Abstract:Objective Non-invasive methods for hemoglobin (Hb) monitoring can provide additional and relatively precise information between invasive measurements of Hb to help doctors' decision-making. We aimed to develop a new method for Hb monitoring based on mask R-CNN and MobileNetV3 with eye images as input. Methods Surgical patients from our center were enrolled. After image acquisition and pre-processing, the eye images, the manually selected palpebral conjunctiva, and features extracted, respectively, from the two kinds of images were used as inputs. A combination of feature engineering and regression, solely MobileNetV3, and a combination of mask R-CNN and MobileNetV3 were applied for model development. The model's performance was evaluated using metrics such as R 2 , explained variance score (EVS), and mean absolute error (MAE). Results A total of 1,065 original images were analyzed. The model's performance based on the combination of mask R-CNN and MobileNetV3 using the eye images achieved an R 2 , EVS, and MAE of 0.503 (95% CI, 0.499–0.507), 0.518 (95% CI, 0.515–0.522) and 1.6 g/dL (95% CI, 1.6–1.6 g/dL), which was similar to that based on MobileNetV3 using the manually selected palpebral conjunctiva images (R 2 : 0.509, EVS:0.516, MAE:1.6 g/dL). Conclusion We developed a new and automatic method for Hb monitoring to help medical staffs' decision-making with high efficiency, especially in cases of disaster rescue, casualty transport, and so on.

Abstract:The development of intelligent blood coagulation diagnoses is awaited to meet the current need for large clinical time-sensitive caseloads due to its efficient and automated diagnoses. Herein, a method is reported and validated to realize it through artificial intelligence (AI)-assisted optical clotting biophysics (OCB) properties identification. The image differential calculation is used for precise acquisition of OCB properties with elimination of initial differences, and the strategy of space-time regulation allows on-demand space time OCB properties identification and enables diverse blood function diagnoses. The integrated applications of smartphones and cloud computing offer a user-friendly automated analysis for accurate and convenient diagnoses. The prospective assays of clinical cases (n = 41) show that the system realizes 97.6%, 95.1%, and 100% accuracy for coagulation factors, fibrinogen function, and comprehensive blood coagulation diagnoses, respectively. This method should enable more low-cost and convenient diagnoses and provide a path for potential diagnostic-markers finding.

Abstract:We suggest a low cost, non invasive healthcare system that measures haemoglobin levels in patients and can be used as a preliminary diagnostic test for anaemia. A combination of image processing, machine learning and deep learning techniques are employed to develop predictive models to measure haemoglobin levels. This is achieved through the color analysis of the fingernail beds, palpebral conjunctiva and tongue of the patients. This predictive model is then encapsulated in a healthcare application. This application expedites data collection and facilitates active learning of the model. It also incorporates personalized calibration of the model for each patient, assisting in the continual monitoring of the haemoglobin levels of the patient. Upon validating this framework using data, it can serve as a highly accurate preliminary diagnostic test for anaemia.

Deep Learning-Based Model for Non-invasive Hemoglobin Estimation via Body Parts Images: A Retrospective Analysis and a Prospective Emergency Department Study

Abstract:Anemia is a significant global health issue, affecting over a billion people worldwide, according to the World Health Organization. Generally, the gold standard for diagnosing anemia relies on laboratory measurements of hemoglobin. To meet the need in clinical practice, physicians often rely on visual examination of specific areas, such as conjunctiva, to assess pallor. However, this method is subjective and relies on the physician's experience. Therefore, we proposed a deep learning prediction model based on three input images from different body parts, namely, conjunctiva, palm, and fingernail. By incorporating additional body part labels and employing a fusion attention mechanism, the model learns and enhances the salient features of each body part during training, enabling it to produce reliable results. Additionally, we employ a dual loss function that allows the regression model to benefit from well-established classification methods, thereby achieving stable handling of minority samples. We used a retrospective data set (EYES-DEFY-ANEMIA) to develop this model called Body-Part-Anemia Network (BPANet). The BPANet showed excellent performance in detecting anemia, with accuracy of 0.849 and an F1-score of 0.828. Our multi-body-part model has been validated on a prospectively collected data set of 101 patients in National Taiwan University Hospital. The prediction accuracy as well as F1-score can achieve as high as 0.716 and 0.788, respectively. To sum up, we have developed and validated a novel non-invasive hemoglobin prediction model based on image input from multiple body parts, with the potential of real-time use at home and in clinical settings.

