Abstract:Objective To evaluate the efficiency of transcatheter closure of secundum atrial septal defect (ASD) with Amplatzer occluder device.Methods Twenty-one patients with ASD was treated using Amplatzer occluder device through the percutaneous procedure under fluoroscopy and transesophageal echocardiography (TEE) guidance.Results The mean diameter of ASD was 20.5 ± 7.1mm (ranged from 6 to 32mm)shown by TEE before the procedure,The mean balloon stretched diameter of the ASD was 23.3±7.7mm (ranging from 9 to 34 mm).The mean diameter of the Amplatzer occluder selected was 25.9±8.1mm (ranging from 10 to 38 mm).For two cases with double ASD,Two occluder device of 22 and 10 mm in diameter were used in one patient,a larger occluder device of 28 mm in diameter was used for closing two defects in the other patient with good result.All patients were treat successfully.The success rate of placement of the Amplatzer occluder was 100%.No complications occurred.All of 21 patients was no residual shunt found during 1～18 months follow-up.Conclusion Transcatheter closure of secundum ASD using Amplatzer septal occluder device was an efficient and safe nonsurgical method.
Non-fluoroscopic percutaneous transcatheter closure of atrial septal defects in children under transesophageal echocardiographic guidance

Abstract:Background This study sought to investigate the feasibility, safety and effectiveness of transcatheter closure of atrial septal defects (ASDs) under the guidance of transesophageal echocardiography (TEE) in children. Methods We reviewed the medical records of patients who underwent percutaneous ASD closure at our center from August 2016 to December 2017. For a total of 88 patients who were identified as having a single-hole defect and were undergoing percutaneous transcatheter ASD closure, a procedure completely guided by TEE was performed. There were 31 male patients and 57 female patients. The patients’ mean age was 60.09 ± 36.42 months (13–182 months), and their mean body weight was 20.16 ± 10.04 kg (9–77 kg). Patients were followed up by performing transthoracic echocardiography and obtaining chest X-rays and electrocardiograms. Results The transcatheter closure of ASDs was successful in all patients. The mean ASD size was 11.58 ± 5.31 mm (3–28 mm), and the mean size of the occlusion device was 16.07 ± 5.29 mm (6–36 mm). The mean procedural times were 13.33 ± 2.82 minutes (6–16 minutes). The mean hospitalization costs were 27,259.66 ± 2507.04 RMB (25,200.00–33,911.45 RMB). The mean postoperative hospital stay was 3.22 ± 0.53 days (3–5 days). Residual shunt, occlusion device shedding or displacement, and pericardial effusion were not observed during or after the operation. Conclusion Percutaneous transcatheter ASD closure completely guided by TEE is a feasible, safe, non-invasive and easy procedure.

Abstract:Objective To apply a surgical approach of minimal invasive closure to atrial septal defect(ASD)without cardiopulmonary bypass.Methods Between Sept.2004 and Apr.2007,46 cases(male 22,female 24)of atrial septal defeet(ASD)were treated by minimal invasive closure without cardiopulmonary bypass.Their age ranged from 8mon tO 16yr(mean 4.7±3.9 yr)and weight ranged from 8.5 to 35 kg.(mean 1 5.2±6.5 kg).The incision was through the right 4th intercostals space after general anesthesia,and the character and size of ASD were confirmed via the transesophageal eehocardiography(TEE).The occluder device suitable to the ASD was placed through the right atrium and released after making sure it's firmly in place of the defect.Results Among them.45 cases of ostium secundum ASD were closed successfully and 1 failure.The size of ASD was from 8 mm to 25 mm,and the shortest distance of the rim of ASD was 7mm to the mitral valve,8 mm to the top of left atrium,6 mm to superior vena cava and 6 mm to inferior vena cava.There were no residual shunts and operative complications.Conclusions The advantages of minimally invasive closure of ASD without cardiopulmonary bypass are less traumas,no radiation,easy and security of manipulation.It is valuable for clinical application.
Transesophageal echocardiography and fluoroscopy for percutaneous closure of atrial septal defects: A comparative study.

