Abstract:To evaluate the impact of regional perfusion and viability of infracted area on left ventricular remodeling and function in patient after percutaneous coronary intervention(PCI) therapy,twenty anterior myocardial infarction patients with PCI in 20 hours were involved,whose CAG coronary blood flows have been normal.Rest DISA SPECT imagines were performed in 10 days after PCI.According to the myocardium viability and the baseline severity of perfusion in the infracted area(LAD extent %),the patients were divided into two groups.In different groups,echocardiography LVEDD and LVEF were recorded as baseline and follow-up evaluations of cardiac remodeling and function at 1 week and 12 months after PCI.The results showed that the UCG LVEDD changed slightly in two groups of patients with viable(n=10) and without viable myocardium in infarcted area(n=10),but there was no significant difference.In patients with viable myocardium,average 3.46% LVEF increase compared with that 1% decrease in patients without viable myocardium in infracted area.The UCG LVEDD did not changed significantly in both groups with different severity of perfusion in the infarcted area.A trend toward an increase of LVEF was documented for patients with severity of perfusion in the infracted area less than 50%(n=9,39.89±10.19% vs.42.22±13.54%,P0.05),but LVEF did not improve for patients with LAD extent % more than 50%(n=11,42.55±6.86% vs.41.2±8.32%).Compared to patients with lower MIBI uptake and without viable myocardium,there was a significantly higher average improvement of LVEF in five patients with lower MIBI uptake and simultaneously with viable myocardium in infracted area,LVEF increased 4.8%,P=0.029.The study of AMI patients after PCI therapy showed that in patients with higher sestamibi uptake or with viable myocardium,especially higher sestamibi uptake and viable myocardium in the same patient,the LVEF increased better than it in patients with lower sestamibi uptake or without viable myocardium in infracted area 12 months after PCI.Patients with low perfusion and no viable myocardium have worse cardiac function and ventricular remodeling 12 months after PCI.

Abstract:Objective To evaluate relationship between early electrocardiographic ST-T change and left ventricular(LV) remodeling in anterior acute myocardial infarction(AMI) after coronary stent implantation.Methods 28 patients with anterior AMI received coronary stent implantation and were divided into group A(n=16,total resolution of ST elevation≥50% or resolution of maximal ST elevation≥70% an hour after operation,or T wave inverted in 24 hours after operation) and group B(n=12,without above ST-T changes) .Echocardiogram was taken one week and six months later,respectively.Result LV end-diastolic and end-systolic volume index(LVEDVI and LVESVI) ,ventricular wall motion abnormality score(VWMAS) and LV ejection fraction(LVEF) were similar in two groups one week post-operation(P>0.05) .Six months later,LVEDVI,LVESVI and VWMA were significantly smaller and LVEF was significantly higher in group A than that in group B(all P<0.05) .Of them,only LVEF in group A was significant difference between first week and sixth month(P<0.05) .Conclusion LV remodeling in long-term reduces in patients with anterior AMI when obvious resolution of ST elevation or inverted T wave occurs early after coronary stent implantation.
Arterial remodeling after coronary angioplasty: a serial intravascular ultrasound study

