Simulation Study of the Potential Hazards of Cardiac Stimulation by Induced Eddy-Currents in Modern MRI

Abstract:In modern MRI, patients are exposed to strong, rapidly switching magnetic gradient fields that may be able to elicit nerve stimulation and cardiac stimulation. Based on a new, High Definition, Finite-Difference Time-Domain (HD-FDTD) method and a realistic inhomogeneous 10-mm-resolution human body model with tissue parameters, this paper firstly provides numerical results of an investigation into induced current spatial distributions inside human tissues when exposed to pulsed z-gradient fields. Then, various cardiac arrhythmia have been simulated based on our realistic heart-torso model and supposed that the induced current reaches the cardiac stimulation threshold. Simulation results show that the induced current is not strong enough to elicit ventricular fibrillation under nowadays several KHz switch frequency of gradient fields, but if raise the gradient field switch frequency further, it is with great risk to elicit ventricular fibrillation.

Abstract:In modern magnetic resonance imaging (MRI), patients are exposed to strong, time-varying gradient magnetic fields that may be able to induce electric fields (E-fields)/currents in tissues approaching the level of physiological significance. In this work we present theoretical investigations into induced E-fields in the thorax, and evaluate their potential influence on cardiac electric activity under the assumption that the sites of maximum E-field correspond to the myocardial stimulation threshold (an abnormal circumstance). Whole-body cylindrical and planar gradient coils were included in the model. The calculations of the induced fields are based on an efficient, quasi-static, finite-difference scheme and an anatomically realistic, whole-body model. The potential for cardiac stimulation was evaluated using an electrical model of the heart. Twelve-lead electrocardiogram (ECG) signals were simulated and inspected for arrhythmias caused by the applied fields for both healthy and diseased hearts. The simulations show that the shape of the thorax and the conductive paths significantly influence induced E-fields. In healthy patients, these fields are not sufficient to elicit serious arrhythmias with the use of contemporary gradient sets. However, raising the strength and number of repeated switching episodes of gradients, as is certainly possible in local chest gradient sets, could expose patients to increased risk. For patients with cardiac disease, the risk factors are elevated. By the use of this model, the sensitivity of cardiac pathologies, such as abnormal conductive pathways, to the induced fields generated by an MRI sequence can be investigated. Staff and administration perspectives in a one-year geriatric rehabilitation developmental process.

Abstract:A health care reform for senior citizens took effect in 1992 in Sweden. Shortcomings as regards geriatric rehabilitation were identified and, in 1995, governmental grants were assigned to local developmental projects. The aim of this study was to report the staff and administration perspectives in a geriatric rehabilitation developmental process over a 1-year period. In all, 782 staff and 45 administration respondents took part. The investigation was based on questionnaires and series of interviews. Significant positive changes with respect to collaboration between the authorities involved were demonstrated at the administrative level, while the most positive results at staff level concerned competence development in the group of employees without college education. A positive process as regards geriatric rehabilitation has started in the study district, indicating that governmental grants assigned for the stimulation of developmental activities are beneficial. Official goals for geriatric rehabilitation, points of departure for the organization of rehabilitation services and the content of general and specialized rehabilitation programmes are pinpointed as some of the areas deserving further attention.
Optimizing the trajectory of deep brain stimulation leads reduces RF heating during MRI at 3 T: Characteristics and clinical translation

Abstract:Radiofrequency (RF) induced tissue heating around deep brain stimulation (DBS) leads is a well-known safety risk during magnetic resonance imaging (MRI), hindering routine protocols for patients. Known factors that contribute to variations in the magnitude of RF heating across patients include the implanted lead's trajectory and its orientation with respect to the MRI electric fields. Currently, there are no consistent requirements for surgically implanting the extracranial portion of the DBS lead. Recent studies have shown that incorporating concentric loops in the extracranial trajectory of the lead can reduce RF heating, but the optimal positioning of the loop is unknown. In this study, we evaluated RF heating of 77 unique lead trajectories to determine how different characteristics of the trajectory affect RF heating during MRI at 3 T. We performed phantom experiments with commercial DBS systems from two manufacturers to determine how consistently modifying the lead trajectory mitigates RF heating. We also presented the first surgical implementation of these modified trajectories in patients. Low-heating trajectories included small concentric loops near the surgical burr hole which were readily implemented during the surgical procedure; these trajectories generated nearly a 2-fold reduction in RF heating compared to unmodified trajectories.Clinical Relevance- Surgically modifying the DBS lead trajectory can be a cost-effective strategy for reducing RF-induced heating during MRI at 3 T.