Abstract:The aim of the study was to compare transesophageal echocardiography (TEE) and fluoroscopy for percutaneous atrial septal defect (ASD) closure. This was a retrospective analysis of children who underwent percutaneous ASD closure. The procedure was guided by TEE without fluoroscopy in 130 patients (TEE group) and by fluoroscopy in 163 patients (fluoroscopy group). Baseline demographic/ clinical characteristics were recorded. Patients were followed until hospital discharge. Outcomes were procedure duration, peri/ postoperative complications, hospital stay, and costs. The TEE and fluoroscopy groups showed no significant differences in age (71.7 +/- 40.7 vs 62.5 +/- 38.8 months), male/female ratio (54/76 vs 66/97), weight (22.0 +/- 12.0 vs 20.1 +/- 9.0 kg), ASD diameter (9.9 +/- 4.2 vs 9.3 +/- 3.9 cm), distances to the superior vena cava (13.4 +/- 4.6 vs 13.3 +/- 4.2 cm), inferior vena cava (13.4 +/- 4.3 vs 13.9 +/- 4.1cm) and atrial septal roof (12.1 +/- 4.0 vs 12.3 +/- 3.2 cm), or atrial septal size (38.2 +/- 6.2 vs 39.4 +/- 26.6 cm); distance to the mitral valve was greater in the TEE group (13.2 +/- 4.4 vs 11.3 +/- 3.9cm; P<.001). The TEE and fluoroscopy groups showed no significant differences in occlusion device size (14.3 +/- 4.6 vs 13.8 +/- 4.0cm) or sheath size (8.7 +/- 1.8 vs 8.7 +/- 0.9 cm), but procedure duration was shorter in the TEE group (21.5 +/- 14.6 vs 28.6 +/- 10.9 minutes; P<. 001). Postoperative fever (>38 degrees C) occurred less frequently in the TEE group than in the fluoroscopy group (0.8% vs 9.2%; P<. 001); there were no significant differences for the other complications. No patient had postoperative residual shunt, occlusion device shedding/displacement, or pericardial effusion. The TEE group had longer hospital stay (3.2 +/- 0.6 vs 2.9 +/- 0.6 days; P<. 001) and higher procedure cost (29,687 +/- 4218 vs 28,530 +/- 1668 CNY (China Yuan); P=.002) than the fluoroscopy group. TEE-guided percutaneous ASD closure can be used as an alternative to fluoroscopy-guided procedures and avoids the use of radiation or contrast agents.

Abstract:Objective To evaluate the feasibility and short term results of transcatheter closure of secundum atrial septal defects (TCSASDs) using Amplatzer occluder device. A new self-centering device, the Amplatzer atrial septal occluder, has recently been evaluated in an animal model with excellent results. This paper reports our clinical research with this device.Methods 30 patients underwent an attempt at TCSASDs at an average age of 35.6 +/- 15.9 y (range, 5 - 62 y) and average weight of 63.6 +/- 14.5 kg (range, 17 - 78 kg) using the Amplatzer atrial septal occluder. The average ASD diameter measured by transesophageal echocardiography (TEE) was 23.6 +/- 4.0 mm (range, 16 - 31 mm), and the average atrial septal defects (ASD) balloon stretched diameter was 25.2 +/- 5.8 mm (range, 9 - 34 mm). All patients had right atrial and ventricular volume overload with a Qp/Qs of 3.4 +/- 1.2 (1.4 - 5.2). A F8-12 catheter was used for delivery of the device in all patients.Results The device was placed correctly in all patients. There was immediate and complete closure in 30 patients. The average device waist size used to close the defect was 25.6 +/- 5.9 mm (range, 9 - 34 mm). There was no episode of device embolization or any other complication. Follow-up was performed using transthoracic echocardiography (TTE) 1 day, 1 month, 3 months, 6 months and 1 year after the procedure. The first day after operation; there was complete closure of the ASD in all patients; there have been no episodes of endocarditis, thromboembolism, or wire fracture.Conclusion TCSASDs using the Amplatzer occluder device was an efficient nonsurgical method which had a very high technical success rate and satisfactoy short term and follow-up results.
Echocardiographic Guidance of Transcatheter Closure of Atrial Septal Defect

