Abstract:Objective This study was to evaluate whether resin luting system could improve the fracture resistance of endodontically treated incisors with prefabricated and cast metal posts.Methods Fourty intact maxillary central incisors after endodontical treatments were randomly assigned to four groups of 10 teeth each. The teeth were then restored with prefabricated titanium alloy posts or cast nickel-chromium post-and-cores and metal-ceramic crowns. The posts and crowns were cemented with either adhesive composite resin luting system or glass ionomer cement. All restored teeth were thermocycled for 5000 cycles (5℃/55℃) as a fatigue test. Each specimen was secured in a universal testing machine. A compressive load was applied at an angle of 135 degrees to the long-axis of the tooth with a crosshead speed of 1.5mm/s until initial fracture. Fracture loads and modes (repairable or catastrophic) were recorded.Results The fracture load was 499.82±168.90N in the group of Ti-alloy post with resin cement (group 1) and was 388.98±125.22N in the group of the same posts with glass ionomer cement (group 2); The fracture load in the group of cast post with resin cement (group 3) was 412.55±99.29N,while 337.41±121.18N in the group of the same posts with glass ionomer cement (group 4). The repairable fracture observed from Group 1 to Group 4 was 40%, 20%, 20% and 30% respectively. No significant difference was found for fracture load and fracture mode between groups with different luting agents (P>0.05).Conclusion It indicates that adhesive composite resin luting system might not significantly improve the fracture resistance of the pulpless incisors restored with metal posts.
Effect of a circumferential ferrule on the survival and success of endodontically treated teeth restored with fiber posts: A systematic review and meta-analysis

Abstract:Statement of problem The success of endodontically treated teeth (ETT) is influenced by factors that include the use of prefabricated glass fiber posts. A consideration for success is the presence of a circumferential ferrule, but systematic reviews and meta-analyses are lacking. Purpose The purpose of this systematic review and meta-analysis was to assess the impact of the presence or absence of a circumferential ferrule on the survival and success rates of ETT restored with prefabricated fiber posts. Material and methods Adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a comprehensive search of the major scientific literature databases was conducted. Eligible studies included randomized clinical trials and prospective cohort studies with a minimum 6-month follow-up, focusing on ETT restored with fiber posts either with or without a uniform circumferential ferrule. Relative risks (RRs) and 95% confidence intervals (CIs) were calculated to compare outcomes. The degree of variability among studies was assessed using the chi-squared test (α=.05). Results Of 3731 identified records, 8 were eligible for inclusion. Direct analysis of success rates (2 studies, 123 teeth) showed a significant increase in success when a uniform circumferential ferrule was present (RR=1.28, 95% CI: 1.06, 1.54, P <.05). However, indirect analysis of survival and success rates (8 studies, 407 teeth) indicated no significant differences in outcomes between the ferrule and no ferrule groups ( P >.05). Conclusions The results of this systematic review and meta-analysis determined that a uniform circumferential ferrule had a positive effect on the success rates of ETT restored with prefabricated fiber posts. However, further high-quality studies are required to provide more robust evidence and to allow potential confounding factors to be considered.

[A comparative study on the effect of glass fiber post and metal post in restoration of anterior tooth defect]

Abstract:Purpose: To analyze the effect and long-term success rate of using glass fiber post or metal post in restoration of anterior tooth defect. Methods: One hundred and fifty cases with anterior tooth defects were chosen after root canal therapy in our hospital. According to different material being chosen, the patients were divided into fiber post group and metal post group with 75 cases in each group. Local gingival sulcus fluid (GCF) levels and alkaline phosphatase (ALP) level in gingival sulcus liquid were measured and compared 4 weeks, 6 months, 1 year, 2 years and 3 years after restoration. Restoration integrity, edge fitness and color matching of two groups were compared after follow-up for 3 years. The data were analyzed with SPSS 19.0 software package. Results: Four weeks, 6 months, and 1 year after restoration, local GCF, ALP levels significantly elevated in both groups. The fiber post group showed more significant elevated levels of GCF and ALP than the metal post group (P<0.05). After 2 and 3 years, the difference between the two groups was not significant. The gingival color matching degrees were significantly better in the glass fiber post group after 1 year compared with that in the metal post group(P<0.05). The success rate of metal post group was significantly higher than that of fiber post group 3 years after restoration(P<0.05). Conclusions: Fiber post and metal post both have satisfactory restoration result for tooth defect in anterior teeth area. However, the use of precious metal alloy post may reduce the possibility of microleakage between tooth-prosthesis surfaces.