Abstract:Background: Restenosis occurs after 30% to 50% of transcatheter coronary procedures; however, the natural history and pathophysiology of restenosis are still incompletely understood. Methods and results: Serial (postintervention and follow-up) intravascular ultrasound imaging was used to study 212 native coronary lesions in 209 patients after percutaneous transluminal coronary angioplasty, directional coronary atherectomy, rotational atherectomy, or excimer laser angioplasty. The external elastic membrane (EEM) and lumen cross-sectional areas (CSA) were measured; plaque plus media (P+M) CSA was calculated as EEM minus lumen CSA. The anatomic slice selected for serial analysis had an axial location within the target lesion at the smallest follow-up lumen CSA. At follow-up, 73% of the decrease in lumen (from 6.6+/-2.5 to 4.0+/-3.7 mm2, P<.0001) was due to a decrease in EEM (from 20.1+/-6.4 to 18.2+/-6.4 mm2, P<.0001); 27% was due to an increase in P+M (from 13.5+/-5.5 to 14.2+/-5.4 mm2, P<.0001). Delta Lumen CSA correlated more strongly with delta EEM CSA (r=.751, P<.0001) than with delta P+M CSA (r=.284, P<.0001). Delta EEM was bidirectional; 47 lesions (22%) showed an increase in EEM. Despite a greater increase in P+M (1.5+/-2.5 versus 0.5+/-2.0 mm2, P=.0009), lesions exhibiting an increase in EEM had (1) no change in lumen (-0.1+/-3.3 versus 3.6+/-2.3 mm2, P<.0001), (2) a reduced restenosis rate (26% versus 62%, P<.0001), and (3) a 49% frequency of late lumen gain (versus 1%, P<.0001) compared with lesions with no increase in EEM. Conclusions: Restenosis appears to be determined primarily by the direction and magnitude of vessel wall remodeling (delta EEM). An increase in EEM is adaptive, whereas a decrease in EEM contributes to restenosis.
Does coronary angioplasty after timely thrombolysis improve microvascular perfusion and left ventricular function after acute myocardial infarction?

Abstract:Background: Recent data show that percutaneous coronary intervention (PCI) in patients with stable postthrombolytic ST-segment elevation myocardial infarction (STEMI) is better than no PCI or ischemia-guided PCI. These results still have to find a pathophysiologic explanation. We hypothesized that complete mechanical recanalization of infarct-related artery improves clinical benefits of thrombolysis as a result of more preserved and better perfused coronary microcirculation. To test this hypothesis, we studied a selected STEMI population presenting very early after symptom onset in whom successful infarct-related artery reperfusion was obtained by thrombolysis followed or not by elective PCI within 24 hours, and we compared these 2 groups with those underwent primary PCI. Methods: This study analyzed 96 patients with STEMI randomized within 3 hours from symptom onset to primary PCI (group A, n = 36), tenecteplase followed within 24 hours by PCI (group B, n = 30), or to tenecteplase alone (group C, n = 30). Microvascular perfusion was assessed by myocardial contrast echocardiography. Regional contrast score, endocardial length and area of contrast defect on day 2 (T1) and at predischarge (T2), left ventricular end-diastolic volume, regional wall motion score, extent of wall motion abnormalities, and ejection fraction at T1, T2, and at 3 months' follow-up were calculated. Results: Baseline clinical and angiographic characteristics were not statistically different between groups. The extent of microvascular damage and of myocardial salvage was similar in primary PCI-treated or in invasively treated patients after lytic administration. Conversely, group C patients, although treated very early with fibrinolytic therapy, showed higher extent of microvascular damage and infarct size and a more depressed left ventricular function after reperfusion and at follow-up. Conclusions: Our data suggest that early PCI after lysis is more effective in preserving myocardial perfusion and function than lysis alone and may be a helpful alternative when primary PCI is not available.
Beneficial Effects of Delayed Opening the Infarct -Related Artery on Late Phase Left Ventricular Function in Acute Anterior Myocardial Infarction

Abstract:Objectives To assess the effect of delayed opening the infarct - related artery(IRA) by percutanous coronary intervention (PCI) on the late phase left ventricular function after acute anterior myocardial infarction. Methods 64 patients with initial Q -wave anterior myocardial infarction and the infarct- related arteries were total occluded conformed by angiogram at 2 to 14 days after onset were divided into successful PCI group and control group (not receiving PCI or the IRA not re - opened). 2 - DE was performed at early phase ( about 3 weeks) , 2 and 6months after onset of AMI respectively to detect the left ventricular function and left ventricular wall motion abnormality (VWMA). The total congestive heart failure events were recorded during 6 months follow-up. Results VWMA scores, left ventricular ejection fraction (LVEF), left ventricular end - diastolic and end-systolic volume indices (LVEDVI and LVDSVI)were similar in 2 groups at early phase and 2 months.There were no differences between early phase and 2months in each group too. VWMA scores and LVEF did not changed at 6 months in each group compared with the early phase and 2 months (P ＞ 0.05 ). But LVEDVI and LVESVI were significantly smaller in the successful PCI group than in the control group (P ＜0.01,P ＜ 0. 05 ). The congestive heart failure events were taken place in 19% of patients in control group compared with 2% in successful PCI group ( P ＞ 0.05 ).Conclusions Although the infarct size does not changed, delayed opening the IRA has beneficial effect to the late phase left ventricular dilatation after acute anterior myocardial infarction.

