Abstract:Objective This study was to evaluate whether resin luting system could improve the fracture resistance of endodontically treated incisors with prefabricated and cast metal posts.Methods Fourty intact maxillary central incisors after endodontical treatments were randomly assigned to four groups of 10 teeth each. The teeth were then restored with prefabricated titanium alloy posts or cast nickel-chromium post-and-cores and metal-ceramic crowns. The posts and crowns were cemented with either adhesive composite resin luting system or glass ionomer cement. All restored teeth were thermocycled for 5000 cycles (5℃/55℃) as a fatigue test. Each specimen was secured in a universal testing machine. A compressive load was applied at an angle of 135 degrees to the long-axis of the tooth with a crosshead speed of 1.5mm/s until initial fracture. Fracture loads and modes (repairable or catastrophic) were recorded.Results The fracture load was 499.82±168.90N in the group of Ti-alloy post with resin cement (group 1) and was 388.98±125.22N in the group of the same posts with glass ionomer cement (group 2); The fracture load in the group of cast post with resin cement (group 3) was 412.55±99.29N,while 337.41±121.18N in the group of the same posts with glass ionomer cement (group 4). The repairable fracture observed from Group 1 to Group 4 was 40%, 20%, 20% and 30% respectively. No significant difference was found for fracture load and fracture mode between groups with different luting agents (P>0.05).Conclusion It indicates that adhesive composite resin luting system might not significantly improve the fracture resistance of the pulpless incisors restored with metal posts.

Abstract:Objective: The purpose of this in vitro study was to evaluate the fracture resistance of various post system using different luting agents under tangential loading after wet thermocycling. Material & methods: Seventy freshly extracted maxillary central incisors were endodontically treated and post-spaces were prepared to receive different post system. They were assigned to 7 groups. Composite resin was used as core material in all the groups. Three type of post system: prefabricated post system (ParaPost(®) XP™), fibre post (ParaPost(®) fiber Lux™), cast nickel-chromium alloy post and two type of luting cements dual cure resin cement (ParaCem(®), Whaledent) and glass ionomer cement (Fuji I, GC) were used. The specimens were thermocycled and subjected to tangential loading at a crosshead speed of 1.5 mm/min. The failure loads were recorded and data were statistically analyzed with one-way ANOVA (p < 0.05) and Student's t tests (p < 0.001). Results: Significant differences of fracture resistance were detected among groups (p < 0.001). The greatest number of repairable fracture mode was recorded in fibre posts and resin cement system. Conclusions: Within the limitations of this in vitro study, it can be stated that fibre posts can be recommended as a better alternative to the cast post and cores and prefabricated metallic posts in the anterior region and resin cement might give additional fracture resistance when used for post and crown cementation.

Abstract:Purpose. The aim of this in vitro study was to evaluate the fracture resistance and type of failure of two adhesive fiber post systems used to restore endodontically treated teeth. Material and Methods. Twenty-seven extracted premolars were selected and divided into three groups ( n=9 ): a control group restored with direct composite core (group 1), teeth restored with single fiberglass posts (group 2), and teeth were restored with multifiber posts (Biolight Plus System) (group 3). Fracture resistance was measured by applying axial compressive loads parallel to the longitudinal axis of the tooth until failure. Data was analyzed with one-way ANOVA followed by post hoc Turkey tests. Results. The results showed that the mean forces to failure of the control group (0.068 kN) were significantly lower than those restored with either fiberglass post systems ( p=0.013 ). There was no significant difference between the multifiber and the single fiberglass post system in terms of resistance to fracture ( p=0.097 ). Although there are more teeth fractured favorably (above the CEJ) in the Biolight Plus group (77.7%) compared to both the control and fiberglass post groups, it was not statistically significant ( p=0.226 ). Conclusion. Within the limitations of this study, restoring endodontically treated with a multifiber post system is an adequate alternative to single fiberglass post system in terms of resistance to fracture.