Analysis of Clinical Application of Magnetic Compatible Cardiac Implantable Electronic Device in a Chinese Population

Abstract:Objective The study was aimed to analyze the clinical application of magnetic compatible cardiac implantable electronic device (CIED) and influence of magnetic resonance investigation (MRI) on the parameters of CIED from our center.MethodsAll of the 545 patients implanted magnetic compatible CIED from August 2011 to December 2016 in the Second Affiliated Hospital,Zhejiang University School of Medicine were enrolled retrospectively.The baseline characteristics and the necessity of MRI were analyzed.We observed the changes of the parameters of CIED after MRI.ResultsAmong 545 patients,307 were male,and 238 were female,with an average age of (69.2±13.0) years old.Hypertension was found in 324(59.4%) patients,diabetes in 90(16.5%) patients,stroke in 72(13.2%) patients,coronary heart disease in 138(25.3%) patients,osteoarthropathy in 30(5.5%) patients,cancer in 52(9.5%) patients.Thirty-seven patients had single-chamber pacemaker,486 patients had dual-chamber pacemaker,17 patients had implantable cardiac defibrillator (ICD),2 patients had cardiac resynchronization therapy pacemaker(CRT-P)and 3 patients had cardiac resynchronization therapy defi brillator(CRT-D).Before the implantation,131 patients underwent 145 times of MR scan.In which 106 times for head,4 times for extremities,20 times for spine,7 times for hepatobiliary and pancreatic diseases,4 times for the heart and 4 times for other parts of the body.Post-procedural MR scan were carried out in 21 patients for 24 times.Among them,13 times were for the head,3 times for extremites,3 times for spine,3 times for hepatobiliary and pancreatic diseases,and 2 times for other parts of the body.The observation showed there was no significant change of parameters of CIED after MRI scanning.ConclusionThere is a growing trend for applying magnetic compatible CIED,and the request for MR scan in Chinese population might differ from those of foreign countries.MR scan is safe and effective for the patients with magnetic compatible CIED according to our center's experience.
Clinical safety of cardiac resynchronization therapy and implantable cardiac defibrillator implantation compatible with magnetic resonance imaging

Abstract:Objective To observe the safety of magnetic resonance imaging (MRI)-conditional cardiac resynchronization therapy,implantable cardiac defibrillator(CRT-D) implantation and postoperative MRI performance.Methods From December 2017 to March 2018,12 heart failure patients with poor response to medication therapy were implanted MRI-conditional CRT-D (Inventra 7 HF-T QP).Pacemaker parameters were observed during implantation,1 month after operation,before MRI scanning and 1,3 months after MRI examination.Results There were 7 males and 5 females.MRI was performed in 10 cases(83.3％).There were no significant changes of CRT-D electrical parameters during implantation,1 month after operation,before and 1,3 months after MRI scanning(all P＞0.05).The variations of electrical parameters at 1 month after MRI were (51 ± 31)Ω for right atrial impedance,(116 ± 90) Ω for left ventricular impedance,(26 ± 4) Ω for right ventricular impedance and (5.6 ± 2.1)Ω for defibrillation impedance.Three cases showed slight artifacts in MRI which did not affect diagnosis,while 1 case had severe artifact around the heart that hindered image reading.No threshold increasing,sensation abnormality and lead dislodgment,myocardium perforation were observed.Conclusion MRI-conditional CRT-D pacemaker is feasible and MRI scanning can be performed safely after implantation.

