Abstract:OBJECTIVE:To investigate the application of CAD/CAM technique in reconstruction of mandible defect.METHODS:Before surgery, three-dimensional computed tomography was performed in 23 patients with mandible lesions, then the CT data were transformed into a readable format to produce facsimile models by using rapid prototyping (RP) techniques. When individual mandibular models were produced, evaluation and surgical simulation were performed on the model, which included measuring range of mandible lesions, prefabrication of mandibular reconstructive titanium plates or distractors, ascertaining precise position of transport disk, and shaping the free vascularized fibula, etc. According to the simulation, the mandible reconstruction was accomplished in operation.RESULTS:The surgical simulation on the model produced by CAD/CAM technique highly resembled the real operation. The measurements of the lesion on the model were completely correspondent with those found in operation, and the prefabricated plates and distractor fitted the contour of the mandible. The operation time shortened and the results were satisfactory. The post-operation X-ray showed that the mandible was symmetrical and the maxillofacial contour maintained.CONCLUSIONS:CAD/CAM technique was very helpful for simulation of anatomically complicated mandible defect. By preoperative simulation of the mandible and lesions, surgeons are able to improve or refine surgical planning so that better results can be assured.

Abstract:A computer aided maxillofacial sequence is presented, applied to orthognatic surgery. It consists of 5 main stages: data acquisition and integration, surgical planning, surgical simulation, and per operative assistance. The planning and simulation steps are then addressed in a way that is clinically relevant. First concepts toward a 3D cephalometry are presented for a morphological analysis, surgical planning, and bone and soft tissue simulation. The aesthetic surgical outcomes of bone repositioning are studied with a biomechanical Finite Element soft tissue model.

Abstract:The craniomaxillofacial hard tissue defects caused by head neck ablative tumor surgery, osteomyelitis or severe trauma would physiologically and psychologically affect patients’life quality. However, the complexity of this regional anatomy makes it a great challenge for plastic surgeons to reconstruct the facial contour and rehabilitate the occlusion function. Computer-aided surgery has been applied in craniomaxillofacial surgery for over three decades. In almost every field including craniofacial trauma, tumor and acquired deformities, computer-aided technique plays an important role in diagnosis, surgery simulation, individualized manipulation and enhancement of accuracy, safety of treatment. The aim of this review is to describe and comment the application of computer-aided surgery technique in craniomaxillofacial surgery and to give a prospect to future development.
Application of CAD/CAM Techniques in Mandible Large-Scale Defect and Reconstruction with Vascularized Fibular Bone Graft

Abstract:OBJECTIVE:To exploit computer-aided design and computer-aided manufactured (CAD/CAM) techniques and application in the reconstruction of mandible large-scale defect with vascularized fibular bone graft.METHODS:Before actually performing surgery, three-dimensional(3D) computed tomography(CT) was performed in 7 patients with mandibular large-scale defects, and 3D CT images were acquired by processing CT data. Then the CT data were transformed into a readable format and transferred to produce facsimile models by means of using rapid prototyping(RP) techniques. When individual mandibular models and enantiomorphous models were produced, evaluation and surgical simulation was performed in model, which included measuring range of mandible lesions, prefabrication of mandibular reconstructive titanium palate, precise position of titanium screws, shaping the free vascularized fibula by mandibular, etc. According to the simulations, the mandible reconstructions were finished in operation.RESULT:CAD/CAM techniques and application can distinctly display the mandibular lesions and ambient relationships, which is very useful for clinical assessment and surgical planning. Particular advantages were the unlimited trials with the imaging method, and the feeling of reality with the model method. The actual operative time was shortened, and surgery results were satisfactory with few complications.CONCLUSION:CAD/CAM techniques are very helpful for simulation of mandible large-scale defect with complicated anatomical and reconstructive problems. By preoperative simulation of procedures, surgeons can improve or refine treatment planning using this method and improve postoperative results.
Orbital and Maxillofacial Computer Aided Surgery: Patient-Specific Finite Element Models To Predict Surgical Outcomes