Abstract:This literature review summarizes the most recent and reliable evidence on post systems. A search was limited to review articles published over the last 10 years in dental journals with an impact factor. Papers cited in the initially retrieved review articles were also included if significant. Preservation of tooth tissue, presence of a ferrule effect, and adhesion are regarded as the most effective conditions for long-term success of post-endodontic restorations. Adhesively luted fibre-reinforced composite post restorations have demonstrated satisfactory survival rates over relatively long follow-up periods. The clinical effectiveness of such restorations has been mainly ascribed to the more biomimetic behaviour of fibre-reinforced composite posts that reduces the risk of vertical root fractures. The most common type of failure when using fibre posts is post debonding and it is generally agreed that achieving stable adhesion to intraradicular dentine is more challenging than to coronal dentine. Several factors related to endodontic treatment, root canal shape, post space preparation, post translucency, adhesive cement handling and curing may have an influence on the outcome of the luting procedure. The most reliable results in fibre post cementation are obtained by etch-and-rinse adhesives in combination with dual-cure resin cements. The use of self-adhesive resin cements has also been proposed. Simplification is an obvious advantage of these new materials. However, the durability of their bond still needs to be verified with long-term clinical studies. Several techniques for pre-treating the fibre-reinforced composite post surface have been tested with the aim of improving the bond strength at the post-core and post-cement interfaces. Silicoating followed by silanization currently appears to be the most effective and convenient method for this purpose. In conclusion, the available evidence validates the use of fibre posts as an alternative to metal posts and preferably to other tooth-coloured posts, such as zirconia dowels, in the restoration of endodontically treated teeth. Longer term clinical trials are expected to further strengthen this evidence.
Fracture Resistance of Endodontically Treated Teeth Restored Using Multifiber Posts Compared with Single Fiber Posts

Abstract:Purpose. The aim of this in vitro study was to evaluate the fracture resistance and type of failure of two adhesive fiber post systems used to restore endodontically treated teeth. Material and Methods. Twenty-seven extracted premolars were selected and divided into three groups ( n=9 ): a control group restored with direct composite core (group 1), teeth restored with single fiberglass posts (group 2), and teeth were restored with multifiber posts (Biolight Plus System) (group 3). Fracture resistance was measured by applying axial compressive loads parallel to the longitudinal axis of the tooth until failure. Data was analyzed with one-way ANOVA followed by post hoc Turkey tests. Results. The results showed that the mean forces to failure of the control group (0.068 kN) were significantly lower than those restored with either fiberglass post systems ( p=0.013 ). There was no significant difference between the multifiber and the single fiberglass post system in terms of resistance to fracture ( p=0.097 ). Although there are more teeth fractured favorably (above the CEJ) in the Biolight Plus group (77.7%) compared to both the control and fiberglass post groups, it was not statistically significant ( p=0.226 ). Conclusion. Within the limitations of this study, restoring endodontically treated with a multifiber post system is an adequate alternative to single fiberglass post system in terms of resistance to fracture.

