Simultaneous evaluation of coronary artery in-stent restenosis by 64-slice spiral CT and coronary angiograpy

Abstract:Background: Coronary arteriongraphy reexamination has significant value in evaluating restenosis after stent implantation. However, it is limited by mini-invasive technique and high price. Objective: Taking selective coronary angiography as a "golden standard", to investigate clinical value of coronary artery in-stent restenosis by 64-slice spiral CT. Design, time and setting: A retrospective case analysis was completed in Zhongda Hospital Affiliated to Southeast University from March 2007 to July 2008. Participants: A total of 22 patients with coronary heart disease were examined simultaneously by 64-slice spiral CT and selective coronary angiography after stent implantation. There were 12 male and 10 female with the average age of (68.0 ± 9.6). Methods: 38 stents in 22 coronary heart disease patients were evaluated by 64-slice spiral CT to evaluate restenosis after stent implantation, and compared the results with selective coronary angiography. Main outcome measures: The restenosis of coronary artery following stent implantation was evaluated by image of 64-slice spiral CT and selective coronary angiography. Results: 64-slice spiral CT could clearly show the left main coronary artery and branches. The results of spiral CT in 32 stents of 38 stents showed patency, which were consistent with that of coronary arteriongraphy. Other 6 stents restenosis were showed by 64-slice CT, while only 4 stents appeared restenosis in selective coronary angiography. The sensitivity and specificity of 64-slice spiral CT in the diagnosis of stent restenosis were 100% and 94.18%, respectively. Conclusion: 64-slice spiral CT has good accuracy in evaluating restenosis after stent implantation, which can differentiate stent patency and occlusion from restenosis.