Abstract:This paper addresses an important issue raised for the clinical relevance of Computer-Assisted Surgical applications, namely the methodology used to automatically build patient-specific Finite Element (FE) models of anatomical structures. From this perspective, a method is proposed, based on a technique called the Mesh-Matching method, followed by a process that corrects mesh irregularities. The Mesh-Matching algorithm generates patient-specific volume meshes from an existing generic model. The mesh regularization process is based on the Jacobian matrix transform related to the FE reference element and the current element. This method for generating patient-specific FE models is first applied to Computer-Assisted maxillofacial surgery, and more precisely to the FE elastic modelling of patient facial soft tissues. For each patient, the planned bone osteotomies (mandible, maxilla, chin) are used as boundary conditions to deform the FE face model, in order to predict the aesthetic outcome of the surgery. Seven FE patient-specific models were successfully generated by our method. For one patient, the prediction of the FE model is qualitatively compared with the patient's post-operative appearance, measured from a Computer Tomography scan. Then, our methodology is applied to Computer-Assisted orbital surgery. It is, therefore, evaluated for the generation of eleven patient-specific FE poroelastic models of the orbital soft tissues. These models are used to predict the consequences of the surgical decompression of the orbit. More precisely, an average law is extrapolated from the simulations carried out for each patient model. This law links the size of the osteotomy (i.e. the surgical gesture) and the backward displacement of the eyeball (the consequence of the surgical gesture).

Abstract:Objectives : This paper presents a new simulator for maxillo-facial surgery, that gathers the dental and the maxillo-facial analyses together into a single computer-assisted procedure. The idea is first to propose a repositioning of the maxilla, via the introduction of a 3D cephalometry, applied to a 3D virtual model of the patient's skull. Then, orthodontic data are integrated into this model, thanks to optical measurements of teeth plaster casts. Materials and Methods : The feasibility of the maxillo-facial demonstrator was first evaluated on a dry skull. To simulate malformations (and thus to simulate a "real" patient), the skull was modified and manually cut by the surgeon, in order to generate a given maxillo-facial malformation (with asymmetries in the sagittal, frontal and axial planes). Results : The validation of our simulator consisted in evaluating its ability to propose a bone repositioning diagnosis that will put the skull as it was in its original configuration. A first qualitative validation is provided in this paper, with a 1.5-mm error in the repositioning diagnosis. Conclusions : These results mainly validate the concept of a maxillo-facial numerical simulator that integrates 3D cephalometry and guarantees a correct dental occlusion.

Abstract:This paper introduces the methodology proposed by our group to model the biological soft tissues deformations and to couple these models with Computer-Assisted Surgical (CAS) applications. After designing CAS protocols that mainly focused on bony structures, the Computer Aided Medical Imaging group of Laboratory TIMC (CNRS, France) now tries to take into account the behaviour of soft tissues in the CAS context. For this, a methodology, originally published under the name of the Mesh-Matching method, has been proposed to elaborate patient specific models. Starting from an elaborate manually-built "generic" Finite Element (FE) model of a given anatomical structure, models adapted to the geometries of each new patient ("patient specific" FE models) are automatically generated through a non-linear elastic registration algorithm. This paper presents the general methodology of the Mesh-Matching method and illustrates this process with two clinical applications, namely the orbital and the maxillofacial computer-assisted surgeries.

