Abstract:Recently multi-contrast magnetic resonance imaging (MRI) has become a powerful tool for plaque analyzing due to its strong potential in enhanced demonstration of carotid wall,but its performance is hampered by the misalignment of difference imaging sequences.To achieve accurate vessel registration of multi-sequence images,a two-step coarse-to-fine registration strategy was proposed in this study.First,the iterative closest point was applied to realize the rigid registration of the centerlines of the multi-contrast images,and then the thin plate spline was used to realize the non-rigid registration based on the boundaries of carotid artery.In the second step,to find the corresponding points of the boundaries in different sequences,a shape context descriptor was innovatively introduced to screen the boundary points.In.addition,the deterministic annealing technique was employed to find a globally optimized solution.The effectiveness of the proposed algorithm was quantitatively evaluated by novel three-dimensional (3D) multi-contrast vessel wall MRI sequences.The results indicated that after registration,the overlap of two boundaries from different sequences was more than 95％,and their mean surface distance was 0.12 mm,which improved the accuracy of registration effectively and laid the foundation for further component analysis.
Clinical effectiveness of contrast medium injection protocols for 80-kV coronary and craniocervical CT angiography—a prospective multicenter observational study

Abstract:Background and objective Decreasing X-ray tube voltage is an effective way to reduce radiation and contrast dose, especially in non-obese patients. The current study focuses on CTA in non-obese patients to evaluate image quality and feasibility of 80-kV acquisition protocols with varying iodine delivery rates (IDR) and contrast concentrations in routine clinical practice. Methods A prospective observational study in patients ≥ 18 years and ≤ 90 kg referred for coronary or craniocervical CTA at 10 centers in China (ClinicalTrials.gov: NCT02840903). Patients were divided into four groups: a standard 100-kV protocol (370 mgI/ml, IDR 1.48 gI/s), and three 80-kV protocols (370 mgI/ml, IDR 1.2 gI/s; 300 mgI/ml, IDR 1.2 gI/s; 300 mgI/ml, IDR 0.96gI/s). The primary outcome was contrast opacification of target vascular segments. Secondary outcomes were image quality (contrast-to-noise ratio, signal-to-noise ratio, visual image quality, and diagnostic confidence assessment), radiation, and iodine dose. Results From July 2016 to July 2017, 1213 patients were enrolled: 614 coronary and 599 craniocervical CTA. The mean contrast opacification was ≥ 300 HU for 80-kV 1.2 gI/s IDR scanned segments; IDR 0.96 gI/s led to lower opacification. Image quality and diagnostic confidence were fair to excellent (≥ 98% of images), despite lower contrast-to-noise ratios and signal-to-noise ratios in 80-kV images. Compared to the standard protocol, 80-kV protocols led to 44–52% radiation dose reductions ( p < 0.001) and 19% iodine dose reductions ( p < 0.001). Conclusion Eighty-kilovolt 1.2 gI/s IDR protocols can be recommended for coronary and craniocervical CTA in non-obese patients, reducing radiation and iodine dose without compromising image quality. Key Points • Using low-voltage scanning CTA protocols, in which tube voltage and iodine delivery rate are reduced proportionally (voltage: 80 kV, IDR: 1.2 gI/s), reduces radiation and contrast dose without compromising image quality in routine clinical practice . • Reducing iodine delivery rate beyond direct proportionality to tube voltage is not beneficial .

