Abstract:Background and objective Decreasing X-ray tube voltage is an effective way to reduce radiation and contrast dose, especially in non-obese patients. The current study focuses on CTA in non-obese patients to evaluate image quality and feasibility of 80-kV acquisition protocols with varying iodine delivery rates (IDR) and contrast concentrations in routine clinical practice. Methods A prospective observational study in patients ≥ 18 years and ≤ 90 kg referred for coronary or craniocervical CTA at 10 centers in China (ClinicalTrials.gov: NCT02840903). Patients were divided into four groups: a standard 100-kV protocol (370 mgI/ml, IDR 1.48 gI/s), and three 80-kV protocols (370 mgI/ml, IDR 1.2 gI/s; 300 mgI/ml, IDR 1.2 gI/s; 300 mgI/ml, IDR 0.96gI/s). The primary outcome was contrast opacification of target vascular segments. Secondary outcomes were image quality (contrast-to-noise ratio, signal-to-noise ratio, visual image quality, and diagnostic confidence assessment), radiation, and iodine dose. Results From July 2016 to July 2017, 1213 patients were enrolled: 614 coronary and 599 craniocervical CTA. The mean contrast opacification was ≥ 300 HU for 80-kV 1.2 gI/s IDR scanned segments; IDR 0.96 gI/s led to lower opacification. Image quality and diagnostic confidence were fair to excellent (≥ 98% of images), despite lower contrast-to-noise ratios and signal-to-noise ratios in 80-kV images. Compared to the standard protocol, 80-kV protocols led to 44–52% radiation dose reductions ( p < 0.001) and 19% iodine dose reductions ( p < 0.001). Conclusion Eighty-kilovolt 1.2 gI/s IDR protocols can be recommended for coronary and craniocervical CTA in non-obese patients, reducing radiation and iodine dose without compromising image quality. Key Points • Using low-voltage scanning CTA protocols, in which tube voltage and iodine delivery rate are reduced proportionally (voltage: 80 kV, IDR: 1.2 gI/s), reduces radiation and contrast dose without compromising image quality in routine clinical practice . • Reducing iodine delivery rate beyond direct proportionality to tube voltage is not beneficial .
Study on the Optimized Scanning Conditions of Cerebral CT Angiography Based on the Angiographic CT Head Phantom

Abstract:Objective To study the impacts of different tube voltage and different noise index(NI) guided automatic tube current modulation on the image quality and radiation dose in cerebrovascular imaging and determine the optimal scanning condition. Methods PH3 angiographic CT head phantom was used for head CTA examination. Scanning protocols: all the scanning objectives were divided into three groups according to the different tube voltages of 80, 100 and 120 kV. Each group applied certain tube current(300,400 mA)and automatic tube current modulation technique with NI from 3 to 10 to perform head CTA. There were 30 scanning proposals with different parameter combinations of tube voltage and tube current. The radiation dose [ CT dose index volume (CTDIvol), dose length product (DLP) and effective dose (ED)], objective indicators of images(CT value of the blood vessels and its noise, CT value of brain tissue and its noise, signal-to-noise ratio , contrast-to-noise ratio) and the subjective scores of the five cerebrovascular segments were recorded. Differences of CT value of the blood vessels and its noise, CT value of brain tissue and its noise, signal-to-noise ratio and contrast-to-noise ratio between different tube voltages and tube currents were analyzed by One-Way ANOVA. Results When the tube voltage was certain, the CTDIvol, DLP and ED were all dropped while NI was increased from 3 to 10. Compared with group(120 kV, 300 mA), CTDIvol of group with 100 kV, 300 mA decreased 35.32%(12.22/34.59), CTDIvol of group(100 kV, NI=6) decreased 46.72%(16.16/34.59). Compared with group(100 kV, 300 mA), CTDIvol of group (100 kV, NI=6) decreased 17.61%(3.94/22.37). When the tube voltage was certain and the tube current and NI were not certain, there is no statistical difference (P>0.05) between CT values of blood vessel and brain, while blood vessel noise, noise of brain, SNR and CNR showed statistical difference (P<0.05). When tube current and NI were certain while tube voltage was varied, all objective indicators discussed above all exhibited statistical difference (P<0.05). SNR and CNR of group(100 kV, NI=6) were higher than group(120 kV, 300 mA) with 6.31%(2.69/42.66)and 7.18%(2.64/36.78), respectively. The tube voltage, NI and tube current had no effect on the subjective scores of first and second grade vessel but greater impact on the fourth and fifth grade vessel. Conclusion In the head CTA scanning, combined the use of NI 6 guided automatic adjustment tube current and low tube voltage(100 kV)technique not only can get better image quality but also significantly decreased the radiation dose.
[Optimization of Scan Protocol in Pulmonary Angiography of Second Generation Dual Source Computed Tomography].

