Abstract:Repeated single-point measurements of thoracic bioimpedance at a single (low) frequency are strongly related to fluid changes during hemodialysis. Extension to semi-continuous measurements may provide longitudinal details in the time pattern of the bioimpedance signal, and multi-frequency measurements may add in-depth information on the distribution between intra- and extracellular fluid. This study aimed to investigate the feasibility of semi-continuous multi-frequency thoracic bioimpedance measurements by a wearable device in hemodialysis patients. Therefore, thoracic bioimpedance was recorded semi-continuously (i.e., every ten minutes) at nine frequencies (8–160 kHz) in 68 patients during two consecutive hemodialysis sessions, complemented by a single-point measurement at home in-between both sessions. On average, the resistance signals increased during both hemodialysis sessions and decreased during the interdialytic interval. The increase during dialysis was larger at 8 kHz (∆ 32.6 Ω during session 1 and ∆ 10 Ω during session 2), compared to 160 kHz (∆ 29.5 Ω during session 1 and ∆ 5.1 Ω during session 2). Whereas the resistance at 8 kHz showed a linear time pattern, the evolution of the resistance at 160 kHz was significantly different (p < 0.0001). Measuring bioimpedance semi-continuously and with a multi-frequency current is a major step forward in the understanding of fluid dynamics in hemodialysis patients. This study paves the road towards remote fluid monitoring.

Frequency of arrhythmia symptoms and acceptability of implantable cardiac monitors in Hemodialysis patients

Abstract:BackgroundArrhythmia-related complications and sudden death are common in dialysis patients. However, routine cardiac monitoring has so far not been feasible. Miniaturization of implantable cardiac monitors offers a new paradigm for detection and management of arrhythmias in dialysis patients. The goal of our study was to determine the frequency of arrhythmia-related symptoms in hemodialysis patients and to assess their willingness to undergo implantation of a cardiac monitor.MethodsWe conducted a survey of in-center hemodialysis patients at a hemodialysis clinic in Baltimore, Maryland. We assessed the frequency of arrhythmia-related symptoms and willingness to undergo placement of an implantable cardiac monitor (LINQ, Medtronic Inc.).ResultsForty six patients completed the survey. The mean age of the survey respondents was 59 years and 65% were male. Symptoms were common with 74% (n = 34) of participants reporting at least one arrhythmia-related symptom and many [22% (n = 10)] had all 3 symptoms. Among the patients with symptoms, 57% (n = 26) reported “heart skipping beats, flopping in chest or beating very hard,” 61% (n = 28) reported “heart racing (palpitations),” and 37% (n = 17) reported feeling that they “passed out or almost passed out.” The majority of the patients felt that the timing of the symptoms was unrelated to dialysis treatments. The acceptability of the monitoring device implantation was high, with 59% (n = 20) of patients with symptoms and 50% (n = 6) of patients without symptoms willing to consider it. The main reason for not considering the device was not wanting to have an implanted device.ConclusionThe prevalence of arrhythmia-related symptoms is high in hemodialysis patients and the majority would consider an implantable cardiac monitor if recommended by their physicians. Routine implantation of cardiac monitoring devices to manage arrhythmias in dialysis patients may be feasible and will provide further insights on the leading causes of morbidity and mortality in dialysis patients.

Abstract:The International Journal of Artificial Organs, Ahead of Print. Introduction:Intra-dialytic hypotension (IDH) remains the commonest problem associated with routine haemodialysis treatments. Fluid shifts from intracellular(ICW) and extracellular(ECW) compartments to refill plasma volume during haemodialysis with ultrafiltration.Methods:We studied the effect of relative changes in ICW and ECW indifferent body segments using multifrequency segmental bioimpedance during haemodialysis and IDH episodes.Results:Of 42 haemodialysis patients,16 patients (38.1%) developed IDH within the first hour of dialysis. Patients with and without early IDH were well-matched for demographics and starting bioimpedance measurements. However, after 60 min, the relative change in in ECW/ICW ratio between the non-fistula arm and leg was significantly different for the early IDH group median −1.07 (−3.33 to 0.8) versus 0.61 (−0.78 to 1.8), p < 0.05, whereas there no differences in ultrafiltration rate, relative blood volume monitoring or on-line clearance.Conclusion:Monitoring serial changes in fluid status in different body compartments with bioimpedance may potentially prevent IDH in the future.

