Abstract:Over the last decade, smartphones have changed radically to support us with mHealth technology, cloud computing, and machine learning algorithm. Having its multifaceted facilities, we present a novel smartphone-based noninvasive hemoglobin (Hb) level prediction model by analyzing hue, saturation and value (HSV) of a fingertip video. Here, we collect 60 videos of 60 subjects from two different locations: Blood Center of Wisconsin, USA and AmaderGram, Bangladesh. We extract red, green, and blue (RGB) pixel intensities of selected images of those videos captured by the smartphone camera with flash on. Then we convert RGB values of selected video frames of a fingertip video into HSV color space and we generate histogram values of these HSV pixel intensities. We average these histogram values of a fingertip video and consider as an observation against the gold standard Hb concentration. We generate two input feature matrices based on observation of two different data sets. Partial Least Squares (PLS) algorithm is applied on the input feature matrix. We observe R2=0.95 in both data sets through our research. We analyze our data using Python OpenCV, Matlab, and R statistics tool.

Abstract:Hemoglobin is the iron containing protein in red blood cells which carries oxygen from lungs to rest of the body tissues. Accurate measurement of hemoglobin is essential for diagnosing anemia, a condition characterized by a deficiency of red blood cells. This measurement is particularly vital before initiating blood transfusions for thalassemia patients. Non-invasive estimation of hemoglobin levels can be achieved through photoplethysmography (PPG)-based methods. PPG is an optical method to measure blood volume changes in successive heart beats. PPG signals can be obtained from fingertip videos using a light source and a photodetector. SmartphonePPG utilizes a smartphone's flashlight as a light source and its camera as a photodetector to acquire PPG signals, offering an affordable and portable point-of-care tool. Despite the ubiquity of smartphones, signals from their cameras often contain noise, making feature selection from PPG characteristics challenging. While PPG-based methods are invaluable, the lack of real-world datasets poses a significant challenge in maximizing the benefits of PPG technology. In this paper, we introduce a dataset comprising 1-minute fingertip video recordings from 150 anemic patients, obtained using a smartphone's camera. The dataset, publicly accessible for research purposes (https://forms.gle/LB4qn81ZMqEuy3V27), covers an age range of 6 months to 32 years, with diverse hemoglobin values (4.3 gm/dL - 12.4 gm/dL). Utilizing this dataset, we propose a deep learning-based technique employing the ResNet-18 architecture to estimate hemoglobin levels. This approach eliminates the need for manual feature extraction and selection from PPG signals, overcoming a limitation in existing smartphone PPG-based hemoglobin estimation systems. Our model achieves a hemoglobin level estimation with an RMSE of 0.81-1.39 when compared with the gold standard laboratory method, Complete Blood Count (CBC) test reports. In contrast, HemaApp, a state-of-the-art research utilizing a machine learning-based classifier (SVM), yields an RMSE of 1.7 on our dataset. The accuracy and simplicity of our model position it as a promising alternative to existing non-invasive hemoglobin level estimation methods.

Abstract:With the ongoing heart problems of the population worldwide, the medical requirements of the people are expected to increase. Electrocardiogram (ECG) is one of the proven to capture the heart response signal to assess the electrical and muscular functions of the heart. The ECG setup is expensive and needs proper training, and of course, it is not instant. For fast, accurate heart parameter monitoring, scientists pay attention to the photoplethysmogram signal (PPG), based on the light intensity of a particular wavelength. Android smartphone with a good quality camera has come to ordinary people's reach and has become one of the most necessary and rugged devices for today and future generations. We can use its powerful features to solve or assess heart state monitoring by capturing the image's necessary data. The mobile camera has a photo emitting diode and a photodetector. The light source illuminates the tissue. The photodetector calculates the small variation in light intensity associated with blood volume change in the vessels (mainly fingertips, toes, and ears). We have captured unfocused contact video to capture PPG using an Android Smartphone. Then, we removed a certain percent of camera touch errors based on average pixel intensity count in the red plane, and it is a new approach that has been introduced in this research. We used a 2nd order Butterworth (IIR) band pass filter for noise removal, FFT Hann Window for frequency analysis and leakage reduction. We have developed an algorithm using MATLAB as a development platform, for accurate pulse (BPM) measurement. Moreover, we have done a comparative analysis of developed algorithm with other available algorithms for PPG-based pulse calculation. Abstract:We suggest a low cost, non invasive healthcare system that measures haemoglobin levels in patients and can be used as a preliminary diagnostic test for anaemia. A combination of image processing, machine learning and deep learning techniques are employed to develop predictive models to measure haemoglobin levels. This is achieved through the color analysis of the fingernail beds, palpebral conjunctiva and tongue of the patients. This predictive model is then encapsulated in a healthcare application. This application expedites data collection and facilitates active learning of the model. It also incorporates personalized calibration of the model for each patient, assisting in the continual monitoring of the haemoglobin levels of the patient. Upon validating this framework using data, it can serve as a highly accurate preliminary diagnostic test for anaemia.

