Abstract:Historically, temporal bone fractures have been classified into three fundamental types: longitudinal, transverse, and mixed fractures. This classification system originated from simulated impact studies on cadaver skulls in the 1940s that provided simple descriptions and frequencies of occurrence of temporal bone fractures. The nature of trauma to the temporal bone has changed because of the increased survival of head injury victims with more complex fracture patterns; hence, many temporal bone fractures can not be characterized by the traditional nomenclature. The traditional categorization system has also been found to be less clinically correlated to concomitant intracranial complications and functional impairment. Kelly and Tami have proposed a more contemporary and clinically relevant categorization of temporal bone fractures into otic capsule—sparing and otic capsule— disrupting fractures (1). Dahiya et al. (2) found that patients with otic capsule—violating fractures were more likely to experience facial paralysis, cerebrospinal fluid leak, and profound hearing loss and were also more likely to experience intracranial complications, including epidural and subarachnoid hemorrhage. The classification system emphasizing the violation or sparing of the otic capsule offers useful parameters for evaluating whether surgical intervention or specific management is required for injuries of this kind. Yanagihara et al. (3) have developed a sophisticated descriptive classification system for surgical purposes, depicting detailed fracture sites that are more anatomically relevant during surgery. However, a classic transverse fracture with cochlea involvement, as in the case presented here, does not fit any of the criteria of Yanagihara et al. (3) because the intratemporal facial nerve was spared. This patient experienced a direct impact to the right side of the face in a motor vehicle accident; no major symptoms were reported except for bloody otorrhea and immediate hearing loss. High-resolution computed tomography is useful in evaluating the course, severity, and location of temporal bone fracture. Images of 1.0-mm thick sections should be adequate for proper identification of a lesion site. In our case study, the high bone density uptake within the cochlear aqueduct suggests bone fragment generated by a shearing force (Fig. 1).