Abstract:OBJECTIVE:To explore the scan protocol of second generation dual source computed tomography (CT) for CT pulmonary angiogram (CTPA) in patients with suspected pulmonary embolism and determine the proper volume of contrast medium.METHODS:A total of 120 patients undergoing CTPA were recruited. All examinations were performed on a second generation dual source CT. The scan delay was determined with the test bolus technique. Four contrast volume protocols were designed, namely groups A, B, C and D respectively (n = 30 each). For CTPA scan of groups A, B, C and D, 35, 40, 45 and 50 ml contrast medium were injected at an injection rate of 4 ml/s, followed by a saline chaser of 55, 50, 45 and 40 ml respectively. Attenuation profiles of different vascular segments (subclavian vein, vena cava, left atrium, pulmonary trunk, S1 artery, S10 artery) were measured to evaluate the timing techniques. The image quality and artifact of iodine map were analyzed by two radiologists for comparisons.RESULTS:The percentage of pulmonary arteries exceeding optimal attenuation (> 200 HU) showed that groups of B, C and D were higher than group A (93% - 100% vs 80% - 90%). The image quality of group A was lower than the others (P < 0.05) while the other groups showed no significant difference. The artifacts of iodine map of group A were much fewer than the others (P < 0.05) and group D was much more than the other groups (P < 0.05). Groups B and C had a score of 4.13 and 3.67 respectively without significant difference (P = 0.091).CONCLUSION:With the aids of test bolus technique and proper injection protocol (40 ml contrast medium followed by 50 ml saline), the high quality image and low volume of iodine can be realized with second generation dual source CT for CTPA.

Abstract:Background: Our study aims to evaluate the image quality and feasibility of 128-slice dual-energy CTA (DE-CTA) for supra-aortic arteries using reduced amounts of contrast medium (CM).Methods: A prospective study was performed in 54 patients receiving CTA of the head and neck with a 128-slice dual-source CT system. Patients were randomized into two groups with a volume of either 40 mL of CM (Group I) or 50 mL of CM (Group II). Arterial and venous enhancements were recorded for quantitative assessment. Qualitative assessments for images without bone removal (BR) were based on a) the visualization of the circle of Willis and b) streak artifacts due to residual CM in the subclavian or internal jugular veins ipsilateral to injection of CM. Qualitative assessment of dual-energy images using BR was based on the presence of bone remnants and vessel integrity. Quantitative data was compared using the Student t test. The chi(2) test was used for the qualitative measurements of streak artifacts in veins while the Mann-Whitney U test was used for the qualitative measurements of images with BR.Results: Arterial and venous attenuation was significantly higher in Group II (P=0.000). Image quality regarding the circle of Willis was excellent in both groups (3.90 +/- 0.30 for Group I and 4.00 +/- 0 for Group II). Imaging of the internal jugular veins was scored higher in Group I (1.87 +/- 0.72) compared with Group II (1.48 +/- 0.51) (P=0.021). Within Group I using BR, mean scores for bone remnants did not differ significantly (P>0.05) but mean scores of vessel integrity (P<0.05) did.Conclusions: Contrast-enhanced head and neck CTA is feasible using a scan protocol with low amounts of contrast medium (40 mL) on a 128-slice dual-energy CTA. The 40-mL protocol provides satisfactory image quality before and after dual-energy bone-removal post-processing.

Abstract:Rationale and Objectives To investigate the feasibility of virtual monochromatic imaging (VMI) of dual-layer spectral detector computed tomography (SDCT) to reduce iodinated contrast material (CM) and radiation dose in craniocervical computed tomography angiography (CTA). Materials and Methods A total of 280 consecutively selected patients performed craniocervical CTA with SDCT were prospectively selected and randomly divided into four groups (A, DoseRight index (DRI) 31, iopromide 370mgI/mL, volume 0.8 mL/kg; B, DRI 26, iopromide 370mgI/mL, volume 0.4 mL/kg; C, DRI 26, ioversol 320mgI/mL, volume 0.4 mL/kg; D, DRI 26, iohexol 300mgI/mL, volume 0.4 mL/kg). 50–70 kiloelectron volts (keV) VMIs in group B were reconstructed and compared to group A to select the optimal keV. Then, the optimal keV in groups B, C and D was reconstructed and compared. Objective image quality, including vascular attenuation, image noise, signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR), was evaluated. Subjective image quality was assessed using a 5-point Likert scale. In addition, the effective dose (ED), iodine load and iodine delivery rate (IDR) were compared between groups A and D. Results 55 keV VMI was the optimal VMI in group B. The objective and subjective image quality of 55 keV VMI in group B were equal to or better than those of the CI in group A. The SNR, CNR and subjective image quality in group D were similar to those in group B (P > 0.05). The ED, iodine load and IDR of group D were reduced by 44%, 59% and 19%, respectively, when compared to those of group A. Conclusion Low dose iodinated CM and radiation for 55 keV VMI in craniocervical CTA using SDCT could still provide equivalent or better image quality than the conventional scanning protocol.