Abstract:Background: Human hemoglobin is a tetrameric metalloporphyrin. The heme part contains iron radicle and porphyrin. The globin part consists of two pairs of amino-acid chains. The absorption spectrum of hemoglobin spans from 250 nm to as high as 2,500 nm, with high coefficients reported in blue and green color zone. The visible absorption spectrum of deoxyhemoglobin has one, while the visible absorption spectrum of oxyhemoglobin shows two peaks. Objective: (1) To study absorption spectrometry of hemoglobin in 420 to 600 nm range; (2) to conduct preclinical experiments to validate a new device and technology based on green color absorption by hemoglobin; (3) to use this new technology and device for phase 1 study in healthy human volunteers for confirmation Design, material and methods: (1) Checking absorption spectrometry of hemoglobin in venous blood. We measured absorption spectrometry of 25 mother-baby pairs as an observational study. Readings were plotted from 400 nm to 560 nm. These included peaks, flat lines and deeps. Graph tracings of all samples – cord blood and maternal blood – showed similar patterns. (2) Preclinical experiments were set up (a) to correlate the reflection of green light by hemoglobin and concentration of hemoglobin, (b) to correlate concentration of O2 and reflection of green light related to oxyhemoglobin, (c) to correlate concentration of melanin in upper and the hemoglobin in lower layer of tissue phantom and to check the sensitivity of new device with green light for measuring Hb in presence of high levels of melanin, and lastly (d) to check if the new device can measure changes in oxy-hemoglobin and deoxy-hemoglobin, again in presence of high levels of melanin with normal as well as with low levels of hemoglobin. The experiments using bilayer tissue phantom were conducted with horse blood in lower cup as dermal tissue phantom and synthetic melanin in upper layer as epidermal tissue phantom. (3) Phase 1 observational studies following a protocol approved by the institutional review board (IRB) were done in two cohorts. Readings were taken using our device and a commercially available pulse oximeter. In the comparison arm we had Point of Care (POC) Hb test (HemoCu or iSTAT blood test). We had 127 data points of POC Hb test and 170 data points for our device and pulse oximeters. This device uses two wavelengths from the visible spectrum of light and uses reflected light. Light of specific wavelengths is shone on the skin of the individual, and the reflected light is collected as ‘optical signal’. This optical signal – after conversion to electrical signal – is processed and finally analysed with a digital display on the screen. Melanin is accounted using Von Luschan’s chromatic scale (VLS) and a specially designed algorithm. Results: In this set of various preclinical experiments using different concentrations of hemoglobin and melanin, we indeed demonstrated good sensitivity of our device. It could pick up signals from hemoglobin despite high levels of melanin. Our device is a non-invasive device to measure hemoglobin like a pulse oximeter. Results of our device and pulse oximeter were compared with those by POC Hb test like HemoCu and iSTAT. Our device showed better trending linearity and concordance than a pulse oximeter. Since the absorption spectrum of hemoglobin is the same is new-borns and adults, we could develop one device for all age groups and for people of all colors. Furthermore, the light is shone on the wrist of the individual and is then measured. So, in future this device has the potential of being incorporated in a wearable or smart watch technology.

Abstract:This work proposes a smartphone video-based approach for the estimation of blood glucose in a non-invasive way. Videos using smartphone camera are collected from the tip of the subjects finger and the frames are subsequently converted into Photoplethysmography (PPG) waveform. Gaussian filter along with Asymmetric Least Square methods have been applied on the PPG signals to remove the high-frequency noise, optical and motion interferences. Different signal features such as Systolic and Diastolic Peaks, the time difference between consecutive peaks (DelT), First Derivative peaks, and Second derivative peaks etc have been extracted from the processed signal. Finally, Principal Component Regression (PCR) has been applied for the prediction of glucose level from the extracted features. The proposed model, while applied to an unbiased dataset, could predict the glucose level with a Standard Error of Prediction (SEP) of around 18.31 mg/dL.