Abstract:OBJECTIVE:The aim of this study was to investigate the image quality of cerebral dual-energy computed tomography (CT) angiography using a nonlinear image blending technique as compared with the conventional linear blending method in patients with spontaneous subarachnoid hemorrhage (SAH). METHODS:A retrospective review of 30 consecutive spontaneous SAH patients who underwent a dual-source, dual-energy (80 kV and Sn140 kV mode) cerebral CT angiography was performed with permission from hospital ethical committee. Optimized images using nonlinear blending method were generated and compared with the 0.6 linear blending images by evaluating cerebral artery enhancement, attenuation of SAH, image noise, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR), respectively. Two neuroradiologists independently assessed subjective vessel visualization per segment using a 5-point scale. RESULTS:The nonlinear blending images showed higher cerebral artery enhancement (307.24 ± 58.04 Hounsfield unit [HU]), lower attenuation of SAH (67.07 ± 6.79 HU), and image noise (7.18 ± 1.20 HU), thus achieving better SNR (43.92 ± 11.14) and CNR (34.34 ± 10.25), compared with those of linear blending images (235.47 ± 46.45 HU for cerebral artery enhancement, 70.00 ± 6.41 HU for attenuation of SAH, 8.39 ± 1.25 HU for image noise, 28.86 ± 8.43 for SNR, and 20.37 ± 7.74 for CNR) (all P < 0.01). The segmental scorings of the nonlinear blending image (31.6% segments with a score of 5, 57.4% segments with a score of 4, 11% segments with a score of 3) ranged significantly higher than those of linear blending images (11.5% segments with a score of 5, 77.5% segments with a score of 4, 11% segments with a score of 3) (P < 0.01). The interobserver agreement was good (κ = 0.762), and intraobserver agreement was excellent for both observers (κ = 0.844 and 0.858, respectively). CONCLUSIONS:The nonlinear image blending technique improved vessel visualization of cerebral dual-energy CT angiography by optimizing contrast enhancement in spontaneous SAH patients.

Abstract:Objective:Rudimental contrast media in the subclavian vein often influence the display of carotid on dual-source computer tomography angiography.This article aimed to investigate this influence,and find a best solution.Methods:One hundred patients were randomly divided into Groups A(n=52) and B(n=48).The former received traditional imaging,with 65 ml contrast media,4.0 ml/s injection flow and caudocranial scanning direction,while the latter kept all the parameters unchanged except the scanning direction,from caudocranial to craniocaudal.Each group was graded by original axial images,maximum intensity projection(MIP),silhouette images and VR images for vascular delineation of the carotid.Results: The carotid artery was clearly displayed in Group B,while the subclavian vein was unsatisfactorily revealed in Group A,with statistically significant differences in extent scores between the two groups(P<0.05).Conclusion: In dual-source computer tomography angiography on the carotid,the mode of craniocaudal scan can reduce the influence of the rudimental contrast media in the subclavian vein and improve the accuracy of diagnosis.

Abstract:For accurate depiction of carotid lesions, multi-contrast protocols are utilized in research and clinical application of carotid MRI. The multi-contrast scheme usually requires long scanning time (30 min), thus scanning cost is high and image quality is vulnerable to patients’ involuntary motions. Partially parallel imaging (PPI) has been applied to accelerate carotid imaging in previous works. However, limited by the geometry of the receiver coils, the signal to noise ratio (SNR) of PPI accelerated carotid images is usually too low for diagnosis. In this work, sharable information from multiple contrast carotid images were extracted and applied to enhance PPI. The target is to achieve 3 times faster acquisition without degrading the diagnostic information.

Abstract:Abstract Contrast agents can improve the sensitivity and resolution of magnetic resonance imaging (MRI) by accelerating the relaxation times of surrounding water protons. The MRI performances of contrast agents are closely related to their structural characteristics, including size, shape, surface modification, and so on. Recently, dynamically switchable MRI contrast agents that can undergo structural changes and imaging functional activations upon reaching the disease microenvironment have been developed for high performance MRI. This perspective highlights the ingenious design, controllable structural transformation, and tunable imaging property of dynamic MRI contrast agents. Additionally, the current challenges of the dynamic MRI contrast agents for medical diagnosis are discussed. Furthermore, the future integration of high‐resolution ultra‐high field MRI technology and cutting‐edge dynamic MRI contrast agents for non‐invasive histopathological level accurate detection of microscopic lesions are commented.
Contrast media volume optimization in high-pitch dual-source CT coronary angiography: feasibility study