Abstract:Intramyocardial injection of various injectable hydrogel materials has shown benefit in positively impacting the course of left ventricular (LV) remodeling after myocardial infarction (MI). However, since LV remodeling is a complex, time dependent process, the most efficacious time of hydrogel injection is not clear. In this study, we injected a relatively stiff, thermoresponsive and bioabsorbable hydrogel in rat hearts at 3 different time points - immediately after MI (IM), 3 d post-MI (3D), and 2 w post-MI (2W), corresponding to the beginnings of the necrotic, fibrotic and chronic remodeling phases. The employed left anterior descending coronary artery ligation model showed expected infarction responses including functional loss, inflammation and fibrosis with distinct time dependent patterns. Changes in LV geometry and contractile function were followed by longitudinal echocardiography for 10 w post-MI. While all injection times positively affected LV function and wall thickness, the 3D group gave better functional outcomes than the other injection times and also exhibited more local vascularization and less inflammatory markers than the earlier injection time. The results indicate an important role for injection timing in the increasingly explored concept of post-MI biomaterial injection therapy and suggest that for hydrogels with mechanical support as primary function, injection at the beginning of the fibrotic phase may provide improved outcomes.

Abstract:BackgroundLeft ventricular (LV) remodeling following acute myocardial infarction (MI) is difficult to predict at an individual level although a possible interfering role of vascular function has yet to be considered to date. This study aimed to determine the extent to which this LV remodeling is influenced by the concomitant evolution of vascular function and LV loading conditions, as assessed by phase-contrast Cardiovascular Magnetic Resonance (CMR) of the ascending aorta.MethodsCMR was performed in 121 patients, 2–4 days after reperfusion of a first ST-segment elevation myocardial infarction and 6 months thereafter. LV remodeling was: (i) assessed by the 6-month increase in end-diastolic volume (EDV) and/or ejection fraction (EF) and (ii) correlated with the indexed aortic stroke volume (mL.m−2), determined by a CMR phase-contrast sequence, along with derived functional vascular parameters (total peripheral vascular resistance (TPVR), total arterial compliance index, effective arterial elastance).ResultsAt 6 months, most patients were under angiotensin enzyme converting inhibitors (86%) and beta-blockers (84%) and, on average, all functional vascular parameters were improved whereas blood pressure levels were not. An increase in EDV only (EDV+/EF-) was documented in 17% of patients at 6 months, in EF only (EDV-/EF+) in 31%, in both EDV and EF (EDV+/EF+) in 12% and neither EDV nor EF (EDV-/EF-) in 40%. The increase in EF was mainly and independently linked to a concomitant decline in TPVR (6-month change in mmHg.min.m2.L−1, EDV-/EF-: +1 ± 8, EDV+/EF-: +3 ± 9, EDV-/EF+: -7 ± 6, EDV+/EF+: -15 ± 20, p < 0.001) while the absence of any EF improvement was associated with high persisting rates of abnormally high TPVR at 6 months (EDV-/EF-: 31%, EDV+/EF-: 38%, EDV-/EF+: 5%, EDV+/EF+: 13%, p = 0.007). By contrast, the 6–month increase in EDV was mainly dependent on cardiac as opposed to vascular parameters and particularly on the presence of microvascular obstruction at baseline (EDV-/EF-: 37%, EDV+/EF-: 76%, EDV-/EF+: 38%, EDV+/EF+: 73%, p = 0.003).ConclusionLV remodeling following reperfused MI is strongly influenced by the variable decrease in systemic vascular resistance under standard care vasodilating medication. The CMR monitoring of vascular resistance may help to tailor these medications for improving vascular resistance and consequently, LV ejection fraction.Trial registrationNCT01109225 on ClinicalTrials.gov site (April, 2010).