A New Software Suite in Orthognathic Surgery : Patient Specific Modeling, Simulation and Navigation

Abstract:Orthognathic surgery belongs to the scope of maxillofacial surgery. It treats dentofacial deformities consisting in discrepancy between the facial bones (upper and lower jaws). Such impairment affects chewing, talking, and breathing and can ultimately result in the loss of teeth. Orthognathic surgery restores facial harmony and dental occlusion through bone cutting, repositioning, and fixation. However, in routine practice, we face the limitations of conventional tools and the lack of intraoperative assistance. These limitations occur at every step of the surgical workflow: preoperative planning, simulation, and intraoperative navigation. The aim of this research was to provide novel tools to improve simulation and navigation. We first developed a semiautomated segmentation pipeline allowing accurate and time-efficient patient-specific 3D modeling from computed tomography scans mandatory to achieve surgical planning. This step allowed an improvement of processing time by a factor of 6 compared with interactive segmentation, with a 1.5-mm distance error. Next, we developed a software to simulate the postoperative outcome on facial soft tissues. Volume meshes were processed from segmented DICOM images, and the Bullet open source mechanical engine was used together with a mass-spring model to reach a postoperative simulation accuracy <1 mm. Our toolset was completed by the development of a real-time navigation system using minimally invasive electromagnetic sensors. This navigation system featured a novel user-friendly interface based on augmented virtuality that improved surgical accuracy and operative time especially for trainee surgeons, therefore demonstrating its educational benefits. The resulting software suite could enhance operative accuracy and surgeon education for improved patient care.
Features of the Acoustic Mechanism of Core-Collapse Supernova Explosions

Abstract:In the context of 2D, axisymmetric, multi-group, radiation/hydrodynamic simulations of core-collapse supernovae over the full 180$^{\circ}$ domain, we present an exploration of the progenitor dependence of the acoustic mechanism of explosion. All progenitor models we have tested with our Newtonian code explode. We investigate the roles of the Standing-Accretion-Shock-Instability (SASI), the excitation of core g-modes, the generation of core acoustic power, the ejection of matter with r-process potential, the wind-like character of the explosion, and the fundamental anisotropy of the blasts. We find that the breaking of spherical symmetry is central to the supernova phenomenon and the blasts, when top-bottom asymmetric, are self-collimating. We see indications that the initial explosion energies are larger for the more massive progenitors, and smaller for the less massive progenitors, and that the neutrino contribution to the explosion energy may be an increasing function of progenitor mass. The degree of explosion asymmetry we obtain is completely consistent with that inferred from the polarization measurements of Type Ic supernovae. Furthermore, we calculate for the first time the magnitude and sign of the net impulse on the core due to anisotropic neutrino emission and suggest that hydrodynamic and neutrino recoils in the context of our asymmetric explosions afford a natural mechanism for observed pulsar proper motions. A Simulation Environment for Maxillofacial Surgery Including Soft Tissue Implications

Abstract:This paper describes work in progress in the area of planning and prediction of maxillofacial surgery. We focus on true 3D problems which cannot be addressed by conventional 2D cephalometric radiographs. Examples are bilateral repositioning of the zygoma’s or treating facial asymmetries. The environment being developed arose from the need of patients for prediction of their expected post-op outlook and from the need of surgeons for better support to reach the desired result. Our simulation environment adheres to a scene-based approach in which CT image-derived visualizations and additional 3D photographs (showing the face surface and its natural complexion) are co-presented and manipulated. Tools for non-planar osteotomy are included. Repositioning of bone fragments is made possible. Furthermore, a voxel displacement formalism is presented to account for soft tissue implications. Combining these simulation tools, a qualitative prediction of the facial outlook is obtained. Results are shown for a case of hypoplastic zygoma repositioning.

First, a 3D cephalometric analysis, which is clini-cally essential for the therapeutic decision. Then, an original method to build a biomechanical model of patient face soft tissue, which provides evaluation of the aesthetic outcomes of an intervention. Both points are developed within a clinical application context for computer aided maxillofacial surgery.