Abstract:Hemoglobin is the iron containing protein in red blood cells which carries oxygen from lungs to rest of the body tissues. Accurate measurement of hemoglobin is essential for diagnosing anemia, a condition characterized by a deficiency of red blood cells. This measurement is particularly vital before initiating blood transfusions for thalassemia patients. Non-invasive estimation of hemoglobin levels can be achieved through photoplethysmography (PPG)-based methods. PPG is an optical method to measure blood volume changes in successive heart beats. PPG signals can be obtained from fingertip videos using a light source and a photodetector. SmartphonePPG utilizes a smartphone's flashlight as a light source and its camera as a photodetector to acquire PPG signals, offering an affordable and portable point-of-care tool. Despite the ubiquity of smartphones, signals from their cameras often contain noise, making feature selection from PPG characteristics challenging. While PPG-based methods are invaluable, the lack of real-world datasets poses a significant challenge in maximizing the benefits of PPG technology. In this paper, we introduce a dataset comprising 1-minute fingertip video recordings from 150 anemic patients, obtained using a smartphone's camera. The dataset, publicly accessible for research purposes (https://forms.gle/LB4qn81ZMqEuy3V27), covers an age range of 6 months to 32 years, with diverse hemoglobin values (4.3 gm/dL - 12.4 gm/dL). Utilizing this dataset, we propose a deep learning-based technique employing the ResNet-18 architecture to estimate hemoglobin levels. This approach eliminates the need for manual feature extraction and selection from PPG signals, overcoming a limitation in existing smartphone PPG-based hemoglobin estimation systems. Our model achieves a hemoglobin level estimation with an RMSE of 0.81-1.39 when compared with the gold standard laboratory method, Complete Blood Count (CBC) test reports. In contrast, HemaApp, a state-of-the-art research utilizing a machine learning-based classifier (SVM), yields an RMSE of 1.7 on our dataset. The accuracy and simplicity of our model position it as a promising alternative to existing non-invasive hemoglobin level estimation methods.

Abstract:Background: Anemia is the most common hematological disorder. The purpose of this study was to establish and validate a deep-learning model to predict Hgb concentrations and screen anemia using ultra-wide-field (UWF) fundus images. Methods: The study was conducted at Peking Union Medical College Hospital. Optos color images taken between January 2017 and June 2021 were screened for building the dataset. ASModel_UWF using UWF images was developed. Mean absolute error (MAE) and area under the receiver operating characteristics curve (AUC) were used to evaluate its performance. Saliency maps were generated to make the visual explanation of the model. Results: ASModel_UWF acquired the MAE of the prediction task of 0.83 g/dl (95%CI: 0.81–0.85 g/dl) and the AUC of the screening task of 0.93 (95%CI: 0.92–0.95). Compared with other screening approaches, it achieved the best performance of AUC and sensitivity when the test dataset size was larger than 1000. The model tended to focus on the area around the optic disc, retinal vessels, and some regions located at the peripheral area of the retina, which were undetected by non-UWF imaging. Conclusion: The deep-learning model ASModel_UWF could both predict Hgb concentration and screen anemia in a non-invasive and accurate way with high efficiency.

