Abstract:Purpose: The purpose of this study was to analyze the clinical outcome of arthroscopic anterior cruciate ligament (ACL) reconstruction with bone-patellar tendon-bone autograft versus allograft. Methods: Between May 2000 and June 2004, 172 patients undergoing arthroscopic bone-patellar tendon-bone ACL reconstruction were prospectively randomized into autograft (n = 86) or allograft (n = 86) groups. The senior surgeon performed all operations using the same surgical technique. Each fixation was performed by means of an interference screw. Patients were evaluated preoperatively and postoperatively at follow-up. Of the patients, 156 (76 in the autograft group and 80 in the allograft group) were available for full evaluation. Evaluations included a detailed history, physical examination, functional knee ligament testing, KT-2000 arthrometer testing (MEDmetric, San Diego, CA), Harner's vertical jump and Daniel's 1-leg hop tests, Lysholm score, Tegner score, International Knee Documentation Committee standard evaluation form, Cincinnati knee score, and radiograph. Results: Demographic data were comparable between groups. The mean follow-up was 5.6 years for both groups. There were no statistically significant differences according to evaluations of outcome between the 2 groups except that patients in the allograft group had a shorter operation time and longer fever time postoperatively compared with the autograft group. The postoperative infection rates were 0% and 1.25% for the autograft group and allograft group, respectively. There was a significant difference (P < .05) in the development of osteoarthritis between the operated knee in comparison to the contralateral knee according to radiographs. However, no significant difference was found between the 2 groups at the final follow-up examination (P > .05). Conclusions: Both groups of patients achieved almost the same satisfactory outcomes after a mean of 5.6 years of follow-up. Allograft is a reasonable alternative to autograft for ACL reconstruction. Abstract:Objective To compare clinical results of arthroscopic anterior cruciate ligament(ACL) reconstruction with bone-patellar tendon-bone(B-PT-B) between allograft and autograft.Methods Twentyseven patients underwent ACL reconstruction using irradiated deep-frozen B-PT-B allograft,including 18 males and 9 females,with an average age of 25.5 years(range,16-49 years).Twenty-five patients underwent the same operation but using autograft B-PT-B,including 19 males and 6 females,with an average age of 26.7 years (range,18-43 years).The same arthroscopie technique was used to perform the reconstruction.Postoperative temperature,erythrocyte sedimentation rate,C reactive protein examined.During follow-up,parameters included International Knee Documentation Committee (IKDC) scores,Tegner scores,Lysholm scores,anterior knee pain and KT-2000 side-to-side difference.Muscle strength were measured by the Biodex dynamometer.Results The average follow-up was 37.1 (range,30-48) months in the allograft group and 36.3 (range,30-54) months in the autograft group.No significant difference was found for IKDC scores (89.5/86.5),Lysholm scores (93.0/94.6),Tegner scores (8.0/7.7),anterior knee pain (44.4%/72.0%)and side-to-side difference[30°: (1.72±1.73) mm vs (1.16±1.32) mm; 90°:(1.37±1.90) nun vs (-0.08±1.62)mm].Quadriceps muscle strength recovered to 82.7%/82.8% (60°/s and 120°/s) and 86.6%/85.3% (60°/sand 120°/s) for the allograft and autograft group with no significant difference.Conclusion There was no significant difference with stability and muscle strength recovery between B-PT-B aUograft and autograft.

Abstract:Objective To compare the clinical outcomes of anterior cruciate ligament(ACL)reconstruction by using allograft and autograft of patella tendon.Methods From August 2003 to December 2005,82 ACL reconstruction patients were studied retrospectively,including 59 cases using autograft of patellar bone-tendon-bone(BPTB)and 23 cases using BPTB allograft.Physical examination,scores of IKDC,Lysholm-Tegner and VAS as well as KT-2000 measurements were used for evaluation pre-and post-operation.Results 41 autograft and 17 allograft patients were finally followed up.One allograft patient suffered re-rupture of the reconstructed ligament.Positive ADT test were found in 5 autograft and 1 allograft patients and positive Lachman test were found in 3 autograft and 1 allograft patients.No statistically significant differences were revealed in ROM.IKDC,Lysholm-Tegner Score and KT-2000 measurements showed no significant differences between two groups,but VAS for autograft group were significantly higher than allograft group(P<0.05).Conclusion ACL reconstruction with allograft and autograft achieved similar clinical outcomes.Allograft could be a reasonable choice for ACL reconstruction.
Double-Bundle Anterior Cruciate Ligament Reconstruction Using Bone–Patellar Tendon–Bone Allograft

