Abstract:Objective: To investigate the difference of long-term quality of life(QOL) of patients who underwent surgery for esophageal cancer,two surgical methods were assessed in this prospective study.Methods: The quality of life questionnaire,EORTC QLQ-C30 Chinese version,was used to assess the patient's quality of life.All the 160 patients from April 1,2010 to April 1,2011 in our department treated for esophagectomy were collected in this study.The QLQ-C30 scores of preoperative and postoperative 3,6 and 12 months were compared with each other.Results: Compared with the preoperative and postoperative quality of life,we found that emo-tional function and physical function decreased significantly,dyspnea and pain were aggravated significantly,and fatigue and financial difficulties significantly increased in postoperative 3 months.Financial difficulties was restored at 12 months postoperative.Physical function,role function,emotional function and overall condition decreased significantly.That fatigue postoperative,and dyspnea 3,6 months postoperative was increased significantly in the pneumonectomy group.Conclusion: The preoperative and postoperative quality of life compared to removal of all non-significant impact on patients quality of life.Cervical esophageal anastomosis decreased the long-term quality of life significantly than that of the intrathoracic esophageal anastomosis.
Health-related quality of life of esophageal cancer patients in daily life after treatment: A multicenter cross-sectional study in China.

Abstract:Background The improvement of diagnostic and therapeutic techniques has prolonged the survival time of patients with esophageal cancer. Little is known, however, about their health-related quality of life (HRQoL) in daily life after treatment. Methods Results Esophageal cancer patients who had been discharged from hospitals more than one year and healthy controls identified by screening were recruited from seven study centers covering eastern, central, and western regions of China. Patients were categorized into severe dysplasia/carcinoma in situ and stages I, II, III, and IV cancer, respectively. The EQ-5D was employed to assess HRQoL. Multivariate regression analyses were conducted. A total of 1456 patients and 2179 controls were recruited. After adjusting for potential confounding factors, the likelihood of reporting problems in the five dimensions of patients was 3.8 to 23.1 times higher than controls, whilst the mean EQ-5D utility score was 0.311 (95% CI, 0.276-0.346) lower than controls. The mean utility scores of each patient subgroup were 0.158, 0.289, 0.303, 0.296, and 0.505 (95% CIs: 0.108-0.208, 0.243-0.336, 0.261-0.346, 0.244-0.347, and 0.437-0.573) lower than controls, respectively. Patients had the greatest impairment in the self-care dimension compared with controls, followed by the usual activities dimension. Therapeutic regimen, duration of illness, other chronic disease status, age, and marital status also had significant impact on different aspects of HRQoL in patients. Conclusions Esophageal cancer significantly impaired patients' HRQoL in daily life after treatment. Advanced cancer stages were associated with larger decrements on health state utility. Utility scores reported here can facilitate further cost-utility analyses.

Abstract:PurposeImproving survival after esophagectomy is an important issue in treating patients with esophageal cancer (EC). In addition to standard hospitalization management, periodic assessment of quality-of-life (QOL) measures may be useful to detect disease progression from patients’ subjective reports. Therefore, this prospective longitudinal study was undertaken to identify prognostic factors for 3-year survival of EC patients after esophagectomy and to evaluate the impact of QOL measures on these prognostic factors.MethodsPatients with EC (n = 67) who had a complete tumor resection and were alive 6 months after esophagectomy were followed in this study for 3 years. Data were collected on patients’ sociodemographics, cancer characteristics, adjuvant therapy, general QOL and EC-specific QOL (before esophagectomy and 6 months afterward), cancer recurrence, and death. Patients’ independent risk factors for 3-year survival were investigated by multivariate Cox regression analysis.ResultsOf the 67 participants with EC, 26 had late mortality, with a median survival for the whole cohort of 38.2 months (95 % CI 31.97–44.35). Independent predictors of early death were early cancer recurrence (within 6 months after surgery), poor cognitive function (95 % CI 1.020–1.041), and worse dyspnea (95 % CI 1.007–1.034).ConclusionsThe most predictive factor for early death in EC patients after esophagectomy was cancer recurrence within 6 months after surgery. However, QOL measures could be a tool to provide clinical information from patients’ perspective suggesting cancer recurrence.

