Effect of build orientation on the fracture resistance and marginal quality of 3D-printed anatomic provisional crowns: An in-vitro study

Abstract:Introduction: Computer-aided design and computer-aided manufacturing (CAD/CAM) technologies have been increasingly used to fabricate provisional restorations in recent years. This study assessed how build orientation influences the fracture resistance and marginal quality of 3D-printed crowns compared with milled provisional crowns. Methods: The test group included 3D-printed crowns (Freeprint temp Shade A2, Detax, Ettlingen, Germany), which were further subdivided based on print orientation (0°, 45°, and 90°; n = 10 for each subgroup). The control group (n = 10) included milled crowns (Coratemp, White Peaks, Germany) with the same design as those of the test group. The margin quality of each crown was assessed at 60 × magnification using a digital stereomicroscope. A load-to-fracture test was performed by applying a force at a rate of 2 mm/min to assess fracture resistance. One sample from each subgroup was also subjected to scanning electron microscope (SEM) analysis. Results: The milled group exhibited the highest fracture resistance and marginal quality. Within the printed subgroups, the 0° group showed the best mean marginal quality, whereas the 90° group showed the lowest mean marginal quality (p < 0.05). Within the test groups, the 90° group had the highest mean fracture resistance (p < 0.05). In the SEM analysis, the milled group exhibited the most homogenous boundaries, whereas among the 3D-printed subgroups, the samples printed at 0° had the best margin quality. Conclusion: The manufacturing method significantly influences the marginal quality and fracture resistance. Milled crowns demonstrated superior marginal quality and fracture resistance compared to those of 3D printed crowns. Furthermore, the print orientation of 0° led to the best marginal quality, whereas printing at 90° led to the highest fracture resistance.

Abstract:Additive manufacturing (AM), also known as additive printing, is increasing its importance in every field of dentistry. There are different methods of AM, one of these being digital light processing (DLP). Several factors can influence the accuracy of printed crowns, and one of them is the building or printing orientation. Therefore, the aim of this study is to evaluate the trueness and precision of temporary crowns produced via DLP with different printing angles. An artificial molar prepared to receive a full crown was scanned. A total of 15 resin crowns were printed through DLP and divided into three groups considering 3 different printing angulations: 90°, 45° or a custom angulation. Trueness and precision were evaluated considering the Root Mean Square Error (RMSE). Regarding the internal fit, the 90° printing angulation showed a higher accuracy compared to the other angulations, whereas regarding the occlusal aspect, Custom angulation crowns were better. Custom Crowns showed a higher precision when considering the internal aspect, being equal to the 90° printing angulation regarding the occlusal aspect. 135° printing angulation value performed equal or worse precision and accuracy in all zones. In terms of trueness and precision, custom angulation crowns showed acceptable internal and occlusal results for 3D printed provisional crowns.

Abstract:Statement of problem Three-dimensional printing is a rapid and cost-effective method that eliminates material waste during the manufacture of indirect restorations. Parameters such as print angle significantly affect the physical properties of the printed material. Several manufacturers suggest using an angle of 45 degrees to perform printing; however, different angles have been used, and the optimum parameters are unclear. Purpose The purpose of this systematic review was to compare the 45-degree print angle with other protocols and evaluate the effect of print angle on the physical and mechanical material properties. Material and methods Two researchers independently searched for relevant studies published up to June 2023 in the PubMed/MEDLINE, Scopus, Web of Science, and Lilacs electronic databases. The population, intervention, comparison, outcome (PICO) question was: "Does the 45-degree print angle promote better physical characteristics for 3-dimensionally printed interim crowns?" Studies that compared the properties of the different print angles of interim crowns were analyzed. Results Ten in vitro studies were selected for qualitative analysis. When evaluating marginal and internal fits, most studies selected the 45-degree angle, resulting in less discrepancy. The results of flexural strength analysis showed a high level of heterogeneity. The 0-degree print angle yielded better results in 3 studies, followed by the 90-degree print angle in 2 studies. The 45-degree angle yielded the highest fracture resistance value in 1 study and similar results to those of the control group (milled PMMA) in another study. Conclusions The marginal and internal adaptation and resistance to flexure of printed interim crowns are satisfactory when a 45-degree print angle is used.