Abstract:Background: Nonanatomic transtibial single-bundle anterior cruciate ligament reconstruction (SB-ACLR) with a bone–patellar tendon–bone (BPTB) allograft has been used for a long time and has shown the same satisfactory clinical results as an autograft; however, it has not been reported if a double-bundle ACLR (DB-ACLR) could be performed with a BPTB allograft and achieve even better results.Hypothesis: The DB-ACLR with a BPTB allograft is technically feasible and will be superior to the SB technique in restoring better anterior and rotating stability.Study Design: Cohort study; Level of evidence, 2.Methods: The study was performed with 56 patients, and 52 (25 in the DB group and 27 in the SB group) of them were followed up at 2 to 5 years. With an irradiated deep-frozen BPTB allograft, a standard single-incision arthroscopic technique was used, and the graft was fixed with bioabsorbable interference screws on both the femoral and tibial sides. Outcome assessment at final follow-up included International Knee Documentation Committee (IKDC), Tegner, and Lysholm scores; side-to-side difference by conventional KT-2000 arthrometer; total anteroposterior (AP) laxity by the back-pushing KT-2000 arthrometer; pivot shift (0, +, ++); range of motion (ROM); and isokinetic muscle strength evaluation.Results: Mean follow-up was 47.3 ± 11.5 and 58.2 ± 6.6 months for the DB group and SB group, respectively. A statistically significant difference in favor of the DB group was found with the total AP laxity at 30° ( P < .05). The overall incidence of pivot shift in the DB group (4% ++) was significantly lower than that in the SB group (26%: 19% + and 7% ++; P = .029). No significant differences were found between the 2 groups in terms of IKDC score, Lysholm score, Tegner score, conventional KT-2000 arthrometer anterior laxity, ROM, and muscle strength.Conclusion: A DB-ACLR with a BPTB allograft is feasible and achieved more satisfactory results than the transtibial SB technique in terms of total AP stability and rotational stability in spite of no significant differences among other clinical parameters.

Abstract:Objective To compare the clinical curative effect between autograft and allograft bone patella tendon-bone (B-PT-B) transplant to reconstruct anterior cruciate ligament (ACL) arthroseopically.Methods ACL reconstruction using autograft and allograft B-PT-B were performed in 66 patients in the study,autograft group had 31 cases,allograft group had 35 cases; observe the biochemistry,immunology and the response of knee joint pre-and postoperation,observe the therapeutic effect after operation,Lysholm and Larson were used to compare the knee function pre-and postoperation.Results Twenty-nine cases in autograft group were followed up for 5 to 10 years,7 years in average and 32 cases in allograft group for 5 to 6.5 years,6 years in average.The time of generating heat had no significant deviation,biochemistry,immunology after operation had no significant deviation; Lysholm score were 93.28+3.12 (autograft group),93.15±3.22 (allograft group).Difference had not statistically significant (P ＞0.05); Larson score were 93.12±2.15(autograft group),92.75±3.25(allograft group),difference had not statistically significant; the X-ray and MRI results after operation: autograft group had 5 cases of bone tunnel expansion,allograft group had 6 cases of bone tunnel expansion; postoperative complications: both of two groups had one cases of suppurative arthritis,9 cases in allograft group suffered reject reaction,and need clysis under arthroscope or by paracentesis,but none need take out the graft; 1 patient of allograft group was broken and reconstruction.Conclusion Autograft and allograft B-PT-B transplant have the same clinical curative effect in ACL reconstruction,and can make satisfactory motion and stability of joint,and both are nice grafts.
Comparison Between Double- and Single-bundle Arthroscopic Anterior Cructiate Ligament Reconstruction Using Bonepatellar Tendon-bone Allograft

