Abstract:Abstract We describe the development and clinical evaluation of an automated smartphone-linked sensor capable of chemical-free, quantitative measurement of hemoglobin concentration ([Hb]) in whole blood samples. We have demonstrated that our sensor could analyze an unprocessed blood specimen with a mean processing time of <8 s and provided the [Hb] results with ~99% accuracy against a reference hematology analyzer with coefficient of variation (CV) of 1.21% measured at [Hb] = 11.2 g/dL. Its diagnostic capability for anemia was evaluated by measuring [Hb] of 142 clinical blood specimens and comparing the results with those from an automated hematology analyzer (ADVIA 2120i, Siemens AG, Germany) and a portable hemoglobinomteter (Hb201+, Hemocue, Sweden). The sensor yielded comparable sensitivities and specificities of 87.50% and 100.00% for males, and 94.44% and 100.00% for females, respectively, for anemic detection. The results suggested that our optical sensor based on the intrinsic photothermal response of Hb molecules and advances in consumer electronics, particularly smartphone capabilities, enables a direct, chemical-free [Hb] assay accessible to people in both developed and developing countries.

Abstract:Vitronectin, a multifunctional glycoprotein present in blood and extracellular matrix, is not only a member of the cell adhesion molecules, but also a regulator of proteolytic enzyme cascades, thereby providing a unique regulatory factor for proteolytic degradation of extracellular matrix and tissue remodeling. Vitronectin interacts with the cell surface through integrins of the alpha(v)-related system. Because vitronectin and its receptor may have a role in various renal physiological and pathological processes, we evaluated their expression in renal tissues of streptozotocin-induced short- and long-term hyperglycemic rats by applying quantitative immunoelectron microscopy and Western blot analysis. Vitronectin was shown over the glomerular basement membrane (GBM) and mesangial matrix (MM), whereas alpha(v) was located along the plasma membrane of endothelial, epithelial, and mesangial cells. Although distribution patterns of vitronectin and alpha(v) integrin labeling in renal tissues from short- and long-term hyperglycemic rats, as well age-matched normoglycemic rats, were similar, increases in their immunoreactive sites were detected in hyperglycemic conditions. Changes also were present in old compared with young normoglycemic animals. The diabetes-related increase in vitronectin was more significant in the GBM than MM, whereas the increase in alpha(v) integrin was as significant in podocytes as mesangial cells. Western blot analysis, performed on isolated glomerular material from normoglycemic and hyperglycemic animals, confirmed those changes. Our results suggest that vitronectin and its receptor, alpha(v) integrin, must have defined roles in molecular mechanisms involved in the pathogenesis of both diabetic and aging nephropathy.

Abstract:Ascorbic acid, dopamine, and uric acid are important electroactive biomolecules for health monitoring and they coexist in serum or urine. Their quantitative determination by electrochemistry could provide the accurate reference for diseases diagnosis and treatment. However, the traditional electrochemical workstations are too large for on-field inspection. Hence, the design of handheld electrochemical system for the detection of biomolecules is significant for point-of-care testing (POCT). In this paper, a smartphone-based integrated voltammetry system using modified electrode was developed for simultaneous detection of biomolecules. The system contained a disposable sensor, a coin-size detector, and a smartphone equipped with application program. Screen-printed electrodes were used as sensors for the detection, on which reduced graphene oxide and gold nanoparticles were electrochemically deposited by the system. The detector was used with voltammetric methods, in which excitation voltage was applied on the sensors and subsequent current responses were detected. The smartphone is the core component to communicate with the detector, calculate data, and plot voltammograms in real-time. Then, the system was applied to detect standard solutions of the biomolecules and their mixtures as examples. The results showed that the peak currents of each substance increased with higher concentration and the method allowed the discrimination of the different potentials of the studied species. Finally, the practical applications of the system were tested through detections of the biomolecules in artificial urine. The results exhibited that the system could be used to detect electrochemical activity of biomolecules with linear, high sensitivity, and specific responses in point-of-care testing.