The Influence of Dental Virtualization, Restoration Types, and Placement Angles on the Trueness and Contact Space in 3D-Printed Crowns: A Comprehensive Exploration

Abstract:The current digital dentistry workflow has streamlined dental restoration production, but the effectiveness of digital virtual design and 3D printing for restorations still needs evaluation. This study explores the impact of model-free digital design and 3D-printing placement angles on restorations, including single crowns and long bridges produced with and without casts. The restorations are 3D printed using resin at placement angles of 0°, 60°, and 90°. Each group of samples was replicated ten times, resulting in a total of 120 restorations. The Root Mean Square Error (RMSE) value was used to evaluate the surface integrity of the restoration. In addition, the contact space, edge gap, and occlusal space of restorations produced by different processes were recorded. The results indicate that there was no significant difference in the RMSE value of the crown group (p > 0.05). Changing the bridge restoration angle from 0° to 90° resulted in RMSE values increasing by 2.02 times (without casts) and 2.39 times (with casts). Furthermore, the marginal gaps in the crown group were all less than 60 μm, indicating good adaptation. In contrast, the bridge group showed a significant increase in marginal gaps at higher placement angles (p > 0.05). Based on the findings, virtual fabrication without casts does not compromise the accuracy of dental restorations. When the position of the long bridge exceeds 60 degrees, the error will increase. Therefore, designs without casts and parallel placement result in higher accuracy for dental restorations.
Accuracy of the intaglio surface of 3D-printed hybrid resin-ceramic crowns, veneers and table-tops: An in vitro study

Abstract:OBJECTIVES: The present study aims to examine the influence of the build angle on the accuracy (trueness and precision) of 3D printed crowns, table-tops and veneers with a hybrid resin-ceramic material.METHODS: One crown, on table-top and one veneer were printed in five different build angles (0°, 30°, 45°, 60°, 90°) (n = 50) with the digital light processing (DLP) system (Varseo XS, Bego) using hybrid resin (Varseo Smile Crown<sup>plus</sup> A3, Bego). All printed restorations were scanned using the laboratory scanner (D2000, 3Shape) and matched onto the initial reference design in metrology software (Geomagic Control X, 3D Systems). The root mean square error (RMSE) was calculated between the scanned and reference data. The data was statistically analyzed using the Tukey multiple comparison test and Wilcoxon multiple comparison test.RESULTS: The crown group showed higher trueness at 30° (0.021 ± 0.002) and 45° (0.020 ± 0.002), and table-tops at 0° (0.015 ± 0.001) and 30° (0.014 ± 0.001) (p < 0.0001). Veneers demonstrated higher trueness at 30° (0.016 ± 0.002) (p < 0.0001). All three restoration types demonstrated the lowest trueness at a 90° build angle and portrayed deviations along the z axis. The veneer and table-top groups showed the lowest precision at 90° (veneers: 0.021 ± 0.008; table-tops: 0.013 ± 0.003). The crown group portrayed the lowest precision at 45° (0.017 ± 0.005) (p < 0.0001).CONCLUSION: The build angle of DLP-printed hybrid resin-ceramic restorations influences their accuracy.CLINICAL SIGNIFICANCE: Considering the build angle is important to achieve a better accuracy of 3D-printed resin-ceramic hybrid restorations. This may help predict or avoid the interference points between a restoration and a die and minimize the clinical adjustments.

