Abstract:Combretastatin A4 phosphate (CA4P) represents the lead compound in a group of novel tubulin depolymerising agents being developed as vascular targeting agents (VTAs). VTAs are drugs that induce rapid and selective vascular dysfunction in tumours. CA4P is a water-soluble prodrug of the cis-stilbene CA4 originally isolated from the tree Combretum caffrum. Preclinical studies have shown that CA4P induces blood flow reductions and subsequent tumour cell death in a variety of preclinical models. Moreover, this activity has been linked to its ability to rapidly alter the morphology of immature endothelial cells by disrupting their tubulin cytoskeleton. Phase I clinical trials have established a maximum tolerated dose in the range 60 – 68 mg/m2 and in addition have established that significant changes to tumour perfusion can be achieved across a wide range of doses. The dose-limiting toxicities include tumour pain, ataxia and cardiovascular changes. The maximum tolerated dose was independent of schedule, indicating the absence of cumulative toxicity. Although unexpected from preclinical studies, some evidence of clinical response was seen using CA4P as a single modality. Based on the Phase I data, combination studies of CA4P with established therapies are in progress and should determine whether the exciting preclinical data obtained when VTAs are used in combination with cytotoxic chemotherapy, radiation, radioimmunotherapy and even antiangiogenic agents, can be translated into man.

Abstract:Objective To set up the reference method for the packed cell volume(PCV).Methods The microhematocrit method for PCV was established according to the NCCLS' guideline and the performance of the method was evaluated;meanwhile the results of hematocrit(Hct) detected by Sysmex SE 9500 and 1800i were compared with the results of PCV obtained by microhematocrit method.Results The minimum centrifugation time was 4.5 min.There was no difference among the dimensions of capillary tubes(P>0.05).The within-run,between-run and overall coefficient of variance(CV) was less than 1%.There was minimal interference for PCV at high concentration of Hb(3g/L),triglyceride(TG=28.9mmol/L),bilirubin(DB=272.7mmol/L),and PLT(846×109/L)(P>0.05).Correlation between the SE 9500/1800i and reference method was excellent(r>0.975,P<0.001).Conclusion These finding demonstrate that the microhematocrit method for PCV is simple,precise,accurate,strong for anti-interference and it could be used to calibrate the Hct for CBC analyzer.

Abstract:Background Stem cell apheresis in the context of autologous stem cell transplantation requires an accurate cluster of differentiantion 34 (CD34+) count determined by flow cytometry as the current gold standard. Since flow cytometry is a personnel and time‐intensive diagnostic tool, automated stem cell enumeration may provide a promising alternative. Hence, this study aimed to compare automated hematopoietic progenitor enumeration carried out on a Sysmex XN‐20 module compared with conventional flow cytometric measurements. Methods One hundred forty‐three blood samples from 41 patients were included in this study. Correlation between the two methods was calculated over all samples, depending on leukocyte count and diagnosis. Results Overall, we found a high degree of correlation (r = 0.884). Furthermore, correlation was not impaired by elevated leukocyte counts (>10 000/μL, r = 0.860 vs 20 000/μL, r = 0.843 vs <20 000/μL, r = 0.875). However, correlation was significantly impaired in patients with multiple myeloma (multiple myeloma r = 0.840 vs nonmyeloma r = 0.934). Summary Stem cell measurement carried out on the Sysmex XN‐20 module provides a significant correlation with flow cytometry and might be implemented in clinical practice. In clinical decision‐making, there was discrepancy of under 15% of cases. In multiple myeloma patients, XN‐20 should be used with caution.

