Abstract:Background: The aim of this study was to evaluate the chronic systemic and local toxicity of a copper intrauterine device in a rat model.Material/Methods: Cu-IUD fragments were inserted into the uterine horns of female Wistar rats for 26 weeks (equivalent 15.42-17.33 years in humans). We tested copper at 20, 40, and 60 times the clinical dosages to study copper toxicity under severe conditions. Classic chronic systemic toxicity evaluations, including body weight, hematology, organ coefficient, and tissue reactions, were performed.Results: The body weight of the rats increased during the 26 weeks, which showed that the Cu-IUD implant had no effect on animal growth. Histological examinations and organ coefficients of the uterus and other major organs indicated no obvious systemic or local impairment. Blood tests confirmed that the hepatic and renal functions remained normal.Conclusions: These results obtained at different dosages and long-term implantation provide solid data confirming the safety of long-term use of Cu-IUDs. However, the elevated leucocyte levels found in this study warrant further investigation.
Feasibility evaluation of a Cu-38 Zn alloy for intrauterine devices: In vitro and in vivo studies

Abstract:The existing adverse effects of copper in copper-containing intrauterine devices (Cu-IUDs) have raised concerns regarding their use. These adverse effects include burst release of cupric ions (Cu2+) at the initial stage and an increasingly rough surface of the Cu-IUDs. In this study, we investigated the use of two copper alloys, Cu-38 Zn and H62 as the new upgrading or alternative material for IUDs. Their corrosive properties were studied in simulated uterine fluid (SUF) by using electrochemical methods, with pure Cu as a control. We studied the in vitro long-term corrosion behaviors in SUF, cytotoxicity to uterine cells (human endometrial epithelial cells and human endometrial stromal cells), in vivo biocompatibility and contraceptive efficacy of pure Cu, H62, and Cu-38 Zn. In the first month, the burst release rate of Cu2+ in the Cu-38 Zn group was significantly lower than those in the pure Cu and H62 groups. The in vitro cytocompatibility Cu-38 Zn was better than that of pure Cu and H62. Moreover, Cu-38 Zn showed improved tissue biocompatibility in vivo experiments. Therefore, the contraceptive efficacy of the Cu-38 Zn is still maintained as high as the pure Cu while the adverse effects are significantly eased, suggesting that Cu-38 Zn can be a suitable potential candidate material for IUDs. STATEMENT OF SIGNIFICANCE: The existing adverse effects associated with the intrinsic properties of copper materials for copper-containing intrauterine devices (Cu-IUD) are of concern in their employment. Such as, burst release of cupric ions (Cu2+) at the initial stage and an increasingly rough surface of the Cu-IUD. In this work, Cu alloyed with a high amount of bioactive Zn was used for a Cu-IUD. The Cu-38 Zn alloy exhibited reduced burst release of Cu2+ within the first month compared with the pure Cu and H62. Furthermore, the Cu-38 Zn alloy displayed significantly improved biocompatibility and a much smoother surface. Therefore, high antifertility efficacy of the Cu-38 Zn alloy was well maintained, while the adverse effects are significantly eased, suggesting that the Cu-38 Zn alloy is promising for a Cu-IUD.

Comparison of Mirena intrauterine device with copper-bearing intrauterine devices with different surface areas of copper: A systematic review

Abstract:Objective: To explore the effectiveness, side effects, and acceptability of Mirena intrauterine device (IUD). Methods: Electronic searches were used to identify relevant literatures published between January 1999 and December 2010. Papers were included and evaluated according to established criteria of evidence-based medicine. Results: Nine papers were included and all of them were controlled clinical trials. Within 12 / 24 months of follow-up after IUD insertion, the pregnancy rate with IUD in situ and the expulsion rate of Mirena were similar to those of GyneFix330, MLCu375, TCu380A, TCu220C and MCu110 IUDs. Compared with women using IUDs with high surface area of copper (≥300mm2), those using Mirena had slightly lower rates of heavy menstrual bleeding, irregular vaginal bleeding and low back-abdominal pain, but slightly higher rate of amenorrhea. Within 12 months of follow-up after IUD insertion, the removal rate due to medical reasons was significantly lower, but the continuation rate was significantly higher for Mirena than for TCu220C IUD. Differences of the removal rate and the continuation rate between Mirena and other types of IUDs were not statistically significant. Conclusion: The overall clinical effects are slightly higher for Mirena than for TCu220C IUD. Compared with IUDs with high surface area of copper and MCu110 IUD, Mirena has similar effectiveness but higher rate of amenorrhea and lower rates of bleeding and pain.

