Abstract:Over the past two decades, the fi eld of oral and maxillofacial surgery (OMS) has grown tremendously. A good scientifi c journal, one that is reputable and that serves its audience justly, is one that features both the novelty and depth of its coverage of our rapidly changing fi eld. The editorial staff at Annals of Maxillofacial Surgery (AMS) has successfully strived for this goal, to disseminate knowledge and to foster a medium for exchange and innovation among fellow oral and maxillofacial surgeons and colleagues of related disciplines. This open access, peer-reviewed journal does particularly well at staying updated and abreast of the emerging research and techniques of OMS, which is arguably the most important job for an annal of a fi eld that has evolved from a mere subspecialty of dentistry to an integral part of the interdisciplinary patient care system. Every breakthrough in the history of our fi eld has taken place not only because of the ingenious step to invent a novel technique, but also because of the many practitioners that have later heard about the technique, seen the value thereof, then popularized and perfected it. A review of recent progress in our fi eld reveals the importance of an active and critical scholarly forum, like AMS that allows the development of revolutionary concepts such as functional and minimally invasive surgical approaches as well as the computer-aided surgical techniques.

Abstract:With rapid social and economic progresses in China in recent years,oral and maxillofacial surgery is being faced with great challenges and new development possibilities.This article analyzes the current status of oral and maxillofacial surgery and investigates its growth trends in future based on a survey of the population and medical implements in Zhejiang province.It is believed that more and more oral and maxillofacial surgeries will be dominantly performed in several larger central cities.The proportion of minimal invasive surgeries in oral and maxillofacial regions will be increased.Diagnosis and management of oral and maxillofacial traumas,congenital and acquired deformities, benign and malignant tumours as well as dental implants will be the mainstream of oral and maxillofacial surgery,in the coming years.
Innovations in Orthodontics: Skeletal Versus Dentoalveolar Expansion Utilizing Clear Aligners and TAD-Assisted Expanders

Abstract:Hypoplastic maxilla is a common skeletal anomaly that compromises function and esthetics. Beyond just a narrow-appearing smile, this abnormality presents significant restorative challenges in adult patients as it is often associated with crowding, compromised axial inclination of the teeth, lack of alveolar bone support, root proximity, and occlusal trauma.1 Recent research also confirms association of maxillary deficiency with nasal stenosis and a predisposition to compromised nasal airflow and pharyngeal collapse during sleep.2,3 Maxillary transverse skeletal deficiency is often but not always associated with posterior dental cross-bite. In most cases, maxillary posterior teeth are flared buccally and mandibular posterior teeth are excessively lingually inclined masking the underlying skeletal problem.4 Advances in 3D imaging in dentistry, namely ultra-low radiation cone-beam imaging technology, have significantly enhanced clinicians' ability to diagnose and subsequently treat a maxillary transverse deficiency.5.
A Single Center Experience: A Retrospective Study Over 10-Years Period on Mandible Fractures

Abstract:Study design: The purpose of this study was to do a retrospective analysis about patients with mandibular fracture who were treated at the department of Maxillofacial Surgery, Regional University Hospital of Ancona, Italy, between 2011 and 2021. Objective: In this study we evaluated clinical and epidemiological findings of mandible fractures focusing on the association between surgical timing, type of surgical approach and mid- and long-term outcomes. Methods: Patients were evaluated based on various parameters including age, sex, etiology, symptoms, comorbidity, clinical findings, mandible fracture type, other facial fractures, treatment, waiting time before the operation, complications and sequelae. In the period described, we recorded 1023 mandibular fractures. 93% of patients underwent surgery under general anesthesia, almost exclusively patients undergoing an open approach to internal fixation. Results: Of the patients, 684 were male (66.86%) and 339 were female (33.13%). The average age of the patients was 42, 38 years (range, 7-94 years). The leading cause of these fractures was traffic accidents (27.3%) and mandibular parasymphysis fractures were the most frequent (34.1%). The most common clinical signs and symptoms were malocclusion, difficulty in chewing, limitation of the buccal opening, hypoesthesia extending through the territory of the inferior alveolar nerve, difficulty in protrusion movements and mandibular lateralization. Conclusions: The continuous research in epidemiology, etiology, materials, and techniques will further refine the treatments of mandible fractures, which are nowadays more and more customized according to the type of trauma.

