Abstract:Magnetic resonance imaging (MRI) has the advantages of multiple parameters and no radiation damage, and its application in clinical diagnosis and scientific research has greatly promoted the rapid development of life sciences. The main magnetic field is an important component of the MRI system, 1 with domestic systems generally having a main magnetic field strength between 1.5 T and 3.0 T. The main magnetic field of different strengths can cause ferromagnetic substances to "fly" toward the magnet from a distance at a very fast speed, resulting in a powerful impact force and causing mechanical injuries to individuals around the magnet. 2 It is actually an external manifestation of the powerful attractive force of the magnet. Objects that are affected by this ferromagnetic effect are commonly referred to as ferromagnetic projectiles. The most common ferromagnetic projectiles that can be mistakenly brought into the MRI exam room by patients and their families include hospital beds, wheelchairs, surgical instruments, oxygen cylinders, monitoring devices, infusion pumps, and various metal objects carried by the examinees themselves (tools, nail clippers, keys, coins, metal accessories, watches, mobile phones, etc.). They can cause instant harm to people or machines. According to Xu Changpu's study, among over 130,000 MR exams at Taihe Hospital affiliated with Hubei University of Medicine, small metallic objects such as coins and hair clips being drawn into the magnet occurred 25 times, which is the most common accident reported, 1 incident involved a metal IV stand being sucked into the magnet, which was resolved only after the machine was shut down and handled by professional transportation personnel. 3 In 2018, Rajesh Maruti Maru accompanied his mother for an MRI scan in India when an oxygen cylinder mistakenly entered the scanning room and was attracted to the magnet, becoming trapped inside the machine, ultimately leading to her death. 4 There have been numerous reports of medical equipment entering the MRI suite and potentially causing serious medical accidents, even resulting in injuries or fatalities, such as stretcher beds, oxygen cylinders, and wheelchairs. 5–7 Medical implants and devices made by ferromagnetic objects are subjected to forces when they enter the magnetic field. 8 Song et al estimated that 10%–20% of all MRI patients had medical devices implanted. 9 If there is conductive metallic material in contact with the patient's skin within the magnet bore, local heating and burns may occur. 10,11 Reports from the United States have shown that out of 100 MRI systems, 20% have experienced ferromagnetic projectile accidents, causing varying degrees of harm. 12 However, the research conducted by Lv Guoyi and others shows that injuries caused by ferromagnetic projection accidents in MRI can be avoided through scientific management. 13 Zhao Xiping and others have proven through their research that establishing a safety system can effectively prevent injuries caused by ferromagnetic projectiles. 14 Li Geng and others analyzed relevant data on patient safety incidents in the use of MRI equipment in the United States and confirmed that strengthening the detection of metal objects carried by patients can reduce the incidence of ferromagnetic projection accidents in MRI. 2 Therefore, patients, their families, and medical personnel entering the magnet area must remove all metal objects to avoid ferromagnetic projection accidents that can cause harm and damage to patients, their families, medical workers, and the equipment. Effective safety education before the exam is an important step in increasing the patient's safety awareness before the MRI exam, strengthening the attitude and awareness of ferromagnetic projectile accidents, and implementing safety self-check behaviors before the exam.
