Abstract:Abstract In recent years, a growing interest has arisen regarding the possible relationship between adverse pregnancy outcomes (APOs) and inadequate maternal hemodynamic adaptations to the pregnancy. A possible association between “placental syndromes,” such as preeclampsia (PE) and fetal growth restriction (FGR), and subsequent maternal cardiovascular diseases (CVD) later in life has been reported. The two subtypes of FGR show different pathogenetic and clinical features. Defective placentation, due to a poor trophoblastic invasion of the maternal spiral arteries, is believed to play a central role in the pathogenesis of early-onset PE and FGR. Since placental functioning is dependent on the maternal cardiovascular system, a pre-existent or subsequent cardiovascular impairment may play a key role in the pathogenesis of early-onset FGR. Late FGR does not seem to be determined by a primary abnormal placentation in the first trimester. The pathological pathway of late-onset FGR may be due to a primary maternal cardiovascular maladaptation: CV system shows a flat profile and remains similar to those of non-pregnant women. Since the second trimester, when the placenta is already developed and increases its functional request, a hypovolemic state could lead to placental hypoperfusion and to an altered maturation of the placental villous tree and therefore to an altered fetal growth. Thus, this review focalizes on the possible relationship between maternal cardiac function and placentation in the development of both early and late-onset FGR. A better understanding of maternal hemodynamics in pregnancies complicated by FGR could bring various benefits in clinical practice, improving screening and therapeutic tools.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>Predicting fetal growth restriction (FGR) is a challenge and recent international guidelines by the Society of Maternal Fetal Medicine (SMFM) and the International Society of Ultrasound in Obstetrics and Gynecology (ISUOG) differ in their definitions of FGR. Our objective was to compare the prediction of small for gestational age (SGA) and a composite neonatal outcome using the SMFM and ISUOG definitions.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>This was a secondary analysis using data from a prospective study of women referred for growth ultrasound examinations. SMFM and ISUOG practice guidelines were used to define the population of patients with FGR. SMFM guidelines defined FGR as EFW or AC less than 10<sup>th</sup> percentile. ISUOG guidelines included EFW or AC less than 3<sup>rd</sup> percentile with various Doppler parameters including umbilical, uterine and middle cerebral arteries. The primary outcome was the prediction of neonatal SGA and a composite of neonatal morbidity and mortality by the two guidelines. The composite of neonatal morbidity was defined as Grade 3 and 4 interventricular hemorrhage, respiratory distress syndrome, neonatal death, pH<7.1, seizure, NICU admission. Test characteristics (sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and positive (LR+) and negative (LR‐) likelihood rations) and area under the ROC curve (AUC) were determined. The association between the FGR detected by each guideline and adverse neonatal outcomes were assessed using logistic regression. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Of 1054 women meeting inclusion criteria, 137 (13.0%) and 80 (7.6%) were defined as having FGR by SMFM and ISUOG definitions, respectively. For prediction of SGA, the SMFM FGR definition had a higher sensitivity (54.7%) compared to the ISUOG FGR definition (38.1%). The ISUOG FGR definition had the highest specificity (97.1%). LR+ was highest for the ISUOG FGR definition. The SMFM and ISUOG FGR definitions were similarly poor in predicting a composite of neonatal morbidity with sensitivities of 15.1% and 11.5%, respectively.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>The SMFM guideline is associated with a higher detection rate for SGA but at the cost of some reduction in specificity. Abstract:Background Fetal growth restriction (FGR) is defined differently by the American College of Obstetrics and Gynecology (ACOG), Society for Maternal-Fetal Medicine (SMFM), and the International Society of Ultrasound in Obstetrics and Gynecology (ISUOG). Objective The purpose of this study was to investigate the predictive effect of three different diagnostic criteria of FGR on small-for-gestational-age (SGA) and adverse neonatal outcome (ANO), so as to find a better FGR-definition for Chinese population. Study Design The clinical data of singleton pregnancy who received regular pregnancy care and gave birth at the Department of Obstetrics and Gynecology of Peking University First Hospital from January 1, 2021, to June 30, 2021 were collected. FGR cases were determined according to three different diagnostic criteria by ACOG, SMFM, or ISUOG. The primary outcome was the prediction of SGA and a composite ANO. SGA was defined as neonatal birth weight less than 10 percentile. An ANO included one of these adverse outcomes: neonatal umbilical arterial blood pH < 7.1, 5-minute Apgar score<7, acute respiratory distress syndrome (ARDS), intraventricular hemorrhage, and neonatal convulsion, and transfer to the neonatal intensive care unit (NICU). The specificity, sensitivity, negative predictive value (NPV) and positive predictive value (PPV) of the different diagnostic criteria for SGA and ANO were compared. The discriminatory capacities of the three FGR-definitions were compared using the area under receiver-operating-characteristics curves (AUC). Results A total of 2340 cases were included in this study, and 115 (4.9%), 63 (2.7%), and 48 (2.1%) cases of FGR were diagnosed using the diagnostic criteria issued by the SMFM, ACOG, and ISUOG respectively. There were 147 (6.28%) cases of SGA neonates. The SMFM criteria had higher sensitivity (40.82% vs 24.49%, 20.41%) for SGA compared to the ACOG and ISUOG criteria. On the contrary, the ISUOG criteria had a higher specificity (99.18% vs 97.49%, 98.77%) than the SMFM and ACOG criteria for predicting SGA. A total of 127 (5.43%) cases were complicated with ANO. All three definitions had low sensitivity for ANO (17.32%, 12.6%, 11.81%) and high specificity of 95.8%, 97.88% and 98.51%. The AUCs of SMFM for predicting SGA (0.692) and ANO (0.566) were slightly higher than those of ACOG (0.616, 0.552) and ISUOG (0.598, 0.552). Conclusion The predictive value of the SMFM and ISUOG definition for SGA and ANO was better than that of the ACOG criteria. In Chinese population, the discriminatory capacities of SMFM FGR definition were superior compared to the other two definitions.

