Improving the Accuracy and Consistency in Clinical Target Volume Delineation for Rectal Carcinoma by an Education Program

Abstract:The aim of this study was to examine the delineation variations in clinical target volume (CTV) for rectal carcinoma among domestic radiation oncologists and evaluate whether an education program could improve the accuracy and consistency. Firstly, radiation oncologists were required to delineate a CTV on the CT scans of a patient diagnosed with locally advanced rectal adenocarcinoma independently based on previous experience. Then was a 150 minutes education program which consists of the following 4 parts: postoperative recurrence pattern and contouring guidelines introducing, a standard CTV displaying, real-time feedback of the delineation problems for each participants and discussion. After that the delineation was repeated on the same CT scans. The volumes obtained before and after the education program were compared. DICE similarity coefficient (DICE), inclusiveness index (Inc.Idx), conformal index (CI) and relative volume difference (Dv%) were statistically analyzed to quantitatively evaluate the disparities between the participants delineation and the standard CTV. Maximum volume ratio (MVR) and Coefficient of variation (CV) were calculated to explore interobserver variation(IOV). The effects of the education program on the delineation accuracy and IOV were assessed. Of the 18 radiation oncologists from 10 different provinces of China, 13 submitted contour set pairs. For quantitative analysis of volume parameters, the average delineated CTV volume decreased from 809.82 to 705.21cc (p<0.001). DICE, Inc.Idx, CI were significantly increased (DICE: 0.84 vs 0.78, p=0.009; Inc.Idx: 0.79 vs 0.69, p=0.002; CI: 0.73 vs 0.65, p=0.011) while Dv% was decreased (21.43%vs30.79%, p=0.002). As for qualitative analysis, the most variability was observed in whether or not to contour external iliac region and ischiorectal fossa. In addition, the education program reduced these variations. For IOV, an 11.80% reduction in MVR from 1.78 to 1.57 and 18.19% reduction in CV from 17.43 to 14.26 were noted. Large differences in the delineation of CTV for rectal carcinoma were present among radiation oncologists in China. Training interventions could improve delineation accuracy and reduce interobserver variations.
