Application of Intensity-Modulated Radiotherapy in Radical Radiotherapy of Cervical Cancer

Abstract:OBJECTIVE:To review the application of intensity modulated radiotherapy (IMRT) in radical radiotherapy of local advanced cervical cancer. METHODS:The PubMed database from Jan 2000 to Oct 2009 was filtered by using the key words,cervical cancer and IMRT. Review criteria were reducing toxicity of intestine and blood,target contouring,and application in late course boost irradiation of IMRT for radical radiotherapy of cervical cancer. Twenty six studies were selected into the review. RESULTS:IMRT for pelvic and pelvic extended fields has reduced the volume of intestine,bone,rectum and kidney irradiated in high dose area,and reduced the incidence of severe toxicity. Positional changes of the body and cervix of uterus results from the filling of rectum and bladder and the changes of tumor volume. Weekly simulation and planning design can reduce the volume of organ at risk irradiated in PTV. The combination of IMRT with brachytherapy was recommended in late course boost radiation for radical radiotherapy. CONCLUSIONS:The signification of IMRT in the radical radiotherapy for cervical cancer is very important. The clinical outcome of IMRT needs future investigation.
