Abstract:Objective To study the correlation of left ventricular remodeling and plasma brain natriuretic peptide undergoing different reperfusion in patients with first st-segment elevation acute anterior myocardial infarction.Methods The study group included 56 patients with acute STE anterior myocardial infarction are devided into three groups: 32 patients with sucessfully early reperfusion(group A),14 patients with sucessfully late reperfusion(group B),10 patients with nonreprefusion.(group C).Plasma BNP concentrations were measured with immunoradiometric assay at the twenty-fourth o'clock and 1month afer AMI.LVEDV、LVESV、LVEF、LVEDVI、LVESVI were taken by Doppler echocardiography at the twenty-fourth to seventy-two o'clock and 1 month after AMI.and we calculate their variables.We investigated plasma BNP concentrations and the echocardiography indexes and their relation at different time undergoing different reperfusion.Normal control group(group D) owns 30 patients.Results The level of BNP is higher in group A、B、C than D,and in groupB、C than A at the twenty-fourth o'clock in patients with first st-segment elevation acute anterior myocardial infarction.There is no difference between group B and C.The early left ventricular remodeling exist in patients with first st-segment elevation acute anterior myocardial infarction.And the degrees are intimately related with different reperfusion treatment.Group C is heavier than group B.Group B is heavier than group A.3、The level of BNP is positive related with the variables of LVEDVI、LVESVI,and negative related with the variables of LVEF.The related degree of left ventricular remodeling and BNP is higher at the twenty-fourth o'clock than at 1month after AMI.Conclusion The level of plasma BNP and the degree of left ventricular remodeling are all influenced by different reprefusion.The level of BNP is positive related with the degree of left ventricular remodeling,and negative related with left ventricular systolic function.BNP is an excellent、convenient and rapid index evaluating the degree of left ventricular remodeling after AMI.
Effects of Combination Treatment with Perindopril and Losartan on Left Ventricular Remodeling and Cardiac Function in Patients with Myocardial Infarction

Abstract:AIM: Inhibiting the renin-angiotensin-aldosterone system prevents left ventricular ( LV) remodeling after myocardial infarction ( MI) . This study was designed to assess the effects of a combination of perindopril and losartan on LV remodeling,cardiac function and serum procollagen type Ⅲ aminoterminal peptide ( PⅢNP) levels in patients with acute MI. METHODS: Patients with anterior MI were divided into 3 groups: MI + perindopril,MI + losartan, and MI + perindopril + losartan. After successful intervention therapy,perindopril 2-4 mg/d or losartan potassium 25-50 mg/d or combination of the both were administered. All patients took aspirin,clopidogrel and statins,while some of the patients were treated with beta-blockers,nitrate and a platelet glycoprotein IIb/Ⅲa receptor antagonist. Three months later, LV dimensions and LV ejection fraction ( LVEF) were measured by ultrasonography. Plasma brain natriuretic peptide ( BNP) ,serum C-reactive protein ( CRP) and PⅢNP levels were evaluated with ELISA or RIA. RESULTS: Baseline characteristics of the 3 groups were the same. All patients showed decreased CRP,increased BNP and PⅢNP levels,and LV dilation and dysfunction after treatment for three months. Compared with the 2 single therapy groups,patients in the combination group showed significantly lower CRP,BNP and PⅢNP levels,less LV dilation and higher LVEF. Serum PⅢ NP level was positively correlated with CRP level and LV end-diastolic volume index( r = 0. 597 and r = 0. 543,respectively,both P 0. 01) ,and negatively correlated with LVEF( r =-0. 565,P 0. 01) . CONCLUSION: For patients with AMI,combination of perindopril and losartan significantly inhibited LV remodeling and improved LV function. Inhibition of myocardial interstitial fibrosis might be part of the mechanism.
Predictors and Long-Term Prognosis of Left Ventricular Aneurysm in Patients with Acute Anterior Myocardial Infarction Treated with Primary Percutaneous Coronary Intervention in the Contemporary Era