Assessing Accuracy and Precision of Hemoglobin Determination in Venous, Capillary Pool, and Single-Drop Capillary Blood Specimens Using three Different HemoCue® Hb Models: The Multicountry Hemoglobin Measurement (HEME) Study

Abstract:Background: Anemia prevalence estimates reported in population surveys can vary based on the blood specimen source (capillary or venous) and analytic device (hematology autoanalyzers or portable hemoglobinometers) used for hemoglobin (Hb) determination. Objectives: This study aimed to compare accuracy and precision of Hb measurement in three blood specimen types on three models of hemoglobinometers against the results from venous blood from the same individuals measured on automated analyzers (AAs). Methods: This multisite (Cambodia, Ethiopia, Guatemala, Lebanon, Nigeria, and Tanzania) study assessed Hb measurements in paired venous and capillary blood specimens from apparently healthy women (aged 15-49 y) and children (aged 12-59 mo) using three HemoCue® Hb models (201+, 301, and 801). Measurements were compared against reference values: venous blood in hematology AA and adjusted via regression calibration or mean difference in HemoCue® Hb. Venous, capillary pool, and single-drop capillary blood specimens were assessed for accuracy and precision. Results: Venous blood measured using HemoCue® Hb 301 exhibited a positive mean error, whereas responses in HemoCue® Hb 201+ and 801 were nondirectional compared with the reference. Adjustment with the reference harmonized mean errors for all devices across study sites to <1.0 g/L using venous blood. Precision was highest for venous blood (±5-16 g/L) in all sites, lowest for single-drop capillary (±9-37 g/L), and intermediate (±9-28 g/L) for capillary pool blood specimen. Imprecision differed across sites, especially with both capillary blood specimens, suggesting different levels of personnel skills. Conclusions: Findings suggest that venous blood is needed for accurate and precise Hb determination. Single-drop capillary blood use should be discouraged owing to high measurement variability. Further research should evaluate the viability and reliability of capillary pool blood for this purpose. Accuracy of HemoCue® Hb devices can be improved via standardization against results from venous blood assessed using AA.

Abstract:Severe intravascular hemolysis leads to the simultaneous presence of free heme pigments (oxyhemoglobin, methemoglobin, and methemalbumin) and bilirubin in human plasma. Standard spectrophotometric methods used to assess in vivo hemolysis inadequately address this complex analytical situation. Thus, we propose a novel quantification algorithm to ensure the highest analytical specificity. A corresponding second‐derivative fitting algorithm was validated according to the guideline of bioanalytical method validation from the European Medicines Agency using plasma specimens (n=1759) spiked with different concentrations of oxyhemoglobin and methemoglobin. The results were compared to standard spectrophotometric quantification methods described by Harboe, Noe, and Fairbanks. Based on the second‐derivative method, simultaneous quantification of oxyhemoglobin and methemoglobin / methemalbumin in samples with total bilirubin concentrations ≤ 4.9 mg/dL (83.8 μmol/L) provided robust results (inaccuracy ≤ 20 %, imprecision ≤ 16 %). Analyzing UV/VIS spectra of plasma from patients with confirmed severe intravascular hemolysis evidenced an underestimation of up to 33 % for the combined free heme pigment content. The employed second‐derivative algorithm allows for automated and highly specific quantification of the free heme pigment content in diluted human plasma, which cannot be realized with standard spectrophotometric evaluation methods. Abstract:Abstract Background Cardiovascular magnetic resonance (CMR)–derived extracellular volume (ECV) requires a hematocrit (Hct) to correct contrast volume distributions in blood. However, the timely assessment of Hct can be challenging and has limited the routine clinical application of ECV. The goal of the present study was to evaluate whether ECV measurements lead to significant error if a venous Hct was unavailable on the day of CMR. Methods 109 patients with CMR T1 mapping and two venous Hcts (Hct 0 : a Hct from the day of CMR, and Hct 1 : a Hct from a different day) were retrospectively identified. A synthetic Hct (Hct syn ) derived from native blood T1 was also assessed. The study used two different ECV methods, (1) a conventional method in which ECV was estimated from native and postcontrast T1 maps using a region-based method, and (2) an inline method in which ECV was directly measured from inline ECV mapping. ECVs measured with Hct 0 , Hct 1 , and Hct syn were compared for each method, and the reference ECV (ECV 0 ) was defined using the Hct 0 . The error between synthetic (ECV syn ) and ECV 0 was analyzed for the two ECV methods. Results ECV measured using Hct 1 and Hct syn were significantly correlated with ECV 0 for each method. No significant differences were observed between ECV 0 and ECV measured with Hct 1 (ECV 1 ; 28.4 ± 6.6% vs. 28.3 ± 6.1%, p = 0.789) and between ECV 0 and ECV calculated with Hct syn (ECV syn ; 28.4 ± 6.6% vs. 28.2 ± 6.2%, p = 0.45) using the conventional method. Similarly, ECV 0 was not significantly different from ECV 1 (28.5 ± 6.7% vs. 28.5 ± 6.2, p = 0.801) and ECV syn (28.5 ± 6.7% vs. 28.4 ± 6.0, p = 0.974) using inline method. ECV syn values revealed relatively large discrepancies in patients with lower Hcts compared with those with higher Hcts. Conclusions Venous Hcts measured on a different day from that of the CMR examination can still be used to measure ECV. ECV syn can provide an alternative method to quantify ECV without needing a blood sample, but significant ECV errors occur in patients with severe anemia.