Abstract:Purpose: To evaluate safety of MRI in patients with fragmented retained leads (FRLs) through numerical simulation and phantom experiments. Methods: Electromagnetic and thermal simulations were performed to determine the worst-case RF heating of 10 patient-derived FRL models during MRI at 1.5 T and 3 T and at imaging landmarks corresponding to head, chest and abdomen. RF heating measurements were performed in phantoms implanted with reconstructed FRL models that produced highest heating in numerical simulations. The potential for unintended tissue stimulation was assessed through a conservative estimation of the electric field induced in the tissue due to gradient-induced voltages developed along the length of FRLs. Results: In simulations under conservative approach, RF exposure at B1+<2 uT generated CEM43<40 at all imaging landmarks at both 1.5 T and 3 T indicating no thermal damage for acquisition times (TA) <10 minutes. In experiments, the maximum temperature rise when FRLs were positioned at the location of maximum electric field exposure was measured to be 2.4 degrees Celsius at 3 T and 2.1 degrees Celsius at 1.5 T. Electric fields induced in the tissue due to gradient-induced voltages remained below the threshold for cardiac tissue stimulation in all cases. Conclusions: Simulation and experimental results indicate that patients with FRLs can be scanned safely at both 1.5 T and 3 T with majority of clinical pulse sequences.

A new method to improve RF safety of implantable medical devices using inductive coupling at 3.0 T MRI

Abstract:Objective: To enhance RF safety when implantable medical devices are located within the body coil but outside the imaging region by using a secondary resonator (SR) to reduce electric fields, the corresponding specific absorption rate (SAR), and temperature change during MRI. Materials and methods: This study was conducted using numerical simulations with an American Society for Testing and Materials (ASTM) phantom and adult human models of Ella and Duke from Virtual Family Models, along with corresponding experimental results of temperature change obtained using the ASTM phantom. The circular SR was designed with an inner diameter of 150 mm and a width of 6 mm. Experimental measurements were carried out using a 3 T Medical Implant Test System (MITS) body coil, electromagnetic (EM) field mapping probes, and an ASTM phantom. Results: The magnitudes of B1+ (|B1+|) and SAR1g were reduced by 15.2% and 5.85% within the volume of interest (VoI) of an ASTM phantom, when a SR that generates opposing electromagnetic fields was utilized. Likewise, the Δ|B1+| and ΔSAR1g were reduced by up to 56.7% and 57.5% within the VoI of an Ella model containing a copper rod when an opposing SR was used. Conclusion: A novel method employing the designed SR, which generates opposing magnetic fields to partially shield a sample, has been proposed to mitigate the risk of induced-RF heating at the VoI through numerical simulations and corresponding experiments under various conditions at 3.0 T.
The Feasibility and Safety of Endomyocardial Biopsy by Lumenless Pacing Lead-Sheath Directed Method During the Cardiac Implantable Electronic Device Implantation

Abstract:Background Patients with cardiovascular implantable electronic devices (CIEDs) indication are complicated with special cardiomyopathy or other unspecified cardiac abnormalities and may need endomyocardial biopsy (EMB). However, EMB by a bioptome is usually avoided to reduce the risk of lead displacement in the CIED peri-procedural period. Objective We aimed to assess the safety and feasibility of a novel approach for transvenous right ventricular (RV) EMB using the lead-sheath method (L-S-M) during CIED implantation and compared it to the traditional bioptome method (T-B-M). Methods Consecutive eighty patients referred for EMB were enrolled. In the L-S-M group, a sheath with a lumenless pacing lead was positioned toward the middle to apical interventricular septum under fluoroscopy (n=60). The CIED implantation was performed through the same venous access site. In the T-B-M group, a bioptome was used (n=20). The clinical characteristics, procedural details, success rate and complications were evaluated. Results 380 RV EMBs procedures were performed with comparable 4.1±0.8 in the L-S-M group and 3.8±0.8 samples/patient in the T-B-M group. In the L-S-M group, seven (11.7%) patients experienced minor complications with 3 transient right bundle branch block, 2 transient atrioventricular block (AVB) and 2 regional minor device pocket hematomas. In the T-B-M group, three (15%) experienced one temporary pacing for transient AVB, one chest discomfort and one regional hematoma. No cardiac tamponade was detected. Conclusions RV EMB by the innovative L-S-M method is technically feasible, safe and can yield valuable and early diagnostic insights for patients who are candidates for CIEDs.