Abstract:Background: According to the WHO, anemia is a highly prevalent disease, especially for patients in the emergency department. The pathophysiological mechanism by which anemia can affect facial characteristics, such as membrane pallor, has been proven to detect anemia with the help of deep learning technology. The quick prediction method for the patient in the emergency department is important to screen the anemic state and judge the necessity of blood transfusion treatment. Method: We trained a deep learning system to predict anemia using videos of 316 patients. All the videos were taken with the same portable pad in the ambient environment of the emergency department. The video extraction and face recognition methods were used to highlight the facial area for analysis. Accuracy and area under the curve were used to assess the performance of the machine learning system at the image level and the patient level. Results: Three tasks were applied for performance evaluation. The objective of Task 1 was to predict patients' anemic states [hemoglobin (Hb) <13 g/dl in men and Hb <12 g/dl in women]. The accuracy of the image level was 82.37%, the area under the curve (AUC) of the image level was 0.84, the accuracy of the patient level was 84.02%, the sensitivity of the patient level was 92.59%, and the specificity of the patient level was 69.23%. The objective of Task 2 was to predict mild anemia (Hb <9 g/dl). The accuracy of the image level was 68.37%, the AUC of the image level was 0.69, the accuracy of the patient level was 70.58%, the sensitivity was 73.52%, and the specificity was 67.64%. The aim of task 3 was to predict severe anemia (Hb <7 g/dl). The accuracy of the image level was 74.01%, the AUC of the image level was 0.82, the accuracy of the patient level was 68.42%, the sensitivity was 61.53%, and the specificity was 83.33%. Conclusion: The machine learning system could quickly and accurately predict the anemia of patients in the emergency department and aid in the treatment decision for urgent blood transfusion. It offers great clinical value and practical significance in expediting diagnosis, improving medical resource allocation, and providing appropriate treatment in the future.

Smartphone-Based Test and Predictive Models for Rapid, Non-Invasive, and Point-of-Care Monitoring of Ocular and Cardiovascular Complications Related to Diabetes

Abstract:Among the most impactful diabetic complications are diabetic retinopathy, the leading cause of blindness among working class adults, and cardiovascular disease, the leading cause of death worldwide. This study describes the development of improved machine learning based screening of these conditions. First, a random forest model was developed by retrospectively analyzing the influence of various risk factors (obtained quickly and non-invasively) on cardiovascular risk. Next, a deep-learning model was developed for prediction of diabetic retinopathy from retinal fundus images by a modified and re-trained InceptionV3 image classification model. The input was simplified by automatically segmenting the blood vessels in the retinal image. The technique of transfer learning enables the model to capitalize on existing infrastructure on the target device, meaning more versatile deployment, especially helpful in low-resource settings. The models were integrated into a smartphone-based device, combined with an inexpensive 3D-printed retinal imaging attachment. Accuracy scores, as well as the receiver operating characteristic curve, the learning curve, and other gauges, were promising. This test is much cheaper and faster, enabling continuous monitoring for two damaging complications of diabetes. It has the potential to replace the manual methods of diagnosing both diabetic retinopathy and cardiovascular risk, which are time consuming and costly processes only done by medical professionals away from the point of care, and to prevent irreversible blindness and heart-related complications through faster, cheaper, and safer monitoring of diabetic complications. As well, tracking of cardiovascular and ocular complications of diabetes can enable improved detection of other diabetic complications, leading to earlier and more efficient treatment on a global scale.

Real-time and accurate estimation of surgical hemoglobin loss using deep learning-based medical sponges image analysis

Abstract:Real-time and accurate estimation of surgical hemoglobin (Hb) loss is essential for fluid resuscitation management and evaluation of surgical techniques. In this study, we aimed to explore a novel surgical Hb loss estimation method using deep learning-based medical sponges image analysis. Whole blood samples of pre-measured Hb concentration were collected, and normal saline was added to simulate varying levels of Hb concentration. These blood samples were distributed across blank medical sponges to generate blood-soaked sponges. Eight hundred fifty-one blood-soaked sponges representing a wide range of blood dilutions were randomly divided 7:3 into a training group (n = 595) and a testing group (n = 256). A deep learning model based on the YOLOv5 network was used as the target region extraction and detection, and the three models (Feature extraction technology, ResNet-50, and SE-ResNet50) were trained to predict surgical Hb loss. Mean absolute error (MAE), mean absolute percentage error (MAPE), coefficient ( R 2 ) value, and the Bland–Altman analysis were calculated to evaluate the predictive performance in the testing group. The deep learning model based on SE-ResNet50 could predict surgical Hb loss with the best performance ( R 2 = 0.99, MAE = 11.09 mg, MAPE = 8.6%) compared with other predictive models, and Bland–Altman analysis also showed a bias of 1.343 mg with narrow limits of agreement (− 29.81 to 32.5 mg) between predictive and actual Hb loss. The interactive interface was also designed to display the real-time prediction of surgical Hb loss more intuitively. Thus, it is feasible for real-time estimation of surgical Hb loss using deep learning-based medical sponges image analysis, which was helpful for clinical decisions and technical evaluation.