Abstract:Background: This in vitro study aimed to compare the fracture resistance of fiber post and reversed-oriented metal post in the restoration of severely decayed primary incisors. Materials and methods: In this in vitro study, forty extracted human primary incisors were sectioned horizontally 1 mm above the CEJ and randomly divided into four groups - Group 1: Central incisors restored with reverse-oriented metal post, Group 2: Lateral incisors restored with reverse-oriented metal post, Group 3: Central incisors restored with fiber post, and Group 4: Lateral incisors restored with fiber post. The coronary portion of the teeth was then restored with nanohybrid universal composite. The fracture resistance was measured using universal testing machine. The type of fracture was determined by employing a stereomicroscope. Data were analyzed using two-way ANOVA test. The level of significance was considered at P < 0.05. Results: The mean fracture resistance of glass fiber posts (208.00 ± 73.19) was higher than the reverse-oriented metal posts (190.37 ± 56.36); however, there was no significant difference between any of the groups (P = 0.353). Conclusion: Within the limitations of this study, it can be concluded that both types of posts studied in the present research can be successfully used in the restoration of severely damaged primary incisors.

Abstract:Aim This study aimed to compare the fracture resistance of different materials used in composite core buildups, including conventional filler composite, nanofiller composite, and short fiber-reinforced composite (SFRC). Methods This in vitro study was conducted on 30 freshly extracted premolars. The teeth were treated using a uniform endodontic procedure, and Fiber Posts (REFORPOST, Angelus) were placed. The teeth were then divided into three groups and restored using different materials. Group 1 was restored using SFRC (everX Posterior, GC, Europe), Group 2 using microfiller composite (Te-Econom Flow, Ivoclar Vivadent), and Group 3 using nanofiller composite (Tetric N-Flow, Ivoclar Vivadent). The restoration materials were then light-cured for 40 seconds. The teeth were placed in a Universal Testing Machine (Instron) and a load was applied with a stainless-steel ball (4 mm diameter) until the tooth fractured. The fracture load for each tooth was recorded, and after the mechanical test, the experimental groups were examined for failure modes. Statistical analysis was performed using SPSS version 21.0 software. A one-way ANOVA test was conducted to compare more than two groups, followed by Tukey's test for post hoc pairwise comparison. Results The mean fracture resistance of the microfiller composite (346.94±44.63) was the lowest among the three groups. When analyzed using Tukey's test at p<0.05, fracture resistance was significantly higher in the SFRC, followed by nanofillers and microfiller composites. Conclusion Due to the increasing demand for aesthetic restorations in recent years, composites have become important in modern restorative dentistry. The development and implementation of composite dental restorative materials rely on a comprehensive understanding of each composite component and consideration of methods for modifying each component. As a result, the findings of this study will be beneficial in determining which material to use based on specific cases.

Abstract:Purpose: This study compared the use of anatomical glass fiber posts using bulk-fill composite resin with computer-aided design and computer-aided manufacturing (CAD/CAM) milled glass fiber post in oversized root canals, through bond strength (BS) and fracture resistance (FR) tests (fracture load). Methods and materials: A total of 48 mandibular premolars were selected, half of them had their crowns removed at the cemento-enamel junction (CEJ) and the other half were sectioned 2 mm above the CEJ. Subsequently, teeth were endodontically treated. After 1 week, the standard preparation of the canals was carried out, and the roots were divided into three groups (n=16), according to the use of different restorative techniques (control: prefabricated glass fiber post [PFP], direct anatomical glass fiber post [AFP], and CAD/CAM milled glass fiber post [MFP]). After luting procedures using Single Bond Universal and RelyX Ultimate (3M ESPE), for eight teeth in each group, six specimens were obtained (two slices from each root third: cervical, middle, and apical). For the remaining eight roots of each group, standardized preparations for metal-free crowns, milling of 5 mol% yttria-stabilized tetragonal zirconia polycrystalline, cementation of the crowns, and periodontal ligament simulation were performed. Then, for each group, the BS was evaluated through the push-out test, and the FR was evaluated in compression. The data obtained from BS were submitted to two-way analysis of variance (ANOVA; group vs root region) and Tukey (α=0.05) and from FR to one-way ANOVA (group) and Tukey (α=0.05). Results: For the BS test, the MFP group was statistically superior to the PFP group in all root regions and was statistically superior to the AFP group only in the cervical region, being statistically similar in the middle and apical root regions. For the FR test, the MFP group was statistically superior to the PFP and AFP groups. Conclusion: The milled fiber post technique can be a legitimate alternative in the restoration of weakened roots with flared root canals.