Abstract:The treatment of craniomaxillofacial (CMF) defects and deformities is highly challenging due to the complex anatomical structures, significant individual variations and high demands for personalized reconstruction. In recent decades, the integration of cutting-edge technologies from fields such as computer science, mechanical engineering, and material science into medicine has greatly facilitated the transformation of CMF surgery from traditional, experience-based approaches to digitalized and intelligent methods. This review thoroughly analyzes the development and application areas of digital technology in CMF surgery including virtual surgical planning and facial prediction, three-dimensional printing, computer-assisted navigation and robotic surgery, virtual and mixed reality, as well as big data and artificial intelligence technologies. The aim is to offer comprehensive and in-depth reference information for the clinical practice in digital CMF surgery.

Abstract:The maxilla is the most important bony support of the mid-face skeleton and is critical for both esthetics and function.Maxillary defects,resulting from tumor resection,can cause severe functional and cosmetic deformities.Furthermore,maxillary reconstruction presents a great challenge for oral and maxillofacial surgeons.Nowadays,vascularized composite bone flap transfer has been widely used for functional maxillary reconstruction.In the last decade,we have performed a comprehensive research on functional maxillary reconstruction with free fibula flap and reported excellent functional and acceptable esthetic results.However,this experience based clinical procedure still remainssome problems in accuracy and efficiency.In recent years,computer assisted techniques are now widely used in oral and maxillofacial surgery.We have performed a series of study on maxillary reconstruction with computer assisted techniques.The computer assisted techniques used for maxillary reconstruction mainly include:(1) Three dimensional (3 D) reconstruction and tumor mapping:providing a 3 D view of maxillary tumor and adjacent structures and helping to make the diagnosis of maxillary tumor accurate and objective;(2) Virtual planning:simulating tumor resection and maxillectomy as well as fibula reconstruction on the computer,so that to make an ideal surgical plan;(3) 3D printing:producing a 3D stereo model for prebending individualized titanium mesh and also providing template or cutting guide for the surgery;(4) Surgical navigation:the bridge between virtual plan and real surgery,confirming the virtual plan during the surgery and guarantee the accuracy;(5) Computer assisted analyzing and evaluating:making a quantitative and objective of the final result and evaluating the outcome.We also performed a series of studies to evaluate the application of computer assisted techniques used for maxillary reconstruction,including:(1)3D tumor mapping technique for accurate diagnosis and treatment of maxillary tumor;(2) Maxillary reconstruction with free fibula flap used computer assisted techniques;(3) Computer assisted orbital floor reconstruction after maxillectomy.The results suggested that computer assisted techniques could significantly improve the clinical outcome of maxillary reconstruction.

Abstract:This paper presents the biomechanical finite element models that have been developed in the framework of the computer-assisted maxillofacial surgery. After a brief overview of the continuous elastic modelling method, two models are introduced and their use for computer-assisted applications discussed. The first model deals with orthognathic surgery and aims at predicting the facial consequences of maxillary and mandibular osteotomies. For this, a generic three-dimensional model of the face is automatically adapted to the morphology of the patient by the mean of elastic registration. Qualitative simulations of the consequences of an osteotomy of the mandible can thus be provided. The second model addresses the Sleep Apnoea Syndrome. Its aim is to develop a complete modelling of the interaction between airflow and upper airways walls during respiration. Dynamical simulations of the interaction during a respiratory cycle are computed and compared with observed phenomena.
Computer guided maxillo-malar piezoelectric osteotomies for midface augmentation

Abstract:Purpose: Despite maxillo-malar osteotomies having been discarded in the most recent literature, in the setting of orthognathic surgery they provide several advantages for middle third augmentation. This paper reintroduces maxillo-malar osteotomies as an effective method to increase volume of midfacial region in combination with contemporary technology, including piezosurgery, virtual surgical planning, and navigation. Material and methods: Eighteen patients with midface hypoplasia were included in this study and underwent orthognathic surgery with maxillo-malar osteotomies. All patients underwent the same workflow: virtual design of maxillo-malar osteotomies, surgery with navigation-assisted piezoelectric osteotomies, and computerized morphometric analysis. Results: Simulated maxillo-malar osteotomies were successfully replicated in the operating room, as shown by accuracy evaluation performed using three-dimensional analysis. In long-term follow-up period, no permanent complications were assessed. Superimposition between postoperative and preoperative CBCTs revealed that the soft tissue area influenced by the underlying skeletal movement was comparable for all cases. Conclusions: Virtual surgical planning, navigation, and piezosurgery are today indispensable tools to perform maxillo-malar osteotomies safely and accurately. We suggest incorporating such osteotomies in the surgeon's armamentarium for patients with severe midfacial hypoplasia as they offer an integrated solution to restore functionality and aesthetics.
Post-traumatic Maxillofacial Reconstruction with Vascularized Flaps and Digital Techniques: 10-Year Experience.