Effects of canalizing infarction related coronary artery to the left ventricular remodelling and function during the recovery phase of acute myocardial infarction

Abstract:Objective:The infarction related coronary artery (IRCA)were canalized 3-4 weeks after acute myocardial infarction (AMI) The effects of using this technique to the left ventricular remodelling and function were observed.Method:The parameters of left ventricular remodelling and function were measured with echocardiography 3-4 weeks after AMI,remeasured those one year after AMI.There were 29 patients in canalizing group.We analyzed and compared those with no-canalizing group(n=29).Result:3-4 weeks after AMI,There wasno significant difference between the two groups in LV end -diastolic volume(EDV),end-systolic Volume (ESV),ejection fraction(EF),end-diastolic circumference(EDC),extent and severity of LV regional all motion abnormolity (RWMA).One year after AMI,EDV、ESV、EDC、Extent of RWMA all decreased in two groups.There were significantly smaller in canalizing group than in no-canalizing group(p0 01) EF increased in canalizing group and decreased in no-canalizing group.There was significant difference between the two groups(p0 01).Conclusion:The canalizition of IRCA during the recovery phase of AMI could prevent LV remodelling and improve LV function.
Equilibrium radionuclide angiography for evaluating the effect of facilitated percutaneous coronary intervention on ventricular synchrony in patients with acute myocardial infarction

Abstract:Background: It is unclear whether facilitated percutaneous coronary intervention (PCI) via a transradial approach therapy is preferable to primary PCI, with improved ventricular synchrony performance (VS), in Chinese patients. Methods and results: The 152 patients with their first anterior acute myocardial infarction (AMI) were randomized to a primary PCI group or facilitated PCI group. In the 1(st) week and 6(th) month after AMI onset, the parameters of VS were measured by equilibrium radionuclide angiography with ventricular phase analysis. The rate of TIMI grade-3 flow in the infarct-related artery pre-PCI in the facilitated PCI group was higher than that in the primary PCI group (30.56% vs. 8.45%, P=0.001). At the 6(th) month post-AMI, the parameters of time to peak ejection rate, phase shift and peak phase standard deviation were lower than in the primary PCI group (P<0.05, respectively). The incidence of recurrent ischemia and new or worsening congestive heart failure post-AMI in the facilitated PCI group was significantly lower than that in the primary PCI group (2.78% vs. 9.86%, P=0.043; 2.78% vs. 12.68%, P=0.028). Conclusions: Facilitated PCI via a transradial approach might significantly inhibit left ventricular remodeling and improve left ventricular function because of the complete, persistent patency of the infarct-related artery with few complications of vessel access and bleeding.
Clinical Study of the Influence of Infarct Zone Myocardial Viability on Left Ventricular Remodeling after Acute Myocardial Infarction

Abstract:Objective To evaluate whether the presence of residual viable myocardium, detected by low dose dobutamine stress echocardiography (DSE), would favorably influence left ventricular remodeling after acute myocardial infarction (AMI). Methods The patients with a first anterior wall AMI successfully treated with primary coronary angioplasty were studied by low dose DSE [5-10 μg/(kg·min) -1 ] 72 hour after AMI for identifying viable myocardium. Serial two-dimensional echocardiography was performed in all patients 24 hours, 72 hours, 4 weeks and 12 weeks after the attack of AMI. Results A total of 57 consecutive patients were divided in two groups based on the presence ( n =36) or absence ( n =21) of myocardial viability by DSE responses. Throughout the study period, left ventricular end diastolic volume index (LVEDVI) was stable in patients with myocardial viability, but left ventricular systolic volume index (LVESVI) decreased significantly. Patients without myocardial viability had a tendency to increase in both LVEDVI and LVESVI. LVEDVI and LVESVI in patients with myocardial viability were much less than that in patients without viability after the 4 th week [LVEDVI: (64.90±13.52) ml/m 2 versus (90.75±34.92) ml/m 2, P 0.01; LVESVI: (29.63±30.36) ml/m 2 versus (54.62±37.90) ml/m 2, P 0.05] and the 12 th week [LVEDVI: (66.82±9.78) ml/m 2 versus (102.71±13.06) ml/m 2, P 0.01; LVESVI: (28.45±12.34) ml/m 2 versus (51.35±12.51) ml/m 2, P 0.05] respectively. Left ventricular ejection fraction (LVEF)in patients with myocardial viability was significantly greater than that in patients without viability at the end of the 4 th week and the 12 th week respectively. Multivariate regression analysis identified myocardial viability and TIMI grade of infarct related artery as independent predictor of left ventricular remodeling. Conclusion Although good epicardial flow had been achieved after intervention therapy in all of AMI patients, some of them showed left ventricular remodeling in echocardiography. The presence of myocardial viability identified by low dose DSE early after acute myocardial infarction can prevent left ventricular remodeling and preserve left ventricular function.
Assessment of the Relationship Between Preinfarction Angina and Myocardial Microcirculation after Revascularization