A Smartphone-Based Biosensor for Non-Invasive Monitoring of Total Hemoglobin Concentration in Humans with High Accuracy

Abstract:In this paper, we propose a smartphone-based biosensor for detecting human total hemoglobin concentration in vivo with high accuracy. Compared to the existing biosensors used to measure hemoglobin concentration, the smartphone-based sensor utilizes the camera, memory, and computing power of the phone. Thus, the cost is largely reduced. Compared to existing smartphone-based sensors, we developed a highly integrated multi-wavelength LED module and a specially designed phone fixture to reduce spatial errors and motion artifacts, respectively. In addition, we embedded a new algorithm into our smartphone-based sensor to improve the measurement accuracy; an L*a*b* color space transformation and the “a” parameter were used to perform the final quantification. We collected 24 blood samples from normal and anemic populations. The adjusted R2 of the prediction results obtained from the multiple linear regression method reached 0.880, and the RMSE reached 9.04, which met the accuracy requirements of non-invasive detection of hemoglobin concentration.

Abstract:Despite its high prevalence, anemia is often undetected due to the invasiveness and cost of screening and diagnostic tests. Though some non-invasive approaches have been developed, they are less accurate than invasive methods, resulting in an unmet need for more accurate non-invasive methods. Here, we show that deep learning-based algorithms can detect anemia and quantify several related blood measurements using retinal fundus images both in isolation and in combination with basic metadata such as patient demographics. On a validation dataset of 11,388 patients from the UK Biobank, our algorithms achieved a mean absolute error of 0.63 g/dL (95% confidence interval (CI) 0.62-0.64) in quantifying hemoglobin concentration and an area under receiver operating characteristic curve (AUC) of 0.88 (95% CI 0.86-0.89) in detecting anemia. This work shows the potential of automated non-invasive anemia screening based on fundus images, particularly in diabetic patients, who may have regular retinal imaging and are at increased risk of further morbidity and mortality from anemia.

Abstract:This study constructed machine/deep learning models for predicting haemoglobin values using 150 palpebral conjunctival images taken by a smartphone. With conjunctiva images segmented by U‐net model, non‐convolutional neural network (CNN)‐based and CNN‐based regression models were developed for predicting haemoglobin values. The correlation coefficients between the actual and predicted haemoglobin values were 0.38 and 0.44 in the non‐CNN‐based and CNN‐based models, respectively. The gradient‐weighted class activation mapping heatmap indicated that the lower half area of the conjunctiva was crucial for haemoglobin value prediction. In conclusion, the CNN‐based model had better results than the non‐CNN‐based model. Summary Palpebral conjunctival hue alteration is used in non‐invasive screening for anaemia, whereas it is a qualitative measure. This study constructed machine/deep learning models for predicting haemoglobin values using 150 palpebral conjunctival images taken by a smartphone. The median haemoglobin value was 13.1 g/dL, including 10 patients with <11 g/dL. A segmentation model using U‐net was successfully constructed. The segmented images were subjected to non‐convolutional neural network (CNN)‐based and CNN‐based regression models for predicting haemoglobin values. The correlation coefficients between the actual and predicted haemoglobin values were 0.38 and 0.44 in the non‐CNN‐based and CNN‐based models, respectively. The sensitivity and specificity for anaemia detection were 13% and 98% for the non‐CNN‐based model and 20% and 99% for the CNN‐based model. The performance of the CNN‐based model did not improve with a mask layer guiding the model's attention towards the conjunctival regions, however, slightly improved with correction by the aspect ratio and exposure time of input images. The gradient‐weighted class activation mapping heatmap indicated that the lower half area of the conjunctiva was crucial for haemoglobin value prediction. In conclusion, the CNN‐based model had better results than the non‐CNN‐based model. The prediction accuracy would improve by using more input data with anaemia.