Abstract:OBJECTIVES:To evaluate the radiation dose and image quality of cerebral CT angiography (CTA) at 70kVp with 30mL iodinated contrast agent. METHODS:One hundred patients were prospectively classified into two groups: Group A (n=50), 120kVp cerebral CTA with 60mL iodinated contrast agent reconstructed by filtered back projection (FBP) and Group B (n=50), 70kVp with 30mL iodinated contrast agent reconstructed by sinogram-affirmed iterative reconstruction (SAFIRE). CT values, noise, signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of the internal carotid artery (ICA) and middle cerebral artery (MCA) were measured. Subjective image quality was evaluated. Effective dose (ED) was calculated. RESULTS:The mean CT values of the ICA and MCA of Group B were higher than those of Group A (all P<0.001). The mean noise of Group A was lower than that of Group B (P<0.001). SNRICA, SNRMCA and CNRICA, CNRMCA of Group A were higher than Group B (all P<0.001). There was no difference in vessel sharpness, noise, and overall quality between the two groups (all P>0.05). ED of Group B (0.2±0.0mSv) was lower than Group A (1.3±0.1 mSv) (p<0.001). CONCLUSION:Cerebral CTA with iterative reconstruction at 70kVp and 30mL iodinated contrast agent is feasible, allowing for substantial dose reduction compared with conventional cerebral CTA protocol.
CT Angiography of the Carotid Arteries with Low Kvp and Low Dose Contrast Media

Abstract:Objective:To evaluate image quality and radiation dose of multidetector computed tomography(MDCT) for carotid CT angiography(CTA) with low kVp and low dose contrast media.Methods:Carotid CTA was performed in 30 patients(group A) with 100kVp and low dose contrast media(0.3mL/kgbw).Carotid CTA was performed in another 30 patients(group B) with 120kVp and 60mL contrast media.Radiation exposure parameters(SSDE and DLP) were compared with independent samples t tests.Image quality was assessed and CT attenuation was measured.Results:Both the mean values of SSDE and DLP of group A were significantly lower than those of group B(24.29±0.98)mGy vs(31.77±2.27)mGy(t=-16.60,P0.01);(889.24±62.00)mGy·cm vs(1185.04±140.05)mGy·cm(t=-10.58,P0.01).There was no significant difference between the overall image qualities of the two groups.Conclusion:Carotid CTA with 100kVp and low dose contrast media can provide satisfactory images with significant reduction of radiation exposure and iodine administration simultaneously.
Computed Tomography Angiography of Carotid and Coronary Artery Via a Single-Bolus Injection Protocol: a Feasibility Study Using 320-Row Multidetector CT.

Abstract:Objectives To investigate the feasibility of using a single-dose injection protocol in CTangiography (CTA) of the carotid and coronary artery with 320-row multidetector CT. Methods A total of 82 consecutive patients with suspected carotid artery disease underwent an original CTA protocol aiming at capturing the extra-cranial carotid arteries and coronary arteries simultaneously using 320-row MDCT. The image quality, attenuation, and CNRs of the carotid and coronary arteries were assessed. The lag time (between two separated volumetric acquisitions) was compared between patients with and without cardiac venous opacification (CVO). The contrast medium volume and radiation dose were recorded. Results The image quality was 99.4 % diagnostic in carotid and 86.9% in coronary artery segments. Themean attenuation of carotid and coronary arteries ranged from 462.2 Hu to 533.7 Hu, 415.9 Hu to 454.7 Hu respectively. The mean CNR of the carotid and coronary artery ranged from 15.8 to 18.9 and 17.7 to 20.4 respectively. The lag time in patients with and without CVO was 5.75±1.64 s vs. 4.21±1.14 s (p<0.05). The mean radiation dose was 6.6±4.1 mSv.The mean contrast media volume was 71.9±9.1 ml. Conclusions The carotid and coronary artery can be imaged simultaneously via our original single-dose injection CTA protocol using 320-row CTwith adequate image quality. Key Points • Carotid and coronary 320-row CTA can be achieved in a single-dose injection. • Longer coverage was achieved with two or more volumes using 320-row CT. • The single-dose protocol allows a reduced contrast agent dose of about 72 ml.
Carotid Arteries Angiography with 64-Slice Spiral CT in Low Dosage of the Contrast Media