Abstract:Objective To evaluate the mid-term clinical results of arthroscopic double-bundle anterior cruciate ligament(ACL) reconstruction with bone-patellar tendon-bone(BPTB) allograft.Methods The study was carried on 61 patients with broken ACL,who received arthroscopic ACL reconstruction by using BPTB between July 2003 and July 2007.According to the operation and graft types,the patients were divided into 3 groups: BPTB double-bundle allograft group(20 patients),BPTB single-bundle allograft group(21 patients),and BPTB single-bundle autograft group(20 patients).Median follow-up were(41.6±3.0),(42.3±3.9),and(42.5±2.7) months respectively in the 3 groups.At the follow-up,the clinical outcome of the patients was evaluated by assessing the International Knee Documentation Committee(IKDC) 2000 subjective scores,Lysholm scores,Tegner scores,anterior knee pain,routine KT-2000 and back-pushing KT-2000 side-to-side difference,thigh circumference difference and Biodex isokinetic dynamometer system.Results No significant difference was found in terms of the IKDC2000 scores,Lysholm scores,Tegner scores,thigh circumference difference,muscle strength and routine KT-2000 side-to-side difference(P>0.05).The Pivot-shift positive rate(5%,1/20) of the BPTB double-bundle allograft group was 1/6 of that of the BPTB single-bundle allograft group(29%,6/21) and 1/4 of that of the BPTB single-bundle autograft group(20%,4/20).Compared to the single-bundle groups,significant less back-pushing KT-2000 anterior laxity at 30°(median: 15 dl-0.5 mm,20 dl-0.5 mm,and 30 dl-0.9 mm) was revealed in the BPTB double-bundle allograft group(P<0.05).The BPTB double-bundle allograft group(15%,3/20) and the BPTB single-bundle allograft group(10%,2/21) showed significant less anterior knee pain than that of the BPTB single-bundle autograft group(75%,15/20,χ2=24.201,P=0.000).Conclusions The double-bundle ACL reconstruction with BPTB allograft showed better total A-P stability,rotatory stability and less donor site morbidity.It is an alternative of primary doubule-bundle ACL reconstruction for the patients with wider intercondylar fossa.
Preliminary Clinical Research of Arthroscopic Anterior Cruciate Ligament Reconstruction Using a Patella Tendon Allograft

Abstract:Objective To evaluate the method and preliminary clinical results of arthroscopic anterior cruciate ligament(ACL)reconstruction using a patella tendon(bone-patella tendon-bone,BPTB)allograft.Methods We reviewed eight patients who underwent arthroscopic ACL reconstruction using a deep-frozen BPTB allograft.The durations of operations,incisions,recoveries of body temperatures,physical signs of ACL and X-ray results were used to evaluate the operation effects.The postoperative ranges of flexion and Lysholm-Ⅱ score were used to assess the function of the knees.Results Mini incisions were needed and the operation time were shortened markedly in the ACL reconstruction using BPTB allgraft.Normal postoperative body temperature was seen in five days and no infections and immune resists were observed.With 4 months'(2 month to 6 month)follow-up all the cases had a remission of symptom and a negative sign of ADT and Lachmantest.Normal interface crews and bone tunnels were seen by X-ray;The mean range of flexion was 105 degree(80～130 degree)by 2 weeks and 120 degee(100～130 degree)by 2 months and the mean pre- and post- operative Lysholm-Ⅱ score of 2 months were 38.5 points(24～50 points)and 88 points(76～97 points),respectively.Conclusion Our study indicates that ACL reconstruction using a deep-frozen BPTB allograft can effectively reduce the operative injury and improve the function of the knee in the early period.Allograft is one of acceptable alternatives of reconstruction of ACL and the long-term results still need further observation.