Abstract:Background: Primary percutaneous coronary intervention (PCI) has been the standard reperfusion strategy for patients with acute myocardial infarction (AMI) in the contemporary era. Meanwhile, the incidence and prognosis of left ventricular aneurysm (LVA) in AMI patients remain ambiguous. The aim of the current study is to identify the predictor and long-term prognosis of LVA in patients with acute anterior myocardial infarction. Methods: We prospectively enrolled 942 consecutive patients with acute anterior myocardial infarction who were treated by primary PCI. The baseline characteristics, procedural features, and one-year clinical outcomes were compared between the patients with and without LVA. The primary endpoint of major adverse cardiovascular and cerebrovascular events (MACCEs) was defined as a composite of cardiac death, target vessel revascularization, and ischemic stroke. Multiple logistic regression was applied to predict LVA formation and the receiver operating characteristic (ROC) curves were plotted to evaluate the accuracy of the multivariate analysis model. Results: The general incidence of LVA was 15.92%. At one-year clinical follow-up, patients in the LVA group had significantly higher incidence of MACCEs (15.33% vs. 6.44%, P<0.01), mainly driven by an increased incidence of cardiac death (8.00% vs. 2.78%, P<0.01), target vessel revascularization ( 5.33% vs. 2.27%, P=0.03), and ischemic stroke (4.00% vs. 1.39%, P=0.03). Multivariate analysis found that longer symptom-to-balloon time (S2B) [odds ratio (OR): 1.16, 95% confidence interval (CI): 1.11-1.21, P<0.01], higher initial and residual SYNTAX score (iSS, OR: 1.19, 95% CI: 1.14-1.24, P<0.01; rSS, OR: 1.33, 95% CI: 1.22-1.45, P<0.01), lower left ventricular ejection fraction (LVEF) (OR: 1.15, 95% CI: 1.11-1.18, P<0.01), and persistent ST segment elevation (OR: 1.89, 95% CI: 1.06-3.38, P=0.03) were independent predictors of LVA formation. Conclusions: LVA is still common in patients with acute anterior myocardial infarction in the contemporary PCI era, and the prognosis of these patients was significantly worse during the one-year clinical follow- up. Strategies of prompt reperfusion and complete revascularization may be helpful in preventing LVA formation and improving clinical outcomes.
Equilibrium radionuclide angiography for evaluating the effect of facilitated percutaneous coronary intervention on ventricular synchrony in patients with acute myocardial infarction

Abstract:Background: It is unclear whether facilitated percutaneous coronary intervention (PCI) via a transradial approach therapy is preferable to primary PCI, with improved ventricular synchrony performance (VS), in Chinese patients. Methods and results: The 152 patients with their first anterior acute myocardial infarction (AMI) were randomized to a primary PCI group or facilitated PCI group. In the 1(st) week and 6(th) month after AMI onset, the parameters of VS were measured by equilibrium radionuclide angiography with ventricular phase analysis. The rate of TIMI grade-3 flow in the infarct-related artery pre-PCI in the facilitated PCI group was higher than that in the primary PCI group (30.56% vs. 8.45%, P=0.001). At the 6(th) month post-AMI, the parameters of time to peak ejection rate, phase shift and peak phase standard deviation were lower than in the primary PCI group (P<0.05, respectively). The incidence of recurrent ischemia and new or worsening congestive heart failure post-AMI in the facilitated PCI group was significantly lower than that in the primary PCI group (2.78% vs. 9.86%, P=0.043; 2.78% vs. 12.68%, P=0.028). Conclusions: Facilitated PCI via a transradial approach might significantly inhibit left ventricular remodeling and improve left ventricular function because of the complete, persistent patency of the infarct-related artery with few complications of vessel access and bleeding.
Beneficial Effects of Delayed Opening the Infarct -Related Artery on Late Phase Left Ventricular Function in Acute Anterior Myocardial Infarction

Abstract:Objectives To assess the effect of delayed opening the infarct - related artery(IRA) by percutanous coronary intervention (PCI) on the late phase left ventricular function after acute anterior myocardial infarction. Methods 64 patients with initial Q -wave anterior myocardial infarction and the infarct- related arteries were total occluded conformed by angiogram at 2 to 14 days after onset were divided into successful PCI group and control group (not receiving PCI or the IRA not re - opened). 2 - DE was performed at early phase ( about 3 weeks) , 2 and 6months after onset of AMI respectively to detect the left ventricular function and left ventricular wall motion abnormality (VWMA). The total congestive heart failure events were recorded during 6 months follow-up. Results VWMA scores, left ventricular ejection fraction (LVEF), left ventricular end - diastolic and end-systolic volume indices (LVEDVI and LVDSVI)were similar in 2 groups at early phase and 2 months.There were no differences between early phase and 2months in each group too. VWMA scores and LVEF did not changed at 6 months in each group compared with the early phase and 2 months (P ＞ 0.05 ). But LVEDVI and LVESVI were significantly smaller in the successful PCI group than in the control group (P ＜0.01,P ＜ 0. 05 ). The congestive heart failure events were taken place in 19% of patients in control group compared with 2% in successful PCI group ( P ＞ 0.05 ).Conclusions Although the infarct size does not changed, delayed opening the IRA has beneficial effect to the late phase left ventricular dilatation after acute anterior myocardial infarction.
Clinical importance of early electrocardiogram ST-T change after coronary stent implantation in anterior acute myocardial infarction