Abstract:[Purpose] Smartphones video cameras can be used to detect the photoplethysmograph (PPG) signal.The pulse wave signal detected by smartphone always mixed mass noise because of finger moving, unevenness of pressure and outer light interference. Previous studies limit to the filtering algorithm that denoising signals, without considering characteristics information of pulse wave itself. [Method] In this paper, we propose an algorithm based on wavelet to detect qualified PPG, which captures three critical characteristic quantities through wavelet high frequency coefficient. [Results] Experiment illustrates that the detected PPG signal contain dicrotic wave, and whats more, further experiment on artery elasticity indexes indicates good robust of the algorithm. [Conclusions] Wavelet Based Measurement on Photoplethysmography by Smartphone Imaging can be used for the calculation of cardiovascular parameter such as angiosclerosis, arrhythmia, and vascular resistance.

Abstract:Abstract We describe the development and clinical evaluation of an automated smartphone-linked sensor capable of chemical-free, quantitative measurement of hemoglobin concentration ([Hb]) in whole blood samples. We have demonstrated that our sensor could analyze an unprocessed blood specimen with a mean processing time of <8 s and provided the [Hb] results with ~99% accuracy against a reference hematology analyzer with coefficient of variation (CV) of 1.21% measured at [Hb] = 11.2 g/dL. Its diagnostic capability for anemia was evaluated by measuring [Hb] of 142 clinical blood specimens and comparing the results with those from an automated hematology analyzer (ADVIA 2120i, Siemens AG, Germany) and a portable hemoglobinomteter (Hb201+, Hemocue, Sweden). The sensor yielded comparable sensitivities and specificities of 87.50% and 100.00% for males, and 94.44% and 100.00% for females, respectively, for anemic detection. The results suggested that our optical sensor based on the intrinsic photothermal response of Hb molecules and advances in consumer electronics, particularly smartphone capabilities, enables a direct, chemical-free [Hb] assay accessible to people in both developed and developing countries.

Abstract:There is a significant unmet clinical need for a reliable point-of-care (POC) estimation of the blood haemoglobin (Hb) method. Current available methods, notably pulse oximetry, have certain limitations related to design and methodology of devices. These have low sensitivity for detecting serial change in the Hb values, especially at the lower range and are inaccurate in people with darker skin. Objective This study aimed at developing a novel, non-invasive technology for the measurement of Hb and oxygen saturation. Design This was an observational study. Recruitment This was approved by the Institutional Review Board at the University of Texas at Arlington and 16 healthy adult volunteers (age 20–40 years) were recruited in this study. The investigational device (Shani) probe (United States Patent 11191460B1) consists of light emitting diodes with wavelengths ranging 520–580 nm, and a photosensor component. The probe is gently placed on the back of the subject's wrist and reflected light is measured as an electrical signal, with digital recordings. Skin tone (or skin colour) was assessed by Von Luschan Chromatic Scale (VLS). Using a specific algorithm accounting for melanin (as determined from VLS Scale) and employing a software, the results can be displayed on screen as Hb values and ratio of tissue oxygen saturation. Results The results of the investigational non-invasive (Shani) device were comparable with the invasive, point of care (POC) method (iSTAT, Abbott Inc.).

Abstract:Blood glucose level needs to be monitored regularly to manage the health condition of hyperglycemic patients. The current glucose measurement approaches still rely on invasive techniques which are uncomfortable and raise the risk of infection. To facilitate daily care at home, in this article, we propose an intelligent, noninvasive blood glucose monitoring system which can differentiate a user's blood glucose level into normal, borderline, and warning based on smartphone photoplethysmography (PPG) signals. The main implementation processes of the proposed system include 1) a novel algorithm for acquiring PPG signals using only smartphone camera videos; 2) a fitting-based sliding window algorithm to remove varying degrees of baseline drifts and segment the signal into single periods; 3) extracting characteristic features from the Gaussian functions by comparing PPG signals at different blood glucose levels; 4) categorizing the valid samples into three glucose levels by applying machine learning algorithms. Our proposed system was evaluated on a data set of 80 subjects. Experimental results demonstrate that the system can separate valid signals from invalid ones at an accuracy of 97.54$%$ and the overall accuracy of estimating the blood glucose levels reaches 81.49$%$. The proposed system provides a reference for the introduction of noninvasive blood glucose technology into daily or clinical applications. This article also indicates that smartphone-based PPG signals have great potential to assess an individual's blood glucose level.