Abstract:Objective This study reports our experience of echocardiographic guidance with transcatheter closure of atrial septal defects(ASD)in patients,using the Amplatzer,a new occlusion device.Methods Nineteen patients with secundum ASD waiting for surgical treatment met transesophageal echocardiography(TEE)[3 transthoracic echocardiography(TTE)]for transcatheter closure.The Amplatzer size was chosen to be equal to or 1～2mm more than the stretched diameter of ASD.Under uhrasound guidance,The device was advanced to ASD with guiding sheath,and then its left disk,waist and right disk was deployed in turn,Once its position was optimal,it was released.Results Sixteen patients were treated successfully except 3 patients with:>34mm stretched diameters,which were too large for the Amplatzers.The mean ASD diameter by echcardiography was significantly smaller than the mean stretched diameter of the ASD[(18.06±7.00)mm vs(22.56±6.25)mm].The devices'positions were optimal and no residual shunt was found by TTE immediately,24 h,1 month and 3 months after the procedure.NO complication was observed during the 3 months fellow up. Conclusions Echocardiography was very important in the selectign of patient,the guidance of the device deployed and the follow-up in the transcatheter closure of ASD with the Amplatzer.

Abstract:There is currently a growing interest in control of stretching of DNA inside nanoconfined regions due to the possibility to analyze and manipulate single biomolecules for applications such as DNA mapping and barcoding, which are based on stretching the DNA in a linear fashion. In the present work, we couple Finite Element Methods and Monte Carlo simulations in order to study the conformation of DNA molecules confined in nanofluidic channels with neutral and charged walls. We find that the electrostatic forces become more and more important when lowering the ionic strength of the solution. The influence of the nanochannel cross section geometry is also studied by evaluating the DNA elongation in square, rectangular, and triangular channels. We demonstrate that coupling electrostatically interacting walls with a triangular geometry is an efficient way to stretch DNA molecules at the scale of hundreds of nanometers. The paper reports experimental observations of λ-DNA molecules in poly(dimethylsiloxane) nanochannels filled with solutions of different ionic strength. The results are in good agreement with the theoretical predictions, confirming the crucial role of the electrostatic repulsion of the constraining walls on the molecule stretching.

Abstract:Atrial septal abnormalities are common congenital lesions that remain asymptomatic in many patients until adulthood. Adults with atrial septal defects (ASD) most commonly have ostium secundum ASD. Transcatheter closure has become increasingly popular in recent years as a successful alternative method to surgery for treating ASD and patent foramen ovale (PFO). The overall rate of ASD transcatheter closure device embolization has been reported to be less than 1%; however, retrieving the device via surgery or by trans-catheter route can be necessary. The current manuscript describes a systematic review of the techniques used to retrieve ASD closure devices, as well as their success rates, complications, and limitations. A comprehensive search was performed covering various databases including PubMed, MEDLINE, SCOPUS, Google Scholar, and Cochrane Library from inception until April 2022 for English-published case reports, case series, and experimental studies investigating the indications, safety, and limitations of ASD closure and ASD device retrieval by trans-catheter approaches. Finally, 20 studies were included in our review. Our findings showed that most of the studies used a single snare technique; of these, all but one reported 100% success. Double tool retrieval methods (snare plus snare, snare plus bioptome, or snare plus forceps) and the gooseneck snare technique yielded 100% success. One study that used the lasso technique reported unsuccessful retrieval and the need for surgical intervention. More recently, the novel "coronary wire trap technique" was introduced, which provides a simpler method for embolized device removal by trapping the device for retrieval using coronary wire.