Abstract:Objective:To prevent hemodialysis associated complications such as symptomatic hypotension and dialysis-refractory hypertension through the adjustment of dry weight,with bioelectric impedance(BEI) technique which could monitor the changes of extracellular volume during hemodialysis.Methods:One hundred maintenance hemodialysis patients with an average dialysis course of 58.67±31.54 months were enrolled. The patients were divided into 3 groups:normal blood pressure group(n=34),symptomatic hypotension group (n=22),and refractory hypertension group (n=44). Using the BEI technique,we measured the changes of ECV% and ICV% and ECV/ICV,and analyzed its influence on heart rate,respiration rate,blood pressure,ultrafiltration volume and ultrafiltration rate.Results:The ultrafiltration rate decreased gradually with the increase of removed fluid by ultrafiltration.Symptomatic hypotension group had the highest ultrafiltration volume and the highest ultrafiltration rate,while the lowest ultrafiltration volume and ultrafiltration rate occurred in the refractory hypertension group. The pre-hemodialysis ECV% and ECV/ICV in three groups were higher than those of normal control group,the refractory hypertension group has the highest values (P<0.05). The post-hemodialysis ECV% and ECV/ICV were significantly lower than their pre-hemodialysis values,but the refractory hypertension group still has a higher values compared with the normal control group (P<0.05). ICV% increased significantly after hemodialysis therapy,but still lower than that in normal control group. In symptomatic hypotension group,with the increase of extracellular fluid removal by hemodialysis ultrafiltration,MAP decreased gradually. A significant positive correlation was observed between ECV and MAP(r=0.914,P<0.001).As for the refractory hypertension group,with the increase of extracellular fluid removal,MAP increased gradually. A significant negative correlation was observed between ECV and MAP(r=-0.782,P=0.035).After up-regulation or down-regulation of the dry body weight,5 patients with symptomatic hypotension and 7 patients with refractory hypertension experienced effective blood pressure control respectively.Conclusion:Extracellular volume monitor techniques such as BEI provides a way to evaluate patient's dry body weight,which turned out to optimize the volume control and blood pressure control.
Magnitude and precision of absolute blood volume estimated during hemodialysis

Abstract:Background: Management of body fluid volumes and adequate prescription of ultrafiltration (UF) remain key issues in the treatment of chronic kidney disease patients.Objective: This study aims to estimate the magnitude as well as the precision of absolute blood volume (Vb) modeled during regular hemodialysis (HD) using standard data available with modern dialysis machines.Methods: The estimation utilizes a two-compartment fluid model and a mathematical optimization technique to predict UF-induced changes in hematocrit measured by available on-line techniques. The method does not rely on a specific hematocrit sensor or a specific UF or volume infusion protocol and uses modeling and prediction tools to quantify the error in Vb estimation.Results: The method was applied to 21 treatments (pre-UF body mass: 65.57±13.44 kg, UF-volume: 3.99±1.14 L) obtained in ten patients (4 female). Pre-HD Vb was 5.4±0.53 L with an average coefficient of variation of 9.8% (range 1 to 22%). A significant moderate correlation was obtained when Vb was compared to a different method applied to the same data set (r = 0.5). Specific blood volumes remained above the critical level of 65 mL/kg in 17 treatments (80.9%).Conclusion: The method offers the opportunity to detect critical blood volumes during HD and to judge the quality and reliability of that information based on the precision of the Vb estimate.
Comparison of Bioimpedance Methods for Estimating Total Body Water and Intracellular Water Changes During Hemodialysis

Abstract:Background. The accurate assessment of body fluid volume is important in many clinical situations. Hannan et al. proposed a single-frequency bioimpedance equation (HE) to calculate extracellular water (ECW) and total body water (TBW). There are two equations based on the bioimpedance spectroscopy (BIS) method for the evaluation of body fluid volume: Xitron equations (XE) and body composition spectroscopy equations (BCSE). The aim of the study was to compare the accuracy of these three equations in body fluid volume point estimation in maintenance hemodialysis (MHD) patients.Methods. The BIS method was performed in MHD patients before and after a hemodialysis (HD) session. TBW, ECW and intracellular water (ICW) were calculated by XE, BCSE and HE, respectively. Hydration status (HS) was calculated using inputs of XE, BCSE and HE. ICW before dialysis was compared to ICW after dialysis. The change of TBW and HS using different equations was compared to actual ultrafiltration volume (AUV) that was calculated as weight difference of pre- to postdialysis.Results. Fifty MHD patients (27 females) were included in the study. Significant changes in ICW were observed using the XE and HE method with ultrafiltration (XE: 15.51 +/- 5.07 versus 16.17 +/- 5.34 L, P < 0.01; HE: 17.40 +/- 5.13 versus 16.55 +/- 4.71 L, P < 0.01). However, no significant ICW change was observed using BCSE (17.47 +/- 4.35 versus 17.54 +/- 4.36 L, P > 0.05). (sic)TBW_XE and (sic)TBW_HE were significantly different from AUV (XE 1.76 +/- 0.89 versus 2.46 +/- 0.89 L, P < 0.01; HE 4.16 +/- 1.36 versus 2.46 +/- 0.89 L, P < 0.01); however, (sic)TBW_BCSE was much closer to AUV (2.27 +/- 0.90 versus 2.46 +/- 0.89 L, P = 0.129). The change of HS using inputs of BCSE was also closer to AUV (2.41 +/- 0.86 versus 2.46 +/- 0.89 L, P = 1.0).Conclusion. Our study indicated that BCSE provided a better point estimation of ICW and TBW.