Background:Anatomic rectangular tunnel anterior cruciate ligament reconstruction (ART-ACLR) can mimic the fiber arrangement of the native ACL and restore normal knee biomechanics, compared with the conventional round tunnel ACLR. ART-ACLR using a bone–patellar tendon–bone (BPTB) graft can provide satisfactory clinical outcomes; however, some issues such as secondary ACL injury and donor-site morbidity, including postoperative anterior knee pain (AKP), remain to be solved. Due to these issues, quadriceps tendon–bone (QTB) grafts have recently become more popular.Purpose:To compare the 2-year clinical outcomes of ART-ACLR with BPTB and QTB autografts.Study Design:Cohort study; Level of evidence, 3.Methods:A total of 134 patients underwent primary ART-ACLR with BPTB (n = 70) or QTB (n = 64). All patients had a minimum follow-up period of 2 years postoperatively. Outcome evaluations included the International Knee Documentation Committee (IKDC), Knee injury and Osteoarthritis Outcome Score (KOOS), side-to-side differences (SSDs) with the KT-1000 knee arthrometer, rate of secondary ACL injury, and incidence of AKP.Results:Regarding age, sex, height, weight,and concomitant procedures, there were no significant differences between the 2 groups. All clinical scores significantly improved from preoperatively to 6 months postoperatively and further increased throughout the 2-year postoperative period in both groups. The IKDC and all subscales of the KOOS, except Sport and Recreation, were equivalent between the BPTB and QTB groups at each postoperative time point. There were no significant differences in the SSD value of KT-1000 arthrometer between the 2 groups. The rates of secondary ACL injury were 10.0% on the ipsilateral side and 2.9% on the contralateral side in the BPTB group and 3.1% on the ipsilateral side and 4.7% on the contralateral side in the QTB group, with no significant difference between both groups. The incidence of AKP was 17.1% and 4.9% in the BPTB group and QTB group, respectively, with significance of P = .02).Conclusion:The clinical scores, SSD value of the KT-1000 arthrometer, and secondary ACL injury rates were equivalent between the BPTB and QTB groups. However, the incidence of AKP was significantly lower in the QTB group, suggesting that QTB could be a favorable graft for ACLR.

Abstract:Objective To compare the clinical results of anterior cruciate ligament(ACL) reconstruction using anterior tibial muscle tendons allograft and hamstring tendons autograft.Methods According to the graft used for ACL reconstruction,60 patients with chronic ruptures of ACL were divided into group A(30 patients,using anterior tibial muscle tendons allograft)and group B(30 patients,using semitendinosus and gracilis tendons autograft).The average follow-up periods after surgery were 29.4 and 31.6 months respectively.Lysholm,TegnerI,KDC clinical rating scales and KT2000 were used for evaluation pre-and post-operation.The muscle strength of the extensionf,lexioni,nternal rotation and external rotation of knees was evaluated by the isokinetic tester.Results There were statistically significant differences in Lysholm,TegnerI,KDC clinical rating scales and KT2000 results between pre-and post-operation(P <0.01).However,there were no statistically significant differences between group A and group B after operation(P >0.05).The Isokinetic muscle strength tester showed that the knee muscle strength of the internal rotation and flexion were decreased post-operative in group Br,ather than group A,which showed no significant differences between pre-and post-operation(P <0.05).Conclusion For ACL reconstruction under arthroscopy,anterior tibial muscle tendons allograft was a favorable substitute for autograft materials for its convenience,satisfactory clinical effect and less complications.

Abstract:With current trends towards individualized anterior cruciate ligament (ACL) surgery, especially in younger patients performing high pivoting sports and patients with hyperlaxity, the bone-patellar tendon-bone (BPTB) autograft and its advantages in primary ACL reconstruction, have been brought into focus. Still concern remains about donor site morbidity, especially kneeling pain, due to bone block harvesting at the distal patella and tibial tuberosity. There is ongoing scientific effort to reduce this weakness of BPTB-harvesting techniques, without satisfying results so far. This study compares anterior knee pain (AKP) in patients receiving hamstring tendon (HS) and those patients treated with BPTB autograft ACL reconstruction, respectively. In the latter group harvest sites were refilled with autologous bone material from an oscillating hollow saw used to create the tibial tunnel. The aim of this study was, to compare both techniques that there is no significant difference in postoperative knee pain and kneeling pain in the early follow up period. Forty-two patients with primary ACL reconstruction (21 BPTB, group A; 21 HS, group B) were retrospectively analyzed obtaining matched pairs based on age (±5 years), length of follow up (18 ± 6 months) and gender.Preoperative radiological analysis of patellofemoral cartilage defects were recorded using MRI scans and arthroscopic images (modified Outerbridge grading). Additionally clinical and patient-reported outcome measures were obtained, assessing range-of-motion, knee-laxity (Lachman, anterior-drawer test), activities of daily living and sports activity scale (KOS-ADL and SAS score) and graft or contralateral ACL tear occurring during follow up. At short term follow up clinical parameters were similar between patients treated either with hamstring or BPTB autograft. Scores showed higher values for daily living (KOS-ADLS) in the short term in group A (P<0,05), in contrary group B showed higher scores for the KOS-SAS (P<0,05). More patients avoid kneeling on the operated knee after BPTB-autograft reconstruction, without impairing activities of daily living or sports participation. No significant difference existed between clinical stability measures or range of motion at follow up. Our study showed that ACL reconstruction with HS or BPTB autograft has successful short term outcome, with high probability of returning to unimpaired activities of daily living. This study shows that refilling of harvest sites with autologous bone material taken from an hollow saw used for creation of the tibial tunnel leads to acceptable donor site morbidity, without restriction in daily activities and sports participation, respectively. It can therefore be assumed that BPTB autograft without leaving bony defects at the harvest sites is a safe procedure with low donor site morbidity, being relevant in individualized ACL reconstruction.