Abstract:Objective To evaluate relationship between early electrocardiographic ST-T change and left ventricular(LV) remodeling in anterior acute myocardial infarction(AMI) after coronary stent implantation.Methods 28 patients with anterior AMI received coronary stent implantation and were divided into group A(n=16,total resolution of ST elevation≥50% or resolution of maximal ST elevation≥70% an hour after operation,or T wave inverted in 24 hours after operation) and group B(n=12,without above ST-T changes) .Echocardiogram was taken one week and six months later,respectively.Result LV end-diastolic and end-systolic volume index(LVEDVI and LVESVI) ,ventricular wall motion abnormality score(VWMAS) and LV ejection fraction(LVEF) were similar in two groups one week post-operation(P>0.05) .Six months later,LVEDVI,LVESVI and VWMA were significantly smaller and LVEF was significantly higher in group A than that in group B(all P<0.05) .Of them,only LVEF in group A was significant difference between first week and sixth month(P<0.05) .Conclusion LV remodeling in long-term reduces in patients with anterior AMI when obvious resolution of ST elevation or inverted T wave occurs early after coronary stent implantation.
Impact of Viable Myocardium Assessed by 99Tcm-Mibi SPECT and 18F-FDG PET Imaging on Clinical Outcome of Patients with Left Ventricular Aneurysm Underwent Revascularization

Abstract:OBJECTIVE:To evaluate the impact of viable myocardium assessed by (99)Tc()m-MIBI SPECT and (18)F-fluorodeoxyglucose (FDG) PET imaging in patients with left ventricular aneurysm (LVA) underwent revascularization (RVS).METHODS:Forty-six consecutive patients with LVA (mean LVEF 36% +/- 7%), underwent (99)Tc(m)-sestamibi SPECT and (18)F-FDG PET examinations and received RVS therapy, were followed-up for a mean period of 80 +/- 27 months. Viable myocardium in aneurysm was defined as perfusion-metabolism mismatch score (MMS) >/= 2.0. Patients were divided into four groups by aneurysm viability and aneurysmectomy. Group A1 (n = 8): viability-; Group A2 (n = 15): viability-, aneurysmectomy; Group B1 (n = 10): viability +; and Group B2 (n = 13): viability +, aneurysmectomy.RESULTS:The cardiac event rates during follow up were similar among groups [A1 (25%, 2/8), B1 (40%, 6/15), A2 (20%, 2/10) and B2 (31%, 4/13; P > 0.05)]. After revascularization, LVEF was improved (> 10%) in groups A2, B1 and B2 (P < 0.05). Multivariate logistic regression analysis showed that LV-MMS (OR = 2.34, 95% CI 1.08 - 5.06, P < 0.05), distal vessel disease (OR = 0.008, 95% CI 0.001 - 0.560, P < 0.05) and nonaneurysm perfusion score (OR = 0.24, 95% CI 0.07 - 0.85, P < 0.05) were significantly associated with the improvement of LVEF after revascularization.CONCLUSIONS:Long term cardiac events rate post revascularization was not affected by viable myocardium or aneurysmectomy in LVA patients. Viable myocardium in LVA patients was associated with better LVEF improvement after revascularization.
Impact of Regional Perfusion and Viability of Infracted Area on Left Ventricular Remodeling and Function in Patients after PCI Therapy