Abstract:Objectives: To evaluate the impact of printer technology and print orientation on the accuracy of directly printed retainers. Materials and methods: Digital retainers were printed with two different printing technologies: digital light processing (DLP) and stereolithography (SLA), using two different orientations: 0° and 90°. After printing, the retainers (n = 40) were scanned using cone-beam computed tomography. The DICOM files were then converted into standard tessellation language (STL) files. Comparison of the printed retainers with a master file was done by superimposition using a three-dimensional (3D) best-fit tool in Geomagic software. A ±0.25 mm tolerance was set to detect differences between the superimposed files. Statistical analysis was conducted (Kruskal-Wallis and Wilcoxon-Mann-Whitney tests, with Bonferroni correction). Results: The lowest median average deviation was observed for the DLP horizontally printed models (median, [interquartile range (IQR)] = 0.01 mm, [-0.01, 0.02]) followed by the SLA horizontally printed retainers (median, [IQR] = 0.05 mm, [0.03, 0.07]). The highest median inside the tolerance levels ratio was observed for the horizontally SLA printed retainers (median, [IQR] = 78.9%, [74.4, 82.4%]) followed by the horizontally DLP printed retainers (median, [IQR] = 78.2%, [74.5, 80.7%]). Conclusions: Both technologies (DLP and SLA) showed 3D printed results compatible with orthodontic clinical needs. Printing orientation was more important than printer type regarding its accuracy. Additional studies are needed to evaluate the accuracy of direct printed appliances clinically.

Abstract:ABSTRACT Objectives To determine the impact of build orientation, increased layer thickness, and dental crowding on the trueness of three-dimensional (3D)–printed models, and to evaluate how these parameters affect the fit of thermoformed appliances. Materials and Methods Ninety-six dental models were printed horizontally and vertically on the building platform using different 3D-printing technologies: (1) a stereolithography (SLA) printer with layer thicknesses of 160 μm and 300 μm and (2) a digital light processing (DLP) printer with layer thicknesses of 100 μm and 200 μm. Each printed model was digitalized and superimposed on the corresponding source file using 3D rendering software, and deviations were quantified by the root mean square values. Subsequently, a total of 32 thermoformed appliances were fabricated on top of the most accurate 3D-printed models, and their fit was evaluated by digital superimposition and inspection by three blinded orthodontists. Paired t-tests were used to analyze the data. Results Significant differences (P < .05) between printing technologies used were identified for models printed horizontally, with the SLA system achieving better trueness, especially in crowded dentitions. No significant differences between technology were found when models were printed vertically. The highest values of deviation were recorded in appliances fabricated on top of DLP-printed models. The results of the qualitative evaluation indicated that appliances fabricated on top of SLA models outperformed the DLP-modeled appliances. Conclusions Three-dimensional printing with increased layer height seems to produce accurate working models for orthodontic applications.

Abstract:Genetic testing for predisposition cancer syndromes has been broadly implemented in the clinical setting in the last several years. Some of the most recognisable familial cancer syndromes for which genetic testing is available are hereditary non-polyposis colorectal cancer (HNPCC), familial adenomatous polyposis (FAP), and hereditary breast and ovarian syndrome (HBOS). Genetic testing in these individuals may have relevant medical and psychological implications for them and their families. Whether individuals at risk for hereditary colorectal syndromes such HNPCC or FAP have different motivations and concerns about genetic testing than individuals with HBOS is not known. The first studies addressing motivations for genetic testing for cancer predisposition were carried out in the mid 1990s.1–6 These studies were conducted not in people actually undergoing testing but in the general population or among first-degree relatives of individuals with breast, ovarian, or colon cancer. They revealed that willingness to undergo genetic testing was associated with higher perception of cancer risk or of being a gene carrier and that the most common reasons for their stated intent to pursue genetic testing were to learn about one’s children’s risks, be reassured regarding one’s own risk, and a wish to increase screening practices. Concerns about genetic testing included mistrust of test accuracy, worry about losing health insurance, and fear of emotional reactions if the test was positive. Despite the high uptake for genetic testing predicted by the above studies, rates of testing were lower than expected when it actually became available for families with HBOS or HNPCC syndromes.7,8 Reported barriers to genetic testing among individuals at risk for HBOS were concerns about losing health insurance, and the belief that test results might not be accurate.7 A low level of education and the presence of depression symptoms, especially among women, were found to be …

Abstract:This paper explores the implications of the use of System Dynamics to model dramatic works and examines the model used in Pamela Lee Hopkinsâ€™ â€œSimulating Hamlet in the classroom.â€ This paper addresses this issue from both a literary and a modelling perspective. It begins by discussing the use of System Dynamics modelling in literature within the framework established by Forrester. Two aspects of the model, motivation and evidence revelation, are then examined against evidence from the text, supported by historical information. Some difficulties inherent in modelling drama are highlighted and the paper concludes that the model does not adequately capture the complexity of the play because System Dynamics modelling is not an appropriate tool for literary analysis.

