Abstract:Notifiable infectious diseases are a major public health concern in China,causing about five million illnesses and twelve thousand deaths every year.Early detection of disease activity,when followed by a rapid response,can reduce both social and medical impact of the disease.We aim to improve early detection by monitoring health-seeking behavior and disease-related news over the Internet.Specifically,we counted unique search queries submitted to the Baidu search engine in 2008 that contained disease-related search terms.Meanwhile we counted the news articles aggregated by Baidu’s robot programs that contained disease-related keywords.We found that the search frequency data and the news count data both have distinct temporal association with disease activity.We adopted a linear model and used searches and news with 1–200-day lead time as explanatory variables to predict the number of infections and deaths attributable to four notifiable infectious diseases,i.e.,scarlet fever,dysentery,AIDS,and tuberculosis.With the search frequency data and news count data,our approach can quantitatively estimate up-to-date epidemic trends 10–40 days ahead of the release of Chinese Centers for Disease Control and Prevention（Chinese CDC）reports.This approach may provide an additional tool for notifiable infectious disease surveillance.

Abstract:Background: A crucial goal of infectious disease surveillance is the early detection of epidemics, which is essential for disease control. In China, the current surveillance system is based on confirmed case reports. In rural China, it is not practical for health units to perform laboratory tests to confirm disease and people are more likely to get 'old' and emerging infectious diseases due to poor living conditions and closer contacts with wild animals and poultry. Syndromic surveillance, which collects non-specific syndromes before diagnosis, has great advantages in promoting the early detection of epidemics and reducing the necessities of disease confirmation. It will be especially effective for surveillance in resource poor settings.Methods/Design: This is a field experimental study. The experimental tool is an innovative electronic surveillance system, combining syndromic surveillance with the existing case report surveillance in four selected counties in China. In the added syndromic surveillance, three types of data are collected including patients' major symptoms from health clinics, pharmaceutical sales from pharmacies and absenteeism information from primary school. In order to evaluate the early warning capability of the new added syndromic surveillance, the timelines and validity of the alert signals will be analyzed in comparison with the traditional case reporting system. The acceptability, feasibility and economic evaluation of the whole integrated surveillance system will be conducted in a before and after study design.Discussions: Although syndromic surveillance system has mostly been established in developed areas, there are opportunities and advantages of developing it in rural China. The project will contribute to knowledge, experience and evidence on the establishment of an integrated surveillance system, which aims to provide early warning of disease epidemics in developing countries.
Research on the Environmental Impact Factors of Hand-Foot-Mouth Disease in Shenzhen, China Using RS and GIS Technologies

Abstract:The outbreak of Hand-Foot-Mouth Disease (HFMD) has brought a serious threat to people's health. There is no research considering both the natural and social environment to study the outbreak and propagation of HFMD quantitatively by now. In this study, we acquired environmental impact factors of HFMD using remote sensing and GIS. Then we analyzed the relationship between density of HFMD cases and environmental impact factors for year 2009 in Shenzhen. It was found that density of HFMD cases was significantly associated with population density, hospital density, road density, annual average of aerosol optical depth, and annual average of normalized difference vegetation index. A linear regression model was constructed with the R2 value of 0.677 using environmental impact factors, and both hospital density and annual average of aerosol optical depth were included in the model.

Abstract:To promote and preserve social harmony, social stability, and sustainable economic development, it is essential to carry out a study on the current development strategy for the prevention of infectious diseases in China. It is also necessary to propose a regulatory system and a prevention-and-control research strategy for infectious diseases that are adaptable to China’s specific national conditions. This paper analyzes the epidemic situation and the prevention and control of infectious diseases in China, and systematically expounds the significance of carrying out a study on the development strategy for the prevention of infectious diseases. Using international experience as a reference, this paper proposes strategic measures and suggestions for a regulatory system and for prevention-and-control research into infectious diseases in China. This study is based on China’s achievements and challenges of the current system.

Abstract:Objective:To explore the data management and quality control of the large-scale epidemiological survey study through an example.Methods:The quality control was applied in the whole process including quality control system,staff training,field organization,laboratory,data entry and so on,and the data quality was assessed by data checking.Results:After the original forms had been verified and the abnormal data had been revised,the error rates of people and the error rates of variable of 3 randomly selected projects were all about 10% and less than 0.5%,respectively.Conclusion:There are many factors that can affect data quality in the medical research.It is essential for data quality to develop the reasonable programs of data management and quality control according to the actual situation,although considerable manpower and material resources are required.