Abstract:To evaluate the impact of regional perfusion and viability of infracted area on left ventricular remodeling and function in patient after percutaneous coronary intervention(PCI) therapy,twenty anterior myocardial infarction patients with PCI in 20 hours were involved,whose CAG coronary blood flows have been normal.Rest DISA SPECT imagines were performed in 10 days after PCI.According to the myocardium viability and the baseline severity of perfusion in the infracted area(LAD extent %),the patients were divided into two groups.In different groups,echocardiography LVEDD and LVEF were recorded as baseline and follow-up evaluations of cardiac remodeling and function at 1 week and 12 months after PCI.The results showed that the UCG LVEDD changed slightly in two groups of patients with viable(n=10) and without viable myocardium in infarcted area(n=10),but there was no significant difference.In patients with viable myocardium,average 3.46% LVEF increase compared with that 1% decrease in patients without viable myocardium in infracted area.The UCG LVEDD did not changed significantly in both groups with different severity of perfusion in the infarcted area.A trend toward an increase of LVEF was documented for patients with severity of perfusion in the infracted area less than 50%(n=9,39.89±10.19% vs.42.22±13.54%,P0.05),but LVEF did not improve for patients with LAD extent % more than 50%(n=11,42.55±6.86% vs.41.2±8.32%).Compared to patients with lower MIBI uptake and without viable myocardium,there was a significantly higher average improvement of LVEF in five patients with lower MIBI uptake and simultaneously with viable myocardium in infracted area,LVEF increased 4.8%,P=0.029.The study of AMI patients after PCI therapy showed that in patients with higher sestamibi uptake or with viable myocardium,especially higher sestamibi uptake and viable myocardium in the same patient,the LVEF increased better than it in patients with lower sestamibi uptake or without viable myocardium in infracted area 12 months after PCI.Patients with low perfusion and no viable myocardium have worse cardiac function and ventricular remodeling 12 months after PCI.
Clinical Value of Different Cardiac Rehabilitation Programs Applied to Patients with Acute Myocardial Infarction Early after Percutaneous Coronary Intervention

Abstract:Objective To investigate the clinical value and impact of cardiac rehabilitation programs applied to patients with acute myocardial infarction(AMI) early after percutaneous coronary intervention(PCI).Methods The enrolled participants were 90 cases of AMI who received PCI in Shaoxing People′s Hospital from January 2014 to May 2016.They were randomized into traditional routine group,traditional strengthening group and new optimization group with 30 cases in each.The cardiac rehabilitation program for traditional routine group was developed based on the rehabilitation program applied to patients one week after emergency PCI included in 2006 Chinese Cardiac Rehabilitation Programs after PCI,mainly consisting of simple forms of exercise such like walking.Both the traditional strengthening group and new optimization group received the cardiac rehabilitation program developed based on the rehabilitation program applied to patients after selective PCI included in 2006 Chinese Cardiac Rehabilitation Programs after PCI.In addition,the new optimization group received the early-applied,personalized and accurate cardiac rehabilitation program developed in accordance with the latest concepts in cardiac rehabilitation received by Avans University,UMC St RAdboud,Queen Elizabeth Hospital and other hospitals in Hong Kong.The traditional strengthening group and new optimization group had greater basal activities and total activities than the traditional routine group.The intervention period for all the groups was six months.Color Doppler echocardiography was performed in the groups immediately after PCI(before the cardiac rehabilitation intervention) and six months after the starting of cardiac rehabilitation program(after the cardiac rehabilitation intervention),respectively for measuring the left ventricular ejection fraction(LVEF),left ventricular end-systolic volume(LVESV),left ventricular end-diastolic volume(LVEDV) and wall motion score index(WMSI).Levels of serum tumor necrosis factor-alpha(TNF-α),nitric oxide(NO),endothelin 1(ET-1) and soluble vascular cell adhesion molecule 1(sVCAM-1) were measured before and after the cardiac rehabilitation intervention.The incidences of malignant arrhythmia,angina pectoris,heart failure and sudden death during the cardiac rehabilitation were observed.Results No significant differences in LVEF,LVESV,LVEDV,WMSI,TNF-α,NO,ET-1 and sVCAM-1 levels were found among the three groups before the cardiac rehabilitation(P>0.05).After the cardiac rehabilitation intervention,the WMSI was lower in the traditional strengthening group than that in the routine traditional group,LVEF was higher in the new optimization group than that in the routine traditional group and traditional strengthening group,WMSI was lower in the new optimization group than that in the routine traditional group and traditional strengthening group(P<0.05).LVEF was higher after the cardiac rehabilitation intervention than that before the cardiac rehabilitation intervention in the three groups(P<0.05).After the cardiac rehabilitation intervention,the levels of TNF-α,ET-1 and sVCAM-1 were lower in the traditional strengthening group than those in the routine traditional group,the level of NO was higher in the traditional strengthening group than that in the routine traditional group,the levels of TNF-α,ET-1 and sVCAM-1 were lower in the new optimization group than those in the routine traditional group and traditional strengthening group,the level of NO was higher in the new optimization group than that in the routine traditional group and traditional strengthening group(P<0.05).The levels of TNF-α,ET-1 and sVCAM-1 were lower,but the level of NO was higher after the cardiac rehabilitation intervention than that before the cardiac rehabilitation intervention in the three groups(P<0.05).During the cardiac rehabilitation intervention,the incidence of malignant arrhythmia differed significantly among the three groups(P<0.05);three groups had similar incidences of angina pectoris and heart failure(P>0.05);no sudden death occurred in all groups.Conclusion Cardiac rehabilitation training applied to patients with AMI early after PCI can obviously improve the cardiac function and prognosis.The humanitarian cardiac rehabilitation program based on personal conditions used in the new optimization group can achieve the optimal effect of rehabilitation,which can also protect the cardiac function to the utmost and improve the prognosis tangibly.
Effects of canalizing infarction related coronary artery to the left ventricular remodelling and function during the recovery phase of acute myocardial infarction

