Abstract:The aim of our study is to evaluate computed tomography (CT) coronary angiography in patients with a high heart rate using 16-slice spiral CT with 0.37-s gantry rotation time. We compare the image quality of patients whose heart rates were over 70 beats per minute (bpm) with that of patients whose heart rates were 70 bpm or less. Sixty patients with various heart rates underwent retrospectively ECG-gated multislice spiral CT (MSCT) coronary angiography. Two experienced observers who were blind to the heart rates of the patients evaluated all the MSCT coronary angiographic images and calculated the assessable segments. A total of 620 out of 891 (69.6%) segments were satisfactorily visualized. On average, 10.3 coronary artery segments per patient could be evaluated. In 36 patients whose heart rates were below 70 bpm [mean 62.2 bpm+/-5.32 (standard deviation, SD)], the number of assessable segments was 10.72+/-2.02 (SD). In the other 24 patients whose heart rates were above 70 bpm [mean 78.6 bpm+/-8.24 (SD)], the corresponding number was 9.75+/-1.74 (SD). No statistically significant difference was found in these two subgroups' t test, P>0.05. The new generation of 16-slice spiral CT with 0.37-s rotation time can satisfactorily evaluate the coronary arteries of patients with high heart rates (above 70 bpm, up to 102 bpm).
Coronary Angiography by 128 Multi-Slice Dual Source CT with Multiple Scan Modes

Abstract:Objective: To evaluate the application of multiple scan modes in 128-slice dual-source CT coronary angiography.Methods: 603 patients of chest pain and chest tightness with heart rate ≤65bpm were examined by CT coronary angiography.186 patients were examined using the prospective ECG-gated flash spiral(FS) mode;102 patients were examined using the prospectively ECG-gated sequence(S) mode;315 patients were examined using the retrospective ECG-gated low-pith spiral(LS) mode,in which 45 cases had arrhythmia.The radiation dose of three modes was used for analysis of variance.The radiation dose was compared between the group of normal cardiac rhythm and the group of arrhythmia.Results: The percentage of diagnosable segments was similar with 99.4% for FS mode,99.5% for S mode and 99.2% for LS mode,but there was significant difference among three modes in radiation dose with(0.91±0.05) mSv for FS mode and(5.48±0.95) mSv for S mode and(9.19±3.00) mSv for LS mode(P0.01).There were 270 patients of normal cardiac rhythm and 45 patients of arrhythmia in LS mode scanning,and there was significant difference in radiation dose with(8.45±2.44) mSv for the group of normal cardiac rhythm and(13.62±2.09) mSv for the group of arrhythmia(t=-13.428,P0.01).Conclusion: The three modes(FS,S and LS) of 128-slice dual-source CT coronary angiography can obtain good image quality,but the radiation dose of FS mode were further significantly lower compared with S mode and LS mode.FS mode is an effective means to achieve low radiation dose for CT coronary angiography.Different scanning modes should be used according to the heart rate and rhythm of the patients to enable scan optimization.

Abstract:Objective To study the application of multidector spiral CT(MSCT) in the diagnosis of pulmonary embolism(PE).Methods The clinical and MSCTPA data of 15 cases with suspected PE were analyzed retrospectively.Results Of 15 cases,14 cases(93.3%)were diagnosed as PE.849 branches(82%) of pulmonary arteries were showed in 1035 branches.PE was detected in 130 branches(15.3%) of 849 branches of pulmonary arteries.438 branches(73%) were showed in 600 branches of subsegmental pulmonary arteries,PE was showed in 43 branches(9.8%)of 438 branches.Direct signs of PE included central filling defect in 3 cases;eccentric filling defect(n=9);embolism attached to the wall of host artery(n=2) and total occlusion of the pulmonary arteries(n=3).Indirect signs includedmosaic sign in 1 case;subpleural infarction(n=3);pulmonary hypertension(n=5);Westermark sign(n=8) and pleural effusion(n=6).Conclusion MSCTPA is a fast,effective,security and non-invasive diagnostic method for PE,it may replace the pulmonaryangiography and will be the first choice for diagnosis of PE.