Abstract:BACKGROUND: Criteria for diagnosis of fetal growth restriction (FGR) differ widely according to national and international guidelines and further heterogeneity arises from the use of different biometric and Doppler reference charts, making the diagnosis of FGR highly variable.OBJECTIVE(S): To compare FGR definitions by Delphi consensus criteria and Society for Maternal Fetal Medicine (SMFM) definition, using different standards/charts for fetal biometry and different reference ranges for Doppler velocimetry parameters.STUDY DESIGN: From the TRUFFLE 2 feasibility study (856 women with singleton pregnancy 32<sup>+0</sup> to 36<sup>+6</sup> weeks of gestation at risk of FGR) we selected 564 women with available mid-pregnancy biometry. For the comparison we used standards/charts for estimated fetal weight (EFW) and abdominal circumference (AC) from Hadlock, Intergrowth, GROW and Chitty. Percentiles for umbilical artery pulsatility index (PI) and its ratios with middle cerebral artery PI were calculated using Arduini and Ebbing reference charts. Sensitivity and specificity for low birthweight and adverse perinatal outcome were evaluated.RESULTS: Different combinations of definitions and reference charts identified substantially different proportions of fetuses within our population as FGR varying from 38% (with Delphi consensus definition, Intergrowth biometric standards and Arduini Doppler reference ranges) to 93% (with SMFM definition and Hadlock biometric standards). None of the different combinations tested appeared effective, with relative risk for birthweight below the 10<sup>th</sup> percentile between 1.4 to 2.1. Birthweight below the 10<sup>th</sup> percentile was observed most frequently when selection was made with the GROW/Chitty charts, slightly less using the Hadlock standard, and lowest with the Intergrowth standard. Using the Ebbing Doppler reference ranges resulted in a far higher proportion identified as FGR than with the Arduini Doppler reference ranges, while Delphi consensus definition with Ebbing Doppler reference ranges gave similar results to the SMFM definition. Delphi consensus definition using Arduini Doppler reference ranges were significantly associated with adverse perinatal outcome, with any biometric standards/charts. SMFM definition could not accurately detect adverse perinatal outcome irrespective of EFW standard/chart used.CONCLUSION(S): Different combinations of FGR definitions, biometry standards/charts and Doppler reference ranges select different proportions of fetuses categorized as FGR. The difference in adverse perinatal outcome may be modest, however these differences will have a significant impact in terms of rate of intervention.

