Abstract:Purpose: The objective of this study was to compare and assess the accuracy of computed tomography (CT)-based target delineation with that of magnetic resonance imaging (MRI)-based on high-dose-rate brachytherapy (HDR-BT) for patients with cervical cancer. Material and methods: Data of 20 patients with locally advanced cervical cancer were collected and evaluated. Dimensions, conformity, and dose parameters of high-risk clinical target volume (CTVHR) as well as D-0.(1cc), D-1(cc), and D-2cc of organs at risk (OARs) based on MRI were compared with those based on CT. Results: Average age of 20 patients included was 57.8 years. Width, thickness, and volumes of CT-based CTVHR (CTVHR-CT) were significantly overestimated compared with those of MRI-based CTVHR (CTVHR-MR). Mean values of dice similarity coefficient (DSC), Hausdorff distance (HD), and centroid distance (AV) of CTVHR were 0.82 cm, 0.96 cm, and 0.35 cm, respectively. Dose values of CTVHR-CT were significantly lower compared with those of CTVHR-MR. Concerning OARs, geometrical and dosimetric values on CT were comparable to those on MRI. Conclusions: The delineated ranges of CTVHR were significantly over-estimated on CT compared with MRI. D-98 and D-90 of CTVHR-CT were lower than CTVHR-MR. DSC and AV of CTVHR and CTVIR were similar to each other; however, there was a difference in terms of HD. CT images regarding pre-BT MR images for delineating were not enough and MRI fusion is still required.
Clinical Feasibility of Soft-Tissue Matching in Cone-Beam Computed Tomography (CBCT) Guidance for Liver Cancer Patients

Abstract:The feasibility of soft-tissue matching in cone-beam computed tomography (CBCT) guidance for liver cancer patients is investigated and its equivalence to fiducial-marker matching on determining patient setup corrections has been studied. On the Elekta Synergy machine, daily CBCT volumetric images were acquired after setup with silver rings (SRs) in liver. The SRs were served as fiducial markers in our study. The CBCT images without SRs in liver were then obtained by digitally removing the SRs in each projection image using photoshop CS4.Considering fiducial-marker matching as a standard, the patient setup errors were compared between two two matching methods. A total of 90 datasets of volumetric images for 10 patients were used in the comparison. Pearson coefficient of correlation for the setup errors was as follows: R2 = 0.718, 0.724, 0.785 in the LR, AP and SI directions, respectively. A Bland-Altman analysis showed no significant trends. The percentage of errors within a ±3 mm tolerance was respectively 95.56%, 95.83%, 91.11%. The p-value (paired permutation test) are respectively 0.1217, 0.919 and 0.7685 in the three cardinal directions and are all greater than 0.05. So, the null hypothesis cannot be refused, that is to say. Two methods have no significant difference. CBCT guidance using soft-tissue is an equivalent method for on-line image-guided radiotherapy for liver-cancer patients when compared to CBCT guidance using fiducial markers. It is feasible to implement CBCT image guidance using soft-tissue matching method clinically.
Patterns of stability and change in health use among elderly people. Do service systems leave an imprint on behavior?

Abstract:Stability and change in patterns of health service use over a 3-year period were determined for a sample of elderly people in an urban area who claimed one of five types of health service provider as a primary source of health care--a hospital, a private physician, a network model health maintenance organization (HMO), a hospital-based group practice program (G-HMO), or a preferred provider organization (PPO). Despite certain differences in use rates for individual services, the total volume of ambulatory service use was equivalent for all five groups as was the relative rank order of use of specific ambulatory services for four of the five groups. People who claimed a hospital as their primary care source had the most unique use patterns over a full range of health care services, characterized by extremely low rates of physician visits and the highest rates of visits to hospital outpatient clinics across three time periods. G-HMO members used health-related services more frequently than did all others. PPO members, at baseline, had lower rates of total and mean hospital days than other source group members except hospital users. People who changed principal source of care during the study period were most likely to report a hospital as their care source initially. Although there is much consistency in hospital and ambulatory use across groups, the persistence of certain use patterns for members of some groups suggests that health care systems can leave an imprint on the health service use of people for whom they provide regular care.
Accuracy study of different registration methods for cone beam CT and planning CT in image-guided radiation therapy