Abstract:Use of CT coronary angiography (CTCA) to evaluate chest pain has rapidly increased over the recent years. While its utility in the diagnosis of coronary artery disease in stable chest pain syndromes is clear and is strongly endorsed by international guidelines, the role of CTCA in the acute setting is less certain. In the low-risk setting, CTCA has been shown to be accurate, safe and efficient but inherent low rates of adverse events in this population and the advent of high-sensitivity troponin testing have left little room for CTCA to show any short-term clinical benefit. In higher-risk populations, CTCA has potential to fulfil a gatekeeper role to invasive angiography. The high negative predictive value of CTCA is maintained while also identifying non-obstructive coronary disease and alternative diagnoses in the substantial group of patients presenting with chest pain who do not have type 1 myocardial infarction. For those with obstructive coronary disease, CTCA provides accurate assessment of stenosis severity, characterisation of high-risk plaque and findings associated with perivascular inflammation. This may allow more appropriate selection of patients to proceed to invasive management with no disadvantage in outcomes and can provide a more comprehensive risk stratification to guide both acute and long-term management than routine invasive angiography.

Diagnostic Value of 16-Slice Spiral Computed Tomography for Detecting Coronary Artery Disease

Abstract:Objective:To evaluate the accuracy and the limit of 16-slice spiral CT in diagnosis of coronary artery disease. Methods: 20 patients were studied by 16-slice spiral CT (retrospectively ECG-gating; 0. 42s rotation). The results were compared with coronary artery angiography. Results: In the 136 coronary artery segments (vessel diameter≥2mm)of 20 patients,122 were judged to be evaluable by CT(89. 7%). The sensitivity and the specificity for the detection of stenosis≥50% were 87. 5 % and 96. 6% respectively. When 14 coronary artery segments were judged to be unevaluable were included in the analysis, the sensitivity was 77. 7%. Conclusions: 16-slice spiral CT could be used in noninvasive assessment of significant coronary artery stenosis with high accuracy. The limit was inadequate temporal resolution and unavoidable artifacts.

Abstract:Objecfive To explore clinical value of 64-slice spiral CT coronary angiography in diagnosing coronary atherosclerotic heart disease. as compared to that with selective coronary angiography.Methods One hundred and thirteen patients who underwent both 64-slice spiral CT coronary angiography as well as selective coronary angiography at an interval of no more than one month at Zhongshan Hospital.Shanghm in 2006 were selected for the study and their imaging reports were analyzed and compared.Results Images of 910 segments of coronary arteries were collected and assessed.Sensitivity,specificity and likelihood ratio for negative test result of 64-slice spiral CT coronary angiography in diagnosis of coronary atherosclerotic heart disease were 73.8 percent.97.0 percent and 24.4,respectively,with an overall agreement of 93.2 percent,positive predictive value of 82.7 percent and negative predictive value of 95.0 percenL Conclusions Sixty-four-slice spiral CT coronary angiography has hish specificity,negative predictive value and positive likelihood ratio,with high accuracy,in diagnosing coronary atherosclemtic heart disease.

Abstract:Objective Clinicians have difficulty in identifying the reasons of chest pain because of the variety.This study aimed at evaluating the value of dual source computed tomography angiography(DSCTA) in finding the causes of chest pain.Methods We retrospectively analyzed the data of patients with chest pain undergoing DSCTA from May 2009 to November 2011,and recorded the prevalence of coronary,aortic and/or pulmonary artery diseases.Results A total of 418 patients were included,292 patients underwent coronary artery DSCTA(CTCA),89 aortic DSCTA(CTA) and 15 pulmonary artery CTA(CTPA).And 2 patients underwent CTCA as well as CTPA,17 underwent CTCA and CTA,while 3 underwent CTPA and CTA.We found that 209(209/292) patients had coronary artery diseases,while 47(47/89) and 5(5/15) patients had aortic and pulmonary artery diseases(pulmonary embolism),respectively.The other 162 patients had no coronary,aortic or pulmonary artery diseases.Conclusion DSCTA plays a key role in differentiating the causes of chest pain,and acute coronary syndrome is the most common cause of chest pain,followed by aortic diseases and pulmonary embolism.