Abstract:Objectives : This article reviews recent advances in computer-aided segmentation methods for oral and maxillofacial surgery and describes the advantages and limitations of these methods. The objective is to provide an invaluable resource for precise therapy and surgical planning in oral and maxillofacial surgery. Study selection, data and sources: This review includes full-text articles and conference proceedings reporting the application of segmentation methods in the field of oral and maxillofacial surgery. The research focuses on three aspects: tooth detection segmentation, mandibular canal segmentation and alveolar bone segmentation. The most commonly used imaging technique is CBCT, followed by conventional CT and Orthopantomography. A systematic electronic database search was performed up to July 2023 (Medline via PubMed, IEEE Xplore, ArXiv, Google Scholar were searched). Results: These segmentation methods can be mainly divided into two categories : traditional image processing and machine learning (including deep learning). Performance testing on a dataset of images labeled by medical professionals shows that it performs similarly to dentists' annotations, confirming its effectiveness. However, no studies have evaluated its practical application value. Conclusions : Segmentation methods (particularly deep learning methods) have demonstrated unprecedented performance, while inherent challenges remain, including the scarcity and inconsistency of datasets, visible artifacts in images, unbalanced data distribution, and the "black box" nature. Clinical significance Accurate image segmentation is critical for precise treatment and surgical planning in oral and maxillofacial surgery. This review aims to facilitate more accurate and effective surgical treatment planning among dental researchers.

Abstract:This comprehensive review explores the advancements in Orthognathic and Oral Maxillofacial Surgery, focusing on the integration of 3D Printing and Virtual Surgical Planning (VSP). Traditional surgical methods, while effective, come with inherent risks and complications, and can lead to variability in outcomes due to the reliance on the surgeon’s skill and experience. The shift towards patient-centric care necessitates personalized surgical methods, which can be achieved through advanced technology. The amalgamation of 3D printing and VSP revolutionizes surgical planning and implementation by providing tactile 3D models for visualization and planning, and accurately designed surgical guides for execution. This convergence of digital planning and physical modeling facilitates a more predictable, personalized, and precise surgical process. However, the adoption of these technologies presents challenges, including the need for extensive software training and the steep learning curve associated with computer-aided design programs. Despite these challenges, the integration of 3D printing and VSP paves the way for advanced patient care in orthognathic and oral maxillofacial surgery.

Abstract:Compared to traditional manufacturing methods, additive manufacturing and 3D printing stand out in their ability to rapidly fabricate complex structures and precise geometries. The growing need for products with different designs, purposes and materials led to the development of 3D printing, serving as a driving force for the 4th industrial revolution and digitization of manufacturing. 3D printing has had a global impact on healthcare, with patient-customized implants now replacing generic implantable medical devices. This revolution has had a particularly significant impact on oral and maxillofacial surgery, where surgeons rely on precision medicine in everyday practice. Trauma, orthognathic surgery and total joint replacement therapy represent several examples of treatments improved by 3D technologies. The widespread and rapid implementation of 3D technologies in clinical settings has led to the development of point-of-care treatment facilities with in-house infrastructure, enabling surgical teams to participate in the 3D design and manufacturing of devices. 3D technologies have had a tremendous impact on clinical outcomes and on the way clinicians approach treatment planning. The current review offers our perspective on the implementation of 3D-based technologies in the field of oral and maxillofacial surgery, while indicating major clinical applications. Moreover, the current report outlines the 3D printing point-of-care concept in the field of oral and maxillofacial surgery.

Abstract:Abstract Oral and maxillofacial surgery is a specialized surgical field devoted to diagnosing and managing conditions affecting the oral cavity, jaws, face, and related structures. In recent years, the integration of 3D printing technology has revolutionized this field, offering a range of innovative surgical devices such as patient-specific implants (PSIs), surgical guides, splints, bone models, and regenerative scaffolds. In this comprehensive review, we primarily focus on examining the utility of 3D printed surgical devices in the context of oral and maxillofacial surgery and evaluating their efficiency. Initially, we provide an insightful overview of commonly utilized 3D printed surgical devices, discussing their innovations and clinical applications. Recognizing the pivotal role of materials, we give consideration to suitable biomaterials and printing technology of each device, while also introducing the emerging fields of regenerative scaffolds and bioprinting. Furthermore, we delve into the transformative impact of 3D printed surgical devices within specific subdivisions of oral and maxillofacial surgery, placing particular emphasis on their rejuvenating effects in bone reconstruction, orthognathic surgery, temporomandibular joint (TMJ) treatment, and other applications. Additionally, we elucidate how the integration of 3D printing technology has reshaped clinical workflows and influenced treatment outcomes in oral and maxillofacial surgery, providing updates on advancements in ensuring accuracy and cost-effectiveness in 3D printing-based procedures.

Abstract:There are few studies in the international scientific literature that specifically address the evaluation of surgical and clinical progress among physicians undergoing specialized training in maxillofacial surgery. Identifying a reliable tool for accurately assessing both theoretical knowledge and surgical skills of trainees is essential. The primary aim of this study is to establish a comprehensive assessment tool capable of evaluating both the theoretical and practical skills of physicians undergoing specialized training in maxillofacial surgery. The methodology employed aims to ensure fairness and effectiveness in skill development, thereby optimizing training activities. To meet this need, an evaluation and self-assessment test was developed for trainees undergoing maxillofacial surgery training at the Ospedali Riuniti of Ancona. Data collection involved digitally administered evaluations and self-assessment tests focused on maxillofacial traumatology, developed based on AO Trauma Surgery References. Data were processed into graphs which revealed a progressive learning trend following an initial adjustment phase, leading to optimal outcomes in both clinical and surgical domains. The evaluation and self-assessment test proved to be a valuable learning tool for gauging the advancement of clinical and surgical skills among maxillofacial surgery residents.