Abstract:Proteins play crucial roles in biological activities and show close relationship with human diseases as biomarkers. Thus, rapid, sensitive, selective, and portable detection of protein is in high demand for point-of-care (POC) testing. Here, a smartphone-controlled biosensor system is developed with electrochemical impedance spectroscopy (EIS) to detect proteins for POC testing. The system was constituted by miniaturized biosensor, hand-held EIS detector, and smartphone, collectively. Printed carbon electrodes and interdigital gold electrodes, were modified with bio-components and integrated into the system as biosensors for protein detection. Then, detections of bull serum albumin (BSA) and thrombin demonstrated analytical performances of the system for quantification of the protein binding and enzyme activities by special antibodies and peptides immobilized on the electrodes. By the impedance measurement, the system could detect BSA and thrombin as low as 1.78 mu g/ml and 2.97 ng/ml, respectively. The results suggested that the smartphone-controlled biosensor system provided a portable and cost-effective platform that allowed integration of diverse biosensor formats for different kinds of protein quantifications for POC testing. Paper-Based Microfluidic Analytical Device Patterned by Label Printer for Point-of-Care Blood Glucose and Hematocrit Detection Using 3D-Printed Smartphone Cassette

Abstract:This study presents a portable, low-cost, point-of-care (POC) system for the simultaneous detection of blood glucose and hematocrit. The system consists of a disposable origami microfluidic paper-based analytical device (μPAD) for plasma separation, filtration, and reaction functions and a 3D-printed cassette for hematocrit and blood glucose detection using a smartphone. The origami μPAD is patterned using a cost-effective label printing technique instead of the conventional wax printing method. The 3D-printed cassette incorporates an array of LED lights, which mitigates the effects of intensity variations in the ambient light and hence improves the accuracy of the blood glucose and hematocrit concentration measurements. The hematocrit concentration is determined quantitatively by measuring the distance of plasma wicking along the upper layer of the origami μPAD, which is pretreated with sodium chloride and Tween 20 to induce dehydration and aggregation of the red blood cells. The filtered plasma also penetrates to the lower layer of the origami μPAD, where it reacts with embedded colorimetric assay reagents to produce a yellowish-brown complex. A color image of the reaction complex is captured using a smartphone inserted into the 3D-printed cassette. The image is analyzed using self-written RGB software to quantify the blood glucose concentration. The calibration results indicate that the proposed detection platform provides an accurate assessment of the blood glucose level over the range of 45–630 mg/dL (R2 = 0.9958). The practical feasibility of the proposed platform is demonstrated by measuring the blood glucose and hematocrit concentrations in 13 human whole blood samples. Taking the measurements obtained from commercial glucose and hematocrit meters as a benchmark, the proposed system has a differential of no more than 6.4% for blood glucose detection and 9.1% for hematocrit detection. Overall, the results confirm that the proposed μPAD is a promising solution for cost-effective and reliable POC health monitoring.

Abstract:While colorimetric-based assays are very convenient to determine biomarkers in point-of-care testing (POCT), they often suffer from pretreatment procedures for separation of plasma or serum from whole blood samples. Here, we report a simple colorimetric paper-based analytical device (c-PAD) that is capable of performing sample-to-answer analysis by directly dropping the whole blood sample on paper. This is accomplished by utilizing sodium alginate hydrogel, which exhibits a nanometer-scale porous structure to effectively prevent the passage of large red blood cells and hemoglobin molecules, to encapsulate enzymes and chromogenic reagents. As the small targets in the blood sample enter the sensing region to trigger a chromogenic reaction, the resulting color signal is recorded by a smartphone. The interference from the red blood to the color signal can be completely avoided without the requirement of any separation process. The analytical performance of the method is evaluated by assaying glucose in real blood samples. The results show that rapid and accurate analysis can be achieved with the limit of detection as low as 0.12 mM. In addition, simultaneous detection of different targets (glucose, cholesterol, and triglycerides) in whole blood can be achieved by fabricating c-PAD with multiple sensing regions. Owing to its several essential advantages including an extremely simple procedure for fabrication, sample-to-answer analysis without tedious pretreatment, and capability to perform high-throughput analysis, the proposed c-PAD will be of great value in POCT applications of whole blood samples.