Abstract:Objective To evaluate the value of clinical application of 16-slice spiral CT pulmonary angiography(16-sSCTPA).Methods Forty-six patients with lung diseases underwent spiral CT pulmonary angiography using GE LightspeedTM 16 CT scanner with 0.5 s/360° velocity,0.625 mm slice thickness pitch 1.375,1.25 mm reconstruction interval.The contrast material injection rate was 3.5～4 ml/s.The control group included 25 normal subjects.Other patients included 16 central pulmonary carcinoma with signs of central pulmonary artery or pulmonary arteries infiltration,4 pulmonary embolism,1 Takayasu's arteritis with pulmonary arterial stenoses.Eight cases were proved by pathology.Results()(16-sSCTPA) showed central pulmonary artery,right or left pulmonary artery,segmental and subsegmental pulmonary arteries clearly of all 25 cases of the normal pulmonary artery.16-sSCTPA demonstrated the relationship between pulmonary arteries and masses in 16 cases,malformation,displacement and narrowing of pulmonary arteries in 17 cases,as well as pulmonary embolism in 4 cases.Conclusion 16-sSCTPA has an important value in assessing diseases of pulmonary arteries.It can be partly in place of the conventional angiography and emission CT.

Abstract:Objective To evaluate the clinical value of 16-slices spiral CT coronary angiography in the detection of coronary artery stenosis.Methods 46 patients were detected with ECG-gated MSCT.And all patients were also investigated with SCA.Results of the determination of≥50% stenoses of the vessel diameter with MSCT were compared to findings of SCA.Results The sensitivity,specificity,accuracy,positive and negative prediction value of MSCTCA were 82.22%、95.68%、92.39%、86.05% and 94.33%,respectively.Conclusion 16-slice spiral CT coronary angiography is a noninvasive,simple and safe method.16-slice spiral CT coronary angiography has a good clinical value in the diagnosing of ≥50% stenoses of the vessel diameter.

Abstract:Objective To investigate the optimal R-R phase of retrospectively electrocardiographically(ECG)-gated image reconstruction for multi-slice spiral CT (MSCT) coronary reconstruction and it's clinical application. Methods A total of 20 patients with known or suspected coronary artery disease (CAD) were detected with retrospectively ECG-gated MSCT (4×1.3 mm cross-section,500-ms rotation,pitch 0.375) during a single breath hold. Tomographic source images were reconstructed at the starting points of 37.5%,50.0%,62.5%,and 75.0% R-R intervals,followed by volume rendering (VR) and virtual vascular endoscopy (VVE) for reconstruction of coronary artery. The quality of images reconstructed with different R-R phases was analyzed in terms of artifacts and visibility. Selective coronary angiography (SCA) was used for reference. Results The left anterior descending artery,the left circumflex artery,and the right coronary artery were all best visualized at 50.0% and 62.5% R-R phases. The sensitivity,specificity,positive predictive value,negative predictive value,and accuracy for the detection of ≥50% stenosis with the optimal R-R phase imagines of MSCT were 91.7%,83.3%,28.2%,99.3%,and 83.9%,respectively. Conclusion The optimal R-R phase of retrospectively ECG-gated reconstruction images can decrease the artifacts of cardiac motion and improve the reconstructive image quality of coronary arteries. With high sensitivity and negative predictive value,MSCT coronary reconstruction is considered as a good screening test before SCA.
Clinical significance of coronary angiography with multi-slice spiral CT

Abstract:Objective:The aim of this investigation was to evaluate the clinical significance of coronary angiography with multi-slice spiral CT(MSCTA) in coronary disease.Method:Plain and enhanced sixteen-slice spiral CT was performed in 108 male elderly patients. Coronary artery calcium scores and stenosis were demonstrated. All patients also underwent rest and exercise single photon emission computerized tomography(SPECT) and selective coronary angiography(SCA). The findings demonstrated on MSCTA and SPECT were compared and analyzed statistically with those seen on SCA which was considered as the gold standard.Result:The sensitivity, specificity, positive predictive value, negative predictive value and accuracy for detecting coronary heart disease by MSCTA were 77%,72%, 88%, 54% and 76%,respectively. The sensitivity by MSCTA for detection of 1-, 2- and 3-vessel coronary heart disease was 61%, 90%, and 99%, respectively. MSCTA combined with SPECT, the sensitivity, specificity, positive predictive value and negative predictive value for detecting coronary heart disease increased up to 99%, 93%, 96%, and 94%, respectively. Coronary artery calcium scores were increasing with more risk factors and severity of coronary arteries. The calcium score≥200 in ≥65 years male patients might have much more clinical significance.Conclusion :Detection of coronary calcium scores and stenoses by MSCTA is a valuable method in assessment of coronary heart disease. Combined with SPECT, the reliability will be greatly increased, which can be used as non-invasive screening methods for coronary disease before performing invasive coronary angiography.