Abstract:A low-cost, robust, and simple mechanism to measure hemoglobin would play a critical role in the modern health infrastructure. Consistent sample acquisition has been a long-standing technical hurdle for photometer-based portable hemoglobin detectors which rely on micro cuvettes and dry chemistry. Any particulates (e.g. intact red blood cells (RBCs), microbubbles, etc.) in a cuvette's sensing area drastically impact optical absorption profile, and commercial hemoglobinometers lack the ability to automatically detect faulty samples. We present the ground-up development of a portable, low-cost and open platform with equivalent accuracy to medical-grade devices, with the addition of CNN-based image processing for rapid sample viability prechecks. The developed platform has demonstrated precision to the nearest $0.18[g/dL]$ of hemoglobin, an R^2 = 0.945 correlation to hemoglobin absorption curves reported in literature, and a 97% detection accuracy of poorly-prepared samples. We see the developed hemoglobin device/ML platform having massive implications in rural medicine, and consider it an excellent springboard for robust deep learning optical spectroscopy: a currently untapped source of data for detection of countless analytes.

Abstract:Quantifying hemoglobin is vital yet invasive through blood draws. We developed a wearable diffuse reflectance spectroscopy device comprising control and sensor boards with photodiodes and light-emitting diodes to noninvasively determine hemoglobin. Neural networks enabled recovery of optical parameters for chromophore fitting to calculate hemoglobin. Testing healthy and elderly subjects revealed strong correlation (r=0.9) between our system and invasive methods after data conversion. Bland-Altman analysis demonstrated tight 95% limits of agreement from −1.98 to 1.98 g/dL between the DRS and invasive hemoglobin concentrations. By spectroscopically isolating hemoglobin absorption, interference from melanin was overcome. Our device has the potential for future integration into wearable technology, enabling hemoglobin level tracking.

Abstract:Over the last decade, smartphones have changed radically to support us with mHealth technology, cloud computing, and machine learning algorithm. Having its multifaceted facilities, we present a novel smartphone-based noninvasive hemoglobin (Hb) level prediction model by analyzing hue, saturation and value (HSV) of a fingertip video. Here, we collect 60 videos of 60 subjects from two different locations: Blood Center of Wisconsin, USA and AmaderGram, Bangladesh. We extract red, green, and blue (RGB) pixel intensities of selected images of those videos captured by the smartphone camera with flash on. Then we convert RGB values of selected video frames of a fingertip video into HSV color space and we generate histogram values of these HSV pixel intensities. We average these histogram values of a fingertip video and consider as an observation against the gold standard Hb concentration. We generate two input feature matrices based on observation of two different data sets. Partial Least Squares (PLS) algorithm is applied on the input feature matrix. We observe R2=0.95 in both data sets through our research. We analyze our data using Python OpenCV, Matlab, and R statistics tool.

Abstract:<p>Hematologists evaluate alterations in blood cell enumeration and morphology to confirm peripheral blood smear findings through manual microscopic examination. However, routine peripheral blood smear analysis is both time-consuming and labor-intensive. Here, we propose using smartphone-based autofluorescence microscopy (Smart-AM) for imaging label-free blood smears at subcellular resolution with automatic hematological analysis. Smart-AM enables rapid and label-free visualization of morphological features of normal and abnormal blood cells (including leukocytes, erythrocytes, and thrombocytes). Moreover, assisted with deep-learning algorithms, this technique can automatically detect and classify different leukocytes with high accuracy, and transform the autofluorescence images into virtual Giemsa-stained images which show clear cellular features. The proposed technique is portable, cost-effective, and user-friendly, making it significant for broad point-of-care applications.</p>