Abstract:Objective: To investigate changes in the level of protein in serum and uncover the underlying pathogenesis of abnormal uterine bleeding (AUB) associated with copper intrauterine devices (Cu IUD). Methods: Protein profiles were investigated via tandem mass tag (TMT)-based quantitative proteomics and bioinformatics technology. Quantification and characterization of candidate proteins were further performed in 33 controls and 45 cases by Luminex assay and enzyme-linked immunosorbent assay. Results: In total, 842 proteins were identified via TMT coupled with liquid chromatography-tandem mass spectrometry (LC-MS/MS) in the serum of individuals with IUDs. Among them, 25 differentially expressed proteins (p < 0.05) were observed, including eight upregulated proteins and 17 downregulated proteins. Ten proteins were verified, and Alpha-1-Antitrypsin (a1AT) had a significantly elevated expression in women with AUB associated with the Cu IUD compared with healthy controls (p = 0.026) and a high area under the curve value (0.656), as well as sensitivity (64.9%) and specificity (71.9%). Conclusion: This is the first study to explore changes in serum protein and the underlying mechanisms of AUB associated with the Cu IUD via TMT technology. a1AT with biomarker potential was validated. These findings might provide an experimental basis for the early diagnosis or treatment of AUB associated with the Cu IUD.

Abstract:Aim Intrauterine device (IUD) is a commonly used contraceptive method worldwide. Abnormal uterine bleeding (AUB) is one of the most common side effects of Cu-IUDs. Since AUB varies among Cu-IUD users, changes in the bleeding-related genetic factors may contribute to AUB. This study aimed to determine the genetic risk factors of AUB after Cu-IUD insertion. Methods We conducted a case-control study on women who experienced AUB after Cu-IUD insertion (case:control = 62:59). Six candidate variants were genotyped using the Sequenom MassARRAY. Genotype and allele frequencies were analyzed using SHEsisPlus. We performed Pearson's Chi-squared test to analyze categorical data, and ESEfinder to predict the impact on splicing regulation. Results MCM8 coding sequence variants: rs3761873-A>C was in Exon 7 and rs16991617 A>G was in Exon 12 of all 19 exons, both of which were significantly different between cases and controls (p(allele) = 0.039 and p(genotype) = 0.092). rs6022 and rs6029 in F5 gene and rs3761873 and rs16991617 in the MCM8 gene showed strong linkage disequilibrium (R-2 > 0.8). ESEfinder indicated that the variants of MCM8 may affect the splicing regulation. Conclusions MCM8 rs376187 and rs16991617 were associated with AUB in Cu-IUDs users. MCM8 may play a role in AUB by regulating functions of reproductive organs and primary ovarian insufficiency. Our findings may improve the understanding of the genetic basis of AUB caused by Cu-IUDs.
Reproductive outcomes following copper‐containing intrauterine device after hysteroscopic lysis for intrauterine adhesions