In Vitro Fracture Strength of Primary Canine Teeth Reinforced With Prefabricated and Customized Fiber-Reinforced Post Systems

Abstract:Objective: The demand for esthetics has increased in today's world and most parents prefer to preserve their children's primary anterior teeth until their natural exfoliation. However, an intracanal post is required to provide retention for reconstruction of severely damaged anterior teeth due to caries or trauma. Various materials and methods may be used for the fabrication of intracanal posts. This study assessed the fracture strength and fracture mode of primary canine teeth reconstructed with prefabricated and customized polyethylene and glass fiber posts. Materials and methods: This in vitro study evaluated 60 extracted primary canine teeth in four groups (n = 15). After pulpectomy and post space preparation with 4 mm depth, composite resin post, prefabricated glass fiber post (Whitepost), customized glass fiber post (Interlig), or customized polyethylene fiber post (Ribbond) were placed in the root canals to provide retention, and the tooth crown was restored with bulk-fill composite resin. The fracture strength was then measured in a universal testing machine. The fracture mode was also evaluated visually. Results: The mean fracture strength was 22.45 ± 5.06, 33.10 ± 8.5, 30.20 ± 7.33, and 32.61 ± 5.73 N/mm2 in the composite resin post, Whitepost, Interlig, and Ribbond groups, respectively. The fracture strength was significantly lower in the composite group than in the remaining three groups (p = 0.000). No other significant differences were found (p > 0.05). Also, no significant difference was observed among the study groups in the fracture mode (p = 0.241). Conclusion: The composite resin post yielded a significantly lower fracture strength than the prefabricated and customized glass and polyethylene fiber posts, but the fracture mode was not significantly different among the four groups.

Abstract:Background Severely damaged primary anterior teeth that require pulp therapy present a high risk of failure due to the loss of tooth substance, resulting from pre-existing decay and endodontic therapy. The ideal post material should have physical and mechanical properties similar to those of dentin. Another concern in restoring endodontically treated primary teeth is the need to find a material that can resorb similar to the natural tooth structure as a part of the exfoliation process, allowing normal eruption of permanent successors. Accordingly, there is no such material other than dentin itself. The introduction of biological dentin posts offers an excellent alternative for restoring such teeth. This study aimed to assess the effect of using dentin posts on the pull-out resistance of endodontically treated primary anterior teeth in comparison to glass fiber posts. Methodology A sample of 30 primary anterior teeth was collected from the outpatient clinic of the Pediatric Dentistry Department, Faculty of Dentistry, Damascus University. A total of 15 freshly extracted permanent teeth with single roots were also collected from the outpatient clinic of the Maxillofacial Surgery Department, Faculty of Dentistry, Damascus University. The roots of the permanent teeth were used to prepare 30 dentin posts using a CAD-CAM machine. After receiving proper endodontic treatment, the primary teeth were divided into two groups (15 teeth in each group). The first group was restored with dentin posts, and the second was restored with glass fiber posts, with the posts measuring 3 mm in length for both groups. Pull-out resistance testing was performed using a Testometric machine. Results The arithmetic mean of the forces applied in the glass fiber posts group was 153.2 ± 39.12 N, while the arithmetic mean of the forces applied in the dentin posts group was 156.7 ± 39.78 N. The data were analyzed at a 95% confidence interval using the independent Student's t-test. There were no statistically significant differences in pull-out resistance between the two groups. Conclusions Dentin posts showed a little increase in pull-out resistance than glass fiber posts. Therefore, the use of dentin posts as intracanal retention in primary anterior teeth is a successful alternative for composite posts.

Abstract:Background: Endodontically treated teeth are thought to be more prone to fracture. The last stage in a successful root canal treatment is to restore root canal-treated teeth with a permanent, definitive postendodontic restoration. Dehydration, dentin loss during endodontic procedures, and the elimination of significant anatomic elements like cusps, ridges, and the pulp chamber's arched roof—all of which contribute significantly to the normal tooth's support—are the causes of the fracture of endodontically treated teeth. Consequently, intracoronal strengthening is necessary to prevent tooth fracture, especially in the posterior teeth where compressive stresses brought on by occlusal forces can cause tooth fractures.1 Since core materials typically replace a significant portion of the tooth structure and must withstand multidirectional masticatory forces,their compressive strength is crucial.2  Aim: The purpose of this study was to compare the compressive strengths of three direct post enododontic restorations.  Material and Methods: There are thirty extracted maxillary premolars chosen. After receiving root canal therapy, the teeth were divided randomly into three groups for immediate post-endodontic restorations: (i) silver amalgam (ii)Fiber-reinforced composite resin (iii) dual cure glass- reinforced composite resin . After that, a Universal Testing Machine was used to apply compressive stress on the teeth.  Statistical Analysis: Fracture loads will be compared statistically, and the data will be analysed using version 21.0 of SPSS software, analysis of variance and the Post-hoc test for multiple comparisons.  Result and Conclusion: Within the limits of the study, it showed that the post hoc tests reveal that the mean difference of compressive load strength is significant between all the groups, Amalgam has highest load strength followed by Fiber reinforced composite. Dual cure composite has lowest compressive load strength.