Abstract:Fifty-seven teachers of nursing, who comprised 42 women and 15 men and who also comprised 12 senior tutors, 16 nurse tutors and 29 clinical teachers, participated in a study to evaluate a workshop which they attended. The method of inquiry used was that of a survey by questionnaire. Two questionnaires were used, one being set at the beginning of the workshop and the other at the end of the proceedings. The learning objectives for the workshop were used as the questionnaire items and comprised 20 in all. When the before and after scores relating to the questionnaire items were compared through the use of t tests significant differences were found on 12 out of the 20 objectives. In the course of the paper the differences found are identified and discussed. Some of the limitations of the method of inquiry used are discussed and finally some conclusions are reached.
Real-Time Dialysis Dose: Ionic Dialysis Versus Classical Urea Kinetic Modeling Indices

Abstract:Introduction Worldwide, over 850 million people need renal replacement therapy, and over 80% of them are on hemodialysis. The term "dialysis adequacy" is most often associated with the achievement of minimally acceptable indices - single pool Kt/V (spKt/V) and urea reduction ratio (URR%) and largely does not take into account other clinical indicators in patients. In addition, it should be taken into account that in the conditions of clinical practice, their measurement is carried out according to standards controlled by the regulatory structures and is carried out with a frequency between one and three months, and not during each dialysis procedure. Accordingly, based on the obtained value, we assume that it refers to the urea clearance of the entire dialysis prescription. A new methodology for measuring dialysis adequacy through ionic dialysis allows dose estimation in real time without the need for additional blood tests by recording the difference in sodium ion conductivity at the dialyzer inlet and outlet. The latter corresponds to the dialysis dose delivered and can be measured at each dialysis session, enabling timely therapeutic intervention at minimal cost to the community. The aim of this original article is to present ion dialysis to the community as a reliable and inexpensive alternative to classical urea kinetic modeling (UKM), evaluating the comparability of the two methods and the possible sources of error in the result. Material and methods This was a retrospective study of 32 patients undergoing hemodialysis at the Clinic of Nephrology and Dialysis at the University Hospital St. Marina (Varna, Bulgaria) for the period between January and December 2020. For each of the patients, four measurements (every three months) of the delivered dialysis dose were performed by ion dialysis and classical urea kinetic modeling in order to assess the comparability and reliability of the two methods. Results The analysis of the results proved a high correlation between the validated indicators of dialysis adequacy (spKt/V; URR) and those registered with online monitoring (online clearance monitor) by ionic dialysis - online Kt/V (onKt/V) while at the same time reporting a significant difference in the two methods - primarily based on the anthropometric formulas used to estimate the volume. The established regression models confirmed the high predictive value of ionic dialysis in relation to the actually delivered dose. Discussion Despite the high bond strength, our recorded values for onKt/V are 8% lower compared to results using UKM. Therefore, onKt/V as an assessment metric has the ability to underestimate the dialysis dose received. Various factors as a possible source of error and its cause have been reported by some authors. Several studies have reported differences of 2-5% in instantaneous conductance measurements, which they associate mainly with differences in the diffusion coefficients of urea and sodium, as well as different effects of dialysis membrane charge or inadequate ultrafiltration correction. Conclusions Our study confirms that online clearance monitoring (OCM) is a practical non-invasive tool for daily use that complements the classic performance of OCM by helping to deliver an adequate dialysis dose with increased patient benefit and minimal cost of financial resources.