Abstract:Aim: This finite elemental stress analysis (FEA) study was aimed to test the effect of fiber and metal posts on stress distribution in premolar teeth with endodontic–periodontal (EP) lesion. Methodology: Three FEA models representing different EP lesions (primary endodontic disease (PED), PED with periodontal involvement and true-combined) were created. Tooth-model without EP lesion was used as control. The root-canals were assumed as root-filled, restored using glass-fiber or metal posts, resin composite build-up, and ceramic crown. A 300 N load was applied from the palatal surface of the crown with a 135° angle. The SolidWorks/Cosmosworks structural analysis program was used for FEA analysis. Results were presented by considering von Mises criteria. Results: Maximum stress values for PED, PED with periodontal-involvement and true-combined lesions were 2.26, 1.25, 0.74 MPa for glass-fiber post; 2.08, 1.51, 1.18 MPa for metal post, respectively. If there is PED, fiber-post gave an advantage to the tooth at core structure and coronal third of the root. Metal post kept the stress inside its body however caused small, high stress concentrated areas at cervical. When there is PED with periodontal-involvement, metal post forwarded less stress toward the apical while fiber-post saved the coronal structure. In true-combined lesion model, metal post kept the stresses within its body and forwarded less stress toward the remaining root structure and periodontium. Conclusion: EP lesions have an effect on stress distribution therefore the post material selection should be done based on the severity of the lesion. Metal posts might be preferrable since it forwards less stress toward the surrounding periodontal tissues during wound healing.

Abstract:Objective To investigate the influence of the fiber post on stress distribution of endodontically-treated molars restored by resin-based composite . Methods The CT images of maxillary 1st molar of the right side were elaborated by software , and two three-dimensional finite element models of the endodontically-treated molar, restored by resin-based composite with or without a fiber post, were developed. Static loads (600 N), simulating the maximum bite force, were applied vertically to the occlusal surfaces of the samples, while 200 N, imitating masticatory force, were applied to functional inclinations at a 45° angle. Von Mises stresses on both the tooth and the composite restoration were calculated. Results With the same load static loads applied, group A and group B had similar Von Mises stress distribution. There was always a stress concentration area on the neck of the buccal side of the tooth in group B, around the cementoenamel junction. Under the maximum bite force, the peak of Von Mises stress of group B was lower than that of group A on the outer surface of residual tooth tissue , but higher on the inner surface. Under the masticatory force, Von Mises stress value of group B was below that of group A. In both cases, stress was reduced on the composite restorations of group B. Conclusions Insertion of the fiber post could reduce stresses on both the residual tooth and the composite restoration, thus it might have some positive effect on preventing root fracture and restoration failure. Stress concentration around tooth neck indicates a higher position of tooth fracture .
Evaluation of Bond Strength and Fracture Load of Teeth With Flared Root Canals Restored With Prefabricated, Anatomical, and Computer-aided Design and Computer-aided Manufacturing Fiber Posts

Abstract:Purpose: This study compared the use of anatomical glass fiber posts using bulk-fill composite resin with computer-aided design and computer-aided manufacturing (CAD/CAM) milled glass fiber post in oversized root canals, through bond strength (BS) and fracture resistance (FR) tests (fracture load). Methods and materials: A total of 48 mandibular premolars were selected, half of them had their crowns removed at the cemento-enamel junction (CEJ) and the other half were sectioned 2 mm above the CEJ. Subsequently, teeth were endodontically treated. After 1 week, the standard preparation of the canals was carried out, and the roots were divided into three groups (n=16), according to the use of different restorative techniques (control: prefabricated glass fiber post [PFP], direct anatomical glass fiber post [AFP], and CAD/CAM milled glass fiber post [MFP]). After luting procedures using Single Bond Universal and RelyX Ultimate (3M ESPE), for eight teeth in each group, six specimens were obtained (two slices from each root third: cervical, middle, and apical). For the remaining eight roots of each group, standardized preparations for metal-free crowns, milling of 5 mol% yttria-stabilized tetragonal zirconia polycrystalline, cementation of the crowns, and periodontal ligament simulation were performed. Then, for each group, the BS was evaluated through the push-out test, and the FR was evaluated in compression. The data obtained from BS were submitted to two-way analysis of variance (ANOVA; group vs root region) and Tukey (α=0.05) and from FR to one-way ANOVA (group) and Tukey (α=0.05). Results: For the BS test, the MFP group was statistically superior to the PFP group in all root regions and was statistically superior to the AFP group only in the cervical region, being statistically similar in the middle and apical root regions. For the FR test, the MFP group was statistically superior to the PFP and AFP groups. Conclusion: The milled fiber post technique can be a legitimate alternative in the restoration of weakened roots with flared root canals.
Comparative Evaluation of Different Post Materials on Stress Distribution in Endodontically Treated Teeth Using the Finite Element Analysis Method: A Systematic Review