Dynamics of Long-Term Quality of Life after Treatment for Esophageal Cancer: A Community-Based Patient Study

Abstract:PURPOSETo characterize the pattern of post-treatment quality of life (QoL) for esophageal cancer (EC) survivors and construct models predicting their long-term QoL.METHODSOn the basis of a randomized trial in an EC high-risk region in China, we interviewed 363 EC survivors and 25,245 permanent residents matched with the survivors on age, sex, and township as the baseline. QoL was measured using three-level version of European Quality of Life 5-Dimensions instrument. We constructed piecewise mixed models estimating the QoL of EC survivors that varied by age, sex, patient type, hospital level, and therapy to ascertain QoL determinants.RESULTSThe post-treatment QoL of EC survivors dropped by 15.7% within the first year and recovered by 9.3% between 1 and 9 years compared with the baseline. Therapy was found to be a determinant of QoL, and a series of therapy-specific models were fitted accordingly, which all showed the pattern of decreasing rapidly and recovering gradually. Endoscopic treatment had the least impact on post-treatment QoL (7.5% drop within 5 years) compared with esophagectomy (12.2% drop within 1 year) and chemoradiotherapy (37.8% drop within 2 years). The usual activities dimension showed the greatest impairment among those patients (34.4% drop within 1 year).CONCLUSIONThis community-based study described the long-term QoL trajectory for EC survivors after different therapeutic modalities and constructed models to predict therapy-specific QoL at different time points after treatment. It provided new insights into decision making in treatment for EC from the perspective of QoL protection, offering a convenient tool for estimating quality-adjusted life-years.

Abstract:Objectives To investigate the health-related quality of life (HRQL) of long-term survivors of inoperable esophageal squamous cell carcinoma (ESCC) treated with definitive radiation therapy, the real-world trends in the use of advanced radiation techniques, and their impact on survival outcomes of ESCC patients. Methods In this multicenter retrospective observational study, the medical records related to demographics and treatment of ESCC patients who were treated with definitive radiation therapy at 14 provincial hospitals in China from 1 January 2015 to 31 December 2016 were analyzed. A HRQL questionnaire was completed by survivors and collected by doctors at the final follow-up. The difference in quality of life between patients with or without recurrence was compared using the Wilcoxon-Mann-Whitney test. Overall survival (OS) was estimated using the Kaplan-Meier method and the group differences were assessed by unstratified log-rank test. The Cox proportional hazards model with Efron's method of tie handling was used to calculate the risk factors for OS. Results The data of a total of 3,308 patients were collected for this study, 248 were excluded because of missing data, and a final of 3,060 patients were included in the analysis. Most patients (2,901, 94.8%) received intensity-modulated radiotherapy (IMRT)/volumetric-modulated arc therapy (VMAT)/tomotherapy (TOMO). The 5-year OS rate was 30%. Patients who received either two-dimensional radiotherapy (2DRT; HR, 2.43; 95% CI: 1.70-3.47; P < 0.001) or three-dimensional radiotherapy (3DRT; HR, 1.45 95%, CI: 1.14-1.84, P = 0.003) had a significantly increased risk of death compared to those who received IMRT/VMAT/TOMO. Of the 716 (23.4%) long-term survivors who completed the HRQL questionnaire, nearly 70% patients were still able to swallow normally or almost normally, and > 80% patients did not experience weight loss. Nearly 80% patients were found life very enjoyable or were fairly enjoying life. Conclusions This large, multicenter retrospective study of ESCC patients who received definitive radiation therapy found that most ESCC survivors are satisfied with their quality of life. Most patients received advanced radiation technology. Patients who received either 2DRT or 3DRT had a significantly increased risk of death compared to those who received advanced radiation technology.

