Comparison of the Clinical Efficacy of Volumetric-Modulated Arc Therapy and Fixed-field Intensity-modulated Radiotherapy Concurrent with Chemotherapy for Cervical Cancer★

Abstract:Objective To compare the clinical acute toxicity reaction and short-term efficacy between volumetric-modulated arc therapy (VMAT) and fixed-field intensity-modulated radiotherapy (f-IMRT) for cervical cancer in radical radiotherapy concurrent chemotherapy. Methods Eighty-three patients treated with VMAT or f-IMRT in radical irradiation concurrent with chemotherapy for cervical cancer were selected in our study. Forty-one of them were treated by VMAT, and the others were treated by f-IMRT. The clinical complications and short-term efficacy were compared between the two groups. Results After treatment, no significant difference was found in the complete remission rate and effective rate between two groups (P>0.05). The local control rates in half a year were respectively 100% and 94.9% (P=0.49) in VMAT and f-IMRT group. The incidence of digestive system acute grade Ⅰ~Ⅱ complications for VMAT and f-IMRT group were 46.3% and 64.3% (P=0.04) respectively. There were no differences in the incidences of urinary and blood system complications. Conclusion VMAT was superior to IMRT in the clinical acute toxicity reaction and short-term efficacy during the radical radiotherapy concurrent chemotherapy for cervical can-cer. VMAT may be the better choice for external beam radiotherapy in cervical cancer.