Abstract:The aim of this study was to present a treatment protocol for the individual repair of post-traumatic maxillofacial bone defects with vascularized flaps assisted by digital techniques. This study reviewed 20 patients with post-traumatic maxillofacial bone defects who underwent reconstruction with composite vascularized bone flaps assisted by digital techniques between April 2009 and July 2019. Preoperative computed tomography (CT) data were imported into ProPlan CMF software to complete virtual fracture reduction and reconstruction. Surgical navigation, three-dimensionally (3D) printed surgical plates, and prefabricated titanium mesh/plates were used to guide the actual surgery. All patients underwent open reduction and internal fixation and reconstruction surgery in one stage. CT data obtained at 1 week postoperative were imported into Geomagic Control software to evaluate the accuracy of the virtual surgical plan. The mean follow-up interval was 24 months (range 6-96 months). Donor and recipient site morbidity and second-stage procedures to rehabilitate the dentition and cosmetic organs were recorded. The flap success rate was 100%. Nine patients had deep circumflex iliac artery flaps and eleven patients had fibula flaps. The accuracy of computer-assisted surgery was 4.4±0.8mm. There were no postoperative complications. This study is novel in presenting a treatment protocol for individual computer-assisted reconstruction for post-traumatic maxillofacial bone defects with vascularized flaps.
Computer aided planning and navigation for orbito-zygomatic reconstruction

Abstract:This paper suggests a full protocol of Computer Aided Surgery as previously recommended in literature addressing the challenging task of primary or secondary reconstruction of orbito-zygomatic dislocation. First, on a specifically developed planning software, the best zygoma reduction and orbital boundaries reconstruction to achieve skeletal symmetry are determined. This treatment plan is then transferred to the 3D Navigation Systems within the operating room. After patient's anatomy registration to his preoperative CT scan data, the navigation system allows zygomatic guiding to its planned reduced location and bone orbital volume restoration control. The feasibility of this technique was checked in 3 patients with major orbito-zygomatic deformities. Preliminary clinical results are presented.

Conclusion FaceMesh emerges as a revolutionary tool in modern maxillofacial surgery. This deep neural network empowers surgeons with detailed insights into facial morphology, aiding in personalized interventions and optimizing surgical outcomes. The real-time assessment of facial dynamics contributes to improved aesthetic and functional results, particularly in complex cases like facial asymmetries or reconstructions. Additionally, FaceMesh has the potential for early detection of medical conditions and disease prediction, further enhancing patient care. Ongoing refinement and validation are essential to address limitations and ensure the reliability and effectiveness of FaceMesh in clinical settings.

Abstract:We present a new model of guided surgery, exclusively using computer assistance, from the preoperative planning of osteotomies to the actual surgery with the aid of stereolithographic cutting guides and osteosynthetic miniplates designed and made preoperatively, using custom-made titanium miniplates thanks to direct metal laser sintering. We describe the principles that guide the designing and industrial manufacturing of this new type of osteosynthesis miniplates. The surgical procedure is described step-by-step using several representative cases of dento-maxillofacial dysmorphosis. The encouraging short-term results demonstrate the wide range of application of this new technology for cranio-maxillofacial surgery, whatever the type of osteotomy performed, and for plastic reconstructive surgery.