Abstract:Airway management in maxillofacial surgery is a critical aspect of anesthesia and perioperative care, demanding a broad array of techniques to ensure effective ventilation and oxygenation. The anatomical and physiological complexities of maxillofacial procedures necessitate a deep understanding of airway management strategies. Patients undergoing maxillofacial surgery often face heightened risks of airway compromise due to trauma, congenital abnormalities, or the surgical interventions themselves, requiring clinicians to be proficient in both routine and advanced techniques. This narrative review synthesizes current evidence and clinical practices in airway management for maxillofacial surgery. It examines the anatomical and physiological considerations, preoperative assessment protocols, intraoperative management, and postoperative care strategies. Preoperative assessments are crucial for identifying potential airway management difficulties, utilizing risk assessments, physical examinations, and imaging. Intraoperative strategies include endotracheal intubation while surgical techniques such as tracheostomy, cricothyroidotomy, submental, and retromolar intubation offer alternatives for securing the airway. Postoperative care focuses on meticulous planning and coordination to prevent complications such as airway obstruction and hypoxemia. Extubation is identified as a particularly high-risk phase, necessitating advanced techniques and multidisciplinary collaboration to ensure patient safety. The review underscores the importance of a comprehensive, multidisciplinary approach to airway management in maxillofacial surgery, highlighting the need for ongoing advancements in techniques and technologies to enhance patient outcomes.

Abstract:Bone grafting in oral and maxillofacial surgery has evolved significantly due to developments in materials science, offering innovative alternatives for the repair of bone defects. A few grafts are currently used in clinical settings, including autografts, xenografts, and allografts. However, despite their benefits, they have some challenges, such as limited availability, the possibility of disease transmission, and lack of personalization for the defect. Synthetic bone grafts have gained attention since they have the potential to overcome these limitations. Moreover, new technologies like nanotechnology, 3D printing, and 3D bioprinting have allowed the incorporation of molecules or substances within grafts to aid in bone repair. The addition of different moieties, such as growth factors, stem cells, and nanomaterials, has been reported to help mimic the natural bone healing process more closely, promoting faster and more complete regeneration. In this regard, this review explores the currently available bone grafts, the possibility of incorporating substances and molecules into their composition to accelerate and improve bone regeneration, and advanced graft manufacturing techniques. Furthermore, the presented current clinical applications and success stories for novel bone grafts emphasize the future potential of synthetic grafts and biomaterial innovations in improving patient outcomes in oral and maxillofacial surgery.

Abstract:3D and 4D printing are cutting-edge technologies for precise and expedited manufacturing of objects ranging from plastic to metal. Recent advances in 3D and 4D printing technologies in dentistry and maxillofacial surgery enable dentists to custom design and print surgical drill guides, temporary and permanent crowns and bridges, orthodontic appliances and orthotics, implants, mouthguards for drug delivery. In the present review, different 3D printing technologies available for use in dentistry are highlighted together with a critique on the materials available for printing. Recent reports of the application of these printed platformed are highlighted to enable readers appreciate the progress in 3D/4D printing in dentistry.

The diagnosis of the underlying illness or condition serves as the basis for all medical, dental, and surgical operations. Dental professionals all across the world have reaped the benefits of a major scientific advance in recent years. Many measurements may now be seen and quantified more easily using computer-assisted cephalometry. The accuracy and high quality of all dental materials have been enhanced thanks to computer-aided design and computer-aided manufacture. There have been several developments in the realm of orthodontics. The growing use of technology in recent years has transformed every sector, including medicine and dentistry. From Angle's time to today's nanorobotic age, orthodontic ideas, biomaterials, and technology have evolved greatly. It has been extensively utilized for diagnosis, treatment planning, three-dimensional printing, appliance systems, digital storage, integration, and data retrieval. Abstract:Oral surgery has undergone dramatic developments in recent years due to the use of biomaterials. The aim of the present review is to provide a general overview of the current biomaterials used in oral surgery and to comprehensively outline their impact on post-operative wound healing. A search in Medline was performed, including hand searching. Combinations of searching terms and several criteria were applied for study identification, selection, and inclusion. The literature was searched for reviews published up to July 2020. Reviews evaluating the clinical and histological effects of biomaterials on post-operative wound healing in oral surgical procedures were included. Review selection was performed by two independent reviewers. Disagreements were resolved by a third reviewer, and 41 reviews were included in the final selection. The selected papers covered a wide range of biomaterials such as stem cells, bone grafts, and growth factors. Bioengineering and biomaterials development represent one of the most promising perspectives for the future of oral surgery. In particular, stem cells and growth factors are polarizing the focus of this ever-evolving field, continuously improving standard surgical techniques, and granting access to new approaches.