Abstract:Alzheimer's disease (AD) is the most common neurodegenerative disease and is caused by an accumulation of A beta plaque deposits in the brains. Evidence is increasing that green tea flavonoids can protect cells from A beta-mediated neurotoxicity. However, the underlying mechanism remains unclear. Here, we used a human neuronal cell line MC65 conditional expression of an amyloid precursor protein fragment (APP-C99) to investigate the protection mechanism of epigallocatechin gallate (EGCG), the main constituent of green tea. We demonstrated that treatment with EGCG reduced the A beta levels by enhancing endogenous APP nonamyloidogenic proteolytic processing. Furthermore, EGCG also decreased nuclear translocation of c-Abl and blocked APP-C99-dependent GSK3 beta activation, and these inhibitory effects occurred through the interruption of c-Abl/Fe65 interaction. Our results indicated that the neuroprotective action of EGCG may take place through some mechanisms other than the promotion of APP nonamyloidogenic proteolysis, as was reported previously.

Abstract:This article studies the impact of strain relief loops on RF-induced heating of active implantable medical devices (AIMDs) during magnetic resonance imaging (MRI) scans at 1.5 T. Two commercially available AIMDs, a 40-cm lead length peripheral nerve stimulator (PNS) and a 60-cm lead length spinal cord stimulator (SCS), were utilized. In the in vitro study, lead-tip heating was calculated using the transfer function (TF) method and compared to direct measurement results for strain relief loops at various locations. In the human model analysis, RF-induced heating along six PNS and two SCS trajectories within the Duke, Ella, and FATS models was evaluated for strain relief loops positioned at various locations along the implantation trajectories. From the in vitro study, we can see up to a 48.3% reduction in the overall lead-tip heating. Inappropriate placement may result in doubled temperature rises. Significant temperature rise variation (up to 9°C) is observed with loop position, loop diameter, number of turns, and patient landmark. Differentiation of cumulative summation analysis is necessary to comprehend the strain relief loop's effect on RF-induced heating. The findings suggest that strategically placing the loop can effectively mitigate RF-induced heating for AIMDs exposed to 1.5-T MR environments.

Abstract:Though interstitial lung disease (ILD) can occur at any age in children, disorders more common in infancy and young children have received increased attention as an important group that is disproportionally affected, linked to lung development and lung injury, and represents disorders not seen in adult ILD. Identifying those children with potential children's ILD (chILD) and establishing a specific chILD diagnosis has evolved and is critical for pediatric pulmonologists, neonatologists, radiologists, and pathologists to recognize. Specific disorders more common in infancy include diffuse developmental disorders, growth abnormalities, pulmonary interstitial glycogenosis, neuroendocrine cell hyperplasia of infancy, and surfactant mutation dysfunction mutations. The presentation, evaluation, treatment, and clinical course are discussed for each of these specific disorders and other categories less common in infants and young children are briefly mentioned. Resources for physicians and families are also reviewed.
Safety Study of Implant Wearers in MRI RF Field Environment

Abstract:The complex electromagnetic field environments in magnetic resonance imaging system(MRI) can have a significant impact on patients carrying implants, the RF heating problems being particularly important. To ensure the safety of the patients, it is necessary to understand the distribution of tissue temperature in the MRI environment and its changes over time. Based on the analysis of tissue temperature rise in MRI, this paper constructs a bird cage coil for generating RF field in MRI system, and constructs ASTM standard/improved phantom and single-cavity pacemaker finite element models, use time-domain finite difference (FDTD) to simulate. Firstly, the correctness of the simulation software and simulation method was validated according to the method of ISO. Then the distribution of the electric field, SAR and temperature field and the temperature change with time were calculated in the environment of 64 MHz, 2 W/kg. The difference in temperature rise with blood heat exchange and no blood heat exchange (standard/improved phantom) was specifically compared. The simulation results show that there are electric field and SAR hotspots near the electrode tip, the wire tail and the case of pacemaker. There are high SAR values on both sides of the phantom, and the shorter the distance from the coil, the higher the SAR. The temperature field distribution is similar to the SAR distribution; the temperature is higher in the area around the end of the wire and the case of pacemaker because the heat accumulation is higher around this area. At the same time, blood heat exchange can reduce the temperature rise to a certain extent.