Abstract:Objective:The infarction related coronary artery (IRCA)were canalized 3-4 weeks after acute myocardial infarction (AMI) The effects of using this technique to the left ventricular remodelling and function were observed.Method:The parameters of left ventricular remodelling and function were measured with echocardiography 3-4 weeks after AMI,remeasured those one year after AMI.There were 29 patients in canalizing group.We analyzed and compared those with no-canalizing group(n=29).Result:3-4 weeks after AMI,There wasno significant difference between the two groups in LV end -diastolic volume(EDV),end-systolic Volume (ESV),ejection fraction(EF),end-diastolic circumference(EDC),extent and severity of LV regional all motion abnormolity (RWMA).One year after AMI,EDV、ESV、EDC、Extent of RWMA all decreased in two groups.There were significantly smaller in canalizing group than in no-canalizing group(p0 01) EF increased in canalizing group and decreased in no-canalizing group.There was significant difference between the two groups(p0 01).Conclusion:The canalizition of IRCA during the recovery phase of AMI could prevent LV remodelling and improve LV function.
[Influence of intracoronary administration of anisodamine on no-reflow, ventricular function and systolic synchrony in acute myocardial infarction patients undergoing percutaneous coronary intervention]

Abstract:Objective: To evaluate the influence of intracoronary administration of anisodamine on myocardial blush grade (MBG) and left ventricular regional and global systolic function and synchrony in the acute myocardial infarction (AMI) patients with no-reflow phenomenon post percutaneous coronary intervention (PCI). Methods: Forty-seven AMI patients who underwent PCI within 12 hours of onset and MBG was 0 - 1 were randomized to receive standard therapy [group B, n = 23, 18 males, mean age (62.72 +/- 11.48) years] or standard therapy plus intracoronary administration of anisodamine [200 microg/ml, group A, n = 24, 18 males, mean age (64.23 +/- 12.27) years]. The left ventriculography (LVG) was performed immediately and 6 months after PCI to measure the ventricular volume, LVEDP and wall motion score (WMS). Equilibrium radionuclide angiography (ERNA) was performed 1 week and 6 months after PCI to determine the parameters of left ventricular regional, global systolic function and systolic synchrony. Incidence of major adverse cardiac events (MACE) during the follow-up was analyzed. Results: Anisodamine [(2530 +/- 340) microg/person)] was well tolerated by patients. The MBG remained unchanged in group B and significantly increased from grade 0.74 +/- 0.32 to grade 2.33 +/- 0.28 10 min after anisodamine injection in group B. Six months post PCI, LVESVI [(40.53 +/- 8.12) ml/m(2) vs. (50.32 +/- 8.26) ml/m(2)], LVEDVI [(80.13 +/- 9.74) ml/m(2) vs. (87.17 +/- 10.25) ml/m(2)], WMS [(8.24 +/- 1.31) vs. (10.23 +/- 1.82)] and LVEDP [(13.36 +/- 4.21) vs. (16.38 +/- 3.21) mm Hg, 1 mm Hg = 0.133 kPa] were significantly lower in group A compared with that in group B (all P < 0.05) while LVEF [(44.02 +/- 5.86)% vs. (38.52 +/- 5.18)%], PER [(1.86 +/- 0.09) EDV/s vs. (1.61 +/- 0.09) EDV/s] and PFR [(2.19 +/- 0.32) EDV/s vs. (1.78 +/- 0.17) EDV/s] measured by ERNA were significantly increased in group A compared with that in group B (all P < 0.05). (2) LrEF(2)-LrEF(8) in group A were higher by 13.96%, 25.02%, 30.36%, 22.86%, 27.67%, 22.07% and 18.71% respectively compared with that in group B. (3) Phase analysis showed that the left ventricular systolic synchrony parameters PS [(46.04 +/- 8.93) degrees vs. (53.19 +/- 162) degrees ], FWHM [(23.02 +/- 6.27) degrees vs. (25.02 +/- 5.31) degrees ] and PSD [(7.92 +/- 4.12) degrees vs. (11.76 +/- 4.11) degrees ] were also significantly lower in group A than that in group B (all P < 0.05). (4) During the 6 months of follow-up, the incidence of MACE in group A was significantly lower than that in group B (P < 0.05). Conclusion: Intracoronary administration of anisodamine is safe and could partly attenuate the no-reflow phenomenon, improve the left ventricular systolic function and synchrony and reduce the incidence of MACE in patients with no-reflow phenomenon post AMI-PCI.