Abstract:The present study aimed to investigate the reproductive outcomes of copper-containing intrauterine devices (IUDs) after hysteroscopic lysis in patients with mild to severe intrauterine adhesions (IUAs), according to the American Fertility Society (AFS) classification. Therefore, a prospective randomized controlled study was conducted at the Affiliated Jinhua Hospital of Wenzhou Medical University (Jinhua, China). A total of 173 women with IUAs were initially recruited between January 2020 and June 2021 and were then randomized to the copper-containing IUD group or the no barrier device group. Following hysteroscopic procedure, the fertility and obstetric outcomes were analyzed. Among the 173 patients enrolled, a total of 109 participants completed the study protocol. The results showed that AFS scores were not significantly different between the two groups prior to hysteroscopy. In addition, no statistically significant differences were recorded in pregnancy and live birth rates between the copper-containing IUD and no barrier device groups. Overall, the results of the current study indicated that the copper-containing IUDs had no positive effect on pregnancy and live birth rates in patients with mild to severe IUAs after hysteroscopic adhesiolysis. The present trial was retrospectively registered in the Chinese Clinical Trial Registry on 28th December 2023 (registration no. ChiCTR2300079233).

Abstract:To solve the main problems of existing coarse grained copper (CG Cu) intrauterine devices (IUD)—namely burst release and a low transfer efficiency of the cupric ions during usage—ultra-fine grained copper (UFG Cu) and single crystal copper (SC Cu) have been investigated as potential substitutes. Their corrosion properties with CG Cu as a control have been studied in simulated uterine fluid (SUF) under different conditions using electrochemical measurement methods. Long-term immersion of UFG Cu, SC Cu and CG Cu samples in SUF at 37°C have been studied for 300days. A lower copper ion burst release and a higher efficiency release of cupric ions were observed for UFG Cu and SC Cu compared with CG Cu in the first month of immersion and 2months later. The respective corrosion mechanisms for UFG Cu, SC Cu and CG Cu in SUF are proposed. In vitro biocompatibility tests show a better cellular response to UFG Cu and SC Cu than CG Cu. In terms of instantaneous corrosion behavior, long-term corrosion performance and in vitro biocompatibility, the three pure copper materials follow the order: UFG Cu>SC Cu>CG Cu, which indicates that UFG Cu could be the most suitable candidate material for intrauterine devices.
Concentration-dependent Cytotoxicity of Copper Ions on Mouse Fibroblasts in Vitro : Effects of Copper Ion Release from TCu380A Vs TCu220C Intra-Uterine Devices

Abstract:Sustained release of copper (Cu) ions from Cu-containing intrauterine devices (CuIUD) is quite efficient for contraception. However, the tissue surrounding the CuIUD is exposed to toxic Cu ion levels. The objective for this study was to quantify the concentration dependent cytotoxic effects of Cu ions and correlate the toxicity due to Cu ion burst release for two popular T-shaped IUDs - TCu380A and TCu220C on L929 mouse fibroblasts. Fibroblasts were cultured in 98 well tissue culture plates and 3-(4,5-dimethylthiazol- 2-yl)-2,5-diphehyltetrazolium bromide (MTT) assay was used to determine their viability and proliferation as a function of time. For cell seeding numbers ranging from 10,000 to 100,000, a maximum culture time of 48 h was identified for fibroblasts without significant reduction in cell proliferation due to contact inhibition. Thus, for Cu cytotoxicity assays, a cell seeding density of 50,000 and a maximum culture time of 48 h in 96 well plates were used. 24 h after cell seeding, culture media were replaced with Cu ion containing media solutions of different concentrations, including 24 and 72 h extracts from TCuIUDs and incubated for a further 24 h. Cell viability decreased with increasing Cu ion concentration, with 30 % and 100 % reduction for 40 μg/ml and 100 μg/ml respectively at 24 h. The cytotoxic effects were further evaluated using light microscopy, apoptosis and cell cycle analysis assays. Fibroblasts became rounded and eventually detached from TCP surface due to Cu ion toxicity. A linear increase in apoptotic cell population with increasing Cu ion concentration was observed in the tested range of 0 to 50 μg/ml. Cell cycle analysis indicated the arrest of cell division for the tested 25 to 50 μg/ml Cu ion treatments. Among the TCuIUDs, TCu220C having 265 mm 2 Cu surface area released 9.08 ± 0.16 and 26.02 ± 0.25 μg/ml, while TCu380A having 400 mm 2 released 96.7 ± 0.11 and 159.3 ± 0.15 μg/ml respectively following 24 and 72 h extractions. The effects of TCuIUD extracts on viability, morphology, apoptosis and cell cycle assay on L929 mouse fibroblasts cells, were appropriate for their respective Cu ion concentrations. Thus, a concentration of about 46 μg/ml (~29 μM) was identified as the LD50 dose for L929 mouse fibroblasts when exposed for 24 h based on our MTT cell viability assay. The burst release of lethal concentration of Cu ions from TCu380A, especially at the implant site, is a cause of concern, and it is advisable to use TCuIUD designs that release Cu ions within cytotoxic limits yet therapeutic, similar to TCu220C.
The Copper T 380 Intrauterine Device: A Summary of Scientific Data