Abstract:In this paper, we propose a smartphone-based biosensor for detecting human total hemoglobin concentration in vivo with high accuracy. Compared to the existing biosensors used to measure hemoglobin concentration, the smartphone-based sensor utilizes the camera, memory, and computing power of the phone. Thus, the cost is largely reduced. Compared to existing smartphone-based sensors, we developed a highly integrated multi-wavelength LED module and a specially designed phone fixture to reduce spatial errors and motion artifacts, respectively. In addition, we embedded a new algorithm into our smartphone-based sensor to improve the measurement accuracy; an L*a*b* color space transformation and the “a” parameter were used to perform the final quantification. We collected 24 blood samples from normal and anemic populations. The adjusted R2 of the prediction results obtained from the multiple linear regression method reached 0.880, and the RMSE reached 9.04, which met the accuracy requirements of non-invasive detection of hemoglobin concentration.

Abstract:The level of creatinine in the human body has clinical implications with regard to a potential association with kidney, muscle, and thyroid dysfunction, hence necessitating fast and accurate detection, especially at the point-of-care (POC) level. This paper presents the design, fabrication, and feasibility of a compact, low-cost and reliable POC photochemical biosensor connected to a smartphone for the determination of whole blood creatinine by differential optical signal readout. Disposable, dual-channel paper-based test strips were fabricated using stackable multilayer films pre-immobilized with enzymes and reagents for the identification and conversion of creatinine and creatine, resulting in dramatic colorimetric signals. A handheld optical reader was integrated with dual-channel differential optical readout to address endogenous interferences in the enzymatic assay of creatinine. We demonstrated this differential concept with spiked blood samples, obtaining a wide detection range of 20–1483 μmol/L and a low detection limit of 0.03 μmol/L. Further interference experiments displayed the differential measuring system's excellent performance against endogenous interference. Furthermore, the sensor's high reliability was confirmed through comparison with the laboratory method, with the results of 43 clinical tests consistent with the bulky automatic biochemical analyzer, with its correlation coefficient R 2 = 0.9782. Additionally, the designed optical reader is Bluetooth-enabled and can connect to a cloud-based smartphone to transmit test data, enabling active health management or remote monitoring. We believe the biosensor has the potential to be an alternative to the current creatinine analysis conducted in hospitals and clinical laboratories, and it has promising prospects for contributing to the development of POC devices.

Abstract:We describe a gold nanoparticle-based sandwich immunoassay for the dual detection and measurement of hemoglobin A1c (HbA1c) and total hemoglobin in the whole blood (without pretreatment) in a single step for personalized medicine. The optimized antibody-functionalized gold nanoparticles immunoreact simultaneously with HbA1c and total hemoglobin to form a sandwich at distinctive test lines to transduce visible signals. The applicability of this method as a personal management tool was demonstrated by establishing a calibration curve to relate % HbA1c, a useful value for type 2 diabetes management, to the signal ratio of captured HbA1c to all other forms of hemoglobin. The platform showed excellent selectivity (100%) toward HbA1c at distinctive test lines when challenged with HbA0, glycated HbA0 and HbA2. The reproducibility of the measurement was good (6.02%) owing to the dual measurement of HbA1c and total hemoglobin. A blood sample stability test revealed that the quantitative measurement of % HbA1c was consistent and no false-positive results were detected. Also, this method distinguished the blood sample with elevated HbF from the normal samples and the variants. The findings of this study highlight the potential of a lateral flow immunosensor as a simple, inexpensive, consistent, and convenient strategy for the dual measurement of HbA1c and total Hb to provide useful % HbA1c values for better on-site diabetes care.