Abstract:Objective To investigate the application of low dosage of the contrast media in carotid arteries angiography with 64-slice spiral CT.Methods According to the contrast dosage,70 patients undergoing carotid CT angiography(CTA).The patients were randomly divided into 2 groups:one group was treated with 50 ml contrast agent and 20 ml saline flush,the other group was administrated with 70 ml contrast agent.With double blindness method,the data were respectively evaluated and analyzed by two radiologists independently.Results A good agreement was reached for two viewers,Kappa value was 0.082.The percentages of excellent image quality at low and normal contrast dose level were 88.6%(31/35),62.9%(22/35) respectively.The differences of image quality between the two groups had statistical significance(Z=-3.236,P=0.001).Conclusion Good CTA image quality can be acquired with reduced contrast dose,thereby we can offer reliable diagnostic imformation for the clinicians.
Study on Imaging Parameters of Multi-Slice Spiral CT Long Coverage Angiography in Intracranial and Cervical Arteries

Abstract:Objective To explore application of the bolus-triggered technique and optimal match between collimation and pitch of MSCTA in intracranial and cervical arteries.Methods(1)The small-dose tests were performed in 19 healthy volunteers and the theoretical threshold was obtained by the time-density curve.(2)forty healthy volunteers were divided randomly into two groups and the scanning parameters were as follows.Group A:collimation 1.0 mm,pitch 1.750. Group B:collimation 2.5 mm,pitch 0.625. Statistical significance was determined with the X~2 test and t test(α=0.05).Results(1)In 19 volunteers,CT value 4 seconds before the peak was 75 HU,the CT value at the beginning segment of the carotid artery and the C_1 segment of internal carotid artery was in accordance with the standard.(2)Image quality ofⅠ,Ⅱgrades artery structures of group B was superior to that of group A.Image quality ofⅢ,Ⅴ grades artery structures of group A was superior to that of group B.Conclusions(1)MSCTA in intracranial and cervical arteries can display systematically the cerebral and carotid arteries.(2)The bolus-triggered technique can improve the image quality of the target vessels.The image quality of the MSCTA of intracranial and cervical arteries is better with the threshold of 75 HU on the basis of 3.5 ml/s injection rate.(3)On the basis of the same other parameters,the optimal scanning parameters are a collimation of 1.0 mm with a pitch of 1.750.
Feasibility of Ultra-low Radiation and Contrast Medium Dosage in Aortic CTA Using Deep Learning Reconstruction at 60 kVp: An Image Quality Assessment

Abstract:Objective To assess the viability of using ultra-low radiation and contrast medium (CM) dosage in aortic computed tomography angiography (CTA) through the application of low tube voltage (60 kVp) and a novel deep learning image reconstruction algorithm (ClearInfinity, DLIR-CI). Methods Iodine attenuation curves obtained from a phantom study informed the administration of CM protocols. Non-obese participants undergoing aortic CTA were prospectively allocated into two groups and then obtained three reconstruction groups. The conventional group (100 kVp-CV group) underwent imaging at 100 kVp and received 210 mg iodine/kg in combination with a hybrid iterative reconstruction algorithm (ClearView, HIR-CV). The experimental group was imaged at 60 kVp with 105 mg iodine/kg, while images were reconstructed with HIR-CV (60 kVp-CV group) and with DLIR-CI (60 kVp-CI group). Student's t-test was used to compare differences in CM protocol and radiation dose. One-way ANOVA compared CT attenuation, image noise, SNR, and CNR among the three reconstruction groups, while the Kruskal–Wallis H test assessed subjective image quality scores. Post hoc analysis was performed with Bonferroni correction for multiple comparisons, and consistency analysis conducted in subjective image quality assessment was measured using Cohen's kappa. Results The radiation dose (1.12 ± 0.23 mSv vs. 2.03 ± 0.82 mSv) and CM dosage (19.04 ± 3.03 mL vs. 38.11 ± 6.47 mL) provided the reduction of 45% and 50% in the experimental group compared to the conventional group. The CT attenuation, SNR, and CNR of 60 kVp-CI were superior to or equal to those of 100 kVp-CV. Compared to the 60 kVp-CV group, images in 60 kVp-CI showed higher SNR and CNR (all P < 0.001). There was no difference between the 60 kVp-CI and 100 kVp-CV group in terms of the subjective image quality of the aorta in various locations (all P > 0.05), with 60 kVp-CI images were deemed diagnostically sufficient across all vascular segments. Conclusion For non-obese patients, the combined use of 60 kVp and DLIR-CI algorithm can be preserving image quality while enabling radiation dose and contrast medium savings for aortic CTA compared to 100 kVp using HIR-CV.