Abstract:Esophageal squamous cell carcinoma (ESCC) is the most common pathological type of esophageal cancer in China. Patients with ESCC have poor long-term survival, especially those with lymphatic metastasis (pN + ESCC). In this retrospective study, we evaluated the correlates of long-term survival time of patients with pN + ESCC. A total of 453 patients with pN + ESCC who underwent surgical R0 resection between Jan 2008 and Sep 2011 were enrolled. The follow-up ended on December 2019. The clinical, pathological, inflammation-related factors and general survival data of these patients were analyzed using SPSS 22.0 software. The 1-, 3-, and 5-year overall survival (OS) rates were 73.7%, 34.6%, and 25.6%, respectively; the 1-, 3-, and 5-year disease-free survival (DFS) rates were 45.0%, 26.3%, and 20.4%, respectively. The median OS and DFS were 23 and 14 months, respectively. On multivariate analyses, gender, site of lesion, number of dissected lymph nodes, stage pTNM, adjuvant therapy, and neutrophil lymphocyte ratio were independent predictors of OS. Site of lesion, stage pTNM, and adjuvant therapy were independent predictors of DFS. Recursive partitioning analysis (RPA) scores of each patient were calculated based on the independent predictors of OS, and the patients were divided into 3 classes: low-risk, medium-risk, and high-risk. The OS, DFS, and local recurrence-free survival were significantly different among these three RPA classes P < 0.001 . Several factors showed an independent association with long-term postoperative survival of pN + ESCC patients after radical surgery. RPA scores can potentially be used to predict the prognosis of ESCC.

Quality of Life in Patients with Liver Cancer after Operation:A 2-Year Follow-Up Study

Abstract:BACKGROUND:Quality of life (QL) is a concept which reflects the physical, social, and emotional attitudes and behaviours of an individual. QL assessment is becoming increasingly recognised as an outcome and predictor for cancer patients. Although hepatectomy has been widely accepted as treatment of choice to offer a chance of cure for patients with liver cancer, little is known about the subjective clinical results after this operation. This prospective study was designed to evaluate the Pre- and postoperative quality of life in patients with liver cancer. METHODS:The quality of life of 36 consecutive patients was measured using gastrointestinal quality of life index (GQLI) regularly 2 years after the operation, starting with a preoperative measurement. RESULTS:The score of mean preoperative GQLI was 106+/-13 points, and it was reduced significantly 2-10 weeks after the operation (86-98) (P<0.05-0.001). The quality of life recovered gradually. The GQLI score was 101+/-21 points 4 months after operation and increased to the preoperative level (P>0.05). In the patients who survived more than 9 months, the GQLI score was higher than that before the operation. Major hepatectomy (lobectomy and combined segmentectomy) reduced the GQLI score more evidently than did minor hepatectomy (simple segmentectomy) in 2-5 weeks after the operation (P<0.05). The age and preoperative liver function of the patients played an important role in the recovery of the quality of life in the early postoperative stage (P<0.05). The patients with tumor recurrence showed a continuous decrease of the quality of life (P<0.05-0.001). CONCLUSIONS:The assessment of the quality of life is meaningful for patients with liver cancer. Tumor recurrence, poor liver function and major operation are the most important factors for reducing the quality of life. Hepatic resection is justified by its effects on the survival and the quality of life of the patients.

Abstract:This systematic review and meta-analysis investigated the impact of quality of life (QoL) on mortality risk in patients with esophageal cancer. A literature search was conducted using the CINAHL, PubMed/MEDLINE, and Scopus databases for articles published from inception to December 2022. Observational studies that examined the association between QoL and mortality risk in patients with esophageal cancer were included. Subgroup analyses were performed for time points of QoL assessment and for types of treatment. Seven studies were included in the final analysis. Overall, global QoL was significantly associated with mortality risk (hazard ratio 1.02, 95% confidence interval 1.01–1.04; p < 0.00004). Among the QoL subscales of QoL, physical, emotional, role, cognitive, and social QoL were significantly associated with mortality risk. A subgroup analysis by timepoints of QoL assessment demonstrated that pre- and posttreatment global and physical, pretreatment role, and posttreatment cognitive QoL were significantly associated with mortality risk. Moreover, another subgroup analysis by types of treatment demonstrated that the role QoL in patients with surgery, and the global, physical, role, and social QoL in those with other treatments were significantly associated with mortality risk. These findings indicate that the assessment of QoL in patients with esophageal cancer before and after treatment not only provides information on patients' condition at the time of treatment but may also serve as an outcome for predicting life expectancy. Therefore, it is important to conduct regular QoL assessments and take a proactive approach to improve QoL based on the results of these assessments.