Abstract:Today, easy availability and efficient detection assays in low-resource settings are mandatory. Therefore, a smartphone-assisted paper-based detection strategy using eosin Y (EY)-embed ZIF-8 nanocomposite for phosphate (Pi), neutral red (NR) and heparin (HP) on site testing was fabricated. After adding the targets in a specific order, the prepared strips produced brilliant color transitions (astonishing colors, ranging from pistachio to bright yellow for Pi, from bright yellow to jacinth for NR and from jacinth to light orange for HP). These variation on tonality can be evaluated by the smartphone-based readout device (Color Assist App) through the ratio change of B/(R + G + B), which exhibits superior performance of on-site detection in wastewater, including high sensitivity (LOD is 0.015, 0.031 and 0.014 mM for Pi, NR and HP, respectively), ultra-fast response time and excellent selectivity. This strips and smartphone dual-mode monitoring strategy may offer a convenient idea to identify Pi, NR and HP, with great prospects for environmental wastewater applications. Additionally, the probe was enabled for bioimaging of Pi, NR and HP in living cells. Finally, the related anti-counterfeiting fluorescent ink of probe was proposed on schedule.

Abstract:We suggest a low cost, non invasive healthcare system that measures haemoglobin levels in patients and can be used as a preliminary diagnostic test for anaemia. A combination of image processing, machine learning and deep learning techniques are employed to develop predictive models to measure haemoglobin levels. This is achieved through the color analysis of the fingernail beds, palpebral conjunctiva and tongue of the patients. This predictive model is then encapsulated in a healthcare application. This application expedites data collection and facilitates active learning of the model. It also incorporates personalized calibration of the model for each patient, assisting in the continual monitoring of the haemoglobin levels of the patient. Upon validating this framework using data, it can serve as a highly accurate preliminary diagnostic test for anaemia.

Abstract:Herein, a simple microfluidic paper-based analytical device (μPAD) by using platinum nanoparticles (Pt NPs) as highly active peroxidase mimic for simultaneous determination of glucose and uric acid was fabricated. The μPAD consisted of one sample transportation layer, four paper-based detection chips, and two layers of hydrophobic polyethylene terephthalate (PET) films. The four detection chips were immobilized with various chromogenic reagents, Pt NPs, and specific oxidase (glucose oxidase or uricase). H2O2 generated by specific enzymatic reactions could oxidize co-immobilized chromogenic reagents to produce colored products by using Pt NPs as efficient catalyst. The multi-layered structure of μPAD could effectively improve the color uniformity and color intensity. Total color intensity from each two detection chips modified with distinct chromogenic reagents were used for quantitative analysis of glucose and uric acid, respectively, resulting in significantly improved sensitivity. The linear range for glucose and uric acid detection was 0.01–5.0 mM and 0.01–2.5 mM, respectively. Satisfied results were obtained for glucose and uric acid detection in real serum samples. An easy-to-use smartphone APP was developed for convenient and intelligent detection. The developed μPAD integrated with smartphone as detector holds great applicability for simple and portable on-site analysis.

Abstract:It is of great significance to develop versatile and cost-effective point-of-care testing (POCT) systems for simultaneous quantitation of multiple analytes on-site. This work reports on: 1) the customized electrodes which exhibit good analytical performance for simultaneous detection of Cd2+, Cu2+, Hg2+, Pb2+ with low detection limits (Cd2+, 0.296 μM; Cu2+, 0.055 μM; Hg2+, 0.351 μM; and Pb2+, 0.025 μM) and wide dynamic ranges (Cd2+, 0.50–12.5 μM; Cu2+, 0.10–9.50 μM; Hg2+, 0.50–12.5 μM; and Pb2+, 0.10–12.5 μM) using differential pulse voltammetry (DPV) measurements. 2) a hand-held electrochemical analyzer which can realize two-way communication with a smartphone and implement DPV measurements; its cost, weight and power consumption are ~$10.0, ~30 g and ~ 253 mW, respectively; 3) a self-developed APP DHMI which can control the analyzer, receive data from the analyzer, and plot the voltammograms on smartphone's screen in real-time. This quantitative POCT system has been successfully demonstrated for simultaneous quantitation of Cd2+, Cu2+, Hg2+, Pb2+ in tap water, mineral water and Pearl River water, and the quantitation results correlated well with ICP-MS measurements. The features of cost-effectiveness, ease-to-use and reliability make this quantitative POCT system useful for on-site monitoring of multiple heavy metal ions, especially in resource-constrained environments.