Laboratory evaluation of the Coulter "three-part electronic differential"

Abstract:Comparisons were made between "electronic" white blood cell (WBC) differential counts based on nuclear volume, differential counts performed by an automated image analyzer, and conventional manual counts performed by experienced technologists in a routine laboratory setting. The correlation between methods was excellent for lymphocytes (r = 0.9) and granulocytes (r = 0.9), but none of the three methods that were tested produced good results in enumeration of mononuclear cells (r = 0.6). Approximately 85% of the samples on which differentials were requested for an inpatient population could be processed by the electronic counter. Although the electronic counter failed to flag all abnormal samples (there were 6% false negatives), the performance of technologists doing 200 cell differentials was similar. Rapidly generated "electronic differentials" might be a useful and cost-effective adjunct to inpatient hospital practice if certain suggestions were implemented.

Abstract:Background: The authors report the relevance of using a point of care test (Helge ® ) for free hemoglobin determination and concordance of the values the with Cobas ® 8000 and spectrophotometer methods. Results: The within-run of the point of care test was <3%. Good correlations among the three methods were observed and an acceptable concordance for hemolysis index values from 50 mg/dl. An excellent agreement between the Cobas 8000 and the spectrophotometer was found. Conclusion: Automated methods represent methods of choice for free hemoglobin determination. An advantage of the Helge system is that it can be applied to samples experiencing a delay in evaluation due to the long distance between the collection site and the central laboratory. Another advantage is its use at the bedside, in the monitoring of extracorporeal membrane oxygenation patients.

The white blood cell differential. Evaluation of rapid impression scanning versus the routine manual count

Abstract:A quick, one and a half-minute qualitative microscopic scan was investigated as an alternative approach to the more labor-intensive 100-cell differential white blood cell count. The scanning results of 400 randomly selected hospital cases were compared with the on-line results of the 100-cell counts. Additionally, 50 cases selected to have a high percentage of abnormal results were each scanned and manually counted by four different readers. The results indicate that the scanning differential is equivalent to the 100-cell manual count in the detection of the presence of abnormal cell types such as immature granulocytes and blasts. Its ability to properly estimate the relative proportions of normal cells, especially lymphocytes, however, does not appear as reliable as the manual count. Most importantly, the analysis demonstrates that the scanning differential count exhibits a set of advantages and disadvantages that is complementary to those of the "three-part differential" technic provided by the newer generation automated hematologic analyzers. The authors therefore propose that these two procedures used in combination offer a suitable alternative to the manual 100-cell differential count.
Capillary drops, capillary pooled, and venous blood samples for determining hemoglobin concentration using HemoCue: A measurement system analysis