Abstract:Objective: To analyze the imaging effect and accuracy of 64-slice spiral CT in the diagnosis of coronary arterystenosis. Methods: 30 patients with suspected coronary heart disease admitted to our hospital from March 2022 to March 2023 were selected. All patients underwent both 64-slice spiral CT and digital subtraction angiography (DSA). DSA is considered the gold standard for diagnosis, so it was used to analyze the diagnostic performance of 64-slice spiral CT. Results: (1) The diagnostic results of digital subtraction angiography and 64-slice spiral CT was analyzed and compared with each other. The 64-slice spiral CT had an accuracy of 96.67% (29/30), a sensitivity 96.55% (28/29), and a specificity of 100.00% (1/1) in diagnosing coronary artery stenosis. (2) There was no significant difference between 64-slice spiral CT and digital subtraction angiography in the positive detection rate of anterior descending artery lesion, the positive detection rate of left main lesion, the positive detection rate of left circumflex artery lesion and the positive detection rate of right coronary artery lesion (P > 0.05). (3) There was no significant difference between 64-slice spiral CT examination and DSA examination in identifying mild stenosis, moderate stenosis, and severe stenosis of coronary arteries (P > 0.05). Conclusion: 64-slice spiral CT examination can accurately determine the degree of occlusion of coronary arteries, which allows for the accurate diagnosis of coronary artery stenosis.
Evaluation of coronary bypass grafts with multislice spiral CT angiography

Abstract:Objective To evaluate the value of multislice CT angiography (CTA) for assessment of coronary artery bypass graft (CABG). Methods A total of 28 patients with 69 coronary artery bypass grafts were examined by CTA with 16-slice and 64-slice CT, in which 26 coronary artery bypass grafts of 10 patients coronary artery bypass grafts were examined by coronary angiography as well. Using multiple linear regression to evaluate the effect of patient's heart rate, fluctuation of heart rate, breathing movement and different CT scanners on image quality of CT coronary angiogram. Results The patient's breathing movement was negatively related to the image quality, the value of standardized coefficients was -0.838. The image quality of 64-slice CT was higher than that of 16-slice CT. Taken the CAG as reference standard, the sensitivity and specificity of CT in evaluating the anastomosis and bypass grafts were 95.7% and 92.3% respectively. Conclusion MSCT can provide high image quality of CABG, which has great potential value in the follow-up of this disease.

Transverse scanning showed the highest sensitivity and specificity for detection of stenosis≥50%(81.2% and 94.4%), followed by virtual endoscopy(71.8% and 84.4%), maximum intensity projection(68.7% and 84.4%), curved multiplanar reformations(65.6% and 87.7%), volume rendering(65.6% and 74.4%). Combined with all techniques, the sensitivity and specificity for the detection of stenosis≥50% were 87.5% and 96.6%, respectively. Conclusion: Transverse scanning showed the highest sensitivity and specificity for the detection of stenosis≥50%. 16-slice spiral CT could be used as a noninvasive method for the assessment of significant coronary artery stenosis with high accuracy.
Three-dimensional reconstruction of 64-slice spiral CT angiography in pulmonary embolism