Abstract:OBJECTIVE: To compare perinatal outcomes on fetuses classified as stage I late-onset fetal growth restriction (FGR) depending on the estimated fetal weight (EFW) centile category and the fetal and maternal Doppler study.MATERIAL AND METHODS: Retrospective cohort study on 131 cases of stage I late-onset FGR (diagnosis ≥32+0 weeks), defined as: EFW < 3rd centile and normal Doppler (G1) or EFW < 10th centile and mean uterine artery pulsatility index (PI) > 95th centile (G2) or EFW < 10th centile and mild fetal Doppler alteration: umbilical artery PI > 95th centile, middle cerebral artery PI < 5th centile, or cerebroplacental ratio < 5th centile (G3). All groups were compared to their perinatal results.RESULTS: There were 37, 30, and 64 cases in G1, G2, and G3, respectively. G1 and G2 showed lower percentages of cesarean section when compared with G3 (18.4, 22.5, and 45.3% (p < 0.01), respectively), being attributable to an excess of cesarean sections for non-reassuring fetal status. These differences remained when definitive birth weight centile was above that considered to define FGR, being 5.9, 12.5, and 41.8% (p < 0.01), respectively.DISCUSSION: In stage I late-onset FGR fetuses, abnormal fetal Doppler is associated with a poorer tolerance to vaginal delivery, even when the birth weight is > 10th centile.

Abstract:Background: The purpose of this study was to describe the placental lesions in pregnancies complicated by hypertensive disorders (HDP) and/or fetal growth restriction (FGR) and in uneventful control pregnancies. Methods: This is a case control study that included singleton pregnancies with HDP and normally grown fetus (HDP-AGA fetus), with HDP and FGR, early FGR, late FGR, and uneventful pregnancies. Feto-placental Doppler velocimetry and sFlt-1/PlGF ratio were performed. Placental histology was evaluated blinded according to the Amsterdam Consensus criteria. Results: Placental lesions with maternal vascular malperfusion (MVM) were significantly more frequent in HDP-FGR and early FGR (92% and 83%). MVM were significantly associated with abnormal feto-placental Doppler parameters, especially in early FGR. Delayed villous maturation (DVM) was associated with late FGR (83%). HDP-AGA fetus cases presented a heterogeneous pattern of placental lesions, including 60% of cases with MVM, but were not associated with abnormal Doppler feto-placental velocimetry. Conclusions: We found a prevalence of placental maternal vascular malperfusion in HDP-FGR and early FGR groups. These lesions were also associated with abnormal, anti-, and angiogenic markers. Conversely HDP-AGA fetus and late FGR presented more heterogeneous placental lesions not severe enough to cause feto-placental Doppler anomalies. These conditions are likely associated with different etiologies, such as maternal pre-pregnancy risk factors for metabolic syndrome. These findings suggest a possible preventive nutritional approach in addition to low-dose aspirin in pregnant women with predisposing factors for HDP-AGA fetuses and late FGR.