Abstract:Background: The use of allografts for anterior cruciate ligament (ACL) reconstruction in young athletes is controversial. No long-term results have been published comparing tibialis posterior allografts to hamstring autografts. Purpose: To evaluate the long-term results of primary ACL reconstruction using either an allograft or autograft. Study Design: Randomized controlled trial; Level of evidence, 1. Methods: From June 2002 to August 2003, patients with a symptomatic ACL-deficient knee were randomized to receive either a hamstring autograft or tibialis posterior allograft. All allografts were from a single tissue bank, aseptically processed, and fresh-frozen without terminal irradiation. Graft fixation was identical in all knees. All patients followed the same postoperative rehabilitation protocol, which was blinded to the therapists. Preoperative and postoperative assessments were performed via examination and/or telephone and Internet-based questionnaire to ascertain the functional and subjective status using established knee metrics. The primary outcome measures were graft integrity, subjective knee stability, and functional status. Results: There were 99 patients (100 knees); 86 were men, and 95% were active-duty military. Both groups were similar in demographics and preoperative activity level. The mean and median ages of both groups were identical at 29 and 26 years, respectively. Concomitant meniscal and chondral pathologic abnormalities, microfracture, and meniscal repair performed at the time of reconstruction were similar in both groups. At a minimum of 10 years (range, 120-132 months) from surgery, 96 patients (97 knees) were contacted (2 patients were deceased, and 1 was unable to be located). There were 4 (8.3%) autograft and 13 (26.5%) allograft failures that required revision reconstruction. In the remaining patients whose graft was intact, there was no difference in the mean Single Assessment Numeric Evaluation, Tegner, or International Knee Documentation Committee scores. Conclusion: At a minimum of 10 years after ACL reconstruction in a young athletic population, over 80% of all grafts were intact and had maintained stability. However, those patients who had an allograft failed at a rate over 3 times higher than those with an autograft.

Abstract:Precision centrifuge is an important electromechanical equipment which is used to test and calibrate model parameters of accelerometers. For a 2-DOF(Degree of Freedom) precision centrifuge, which offers a high overload environment and two rotating degree of freedom at the same time, a continuously rotary countershaft is configured reasonably to counteract the rotation motion of the mainshaft system in order to maintain the directivity of the unit under test in the inertial space. Therefore, the pointing accuracy is a significant index, which is influenced mostly by the angular velocity tracking error between the mainshaft and the countershaft. In this paper, the mathematical models of the mainshaft and the countershaft are established by experimental data and frequency spectrum analysis tools, respectively. Also, several synchronous control methods are presented and compared for the 2-DOF arm-typed precision centrifuge by simulation experiments.