Evaluation of a High Concentrated Contrast Media Injection Protocol in Combination with Low Tube Current for Dose Reduction in Coronary Computed Tomography Angiography: A Randomized, Two-center Prospective Study.

Abstract:Rationale and Objectives: The study aimed to prospectively evaluate the radiation dose reduction potential and image quality (IQ) of a high-concentration contrast media (HCCM) injection protocol in combination with a low tube current (mAs) in coronary computed tomography angiography. Materials and Methods: Eighty-one consecutive patients (mean age: 62 years; 34 females; body mass index: 18-31) were included and randomized-assigned into two groups. All computed tomography (CT) examinations were performed in two groups with the same tube voltage (100 kV), flow rate of contrast medium (5.0 mL/s), and iodine dose (22.8 g). An automatic mAs and low concentration contrast medium (300 mgl/mL) were used in group A, whereas effective mAs was reduced by a factor 0.6 along with HCCM (400 mgl/mL) in group B. Radiation dose was assessed (CT dose index [CTD1,01] and dose length product), and vessel-based objective IQ for various regions of interest (enhancement, noise, signal-to-noise ratio, and contrast-to-noise ratio), subjective IQ, noise, and motion artifacts were analyzed overall and vessel-based with a 5-point Likert scale. Results: The CT attenuation of coronary arteries and image noise in group B were significantly higher than those in, group A (ranges: 507.5-548.1 Hounsfield units vs 407.5-444.5 Hounsfield units; and 20.3 +/- 8.6 vs 17.7 +/- 8.0) (P <= 0.0166), There was no significant difference between the two groups in signal-to-noise ratio, contrast-to-noise ratio, and subjective IQ of coronary arteries (29.4-31.7, 30.0-37.0, and medium score of 5 in group A vs 29.4-32.4, 27.7-36.3, and medium score of 5 in group B, respectively, P >= 0.1859). Both mean CTDI,, and dose length product in group B were 58% of those of group A. Conclusions: HCCM combined with low tube current allows dose reduction in coronary computed tomography angiography and does not compromise IQ. (C) 2017 The Association of University Radiologists. Abstract:To assess the impact of low-dose contrast media (CM) injection protocol with deep learning image reconstruction (DLIR) algorithm on image quality in coronary CT angiography (CCTA). In this prospective study, patients underwent CCTA were prospectively and randomly assigned to three groups with different contrast volume protocols (at 320mgI/mL concentration and constant flow rate of 5ml/s). After pairing basic information, 210 patients were enrolled in this study: Group A, 0.7mL/kg (n = 70); Group B, 0.6mL/kg (n = 70); Group C, 0.5mL/kg (n = 70). All patients were examined via a prospective ECG-triggered scan protocol within one heartbeat. A high level DLIR (DLIR-H) algorithm was used for image reconstruction with a thickness and interval of 0.625mm. The CT values of ascending aorta (AA), descending aorta (DA), three main coronary arteries, pulmonary artery (PA), and superior vena cava (SVC) were measured and analyzed for objective assessment. Two radiologists assessed the image quality and diagnostic confidence using a 5-point Likert scale. The CM doses were 46.81 ± 6.41mL, 41.96 ± 7.51mL and 34.65 ± 5.38mL for Group A, B and C, respectively. The objective assessments on AA, DA and the three main coronary arteries and the overall subjective scoring showed no significant difference among the three groups (all p > 0.05). The subjective assessment proved that excellent CCTA images can be obtained from the three different contrast media protocols. There were no significant differences in intracoronary attenuation values between the higher HR subgroup and the lower HR subgroup among three groups. CCTA reconstructed with DLIR could be realized with adequate enhancement in coronary arteries, excellent image quality and diagnostic confidence at low contrast dose of a 0.5mL/kg. The use of lower tube voltages may further reduce the contrast dose requirement.