Abstract:Aim: This study aimed to evaluate left atrial and left ventricular volumes, strains, and strain rates in patients with acute ST-segment elevation myocardial infarction (STEMI) treated with percutaneous coronary intervention (PCI) using real-time three-dimensional echocardiography and layer-specific strain techniques. The relationships between these parameters and left ventricular remodeling (LVR) and prognosis were also explored. Methods: The study included 217 patients with first-episode STEMI who underwent emergency PCI. Echocardiography and myocardial strain analyses were performed within 24 h post-PCI. Patients were categorized into early and non-early LVR groups based on the increase in left ventricular end-diastolic volume (LVEDV). The occurrence of major cardiovascular adverse events (MACE) was followed up for one-year post-PCI. Differences in clinical data and ultrasound parameters between the groups were compared, and the predictive value of myocardial strain indicators for late LVR was analyzed using a multivariate logistic regression model and receiver operating characteristic (ROC) curves. Results: Early LVR occurred in 54.8% of patients and was characterized by decreased left ventricular systolic function, more segments with abnormal movement, and reduced absolute strain values in the three layers of the left ventricular wall myocardium, with a compensatory increase in left atrial strain rate during the contraction phase. The early LVR group showed a higher incidence of MACE at the one-year follow-up. At 6 months post-PCI, 29.9% of patients developed late LVR, which was not completely related to early LVR. Late LVR was associated with a higher incidence of MACE. The longitudinal strain value of each layer of left ventricular myocardium obtained from layer-specific strain imaging showed predictive value for advanced LVR. Conclusions: More than half of patients with STEMI develop early LVR post-PCI, with a higher incidence of MACE during one-year of follow-up, necessitating attention to early LVR in clinical practice. Late LVR, which develops in some patients after 6 months, is also linked to a higher incidence of MACE. Accurate monitoring of the myocardial deformation function using layer-specific strain imaging is expected to become a reference indicator for clinical diagnosis and treatment. Monitoring the structure and function of the left atrium and mitral valve alongside the LVR is important for prognostic assessment and for formulating diagnostic and treatment plans.

Abstract:Background Ventricular arrhythmia is a leading cause of cardiac death among patients with post-infarction left ventricular aneurysm (PI-LVA). The effect of coronary revascularization in PI-LVA patients with ventricular tachyarrhythmia remains unknown. This study aims to investigate the impact of revascularization therapy on clinical outcomes in these patients.Methods A total of 238 PI-LVA patients were enrolled, and 59 patients were presented with sustained ventricular tachycardia (VT) or ventricular fibrillation (VF). Patients were classified into 4 groups by treatment strategies (medical or revascularization) and the presence of VT/VF: group 1 (n = 57): VT/VF- and revascularization-; group 2 (n = 122): VT/VF- and revascularization+; group 3 (n = 34): VT/VF+ and revascularization+; and group 4 (n = 25): VT/VF+ and revascularization-. The clinical outcomes were compared, and the primary endpoint was cardiac death or heart transplantation.Results Patients were followed up for 45 +/- 16 months, and 41 patients (17.2%) reached the primary endpoint. Kaplan-Meier analysis showed that in VT/VF- patients, revascularization associated with higher cardiac survival compared with medical therapy (log-rank p = .002), but in VT/VF+ patients, revascularization did not predict better cardiac outcome (log-rank p = .901). Cox regression analysis revealed PET-EF (HR 4.41, 95% CI: 1.72-11.36, p = .002) and moderate/severe mitral regurgitation (HR 2.32, 95% CI: 1.02-5.30, p = .046) as independent predictors of adverse cardiac outcome in patients with VT/VF.Conclusion PI-LVA patients with VT/VF are at high risk of adverse cardiac outcome, and coronary revascularization does not mitigate this risk, although revascularization was associated with higher cardiac survival in PI-LVA patients without VT/VF.
The impact of exercise rehabilitation on left ventricular remodeling and systolic function in acute myocardial infarction patients]