Abstract:BackgroundWe aimed to obtain a set of health state utility scores of patients with esophageal cancer (EC) and precancerous lesions in China, and to explore the influencing factors of health‐related quality of life (HRQoL).MethodsA hospital‐based multicenter cross‐sectional study was conducted. From 2013 to 2014, patients with EC or precancerous lesions were enrolled. HRQoL was assessed using a European quality of life‐5 dimension (EQ‐5D‐3L) instrument. Multivariable linear regression analysis was performed to explore the influencing factors of the EQ‐5D utility scores.ResultsA total of 2090 EC patients and 156 precancer patients were included in the study. The dimension of pain/discomfort had the highest rate of self‐reported problems, 60.5% in EC and 51.3% in precancer patients. The mean visual analog scale (VAS) score for EC and precancer patients were 68.4 ± 0.7 and 64.5 ± 3.1, respectively. The EQ‐5D utility scores for EC and precancer patients were estimated as 0.748 ± 0.009 and 0.852 ± 0.022, and the scores of EC at stage I, stage II, stage III, and stage IV were 0.693 ± 0.031, 0.747 ± 0.014, 0.762 ± 0.015, and 0.750 ± 0.023, respectively. According to the multivariable analyses, the factors of region, occupation, household income in 2012, health care insurance type, pathological type, type of therapy, and time points of the survey were statistically associated with the EQ‐5D utility scores of EC patients.ConclusionsThere were remarkable decrements of utility scores among esophageal cancer patients, compared with precancer patients. The specific utility scores of EC would support further cost‐utility analysis in populations in China.

Abstract:e15519 Background: The Lymph node metastasis (LNM) trends toward inferior outcomes for the esophageal cancer (EC) patients after radical operation. However, it is not well known for status to metastasis of regional lymph node (RLN) and impacts of RLN metastasis on long-term survival following esophagectomy, especially in squamous cell carcinoma. Methods: Data was extracted from the Collaborative Prospective Esophageal Cancer Database (designed by LinkDoc Technology Co, Ltd.) for patients with stage T1-3 thoracic tumors who underwent radical esophagectomy between 1 January 2010 and 30 June 2017 across ten hospitals with high volume of surgeries in China. The rate of LNM in every lymph node station was defined as the number of patients with LNM in the station divided by the number of patients with examination of lymph node (ELN) in the station. The hazard ratios (RLN metastasis versus no RLN metastasis) for postoperative survival were analyzed by method of multivariate cox’s proportional hazard models, and confounders such as age, sex, histological grade, stage T, tumor size and neoadjuvant therapy were adjusted. Results: Total 6485 patients (mean age: 61.80±8.13) were included, composing of 75.2% male and 24.8% female. For tumor types, there were 95.5% squamous cell carcinoma, 2% adenocarcinoma and 2.5% others. The rates of RLN metastasis and hazard ratios are in the table. Conclusions: Besides lymph node station 8, LNM was commonly found at station 1,2,4,16,17 and 20 in thoracic stage T1-3 EC. The RLN metastasis predicted inferior postoperative long-term survival. Abstract:Objective To analyze the long-term survival rates and influencing factors of surgical treatment of esophageal squamous cell carcinoma. Methods All patients with esophageal squamous cell carcinoma who underwent surgical treatment at Shanghai Chest Hospital from 2012 t0 2014 were reviewed. 864 patients undergoing transthoracic surgery were selected as the study subjects. Their general conditions, surgical approaches, postoperative complications, and long-term survival outcomes wcre analyzcd.Results Of all the patients, 84.7% wcre mcn, and 44.9% and 30.4% were cll and clll patients, respectively. Neoadjuvant therapy was used in 8. 3% of patients. The right thoracic approach was used in 88.7% of patients. The proportion of minimally invasive surgery was 23.6%. The overall complication rate was 41.8%, and the 90-day mortality rate was 3. 1%. The recurrence rate was 38.1%. The l-year, 3-year, and 5-year overall survival rates were 85. 5%, 61. 2%, and 49. 3%,rcspectivcly. Minimally invasive surgery improved long-term survival ( MIE υs OPEN 66. 5% υs 44 . 0% , P<0.001). Adjuvant therapy was shown to improve overall survival in both lymph node positive and negative T3N0 patients ( 33. 3% υs 31. 5%, P= 0. 001 ; 66. 0% υs 49. 5% , P= 0. 004) . Conclusions Surgical resection combined with postoperative adjuvant therapy can achieve satisfactory long-term survival of esophageal squamous cell carcinoma. The role of neoadjuvant therapy requires further study.