Abstract:Background The efficient closure of patent foramen ovale (PFO) with short canal length and large width, usually associated with atrial septal aneurysm (ASA), remains challenging. These cases frequently require the use of larger PFO devices to avoid the risk of device embolism at the cost of a higher rate of residual shunting, mostly due to interdisk spacing. The use of atrial septal defect (ASD) prosthesis could be useful in these patients. Purpose To describe feasibility and early outcomes of the use of ASD device for percutaneous closure of widely open PFO representing an "ASD-like anatomy". Methods Since January 2022 in a high volume French university center, we started to use ASD device for percutaneous closure of selected cases of PFO with complex anatomy as mainly defined by a sheathed width of PFO ≥ 5 mm and sheathed canal length ≤ 10 mm (Figure 1) facing major per-operative residual shunt with PFO prostheses in some patients. In addition to the descriptive results, we compared this ASD device subgroup with a cohort of patients with similar anatomy but previously treated in our center with PFO device. All available transesophageal echocardiography (TEE) examinations were reviewed. The main objective was to describe the feasibility of the procedure (success and safety), the secondary endpoint was immediate post-procedural moderate to severe residual right-to-left shunt (RLS), defined as ≥ 10 bubbles in the left atrium within 3 cardiac cycles after transfemoral agitated saline injection. Results A total of 65 patients was treated with ASD devices, procedural success was achieved in all patients with a low rate (4,2%) of intrahospital complications (two supraventricular arrythmia, one anaphylactic shock and one minor laryngeal injury) and without device embolism. As compared with a cohort of 127 patients of similar anatomy treated with PFO device, patients in the ASD device group had more complex characteristics especially higher mean sheathed PFO width and shorter mean sheathed PFO canal length. Immediate post-procedural residual moderate to severe RLS did not differ significatively between groups (Table 1). No or minimal shunting was observed in 91% and 92% of cases (p=NS) but the three patients with severe RLS were all in the PFO device group. Conclusion Percutaneous closure of "ASD-like" PFO with ASD prostheses is feasible and safe. Larger studies and longer follow-ups are now required to confirm the expected improved sealing effect with this strategy.

Abstract:AIM: To report on the feasibility, efficacy and long-term outcome of transcatheter closure of fenestrated Atrial Septal Aneurysm (ASA).METHODS: Between April 2000 and May 2020, 568 patients with fenestrated ASA underwent transcatheter closure at two large-volume Pediatric Cardiology and GUCH Units (Naples and Massa, Italy). Procedural indications were cardiac volume overload in 223 patients (39%) or prophylaxis of paradoxical embolization in 345 patients (61%). Patients' median age and weight were 42 years (range 19-52) and 66 kg (range 54-79), respectively. One-hundred and thirty-nine patients (25%) were younger than 18 years.RESULTS: Single fenestration was found in 311 patients (55%) (Group 1), whereas 257 patients (45%) had multifenestrated ASA (Group 2). The procedure was successful in all but seven patients (1.2%). In Group 1, closure was achieved with a single device. In Group 2, one device was used in 168 patients (67%), two devices in 74 patients (28%), three devices in 12 patients (5%), and four devices in 1 patient (0.3%). The early complication rate was 3%, without any difference according to anatomy or patient's age. At discharge, complete shunt closure was higher in Group 1 (92% vs. 72%, P < 0.0001), without any difference between groups at the last follow-up evaluation (100% vs. 99%, P = 0.12). Procedural safety was persistent during the long-term follow-up (mean 7.2 ± 5.4, range 0-19 years): freedom from adverse events was 97% at 10-15 years. Seventeen patients (3%) were submitted to reintervention, mostly as prophylaxis of paradoxical embolization.CONCLUSION: Transcatheter closure of perforated ASA is technically feasible in a high percentage of cases, with excellent long-term outcomes.

Perimembranous Ventricular Septal Defect with Aneurysm Two Options for Transcatheter Closure

Abstract:We present our experience with 2 options for device closure of perimembranous ventricular septal defect with aneurysm. Thirty-four patients with perimembranous ventricular septal defect with aneurysm, aged from 14 to 42 years, underwent transcatheter closure with modified double-disk occluders. A sheath was used to deliver the occluder after establishment of a stable "arteriovenous loop" under fluoroscopy. Electrocardiography and transthoracic echocardiography were used for follow-up. All but 1 patient experienced successful transcatheter closure of perimembranous ventricular septal defect with aneurysm, when occluders were used in 2 different positions. There were 19 patients whose perimembranous ventricular septal defects were closed at the inlet of the aneurysm and 15 patients whose defects were closed at the outlet. Eight patients had a residual shunt immediately after the procedure, which disappeared during follow-up. One patient developed minor aortic regurgitation. Four patients who manifested different types of conductive block were all in the group that underwent closure at the inlet of the aneurysm. No other complications were observed during follow-up.We infer that perimembranous ventricular septal defect with aneurysm can be successfully closed with modified double-disk occluders. Each of the 2 options that we have presented for transcatheter closure of perimembranous ventricular septal defect with aneurysm has its advantages and disadvantages. Ultimately, the configuration of the lesion should decide the type and position of the device.
Transthoracic Echocardiographic Value in Transcatheter Closure of Perimembranous Ventricular Septal Defect Using Amplatzer Occluder