Abstract:Hemoglobin plays a major role for the existence of the healthy human being it carries oxygen supply to every cell for its survival in the entire body. As its level varies from person to person between 12-17 grams per deciliter if very high or low cause diseases. Usually low hemoglobin cause anemia in many of the women during pregnancy. In the present article the hemoglobin measurement is done noninvasively using the photoplethysmography method at the fingertip by Infrared LED with a wavelength of (880 – 940) nm and Red LED with wavelength of 660nm. The empirical calculation of Hemoglobin estimation is carried indirectly through model of oxygen saturation in skin-tissue- bone attenuation of the light on to the extremities. The analysis is supported by the Arduino IDE and MATLAB toolboxes for filtering the noise in signal and processing for target parameters. Further these real time parameters were sent to expertise to check for corrective ness and with the actual invasive methods for diagnosing its course of action. A Finger on the Pulse of Cardiovascular Health: Estimating Blood Pressure with Smartphone Photoplethysmography-Based Pulse Waveform Analysis

Abstract:Utilizing mobile phone cameras for continuous blood pressure (BP) monitoring presents a cost-effective and accessible approach, yet it is challenged by limitations in accuracy and interpretability. This study introduces four innovative strategies to enhance smartphone-based photoplethysmography for BP estimation (SPW-BP), addressing the interpretability-accuracy dilemma. First, we employ often-neglected data-quality improvement techniques, such as height normalization, corrupt data removal, and boundary signal reconstruction. Second, we conduct a comprehensive analysis of thirty waveform indicators across three categories to identify the most predictive features. Third, we use SHapley Additive exPlanations (SHAP) analysis to ensure the transparency and explainability of machine learning outcomes. Fourth, we utilize Bland-Altman analysis alongside AAMI and BHS standards for comparative evaluation. Data from 127 participants demonstrated a significant correlation between smartphone-captured waveform features and those from standard BP monitoring devices. Employing multiple linear regression within a cross-validation framework, waveform variables predicted systolic blood pressure (SBP) with a mean absolute error (MAE) of 3.08-16.64 mmHg and diastolic blood pressure (DBP) with an MAE of 2.86-13.16 mmHg. Further application of Random Forest models significantly improved the prediction MAE for SBP to 2.61-15.21 mmHg and for DBP to 2.14-11.22 mmHg, indicating enhanced predictive accuracy. Correlation and SHAP analysis identified key features for improving BP estimation. However, Bland-Altman analysis revealed systematic biases, and MAE analysis showed that the results did not meet AAMI and BHS accuracy standards. Our findings highlight the potential of SPW-BP, yet suggest that smartphone PPG technology is not yet a viable alternative to traditional medical devices for BP measurement.

Abstract:Background and objective: Hypertension is a potentially dangerous health condition that can be detected by measuring blood pressure (BP). Blood pressure monitoring and measurement are essential for preventing and treating cardiovascular diseases. Cuff-based devices, on the other hand, are uncomfortable and prevent continuous BP measurement. Methods: In this study, a new non-invasive and cuff-less method for estimating Systolic Blood Pressure (SBP), Mean Arterial Pressure (MAP), and Diastolic Blood Pressure (DBP) has been proposed using characteristic features of photoplethysmogram (PPG) signals and nonlinear regression algorithms. PPG signals were collected from 219 participants, which were then subjected to preprocessing and feature extraction steps. Analyzing PPG and its derivative signals, a total of 46 time, frequency, and time-frequency domain features were extracted. In addition, the age and gender of each subject were also included as features. Further, correlation-based feature selection (CFS) and Relief F feature selection (ReliefF) techniques were used to select the relevant features and reduce the possibility of over-fitting the models. Finally, support vector regression (SVR), K-nearest neighbour regression (KNR), decision tree regression (DTR), and random forest regression (RFR) were established to develop the BP estimation model. Regression models were trained and evaluated on all features as well as selected features. The best regression models for SBP, MAP, and DBP estimations were selected separately. Results: The SVR model, along with the ReliefF-based feature selection algorithm, outperforms other algorithms in estimating the SBP, MAP, and DBP with the mean absolute error of 2.49, 1.62 and 1.43 mmHg, respectively. The proposed method meets the Advancement of Medical Instrumentation standard for BP estimations. Based on the British Hypertension Society standard, the results also fall within Grade A for SBP, MAP, and DBP. Conclusion: The findings show that the method can be used to estimate blood pressure non-invasively, without using a cuff or calibration, and only by utilizing the PPG signal characteristic features.