Abstract:Objective To evaluate the feasibility of low-tube-voltage, low injection rate, low contrast agent dosage in combination with iterative model reconstruction ( IMR) algorithm in CT angiography ( CTA) of the head-and-neck vessels .Methods Sixty patients who underwent CT angiography of the head-and-neck vessels were randomly divided into groups A and B with 30 cases in each group .Patients in group A received a conventional scan with 120 kVp and filterback projected ( FBP) reconstruction .Patients in group B received a low-dose scan with 80 kVp, and image reconstruction with FBP ( group B1) and IMR (group B2)algorithm.The contrast agent protocol were as follows: the injection time in all patients was 10s, the injection rate was 4.5-5.5 ml/s in group A while 3.5-4.0 ml/s in group B.The CT values of artery, image noise, signal to noise ratio ( SNR) and contrast to noise ratio ( CNR) were measured and compared among three groups with One-way ANOVA analysis . Image quality was evaluated by two radiologists with five scale method , and compared with Kruskal-Wallis test .The CT dose index volume (CTDIvol) and dose length product ( DLP) were recorded and compared between groups with two independent samples t-test.Results The image quality scores of groups A , B1and B2 were 3-5, 2-4 and 3 -5, respectively .Image quality of twelve patients in group B 1 couldn′t meet the diagnostic requirements but none in group A and B 2.The objective image parameters SNR and CNR for group B 2 were equal to group A ( P >0.05), while those for group B1 were lower than group A (t=13.39, 9.45, P<0.05) and group B2 (t=-12.14, -9.96,P<0.05).CTDIvol and DLP for group B were separately 80.9%, 81.3%lower than those of group A(t=39.1, 32.2,P<0.05).The injection rate and contrast agent volume for group B were separately 22.0%, 22.1% lower than those of group A ( t=20.8, 20.8, P<0.01) .Conclusions It is feasible in CT angiography of the head-and-neck vessels with lower tube-voltage, lower injection rate, lower contrast agent dose and combining with iterative model reconstruction algorithm.This protocol can reduce the radiation dose by 81.3% while maintaining image quality .Trial registration Chinese clinical trial registry ,ChiCTR-BOC-16010060.

Abstract:Objective: To investigate the feasibility of low-tube-voltage (80kVp) coronary CT angiography (CCTA) combined with contrast medium (CM) reduction and iterative model reconstruction (IMR) on patients with standard body mass index compared with clinical routine protocol.Methods: Retrospectively gated helical CCTA scans were acquired using a 256-slice multi-slice CT (Brilliance iCT; Philips Healthcare, Cleveland, OH) on 94 patients with standard body mass index (20-25 kgm(-2)) who were randomly assigned into 2 groups. The scan protocol for Group 1 was 100kVp and 600mAs with 70 ml CM at an injection rate of 4.5-5.5 mls(-1); images were reconstructed by a hybrid iterative reconstruction technique (iDose(4); Philips Healthcare). Group 2 was scanned at 80kVp and 600mAs with 35 ml CM at an injection rate of 3.5-4.5 mls(-1); images were reconstructed with IMR. Objective measurements such as the mean image noise and contrast-to-noise ratio of the two groups were measured on CT images and compared using the paired t-test. In addition, a subjective image quality evaluation was performed by two radiologists who were blinded to the scan protocol, using a 5-point scale [1 (poor) to 5 (excellent)]. The results of the two groups were compared using Mann-Whitney U test.Results: The iodine delivery rate of Group 2 was 1.0 +/- 0.5 gls(-1) compared with 2.1 +/- 0.5 gls(-1) in Group 1 resulting in a reduction of 52.4%. In addition, an effective radiation dose reduction of 56.4% was achieved in Group 2 (2.4 +/- 1.2mSv) compared with Group 1 (5.5 +/- 1.4mSv). The mean CT attenuation, contrast-to-noise ratio and image quality of all segments in Group 2 were significantly improved compared with those in Group 1 (all, p<0.01).Conclusion: The use of IMR along with a low tube voltage (80kVp) combined with a low CM protocol for CCTA can reduce both radiation and CM dose with improved image quality.Advances in knowledge: In this study, we used a novel knowledge-based IMR which remarkably reduced the image noise. We compared the quality of the images obtained when the tube voltage was reduced to 80 kVp and that of those obtained according to the clinical routine protocols to determine whether ultra-low-dose imaging plus IMR is feasible in CCTA scans. We found that a low dose protocol combined with 80 kVp and reduced CM for CCTA can reduce both radiation dose and CM dosewith improved image quality by the use of IMR in non-obese patients.