Abstract:To investigate the influence of rehabilitation on left ventricular remodeling and systolic function in acute myocardial infarction patients.Patients meeting the inclusion criteria with uneventful clinical course after a first myocardial infarction were randomly assigned to a 3-month exercise training period (exercise group, n = 35) or a control group (n = 29). Before randomization, all patients underwent NT-proBNP test, 2-dimensional echocardiography, and submaximal exercise test.(1) At baseline, there were no significant differences in NT-proBNP, left ventricular end-diastolic diameter (LVDd) and left ventricular ejection fraction (LVEF) between the exercise and control groups. After 3 months, a significant decrease in NT-proBNP was observed only in the exercise group [from (845.29 +/- 93.48) ng/L to (335.64 +/- 246.14) ng/L, P < 0.05], but not in the control group [from (1091.62 +/- 101.78) ng/L to (1099.83 +/- 168.75) ng/L, P > 0.05) and there was significant difference in NT-proBNP level between the two groups after 3 months (P < 0.01). The LVDd increased in the control group [from (50.9 +/- 5.8) to (52.6 +/- 5.4) mm, P < 0.05] but not in the exercise group [from (50.7 +/- 4.5) to (50.3 +/- 3.9) mm, P > 0.05] and there was significant difference in LVDd between the two groupsafter 3 months (P < 0.05). Conversely, LVEF improved in the exercise group [from (55.0 +/- 8.6)% to (60.0 +/- 8.0)%, P < 0.05] but not in the control group (P > 0.05) and there was significant difference in LVEF between the two groups after 3 months (P < 0.05). (2) NT-proBNP was inversely correlated with LVEF.Rehabilitation exercise training under instructions based on family condition in the early and recovery stage of AMI can lower the NT-proBNP level, improve ejection fraction, and prevent the increase of left ventricular end-diastolic diameter. Therefore, it may reduce unfavorable remodeling response and improve cardiac systolic function hereafter.

Abstract:Objective To observe prospectively the changes of left ventricular size and function, and to evaluate the impact of primary percutaneous transluminal coronary angioplasty (p-PTCA) and stent implantation and thrombolytic therapy on left ventricular remodeling and function. Methods The patients with a first acute myocardial infarct and without contraindications to thrombolytic therapy were randomly assingned to p-PTCA group or urokinase (1?500?000 IU) therapy group within 12 hrs from the onset of symptoms or within 12～24 hrs if the evidence of myocardial ischemia in infarc zones was available. Two-demensional echocardaography were applied in the 2 nd , 12 th , and 24 th week after infarction, and major cardiac events were recorded. Results Sixty-five patients were enrolled (male 55, female 10, age 58.95±8.94 y). The patency of infarct related artery of 32 patients assigned to p-PTCA was 100%, while that of thrombolytic therapy group (33 patients) was 75.76%. The differences of end-systolic volume index (ESVI), end-diastolic volume index (EDVI), left ventricular ejection fraction (LVEF), and regional wall motion score index (RWMI) were significant statistically between p-PTCA group and thrombolytic therapy group ( P 0.05, P 0.05, P 0.01, P 0.05, respectively) in 2 nd week after infarction. The trend was true of that of the 12 th week after infarction. Between p-PTCA group and thrombolytic therapy group in the 24 th week after infarction, there was no significant difference in EDVI, LVEF, and RWMI, except ESVIference in global wall motion score index (GWMI) during the whole observation period. Conclusion Compared with urokinase therapy, p-PTCA would have better effect on preventing left ventricular remodeling and improving left ventricular function. The latter might reduce the necessity for revascularization also.