Abstract:Abstract Objective To evaluate the association between early-onset fetal growth restriction (FGR), late-onset FGR, small for gestational age (SGA) and adequate for gestational age (AGA) fetuses and adverse perinatal outcomes. Methods This was a retrospective longitudinal study in which 4 groups were evaluated: 1 — early-onset FGR (before 32 weeks) (n = 20), 2 — late-onset FGR (at or after 32 weeks) (n = 113), 3 — SGA (n = 59), 4 — AGA (n = 476). The Kaplan-Meier curve was used to compare the time from the diagnosis of FGR to birth. Logistic regression was used to determine the best predictors of adverse perinatal outcomes in fetuses with FGR and SGA. Results A longer time between the diagnosis and birth was observed for AGA than for late FGR fetuses (p < 0.001). The model including the type of FGR and the gestational age at birth was significant in predicting the risk of hospitalization in the neonatal intensive care unit (ICU) (p < 0.001). The model including only the type of FGR predicted the risk of needing neonatal resuscitation (p < 0.001), of respiratory distress (p < 0.001), and of birth at < 32, 34, and 37 weeks of gestation, respectively (p < 0.001). Conclusion Fetal growth restriction and SGA were associated with adverse perinatal outcomes. The type of FGR at the moment of diagnosis was an independent variable to predict respiratory distress and the need for neonatal resuscitation. The model including both the type of FGR and the gestational age at birth predicted the risk of needing neonatal ICU hospitalization.
Placental characteristics in different types of selective fetal growth restriction in monochorionic diamniotic twins

Abstract:Introduction: This study aimed to explore the differences in placental characteristics among three types of selective fetal growth restriction (sFGR) in monochorionic diamniotic twin pregnancies. Material and methods: A total of 123 placentas with sFGR between April 2013 and October 2019 were retrospectively analyzed after dye injection. Placental characteristics were compared among the three types. Results: The gestational age at diagnosis and delivery was less in sFGR II and III than in sFGR I (22.9 [21.7-33.6], 23.3 [20.0-26.1] and 25.7 [19.0-35.0] weeks, p < 0.001; 32.3 [31.6-35.1], 34.1 [29.9-34.7] and 35.5 [34.0-37.0] weeks, p < 0.001). The birthweight discordance ratio was less in sFGR I than in sFGR II (0.28 [0.14-0.43] and 0.30 [0.23-0.37], p < 0.001). The prevalence of a thick artery-artery anastomosis was higher in sFGR III than in sFGR I or II (81.8%, 44.9% and 48.6%, p = 0.010). The placental territory discordance ratio was higher in sFGR II and III than in sFGR I (0.60 [0.17-0.88], 0.60 [0.01-0.80] and 0.50 [0.01-0.71], p = 0.001). Conclusions: Compared with sFGR I, the earlier onset time of sFGR II and III might be due to their higher placental territory discordance. The prevalence of thick artery-artery anastomoses was expected to be higher in sFGR III than in sFGR I or II.
Intended delivery mode and neonatal outcomes in pregnancies with fetal growth restriction