Abstract:Intrauterine devices are the most widely used of all reversible contraceptive methods worldwide. Modern IUDs are among the most effective and long-acting methods of family planning and are acceptable options for many women. This monograph presents highlights of the clinical performance of the Copper T 380 over eight years, including the latest data on effectiveness, expulsions, and continuation rates. It incorporates data from Population Council and World Health Organization studies, and work by scientists at Family Health International and in several countries. Clinical studies of the three models of the Copper T 380 have provided substantial evidence of the safety, effectiveness, convenience, acceptability, and long-acting quality of this IUD. The bulk of the material in this document presents preclinical and clinical performance, including mechanisms of action, effectiveness, outcome of accidental pregnancy, rates of expulsion and ectopic pregnancy, side effects, continuation rates, return to fertility, and lactation and IUD use. There is also discussion of the data on IUD use and pelvic inflammatory disease, and the importance of performing skilled insertions under aseptic conditions.
Corrosion of wire and tube of copper-bearing intrauterine device in simulated uterine solution

Abstract:BACKGROUND: Copper-bearing intrauterine device has a good contraception effect involving in release of copper ion in the simulated uterine solution. Therefore, the corrosion of copper directly relates to security, efficacy, and long-term application of copper-bearing intrauterine device. OBJECTIVE: To investigate the corrosion of tube and wire of copper-bearing intrauterine device in the simulated uterine solution. DESIGN, TIME AND SETTING: An in vitro observational study was performed at National Institute for the Control of Pharmaceutical and Biological Product and Electron Microscope Laboratory of Beijing University of Science and Technology from October to December 2008. MATERIALS: Copper-bearing intrauterine device (TCu380A and TCu220C) was provided by Tianjin Medical Apparatus and Instruments Factory. METHODS: Copper wire and copper tube of the same surface area were dipped in the simulated uterine solution which was changed in regularity. Surface morphology of samples was observed under scanning electron microscope before dipping, and at day 10, 30, and 60 after dipping. MAIN OUTCOME MEASURES: Surface morphology. RESULTS: Draw trace was observed on the surface of copper wire, which in some extent contributed to the burst release of copper-bearing intrauterine device (TCu380A). While in the initial immersion phase, pits appeared on the surface of copper tube and increased the copper surface area. So cupric ion release rate of copper-bearing intrauterine device (TCu220C) increased firstly and then decreased. CONCLUSION: There was significant different in surface morphology between copper tube and copper wire in the simulated uterine solution in an early phase. For example, uniformed and dense small pits were distributed on the surface of copper wire, but irregular and large pits were distributed on the surface of copper tube.
Cupric Ion Release of Different Cu-containing Intrauterine Device in Simulated Uterine Solution

Abstract:Objective Cupric ion releases of different Cu-containing intrauterine devices were measured and cupric ion release equations were fit.Methods Cu was determined by flame atomic absorption spectrometry(FAAS).Reproducibility and recovery of the method was examined.Cupric ion release was predicted by Excel regression analysis.Results Time and cupric ion release of TCu220C-IUD show exponential regression relation.Time and cupric ion release of other IUDs show power function regression.The percentage of change for cupric ion release influenced by the change for observed value was from 94.21% to 98.07%.Conclusion The cupric ion releases for different Cu-containing intrauterine device were available by flame atomic absorption spectrometry(FAAS) and Excel regression analysis.The burst release of TCu380A-IUD was the worst.