Abstract:Several external and internal factors can affect the performance and variability of Hemoglobin concentration [Hb] measurements using HemoCue, and documentation on the contribution of different sources of [Hb] variation is limited. We used an experimental repeated measurements design with nine randomly selected participants, three HemoCue devices, and three trained field workers. HemoCue measurements for all samples were repeated three times. The [Hb] measurement system considers four sources of error: 1) HemoCue devices, 2) field workers, 3) between individuals, and 4) within individuals. A concordance analysis was used to assess accuracy and precision, and a linear mixed model was used to estimate mean differences (bias) from blood specimens, anticoagulants, and to estimate the contribution of the 4 sources of error to [Hb] measurements. Positive mean [Hb] differences were found: 1.34 g/dL for capillary drops, 0.81 g/dL for pooled capillary blood samples, 0.756 g/dL for venous blood stored with EDTA, and 0.911 g/dL for venous blood stored with heparin. The mean [Hb] difference for venous blood with EDTA was used as a correction factor for all results measured using a HemoCue. After adjustment, capillary drops showed a mean difference of 0.585 g/dL, and pooled capillary samples were not significantly different. The individual variability was 95.8% of total variance, HemoCue devices contributed 2.1% of measurement error, field staff contributed 0.4%, and the residual was 1.7%. The HemoCue [Hb] measurement system is reliable in controlled environments, with a small measurement error of 4.2%.

Abstract:Background: Automated hematology analyzers offer precise hemoglobin measurements, but are expensive and impractical for field, point of care, primary care and remote settings use. The portable and cost-effective Hemocue device provides an alternative. Comparing their accuracies is crucial to prevent diagnostic discrepancies and misdiagnoses. This study aimed to determine the accuracy of Hb HemoCue machine by comparing its performance to automated analyzer at KCMC clinical laboratory where both equipment are used. Methods: A cross-sectional study was conducted at Kilimanjaro Christian Medical Centre (KCMC) Clinical Laboratory among adult patients whose hemoglobin concentrations were measured in May to June 2024. Hemoglobin levels were estimated using two distinct methods: the Hb HemoCue machine and repeatedly tested using an automated hematology analyzer. Results: Hemoglobin (Hb) concentration values obtained from the HemoCue machine and the automated analyzer, had a mean difference of 0.001 g/dl (95% Cl: -0.036 to 0.038), t value of 0.062, and a p-value of 0.95, indicating a non-statistically significant differences between the two measurement methods. The Bland-Altman plot analysis indicated that the mean difference (bias) between the two methods was 0.0012 g/dL, and the limits of agreement ranged from -0.481 to 0.482 g/dL, suggesting that the HemoCue machine tends to slightly overestimate Hb values compared to the automated hematology analyzer. The Pearson correlation coefficient for the Hb concentrations measured using HemoCue and automated analyzer was 0.995, indicating a very strong positive correlation. Receiver operating characteristics (ROC) curve showed that the area under the curve (AUC) for analyzer and HemoCue was 1.000 indicating that both methods have good diagnostic accuracy of measuring Hb concentrations. Conclusion: The study revealed strong agreement between HemoCue and automated hematology analyzer for measuring hemoglobin concentrations. Both methods demonstrated high diagnostic accuracy suitable for clinical use. Although HemoCue slightly overestimated hemoglobin, this difference was deemed insignificant. The study endorses HemoCue as a reliable tool for hemoglobin concentration measurement alongside automated analyzers.

Nanometer precise red blood cell sizing using a cost-effective quantitative dark field imaging system

Abstract:Because of the bulk, complexity, calibration requirements, and need for operator training, most current flow-based blood counting devices are not appropriate for field use. Standard imaging methods could be much more compact, inexpensive, and with minimal calibration requirements. However, due to the diffraction limit, imaging lacks the nanometer precision required to measure red blood cell volumes. To address this challenge, we utilize Mie scattering, which can measure nanometer-scale morphological information from cells, in a dark-field imaging geometry. The approach consists of a custom-built dark-field scattering microscope with symmetrically oblique illumination at a precisely defined angle to record wide-field images of diluted and sphered blood samples. Scattering intensities of each cell under three wavelengths are obtained by segmenting images via digital image processing. These scattering intensities are then used to determine size and hemoglobin information via Mie theory and machine learning. Validation on 90 clinical blood samples confirmed the ability to obtain mean corpuscular volume (MCV), mean corpuscular hemoglobin concentration (MCHC), and red cell distribution width (RDW) with high accuracy. Simulations based on historical data suggest that an instrument with the accuracy achieved in this study could be used for widespread anemia screening.