Abstract:The aim of this systematic review is to summarize and conclude findings to reveal the stress ranges developed by various post materials by finite element analysis. This, in turn, aids in the selection of better post material clinically. The electronic databases PubMed and Google Scholar were searched in this review by using specific inclusion and exclusion criteria. Among 14586 articles, 22 articles were included in this systematic review, as they satisfied the eligibility criteria. The search covered all articles published from 1997 to December 2021. All records identified were retrieved and imported into the Rayyan bibliographic software, which is a systematic review screening software. Later, data extraction and analysis of 22 articles were done. Twenty-two articles, which were all finite element analysis studies, were included. Among these, 18 studies used maxillary central incisor scanned models, two studies used a maxillary canine model, and the remaining two used the mandibular premolar model for finite element analysis. All these tooth models are restored with post models made of different materials. This systematic review revealed a difference in stress distribution in endodontically treated teeth when using different post materials. Among 22 studies, 15 studies evaluated glass fiber posts and the results showed that they induce less stress on restored endodontically treated teeth when compared to other posts, with maximum stress concentration at the cervical third of the root. Prefabricated posts like stainless steel and Titanium showed more stress on the restored tooth structure with stress concentration at the cervical and apical third of the root. Prefabricated zirconia also showed more stress on the restored tooth with maximum stress concentration at the middle third of the root.

Abstract:Objective: To observe the influence of defect modes on the fatigue strength of glass fiber post restoration. Methods: Collected 18 intact maxillary central incisors and divided into 3 groups (n=6). The crowns were removed and prepared into 3 defect types: severe damage, moderate damage, and slight damage. All the roots were restored with glass fiber posts and tested the fatigue strength. Then tested the residual strength of survival specimens. Results: slight damaged group demonstrated the best result and severe damaged group had less fatigue strength than slight damaged group. Moderately damaged group had the worst fatigue strength(P<0.05). The residual strength of slight group is 383.80±138.94N. Conclusion: The root defect modes affect the fatigue strength of the fiber post restoration. Weakened cervical root canal decreases the fatigue strength of glass fiber restoration.
Fatigue survival of endodontically treated teeth restored with different fiber-reinforced composite resin post strategies versus universal 2-piece fiber post system: An in vitro study

Abstract:Statement of problem: Various strategies have been proposed to reduce the cement space of foundation restorations for endodontically treated teeth. However, they may add more operative steps, or the dentist must keep different sizes of drills and posts in stock. A 2-piece universal adjustable post system has been developed to overcome this problem, but whether the system has acceptable fatigue survival performance is unclear. Purpose: The purpose of this in vitro study was to evaluate the fatigue survival and stress distribution of endodontically treated teeth without a ferrule and restored with different glass fiber post strategies versus a recently introduced universal 2-piece fiber post system. Material and methods: Bovine incisor roots were randomly assigned to 3 groups as per the post used (n=13): adapted glass fiber post with post space preparation of the same size, composite resin-custom glass fiber post (CTM), and universal 2-piece glass fiber-reinforced composite resin post (UNI). The posts were adhesively luted, the composite resin core was added, and a composite resin crown was produced with computer-aided design and computer-aided manufacturing (CAD-CAM), and then adhesively luted to each core. A fatigue test was performed with the stepwise stress method (10 000 cycles/step; 20 Hz; load=100 N to 750 N; step=50 N) until fracture, and the failure mode analyzed. The stress distribution was evaluated by finite element analysis with the maximum principal stress criteria by following the parameters of the in vitro test. The solids were considered homogeneous, linear, and isotropic, except for the glass fiber post (orthotropic), and a load of 450 N at 30 degrees was applied. The fatigue failure load and the number of cycles for failure were analyzed with Kaplan-Meier and Mantel-Cox (log rank test) (α=.05). The finite element analysis results were analyzed with colorimetric graphs. Results: The highest fatigue failure load and the number of cycles for failure were found in the UNI system, whereas the lowest results were found in the CTM group. All groups exhibited repairable failures. The finite element analysis showed the lowest stress in root dentin in the UNI system. The CTM system had the largest stress regions at the dentin and dentin-core interface. Conclusions: The use of a 2-piece universal glass fiber post system resulted in more fatigue behavior compared with composite resin-custom glass fiber posts.
Clinical and laboratorial performance of rehabilitation of endodontically treated teeth: A systematic review

