Application of contour-based optimization technique in the intensity-modulated radiation therapy for breast carcinoma

Abstract:Objective Radiation therapy plays a major role in the management of breast carcinoma.The intensity-modulated radiation therapy(IMRT) provides more safety for the normal tissues and organs surrounding the clinical target volume.This paper aimed to study the forward optimization technique in IMRT for breast carcinoma.Methods We included in this retrospective study 152 patients with breast carcinoma treated by IMRT.All treatment plans were designed with the contour-based optimization technique and performed according to the patient′s anatomy,with the apertures defined according to certain isodose-lines achieved with an open field.Results The physicians were satisfied with the dose distributions and dose-volume histograms of all the patients.The dose of 95%(D95) of planning gross tumor volume(PGTV) was ≥60 Gy for 38 patients(38/45),that of gross tumor volume(GTV) ≥60 Gy for 45 patients(45/45);the V95s of clinical target volume(CTV) was ≥50 Gy for 125 patients(125/152),and that of planning target volume(PTV) ≥50 Gy for 60 patients(60/152).The D20 of the affected lung was <20 Gy for 100 patients(100/152).The mean dose of uninjured breasts was 5.2 Gy.The mean number of segments was 32(range 18-45),with a median of 26.The mean total monitor unit(MU) was 480.8(range 380.5-750.2),with a median of 455.4.The mean treatment time was 17.2 min(range 12.9-28.3),with a median of 16.5 min for the 88 patients treated after December 2007.Conclusion IMRT is an effective new option for breast carcinoma.Ideal dose distributions can be obtained using rational contour-based optimization technique.