Abstract:Objective To evaluate the accuracy of image registration based on bony structure (RBS) and grey-scale (RGS) in positioning correction of radiation treatment,and their reliability in clinical application.Methods Setup errors of anthropomorphic phantom (chest& abdomen,head& neck) were simulated with x-,y-,z-directions.CBCT images were acquired for each simulation and registered with planning CT.using bony structure and grey-scale registration separately.Geometry accuracy of all registration were then obtained and analyzed.Results The errors of RBS and RGS in x-,y-,z-directions were (-0.65 ±0.22) mm and (-0.70±0.17) mm (P=0.00),(1.02 ±0.27) mm and (0.90±0.20) mm (P =0.00),(1.46 ± 0.53) mm and (1.47 ± 0.47) mm (P =0.54) for head& neck positioning; with (0.82±0.33) mm and (0.79±0.18) mm (P=0.03),(2.45±1.17) mm and (1.61 ±0.84) mm (P =0.00),(1.44 ± 3.25) mm and (0.19 ± 1.11) mm (P =0.00) for chest& abdomen positioning.Conclusions RGS is more accurate and stable than RBS.The accuracy of image registration is a little better for head& neck than that for chest& abdomen.The algorithms of image registration used in clinical application needs to be tested independently and the systematic error needs to be corrected before applying in different treatment techniques according to their accuracy requirement.

Abstract:Purpose: To assess the fiducial motion in abdominal stereotactic body radiotherapy (SBRT) using the cone-beam computed tomography (CBCT) projections acquired for pre-treatment patient set-up. Materials and Methods: Pre-treatment CBCT projections and anterior-posterior (AP) and lateral (LAT) pair of fluoroscopic sequences of 7 pancreatic and 6 liver SBRT patients with implanted fiducials were analyzed for 49 treatment fractions retrospectively. A tracking algorithm based on template matching and sequential stereo triangulation algorithms was used to track the fiducials in the CBCT projections and the fluoro sequence pairs. We predicted the clinical couch adjustment from CBCT tracking and compared it with the clinical couch decision made during the patient's treatment. Results: In 3D coordinate, the fiducial motion ranges for pancreas cases were 9.90+/-3.52 mm, 10.65+/-5.91 mm, and 10.74+/-6.24 mm for CBCT, AP, and LAT fluoro, respectively, while in the liver, they were 13.93+/-3.39 mm, 11.17+/-3.75 mm, and 11.52+/-4.33 mm, respectively. Prediction of couch adjustment in LAT, SI, and AP coordinates from CBCT tracking agrees with the actual clinical couch correction within 0.92+/-0.74 mm, 1.37+/-1.26 mm, and 0.68+/-0.56 mm for pancreas cases and within 1.12+/-0.96 mm, 1.15+/-0.92 mm and 0.90+/-0.86 mm for liver cases, respectively. Conclusion: Tracking pre-treatment CBCT projections using template matching and sequential stereo triangulation is suitable for assessing fiducial motion and adjusting the patient setup for abdominal SBRT. CBCT can be used for motion modeling, potentially eliminating the need for additional fluoroscopic pair acquisition and thus reducing the imaging dose to the patient and the total treatment time.

Abstract:Postmastectomy radiation therapy (PMRT) is often prescribed for patients with advanced breast cancer and has been shown to reduce local-regional recurrence and improve survival. The radiotherapy target is the chest wall, axillary lymph nodes, and the supraclavicular fossa. Patients are normally positioned for therapy with arms extended above the head in order to minimize both dose to the uninvolved arm tissue, and perturbations to the delivered dose distribution. The shoulder is the most mobile joint in the human body and difficult to position with a high level of reproducibility, thus the target area can be substantially affected by shoulder and arm position. Accordingly, this treatment is a good candidate for the regular use of imaging for patient position verification. Many linear accelerators today are equipped with kilovoltage X-ray imaging systems capable of cone-beam CT imaging, and CBCT is potentially an excellent tool for image-guided radiotherapy (IGRT) of the breast and chest wall.