Abstract:Objective To evaluate the clinical value of three-dimensional imaging of pulmonary vessels with 64-slice spiral CT pulmonary angiography (64-MSCTPA) in pulmonary embolism (PE). Methods 64-MSCTPA was performed in 19 clinical suspected patients with PE and 4 with coronary heart disease. All the primitive images of pulmonary vessels and coronary artery were produced at the console workstation. The distribution of pulmonary vessels emboli was evaluated from different angle. Of 23 patients, PE were confirmed in 5 cases by DSA and compared with the findings of 64-MSCTPA. Results Spatial relationship and anatomical details of pulmonary arteries and veins could exactly be shown on all pulmonary vessels three-dimensional images in above-mentioned patients. On the basis of 64-MSCTPA, PE could be diagnosed definitely,in addition, collateral circulation formation could be clearly demonstrated. Of 23 patients, PE were found in 16 cases of PE were found by 64-MSCTPA. Two hundrends and sixty-four emboli were demonstrated in total, among which there are 149 emboli located in segmental ateries and 115 in subsegmental ateries (grade 5-6 branches).The minimal diameter of pulmonary atery with thrombus was 1 mm. Besides, apparent collateral circulation was displayed in 1 case between upper limb and lateral chest wall because of thrombus of subclavian and axillary vein, excentric filling defects of lumina were satisfactorily displayed in 1 case which located between the debouchement of left superior pulmonary vein and left atrium. There are distinguishedly statistical significance on the detection rate of emboli located in segmental atery and subsegmental atery between DSA and 64-MSCTPA. Conclusion 64-MSCTA has very high clinical value in detecting segmental and subsegmental pulmonary thrombi in PE patients, and provides useful information for the diagnosis and treatment of pulmonary vascular diseases.

Abstract:Acute chest pain (ACP) is one of the most common presenting symptoms at the emergency department. The differential diagnosis is vast. To exclude life-threatening causes, radiologists encounter an increasing amount of thoracic computed tomography (CT) examinations including CT angiography of the heart and great vessels. The dual- and triple-rule CT examinations are currently implemented in clinical practice. We retrospectively identified chest CT examinations in the setting of acute chest pain in our hospitals and collected a variety of common and uncommon cases. In this pictorial essay, we present the most educative cases from patients who presented with acute chest pain in the emergency department of our hospitals and for whom a thoracic CT was ordered. Abstract:Background Computed tomography (CT) for excluding acute aortic syndrome (AAS) and pulmonary embolism (PE) simultaneously in patients with chest pain could be used to exclude coronary artery disease (CAD). Purpose To evaluate the frequency of further testing for CAD in patients receiving a CT in the emergency department (ED) for simultaneous evaluation for AAS and PE. Material and Methods This retrospective study was conducted over a three-year period including all patients with acute chest pain visiting our ED. All patients were included that received an electrocardiography (ECG)-gated CT of the entire chest enquiring simultaneously for AAS and PE. Those patients were followed up for 30 days after their initial ED visit whether they received further testing for CAD. Results Within the study period, a total of 157 patients with acute chest pain received a chest pain CT for simultaneous evaluation of both AAS and PE. Image quality was deemed sufficient to evaluate the coronary arteries in 80% of the patients. Thirty-seven patients (24%) underwent additional testing for CAD within 30 days of their ED visit, including catheter coronary angiography (n = 25), cardiac-stress single-photon emission-CT (n = 6), and cardiac magnetic resonance imaging (MRI) (n = 6). Conclusion Of patients presenting to the ED with acute chest pain who received a chest pain CT for simultaneous evaluation of AAS and PE, 24% had further imaging for CAD within 30 days of the initial ED visit. Immediate evaluation of the coronary arteries as part of a chest pain CT should be considered here for not delaying diagnosis.

Abstract:OBJECTIVE:To evaluate the utility of multi-slice spiral CT in the diagnosis of pulmonary hypertension.METHODS:The clinical and multi-slice spiral CT data of 22 patients with pulmonary hypertension were retrospectively analyzed. The diagnoses included chronic thromboembolic pulmonary hypertension (CTEPH) (n = 9), primary pulmonary hypertension (PPH) (n = 7), connective tissue diseases (n = 2), portal-pulmonary hypertension (n = 2), eosinophilia-myalgia syndrome (n = 1), and fibrosing mediastinitis (n = 1). Lung parenchyma, such as ground-glass opacity, mosaic attenuation and septal thickness, mediastinum, pulmonary vascular, right ventricular enlargement and interventricular septum displacement were evaluated on CT.RESULTS:CT pulmonary angiography (CTPA) accurately diagnosed 95% of pulmonary hypertension and the signs of right ventricular dysfunction. Pulmonary embolism was found in 9 patients. The incidence of mosaic attenuation was significantly higher in CTECH than in PPH (P = 0.034).CONCLUSION:CTPA can accurately diagnose pulmonary hypertension and thereby provide useful information for the etiological diagnosis.