Abstract:BACKGROUND: Screening of hemoglobin (Hb) before blood donation is one among the vital tests. It is performed to select a blood donor to prevent the collection of blood from an anemic person. However, no accurate, cost-effective, reliable, and standardized method is available to estimate Hb. OBJECTIVE: The aim is to evaluate the efficacy of filter paper cyanmethemoglobin (FPCH) method with the automated hematology analyzer in the estimation of Hb concentration for screening of a suitable donor. METHODOLOGY: This was a cross-sectional study in which the blood samples of 2000 patients visiting KLE's Dr. Prabhakar Kore Charitable Hospital, Belagavi, were collected in vials and directly estimated for Hb using automated hematology analyzer. To evaluate the efficacy of FPCH, 20 μL of blood sample was transferred onto Whatman filter paper and dried at room temperature. After drying, it was placed in 5 mL of Drabkin's solution for 30 min. Optical density was estimated by measuring the absorbance. Data were analyzed using SPSS version 20. The correlation coefficient, paired t -test, and difference between the means of both the methods were calculated. RESULTS: The mean Hb estimated by FPCH was 11.25 g/dL and automated hematology analyzer gave 11.35 g/dL. The difference in the means of both the methods was 0.1 g/dL. Paired t -test was done to test the level of significance and the result was 8.151 (95% confidence interval: 0.08–0.13 g/dL, P < 0.001). The correlation coefficient was found to be 0.976 ( P < 0.001). CONCLUSION: FPCH is an efficient method, which is comparable to the automated hematology analyzers for Hb estimation. It could be used as an alternative screening tool for detection of Hb in a blood donation camp.

Method comparison between Alinity hq and Sysmex XT-4000i in an emergency laboratory

Abstract:Due to technological advancements, haematology analysers are becoming increasingly more complex. Before introducing new analyzers, laboratories must compare the agreement between the new and the old instruments. This study aimed to quantify the method agreement between Sysmex XT-4000i and Alinity hq analysers in order to establish whether they can be used interchangeably. A total of 415 complete blood counts (CBC) from adult patients of the Emergency Clinical County Hospital of Târgu Mureș, Romania, were analysed within 4 h from the collection on Sysmex XT-4000i (considered the reference method), then on Alinity hq. Statistical analysis consisted of outlier removal, Spearman Correlation, Bland-Altman test, and Passing-Bablok regression. For each CBC parameter, the analytical difference between methods was compared with the Reference Change Value (RCV) at medical decision levels (MDL). Despite using different technologies, the instruments have a good agreement regarding cell differentiation and counting. Cell counting and haemoglobin measurement showed a good agreement at all (Medical Decision Limits) MDLs. The analytical difference between methods surpassed the (Reference Change Value) RCV with 1.2% at the 14% MDL of HCT and with 0.2% at the 100 fL MDL of MCV. This study can not tell whether Sysmex or Alinity is superior, only if the two methods agree. The poorer agreement observed for RBC indices, especially MCHC, suggests an accumulation of differences caused by the different working principles of the two methods. However, it is reasonable to assume that such small differences will not affect clinical decision-making and patient outcome.

Abstract:The ELite 580 (Erba Lachema) Hematology Analyzer is a newly developed automated hematology analyzer that uses impedance and flow cytometry technologies. The analyzer provides 25 reportable parameters including a 5-part differential and additionally 4 research parameters with a throughput of 80 samples per hour. To evaluate the performance of the fully automated 5-part differential hematology analyzer ELite 580 as per the international standards and correlate the results with a reference analyser, LH 780 from Beckman Coulter. We evaluated the analyzer's performance by determining background noise, carryover, precision, linearity and correlation of complete blood cell count parameters between the ELite 580 from Erba Lachema and the LH 780 from Beckman Coulter at a tertiary care hospital, according to the Clinical and Laboratory Standards Institute guidelines. The ELite 580 showed minimal background noise and carryover, high precision, accuracy and linearity over a wide analytical range for white blood cells, hemoglobin, red blood cells, hematocrit, and platelet parameters. Correlation between the ELite 580 and the LH 780 was good for all parameters (R2 > 0.90) except for mean corpuscular hemoglobin concentration (MCHC) and the basophil count. ELite 580 Hematology Analyzer with its precise and accurate results and ease of operation is suitable for clinical use in medium to large sized hematology laboratories.