Abstract:Background: Stereotactic body radiation therapy is an advanced technique, which delivers ablative doses to lung lesions. Target verification is done either by orthogonal x-rays or cone beam CT. This study was undertaken to compare these two verification methods. Aim: To evaluate the efficacy of ExacTrac and Cone Beam Computed Tomography (CBCT) for target repositioning while delivering Stereotactic Body Radiation Therapy (SBRT) for lung lesions and derive the population-based margin. Materials and methods: All patients who had undergone SBRT for lung lesions from February to September 2009 were involved. Patients were immobilized using the BodyFix double vacuum immobilization system, indexed to the computed tomography (CT) simulator and treatment machine. Four-dimensional (3-D) scan was done to generate internal target volume (ITV) and a free breathing CT scan for planning was done on the BrainLab iPlan 4.1 software. During treatment, patient's position was verified using ExacTrac and CBCT. The resulting vertical, lateral, and longitudinal shifts were noted. The random and systematic error were calculated and the margin recipe derived using the Van Herk formula. Results: Sixteen patients had undergone SBRT for lung tumors from February to September 2009. Data from eight patients who had undergone 34 sessions of SBRT was analyzed. The systematic error for lateral, longitudinal, and vertical shifts for ExacTrac and CBCT were 3.68, 4.27, 3.5 mm and 0.53, 0.38, 0.70 mm, respectively. The random error were 1.10, 1.51, 1.96 mm and 0.32, 0.81, 0.59 mm. The lateral, longitudinal and vertical Van Herk margin recipe for ExacTrac were 9.98, 11.72, 10.18 mm, respectively, and for CBCT was 2.17, 1.53,1.55 mm. Conclusions: The systematic and random errors for CBCT were significantly lesser as compared to the errors with Exactrac.
Impact of different anatomical landmarks on registration in imaging-guided radiation for lung cancer

Abstract:Objective To evaluate the impact of different anatomical landmarks on registration in imaging-guided radiation (IGRT) for lung cancer. Methods For 20 patients with non-small cell lung cancer receiving stereotactic body radiotherapy (SBRT) in Fudan University Cancer Hospital, 100 frames of kilo-voltage cone-beam computed tomography scanning were evaluated in this study. The spine, carina and tumor were selected as landmarks for registration, respectively. Results of registration using different landmarks were documented and compared. Results The average set-up errors in the left-right, superiorinferior and anterior-posterior directions were -0. 08 cm ±0. 32 cm, -0. 16 cm ±0. 45 cm and 0. 06 cm ±0. 23 cm with the spine for registration;0. 06 cm ±0. 34 cm, -0. 13 cm ±0. 45 cm and -0. 02 cm±0.23 cm with the carina;and -0. 17 cm ±0.25 cm, 0.03 cm ±0.47 cm and 0. 15 cm ±0.38 cm with tumor. The registration results between using the carina and tumor as landmarks were statistically significant different (q=4.61, P=0. 002 ;q = 2. 23 , P=0. 118;q=3.44, P=0. 017). The registration results were equal when using the spine and tumor as landmarks ( q = 1.85, P = 0. 195; q = 2. 54, P = 0. 075; q = 1.89,P=0. 185), as well as using the carina and tumor as landmarks (q=2.76, P=0. 054;q=0.31, P=0. 826 ;q = 1.55, P = 0. 276). Conclusions For early stage lung cancer, the spine and tumor can be used equally as registration landmarks in imaging-guided SBRT. The carina is not suggested for its poor reproducible position.
Positioning accuracy for lung stereotactic body radiotherapy patients determined by on-treatment cone-beam CT imaging

Abstract:Stereotactic body radiotherapy for early stage non-small cell lung cancer is an emerging treatment option in the UK. Since relatively few high-dose ablative fractions are delivered to a small target volume, the consequences of a geometric miss are potentially severe. This paper presents the results of treatment delivery set-up data collected using Elekta Synergy (Elekta, Crawley, UK) cone-beam CT imaging for 17 patients immobilised using the Bodyfix system (Medical Intelligence, Schwabmuenchen, Germany). Images were acquired on the linear accelerator at initial patient treatment set-up, following any position correction adjustments, and post-treatment. These were matched to the localisation CT scan using the Elekta XVI software. In total, 71 fractions were analysed for patient set-up errors. The mean vector error at initial set-up was calculated as 5.3 ± 2.7 mm, which was significantly reduced to 1.4 ± 0.7 mm following image guided correction. Post-treatment the corresponding value was 2.1 ± 1.2 mm. The use of the Bodyfix abdominal compression plate on 5 patients to reduce the range of tumour excursion during respiration produced mean longitudinal set-up corrections of -4.4 ± 4.5 mm compared with -0.7 ± 2.6 mm without compression for the remaining 12 patients. The use of abdominal compression led to a greater variation in set-up errors and a shift in the mean value.
The use of IGRT technique to measure the target position errors in SBRT treatment for lung cancer