Abstract:Background The aim of this study was to compare the long-term quality of life (QoL) in patients after Sweet, Ivor-Lewis, or Mckeown esophagectomy. Methods Esophageal cancer patients after Sweet, Ivor-Lewis, or Mckeown esophagectomy from 2010 to 2012 were included. QoL was assessed according to the European Organization for Research and Treatment of Cancer general questionnaire: QLQ-C30 and esophagus-specific questionnaire: QLQ-OES18. Results A total of 126 qualified patients who have been alive for more than 3 years without tumor recurrence were divided into three groups: the Sweet group ( n = 40), Ivor-Lewis group ( n = 38), and Mckeown group ( n = 48). Among these three groups, the QLQ-C30 mean scores of global health status, functional and symptom scales, and general QoL were similar. The symptom scales of QLQ-OSE18 showed that patients who had a Mckeown operation experienced more problem of eating ( P = 0.029), choking when swallowing ( P = 0.010) and coughing ( P = 0.016), while patients undergoing Sweet operation complained more symptom of reflux ( P = 0.003) and pain ( P = 0.000). Conclusions All three types of esophagectomy provided a generally good long-term QoL. However, patients in Sweet and Mckeown group tend to suffer from a higher symptomatic burden as compared to Ivor-Lewis approach.
Surgical Quality Determines the Long-Term Survival Superiority of Right over Left Thoracic Esophagectomy for Localized Esophageal Squamous Cell Carcinoma Patients: a Real-World Multicenter Study.

Abstract:Objective: The objective was to compare the long-term overall survival (OS) of right versus left thoracic esophagectomy, and to evaluate whether surgical quality impacts comparison result. Background: Controversy regarding the optimal thoracic esophagectomy approach persists for esophageal squamous cell carcinoma (ESCC). No study has assessed the effect of surgical quality in comparison between right and left approaches. Methods: The authors consecutively recruited 5556 operable ESCC patients from two high-volume centers in China, of whom 2220 and 3336 received right and left thoracic esophagectomy, respectively. Cumulative sum was used to evaluate the learning curve for operation time of right approach, as the indicator of surgical proficiency. Results: With a median follow-up of 83.1 months, right approach, harvesting more lymph nodes, tended to have a better OS than left approach (Mean: 23.8 vs. 16.7 nodes; adjusted hazard ratio (HR)=0.93, 95% CI: 0.85–1.02). Subset analysis by the extent of lymphadenectomy demonstrated that right approach with adequate lymphadenectomy (≥15 nodes) resulted in statistically significant OS benefit compared with left approach (adjusted HR=0.86, 95% CI: 0.77–0.95), but not with limited lymphadenectomy. Subset analysis by surgical proficiency showed that proficient right approach conferred a better OS than left approach (adjusted HR=0.75, 95% CI: 0.64–0.88), but improficient right approach did not have such survival advantage. Conclusions: Surgical quality plays a crucial role in survival comparison between surgical procedures. Right thoracic esophagectomy performed with adequate lymphadenectomy and surgical proficiency, conferring more favorable survival than left approach, should be recommended as the preferred surgical procedure for localized ESCC.
Analysis of short-and long-term outcomes after esophagectomy in elderly cancer pa-tients

Abstract:Objective:To analyze the short-and long-term outcomes of elderly patients after esophagectomy by comparing preopera-tive comorbidities, postoperative complications, and survival rates among different age groups. Methods:We retrospectively reviewed the data of 253 patients who underwent esophagectomy from January to December 2010 in The First Affiliated Hospital of University of Science and Technology of China. Eighty-eight (34.8％) patients were aged<60 years (group A), 145 (57.3％) were aged 60-75 years (group B), and 20 (7.9％) were aged≥75 years (group C). The short-and long-term outcomes of the patients were analyzed. Results:There were significant differences in three group of patients with hypertension before operation (P<0.05). There were statistically sig-nificant differences among all complications, major complications, pulmonary infection, arrhythmia, and respiratory insufficiency (P<0.05), except for anastomotic leakage and other complications (P>0.05). The mean follow-up was 50.7 months. The median overall sur-vival and 1-, 3-, and 5-year overall survival rates were 68 months and 98.9％, 86.8％, and 69％, respectively, for group A;61 months and 93.1％, 76.1％, and 51％, respectively, for group B;and 32 months and 63.3％, 46％, and 28.8％, respectively, for group C (P<0.001). The median progression-free survival (PFS) and 1-, 3-, and 5-year PFS rates were 60 months and 98.86％, 85.2％, and 45.5％, respectively, for group A;43 months and 87.6％, 53.1％, and 26.9％, respectively, for group B;and 11 months and 30％, 20％, and 10％, respectively, for group C (P<0.001). The differences in survival rates between groups A and B, A and C, and B and C were statistically significant (P<0.001). The multivariate analysis showed that age and TNM stage IV were independent risk factors for overall survival and PFS (P<0.05). Conclusions:The long-term survival rate decreases significantly in elderly patients with esophageal cancer. In addition, age and advanced pathological stage of tumor are independent risk factors for long-term outcomes.
Frailty and health-related quality of life in elderly patients undergoing esophageal cancer surgery: a longitudinal study