Abstract:Blood glucose (BG) prediction is essential to the success of glycemic control in type 1 diabetes (T1D) management. Empowered by the recent development of the Internet of Medical Things (IoMT), continuous glucose monitoring (CGM) and deep learning technologies have been demonstrated to achieve the state of the art in BG prediction. However, it is challenging to implement such algorithms in actual clinical settings to provide persistent decision support due to the high demand for computational resources, while smartphone-based implementations are limited by short battery life and require users to carry the device. In this work, we propose a new deep learning model using an attention-based evidential recurrent neural network and design an IoMT-enabled wearable device to implement the embedded model, which comprises a low-cost and low-power system on a chip to perform Bluetooth connectivity and edge computing for real-time BG prediction and predictive hypoglycemia detection. In addition, we developed a smartphone app to visualize BG trajectories and predictions, and desktop and cloud platforms to backup data and fine-tune models. The embedded model was evaluated on three clinical data sets including 47 T1D subjects. The proposed model achieved superior performance of root mean square error (RMSE), mean absolute error, and glucose-specific RMSE, and obtained the best accuracy for hypoglycemia detection when compared with a group of machine learning baseline methods. Moreover, we performed hardware-in-the-loop in silico trials with ten virtual T1D adults to test the whole IoMT system with predictive low-glucose management, which significantly reduced hypoglycemia and improved BG control.

Abstract:BACKGROUND: Dynamic and precise estimation of blood loss (EBL) is quite important for perioperative management. To date, the Triton System, based on feature extraction technology (FET), has been applied to estimate intra-operative haemoglobin (Hb) loss but is unable to directly assess the amount of blood loss. We aimed to develop a method for the dynamic and precise EBL and estimate Hb loss (EHL) based on artificial intelligence (AI).METHODS: We collected surgical patients' non-recycled blood to generate blood-soaked sponges at a set gradient of volume. After image acquisition and preprocessing, FET and densely connected convolutional networks (DenseNet) were applied for EBL and EHL. The accuracy was evaluated using R2, the mean absolute error (MAE), the mean square error (MSE), and the Bland-Altman analysis.RESULTS: For EBL, the R2, MAE and MSE for the method based on DenseNet were 0.966 (95% CI: 0.962-0.971), 0.186 (95% CI: 0.167-0.207) and 0.096 (95% CI: 0.084-0.109), respectively. For EHL, the R2, MAE and MSE for the method based on DenseNet were 0.941 (95% CI: 0.934-0.948), 0.325 (95% CI: 0.293-0.355) and 0.284 (95% CI: 0.251-0.317), respectively. The accuracies of EBL and EHL based on DenseNet were more satisfactory than that of FET. Bland-Altman analysis revealed a bias of 0.02 ml with narrow limits of agreement (LOA) (-0.47 to 0.52 mL) and of 0.05 g with narrow LOA (-0.87 to 0.97 g) between the methods based on DenseNet and actual blood loss and Hb loss.CONCLUSIONS: We developed a simpler and more accurate AI-based method for EBL and EHL, which may be more fit for surgeries primarily using sponges and with a small to medium amount of blood loss.

HgbNet: predicting hemoglobin level/anemia degree from EHR data

Abstract:Anemia is a prevalent medical condition that typically requires invasive blood tests for diagnosis and monitoring. Electronic health records (EHRs) have emerged as valuable data sources for numerous medical studies. EHR-based hemoglobin level/anemia degree prediction is non-invasive and rapid but still faces some challenges due to the fact that EHR data is typically an irregular multivariate time series containing a significant number of missing values and irregular time intervals. To address these issues, we introduce HgbNet, a machine learning-based prediction model that emulates clinicians' decision-making processes for hemoglobin level/anemia degree prediction. The model incorporates a NanDense layer with a missing indicator to handle missing values and employs attention mechanisms to account for both local irregularity and global irregularity. We evaluate the proposed method using two real-world datasets across two use cases. In our first use case, we predict hemoglobin level/anemia degree at moment T+1 by utilizing records from moments prior to T+1. In our second use case, we integrate all historical records with additional selected test results at moment T+1 to predict hemoglobin level/anemia degree at the same moment, T+1. HgbNet outperforms the best baseline results across all datasets and use cases. These findings demonstrate the feasibility of estimating hemoglobin levels and anemia degree from EHR data, positioning HgbNet as an effective non-invasive anemia diagnosis solution that could potentially enhance the quality of life for millions of affected individuals worldwide. To our knowledge, HgbNet is the first machine learning model leveraging EHR data for hemoglobin level/anemia degree prediction.