Abstract:Objective:To discuss the significance of image‐guided radiation therapy (IGRT ) technique in measuring and correcting of target position errors in stereotactic body radiotherapy (SBRT ) treatment for lung cancer .Methods :Totally 14 cases underwent SBRT treatment were enrolled in this study .Patients were scanned by kilo‐voltage cone beam CT (KV‐CBCT ) before and after treatment ,and online registered to the planning CT images ;the target position error was corrected before treamtnet ,and the CBCT was scanned and registered again after treatment .The target position errors were recorded before and after treatment .The CTV‐PTV margin was calculated by the formula 2 .5Σ+0 .7σ.Results:The maximum pre‐treatment error in right‐left ,head‐feet and back‐belly directions were 10 mm ,16 .9 mm and 8 .7 mm ,respectively .The mean value of target position errors in right‐left ,head‐feet and back‐belly directions were (2 .4 ± 2 .8) mm ,(3 .4 ± 4 .6) mm and (2 .9± 3 .4)mm ,respectively .The maximum post‐treatment target position errors in right‐left ,head‐feet and back‐belly directions after online correction were 7 .6 mm ,4 .6 mm ,and 7 .0 mm ,respectively ,and the mean values were (2 .1 ± 2 .0)mm , (1 .5 ± 1 .6) mm and (1 .7 ± 2 .0) mm ,respectively .Without the usage of image‐guide ,the PTV margins in right‐left ,head‐feet and back‐belly directions were 7 .96 mm ,11 .72 mm and 9 .63 mm ,respectively ;with the usage of image‐guide ,the PTV margins in right‐left ,head‐feet and back‐belly directions were 6 .65 mm ,4 .87 mm and 5 .65 mm ,respectively .Conclusions :The IGRT technology can online correct the pre‐treatment target position errors and determine the target position removing during treatment .It can improve the treatment accuracy ,and contribute to set the PT V margins .
Analysis of the Setup Errors of Medical Image Registration-Based Cone-Beam Ct for Lung Cancer

Abstract:PURPOSE:This study aimed to investigate the feasibility of efficiently using a rigid image registration (RIR) algorithm or a deformable image registration (DIR) algorithm to match medical images and evaluate the impact of setup errors on intensity modulated radiation therapy of lung cancer patients.METHODS:Ten lung cancer patients were chosen randomly each day and were subjected to image-guided radiotherapy. The clinical registration between cone-beam computed tomography (CBCT) images and treatment planning system CT images was performed by applying both RIR and DIR; the clinical registration was evaluated on the basis of the contour index, including dice similarity coefficient, sensitivity, and positive predictive value; the optimal scheme of image registration was selected to ensure that the actual irradiation isocenter was consistent with the treatment planning isocenter. In each patient, the translational errors in the right-left (x), superior-inferior (y), and anterior-posterior (z) directions and the rotational errors in the u, υ, and w directions formed by the x, y, and z directions were calculated and analyzed daily in the whole course of treatment; margins were calculated according to this equation: M = 2.5∑+ 0.7δ.RESULTS:The tumors and the surrounding soft tissues of the patients are shown more clearly in the CBCT images than in the CT images. DIR can be applied more efficiently than RIR to determine the morphological and positional changes in the organs shown in the images with the same or different modalities in the different period. The setup errors in translation in the x, y and z axes were 0.05±0.16, 0.09±0.32 and -0.02±0.13 cm, respectively; by contrast, the setup errors in rotation in u, υ and w directions were (0.41±0.64)°, (-0.08±0.57)° and (-0.03±0.62)°, respectively. The setup errors in the x, y and z axes of the patients indicated that the margins expansions were 0.82, 1.15 and 0.72 cm, respectively.CONCLUSION:CBCT with DIR can measure and correct the setup errors online; as a result, setup errors in lung cancer treatments can be significantly reduced and the accuracy of radiotherapy can be enhanced.
Reproducibility Evaluation of the Manual Alignment Method for Kilovoltage Cone-beam CT Guided Radiotherapy