Abstract:To investigate the feasibility of contrast media (CM) volume reduction in dual-source coronary computed tomography angiography high-pitch mode without affecting coronary artery enhancement. Eighty patients were involved in a preliminary experiment with a default injection protocol (60 ml of CM). The age, BMI, test bolus (TB) enhancement peak and the CT values of coronary artery for each patient were recorded and the key factors for determining coronary artery enhancement were investigated. Based on the results of the preliminary experiment, 120 patients were involved in the main experiment with a new injection protocol. For each patient, the CT values and noise of left coronary sinus (LCS), and the distal segment of right coronary artery were measured. In the preliminary experiment, the peak enhancement of TB correlated most strongly with the HU values of coronary artery. Consequently, the new injection protocol was devised to catalog patients into four groups (30, 40, 50 and 60 ml) of CM based on their TB peak enhancement. In the main experiment, the 30 ml CM injection group whose peak attenuation of TB were the highest (30 vs. 40,50,60 ml: 323.0 ± 27.5 vs. 264.2 ± 11.9, 242.1 ± 8.8, 206.2 ± 18.2 HU, p < 0.05), obtained the highest attenuation of LCS (30 vs. 40,50,60 ml: 365.0 ± 41.2 vs. 341.8 ± 40.0, 326.9 ± 34.7, 312.5 ± 38.2 HU p < 0.05). Contrast optimization is feasible in high-pitch DSCT coronary angiography. Certain patients may receive 30 ml of CM without affecting vessel enhancement.
A Prospective Study of Optimized Contrast Media Injection Protocol for Coronary Computed Tomography Angiography

Abstract:Purpose: To validate a software prototype based on pharmacokinetic and physiologic models, which aimed to optimize contrast media (CM) injection protocol for coronary computed tomography angiography (CCTA) according to patient's test-bolus data. Methods: All the patients underwent CCTA in our hospital during June-Spetember 2016 were randomly assigned into group A (with bolus tracking technique), group B (with test bolus technique) or group C (with prototype software). The desired enhanced attenuation value was set as 350 HU. Attenuation values were measured manually by a radiologist. All the data were analyzed with ANOVA and Levene test by SPSS (version 18.0) (SPSS Inc, Chicago, Ill). Results: A total of 199 cohort patients were recruited. The enhancement level of group A was the highest among these three groups, and was higher than the targeted attenuation (350 HU). Compared with group B, the group C obtained better enhancement effect in middle (S2) and distal segment (S3) of right coronary artery and similar enhancement effect in the other segments. Meanwhile, the amount of CM used in group C was the least. The standard variation of enhancement effect of group C was the lowest except S2 and S3, which meant group C had the least inter-individual enhancement variations except S2 and S3. Conclusions: Compared with traditional injection protocol, the prototype of software can be used to obtain better attenuation (closer to the targeted attenuation) with significantly fewer CM, while inter-individual enhancement variations of coronary artery segments are the least except the middle and distal segment of right coronary artery.

Abstract:Objective: To investigate the optimum contrast medium injection plan of coronary angiography by 64-slice CT. Methods: Thirty consecutive patients undergoing coronary angiography by 64-slice CT were divided into three groups, namely group A, group B1 and group B2, each group included 10 patients. Patients in group A were less than 70 kg, patients in group B1 and group B2 were heavier than 70 kg. Group A and group B1 were injected 50 ml contrast medium with speed of 4.5 ml/s. Group B2 was injected 70 ml contrast medium with speed of 5 ml/s. The enhanced effect of three groups of patient in aorta, the trunk and branch of coronary artery were compared. Results: The difference of enhancement in every vessel between group A and group B2 was not significant. Enhancement in partial vessel in group B1 was significantly less than in group B2. Conclusion: The optimum contrast medium injection plan of coronary angiography by 64-slice CT was feasible.