Abstract:Background: Anterior cruciate ligament reconstruction is the recommended treatment for patients of all ages who are involved in sports and have symptoms of knee instability. Hypothesis: In patients older than 40 years, allograft reconstruction will have better subjective and objective results than autograft reconstruction, proving allograft to be a better graft source for this patient population. Study design: Cohort study; Level of evidence, 3. Methods: The authors identified 63 patients meeting the criteria for this study, including 38 patients in the allograft group and 25 patients in the autograft group. All patients were older than 40 years and had at least 2 years' follow-up. Objective parameters included preinjury and postoperative Tegner activity rating scale and Lysholm scores, range of motion, thigh circumference differences, side-to-side difference at maximum manual force in anterior displacement by KT-1000 arthrometer, and clinical examination for Lachman and pivot-shift tests. Using a 15-point visual analog scale, the authors performed subjective evaluations. Results: Both groups' Tegner activity rating scale scores returned to preoperative levels. Visual analog scales and range of motion data were similar for both groups. KT-1000 arthrometer data showed a mean maximum difference of 1.46 mm for the allograft group and 0.10 mm for the autograft group (P = .398). Three patients in the allograft group showed greater than 5 mm difference, compared with none in the autograft group. There was 1 clinical failure in the allograft group. In the allograft group, 57% of patients had returned to sports by 6 months versus 25% of patients in the autograft group (P = .005), increasing to 71% and 43%, respectively, at final follow-up (P = .127). Conclusion: Allograft bone-patellar tendon-bone advantages include quicker return to sporting activities; disadvantages include increased laxity and higher incidence of failure. The advantage of autograft bone-patellar tendon-bone is that it appears to be a tighter graft. The authors found that allograft was not a superior graft source in this patient population, leading them to offer both options.

Abstract:The optimized graft for use in anterior cruciate ligament (ACL) reconstruction is still in controversy. The bone–patellar tendon–bone (BPTB) autograft has been accepted as the gold standard for ACL reconstruction. However, donor site morbidities cannot be avoided after this treatment. The artificial ligament of ligament advanced reinforcement system (LARS) has been recommended for ACL reconstruction. The purpose of this study is to compare the midterm outcome of ACL reconstruction using BPTB autografts or LARS ligaments. Between July 2004 and March 2006, the ACL reconstruction using BPTB autografts in 30 patients and LARS ligaments in 32 patients was performed. All patients were followed up for at least 4 years and evaluated using the Lysholm knee score, Tegner score, International Knee Documentation Committee (IKDC) score, and KT-1000 arthrometer test. There were no significant differences between the two groups with respect to the data of Lysholm scores, Tegner scores, IKDC scores, and KT-1000 arthrometer test at the latest follow-up. Our study demonstrates that the similarly good clinical results are obtained after ACL reconstruction using BPTB autografts or LARS ligaments at midterm follow-up. In addition to BPTB autografts, the LARS ligament may be a satisfactory treatment option for ACL rupture.
Functional Outcomes of Bone-Patellar Tendon-Bone Versus Quadrupled Semitendinosus and Gracilis Autografts for Anterior Cruciate Ligament Reconstruction

Abstract:Introduction Anterior cruciate ligament reconstruction (ACLR) with autografts has been available for decades; however, the choice of graft is still debated. Here, we compared the functional outcomes of the two most widely used autografts, bone-patella tendon-bone (BPTB) and quadruple-stranded semitendinosus/gracilis (ST/G) autografts, at six months following ACLR. Materials and methods This prospective study was performed in the Department of Orthopedics of Yenepoya Medical College and Hospital located in Mangalore, Karnataka, India, a tertiary care institute over a period of 18 months (November 2018 to April 2020). The study included 38 adult patients who underwent ACLR and were randomly divided into two groups: BPTB autograft (n=19) or ST/G autograft (N=19). The patients were followed up at one-, three-, and six months. Postoperatively, surgical morbidity, knee stability functional outcome on Lysholm score, and knee range of motion (ROM) were assessed. Results The groups were homogenous and comparable regarding age, sex, side of ACL affected, duration of tear to treatment, and muscle wasting (all p-values > 0.05). At six months, the majority of the patients had a tibial translation of 0-2 mm on the Lachman test and anterior drawer test, and the groups did not differ significantly (both p-values > 0.05). Additionally, at six months, both groups demonstrated a significant increase in mean Lysholm score and mean ROM (both p-values < 0.001). However, the groups did not differ in mean Lysholm score and mean ROM at baseline and any of the follow-up visits (all p-values > 0.05). Conclusion At six months, ACLR with BPTB and ST/G autografts produced significant and comparable knee stability, functional outcome, and ROM.