Abstract:<b>Objective:</b> To compare neonatal outcomes in pregnancies with fetal growth restriction (FGR) by intended delivery mode.<b>Methods:</b> This is a retrospective cohort study of singleton pregnancies with FGR that were delivered ≥34.0 weeks gestation. Neonatal outcomes were compared according to the intended delivery mode, which the attending obstetrician determined. Of note, none of the subjects had a contraindication to labor. Crude and adjusted odds ratios (ORs) and corresponding confidence intervals (CIs) were calculated <i>via</i> logistic regression models to assess the potential association between intended delivery mode and neonatal morbidity defined as a composite outcome (i.e. umbilical artery pH ≤7.1, 5-min Apgar score ≤7, admission to the neonatal intensive care unit, hypoglycemia, intrapartum fetal distress requiring expedited delivery, and perinatal death). A sensitivity analysis excluded intrapartum fetal distress requiring emergency cesarean delivery from the composite outcome since only patients with spontaneous labor or labor induction could meet this criterion. Potential confounders in the adjusted effects models included maternal age, body mass index, hypertensive disorders, diabetes, FGR type (i.e. early or late), and oligohydramnios.<b>Results:</b> Seventy-two (34%) patients had an elective cesarean delivery, 73 (34%) had spontaneous labor and were expected to deliver vaginally, and 67 (32%) underwent labor induction. The composite outcome was observed in 65.3%, 89%, and 88.1% of the groups mentioned above, respectively (<i>p</i> < 0.001). Among patients with spontaneous labor and those scheduled for labor induction, 63% and 47.8% required an emergency cesarean delivery for intrapartum fetal distress. Compared to elective cesarean delivery, spontaneous labor (OR 4.32 [95% CI 1.79, 10.42], <i>p</i> = 0.001; aOR 4.85 [95% CI 1.85, 12.66], <i>p</i> = 0.001), and labor induction (OR 3.92 [95% CI 1.62, 9.49] <i>p</i> = 0.002; aOR 5.29 [95% CI 2.01, 13.87], <i>p</i> = 0.001) had higher odds of adverse neonatal outcomes.<b>Conclusion:</b> In this cohort of FGR, delivering at ≥34 weeks of gestation, pregnancies with spontaneous labor, and those that underwent labor induction had higher odds of neonatal morbidity than elective cesarean delivery.

The time of prenatal follow-up of early- and late-onset FGR cases was similar, but the degree of prematurity of the former made the evolution more unfavorable.Early-onset FGR had a lower prevalence but was associated with higher maternal and fetal morbidity and mortality than late-onset FGR.</span>
An integrated approach to fetal growth restriction

Abstract:Fetal growth restriction (FGR) is among the most common complications of pregnancy. FGR is associated with placental insufficiency and poor perinatal outcomes. Clinical management is challenging because of variability in clinical presentation. Fetal smallness (estimated fetal weight <10th centile for gestational age) remains the best clinical surrogate for FGR. However, it is commonly accepted that not all forms of fetal smallness represent true FGR. In a significant subset of small fetuses, there is no evidence of placental involvement, perinatal outcomes are nearly normal, and they are clinically referred to as "only" small for gestational age (SGA). Doppler may improve the clinical management of FGR; however, the need to use several parameters sometimes results in a number of combinations that may render interpretation challenging when translating into clinical decisions. We propose that the management of FGR can be simplified using a sequential approach based on three steps: (1) identification of the "small fetus," (2) differentiation between FGR and SGA, and (3) timing of delivery according to a protocol based on stages of fetal deterioration.