Abstract:Objective To evluate the feasibility of the lower tube voltage combined with low concentration contrast agent iodixanol(270 mg I/mL)in coronary CTA with 640-slice volumetric CT.Methods89 patients receiving coronary CTA examination were randomly divided into two groups.Group A(44 cases)received iodixanol(320 mgI/mL)combined with 120 kV,while group B(45 cases)received iodixanol(270 mg I/mL)combined with 100 kV.Subjective evaluation was done by two experienced doctors of cardiovascular imaging diagnosis by five-point method.CT value,image noise,CNR,SNR and effective radiation dose were calculated to do objective evaluation.Results The CT value of aortic root in group A was significantly higher than that in group B,but the CT value in both groups were higher than250 HU,which could meet the diagnostic requirements;The CT value of rest each vascular segment in group B was higher than those in group A;CNR and SNR of aortic root in two groups were of statistically significant difference(P<0.05),while CNR and SNR of the rest each vascular segment in two groups were of no statistically significant difference(P>0.05);effective radiation dose in group B was significantly lower than that in group A,and the difference was statistically significant(P<0.05).Conclusion In coronary CTA with 640-slice volumetric CT,the application of iodixanol(270 mg I/mL)combined with the lower tuber voltage can get a better image quality that could meet the diagnostic requirements,reduce the risk of CIN,and significantly lower the radiation dose at the same time,so the feasibility is better.
Feasibility of CT Angiography for the Renal Arteries with Low Dose Contrast Material in the Different Injection Condition

Abstract:Objective To investigate the feasibility of 64-slice spiral CT angiography (CTA) with low dose CM for the renal arteries in the different injection condition.Materials and Methods Dynamic CT scan was performed at the same abdominal aorta level in 10 volunteers for measuring the CT value and the theoretical threshold was obtained by the time-density curve (TDC).CTA of the renal artery were performed on 80 patients suspected with renal artery lesions.80 patients were divided randomly into two groups(A and B,with 40 patients in each group)according to the inject method of CM,and then the dose of CM,CT attenuation values of the abdominal aortic artery (AAA) and bilateral renal arteries (RA) were measured for each group and the image quality was also recorded as excellent,good,and poor.Results The dose of group B was less than group A .CT attenuation values of both the AAA and RA were over 200 HU,and no significant difference between two groups was found.The image quality of renal arteries in two groups was well depicted as excellent or good and also was no significant difference.Conclusion It is feasible to use low dose CM in 64-slice spiral CTA for the renal arteries in the different injection condition.
Dual Energy CT Intracranial Angiography: Image Quality, Radiation Dose and Initial Application Results

Abstract:Objective To assess the clinical value of dual-energy intracranial CT angiography (CTA).Methods Forty-one patients suspected of intracranial vascular diseases underwent dual-energy intracranial CT angiography, and 41 patients who underwent conventional subtraction CT were enrolled as the control group.Image quality of intracranial and skull base vessels and radiation dose between dual-energy CTA and conventional subtraction CTA were compared using two independent sample nonparametrie test and independent-samples t test, respectively.Prevalence and size of lesions detected by dual-energy CTA and digital subtraction CTA were compared using paired-samples t test and Spearman correlative analysis. Results The percentage of image quality scored 5 was 70.7% (29/41) for dual-energy CTA and 75.6% (31/41) for conventional subtraction CTA.There was no significant difference between the two groups(Z= -0.455, P=0.650).Image quality of vessels at the skull base in conventional subtraction CTA was superior to that in dual-energy CTA, especially for the petrosal and syphon segment (Z=-4.087, P= 0.000).Radiation exposure of dual energy CTA and conventional CTA were (396.54±17.43) and (1090.95±114.29) mGy · cm respectively.Radiation exposure was decreased by 64% (t=-38.52, P=0.000) by dual energy CTA compared with conventional subtraction CTA.Out of the 41 patients,19 patients were diagnosed as intracranial aneurysm, 2 patients as arteriovenous malformation (AVM), 3 patients with Moya-moya's disease, and the remaining 17 patients with negative results.Nine patients with intracranial aneurysm, 2 patients with AVM, 3 patients with Moya-moya's disease, and 2 patients with negative findings underwent DSA or operation, with concordant findings from both techniques.Diameter of aneurysm neck, long axis and minor axis by dual-energy CTA was (2.90±1.61), (5.23±1.68) and (3.83±1.69) nun, respectively; Diameter of aneurysm neck, long axis and minor axis by DSA was (2.95±1.71), (5.10±1.60) ,(3.83±1.65) nan,respectively.There was no significant difference for the diameters of aneurysm between dual energy CTA and DSA ((t=-0.734,1.936,0.12.5 respectively, P=0.482,0.085,0.903 respectively), and good correlation was found between diameter measurements using the two techniques(r=0.964,0.976,0.973, respectively, all P=0.000) Conclusions Compared with conventional subtraction CTA, dual energy CTA has good image quality for intracranial vessels; however, image quality of the skull base vessels is worse, especially for the petrosal and syphon segment.Dual energy CTA has decreased radiation dose and a high diagnostic accuracy, being a practical imaging madality for diagnosis of intracranial vascular lesions.