Abstract:Tweetable abstract This study found the underlying genetic mechanism of abnormal uterine bleeding in Chinese Han Cu-intrauterine devices users. By using sequencing techniques, NFASC and PIGR gene variants may lead to bleeding by regulating coagulation fibrinolysis and endometrial epithelium inflammation functions. Aim: This study aimed to explore the genetic risk factors and validate variants of abnormal uterine bleeding after copper intrauterine device insertion. Methods: Whole-exome sequencing was performed and several variants were validated by Sequenom MassARRAY. Results: Eight variants showed potential clinical damage according to American College of Medical Genetics and Genomics criteria. By combined analysis of screening and validation, NFASC RS2802808 C>G p.Ile971Met (P-allele = 0.009 and P-genotype = 0.027) and PIGR RS2275531 C>T p.Gly365Ser (P-allele = 0.009 and P-genotype = 0.013) variants were identified as significantly associated with abnormal uterine bleeding with a false discovery rate NFASC and PIGR may play a role in abnormal uterine bleeding by regulating coagulation fibrinolysis and endometrial epithelium inflammation functions. Conclusion: These findings provide a genetic basis for clinical individualization and precision of intrauterine device implantation. Plain language summary Abnormal uterine bleeding (AUB) after Cu intrauterine device (Cu-IUD) insertion is the most common side effect of Cu-IUD use. AUB is a multifactorial process that relates to endometrial-related genetic factors, ovulatory function-related genetic factors, coagulation, the fibrinolytic system, contraction of the uterine arteries and endometritis inflammatory factor. This is the first study to explore the underlying genetic mechanisms of AUB related to the use of Cu-IUDs by whole-exome sequencing in the Chinese Han population. The authors found that variants of NFASC and PIGR genes were significantly associated with AUB in women using Cu-IUDs. NFASC and PIGR may be involved in coagulation fibrinolysis and endometrial epithelium inflammation functions, indicating its potential functions in AUB. This study could provide a genetic basis for studies on the individualization and precision of IUD use in the future.

Abstract:The comprehensively adopted copper-containing intrauterine devices (Cu-IUDs) present typical adverse effects such as bleeding and pain at the initial stage of post-implantation. The replacement of Cu material is demanded. Zinc and its alloys, the emerging biodegradable materials, exhibited contraceptive effects since 1969. In this work, we evaluated the feasibility of bulk Zn alloys as IUD active material. Using pure Cu and pure Zn as control groups, we investigated the contraceptive performance of Zn-0.5Cu and Zn-1Cu alloys via in vitro and in vivo tests. The results showed that the main corrosion product of Zn-Cu alloys is ZnO from both in vitro and in vivo studies. CaZn2(PO4)2∙2H2O is formed atop after long-term immersion in simulated uterine fluid, whereas CaCO3 is generally formed atop after implantation in the rat uterine environment. The cytocompatibility of the Zn-1Cu alloy was significantly higher than that of the pure Zn and pure Cu to the human endometrial epithelial cell lines. Furthermore, the in vivo results showed that the Zn-1Cu alloy presented much improved histocompatibility, least damage and the fastest recovery on endometrium structure in comparison to pure Zn, Zn-0.5Cu and pure Cu. The systematic and comparing studies suggest that Zn-1Cu alloy can be considered as a possible candidate for IUD with great biochemical and biocompatible properties as well as high contraceptive effectiveness.Statement of Significance: The existing adverse effects with the intrinsic properties of copper materials for copper-containing intrauterine devices (Cu-IUD) are of concerns in their employment. Abstract:This preliminary study evaluated the effectiveness of a novel hormone- and drug-free copper intrauterine device POD (CuiUPOD) with one to three copper-coated magnetic units in controlling Streptococcus equi subsp. zooepidemicus in seven intrauterine-inoculated mares. Initially, one unit was inserted into each mare and animals were followed weekly with trans-rectal ultrasonography, blood progesterone assay, uterine swabs for cytology and microbial culture, uterine specimens for biopsy, and trans-abdominal detection of the CuiUPOD using a handheld magnetic detector or a cell phone compass. Infection persisted after a CuiUPOD with one magnetic unit was inserted, but subsided shortly after two additional magnetic units were added. By day 60, at device removal, and after a 30-day exposure to copper, none of the mares presented with clinical signs of infection. On a scale of I-III (I, IIA, IIB, III), with I representing a healthy uterus, endometrial biopsies at the time of CuiUPOD removal, showed improved biopsy scores in two of seven mares (29%; p < 0.05). The in vivo experiments, complemented by an in vitro experiment, demonstrated a time- (24-96 hours) and dose-dependent response to Cu: three CuiUPODs - OD readings = 0.538; two CuiUPODs - OD = 0.513; and one CuiUPOD - OD = 0.452. As the concentration of copper increased, so did the antimicrobial effect. These findings suggest a promising use for the one-time application of a CuiUPOD with two or three magnetic units to mitigate uterine infections in mares.