Abstract:Purpose This study aims to elucidate the longitudinal alterations in frailty and health-related quality of life experienced by elderly patients undergoing surgical treatment for esophageal cancer. Additionally, it seeks to ascertain the impact of preoperative frailty on postoperative health-related quality of life over time. Methods 131 patients were included in the prospective study. Patients' frailty and health-related quality of life were assessed utilizing the Tilburg and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (EORTC-QLQ-C30) at preoperative, 1 week, 1-month, and 3 months postoperatively. Statistical analyses were performed using generalized estimating equations (GEE), repeated-measures analysis of variance (ANOVA), and linear mixed models (LMMs). Results Out of 131 patients, 28.2% had frailty before surgery, and the prevalence of frailty consistently higher after surgery compared to baseline (67.9%, 51.9%, 39.7%). There was no significant change in frailty scores in preoperative frail patients within 3 months following surgery ( P = .496, P < .999, P < .999); whereas in preoperative non-frail patients, the frailty scores increased at 1 week ( P < .001) and then decreased at 1-month ( P = .014), followed by no change at 3 months. In addition, preoperative frail patients had significantly worse global quality of life (QL) (β=-4.24(-8.31; -.18), P = .041), physical functioning (β=-9.87(-14.59; -5.16), P < .001), role functioning (β=-10.04(-15.76; -4.33), P = .001) and social functioning (β=-8.58(-15.49; -1.68), P = .015), compared to non-frail patients. Conclusions A significant proportion of participants exhibited a high prevalence of preoperative frailty. These patients, who were preoperatively frail, exhibited a marked reduction in health-related quality of life, a more gradual recovery across various functional domains, and an increased symptom burden during the follow-up period. Therefore, it is crucial to meticulously identify and closely monitor patients with preoperative frailty for any changes in their postoperative physiology, role, and social functioning.

Abstract:Objective: For patients undergoing oncologic surgery, the quality of life (QoL) is generally accepted as an important outcome parameter in addition to long-term survival, mortality and complication rates. This study focussed on the QoL in patients after oesophagectomy for cancer, comparing the method of reconstruction (narrow gastric tube vs whole stomach). Methods: In a prospective randomised single-centre study from 2007 to 2008, 104 patients underwent oesophagectomy for cancer. To assess the QoL, a questionnaire in reference to the EORTC-QLQ-C30 and the QLQ-OES24 was administered at 3 weeks, 6 months and 1 year after surgery. Clinical data were collected prospectively, and follow-up was performed regularly. Results: There were no significant differences between the narrow gastric tube group (NGT group, n = 52) and the whole-stomach group (WS group, n = 52) with regard to patient and cancer characteristics, operative procedure, postoperative intensive care unit (ICU) hospitalisation, and overall survival at 1 year. Regarding the postoperative complication, there were more cases of postoperative reflux oesophagitis and impairment of pulmonary function in the WS group (P < 0.05). Regarding the QoL investigation, the scores of QoL dropped for all patients at 3 weeks after surgery. Slowly, recovery was found at both 6 months and 1 year in both groups. Patients in the NGT group reported significantly (P < 0.05) better scores of QoL at both 6 months and 1 year. Conclusions: Patients who underwent gastric tube reconstruction develop less postoperative digestive tract complications, and have a quicker recovery and a better QoL during the follow-up period. Further investigation and data collection will allow the assessment of this procedure beyond 1 year after operation. (C) 2010 European Association for Cardio-Thoracic Surgery. Abstract:Objective To evaluate long-term quality of life of patients treated with accelerated hyperfractionated radiotherapy for esophageal carcinoma.Methods SOMA scale,Symptom Checklist 90(SCL 90) and Life Satisfaction Index A (LSIA) questionnaire were mailed to the long term survivors treated with accelerated hyperfractionated radiotherapy for esophageal carcinoma for assessment of long term quality of life including physical symptoms,psychological status and life satisfactory evaluation. Control group was also used.Results There was no significant difference between accelerated hyperfractionated radiotherapy group and control group in terms of the scoring of quality of life such as SCL 90 and LSI A. While in the treatment group,there was also no significant difference between continuous accelerated hyperfractionated(CAHF) and late course accelerated hyperfractionated (LCAF)subgroups according to the scoring of quality of life such as late radiation reaction,SCL-90 and LSI-A.Conclusions (1)Preliminary results demonstrated that long-term survivors treated with accelerated hyperfractionated radiotherapy for esophageal carcinoma retain good quality of life including physical symptoms,psychological status and life satisfactory evaluation.(2)Preliminary results demonstrated that there is no significant difference in long term quality of life between the two groups.(3)Methods of evaluation for long term quality of life for patients treated with esophageal carcinoma needs further investigation.Involvement of more patients and establishment of control group are also worthwhile.