Abstract:It is essential to develop portable, versatile, and reliable diagnostic devices in point-of-care testing (POCT). The detection of biomarkers requires selective separation and large specific surface for high sensitivity and accuracy at trace levels in whole blood samples using POCT devices. Herein, a kind of 3D electrochemical biosensors were designed via in-situ synthesizing polyaniline (PANI) and platinum nanoparticles (Pt NPs) on polysulfone hollow fiber membrane (HFM) scaffolds with gradient porous structure. The gradient porous HFMs scaffolds provide uniform capillary flow, self-driven blood separation and sufficient enzyme immobilization sites. Simultaneously, the in-situ deposited materials fulfill interconnected conductive networks, thus ensuring accurate and rapid detection of the sensors without hindering capillary progress. These sensors display ultralow sampling (similar to 3 mu L), fast fluid flow (>1 mu L/ms), wide linear range (glucose: 0-24 mM, R-2=0.992; cholesterol: 0-9 mM, R-2=0.999), high sensitivity and accuracy especially under different hematocrits in POCT applications towards glucose and cholesterol. The innovative integration of POCT biosensors with interconnected conductive nanoparticles, selective blood separation and gradient porous structures can find wide application in resource-limited regions, large population screening, and public health emergencies.

Abstract:Abstract: Sickle cell disease (SCD), a haematological illness that affects people all over the world, can lead to painful episodes, anaemia, organ damage, stroke, and even death. Sub-Saharan Africa and other nations with scarce resources are where it is more prevalent. Traditional laboratory-based diagnostic techniques for SCD are laborious, complicated, and unable to be completed at the point-of-care (POC) and at home. To distinguish between sickled and normal red blood cells (RBCs), optical microscope-based classification and counting requires a large amount of time, expensive setup, and experienced human labour. The need for a POC and home-based test to detect and track SCD and lower mortality in areas with limited resources exists. The effective therapy of SCD can benefit from an early and prompt diagnosis. This article discusses For the purpose of taking RBC photos from the SCD patients under normoxia and hypoxia, we used a smartphone-based image capture technique. To distinguish RBCs from the patient's blood before and after cell sickling, a computer algorithm is created. We acquired the same percentage of sickle cells in blood samples using the developed smartphone-based technology as those obtained using the traditional method (standard microscope). The created testing technique demonstrates the Potential savings from using a smartphone to administer the test could lower the entire cost of diagnosing and treating SCD, making the procedure more useful in environments with limited resources. Our system is very helpful in determining the seriousness of the SCD and does not call for any specific storage requirements. Abstract:A low-cost, robust, and simple mechanism to measure hemoglobin would play a critical role in the modern health infrastructure. Consistent sample acquisition has been a long-standing technical hurdle for photometer-based portable hemoglobin detectors which rely on micro cuvettes and dry chemistry. Any particulates (e.g. intact red blood cells (RBCs), microbubbles, etc.) in a cuvette's sensing area drastically impact optical absorption profile, and commercial hemoglobinometers lack the ability to automatically detect faulty samples. We present the ground-up development of a portable, low-cost and open platform with equivalent accuracy to medical-grade devices, with the addition of CNN-based image processing for rapid sample viability prechecks. The developed platform has demonstrated precision to the nearest $0.18[g/dL]$ of hemoglobin, an R^2 = 0.945 correlation to hemoglobin absorption curves reported in literature, and a 97% detection accuracy of poorly-prepared samples. We see the developed hemoglobin device/ML platform having massive implications in rural medicine, and consider it an excellent springboard for robust deep learning optical spectroscopy: a currently untapped source of data for detection of countless analytes.