Abstract:Objective This systematic review aimed to analyze the clinical (survival rate, failure risk, or fracture) and laboratory performance (fracture mode or failure) of rehabilitations of endodontically treated teeth, with and without posts. Materials and Methods A systematic search was conducted in the Pubmed, Scopus, Web of Science, Embase, Cochrane Library, and OpenGrey databases up to March 2023, according to PRISMA guidelines. In vitro and clinical studies that compared the clinical and laboratory performance of endodontically treated teeth with and without intraradicular posts were included. Studies selection, data extraction, and risk of bias analysis were performed. Results Thirty‐one in vitro and 7 clinical studies were included. For in vitro studies, fiberglass post (n = 24) was the most mentioned. The follow‐up time of the clinical studies ranged from 1 to 17 years, with the fiber‐reinforced composite post (n = 3) being the most evaluated, and only failure risk proved to be more favorable for using intraradicular posts. Conclusion Rehabilitations of endodontically treated teeth with and without intraradicular retainers showed no difference in fracture resistance and failure mode, evaluated by in vitro studies. Clinical studies showed no difference in survival rate, but failure risk proved to be more favorable for the use of posts. Clinical Significance This analysis revealed significant variability between results, however, most laboratory and clinical studies revealed no difference with using the post. Furthermore, it is important to emphasize the need to evaluate the coronary remnant and the general characteristics of the tooth in each situation.

[Investigation on the Short-Term Clinical Application of Two Types of Glass Fiber Posts].

Abstract:OBJECTIVE:The clinical results of restoring defective teeth with two types of glass fiber prothetic systems is investigated to acquire clinical experience for further application of glass fiber posts with independent intellectual property rights.METHODS:A total of 120 out-patients with restored defective teeth were selected from the Department of Stomatology, Beijing Xuanwu Hospital of Traditional Chinese Medicine and Peking University School and Hospital of Stomatology and randomly divided into two groups. OUYA FIBER posts and Tenax Fiber White posts were used to restore defective teeth in the experimental group and the control group, respectively. Clinical evaluation was conducted one week and three months post-restoration.RESULTS:Both clinical satisfactory rates of OUYA FIBER posts and Tenax Fiber White posts were higher than 98% one week post-restoration and higher than 96% three months post-restoration. No significant differences were observed between the clinical results of restoring defective teeth with the two types of fiber posts. Throughout the healing process, no root canal fracture or marginal staining were observed.CONCLUSION:OUYA FIBER post and Tenax Fiber White post showed similar clinical outcomes during the period of observation in this study. However, the long-term effects should be observed in future studies.
The Use of Fiber-Reinforced Composites for Restoration of Endodontically Treated Teeth: A Review

Abstract:This review summarized the most recently published literature pertaining to the use of fiber-reinforced composite (FRC) to restore endodontically treated teeth (ETT) and its influence on the mechanical behavior of restored teeth. In-depth¬ literature review in Google Scholar, ScienceDirect and PubMed was performed for these keywords: “fiber-reinforced composite”; “fiber post”; “endodontically treated”; “short fiber-reinforced composite”— only published full-text articles between 2009 and 2021 are included in this review. Fourteen articles were selected for this review. The studies concentrated on laboratory-based research conducted on human and bovine extracted teeth with different restorative techniques. Fatigue survival and load to failure tests with assessment of survival rate and fracture strength values respectively, as well as fracture pattern analysis were done. We concluded that FRC materials have improved fracture strength of restored teeth compared to conventional composites using different restorative techniques, however not to the attained level of an intact natural tooth. In addition, authors recommend a better simulation of oral environment, such as dynamic fatigue-testing.