Abstract:Objective. To prove the advantages of new diagnostic criteria for fetal growth restriction (FGR) with centile assessment of fetal biometric parameters and Doppler parameters of blood flow in the maternal-placental-fetal system. Patients and methods. A retrospective analysis was made of 364 obstetric histories and perinatal outcomes in patients diagnosed with FGR during pregnancy according to the “former” classification. Simultaneously, fetal growth was assessed using the new classification (2022); a comparative analysis of the diagnostic significance of FGR criteria in both classifications was performed. The prospective phase involved 90 patients in whom the diagnosis of FGR was based on the centile assessment of fetal biometric parameters and Doppler parameters. The examination included Doppler ultrasound and computerized cardiotocography. The histological examination of the placenta was carried out in 23 cases of FGR. Results. The use of new diagnostic criteria for FGR based on the centile assessment of fetal biometric parameters and Doppler parameters significantly reduced the rate of false-positive diagnoses of FGR from 40% to 15%. The histological examination of 21 (82%) out of 23 placentas revealed chronic placental insufficiency with equal frequency in the early (32%) and late (37%) phenotype of FGR. Conclusion. Thus, the use of new diagnostic criteria for FGR can improve the fetal growth assessment and optimize pregnancy and labor management tactics, accompanied by a decrease in the rate of unjustified preterm labor, cesarean sections, and improved perinatal outcomes. Abstract:Fetal growth restriction (FGR) refers to pregnancy complications associated with an increased risk of perinatal morbidity and mortality and is defined in the Russian-language literature as the fetal size and weight retardation in relation to the norm for a given gestational age, and in the English-language literature – as the inability of the fetus to realize its genetically determined growth potential. FGR is the cause of 43% of stillbirths of unspecified etiology, and some cases remain undiagnosed even in high-risk populations due to the lack of universal diagnostic standards for this pathology. The review presents a critical analysis of the existing definitions of FGR, the latest data on risk factors, an assessment of diagnostic methods for its early and late forms, the prospects of using biomarkers and instrumental methods of examination in predicting adverse perinatal outcomes, and an algorithm for the management of pregnancy complicated by FGR. For a more complete coverage of the literature and deeper understanding of the nosology, attention is focused on FGR that is not accompanied by preeclampsia and other hypertensive disorders, which occur in about 30% of cases. Abstract:Fetal growth restriction (FGR) is a leading cause of perinatal morbidity and mortality. Altered placental formation and functional capacity are major contributors to FGR pathogenesis. Relating placental structure to function across the placenta in healthy and FGR pregnancies remains largely unexplored but could improve understanding of placental diseases. We investigated integration of these parameters spatially in the term human placenta using predictive modelling. Systematic sampling was able to overcome heterogeneity in placental morphological and molecular features. Defects in villous development, elevated fibrosis, and reduced expression of growth and functional marker genes (IGF2, VEGA, SLC38A1, and SLC2A3) were seen in age-matched term FGR versus healthy control placentas. Characteristic histopathological changes with specific accompanying molecular signatures could be integrated through computational modelling to predict if the placenta came from a healthy or FGR pregnancy. Our findings yield new insights into the spatial relationship between placental structure and function and the etiology of FGR.

Early-onset fetal growth restriction: comparison of two management protocols in a single tertiary center

Abstract:Compare the neonatal outcomes of two protocols of diagnosis and surveillance of pregnancies complicated by early-onset FGR in a tertiary hospital. This is a retrospective cohort study of pregnant women diagnosed with early-onset FGR between 2017 and 2020. We compared the obstetric and perinatal outcomes between two different management protocols (before and after 2019). Seventy-two cases of early-onset FGR were diagnosed in the forementioned period: 45 (62.5%) were managed according to protocol 1 and 27 (37.5%) according to protocol 2. Mean gestational age at delivery was significantly different between groups: 34.9 ± 3.1 weeks (95% CI 34.0–35.9) in group 1 and 32.3 ± 4.4 weeks (95% CI 30.4–33.9) in group 2. 74.1% (20) of newborns in group 2 were admitted in de NICU, a significant difference when compared with 46.7% of group 1. There were no statistically significant differences in the remaining serious neonatal adverse outcomes. This is the first study published comparing two different protocols of management of FGR. The implementation of the new protocol seems to have led to a decrease in the number of fetuses labeled as growth restricted and to a decrease in the gestational age of delivery of such fetuses, but without increasing the rate of serious neonatal adverse outcomes. Clinical Comparative Analysis of Fetal Growth Restriction With/without Maternal Complications