Abstract:OBJECTIVE:To evaluate the fatigue and fracture resistance of the flared roots restored with computer aided design (CAD) and computer aided manufacturing (CAM) glass fiber posts.METHODS:In the study, 32 maxillary central incisors with roots longer than 13 mm were selected and their canals were flared, and the roots were allocated into 4 groups (n=8) by a random number chart: CAD/CAM glass fiber posts, prefabricated quartz fiber posts, cast gold alloy posts, and CAD/CAM zirconia posts. The posts were luted to the roots by resin cement and fabricate zirconia crown for every specimen. An addition-type silicone impression material was used to simulate the periodontal ligament. All the specimens were submitted to 1.2×10(6) cycles loaded with a 49 N force, at 45 degree to the long axis of the teeth simultaneously with 3 000 thermal cycles (5 °C-50 °C-5 °C). After that, the specimens were subjected to a load at a crosshead speed of 1 mm/min in a servo-hydraulic testing machine applied at 45 degree to the long axis of the tooth until fracture. The data were subjected to ANOVA test and the patterns of the failure were examined.RESULTS:After the cycling loading, 4 crowns from prefabricated quartz fiber posts groups were deboned, and no other failure was found after the cycling loading; the fracture strengths of CAD/CAM glass fiber posts group [(441.5± 103.2) N] and cast gold alloy posts group [(462.9±170.0) N] were higher (F=4.613, P<0.05) than those of CAD/CAM zirconia posts group [(284.1±99.0) N] and prefabricated quartz fiber posts group [(315.4±112.3) N]; the entire specimens presented unfavorable failures.CONCLUSION:Although further in vitro and in vivo studies are necessary, the results of this study show that the use of CAD/CAM glass fiber posts and cast gold alloy posts may achieve better outcomes in flared roots than that of CAD/CAM zirconia posts and prefabricated quartz fiber posts.

Abstract:Background: Endodontic treatment is the most common method for resolving pulpal and periapical pathology. However, various studies have reported that almost 11%-13% of all teeth that undergo extraction after endodontic treatment show the presence of cracks, craze lines, and vertical root fractures. Teeth with inadequate post endodontic restoration are more prone to fracture and coronal leakage, resulting in the diffusion of oral fluids, bacteria, bacterial products, and possibly root canal treatment failure. Furthermore, studies have advocated the use of endodontically treated teeth with restorative materials that have a similar or higher elastic modulus than the tooth for providing stiffness against forces that cause root fracture. Intraorifice barriers made of restorative materials that can bond to radicular dentin could thus be used to reinforce the radicular dentin while also preventing coronal microleakage. Although the sealing ability of intraorifice barriers has been widely compared in the literature, there have been few studies on the strengthening effect of the materials used in the study as intraorifice barriers when placed into the root canal. As a result, the current in vitro study aims to assess the effect of various materials as intraorifice barriers (Cention N, Resin modified glass ionomer cement, and short fiber reinforced flowable composite) on the force required fracture teeth after root canal treatment. Methods: This in vitro study will be done on extracted human mandibular premolars with single root canal where after doing root canal treatment 2-3 mm obturating material would be replaced by intra orifice barriers (Cention N, resin modified glass ionomer cement [RMGIC], and short fiber reinforced flowable composite). The force required to fracture teeth will be calculated using universal testing machine.