Abstract:INTRODUCTION Chronic diabetic complications are the main problems in diabetology causing work unfitness, disability and premature death. Complete and homogenous assessment of diabetic complications is possible in hospitalised patients. AIM Assessment of the occurrence of chronic diabetic complications and their risk factors in patients with type 1 diabetes mellitus (DMT1). MATERIAL AND METHODS Retrospective analysis included clinical data from 241 patients admitted to the Department of Metabolic Diseases CMUJ in Krakow, 159 women (age 29.76 +/- 9.7, diabetes duration 10.00 +/- 8.6) and 82 men (age 36.52 +/- 13.4, diabetes duration 11.78 +/- 11.08). We analysed the occurrence and risk factors of diabetic retinopathy, diabetic nephropathy, peripheral and autonomic neuropathy, diabetic foot syndrome and coronary artery disease (CAD). The following risk factors were considered: BMI, diabetic control (HbA, c), lipid profile, hypertension, smoking, alcohol consumption and family occurrence of cardiovascular diseases and diabetes. RESULTS AND CONCLUSIONS The frequency of chronic diabetic complications was as follows: diabetic retinopathy 41.5%, peripheral polyneuropathy 29%, nephropathy 17%, cardiovascular autonomic neuropathy 8.7%, diabetic foot syndrome 8.3% and coronary artery disease 7.1% of patients. Half of retinopathic patients had peripheral polyneuropathy, at the same time retinopathy was the most frequent complication in patients with diabetic foot (90%), CAD (88.1%), nephropathy (85%), polyneuropathy (65.6%) and cardiovascular autonomic neuropathy (71.4%). Diabetic nephropathy was found in 70% with diabetic foot, in 58.7% with CAD, in 52.2% with cardiovascular autonomic neuropathy, in 41.5% with polyneuropathy and in 35% with retinopathy. The presence of diabetic foot was associated with the risk for co-occurrence of all the remaining diabetic complications. In CAD patients microangiopathy was much more frequent than the reverse relationship. All the complications except for CAD were more frequent in men with DMT1. Risk factors that correlated with all chronic diabetic complications were diabetes duration and arterial hypertension. Age, lipid abnormalities, smoking and alcohol consumption, family history were associated with varying degrees of DMT1 complications. In patients with microangiopathy, neuropathy and CAD the following components of metabolic syndrome were found: obesity, hypertension, dyslipidemia implying the contribution of insulin resistance to the pathogenesis of these complications.
Single crown vs. composite for glass fiber post-retained restorations: an 8-year randomized clinical trial

Abstract:Objectives This study aimed to compare the success and survival rates of metal-ceramic crowns and composite resin restorations applied in root filled teeth that received a glass fiber post. Methods A prospective, randomized controlled trial, with equivalent parallel groups was designed. Eighty-two teeth were randomly allocated to the metal-ceramic or composite resin groups. Multivariate Cox regression analysis with shared frailty for patients and Kaplan-Meier curves were performed using success and survival rates (p<0.05). Results Seventy-five post-retained restorations (34 metal-ceramic crowns and 41 composite restorations) in 62 patients were analyzed. The median follow-up was 8.1 years [IQR 4.0-9.9]. Twenty-seven failures were observed. Twenty-two failures (81.5%) were observed in the composite resin group, of which six (27.3%) were not repairable. Five failures (18.5%) were observed in the metal-ceramic crown group, of which three (66.6%) were non-repairable. The cumulative success rate at 8 years was 85.0% for crowns (AFR=1.31%) and 43.2% for composite resins (AFR=6.58%), while the survival rate was 93.8% for crowns (AFR=0.52%) and 97.6% for composite resins (AFR=0.20%). Considering the success rates, adjusted multivariate Cox regression showed that composite resin had a Hazard Ratio of 5.07 (95%CI, 1.99–12.89) greater than the metal-ceramic crown. No significant difference in the failure risk was observed when the survival rates were considered (HR=0.38, 95%CI (0.10 – 1.44), p=0.156). Co-variables did not affect the success and survival rates (p>0.05). Conclusions Metal-ceramic crowns showed a higher success rate than composite restorations. The survival rates were similar, but composite restorations presented a higher need for repairs. Clinical Significance Post-retained composite restorations may need more reinterventions during the lifecycle, although more preservation of sound tooth structure is expected with a large restoration of resin post-and-core. These aspects have to be discussed with the patient for decision-making planning.