Abstract:Purpose To address the issue of RF‐induced heating for partially in and partially out (PIPO) medical devices during 1.5 T MRI scans by proposing a method of minimizing the external portion. Methods A method of tightly winding the external segment of the PIPO device is proposed to minimize the overall device effective reception length during MRI scans to mitigate the RF‐induced heating. Two commercially available PIPO medical devices and simplified solid wires were used to demonstrate the concept. RF heating results are compared between typical and minimized‐length trajectories under the American Society for Testing and Materials (ASTM) testing procedure. In addition, 16 scaled and validated device models were used in conjuncture with human body numerical simulations within three virtual human models to estimate clinically relevant heating. Results The wound segments in PIPO devices functioned as a lumped element rather than a receiving antenna, reducing induced energy/heating as compared to the original PIPO devices under typical straight or loop configurations. Minimizing the lead's external portion can reduce the RF‐induced heating by significant factors for all studied cases during ASTM phantom measurements and in human body simulations. Conclusion Our findings show a significant reduction in RF heating by minimizing the external segment, thereby enhancing patient safety during 1.5 T MRI procedures. Although limited to four devices at 1.5 T across two applications, the extent of heating reduction may vary for others. Nonetheless, tightly winding the external segment of PIPO electrodes holds promise for improving device safety under MRI.

Abstract:Background Magnetic resonance imaging (MRI) safety in patients with an epicardial cardiac implantable electronic device (CIED) is uncertain. Objective To assess the safety and adverse effects of MRI in patients who had surgically implanted epicardial CIED. Methods Patients with surgically implanted CIEDs who underwent MRI with an appropriate Cardiology–Radiology collaborative protocol between January 2008 and January 2021 were prospectively studied in two clinical centers. All patients underwent close cardiac monitoring through MRI procedures. Outcomes were compared between the epicardial CIED group and matched the non-MRI-conditional transvenous CIED group. Results Twenty-nine consecutive patients with epicardial CIED (male 41.4%, mean age of 43 years) underwent 52 MRIs in the 57 anatomic regions. Sixteen patients had pacemakers, 9 had cardiac defibrillators or cardiac resynchronization therapy defibrillators, and 4 had no device generators. There were no significant adverse events in epicardial or transvenous CIED groups. The battery life, pacing, sensing thresholds, lead impedance and cardiac biomarkers were not significantly changed, except one patient had a transient decrease in atrial lead sensing function. Conclusion MRI of CIEDs with epicardially implanted leads does not represent a greater risk than the transvenous CIEDs when performed with a multidisciplinary collaborative protocol centered on patient safety.

Safety evaluation of a leadless transcatheter pacemaker for magnetic resonance imaging use

Abstract:Background: Increased magnetic resonance imaging (MRI) adoption and demand are driving the need for device patients to have safe access to MRI. Objective: The aim of this study was to address the interactions of MRI with the Micra transcatheter pacemaker system. Methods: A strategy was developed to evaluate potential MRI risks including device heating, unintended cardiac stimulation, force, torque, vibration, and device malfunction. Assessment of MRI-induced device heating was conducted using a phantom containing gelled saline, and Monte Carlo simulations incorporating these results were conducted to simulate numerous combinations of human body models, position locations in the MRI scanner bore, and a variety of coil designs. Lastly, a patient with a Micra pacemaker who underwent a clinically indicated MRI scan is presented. Results: Compared to traditional MRI conditional pacemakers, the overall risk with Micra was greatly reduced because of the small size of the device and the absence of a lead. The modeling results predicted that the nonperfused temperature rise of the device would be less than 0.4°C at 1.5 T and 0.5°C at 3 T and that the risk of device heating with multiple device implants was not increased as compared with a single device. The clinical case study revealed no MRI-related complications. Conclusion: The MRI safety assessment tests conducted for the Micra pacemaker demonstrate that patients with a single device or multiple devices can safely undergo MRI scans in both 1.5- and 3-T MRI scanners. Impact of Patient Postures on the RF-Induced Heating for the Active Implantable Medical Device (AIMD) at 1.5T MRI