Abstract:Purpose To compare the dimensional accuracy of 3D-printed mandibular complete dentures with different build orientations. Material and Methods A mandibular complete denture was digitized as a virtual reference file. The reference file was 3D-printed at the 0 degrees, 45 degrees, and 90 degrees build orientations with a MultiJet 3D printer (Projet MJP 3600 Dental, 3D systems, Rock Hill, SC). A total of 27 complete dentures were 3D-printed with 9 samples for each orientation. All printed dentures were digitized and separated into teeth, denture extension and intaglio test surfaces. The dimensional accuracy (in root mean square, RMS) was evaluated by comparing whole denture and 3 test surfaces with the reference file. One-way analysis of variance (ANOVA) and a Post-Hoc all pairs Bonferroni test were used to determine statistical differences (alpha = 0.05). Results For the dimensional accuracy on whole denture, the 45 degrees build orientation group showed the smallest RMS (0.170 +/- 0.043 mm) than those of the 0 degrees build orientation group (0.185 +/- 0.060 mm, p < 0.001) and 90 degrees build orientation group (0.183 +/- 0.044 mm, p < 0.001). For the dimensional accuracy on the teeth, denture extension and intaglio test surfaces, the 45 degrees build orientation group also show the smallest RMS values (0.140 +/- 0.044 mm at teeth surface, 0.176 +/- 0.058 mm at denture extension and 0.207 +/- 0.006 mm at intaglio surface). The 0 degrees and 90 degrees build orientation groups had similar accuracy at the teeth (0.149 +/- 0.056 mm versus 0.154 +/- 0.056 mm, p = 0.164) and denture extension surfaces (0.200 +/- 0.025 mm vs 0.196 +/- 0.013 mm, p = 1.000). However, 0 degrees build orientation group (0.228 +/- 0.010 mm) has significantly higher RMS values then those of 90 degrees build orientation group (0.218 +/- 0.057 mm) in the intaglio surface (p = 0.032). The teeth surfaces were most accurate in each build orientation groups, while the intaglio surfaces were least accurate. Conclusions The build orientation affected the dimensional accuracy of 3D-printed mandibular complete dentures, and the 45 degrees build orientation resulted in the most accurate 3D-printed denture from a MultiJet 3D printer.

Abstract:Objective Physical and digital casts are required for fixed dental prosthesis (FDP) fabrication. However, the effect of abutment tooth geometry on the accuracy of FDP casts obtained by digital and analog workflows remains unclear. This study evaluated the accuracy of obtaining dental casts for veneer and full crowns with digital and analog workflows. Materials and Methods A typodont model of the maxilla with unprepared teeth and veneer and full crown preparations was digitalized with intraoral scanning (TRIOS 3; 3Shape), digital model (DM) group, and the digital files were exported to two additive manufacturing technologies, resulting in the groups digital light processing (DLP) (Hunter; FlashForge) and polyjet (PLJ) (Eden 500 V; Stratasys). In addition, plaster casts (PCs) were obtained with polyvinyl siloxane (Express XT; 3M ESPE) double impressions and type IV plaster (FujiRock; GC America) as a control group. The sample size per group (n = 6) was defined in the pilot study. The digitization of the typodont and experimental casts was performed with a laboratory scanner (inEos X5; Dentsply Sirona). The measurement deviation (trueness and precision) to the reference model was performed with reverse engineering software (Geomagic Control X 2018; 3D Systems). Two‐way ANOVA was used to assess the effect of manufacturing techniques and abutment tooth geometry on model accuracy (primary outcome). In addition, one‐way ANOVA was used to assess significant differences between manufacturing techniques for complete arches, including abutments and unprepared teeth (secondary outcome). Tukey post hoc tests were used to compare means among groups. All statistical analyses considered a significant level of p ≤ 0.05. Abstract:BACKGROUND: The accuracy of dental crowns is crucial for their longevity and effectiveness. OBJECTIVE: This study aims to investigate how the precision of crowns is affected by two different fabrication methods, either subtractive (milling) or additive (3D printing), within computer-aided design/computer-aided manufacture (CAD/CAM) technology. METHODS: A standardised digital scan of a maxillary first molar with a shoulder margin (.stl file) was used to design and fabricate crowns through both subtractive (milling) and additive (3D printing) processes. The crowns' marginal and internal fits were assessed comprehensively. Statistical analysis, including two-way ANOVA and independent t-tests, revealed significant differences in fitting accuracy between the two methods. RESULTS: Crowns produced via 3D printing demonstrated superior fitting with minimal marginal (14 ± 5 μm) and internal discrepancies (22 ± 5 μm) compared to milling (marginal: 22 ± 4 μm, internal: 23 ± 3 μm), indicating a statistically significant advantage in precision (p⁢s⩽ 0.022 for marginal fit). CONCLUSION: The findings suggest that 3D printing may offer a more accurate alternative to milling in the fabrication of digital dental prostheses, potentially revolutionising the field with its enhanced precision capabilities.