Abstract:We immersed copper T380A and T220C IUDs in simulated uterine solution (SUS) and also in simulated body fluid (SBF). The release rate and corrosion products of copper ions after soaking in SUS and SBF for different time spans were studied by an atomic absorption spectrophotometer, scanning electron microscopy with energy dispersive x-ray microanalysis (SEM/EDX) and x-ray diffraction. The highest corrosion rate of the TCu380A IUD was obtained on the first day, while the highest corrosion rate of the TCu220C IUD was obtained on the third day. The copper corrosion rate of the TCu380A IUD was much faster than that of the TCu220C IUD in the first few days. The corrosion extent of copper wire was higher than that of a copper tube when first immersed in solution. For the same IUD in the same solution, the corrosion extent in SBF was higher than in SUS. The main compounds identified were cuprite (Cu2O). Copper wire and copper tubes were implanted in the uteruses of rats. In vivo experiments were also done. The corrosion rate in vivo was higher than that in vitro.
Urinary Trace Elements in Association with Premature Ovarian Insufficiency and Reproductive Hormones in a Chinese Population.

Abstract:Homeostasis disturbance of trace elements has been linked to adverse reproductive consequences, including premature ovarian insufficiency (POI) in women, but limited evidence has been reported so far. This case-control study evaluated the associations between 5 common urinary trace elements [copper (Cu), manganese (Mn), Iron (Fe), Selenium (Se), and zinc (Zn)] and the odds for POI. Urinary concentrations of these 5 metals and serum levels of POI-related reproductive hormones of 169 cases and 209 healthy controls were measured. The urinary levels of Cu and Se in women with POI were significantly higher than those in the controls. The positive associations were observed between Cu levels and the odds of POI [for the medium tertile: odds ratio (OR) = 3.79, 95% CI: 1.98-7.27, p < 0.001; for the highest tertile: OR = 3.85, 95% CI: 2.00-7.41, p < 0.001]. The highest tertile of urinary Se levels was associated with increasing POI risk (for the highest tertile: OR = 2.54, 95% CI: 1.38-4.70, compared with the lowest tertile, p for trend = 0.001). In POI patients, urinary concentrations of Zn and Fe were negatively associated with serum levels of follicle-stimulating hormone (FSH). Our findings suggested that higher exposure levels of Cu and Se might lead to an increased risk of POI.
A Novel Copper/Polydimethiylsiloxane Nanocomposite for Copper-Containing Intrauterine Contraceptive Devices