Abstract:This article explores the influence of patient postures on radiofrequency induced heating in active implantable medical devices (AIMDs) within a 1.5T magnetic resonance imaging system. Employing the transfer function method, the study assesses RF-induced heating for AIMDs in four different postures. Three AIMD types deep brain stimulation (DBS), cardiac rhythm management (CRM), and sacral neuromodulation (SNM)] are studied across three human models (Duke, Ella, and Fats) at four imaging landmarks under two RF exposure levels: B1+ and specific absorption rate (SAR) restrictions. When comparing temperature rises observed in various postures to those obtained from the typical/standard posture, it was noted that temperature rises can vary significantly. Specifically, temperature rises were found to have variations of up to 82% and 96% for prone and supine arm loop postures, respectively, while remaining within 27% for leg and trunk arm loop postures. Across DBS, CRM, and SNM systems, averaged temperature rises can vary up to 96%, 96%, and 89%, respectively. Noteworthy landmark-specific variations are observed, with changes of up to 43%, 33%, 95%, and 96% for head, cardiac, abdomen, and hip scans. The study underscores the significant impact of patient posture on AIMD heating, emphasizing the necessity for evaluations under diverse conditions.

Wirelessly interfacing sensor-equipped implants and MR scanners for improved safety and imaging

Abstract:Purpose: To investigate a novel reduced RF heating method for imaging in the presence of active implanted medical devices (AIMDs) which employs a sensor-equipped implant that provides wireless feedback. Methods: The implant, consisting of a generator case and a lead, measures RF-induced E $$ E $$ -fields at the implant tip using a simple sensor in the generator case and transmits these values wirelessly to the MR scanner. Based on the sensor signal alone, parallel transmission (pTx) excitation vectors were calculated to suppress tip heating and maintain image quality. A sensor-based imaging metric was introduced to assess the image quality. The methodology was studied at 7T in testbed experiments, and at a 3T scanner in an ASTM phantom containing AIMDs instrumented with six realistic deep brain stimulation (DBS) lead configurations adapted from patients. Results: The implant successfully measured RF-induced E $$ E $$ -fields (Pearson correlation coefficient squared [R2 ] = 0.93) and temperature rises (R2 = 0.95) at the implant tip. The implant acquired the relevant data needed to calculate the pTx excitation vectors and transmitted them wirelessly to the MR scanner within a single shot RF sequence (<60 ms). Temperature rises for six realistic DBS lead configurations were reduced to 0.03-0.14 K for heating suppression modes compared to 0.52-3.33 K for the worst-case heating, while imaging quality remained comparable (five of six lead imaging scores were ≥0.80/1.00) to conventional circular polarization (CP) images. Conclusion: Implants with sensors that can communicate with an MR scanner can substantially improve safety for patients in a fast and automated manner, easing the current burden for MR personnel.
Comparative Study of RF Heating in Deep Brain Stimulation Devices During MRI at 1.5 T and 0.55 T: Challenging the Assumption of Safety at Low Field Strengths

Abstract:Purpose: Low-field MRI has been assumed to be implant-friendly based on limited studies. However, RF-induced heating due to an implant is a complex resonance phenomenon, highly dependent on the implant's configurations and the applied RF frequencies. This study aims to evaluate the RF heating of DBS implants during MRI at low-field strengths compared to higher field 1.5 T MRI. Methods: A commercial deep brain stimulation (DBS) implant was used in full system as well as lead only configurations to evaluate and compare RF heating during MR imaging at 0.55 T and 1.5 T. The transfer function of the device at both configurations was measured and validated at each of the frequencies, which was then used for the in vivo prediction of RF heating for realistic DBS configurations at head, chest and abdomen imaging landmarks. Results: For the lead only case, the RF heating due to the DBS was substantially smaller during imaging at 0.55 T compared to that at 1.5 T. However, for the full DBS system (longer implant), the RF heating at 0.55 T was comparable to and for some cases even higher than that at 1.5 T, reaching a level that poses risk of tissue damage in patients. Conclusions: While RF heating generally tends to be lower at low-field MRI, the case with longer implanted leads demands extra caution, due to the higher possibility of matching resonant condition at low-field-strength frequencies. Thus, specific risk evaluation for each implant and configuration is required rather than assuming that lower field strength imaging is safer.