Abstract:Objective To study the transthoracic echocardiographic(TTE) value in transcatheter closure of perimembra- nous ventricular septal defect(PMVSD)with amplatzer occluder. Methods The size of PMVSD and the distance between the defect and Aortic valve were measured by different angles in TTE before the operation in eight patients. During the operation, the position of guide wires、heath and occlusion device should be monitored. At the same time, whether the PMVSD was occluded firmly, the peripheral residual shunt around occlusion device were observed by TTE too. Result Ten patients were treated successfully. The PMVSD diameter by TTE was similar to the stretched diameter of the PMVSDs. There was no shunt at once. All the occlusion device were fixed tightly and none of them fell off two days and three month later. Conclusion Transcatheter closure of PMVSD with Amplatzer occluder guided by TTE before and dur- ing operation is a simple and important method.

Abstract:Background Transcatheter device closure is the preferred therapeutic option for ostium secundum atrial septal defect(OS-ASD). Standard practice is to perform it under transoesophageal echocardiography(TEE) guidance. However, TEE is performed under general anaesthesia (GA) and leads to increases in cost, procedural duration, ICU stay, total hospital stays, risk of oesophageal injury and complications of GA. Transthoracic echocardiography(TTE) is an excellent alternative and prior studies have shown it to be a safe alternative in paediatric population. In current study we analysed safety and efficacy of TTE-guided device closure of OS-ASD in all comer population irrespective of age, weight, defect size. Methods The study enrolled all patients who underwent device closure of OS-ASD between March 2017 and December 2023. All procedures were performed under LA for adults and conscious sedation for children. The device was placed under fluoroscopy guidance and position were confirmed on TTE. Procedural success, device, defect size ratio, procedural time, fluoroscopy time, complications, intensive care unit/hospital stay duration were analysed and compared with our own experience of 125 patients with TEE-guided closures before switching to complete TTE-guided procedure(before 2017). Results 340 patients underwent TTE-guided device closure of ASD. Procedure success was 97.4% with acceptable risk of major complications. Three(0.8%) had embolization into pulmonary artery, successfully operated surgically. Two had tamponade requiring drainage. In 9 patients, device could not be deployed despite multiple attempts and approaches. All of these had defect size of > 34mm and deficient IVC rim along with shorter (other than aortic) rim. Findings were compared with our own previous experience of TOE guided closure (practised before 2017). Compared to TOE group, TTE group had significantly lower total procedural duration (75±4 vs. 28 ±1 min; p < 0.01), intensive care unit stay (24±10 vs. 10.5±1 h; p < 0.01), and hospital (48±12 vs. 25.5±4h; p < 0.01) stay duration. Fluoroscopic time was similar in both. There was no mortality in either group. Major complications were comparable, TTE-guided group had less post-procedure complications of vomiting and headache. Additional findings of less pre-procedure fasting duration and early commencement of post procedure oral intake were noted in TTE group. Conclusion TTE-guided device closure of OS-ASD is a safe and feasible in all-comer population in experienced hands. It can definitely negate the use of TEE except in very poor echo window. The added advantages include utilization of physiological variations in PV/LV pressure during cough, Valsalva manoeuvres in awake patients. It definitely reduces the procedural duration, cost of procedure and dependence on GA and trained transoesophageal echocardiographer.

Abstract:OBJECTIVE:To evaluate transesophageal echocardiography (TEE)-guided periventricular device closure for treatment of ventricular septal defects (VSD) in children.METHODS:The Amplatzer device was applied, the size of which was 1 or 2 mm larger than the VSD size assessed by TEE. Via a small lower sternotomy, the device was introduced through right ventricular surface to close the VSD under continuous TEE guidance without cardiopulmonary bypass.RESULT:The procedures were successful in 6 cases, including 4 cases with muscular VSD and 2 cases with perimembranous VSD. The diameter of VSD was 3 mm to 10 mm and one case had pulmonary hypertension. One case of multi-muscular VSD with transposition of the great arteries failed to the treatment because the mitral open was interfered by the device. One case had bundle branch block in ICU stay and returned to normal without special therapy. The average operation time was 30 min and blood loss was 5 approximately 20 ml, no patients required blood transfusion; the average length of ICU stay was 24 h. Patients were discharged 1 to 2 d postoperatively. At a median follow-up for 12 months, all patients were asymptomatic and had no residual ventricular shunts.CONCLUSION:With the guide of TEE, intra-operative hybrid therapy is a safe and effective method for closure of muscular and perimembranous VSD without cardiopulmonary bypass in children.
Follow Up of Medium-Term or Prostecdtive Efficacy with Transcatheter Closure of Secundum Type Atrial Septal Defects Using the Homemade Amplatzer Septal Occluder in Middle-Aged or Geratic Patients