Abstract:Objectives: This study assessed functional outcomes and quality of life (QoL) in the long term in individuals treated for laryngohypopharyngeal cancer (LHC) by estimating their life expectancy (LE), survival-weighted psychometric scores (SWPSs), and quality-adjusted LE (QALE). Materials and methods: To estimate survival outcomes, we retrospectively reviewed the data of 1576 patients treated for primary LHC between January 2010 and December 2018 and followed them until death or December 2020. We also prospectively collected QoL and functional data between October 2013 and November 2022 from 232 patients by administering the Taiwanese Chinese versions of the QoL Questionnaire Core 30, Head and Neck 35, and EQ-5D-3L. To estimate LE, we employed linear extrapolation of a logit-transformed curve. We calculated QALE and SWPSs by combining the QoL data with the LE results. Results: We estimated the LE of the patients with LHC to be 7.8 years and their loss of LE to be 15.7 years. The estimated QALE was 7.0 QALYs, with a loss of QALE of 16.5 QALYs. Lifetime impairment durations were estimated for cognitive (4.9 years), physical (4.2 years), emotional (3.4 years), social (3.4 years), and role functions (2.7 years). We estimated the durations of problems related to swallowing, speech, and teeth to be 6.2, 5.6, and 4.8 years, respectively. The patients were expected to be dependent on feeding tubes for 1.2 years. Conclusions: Patients with LHC experience significant reductions in both LE and QALE. SWPSs may constitute a valuable tool for obtaining subjective information regarding how LHC affects multifaceted QoL outcomes.

Abstract:BACKGROUND:To investigate the impact of prolonged length of stay (LoS) on long-term mortality in patients who have undergone curative resection for esophageal cancer (EC).METHODS:Between January 2001 and December 2009, patients who underwent an esophagectomy for EC at Fudan University Shanghai Cancer Center were enrolled in this study. We retrospectively analyzed the medical charts of all of the enrolled patients. To determine the effect of postoperative LoS on long-term survival, we separated the patients into three groups based on the lengths of their postoperative LoS, including an LoS of less than 2 weeks (Group 1, ≤2 W), an LoS between 2 and 3 weeks (Group 2, ≤3 W) and an LoS of more than 3 weeks (Group 3, >3 W). Perioperative and long-term outcomes were compared between the groups.RESULTS:In total, 348 patients were included in this study. All of the patients underwent an esophagectomy with 3-field lymph node dissection (3FLND). The median postoperative hospital stay was 14 days (range: 8-153 days). Complications were observed in 123 patients (15.9% in Group 1 vs. 73.2% in Group 2 vs. 96.6% in Group 3, P<0.001). The median duration of follow-up was 39 months (range: 3-120 months). There were significant reductions in preventive adjuvant therapy (P=0.003) and postoperative salvage therapy (P<0.001) among the three groups. The 5-year survival rate was significantly different among the groups (43% vs. 36% vs. 29%, respectively, P=0.006). There was no difference in the 5-year disease-free survival rate among the three groups (23% vs. 21% vs. 19%, P=0.238).CONCLUSIONS:Prolonged LoS was significantly associated with reduced rates of overall survival (OS). The insufficient administration of adjuvant therapy may partly account for these findings.