Abstract:Objective To assess the relationship of preinfarction angina and myocardial microcirculation and cardiac function after revascularization using intravenous myocardial contrast echocardiography (MCE).Methods Fifty patients with acute myocardial infarction,according to presence of angina in one week before infarction,were divided into angina group(Group A) and non-angina group(Group B) by means of MCE (Levovist),and harmonic power Doppler (Philips Sonos 5500) after successful revascularization.Group A consisted of 27 cases, Group B 23 cases.Results The incidence of no flow after the operation (myocardial contrast score index 0.5 ) was lower in Group A than Group B ( 18.5% vs 60.9% , P 0.001 ).The end-diastolic dimension (EDD) was not significantly different between Group A and Group B in the first week after revascularization( P 0.05 ).The EDD decreased significantly in the fifth week after revascularization in Group A,but not changed in Group B( P 0.05 ).The left ventricular ejection fraction (EF) was improved in Group A.No change of EF was found in Group B( P 0.05 ).Conclusions Preinfarction preserves microcirculation and cardiac function,helps to improve left ventricular remodeling.
Effect of primary percutaneous transluminal coronary angioplasty and stent implantation on left ventricular remodeling and function in patients with acute myocardial infarction: compared with thrombolytic therapy

Abstract:Objective To observe prospectively the changes of left ventricular size and function, and to evaluate the impact of primary percutaneous transluminal coronary angioplasty (p-PTCA) and stent implantation and thrombolytic therapy on left ventricular remodeling and function. Methods The patients with a first acute myocardial infarct and without contraindications to thrombolytic therapy were randomly assingned to p-PTCA group or urokinase (1?500?000 IU) therapy group within 12 hrs from the onset of symptoms or within 12～24 hrs if the evidence of myocardial ischemia in infarc zones was available. Two-demensional echocardaography were applied in the 2 nd , 12 th , and 24 th week after infarction, and major cardiac events were recorded. Results Sixty-five patients were enrolled (male 55, female 10, age 58.95±8.94 y). The patency of infarct related artery of 32 patients assigned to p-PTCA was 100%, while that of thrombolytic therapy group (33 patients) was 75.76%. The differences of end-systolic volume index (ESVI), end-diastolic volume index (EDVI), left ventricular ejection fraction (LVEF), and regional wall motion score index (RWMI) were significant statistically between p-PTCA group and thrombolytic therapy group ( P 0.05, P 0.05, P 0.01, P 0.05, respectively) in 2 nd week after infarction. The trend was true of that of the 12 th week after infarction. Between p-PTCA group and thrombolytic therapy group in the 24 th week after infarction, there was no significant difference in EDVI, LVEF, and RWMI, except ESVIference in global wall motion score index (GWMI) during the whole observation period. Conclusion Compared with urokinase therapy, p-PTCA would have better effect on preventing left ventricular remodeling and improving left ventricular function. The latter might reduce the necessity for revascularization also.