Abstract:Objective To evaluate reproducibility of the manual alignment method for kilovohage cone- beam CT guided lung cancer radiotherapy.Methods sixteen non-small cell lung cancer patients entered this study.Weekly on-line KVCBCT guided set-up error correction was performed.A total of ninety-six pretreat- ment KVCBCT images were available for analysis.The landmarks of the manual alignment included the lung top and spine around the isocenter.Images were aligned in three dimensions,and the LR(left-right),SI(superior -inferior),and AP(anterior-posterior)components of translation of the target isocenter were documented and analyzed for each registration.Intra-observer variability was investigated:at least one week after treatment all KVCBCT studies were reexamined by the same physician.Additionally,all KVCBCT studies were examined by a second physician,a radiation therapist and inter-observer variability was tested.Results The intra-observer variability of the manual registration method was investigated.All CBCT studies were reevaluated by the same phy- sician.Compared with the original results,the percentage of the more than 3 mm differences in LR,SI,and AP directions are 0,13% and 6%,respectively.The percentage of the more than 3 mm differences in LR,SI,and AP directions between two physicians are 11%,19% and 14%.The difference of the registration results between radia- tion therapist and physician in LR,SI,and AP directions are 16%,27% and 27%.Conclusions The repro- ducibility of the manual alignment method was poor,especially in the inter-observer evaluation.The optimal a- lignment method for KVCBCT guided lung cancer radiation needs further research.
Comparison of Helical, Maximum Intensity Projection (MIP), and Averaged Intensity (AI) 4D CT Imaging for Stereotactic Body Radiation Therapy (SBRT) Planning in Lung Cancer

Abstract:BACKGROUND AND PURPOSE:To compare helical, MIP and AI 4D CT imaging, for the purpose of determining the best CT-based volume definition method for encompassing the mobile gross tumor volume (mGTV) within the planning target volume (PTV) for stereotactic body radiation therapy (SBRT) in stage I lung cancer.MATERIALS AND METHODS:Twenty patients with medically inoperable peripheral stage I lung cancer were planned for SBRT. Free-breathing helical and 4D image datasets were obtained for each patient. Two composite images, the MIP and AI, were automatically generated from the 4D image datasets. The mGTV contours were delineated for the MIP, AI and helical image datasets for each patient. The volume for each was calculated and compared using analysis of variance and the Wilcoxon rank test. A spatial analysis for comparing center of mass (COM) (i.e. isocenter) coordinates for each imaging method was also performed using multivariate analysis of variance.RESULTS:The MIP-defined mGTVs were significantly larger than both the helical- (p=0.001) and AI-defined mGTVs (p=0.012). A comparison of COM coordinates demonstrated no significant spatial difference in the x-, y-, and z-coordinates for each tumor as determined by helical, MIP, or AI imaging methods.CONCLUSIONS:In order to incorporate the extent of tumor motion from breathing during SBRT, MIP is superior to either helical or AI images for defining the mGTV. The spatial isocenter coordinates for each tumor were not altered significantly by the imaging methods.

Abstract:Purpose : To use our CBCT shift results to determine how much of the CTV to PTV margin is necessary to compensate for the daily setup uncertainty inherent in the treatment of head and neck cancer patients. Method and Materials : To accommodate the daily setup uncertainly, a 3–6 mm uniform margin is used for head and neck IMRT patients to expand CTV to PTV. In order to verify patient setup accuracy, CBCT is taken prior to the treatment. Our protocol calls for CBCT for the first five consecutive fractions, followed by two CBCT every week. Each CBCT is registered to the planning CT to determine the corresponding translational shift. The registration is based on the region‐of‐interest—the rigid bones near the tumor. On the days no CBCT is taken, the average of the previous three calculated translational shifts is used instead. No rotational correction is considered. Results : A total of 22 head and neck IMRT patients were studied with a Siemens Artiste linear accelerator using an MVision CBCT scanner. Each patient had approximately 14 sets of CBCT images taken over the entire course of treatment. The average interfraction shift was 0.6±1.6, 1.4±1.5, 0.1±1.6 mm in the anterior‐posterior, left‐right, and superior‐inferior dimensions, respectively. These data indicate that in the absence of CBCT image guided radiotherapy, a CTV to PTV margin of 2.5, 4.7 and 1.3 mm is needed in the anterior‐posterior, left‐right, and superior‐inferior dimensions, respectively. Conclusion : Our results have shown that asymmetric margins of 2.5, 4.7 and 1.3 mm can be used for CTV to PTV expansion for head and neck cancer radiotherapy if no IGRT presented. Our CBCT protocol will allow a reduction in the CTV to PTV margin. Further investigation needs to be done to find out the specific CTV to PTV margin with different CBCT frequency.