Abstract:In the post-covid19 era, every new wave of the pandemic causes an increased concern among the masses to learn more about their state of well-being. Therefore, it is the need of the hour to come up with ubiquitous, low-cost, non-invasive tools for rapid and continuous monitoring of body vitals that reflect the status of one's overall health. In this backdrop, this work proposes a deep learning approach to turn a smartphone-the popular hand-held personal gadget-into a diagnostic tool to measure/monitor the three most important body vitals, i.e., pulse rate (PR), blood oxygen saturation level (aka SpO2), and respiratory rate (RR). Furthermore, we propose another method that could extract a single-lead electrocardiograph (ECG) of the subject. The proposed methods include the following core steps: subject records a small video of his/her fingertip by placing his/her finger on the rear camera of the smartphone, and the recorded video is pre-processed to extract the filtered and/or detrended video-photoplethysmography (vPPG) signal, which is then fed to custom-built convolutional neural networks (CNN), which eventually spit-out the vitals (PR, SpO2, and RR) as well as a single-lead ECG of the subject. To be precise, the contribution of this paper is two-fold: 1) estimation of the three body vitals (PR, SpO2, RR) from the vPPG data using custom-built CNNs, vision transformer, and most importantly by CLIP model; 2) a novel discrete cosine transform+feedforward neural network-based method that translates the recorded video- PPG signal to a single-lead ECG signal. Abstract:Photoplethysmography (PPG) is a non-invasive technology that measures changes in blood volume in the microvascular bed of tissue. It is commonly used in medical devices such as pulse oximeters and wrist worn heart rate monitors to monitor cardiovascular hemodynamics. PPG allows for the assessment of parameters (e.g., heart rate, pulse waveform, and peripheral perfusion) that can indicate conditions such as vasoconstriction or vasodilation, and provides information about microvascular blood flow, making it a valuable tool for monitoring cardiovascular health. However, PPG is subject to a number of sources of variations that can impact its accuracy and reliability, especially when using a wearable device for continuous monitoring, such as motion artifacts, skin pigmentation, and vasomotion. In this study, we extracted 27 statistical features from the PPG signal for training machine-learning models based on gradient boosting (XGBoost and CatBoost) and Random Forest (RF) algorithms to assess quality of PPG signals that were labeled as good or poor quality. We used the PPG time series from a publicly available dataset and evaluated the algorithm s performance using Sensitivity (Se), Positive Predicted Value (PPV), and F1-score (F1) metrics. Our model achieved Se, PPV, and F1-score of 94.4, 95.6, and 95.0 for XGBoost, 94.7, 95.9, and 95.3 for CatBoost, and 93.7, 91.3 and 92.5 for RF, respectively. Our findings are comparable to state-of-the-art reported in the literature but using a much simpler model, indicating that ML models are promising for developing remote, non-invasive, and continuous measurement devices.

Abstract:Efficient blood pressure (BP) monitoring in everyday contexts stands as a substantial public health challenge that has garnered considerable attention from both industry and academia. Commercial mobile phones have emerged as a promising tool for BP measurement, benefitting from their widespread popularity, portability, and ease of use. Most mobile phone-based systems leverage a combination of the built-in camera and LED to capture photoplethysmography (PPG) signals, which can be used to infer BP by analyzing the blood flow characteristics. However, due to low Signal-to-Noise (SNR), various factors such as finger motion, improper finger placement, skin tattoos, or fluctuations in environmental lighting can distort the PPG signal. These distortions consequentially affect the performance of BP estimation. In this paper, we introduce a novel sensing system that utilizes the built-in accelerometer of a mobile phone to capture seismocardiography (SCG) signals, enabling accurate BP measurement. Our system surpasses previous mobile phone-based BP measurement systems, offering advantages such as high SNR, ease of use, and power efficiency. We propose a triple-stage noise reduction scheme, integrating improved complete ensemble empirical mode decomposition with adaptive noise (ICEEMDAN), recursive least squares (RLS) adaptive filter, and soft-thresholding, to effectively reconstruct high-quality heartbeat waveforms from initially contaminated raw SCG signals. Moreover, we introduce a data augmentation technique encompassing normalization coupled with temporal-sliding, effectively augmenting the diversity of the training sample set. To enable battery efficiency on smartphone, we propose a resource-efficient deep learning model that incorporates resource-efficient convolution, shortcut connections, and Huber loss. We conduct extensive experiments with 70 volunteers, comprising 35 healthy individuals and 35 individuals diagnosed with hypertension, under a user-independent setting. The excellent performance of our system demonstrates its capacity for robust and accurate daily BP measurement.