Abstract:【Objective】 To evaluate the safety and medium-term or prostecdtive efficacy of the transcatheter occlusion of secundum type atrial septal defect(ASD) using the homemade Amplatzer septal occluder in middle-aged or geratic patients.【Methods】 118 cases(28 males,90 females) aged from 40 to 79 years old(mean age(52.7±9.2) years) with ASD which maximal dimensions from 6 to 38(average(23±4) mm) were confirmed by clinical manifestation,electrocardiogram(ECG),X-ray fluoroscopy,transthoracic color Doppler echocardiography(TTE) from January 2007 through May 2011.According to the edge conditions,ASD were divided into central types(42 cases) and edge-insufficiency types(76 cases).Edge-insufficiency ASD was in-cluded in four types: aortic edge-insufficiency(65 cases),posterior(aortic contralateral edge) edge-insufficiency(2 cases),arium-roof edge-insufficiency(4 cases),mitral valve edge-insufficiency(5 cases).In addition there were consists of 114 single aperture and 4 multiaperture ASD.All of them were received the transcatheter occlusion successfully.Afteroperation all cases received ECG,X-ray fluoroscopy and TTE after 1,3,6,12 months and interval of one year.The shortest follow-up time was 6 months and the longest 4 years.【Results】 118 cases were all occluded successfully,successful rate of 100%.The sizes of homemade Amplatzer septal occluder who using were 10 ～42(27.2 ±7.1) mm.In follow-up period,X-ray fluoroscopy was showed pulmonary venous pleonaemia reduced and cardio-thoracic ration decreased.After 1 month and 3 months all cases with enlargement of internal diameter of atrium dextrum and right ventricular,step-up of pulmonary arterial systolic pressure were found reduced obviously(P 0.05),and more after 6 months(118 cases),1 year(101 cases),2 years(67 cases),3 years(36 cases),4 years(14 cases)(P 0.01).All of them were alived and there was no significant complication in all follow-up period.None of them then underwent therapy again.【Conclusions】 It is safety and good medium-term or prostecdtive efficacy that transcatheter closure of ASD using the homemade Amplatzer septal occluder in middle-aged or geratic patients,and there are advantage of easy procedure,less complication,high successful rate and short length of stay.
Transcatheter Closure of Secundum Atrial Septal Defect with the Amplatzer Septal Occluder Device under Transthoracic Echocardiogram

Abstract:Objective: Transesophageal echocardiogram (TEE) is requisite for safely releasing atrial septal defect occlusion device, including the Amplatzer, a new occlusion device. However, patients may get some sufferinges during TEE procedure under local anesthesia, and patients′ medical expenditure will increase either. The purpose of this study was to investigate whether transthoracic echocardiogram (TTE) can replace TEE during this procedure. Method: Thirty nine patients were enrolled in the study. ASD occlusion were achieved with the Amplatzer occluder under X ray and TTE. TTE was performed immediately before and 24 hours later after the procedure and again TTE was done for 22 patients at 5.5 (3~12) months in average after the operation. Result: The procedure was failed in 4 patients, in two the ASD diameter was too big to be closed, in one the occluder was unable to be implanted properly and in one complicated with acute cardiac tamponade. Thus closure of ASD was performed in 35( 89.7 %) patients. The average diameter of the occluder was 12~38( 26.4 ± 4.0 )mm.The occlusion were successfully achieved with TTE in 33( 91.7 %) patients, only in 3 patients who needed help of TEE including 1 failed case. Faint residual shunt detected by echocardiogram has been seen only in 3 cases immediately after releasing the occluder, and it disappeared 24 hours later. The right atria and the ventricles became smaller compared with those before ASD closure, the systolic pulmonary pressure in average decreased significantly from ( 38.1 ± 6.0 ) to ( 27.3 ± 4.0 ) mmHg (P 0.05 ) without new residual shunt and all occluders get their original place. Conclusion: ①The Amplatzer occluder is very efficient and user friendly with a very high success.②Most ASDs(more than 90%) which are suitable for the Amplatzer occluder can be safely implanted under guidance of TTE. ③In those with a very fragile and small septal rim(around 5 mm) and with a diameter of ASD34 mm, TEE should be used in order to avoid the occluder moving or embolization.