Abstract:Objective To evaluate the impact of the myocardial reperfusion status after percutaneous coronary intervention(PCI) on improvement of left ventricular function in acute myocardial infarction patients by intracoronary myocardial contrast echocardiography(MCE).Methods Eighteen acute myocardial infarction patients were selected and injected with SonoVue via left main or right coronary artery before and 15 minutes after PCI respectively.MCE images were visually valued with score on the perfusion situation of segmental myocardial regions.Left ventricular function in all patients was evaluated at 1 month after PCI.The perfusion score index(PSI) and wall motion score index(WMSI) were calculated by definite formulae.Results Among 30 myocardial segments with PSI as 0 before PCI,there were 20(66.7%) had the WMSI of 3-5 at 1 month after PCI.Among 11 myocardial segments with PSI as 2 before PCI,there were 8(72.7%) had the WMSI of 1-2 at 1 month after PCI.Statistical analysis showed that the level of myocardial reperfusion before PCI was related to the postoperative 1-month status of left ventricular segmental wall motion(P<0.05).Among 12 myocardial segments with PSI as 0 after PCI,there were 11(91.7%) had the WMSI of 3-5 at 1 month after PCI.Among 22 myocardial segments with PSI as 2 after PCI,there were 17(77.2%) had the WMSI of 1-2 at 1 month after PCI.Statistical analysis showed that the level of myocardial reperfusion after PCI was also related to the 1-month status of left ventricular segmental wall motion(P<0.01),and was obviously associated with left ventricular systolic function at 1 month after PCI(P<0.01).Conclusion The myocardial reperfusion status affects the improvement of left ventricular function after PCI in AMI patients.Intracoronary MCE can estimate the level of myocardial perfusion and left ventricular function.It might have the predictive value on clinical prognosis.
Development of Myocardial Perfusion in Acute Myocardial Infarction Patients after Emergency Percutaneous Coronary Intervention

Abstract:Objective: To investigate the development of myocardial microvascular perfusion and the clinical significance of no reflow phenomenon in patients with acute myocardial infarction (AMI) after recanalization of infarction-related arteries. Methods: In 48 patients with first acute myocardial infarction underwent successful emergency percutaneous coronary intervention (PCI) , Single-photon emission computed tomography ( SPECT) was used immediately after PCI and repeated at 24 hours, 2 weeks and 3 months after PCI. All patients were assigned to no-reflow group ( n = 22 ) and reflow group ( n = 26 ) by SPECT score. To analyze the development of myocardial perfusion defect score, left ventricular ejection fraction (LVEF) , left ventricular end-diastolic volume (LVEDV) and left ventricular end-systolic volume (LVESV) in all patients. Results: ( 1) Immediately, 24 hours, 2 weeks and 3 months after PCI, the mean values of SPECT myocardial perfusion defect score were 8. 7±2. 4,6. 8±2. 4,4. 8±2.6 and 4. 3±1.9 respectively in no-reflow patients, Compared with 4. 1±1.0, 1.7±1.2,0. 5±1. 1 and 0. 4±1. 0 in patients with reflow. The differences were remarkable within the first 24 hours between groups (P 0. 05). The patients in no-reflow group had a higher myocardial perfusion defect score than those in reflow group (P 0. 01). (2) LVEF was significantly improved at 3 months and significantly reduced immediately after PCI ( P 0. 05 , respectively ). The patients in reflow group had a higher LVEF than those in no-reflow group immediately after PCI. ( 3 ) LVEDV was significantly greater at 2 weeks and 3 months than immediately after PCI (P 0. 01). LVESV was significantly greater at 2 weeks than immediately after PCI ( P 0. 01) and significantly less at 3 months than immediately after PCI ( P 0. 05). The patients in no-reflow group had a higher LVEDV and LVESV than those in reflow group at every time point (4) The myocardial perfusion defect score immediately after PCI was negatively correlated with LVEF at 3 months after PCI (r = -0. 546, P 0. 01 and r = - 0. 536 , P 0. 05 , respectively ). Conclusion: Myocardial no-reflow phenomenon has an amelioration tendency with time going, and the tendency may be stable after 2 weeks; Early myocardial perfusion has a close correlation with late myocardial function and left ventricle reconstitution.
Left Ventricular Unloading Using Intra-aortic Entrainment Pumping Before Reperfusion Reduces Post-AMI Infarct Size