Abstract:BACKGROUND:Previous studies show that the fracture resistant force of endodonticaly treated teeth can be improved by post. But this idea has been controversial in recent years. Many scholars believe that the root canal preparation might weaken dental root the post increases the risk of tooth fracture. OBJECTIVE: To evaluate the fracture resistance ability of severe wedge-shaped-defect premolar to oblique loading, which was restored with fiber reinforced composite posts by different therapy methods. METHODS:A total of 50 human maxilary premolars were randomly divided into five groups, with ten teeth in each group. They were given folowing treatments: Group A: 10 untreated premolars; Rest 40 premolars of Groups B, C, D and E were prepared 1/3 bucco-lingual distance for artificial severe wedge-shaped defects at the buccal cervix. Group B: untreated severe wedge-shaped defects premolars; Group C: severe wedge-shaped defects premolars were endodonticaly treated, remaining dentin over hang above the wedge shaped defect, LuxaPost posts reinforced in buccal canal and lingual canal, LuxaCore composite resin restored dentin defect; Group D: severe wedge-shaped defects premolars were endodonticaly treated, LuxaPost posts reinforced in buccal canal and lingual canal, LuxaCore composite resin restored dentin defect, and then covered with ful metal crown; Group E: severe wedge-shaped defects premolars were endodonticaly treated, removing dentin over hang above the wedge shaped defect, LuxaPost posts reinforced in buccal canal and lingual canal, restored with LuxaCore composite resin, and then covered with ful metal crown. Fracture resistance of each specimen was measured in each group. RESULTS AND CONCLUSION: The fracture strength of each group were Group A (1 002.69±147.62) N, Group B (439.28±66.34) N, Group C (958.30±101.23) N, Group D (1 207.09±143.48) N, and Group E (1 056.44±139.30) N. Group D had the highest fracture strength (P < 0.01), while Group B had the lowest fracture strength (P < 0.01). There were no significant difference among the fracture strength of Group A, Group C and Group E. Our findings indicated that the fracture resistance of the severe wedge-shaped defected premolar can be improved by fiber reinforced composite post and dentin above wedge shaped defect remained.

Abstract:OBJECTIVE:To evaluate the fatigue and fracture resistance of the flared roots restored with computer aided design (CAD) and computer aided manufacturing (CAM) glass fiber posts.METHODS:In the study, 32 maxillary central incisors with roots longer than 13 mm were selected and their canals were flared, and the roots were allocated into 4 groups (n=8) by a random number chart: CAD/CAM glass fiber posts, prefabricated quartz fiber posts, cast gold alloy posts, and CAD/CAM zirconia posts. The posts were luted to the roots by resin cement and fabricate zirconia crown for every specimen. An addition-type silicone impression material was used to simulate the periodontal ligament. All the specimens were submitted to 1.2×10(6) cycles loaded with a 49 N force, at 45 degree to the long axis of the teeth simultaneously with 3 000 thermal cycles (5 °C-50 °C-5 °C). After that, the specimens were subjected to a load at a crosshead speed of 1 mm/min in a servo-hydraulic testing machine applied at 45 degree to the long axis of the tooth until fracture. The data were subjected to ANOVA test and the patterns of the failure were examined.RESULTS:After the cycling loading, 4 crowns from prefabricated quartz fiber posts groups were deboned, and no other failure was found after the cycling loading; the fracture strengths of CAD/CAM glass fiber posts group [(441.5± 103.2) N] and cast gold alloy posts group [(462.9±170.0) N] were higher (F=4.613, P<0.05) than those of CAD/CAM zirconia posts group [(284.1±99.0) N] and prefabricated quartz fiber posts group [(315.4±112.3) N]; the entire specimens presented unfavorable failures.CONCLUSION:Although further in vitro and in vivo studies are necessary, the results of this study show that the use of CAD/CAM glass fiber posts and cast gold alloy posts may achieve better outcomes in flared roots than that of CAD/CAM zirconia posts and prefabricated quartz fiber posts.