Abstract:Objective To compare the fracture resistance of endodontically treated teeth restored with two post systems, comparing different moduli of elasticity. Methods Twenty recently extracted human maxillary central incisors were endodontically treated and randomly divided into two groups of ten each. Teeth in group-1 received glass fiber posts (ParaPost Fiber White (Coltene/ Whaledent Inc), composite resin cores, and full crown casts. Teeth in group - 2 received cast Ni-Cr posts and cores and full crown casts. About 1. 5mm ferrule was prepared for each of the teeth. Each specimen was embedded in acrylic resin and secured in a universal material testing machine. A compressive load was applied on the lingual surface at a 130 - degree angle to the long axis of the tooth until fracture occurred, at a crosshead speed of 1mm/min) . Independent sample t-test were used to determine the significance of the fracture loads between the two groups ( P 0.05). Fisher's exact test was conducted for evaluation of the mode of failure ( P 0.05). Results The fracture loads were 43.406±18.959kg and 33.680±9.675kg for groups-1 and groups- 1, respectively. There was no statistically significant difference between the two groups ( P 0.05). Repairable fractures were observed in group - 1; whereas un - repairable fractures were observed in group - 2 ( P 0. 001) . Conclusion When a 1. 5 mm ferrule was preserved, two post systems with different modules of elasticity had no significant influence on the fracture resistance of endodontically treated teeth restored with full crowns. Repairable fractures were observed in teeth restored with glass fiber posts, which had similar modulus of elasticity to that of dentin.

Abstract:Acute severe asthma exacerbations resulting in emergency department visits and hospitalization usually constitute a failure of long-term control therapy. However, even patients with relatively mild asthma can have severe life-threatening episodes. In both children and adults, viral respiratory infections are the major triggering event, although outbreaks of severe asthma have been associated with high concentrations of aeroallergens. Patients should be provided with written action plans on what to do for acute deterioration, and more severe patients may keep prednisone at home to begin after consultation with their physician. The primary therapy of acute asthma exacerbations remains frequent administration of aerosol β2-agonists and systemic corticosteroids for those patients not fully responding to the β2-agonists. Mild exacerbations may be treated with an increased dosage of inhaled corticosteroids. Patients at risk for acute exacerbations may benefit from peak flow measurement, particularly those who have difficulty perceiving airway obstruction. It is recommended that patients remain on full dose of prednisone until they achieve 70-80 percent of predicted normal or personal best peak flow. In the emergency department, the use of β2-agonists by metered-dose inhaler and holding chamber is as effective as nebulizer if given in a sufficient dose 6-10 puffs equivalent to 5 mg via nebulizer. In those patients not responding completely, the addition of ipratropium bromide has shown to produce additive bronchodilation and reduce hospitalizations. Other therapies such as magnesium sulfate, intravenous β2-agonists, heliox and ketamine have been used, but data demonstrating efficacy are insufficient to warrant recommending their general use.

Abstract:Statement of problem: Various strategies have been proposed to reduce the cement space of foundation restorations for endodontically treated teeth. However, they may add more operative steps, or the dentist must keep different sizes of drills and posts in stock. A 2-piece universal adjustable post system has been developed to overcome this problem, but whether the system has acceptable fatigue survival performance is unclear. Purpose: The purpose of this in vitro study was to evaluate the fatigue survival and stress distribution of endodontically treated teeth without a ferrule and restored with different glass fiber post strategies versus a recently introduced universal 2-piece fiber post system. Material and methods: Bovine incisor roots were randomly assigned to 3 groups as per the post used (n=13): adapted glass fiber post with post space preparation of the same size, composite resin-custom glass fiber post (CTM), and universal 2-piece glass fiber-reinforced composite resin post (UNI). The posts were adhesively luted, the composite resin core was added, and a composite resin crown was produced with computer-aided design and computer-aided manufacturing (CAD-CAM), and then adhesively luted to each core. A fatigue test was performed with the stepwise stress method (10 000 cycles/step; 20 Hz; load=100 N to 750 N; step=50 N) until fracture, and the failure mode analyzed. The stress distribution was evaluated by finite element analysis with the maximum principal stress criteria by following the parameters of the in vitro test. The solids were considered homogeneous, linear, and isotropic, except for the glass fiber post (orthotropic), and a load of 450 N at 30 degrees was applied. The fatigue failure load and the number of cycles for failure were analyzed with Kaplan-Meier and Mantel-Cox (log rank test) (α=.05). The finite element analysis results were analyzed with colorimetric graphs. Results: The highest fatigue failure load and the number of cycles for failure were found in the UNI system, whereas the lowest results were found in the CTM group. All groups exhibited repairable failures. The finite element analysis showed the lowest stress in root dentin in the UNI system. The CTM system had the largest stress regions at the dentin and dentin-core interface. Conclusions: The use of a 2-piece universal glass fiber post system resulted in more fatigue behavior compared with composite resin-custom glass fiber posts.