Abstract:Objectives: In this in vitro study, the effects of additive manufacturing (AM) methods and build angles on the trueness and precision of 3D-printed palatal plate orthodontic appliances for newborns and infants were examined. Methods: Specimens were fabricated by different representative AM methods, including digital light processing (DLP), fused filament fabrication (FFF), and MultiJet printing (MJP). Three build angles (0°, 45°, and 90°) were used. After scanning, all specimens were analyzed using the 3D inspection software. The root mean square values were measured for trueness and precision. Color maps were created to detect deviations in samples. The data were statistically analyzed with a two-way ANOVA. Results: The trueness and precision were statistically influenced by both AM methods and build angles (p < 0.05). Moreover, the root mean square values of the 45° DLP (0.0221 ± 0.0017 μm) and the 0° MJP (0.0217 ± 0.0014 μm) were significantly lower compared to those in other groups (p < 0.001). Conclusions: AM methods (DLP, FFF, and MJP) and build angles (0°, 45°, and 90°) significantly impacted the dimensional accuracy of additively manufactured palatal plate orthodontic appliances. Also, the 45° DLP and the 0° MJP were associated with the highest trueness and precision. Clinical significance: All tested AM methods with different build angles yielded clinically acceptable outcomes (within an acceptance range of ±300 μm for trueness), achieving the highest accuracy with a technology-specific suitable build angle.
Fit comparison of interim crowns manufactured with open and proprietary 3D printing modes versus milling technology: An in vitro study

Abstract:Objectives: This study aimed to assess the fit of interim crowns produced using DLP-based 3D printing with different manufacturing workflows-open and proprietary-versus milling technology. Methods: A total of 120 crowns were evaluated using the replica technique. The control group (Mill, n = 30) was manufactured via subtractive technology. Experimental groups were printed using a DLP printer (SprintRay Pro95). In the proprietary mode (SR100, n = 30), manufacturer resin was used with a 100-μm layer thickness (LT) and a splashing cleaning postprocessing. In the open mode, validated resin was used. Group B100 (n = 30) had a 100-μm LT, and group B50 (n = 30) had a 50-μm followed by postprocessing in an ultrasonic bath with full immersion in isopropyl alcohol. Kruskal-Wallis tests with Bonferroni correction was applied after normal analysis (α = 0.05). Results: Group B50 exhibited the best overall fit (123.87 ± 67.42 μm), which was comparable to the gold standard Milling group, which demonstrated the lowest marginal fit (p = 0.760). SR100 showed significantly poorer performance compared to Mill, B50, and B100 (p < 0.001). Conclusions: 3D printed and milled interim crowns generally demonstrated clinically acceptable fit, with the exception of the SR100 group. Postprocessing notably influenced crown fit, with the open mode with total immersion in isopropyl alcohol being superior. Clinical significance: The present study demonstrates that the selection of an optimal manufacturing and postprocessing workflow results in superior fit for interim crowns. This enables dental professionals to evaluate protocols and ensure reliable outcomes with improved clinical outcomes in interim crown fabrication.
Effects of design software program and manufacturing method on the marginal and internal adaptation of esthetic crowns for primary teeth: A microcomputed tomography evaluation