Abstract:In this article, a novel composite of copper (Cu) nanoparticles and polydimethiylsiloxane (PDMS) has been prepared and investigated for the potential application in Cu-containing intrauterine device. The Cu/PDMS composite with various mass fraction of Cu nanoparticles was fabricated via the hot vulcanizing process. The chemical structures and surface morphologies of the Cu/PDMS composites were characterized confirming the physical interaction between Cu nanoparticles and PDMS. The surface morphology observation using scanning electron microscope and atomic force microscope showed the agglomeration of Cu nanoparticles in PDMS matrix and the distribution of the agglomerations was more uniform with increased amount of Cu nanoparticles. The cupric ion release behaviors of the Cu/PDMS composites with different amounts of Cu nanoparticles were investigated in simulated uterine fluid at 37°C for 150 days. The corrosion morphologies of the Cu/PDMS composites were also characterized. Both the burst release rate of the cupric ion in the first few days and the steady release rate after 30-day immersion were improved. The cytotoxicity test has been done for the Cu/PDMS composites.
Effects of A Novel Complexing Copper-containing IUD on Endometrial Morphology in Rats

Abstract:Objective To explore the effects of a novel complexing copper‐containing intrauterine device(IUD)on the endo‐metrial morphological changes in rats in order to evaluate the safety of the IUD.Methods Fifty female sexually mature Spra‐gue‐Dawley(SD)rats at the age of 10-12 weeks were randomly divided into five groups :bulk copper group(Cu group ,n=10) , SiO2/PVA group(n=10) ,CuCl2/SiO2/PVA group Ⅰ(copper ion released at 5 -10 μg/220 mm2 per day ,n=10)and CuCl2/SiO2/PVA group Ⅱ(copper ion released at 10-20 μg/220 mm2 per day ,n=10)and normal control group(NC group ,n=10) . Different IUDs were advanced towards the ovaries and implanted into the uterus through the incision in the left side of the ute‐rus adjacent to the vagina. The endometrial tissues ,ovaries ,heart ,liver ,spleen ,lungs and kidneys were removed 30 and 60 days after the implantation of IUD. The morphological changes were observed by using hematoxylin‐eosin(HE)staining and light mi‐croscopy. Results Gross examination revealed the normal morphology of the heart ,liver ,spleen ,lungs ,kidneys ,uterus and ova‐ries in each group. The morphology and structure of the epithelia of the endometrium changed 30 and 60 days after the implanta‐tion of IUD in each group and no obvious changes were found in other important organs. The inflammation was milder in CuCl2/SiO2/PVA groups than in Cu group. Mild inflammation was observed in CuCl2/SiO2/PVA group Ⅰ at 30 and 60 days.Conclusion The morphological changes of the endometrium were less pronounced in the novel complexing copper‐contai‐ning IUD(low dose)group than in bulk copper‐containing IUD group ,suggesting that the novel complexing copper‐containing IUD induces minor endometrial damage ,and it may reduce adverse effects in the initial period of IUD implantation.