Abstract:BACKGROUND:Studies into the costs of syndromic surveillance systems are rare, especially for estimating the direct costs involved in implementing and maintaining these systems. An Integrated Surveillance System in rural China (ISSC project), with the aim of providing an early warning system for outbreaks, was implemented; village clinics were the main surveillance units. Village doctors expressed their willingness to join in the surveillance if a proper subsidy was provided. This study aims to measure the costs of data collection by village clinics to provide a reference regarding the subsidy level required for village clinics to participate in data collection.METHODS:We conducted a cross-sectional survey with a village clinic questionnaire and a staff questionnaire using a purposive sampling strategy. We tracked reported events using the ISSC internal database. Cost data included staff time, and the annual depreciation and opportunity costs of computers. We measured the village doctors' time costs for data collection by multiplying the number of full time employment equivalents devoted to the surveillance by the village doctors' annual salaries and benefits, which equaled their net incomes. We estimated the depreciation and opportunity costs of computers by calculating the equivalent annual computer cost and then allocating this to the surveillance based on the percentage usage.RESULTS:The estimated total annual cost of collecting data was 1,423 Chinese Renminbi (RMB) in 2012 (P25 = 857, P75 = 3284), including 1,250 RMB (P25 = 656, P75 = 3000) staff time costs and 134 RMB (P25 = 101, P75 = 335) depreciation and opportunity costs of computers.CONCLUSIONS:The total costs of collecting data from the village clinics for the syndromic surveillance system was calculated to be low compared with the individual net income in County A.

Abstract:Epidemiology is a discipline integrating methodology and applied science, whose mission is to prevent and control diseases and promote health. This review introduces the new progress of epidemiology from five aspects: communicable diseases, chronic diseases, systems epidemiology, implementation research and big data of health care. New projects and constantly emerging technologies in the field of infectious diseases are inspiring, while more attention should be paid to the environmental factors of pathogen variation. In the field of chronic diseases, there is an urgent need to study the multimorbidity of the elderly. The role of infectious inducers and human microbiota in the occurrence and development of chronic diseases has been gradually revealed. Systems epidemiology, which is of great significance to achieve precision prevention is a new branch and an important supplement of modern epidemiology. Implementation research, is a bridge connecting basic scientific research and public health practice and will provide evidence to support the effective implementation of the Health China Action Plan. The development of health care big data is based on digital public health, which provides a broad research platform and abundant data resources for epidemiology, and will promote the fundamental transformation of the service and management mode of public health.

Abstract:This study aims at examining the application and development of digital teaching materials in the field of epidemiology, encompassing both China and international contexts. The research involved conducting search on websites and literature databases to assess the status of digital teaching materials in epidemiology, nationally and internationally. At present, in China, digital teaching materials used in epidemiology are primarily presented in the form of printed books with added QR codes, providing teaching resources such as videos and exercises. However, issues with the level of interactivity have been identified. In foreign countries, with stronger emphasis placed on personalization, interactivity, and the use of rich media technologies in the digital teaching materials, epidemiologically. Enhanced digitization regarding materials and learning outcomes is achieved through features such as real-time notes, interactive animations, and quizzes. These approaches are considered worth considering for adoption. This study provides valuable insights for the digital transformation of epidemiology education.

Abstract:This paper introduced the project of "reestablishing the disease control and prevention system of China" in brief, including background, objectives, funding resources, researching objects and sampling methods. This project which funded by National Outstanding Younger Fund and the research fund of MOH aimed at nailing down the key problem existed in disease control and prevention system of China, demonstrating the reasons and mechanism of key problem, developing feasible policy idea and strategy. This paper also introduced some issues concerning the reestablishing of the disease control and prevention system of China: the definition of public function, the standard of human resource allocation and the standard of financing. The Centers for Disease Control and Prevention in 8 provinces, 80 cities and 80 counties have been sampled to provide information that project needed. In addition, this project also cited some data which come from the early study, in which 3 provinces, 12 counties, 49 towns, 179 villages and 9781 rural families have been sampled and investigated.

Abstract:Objectives In epidemiological research, major studies have focused on theoretical models; however, few methods of visual analysis have been used to display the patterns of disease distribution. Design For this study, a method combining the space-time cube (STC) with space-time scan statistics (STSS) was used to analyze the pattern of incidence of hand-foot-mouth disease (HFMD) in Guangdong Province from May 2008 to March 2009. In this research, STC was used to display the spatiotemporal pattern of incidence of HFMD, and STSS were used to detect the local aggregations of the disease. Setting The hand-foot-mouth disease data were obtained from Guangdong Province from May 2008 to March 2009, with a total of 68,130 cases. Results The STC analysis revealed a differential pattern of HFMD incidence among different months and cities and also showed that the population density and average precipitation are correlated with the incidence of HFMD. The STSS analysis revealed that the most likely aggregation includes the Shenzhen, Foshan and Dongguan populations, which are the most developed regions in Guangdong Province. Conclusion Both STC and STSS are efficient tools for the exploratory data analysis of disease transmission. STC clearly displays the spatiotemporal patterns of disease. Using the maximum likelihood ratio, the STSS model precisely locates the most likely aggregation.

Abstract:Infectious disease outbreaks transcend the medical and public health realms, triggering widespread panic and impeding socio-economic development. Considering that self-limiting diarrhoea of sporadic cases is usually underreported, the Salmonella outbreak (SO) study offers a unique opportunity for source tracing, spatiotemporal correlation, and outbreak prediction. To summarize the pattern of SO and estimate observational epidemiological indicators, 1,134 qualitative reports screened from 1949 to 2023 were included in the systematic review dataset, which contained a 506-study meta-analysis dataset. In addition to the dataset comprising over 50 columns with a total of 46,494 entries eligible for inclusion in systematic reviews or input into prediction models, we also provide initial literature collection datasets and datasets containing socio-economic and climate information for relevant regions. This study has a broad impact on advancing knowledge regarding epidemic trends and prevention priorities in diverse salmonellosis outbreaks and guiding rational policy-making or predictive modeling to mitigate the infringement upon the right to life imposed by significant epidemics.