Abstract:Purpose The accuracy of target delineation for node-positive thoracic tumors is dependent on both four-dimensional computed tomography (4D-CT) and contrast-enhanced three-dimensional (3D)-CT images; these scans enable the motion visualization of tumors and delineate the nodal areas. Combining the two techniques would be more effective; however, currently, there is no standard protocol for the contrast media injection parameters for contrast-enhanced 4D-CT (CE-4D-CT) scans because of its long scan durations and complexity. Thus, we aimed to perform quantitative and qualitative assessments of the image quality of single contrast-enhanced 4D-CT scans to simplify this process and improve the accuracy of target delineation in order to replace the standard clinical modality involved in administering radiotherapy for thoracic tumors. Methods Ninety consecutive patients with thoracic tumors were randomly and parallelly assigned to one of nine subgroups subjected to CE-4D-CT scans with the administration of contrast agent volume equal to the patient’s weight but different flow rate and scan delay time (protocol A1: flow rate of 2.0 ml/s, delay time of 15 s; A2: 2.0 ml/s, 20 s; A3: 2.0 ml/s, 25 s; B1: 2.5 ml/s, 15 s; B2: 2.5 ml/s, 20 s; B3: 2.5 ml/s, 25 s; C1: 3.0 ml/s, 15 s; C2: 3.0 ml/s, 20 s; C3: 3.0 ml/s, 25 s). The Hounsfield unit (HU) values of the thoracic aorta, pulmonary artery stem, pulmonary veins, carotid artery, and jugular vein were acquired for each protocol. Both quantitative and qualitative image analysis and delineation acceptability were assessed. Results The results revealed significant differences among the nine protocols. Enhancement of the vascular structures in mediastinal and perihilar regions was more effective with protocol A1 or A2; however, when interested in the region of superior mediastinum and supraclavicular fossa, protocol C2 or C3 is recommended. Conclusion Qualitatively acceptable enhancement on contrast-enhanced 4D-CT images of thoracic tumors can be obtained by varying the flow rate and delay time when minimal contrast agent is used.
Diluted contrast media combined with high-resolution C-arm CT for evaluating apposition of flow diversions in a porcine model

Abstract:Interventional Neuroradiology, Ahead of Print. BackgroundIn intracranial stenting, good stent apposition is crucial, and high-resolution C-arm computer tomography (CT) is utilized to assess whether stent apposition is complete. This study was aimed at finding optimal hypertonic or isotonic contrast media injection concentration for high-resolution C-arm CT to assess apposition of flow diversion (FD) after carotid artery stenting in swine.MethodsTwelve FD stents were implanted into the left carotid artery of Bama swine through the endovascular method. During high-resolution C-arm CT scanning, 6%, 8%, 10%, 12%, and 14% dilution percentages of hypertonic contrast media (iopromide 370 mg/ml) and 10%, 12%, 14%, 16%, and 18% dilution percentages of isotonic contrast media (iodixanol 320 mg/ml) were separately injected. A radiologist and a neuro-interventional specialist evaluated and qualitatively scored the post-processed images, and intravascular ultrasound (IVUS) was used to verify the accuracy of these images.ResultsOverall, 12 FD stents were implanted into the left common carotid artery of 12 swine, with a technical success rate of 100%. The best reconstructed images used to observe stent apposition were achieved with iopromide diluted to a concentration of 12% (all P < .05) or iodixanol diluted to a concentration of 16% (all P < .05). Malapposition was noted in one case, and good apposition was noted in 11 cases. These results were consistent with IVUS observations.ConclusionInjecting iopromide or iodixanol diluted to 12% or 16% during high-resolution C-arm CT scanning, respectively, can help evaluate FD stent apposition and obtain optimal image quality.

Individually Optimized Contrast Material Injection Protocol for 64-Detector Row Computed Tomography Coronary Angiography

Abstract:Objective:The aim of this study was to determine if the injection rate and volume of contrast material in 64-detector row CT coronary angiography could be individually optimized based on the weight of patients.Methods:Patients undergoing 64-detector row CT coronary angiography in April 2011 were divided into three groups(Group A n=56,Group B n=32,Group C n=32).The weight-rate indices of the three groups were group A 0.06,group B 0.07 and group C 0.08.The injection rate(IR) and injection volume(IV) of contrast material were set based on the formula.For each group,IR,IV and enhancement of different segments of coronary artery were measured and statistically analyzed by one-way ANOVA.Results:The BMI of three groups were not statistically different(P0.05).The average IR and IV of group A were significantly lower than those of group B and group C(P0.05).Group B and group C were not statistically different(P0.05).But the average IV of each group was lower than conventional dosage.The average CT values of the coronary segments in three groups were higher than 250HU and they were higher than 300HU in group B and group C.The enhancement of the coronary segments in group B and group C was significantly higher than that in group A(P0.01).But it was not statistically different between group B and group C(P0.05) unless proximal segment of the right coronary artery.Conclusion:The injection rate and injection volume of contrast material in 64-detector row CT coronary angiography can be personalized based on the weight-rate index of 0.07 with acceptable image quality.