Abstract:Background: A coil, also known as an intrauterine contraceptive device (IUCD or ICD), is a small birth control method that is put into the uterus to stop pregnancy. It usually starts in the endocervix and vagina and spreads up the body, causing endometritis, salpingitis, tubo-ovarian abscesses, parametritis, oophoritis, and/or pelvic peritonitis. objective: The objective of the current study was to investigate the prevalence of bacteria causing vaginitis among women using different types of intra uterine devices Materials and methods: The study was conducted in Kirkuk city from November 1, 2023, to April 30, 2024, using high vaginal swabs from 186 fertile women using intrauterine devices (IUDs). Demographic, medical, and gynecologic histories were recorded, and all patients underwent pelvic examinations. Swabs from the posterior fornix of the vagina were taken for microbiological analysis. The study included women aged 18-45 using IUDs, and excluded those who had received antibiotics, had preexisting systemic or infectious diseases, or experienced bleeding. Ethical approval was obtained, and informed consent was signed by all participants. The swabs were cultured on blood agar , MacConkey agar, Mannitol salt agar and deman rosa sharpe agar and Eosin methylene blue agar and incubated at 37°C for 24 hours. The colonies were picked, purified, and identified through a series of biochemical tests, including Gram staining, motility tests, and enzyme and fermentation tests. The Vitek 2 system was used for microbial identification and susceptibility testing. Results: In a study of 148 women using copper IUDs, 66 (44.59%) tested positive for vaginal swab cultures, while 82 (55.41%) tested negative. Similarly, among 38 women using hormonal IUDs, 17 (44.74%) had positive culture results, and 21 (55.26%) tested negative. Overall, out of 186 women tested, 83 (44.62%) had positive cultures. Among the 83 positive cases, 66 (79.52%) were in copper IUD users, and 17 (20.48%) were in hormonal IUD users. Escherichia coli was the most common bacterium, found in 40.91% of positive cultures, followed by Staphylococcus epidermidis (13.64%), and other bacteria including Staphylococcus aureus, Actinomyces israelii, and Proteus mirabilis, each at 9.09%. In women with positive cultures using copper IUDs, E. coli was most prevalent (47.06%), followed by Staphylococcus aureus and Staphylococcus epidermidis (17.65% each). Among women with positive cultures, 13.25% had diabetes, compared to 4.85% with negative cultures. Hypertension was present in 3.61% of women with positive cultures versus 1.94% with negative cultures. Duration of IUD use also showed a significant correlation with culture results. Women with positive cultures using copper IUDs had the highest percentage in the 1-2 year duration category (43.94%), while those using hormonal IUDs had the highest in the same category (47.06%). Antibiotic sensitivity testing revealed that Imipenem, Meropenem, Tigecycline, and Colistin were 100% effective against E. coli isolates from women using IUDs. However, high resistance rates were observed for Ampicillin/Sulbactam (71.79%) and Ciprofloxacin (84.62%). Ceftolozane/Tazobactam and Gentamicin had relatively high sensitivity rates, while Ceftazidime/Avibactam showed moderate sensitivity. Conclusions The study reveals a significant proportion of women with copper and hormonal IUDs tested positive for vaginal infections, with Escherichia coli being the predominant bacterium isolated,significant differences were noted in the clinical presentation and comorbidities of women with positive cultures compared to those with negative cultures.study identified antibiotics with high sensitivity rates against E. coli isolates, such as Imipenem, Meropenem, Tigecycline, and Colistin, highlighting potential treatment options for IUD-associated infections caused by multidrug-resistant strains. Conversely, antibiotics like Ampicillin/Sulbactam and Ciprofloxacin demonstrated high resistance rates
Postplacental Insertion of Levonorgestrel Intrauterine System Versus Copper Intrauterine Device: A Prospective Study

Abstract:Purpose: To compare between postplacental insertion of levonorgestrel intrauterine system versus copper intrauterine device regarding expulsion rates, patient satisfaction, complications, and continuation rates. Methods: This prospective observational study was conducted on 1100 participants divided in to two groups: group (1) CU-IUD group and group (2) LNG-IUS group where women were assigned for postplacental insertion of either CU-IUD or LNG-IUS, respectively. Follow-up at 6 weeks, 3 and 6 months postpartum and data were collected and analyzed to evaluate outcomes. Results: No statistical difference between both groups regarding patients' characteristics, the overall expulsion rate was higher in LNS-IUS group than CU-IUD group; 77 patients (14%) and 50 patients (9%), respectively, (P value < 0.05), odds ratio: 1.63 at CI: (1.12-2.37). No significant difference between the two groups regarding pain intensity, perforation, abnormal uterine bleeding, and clinical endometritis (P > 0.05). Overall satisfaction rate at six months was 478(87%) in the CU-IUD group and 472(85.8%) in the LNS-IUS group (P value > 0.05), odds ratio: 1.1 at CI: (0.78-1.55). Continuation rate at s6 months was comparable between the two groups 485 (88.2%) and 480 (87.3%) in CU-IUD group and LNS-IUS group respectively, (P value < 0.05), odds ratio: 1.09 at CI: (0.76-1.56). Conclusion: The rate of expulsion of LNG-IUS is higher than copper IUD when inserted postplacental, yet the continuation and acceptability rates were comparable between the two groups.
