Study on the value of integrated echocardiography in transcatheter closure of perimembranous ventricular septeal defect

Abstract:Objective:To evaluate the value of integrated echocardiography in transcatheter closure of perimembranous ventricular septal defect(PMVSD).Methods:Transcatheter closure of PMVSD was performed under the guidance of echocardiography and fluoroscopy in 258 patients. Echocardiography was performed at 10 min, 2 h and 1, 3, 6, 12 months after occluder.Results:238 of 258 patients(92.2%) were successful implantation of occlusion device. 20 of 258 patients (7.8%) were unsuccessful due to the inaccurate measurements of the size,location of PMVSD, and relation to valve tissue by transthoracic echocardiography. The median PMVSD size assessed by echocardiography was 7.1 mm (range 3.8 to 11.7 mm); The shunting diameter was 4.1±1.51mm,the distance from PMVSD to aortic valve was 4.2±2.21 mm,the median device size used was 6.4 mm (range 4 to 12 mm);The relation between the size of device and shunting was De=1.04Dv+1.87.The residual shunt rates by echocardiography at 10 min, 2 hours, 1 month, and 3 months were 21.8%, 6.7%, 1.2%, 0.8%.Conclusion:Integrated Echocardiography can clearly demonstrat the location, size of PMVSD and relation to valve tissue. It is valuable for patients selection before the operation, deployment of the occlusion device during operation and evaluating the therapic effects, is a safe and effective guidance technique.