Abstract:Objective This study aimed to ascertain the minimum gadolinium dosage on contrast-enhanced (CE) T2 fluid-attenuated inversion recovery (FLAIR) at appropriate imaging time. Methods Different dosages of gadodiamide were imaged with a 3.0-T magnetic resonance scanner for T2-FLAIR and T1WI. Twenty glioma-induced rat models were randomly assigned into 4 groups (1/2, 1/4, 1/6, 1/8 of routine dosage) and imaged for T2-FLAIR and T1WI preinjection and postinjection of gadodiamide. Contrast-enhanced T2-FLAIR was acquired for 8 repetitions postinjection. Enhancement effects were assessed by calculating contrast-noise ratio and contrast ratio using Kruskal-Wallis and Mann-Whitney rank sum test. Results The in vitro experiment showed that gadodiamide at 1/4 of the T1WI dosage presented the best contrast on CE-T2-FLAIR. For in vivo study, the best enhancement effect on CE-T2-FLAIR was achieved at 1/2 of the routine dosage at 8 to 12 minutes of delayed scanning. Compared with CE-T1WI at routine dosage, CE-T2-FLAIR at 1/2 gadodiamide dosage presented similar enhancement effects with no statistical difference (P = 0.244 and 0.090 for contrast-noise ratio and contrast ratio, respectively). Conclusions Contrast-enhanced T2-FLAIR imaging with half of T1WI routine gadodiamide dosage can produce similar enhancement effects to CE-T1WI when characterizing brain gliomas. The cut-down of contrast agent dosage may help reduce gadolinium accumulation in certain tissues.

Abstract:Objective: To evaluate the feasibility of low dose three-dimensional contrast-enhanced MR angiography (3D CE-MRA) of carotid arteries. Methods: Fifty consecutive patients suspicious of carotid stenosis underwent carotid 3D CE-MRA. The patients were divided into 3 groups, including group A (n=20), group B (n=10) and group C (n=20). Contrast dose of the 3 groups was 20 ml, 15 ml and 12 ml, respectively. The image quality of maximum intensity projection (MIP) was assessed. The signal intensity (SI), signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) of the source images of group A, B and C were compared. Results: No significant difference of MIP imaging quality was found among the 3 groups. SI of group A, B, C was 779.62±127.97, 834.30±147.27 and 797.28±145.07 (P=0.600), and SNR was 152.45±26.83, 175.98±31.49 and 159.74±31.82 (P=0.137), while CNR was 139.17±25.77, 162.78±30.07 and 146.70±31.78 (P=0.124), respectively. Conclusion: Low dose administration of contrast (12 ml) is recommended for CE MRA of the carotid arteries.

Abstract:DCE-MRI is based on the serial acquisition of the fast T1WI.Through multi-phase contrast enhanced scanning,the time-signal intensity curve of the regionof-interest (ROI) can be acquired.Meanwhile,the quantitative parameters reflecting tissue perfusion and vascular permeability are obtained by selecting the proper pharmacokinetic model.This method has been widely applied in many clinical aspects,including tumor diagnosis,assessment of therapeutic response and prognostic evaluation.In this paper,we conducted a systematic review focused on main factors that may influence DCE-MRI outcomes.These factors include scanning protocol,arterial input functions (AIF),the motion displacement and the pharmacokinetic models.Optimizing these main factors may improve the reproducibility and accuracy of the DCE-MRI results.
Contrast-enhanced MRI of carotid atherosclerosis: Dependence on contrast agent