Abstract:Objective:To study the clinical characteristics,treatment and outcome of FGR cases with/without maternal complications.Methods:The clinical data of 38 patients with FGR(no deformity)treated in Peking University People′s Hospital from April 2009 to April 2011 was retrospectively analyzed.Twenty-five pregnant women with complications were taken as group FGR(A),the other thirteen women were taken as group FGR(B).The pregnant complications,average age,gravidity,ultrasonic index and outcome were analyzed.Results:①Maternal complication is a major determinant of FGR,in which the pregnancy induced hypertension is the leading(72.0%,18/25).②The pre-pregnant BMI,the rate of abnormal S/D flow ratio and cesarean section of group FGR(A) were all obviously higher than group FGR(B) (P0.05),while there was no significant difference between these two groups in the weight gain during pregnancy,the prenatal BMI and delivery gestational week (P0.05).③16 cases in group FGR(A) received intrauterine treatment,and 5 cases in group FGR(B) as well.④There were 3 cases of pregnancy induced hypertension who lost the fetus in group FGR(A),one case in group FGR(B)had the fetus loss for torsion of the cord,the fetal mortality was 10.5%(4/38).⑤The average birth weight of group FGR(A) was 2051.82±359.43 g,which was obviously lower than 2385.42±446.03 g of group FGR(B) (P=0.024).Up to now,no postnatal problems such as cerebral palsy,retinopathy occurred in both groups.Conclusions:Maternal complication as pregnancy induced hypertension is a major determinant of FGR(without deformity),which may lead to fetal abnormal S/D flow ratio,higher rate of fetus loss,cesarean section and lower birth weight.Identifing fetuses at risk of developing growth restriction at an early time and appropriate treatment for maternal complications would improve the postnatal prognosis.

Abstract:Objective To evaluate the placental characteristics in monochorionic (MC) twin pregnancy with selective fetal growth restriction (sFGR). Methods Fifty-five placentas from women with MC twin pregnancy were included, who had terminated pregnancy in the Peking University Third Hospital between June 1, 2013 and June 1, 2014, including 23 cases with sFGR and 32 uncomplicated cases as control group. We perfused the placentas within 24 h after delivery, and pigment of four different colors was used to perfuse the umbilical arteries and veins of both twins and determine the types of vascular anastomosis. Umbilical cord insertion, placental territory discordance (PTD, the territory difference between two placentas/the bigger one), and the type, number and diameter of placental superficial vascular anastomosis were analyzed using two independent samples t-test, nonparametric test,χ2 test or Fisher's exact test. Results The PTD was 0.60(0.10-0.80) vs 0.22(0.00-0.90) in sFGR group and control group (Z=-3.913) respectively, and the proportion of placenta with uneven share was 91.3%(21/23) vs 50.0%(16/32) (Fisher's exact test), which were significantly higher in sFGR group (both P < 0.01). The proportion of non-central cord insertion was 82.6% (19/23), 13.0% (3/23) and 40.6% (26/64), respectively, in smaller fetus of sFGR, bigger fetus of sFGR and control group, which was significantly higher in smaller fetus of sFGR than in the other two groups (Fisher's exact test, both P < 0.01). The proportion of arterioarterial (AA), arteriovenous (AV) and venovenous (VV) anastomosis in sFGR group and control group was 78.3%(18/23) vs 75.0%(24/32), 82.6%(19/23) vs 71.9%(23/32), and 17.4%(4/23) vs 15.6%(5/32);there were no significant differences between two groups (Fisher's exact test,all P>0.05). The number of AA, AV and VV anastomosis in sFGR group and control group was 1.0 (0.0-2.0) vs 1.0 (0.0-4.0), 3.0 (0.0-10.0) vs 2.0 (0.0-5.0), and 0.0 (0.0-1.0) vs 0.0 (0.0-3.0) (Z=-0.256, -0.142 and -0.123);the total diameter of AA, AV and VV anastomosis was 2.7 (0.0-7.0) vs 2.2 (0.0-9.7), 4.0 (0.0-13.7) vs 3.4 (0.0-11.5), and 0.0 (0.0-7.9) vs 0.0 (0.0-7.1) mm (Z=-0.070, -0.087 and -0.087);there were no significant differences between two groups (all P>0.05). The total number of all anastomosis was 3.5 (0.0-10.0) vs 3.5 (0.0-6.0) (Z= - 0.567); the total diameter of all anastomosis was 6.9 (0.0-22.4) vs 5.9 (0.0-17.1) mm (Z= - 0.556); there were no significant differences between two groups (all P>0.05). Conclusions Placental sharing discordance and non-central cord insertion may be the risk factors for MC pregnancies complicated with sFGR.
