Comparison of modified early warning score and APACHEⅡ score in detecting developing critical illness

Abstract:Objective To compare the clinical value of Modified Early Warning score(MEWS) and APACHE Ⅱ in evaluating developing critical illness.Methods 360 death cases of inpatient with complete clinical data were analyzed.The clinical data include gender,age,life signs,respiratory support,Oxygen Saturation,consciousness,urinary output,results of chemical examination,diagnosis,and so on.MEWS and APACHEⅡ score were evaluated at the 360 death cases.The correlation between MEWS and APACHE Ⅱ score were explored according to correlation analysis,optimal severe-illness-identification cutoff level of MEWS were explored according to ROC curve.The MEWS variation was explored according to K Independent Samples Test between different department,genders and age.Results MEWS score was positively correlate with APACHE II score(p<0.001).The optimal severe-illness-identification cutoff level of MEWS was 3.The nonparametric test results with several independent samples showed that MEWS had no statistical significant variation between medical and surgery department(p>0.05).MEWS had no statistical significant variation between different genders(p>0.05).MEWS had some statistical significant variation between different ages(p<0.05).Conclusion MEWS may take the place of APACHE II,and it can be used to discriminate severity patients.The age of inpatient should be concidered.The parameter group contains systolic pressure,heart rate,respiratory rate,consciousness level and body temperature.The severe-illness cutoff level≥3,the sensitivity is 65.9%,the specificity is 86.0%.