Allocating Limited Surveillance Effort for Outbreak Detection of Endemic Foot and Mouth Disease

Abstract:Foot and Mouth Disease (FMD) affects cloven-hoofed animals globally and has become a major economic burden for many countries around the world. Countries that have had recent FMD outbreaks are prohibited from exporting most meat products; this has major economic consequences for farmers in those countries, particularly farmers that experience outbreaks or are near outbreaks. Reducing the number of FMD outbreaks in countries where the disease is endemic is an important challenge that could drastically improve the livelihoods of millions of people. As a result, significant effort is expended on surveillance; but there is a concern that uninformative surveillance strategies may waste resources that could be better used on control management. Rapid detection through sentinel surveillance may be a useful tool to reduce the scale and burden of outbreaks. In this study, we use an extensive outbreak and cattle shipment network dataset from the Republic of Türkiye to test three possible strategies for sentinel surveillance allocation that differ in their data requirements: ranging from low to high data needs, we allocate limited surveillance to [1] farms that frequently send and receive shipments of animals (Network Connectivity), [2] farms near other farms with past outbreaks (Spatial Proximity) and [3] farms that receive many animals from other farms with past outbreaks (Network Proximity). We determine that all of these surveillance methods find a similar number of outbreaks – 2-4.5 times more outbreaks than were detected by surveying farms at random. On average across surveillance efforts, the Network Proximity and Network Connectivity methods each find a similar number of outbreaks and the Spatial Proximity method always finds the fewest outbreaks. Since the Network Proximity method does not outperform the other methods, these results indicate that incorporating both cattle shipment data and outbreak data provides only marginal benefit over the less data-intensive surveillance allocation methods for this objective. We also find that these methods all find more outbreaks when outbreaks are rare. This is encouraging, as early detection is critical for outbreak management. Overall, since the Spatial Proximity and Network Connectivity methods find a similar proportion of outbreaks, and are less data-intensive than the Network Proximity method, countries with endemic FMD whose resources are constrained could prioritize allocating sentinels based on whichever of those two methods requires less additional data collection.

Abstract:<p class="a-plus-plus">During the prevention of coronavirus disease 2019 (COVID-19), epidemiological data is essential for controlling the source of infection, cutting off the route of transmission, and protecting vulnerable populations. Following Law of the People's Republic of China on Prevention and Treatment of Infectious Diseases and other related regulations, medical institutions have been authorized to collect the detailed information of patients, while it is still a formidable task in megacities because of the significant patient mobility and the existing information sharing barrier. As a smart city which strengthens precise epidemic prevention and control, Shanghai has established a multi-department platform named "one-net management" on dynamic information monitoring. By sharing epidemiological data with medical institutions under a safe environment, we believe that the ability to prevent and control epidemics among medical institutions will be effectively and comprehensively improved.</p>
Nation-Wide Routinely Collected Health Datasets in China: A Scoping Review

Abstract:Objectives: The potential for using routinely collected data for medical research in China remains unclear. We sought to conduct a scoping review to systematically characterise nation-wide routinely collected datasets in China that may be of value for clinical research. Methods: We searched public databases and the websites of government agencies, and non-government organizations. We included nation-wide routinely collected databases related to communicable diseases, non-communicable diseases, injuries, and maternal and child health. Database characteristics, including disease area, data custodianship, data volume, frequency of update and accessibility were extracted and summarised. Results: There were 70 databases identified, of which 46 related to communicable diseases, 20 to non-communicable diseases, 1 to injury and 3 to maternal and child health. The data volume varied from below 1000 to over 100,000 records. Over half (64%) of the databases were accessible for medical research mostly comprising communicable diseases. Conclusion: There are large quantities of routinely collected data in China. Challenges to using such data in medical research remain with various accessibility. The potential of routinely collected data may also be applicable to other low- and middle-income countries.

Abstract:[Objective] To explore the situation of infectious disease control and prevention in schools and kindergartens and to provide the basis to next steps of enhancing the infectious management in schools.[Methods] Used multiplestage sampling method to select schools and kindergartens;all selected schools and kindergartens were surveyed.[Results] Total number of schools and kindergartens was 280.80.71% of them had the system about infectious diseases reporting.72.50% had the health examination in the morning.But only 60.71% of all schools kept the records of morning examination.Only 30% had records about missing school.40.85% of all schools had no school doctor which was required by the school regulation.97.47% of staff working in dining-room had the health certificates.[Conclusion] Both regulations establishment and implementation in urban schools are better than that in rural schools.Most of the schools and kindergartens have the systems about infectious disease prevention and control.But there are still some of risk factors of infectious disease epidemic / outbreaks in schools and kindergartens.Sanitation supervise should be enhanced.Methods should be developed to increase the ability of infectious disease prevention and control in educational institute.