Abstract:SummaryThe effect of low voltage and low concentration contrast agent on image quality of coronary CT angiography, radiation dose and iodine intake was evaluated. A total of 121 patients with body mass index (BMI) <26 kg/m2 and heart rate (HR) <70 beats/min were randomly divided into four groups: group A (n=31, 80 kVp, 270 mgI/mL); group B (n=33, 100 kVp, 270 mgI/mL); group C (n=30, 100 kVp, 320 mgI/mL); group D (n=27, 100 kVp, 400 mgI/mL). The automatic current modulation system and the iterative algorithm for reconstruction were adopted in each group. The CT values and SD values of the aortic root (AR), subcutaneous fat, left coronary artery opening (LCA), and right coronary artery opening (RCA) were measured in all groups, the signal-to-noise ratio (SNR) and contrast noise ratio (CNR) were calculated, and effective radiation dose and iodine intake were recorded. The subjective assessment for image quality was performed by two physicians using a 4-point scale. The results were compared using the one-way ANOVA and rank sum tests. The image quality of the four groups met the clinical diagnostic requirements. The CT values of AR in groups A, B, C, and D were 537.6±71.4, 447.2±81.9, 445.2±64.9 and 518.5±94.9 Hu, respectively, with no significant difference between group A and group D, or between group B and group C, while CT values in groups B and C were significantly lower than those in groups A and D (P<0.05). In groups A, B, C, and D, the LCA SNR values were 22.7±9.1, 23.3±9.1, 23.3±7.7 and 26.6±8.9, and the RCA CNR values were 26.9±9.8, 28.5±11.4, 27.7±8.8 and 32.1±10.6, respectively. The AR visual scores in groups A, B, C and D were 3.8±0.2, 3.9±0.3, 3.9±0.3 and 4.0±0.3, respectively. There were no significant differences in SNR, CNR and visual score among the four groups (P>0.05). The radiation doses in groups A, B, C and D were 2.6±1.4, 3.6±1.8, 4.9±3.5 and 4.9±2.8 mSv, respectively. The radiation dose in group A was significantly less than that in the rest three groups (P<0.05). The iodine intakes in groups A, B, C and D were 14.9±1.5, 15.0±1.5, 17.7±2.0 and 18.1±2.5 g, respectively. There was no significant difference in the intake of iodine between groups C and D, or between groups A and B, while iodine intake in groups A and B were significantly reduced as compared with that in groups C and D (P<0.05). It was concluded that for patients with low BMI and controlled HR, compared to 100 kVp tube voltage combined with multiple concentration contrast agents, 80 kVp combined with 270 mgI/mL contrast agent is enough to ensure the quality of the images, and can reduce the radiation dose significantly, while reducing the amount of iodine intake notably, thus reducing the incidence of adverse reaction.

Abstract:Abstract We evaluated the feasibility and image quality of prospective electrocardiography (ECG)-triggered coronary computed tomography angiography (CCTA) using a body surface area (BSA) protocol for contrast-medium (CM) administration on both second- and third-generation scanners (Flash and Force CT), without using heart rate control. One-hundred-and-eighty patients with suspected coronary heart disease undergoing CCTA were divided into groups A (BSA protocol for CM on Flash CT), B (body mass index (BMI)-matched patients; BMI protocol for CM on Flash CT), and C (BMI-matched patients; BSA protocol for CM on Force CT). Patient characteristics, quantitative and qualitative measures, and radiation dose were compared between groups A and B, and A and C. Of the 180 patients, 99 were male (median age, 62 years). Average BSA in groups A, B, and C was 1.80 ± 0.17 m 2 , 1.74 ± 0.16 m 2 , and 1.64 ± 0.17 m 2 , respectively, with groups A and C differing significantly ( P < 0.001). Contrast volume (50.50 ± 8.57 mL vs. 45.00 ± 6.18 mL) and injection rate (3.90 ± 0.44 mL/s vs. 3.63 ± 0.22 mL/s) differed significantly between groups A and C ( P 250 HU, average scores > 4) achieved slightly lower diagnostic image quality than group B. The BSA protocol for CM administration was feasible in both Flash and Force CT, and therefore may be valuable in clinical practice.
