the methodology for using TLD and radiochromic film to measure the planned target volume (PTV) and organ at risk (OAR) doses and 2D dose distribution in IMRT,in order to provide technical guidance on the dose quality audit in IMRT at home.Methods China has participated in the research project launched by the international multi-radiotherapy centre (IMRC).IMRT polystyrene phantom provided by IAEA was scanned by CT scanner and then the scanned images were transmitted to TPS to outline prescribed dose to PTV and to OAR.The former was limited to 400 cGy while the latter limited to 200 cGy.IMRT was implemented with the phantom irradiated using 6 MV X-ray.The irradiated TLDs and films were sent to IAEA dosimerty laboratory for measurement and calculation.Jiangsu,Sichuan,Hubei and Henan provinces were selected to engage with this study for their variety of accelerators and highly skilled physicists.The procedures used were the same as in the IMRC and the irradiated TLDs and films were required to send to external audit group for measurement and calculation.Results According to IAEA requirement,the relative deviations of the TLD-measured and TPS planned doses are within ±7.0％ for PTV and OAR.The China's research results at the IMRC have shown that the relative deviation of TLD-measured and TPS-planned values for the upper and lower PTV were-0.2％ and 0.8％,respectively,consistent with the IAEA requirement,and the values for upper and lower OAR were -0.6％ and-1.0％,respectively,consistent with the requirement.As the results have shown in four provinces,the relative deviations of the TLD-measured and TPS-planned were within 0 to 10.6％ for upper and lower PTV and-0.6％ to 20.9％ for upper and lower OAR.According to IAEA requirement,the passing rate should be greater than 90％ for 3 mm /3％ for 2D dose distribution.China's result at the IMRC is 100％,being excellent.The four provinces' results have shown that 2D dose distribution pass rate of 3 mm/3％ was in the range of 45.0％-100.0％.Conclusions The uses of TLD in quality audit for PTV and OAR doses and the radiochromic film in 2D dose distribution pass rate in IMRT are characterized by scientific feasibility,strong operability,easy-to-mail and data realibility.They are can be applied to quality assurance and audit in medical institutions in the country to on a large scale.
Preliminary Research on Quality Control of PACS Image Display Device

Abstract:Objective To study the quality assurance(QA) and quality control(QC) of PACS image display device in the department of radiology equipped with PACS and radiology information system (RIS). Methods Routine maintenance and periodic calibration were performed using photometer and automatic calibration software according to American Association of Physicists in Medicine Task Group 18 (AAPM TG18) test patterns and evaluating criterion. Digital image and communication in medicine (DICOM) gray scale standard display function (GSDF) standard calibration ,maximum luminance and minimum luminance,luminance uniformity,display resolution and geometry distortion adjustment were performed quarterly on BARCO CRT displays. Results All the results were well conformed to the criteria recommended by the AAPM TG18. About 95 percents of radiological images were interpreted on the PACS diagnostic image display.Conclusion The QC program of PACS image display device is an essential to ensure high-quality in digital medical environment.
Identical Quality Assurance for Volumetric Modulated Arc Therapy in Elekta and Varian Machines

Abstract:Volumetric modulated arc therapy (VMAT) has been adopted by many clinics for its higher delivery efficiency compared to conventional intensity modulated radiotherapy techniques. Currently, the quality assurance (QA) has remained a challenge in that no identical tests are available for accelerators from different vendors. This study is the first attempt to design identical QA tests for the VMAT technique for Varian and Elekta machines. Identical procedures testing MLC positions and movements, dose rate variations, and gantry positions and movement were created for both machines. These included picket fence (PF), dose rate vs. gantry speed (DRGS) and MLC speed vs. dose rate (MLCDR) tests. Deliverable plans for these tests were made with in-house software that was deliverable for linacs from two vendors (Elekta Synergy and Varian Trilogy). The electronic portal imaging device (EPID) was used for these tests. An automated analysis method was established and software was created to quantitatively evaluate the result. The systematic gap position and width errors from PF tests were within 0.5 mm. We evaluated the detectability of this program for introduced errors down to 0.2 mm. Linear relationships existed between the introduced errors and measured errors. In the DRGS test, 99.8% and 100.0% of the intensity deviations from expected profiles were less than 3% for the Synergy and Trilogy, respectively. For the MLCDR, the intensity deviations from expected profiles less than 3% were 100.0% for Synergy and 98.1% for Trilogy. Identical test series were created and implemented for VMAT accelerators from two vendors. Test results were reported from both accelerators. Comparable results were obtained from both vendors, enabling uniform criteria to be established for VMAT quality assurance.