Abstract:Purpose: The intent of spinal SBRT is to maximize the biological effective dose and improve local control, while sparing the adjacent spinal cord. We report on the spinal SBRT positional accuracy for multiple, consecutive vertebrae in a single course, using the Elekta Synergy-S and BodyFIX immobilization systems. Methods: After initial patient adjustment, verification cone-beam CT (CBCT) images were acquired before, during and after treatment. These images were used to assess immobilization and correct any misalignment exceeding 1 mm or 1°, in all six degrees-of-freedom using the HexaPOD robotic couch. This analysis is based on 415 verification images from 67 consecutive courses of treatment. These treatment courses comprised 25 single thoracic vertebrae, 16 multiple thoracic vertebrae, 20 single lumbar vertebrae and 6 multiple lumbar vertebrae. Results: The absolute intra-fraction motion averaged over all directions (±std dev.) for the T-single, T-multiple, L-Single and L-Multiple was 0.54 (±0.73) mm, 0.54 (±0.88) mm, 0.36 (±0.57) mm, and 0.47 (±0.63) mm respectively. The percentage that exceeded the 1.5 mm planning margin was 3.8%, 4.0%, 1.0% and 0.85% respectively. T-spine treatments were out-of-tolerance more frequently than the L-spine. There was a statistically significant difference between single and multiple lumbar treatments (unpaired t-test, p<0.01), but this was not clinically significant as 99% were within our 1.5 mm margin. Conclusions: Near-rigid immobilization with the acquisition of intra-fraction CBCT images and the correction of misalignments in all six degrees-of-freedom provides the necessary precision to safely perform SBRT of consecutive spinal metastases within one course of treatment.
Image Registration and Target Volume Margins in Cone-Beam Computed Tomography-Guided Intensity-Modulated Radiotherapy for Prostate Cancer

Abstract:Objective To analyze the data from intensity-modulated radiotherapy ( IMRT) for prostate cancer guided by kilovoltage cone-beam computed tomography (CBCT), and to provide a clinical basis for selecting the optimal image registration method and reasonable target volume margins.Methods A total of 16 patients with prostate cancer who received radical IMRT were enrolled, and CBCT for online position verification was performed 214 times.The images were obtained after conventional skin marking and laser alignment, and automatic registration, bone registration, soft tissue registration, and manual registration were performed for CBCT images and planned CT images.The differences between these four registration methods were evaluated, and the margins for extending clinical target volume into planning target volume (PTV) were calculated.Results The setup errors in left-right, anterior-posterior, and cranial-caudal directions for automatic registration, bone registration, soft tissue registration, and manual registration were-0.6±2.8 mm/-0.6±4.5 mm/-0.6±3.8 mm,-0.7±2.7 mm/-0.9±4.5 mm/-0.8±4.1 mm,-0.8± 2.6 mm/-0.3±4.4 mm/-1.1±4.0 mm, and-0.6±2.9 mm/-0.7±5.1 mm/-0.9±3.9 mm, respectively. There were no significant differences between the four registration methods.The margins for extension in the left-right, anterior-posterior, and cranial-caudal directions were calculated as 4.7 mm, 5.2 mm, and 6.5 mm, respectively.Conclusions With a comprehensive consideration of various factors, a default setting of automatic registration and manual fine adjustment is appropriate for CBCT-guided radiotherapy for prostate cancer.The margins for extension in the left-right, anterior-posterior, and cranial-caudal directions are 4.7 mm, 5.2 mm, and 6.5 mm, respectively.

Abstract:Objective To investigate the effect of the correction of the position error of the abdominal tumor by cone beam scanning. Methods Six patients with abdominal tumor were selected with double-blind method, and electronical portal imaing device was used to collect the information of the patient dosimetry during radiotherapy.EDOSE tool was applied to 3D dose reconstruction so as to execute real-time 3D dose verification of the treatment plan. Each patient had 5 sets of data acquired when image guidance existed and another 5 sets when image guidance was absent. The treatment effects with or without image guidance were compared.Results The overall γ passing rate with cone beam CT image guidance gained no advantages over that without image guidance, though the setup error was corrected to some extent. Image guidance contributed nothing to increasing target area coverage while decreasing exposure dose to normal tissue. Conclusion Setup error correction by the image guidance based on bone marker has no significant effect on improving 3D dose verification and dose accuracy during the radiotherapy of the abdominal tumor patient.
Online Kilovoltage Cone-Beam Ct Guided Lung Cancer Radiation: Initial Clinical Experiences From Shanghai