Abstract:Abstract Objective We aim to assess the left ventricular strain in patients with ventricular aneurysm(VA) after myocardial infarction(MI) using cardiac magnetic resonance-feature tracking (CMR-FT) and to evaluate its value for long term prognosis of patients.Methods Sixty-five patients who underwent CMR with VA after MI from January 2018 to December 2019 in Drum Tower Hospital Affiliated Hospital of Nanjing University School of Medicine were selected for the study. They were divided into two groups based on New York Heart Association (NYHA): 25 cases of NYHA I as group A and 40 cases of NYHA II-IV as group B. CMR was performed in both groups to quantify the parameters of overall and segmental left ventricular myocardial strain in patients with aneurysm. 37 of whom underwent a second CMR 3-12 months after cardiac infarction to investigate the effects of aneurysm on patients' left ventricular strain and left ventricular cardiac function.Results Patients from group B have larger VA basilar transverse diameter and significant more impaired LV Global longitudinal strain(GLS)、Global circumferential strain(GCS)、Global radial strain(GRS) (-12.34±7.31 vs. -7.68±6.11;p=0.0072, -21.31±13.49 vs. -14.93±10.44;p=0.0361, 37.13±27.87 vs. 22.00±20.05;p=0.0135) without change in infarct size. GLS, GCS, GRS were significant indicators of NYHA classification after AMI by multivariate regression analysis.Conclusions Myocardial strain assessed by CMR-FT may be an independent predictor of NYHA of patients with aneurysm after MI and could be used for identifying high-risk patients with VA.

Abstract:Aims Due to its low incidence, poor prognosis, and high mortality in the acute phase, the long‐term prognosis of the left ventricular aneurysm (LVA) complicated by ventricular septal rupture (VSR) has received little attention. This study focus on the long‐term prognosis of patients with LVA complicated by relatively stable VSR. Methods and results Over a decade of retrospection, 68 patients with both LVA and VSR were compared with 136 patients with LVA alone after propensity score matching. Patients with both LVA and VSR were further divided into two groups depending on whether pre‐operative intra‐aortic balloon pump (IABP) was used (23 pre‐operative IABP vs. 45 non‐pre‐operative IABP). The primary endpoint was defined as major adverse cardiovascular and cerebrovascular events, a composite endpoint including mortality, myocardial infarction, revascularization, stroke, and heart failure. Abstract:Objective To evaluate the clinical effect of percutaneous coronary intervention(PCI) using routine echocardiography,and compare the ability of emergency PCI with delayed PCI in improving left ventricular function in patients with acute myocardial infarction(AMI).Methods Twenty-two AMI patients with left anterior descending(LAD) coronary to be the "criminal vessel" were divided into emergency PCI group and delayed PCI group.Biplane Simpson's formula was used to investigate the change of left ventricular systolic function in all patients.Results left ventricular systolic function improved significantly at 1 and 3 month in both group than 7days after PCI,while left ventricular diameter decreased significantly.No significant difference was found in left ventricular systolic function between the two groups at any follow-up time.Conclusion delayed PCI after initial antithrombotic agent infusion can improve left ventricular function as well as emergency PCI.Left ventricular function achieved significant improvement at 1 and 3 month's follow-up.
Long-term Effect of Large-Dose Atorvastatin on Ventricular Remodeling in the Patients with Acute Myocardial Infarction

Abstract:Objective To investigate the effect of large-dose atorvastatin on ventricular remodeling and cardiac function in the patients with acute myocardial infarction(AMI).Methods 91 patients with first acute ST segment elevation myocardial infarction were selected.In control group,low density lipoprotein cholesterol(LDL-C) of patients were lowered to 2.6 mmol/L and the others were divided into conventional-dose atorvastatin group(10 mg/day,for 24 weeks) or large-dose atorvastatin group(40 mg/day,for 24 weeks) at random.By using transthoracic Doppler echocardiography,LVESV,LVEDV,LVMI,WMSI,LVEF and VE/VA were measured within 24 hours after AMI and 24 weeks later,respectively.The serum levels of PⅢNP,MMP-9 and hs-CRP were also measured within 24 hours after AMI and 24 weeks later,respectively.Results ①Compared with conventional-dose atorvastatin group and the control group,the LVESV,LVEDV,LVMI and WMSI in large-dose atorvastatin group decreased significantly,and the levels of VE/VA and LVEF increased significantly(P0.01),and there were significant differences in above-mentioned indexes between conventional-dose group and the control group 24 weeks later(P0.05).②12 weeks later,the levels of hs-CRP,MMP-9 and PⅢNP in large-dose atorvastatin group were significantly lower than those in both conventional-dose atorvastatin group and the control group(P0.01),whereas the levels of these in conventional-dose atorvastatin group were also lower than those in the control group significantly(P0.05,P0.01).③There were significant correlation between serum levels of hs-CRP,MMP-9,PⅢNP and LVEDV,LVEF.Conclusions Atorvastatin can suppress ventricular remodeling and improve cardiac systolic and diastolic function after AMI,and its effect could be further strengthened by increasing the dosage of atorvastatin.