Abstract:This article evaluates a new compliant balloon catheter for sizing of Atrial Septal Defects (ASDs) in vitro and in vivo using X-ray and echo measurements. A phantom consisting of a plastic plate with 17 circular holes varying from 8 to 30 mm was used to determine the accuracy of the measurements. The catheter has a 7 Fr shaft with a double lumen. The balloon is made from a 0.002" plastic membrane. Four platinum marker bands on the shaft of the catheter allow radiographic measurements. The balloon catheter was placed through various holes of the phantom and inflated with 38% contrast medium. X-ray measurement by video and cine recording were performed and compared with the true measurement of the model. The study was carried out in a double blind fashion. For echo measurements, the plastic model was immersed in a water bath. The balloon was inflated with water and measured in a long-axis view. The actual diameters were compared with the measurements obtained by video, cine, and echo recordings. One young pig with a surgically created ASD was used for in vivo measurement. The balloon catheter was passed through the ASD over an exchange wire, inflated with diluted contrast medium until a waist was observed, which was measured by video and cine techniques. The animal was euthanized, and the defect was measured by a tapered measuring probe. The difference between the actual size and measurements obtained by video recordings was 0.54 +/- 0.30 mm, by cine 0.62 +/- 0.20 mm, and by echo measurements 0.60 +/- 0.43 mm. All measurements were less than 1 mm in error except for four measurements (<1.5 mm). A strong correlation was present between each two groups (r = 0.99, P < 0.0001). In the in vivo study, the size of the video and cine recordings (14.2 and 13.9 mm, respectively) were very closer to the actual size (14 mm) as measured by a probe in the postmortem specimen. The new balloon catheter provides a more rapid and precise determination of the stretched diameter of atrial septal defects. It eliminates the error of pulling a catheter through the communication. This new technique should be of benefit to all investigators using septal occlusion devices and will facilitate the selection of the properly sized Amplatzer septal occluder.

Abstract:An ostium secundum atrial septal defect (ASD) is a CHD that can be treated percutaneously since 1974, mostly cases with only one main defect. In cases with fenestrations close to the main defect, a single occluder can be used for treatment because the discs extend beyond the waist of the device. In some cases where the defects are far from each other, they may require either more than one device or surgical closure. We present two patients in whom we observed fenestrations far from the primary defect. Initially, the main ASDs were closed with an ASD occluder, and then the fenestrations were closed with a patent ductus arteriosus (PDA) coil, resulting in complete closure of both defects. This shows that closing small fenestrations that are far away from the primary interatrial defect without rims and using other devices instead, such as a PDA coil, is feasible and can avoid the need for an open-heart surgical procedure; moreover, it is important to note that leaving these fenestrations open can have the same physiology as a patent foramen oval.

Abstract:Abstract Background Atrial septal defect (ASD) is associated with a risk of developing atrial fibrillation (AF) higher than in the general population, even after percutaneous or surgical septal closure. Catheter ablation is an effective treatment strategy for preventing recurrences and reducing the AF burden. However, electrophysiologists are faced with technical difficulties and the risk of complications with the left atrium access in patients with prior ASD percutaneous closure. Case summary We report a case of a patient with highly symptomatic paroxysmal AF, with an ASD, closed percutaneously many years before, who underwent a successful catheter AF ablation, using a single transeptal (TSP) approach guided by transesophageal echocardiography (TEE). Discussion In patients with ASD and an occluder device implanted, there is a potential risk for septal tear during the TSP passage, device dislodgement, or thrombus formation on the device. Atrial fibrillation ablation in this subset of patients has often been protracted and scarcely reported. TEE and intra-cardiac echocardiography have been increasingly used for interventional procedure guidance during AF ablation. As described here, AF ablation using a simplified single TSP guided by TEE is feasible, safe, and effective after device ASD closure.