Abstract:To introduce the initial clinical experiences of online Kilovoltage Cone-beam CT (KVCBCT) guided lung cancer radiation in our department. From March 2007 to January 2008, 30 patients with pathologically confirmed lung cancer were included in this study. After patients were positioned using their skin marks, KVCBCT scans were performed before treatment and aligned to planning CT scans using the self-developed semi-automatic alignment method. Treatment table were repositioned according to the alignment results. A second KVCBCT scan was acquired immediately after the table correction. A total of 308 KVCBCT images from 154 fractions were analyzed. The reproducibility of the semi-automatic image alignment method, workflow of the online correction, translational and rotational setup error in three dimensional conditions and residual setup error after online correction were evaluated. The reproducibility of the semi-automatic alignment method was evaluated. The alignment difference was smaller in intraobserver evaluation than in interobserver. The reproducibility of different physicians was better than the physician and radiation therapist in anterior-posterior (AP) and left-right (LR) directions. In superior-inferior (SI) direction, the percentage of alignment difference larger than 3 mm between different physicians, the physician and radiation therapist were both near 15%. The online correction protocol adds 7.6 min to the total treatment time and the workflow proved to be feasible in clinical practice. The total systematic error (∑) before online correction was 3.5 mm, 3.5 mm and 3.0 mm, the overall random error (σ) was 2.7 mm, 2.9 mm and 3.0 mm in the LR, SI, and AP direction, respectively. After online correction, the total systematic error (∑) was 0.7 mm, 0.7 mm and 0.6 mm, the overall random error (σ) was 1.2 mm, 1.5 mm and 1.3 mm in three directions. The average rotational setup error (± 1 SD) was -0.3 ± 1.5°, 0.2 ± 0.9 ° and 0.3 ± 0.8 ° in pitch, roll and yaw, respectively. According to the formalisms suggested by van Herk et al, 3mm can apply to the margin of clinical target volume (CTV) to PTV, when using the online correction. The KVCBCT guided radiation system can be applied successfully in lung cancer patients with reproducible image alignment method. With the help of the system, we have an opportunity of new realization for patient setup error, including both translational and rotational. We can improve patient setup accuracy and reduce the CTV to PTV margin from 11 mm to 3 mm with limited workload.
Evaluating AI-generated CBCT-based synthetic CT images for target delineation in palliative treatments of pelvic bone metastasis at conventional C-arm linacs

Abstract:Purpose One-table treatments with treatment imaging, preparation and delivery occurring at one treatment couch, could increase patients' comfort and throughput for palliative treatments. On regular C-arm linacs, however, cone-beam CT (CBCT) imaging quality is currently insufficient. Therefore, our goal was to assess the suitability of AI-generated CBCT based synthetic CT (sCT) images for target delineation and treatment planning for palliative radiotherapy. Materials and Methods CBCTs and planning CT-scans of 22 female patients with pelvic bone metastasis were included. For each CBCT, a corresponding sCT image was generated by a deep learning model in ADMIRE 3.38.0. Radiation oncologists delineated 23 target volumes (TV) on the sCTs (TVsCT) and scored their delineation confidence. The delineations were transferred to planning CTs and manually adjusted if needed to yield gold standard target volumes (TVclin). TVsCT were geometrically compared to TVclin using Dice coefficient (DC) and Hausdorff Distance (HD). The dosimetric impact of TVsCT inaccuracies was evaluated for VMAT plans with different PTV margins. Results Radiation oncologists scored the sCT quality as sufficient for 13/23 TVsCT (median: DC=0.9, HD=11 mm) and insufficient for 10/23 TVsCT (median: DC=0.7, HD=34 mm). For the sufficient category, remaining inaccuracies could be compensated by +1 to +4 mm additional margin to achieve coverage of V95%>95% and V95%>98%, respectively in 12/13 TVsCT. Conclusion The evaluated sCT quality allowed for accurate delineation for most targets. sCTs with insufficient quality could be identified accurately upfront. A moderate PTV margin expansion could address remaining delineation inaccuracies. Therefore, these findings support further exploration of CBCT based one-table treatments on C-arm linacs.