Abstract:Statement of problem The adaptation of digitally produced crowns is affected by the design software program and manufacturing method. The effect of artificial intelligence (AI) software program design on the adaptation of the crowns is unclear and comparative evaluations should be documented. Purpose The purpose of this study was to assess the marginal and internal gaps, the absolute marginal discrepancies, and the 3-dimensional (3D) discrepancy volumes of the resin-based milled and 3D printed crowns for primary teeth designed with computer-aided design (CAD) and AI software programs by using microcomputed tomography (μCT). Material and methods A total of 40 resin-based esthetic crowns were produced for a prepared typodont tooth (right mandibular primary second molar) according to the design software program (CAD and AI) and manufacturing method (milling and 3D printing) (n=10). Four experimental groups were generated as CAD-milled, CAD-3D printed, AI-milled, and AI-3D printed. The marginal, axial, and occlusal gap values, the absolute marginal discrepancies, and the 3D discrepancy volumes of the specimens were measured by using μCT. The data were analyzed by using 2-way ANOVA and the Tukey HSD tests (α=.05). Results The lowest value for the marginal gap (54 ±43 μm) was observed in the CAD-milled group and the marginal gap value of the AI-3D printed group was significantly lower than the AI-milled group ( P <.05). The lowest value for the axial gap (63 ±7 μm) was observed in the AI-3D printed group, and the highest value (145 ±58 μm) was observed in the CAD-milled group; the result for the occlusal gap value was opposite. The highest absolute marginal discrepancy value was observed in the CAD-milled group. The 3D discrepancy volumes increased in the order of the CAD-3D printed, AI-milled, CAD-milled, and AI-3D printed groups. Conclusions The marginal and internal gap values of the resin-based crowns were affected by the design software program and manufacturing method; however, tested groups showed clinically acceptable gap values.

Abstract:STATEMENT OF PROBLEM:Additive manufacturing is emerging as an alternative method of fabricating dental restorations, but the support design needs to be optimized. PURPOSE:The purpose of this in vitro study was to evaluate the 3-dimensional trueness and adaptations of zirconia crowns manufactured by stereolithography (SLA) with an occlusal full-supporting structure, compared with those SLA-printed with pillar supports, and those made by milling. MATERIAL AND METHODS:A zirconia abutment was prepared, and an anatomic contour crown was designed. The crowns were manufactured by SLA and milling (n=6). For SLA manufacturing, a full-supporting base and pillar supports were designed. The 3-dimensional (3D) trueness of the fabricated crowns was characterized by 3D deviation analysis. The adaptations of crowns in the SLA-base and milling groups were measured by using a triple-scan method. Color-difference maps and the root mean square (RMS) values were used to characterize the 3D trueness. One-way analysis of variance (ANOVA) and Tukey post hoc test were used to analyze the difference in RMS values among the 3 groups, and Student t test was used to analyze the difference in cement-gap width between the milling group and the SLA group with the full-supporting base (α=.05). RESULTS:The 3D deviation analysis showed that in the external area, the RMS value of the SLA-pillar group was significantly higher than that of the SLA-base and the milling groups (P<.05). In the intaglio area, the milling group showed a lower RMS value than the 2 SLA groups (P<.05). The color-difference maps showed the SLA-base group had smaller positive errors at the cusp inclines than the SLA-pillar group. No statistically significant difference was found in adaptations between the SLA-base and milling groups (P>.05). CONCLUSIONS:The occlusal full-supporting base provided improved support in fabricating the crowns, and no remnants were left after removal. The zirconia crowns manufactured by SLA with an occlusal full-supporting structure had good external 3D trueness and clinically acceptable adaptation.