Abstract:The use of intensity-modulated radiation therapy (IMRT) is rapidly advancing in the field of radiation oncology. Intensity-modulated radiation therapy allows for improved dose conformality, thereby affording the potential to decrease the spectrum of normal tissue toxicities associated with IMRT. Preliminary results with IMRT are quite promising; however, the clinical data is relatively immature and overall patient numbers remain small. High-quality IMRT requires intensive physics support and detailed knowledge of three-dimensional anatomy and patterns of tumour spread. This review focuses on basic principles, and highlights the clinical implementation of IMRT in head and neck and prostate cancer.

Abstract:Helical tomotherapy (HT), as a new IMRT technology, utilizes a fan beam of radiation for treatment. It combines the main characteristics of a modern CT scanner and a linear accelerator to achieve the function of helical tomotherapy. Due to the complexity of the system with a highly integrated and automated features, monitoring its operation, continuing to improve the stability and reliability, and simplifying its quality control procedures has become an important part of quality assurance (QA) for HT. Based on the results of afi ve-year quality control (QC) program, and the initial application of new QA equipment, this study wil summarize the standardization mode of its QA and explore the changes of QA mode.

Abstract:Objetive: To design and apply of a quality control system for a network of radiotherapy.Methods: A radiotherapy network based on PACS,which was introduced the model of client-server machine and local network,was to establish a free communication between radiotherapy and related facilities.Patient data including treatment data and image data were classified in an integrated database.Results: A network security system,a purview of different user,quality control(QC) of MU(machine unit) calculated by software,QC of MU calculated by TPS and QC of data communication and backup were five parts of QC system.Conclusions: The application of QC system has guaranteed radiotherapy network to avoid errors,and is an important part of the radiotherapy network.

Abstract:Objective To develop the methodology for using TLDs and films to measure absorbed dose and 2D dose distribution produced by the multi-leaf collimator (MLC) in intensity modulated radiotherapy (IMRT),in order to provide the guidance on dose quality audit in IMRT.Methods A total of 30 different-typed accelerators were selected from 27 hospitals in Jiangsu,Sichuan,Hubei and Henan provinces,including 17 Varian accelerators,10 Elektas and 3 Simens.The same batch of films and TLDs were put in a 2 cm-thick solid plate for fixation and then loaded in a 15 cm × 15 cm × 15 cm polysyrene solid phantom supplied by International Atomic Energy Agency(IAEA) in terms of 90 cm SSD,19 cm depth,10 cm × 10 cm field at different doses.The standard dose curves wcrc established for film and TLD,respectively.The irradiated film was measured and then sent to the External Audit Group (EAG) in China.The TLD-and film-absorbed doses were compared with TPS-calculated doses.The 2D dose distribution on the IRMT MLC field was measured using films.The homogeneous phanton of 30 cn × 30 cm was scanned by CT and the image was transferred to the TPS.The IMRT was implemented with 6 Gy fractionated irradiation by placing a 25 cm × 25 cm film on the phantom surface at 95 cm SSD and at 5 cm depth.The irradiated film was sent to the IAEA dosimetry laboratory for measurement and calculation.2D dose distribution verification was conducted in thc same way consistent with the procedure of international multi-radiotherapy center.The 3 mm/3％ passing rate was calculated for 2D dose distribution and compared with the film-measured and TPS calculated result.Results IAEA requires the relative deviation of TLD and film measured absorbed dose are with in ± 5％.The relative deviation of TLD-and filmmeasured to TPS-calculated absorbed dose was within the range of ±0.7％-± 8.5％ and within ±0.3％ ±7.8％ in Jiangsu,Sichuan,Hubei and Henan provinces,respectively.IAEA requires the 3 mm/3％ passing rate of film-measured 2D distribution to be 90％.The result of the present study were up to 94.0％.The verification result of 2D dose distribution were within 70.0％-99.9％ in Sichuan,Jiangsu,Hubei and Henan provinces.Conclusions The adsorbed dose and 2D distribution can be audited using TLDs and films for MLC in IRMT.The method is scientific and applicable,economical and convenient for development of dose quality audit for a wide range of IRMT.