Abstract:Purpose: To investigate the dependence of contrast-enhanced magnetic resonance imaging (MRI) of carotid artery atherosclerotic plaque oil the use of gadobenate dimeglumine versus gadodiamide. Materials and Methods: Fifteen subjects with carotid atherosclerotic plaque were imaged with 0.1 mmol/kg of each agent. For arteries with interpretable images, the areas of the lumen, wall, and necrotic core and overlying fibrous cap (when present) were measured, as were the percent enhancement and contrast-to-noise ratio (CNR). A kinetic model was applied to dynamic imaging results to determine the fractional plasma volume, upsilon(p), and contrast agent transfer constant, K(trans). Results: For 12 subjects with interpretable images, the agent used did not significantly impact any area measurements or the presence or absence of necrotic core (P > 0.1 for all). However, the percent enhancement was greater for the fibrous cap (72% vs. 54%; P < 0.05) necrotic core (51% vs. 42%; P = 0.12), and lumen (42% vs. 63%; P < 0.05) when using gadobenate dimeglumine, although no apparent difference in CNR was found. Additionally, K(trans) was lower when using gadobenate dimeglumine (0.0846 min(-1) vs. 0.101 min(-1); P < 0.01), although upsilon(p) showed no difference (9.5% vs. 10.1%; P = 0.39). Conclusion: Plaque morphology measurements are similar with either contrast agent, but quantitative enhancement characteristics, such as percent enhancement and K(trans), differ.
Application of contrast agents in the evaluation of stroke: conventional MR and echo-planar MR imaging

Abstract:The availability of new therapeutic interventions, including neuroprotective agents and endovascular thrombolysis, has given new hope to patients suffering an acute stroke. Early intervention remains a key factor in the effectiveness of these new and traditional treatments. More importantly, the capability to assess the viability and reversibility of the ischemic tissue became essential for better delineation and differentiation of infarcted versus ischemic tissue and patient management. Abnormal MR imaging (MRI) findings during acute stroke usually reflect the underlying pathophysiologic changes, which can be classified into three sequential stages: (a) hypoperfusion, (b) cellular dysfunction and (c) breakdown of the blood-brain barrier. The first stage is a kinetic phenomenon (not biologic) and, therefore, can be detected immediately. Contrast agents accentuate the abnormal flow kinetics and facilitate the early diagnosis of ischemia using either conventional MRI or newly developed echo-planar perfusion imaging (EPPI). The demonstration of abnormal arterial or parenchymal enhancement on conventional MRI during acute stroke provides the earliest sign of vascular occlusion/stenosis. EPPI, in contrast, provides information related to microcirculation (< 100 microns) and tissue reserve (cerebral blood volume) that cannot be obtained by conventional angiography and is directly related to the target end-organ. Further information obtained from both contrast MRI and EPPI may have a predictive value in the clinical outcome of acute stroke patients.

Abstract:Ischemic stroke has great correlation with carotid atherosclerosis and is mostly caused by vulnerable plaques. It's particularly important to analysis the components of plaques for the detection of vulnerable plaques. Recently plaque analysis based on multi-contrast magnetic resonance imaging has attracted great attention. Though multi-contrast MR imaging has potentials in enhanced demonstration of carotid wall, its performance is hampered by the misalignment of different imaging sequences. In this study, a coarse-to-fine registration strategy based on cross-sectional images and wall boundaries is proposed to solve the problem. It includes two steps: a rigid step using the iterative closest points to register the centerlines of carotid artery extracted from multi-contrast MR images, and a non-rigid step using the thin plate spline to register the lumen boundaries of carotid artery. In the rigid step, the centerline was extracted by tracking the cross-sectional images along the vessel direction calculated by Hessian matrix. In the non-rigid step, a shape context descriptor is introduced to find corresponding points of two similar boundaries. In addition, the deterministic annealing technique is used to find a globally optimized solution. The proposed strategy was evaluated by newly developed three-dimensional, fast and high resolution multi-contrast black blood MR imaging. Quantitative validation indicated that after registration, the overlap of two boundaries from different sequences is 95%, and their mean surface distance is 0.12 mm. In conclusion, the proposed algorithm has improved the accuracy of registration effectively for further component analysis of carotid plaques.