Abstract:Background: Anterior myocardial infarction standard of care prioritizes swift coronary reperfusion. Recent studies show left ventricular (LV) unloading with transvalvular axial flow pumps for 30 minutes before reperfusion (vs immediate reperfusion) decreases 28-day infarct size. Intra-aortic entrainment pumping, using hardware located away from the heart to provide support throughout the cardiac cycle, decreases effective systemic vascular resistance and augments visceral blood flow and pressure, and may reproduce this benefit with a decreased risk. This study characterized the hemodynamic effects of unloading before and during reperfusion using intra-aortic entrainment pumping and investigated whether unloading decreased anterior myocardial infarction scar size. Methods and results: Yorkshire swine were subjected to 90 minutes of left anterior descending artery balloon occlusion and randomly assigned to immediate reperfusion (n = 6) vs 30 minutes unloading before reperfusion followed by 120 minutes of further unloading (n = 7). Unloading was achieved using percutaneous entrainment pumping in the descending aorta. The anterior myocardial infarction model matches that used in recent transvalvular pumping studies. Mortality before randomization was 22%. After randomization, mortality was 36% for immediate reperfusion and 0% for unloading. Unloading showed immediate hemodynamic benefit that increased through reperfusion and continued support, leading to distinct differences in cardiac function between groups after 30 minutes of reperfusion. Unloading increased stroke volume and cardiac efficiency at this timepoint relative to preocclusion baseline and reduced 28-day LV scar size by 37%-45%. Conclusions: We present the first preclinical data showing extracardiac LV unloading before coronary reperfusion using intra-aortic entrainment pumping decreases 28-day infarct size. Extracardiac unloading to decrease LV scar size may provide an alternative to transvalvular pumping with potential advantages, including reduced risk.

Abstract:Objective To explore the relationship of the number of balloon inflations during primary percutanoeus coronary intervention(PCI) with myocardial perfusion and the extent of myocardial necrosis,and to identify the potential clinical efficacy of ischemic postcondioning in patients undergoing Primary PCI.Methods One hundred and fourty patients with ST-segment elevation myocardial infarction(STEMI) who underwent primary PCI were eligibly enrolled for retrospective analysis.The infarct-related artery(IRA) in all patients had to be either left anterior descending artery(LAD) or right coronary artery(RCA) with TIMI 0 flow grade and no collateral filling prior to intervention,and had to be completely reperfused with TIMI Ⅲ flow grade after PCI.Patients with cardiac arrest,cardiogenic shock,previous MI and pre-infarct angina were excluded.Patients were divided into two groups for analysis based on the number of balloon inflations performed during primary PCI:Group A included all patients in whom 1-2 inflations were applied,while patients in Group B received 3 or more inflations.Myocardial perfusion was evaluated by myocardial blush grade and ST-segment resolution.The extent of myocardial necrosis was evaluated by peak values of CK and CK-MB release and predischarge left ventricular ejection fraction(LVEF) measured by echocardiogram.Results The 2 groups showed similar baseline characteristics,while patients in Group B received more balloon inflations than those in Group A(4.7±0.9 vs.1.8±0.2,P<0.05).The myocardial blush grade was significantly higher in Group B than in Group A(2.35±0.32 vs.1.89±0.29,P<0.05).The maximal ST-segment elevation was comparable in Group A and Group B at the time of admission(4.53±0.62 mm vs.4.31±0.58 mm,P>0.05),but decreased to 1.65±0.49 mm and 0.98±0.37 mm(P<0.05),repectively,at 2 hours after PCI.The peak values of CK and CK-MB release were both significantly lower in Group B than in Group A(1368±726 U/L vs.1976±869 U/L,P<0.05;132±76 U/L vs.189±84 U/L,P<0.05),while the predischarge LVEF in Group B was higher compared with that in Group A(54.76%±11.24% vs.47.68%±10.17%,P<0.05).Conclusion Myocardial perfusion and the extent of myocardial necrosis were improved in STEMI patients requiring multiple balloon inflations during primary PCI compared with those receiving minimal inflations,indicating that multiple balloon inflations and relief of sustained myocardial ischemia in a stuttered manner may mimic ischemic postconditioning and evoke cardioprotection in clinical practice.