Abstract:Purpose This study aimed to improve the safety and accuracy of radiotherapy by establishing tolerance (TL) and action (AL) limits for the gamma index in patient‐specific quality assurance (PSQA) for intensity‐modulated radiation therapy (IMRT) and volumetric‐modulated arc therapy (VMAT) using SunCHECK software, as per AAPM TG‐218 report recommendations. Methods The study included 125 patients divided into six groups by treatment regions (H&N, thoracic and pelvic) and techniques (VMAT, IMRT). SunCHECK was used to calculate the gamma passing rate (%GP) and dose error (%DE) for each patient, for the planning target volume and organs at risk (OARs). The TL and AL were then determined for each group according to TG‐218 recommendations. We conducted a comprehensive analysis to compare %DE among different groups and examined the relationship between %GP and %DE. Results The TL and AL of all groups were more stringent than the common standard as defined by the TG218 report. The TL and AL values of the groups differed significantly, and the values for the thoracic groups were lower for both VMAT and IMRT. The %DE of the parameters D95%, D90%, and Dmean in the planning target volume, and Dmean and Dmax in OARs were significantly different. The dose deviation of VMAT was larger than IMRT, especially in the thoracic group. A %GP and %DE correlation analysis showed a strong correlation for the planning target volume, but a weak correlation for the OARs. Additionally, a significant correlation existed between %GP of SunCHECK and Delta4. Conclusion The study established TL and AL values tailored to various anatomical regions and treatment techniques at our institution. Establishing PSQA workflows for VMAT and IMRT offers valuable clinical insights and guidance. We also suggest developing a standard combining clinically relevant metrics with %GP to evaluate PSQA results comprehensively.

By establishing quality control system in radiation therapy, standardization of radiation therapy workflow, strengthening quality control of devices and physical technique and paying attention to safety protection and staff training.

Abstract:3JECOG P-01 is a prospective, random, multicenter study of simultaneous integrated boost intensity modulated radiation therapy (SIB-IMRT) with S1 in elderly patients with esophageal cancer (EC). The purpose of our study is to discover the difference in target delineation among different hospitals, and explore a possible method to achieve higher quality assurance. Three representative EC patients，including upper, middle, and lower thoracic portions, were selected and sent to each hospital to be contoured based on their own contouring principles for the first time. Subsequently, a contouring protocol, revised at the second Multicenter Conference, was sent to each center for the second time of contouring.After completion, an experienced physicist was invited to accomplish all radiotherapy plans. The degree of target dispersion was quantified using standard deviation (St.D) for both target volumes and dosimetric parameters, as well as spatial conformability (SC) for the spatial overlap of multicenter target volumes. We defined the first contouring volumes as the routine group and the second as the protocol group. A total of 16 radiation oncologists were involved in this study, of which 9 provided both groups of target volumes. For distal case, degree of target dispersion in clinical target volume (CTV) and mean lung dose (MLD) was large between the two groups, with St.D of 80.54, and 114.07 in the routine group , comparing individually with 65.84 and 65.39 in the protocol group. Besides, SC in PTV and gross tumor volumes（GTV）showed an significant enhancement, from 0.42 and 0.40 to 0.56 (P=0.009) and 0.74 (P=0.005).Similar results were observed in upper and middle cases, in that SC in GTV enhanced significantly, and St.d in dosimtric parameters was reduced to a certain extent. This is in contrast to SC in CTV, which did not achieve significant increase. Significant intercentral variation was observed in SIB-IMRT fields for elder EC patients, especially for those cases with distant lymph node metastasis. A clear contouring protocol, evidently necessary for mandatory quality assurance, can ensure that the degree of dispersion in target volumes, as well as its relative dosimetric parameters, are controlled. Ultimately this will ensure a solid foundation for the reliability of the research results.