Randomized controlled trials (RCTs) and observational studies reporting comparisons of QT versus BPTB or HT autografts for ACL reconstruction were included. All analyses were stratified according to study design: RCTs or observational studies. Results: A total of 24 studies were included: 7 RCTs and 17 observational studies. The 7 RCTs included 388 patients, and the 17 observational studies included 19,196 patients. No significant differences in graft failure (P = .36), the International Knee Documentation Committee (IKDC) subjective score (P = .39), or the side-to-side difference in stability (P = .60) were noted between QT and BPTB autografts. However, a significant reduction in donor site morbidity was noted in the QT group compared with the BPTB group (risk ratio [RR], 0.17 [95% CI, 0.09-0.33]; P < .001). No significant differences in graft failure (P = .57), the IKDC subjective score (P = .25), or the side-to-side stability difference (P = .98) were noted between QT and HT autografts. However, the QT autograft was associated with a significantly lower rate of donor site morbidity than the HT autograft (RR, 0.60 [95% CI, 0.39-0.93]; P = .02). A similar graft failure rate between the QT and control groups was observed after both early and late full weightbearing, after early and late full range of motion, and after using the QT autograft with a bone plug and all soft tissue QT grafts. However, a significantly lower rate of donor site morbidity was observed in the QT group compared with the control group after both early and late full weightbearing, after early and late full range of motion, and after using the QT autograft with a bone plug and all soft tissue QT grafts. No difference in effect estimates was seen between RCTs and observational studies. Conclusion: The QT autograft had comparable graft survival, functional outcomes, and stability outcomes compared with BPTB and HT autografts. However, donor site morbidity was significantly worse with the QT autograft than with BPTB and HT autografts.

Abstract:Creutzfeldt-Jakob disease is a rare type of spongiform encephalopathy. Affected patients present with constitutional symptoms, which progress to severe mental deterioration and movement disorders. Dizziness is the most common early otologic symptom. Few reports in the literature describe patients with Creutzfeldt-Jakob disease who present with sudden-onset hearing loss as their primary symptom for seeking treatment. This paper discusses one such patient and reviews the clinical presentation, treatment options, and relevant literature.
[A Prospective Clinical Study on Autologous Periosteum Wrapping Tendon Allograft for Anterior Cruciate Ligament Reconstruction].

Abstract:OBJECTIVE:To study the effectiveness of anterior cruciate ligament (ACL) reconstruction using autologous periosteum wrapping tendon allograft by comparing with using simple tendon allograft.METHODS:Between March 2008 and November 2008, 68 patients with ACL injury were treated, who were in accordance with the inclusion criteria. They were divided into 2 groups randomly according to different treatment methods: ACL was reconstructed with autologous periosteum wrapping tendon allograft in 31 patients (test group) and with simple tendon allograft (control group) in 37 patients. There was no significant difference in gender, age, disease duration, the cause of injury, and functional score preoperatively between 2 groups (P > 0.05). Anatomic single-bundle ACL reconstruction was performed in 2 groups.RESULTS:Little exudation at tibial tunnel incision was found in 1 case respectively in both groups at 2 weeks after operation and was cured by dressing change and antibiotics. The other incisions healed by first intention. The patients were followed up 24-29 months (mean, 26 months) in the test group and 24-32 months (mean, 27 months) in the control group. CT showed bone tunnel enlargement in both groups at 2 years after operation, but the rate of the tunnel enlargement was less in the test group (5/31, 16.1%) than in the control group (14/37, 37.8%), showing significant difference (chi2 = 3.948, P = 0.047). At 2 years after operation, the results of Lachman test and pivot shift test were negative in 23 cases (74.2%) and 25 cases (80.6%) of the test group, and in 26 cases (70.3%) and 30 cases (81.1%) of the control group, respectively. KT-1000 examination showed the displacement of the test group [(1.74 +/- 0.88) mm] was less than that of the control group [(2.36 +/- 0.83) mm], showing significant difference (t = -2.979, P = 0.004). There was no significant difference in Lysholm score, Hospital for Special Surgery (HSS) score, Tegner score, and International Knee Documentation Committee (IKDC) score between 2 groups at 2 years after operation (P > 0.05).CONCLUSION:Compared with simple tendon allograft, ACL reconstruction with autologous periosteum wrapping tendon allograft can improve tendon-bone healing, and decrease the rate of bone tunnel enlargement, so it has good short-term outcome.