Abstract:Visual abstract presenting the methodology and summary of the results. Background Ultrafiltration (UF) is an essential process of restoring fluid homeostasis during hemodialysis (HD). Fluid shifts across the extracellular compartments during UF, predominantly across the capillary interface and between the macro‐ and microcirculation. A mismatch between UF and transcapillary fluid transport can lead to hemodynamic instability leading to cardiac morbidity. We wished to study intradialytic fluid transport characteristics and their variation during UF to identify factors that govern variability in transcapillary fluid movement in HD. Methods Twenty‐two patients undergoing stable HD sessions were studied to measure and monitor absolute blood and plasma volume throughout UF. A computational mathematical model of predicted plasma volume decay during UF was analyzed with respect to the intradialytic real‐time data profile. Pre‐ and post‐dialysis fluid status was assessed using multifrequency bioimpedance spectroscopy. Serum electrolytes, osmolality, and total protein concentration were measured pre‐ and post‐dialysis and during the intradialytic phase. Results Two distinct profiles of PV responses were detected. 60% of the patients presented plasma volume decline, characterized by a high percentage of volume decrease during the first hour, and a subsequent slower decrease with early rebound. The model was modified to achieve a proper fit of these volume profiles, assuming time‐dependent changes in selected parameters governing the refilling flow. Conclusions Although the modified model could more accurately fit the data, the new parameter values often fell outside of a physiologically acceptable range, suggesting that other factors not included in the classic description of transcapillary fluid transport might be the cause of the observed patterns.

Volume Control in Peritoneal Dialysis Patients Guided by Bioimpedance Spectroscopy Assessment

Abstract:Background: Volume overload is a common problem in peritoneal dialysis patients and may contribute to the high cardiac mortality in this patient population. Controlling volume status is difficult due to the lack of appropriate and accurate assessment of dry weight. This randomized, controlled study was conducted to test if the recent use of overhydration value (OH) provided by bioimpedance spectroscopy and patients’ education would help to control overhydration. Methods: 160 continuous ambulatory peritoneal patients were included in this study. All the patients were randomly allocated to 2 groups: in Group 1 the patients and their primary nurses were informed of the OH values provided by bioimpedance spectroscopy whereas in Group 2 the values were not revealed and patients’ volume was measured by the standard methods. Fluid status was evaluated by means of repeated bioimpedance analysis and clinical assessment. Urine volume, blood pressure and use of antihypertensive medications were recorded. Results: There were no differences in gender, age, diabetes, height, weight and clinical hydration status between the 2 groups at the baseline. In Group 1, OH (p < 0.05), extracellular volume and the extracellular volume to intracellular volume ratio decreased steadily during the 3-month follow-up. On the contrary, all 3 parameters increased significantly in Group 2. SBP decreased significantly in Group 1 but increased significantly in Group 2. Conclusion: Our study shows that the use of OH as determined by bioimpedance spectroscopy may facilitate volume control in peritoneal dialysis patients.
Development and validation of a new dry weight estimation method using single frequency bioimpedance in hemodialysis patients.

Abstract:Background: We proposed a new method to estimate dry weight (DW) using single frequency bioimpedance. Methods: We hypothesized that the change in whole body resistance at 50 kHz (R(50)) was proportional to the ultrafiltration volume (UFV) during a hemodialysis (HD) session. When the targeted resistance estimated in healthy subjects was reached, the patient achieved his/her DW. UFV and R(50) were monitored in 40 HD patients. Another 43 HD patients were stratified into 2 groups to validate this method. Results: The change in whole body resistance was proportional to UFV in each of the 40 HD patients. In the DW(decrease) group, pre-dialysis systolic blood pressure (n = 29, 154.5 +/- 22.8 vs. 146.9 +/- 22.3, p < 0.05) and antihypertensive medicine (4.7 +/- 3.6 vs. 3.3 +/- 2.2, p < 0.05) decreased without adverse symptoms change. In the DW(increase) group, the number of adverse symptoms in 1 week (n = 14, 26 vs. 6, p < 0.05) decreased without a change in systolic blood pressure. Age-related extracellular to total body water volume ratio (Ecv/TBW)--can it be used for "dry weight" determination in dialysis patients? Application of multifrequency bioimpedance measurement

Abstract:The article suggests a novel method for quantitative determination of optimal dry weight in dialysis patient based on their extracellular volume (ECV) to total body water (TBW) ratio and its relation to age. Values of ECV and TBW are evaluated by means of whole body multifrequency bioimpedometry. In an effort to find a suitable marker of hydration status in an individual from bioimpedance data, significant correlation has been found between ECV/TBW ratio and age in health. Assuming that all excess fluid in dialysis patients is stored exclusively in ECV and that distribution of their TBW at the state of optimal dry weight corresponds to that of a healthy person of the same age, the pre-dialysis ECV/TBW could be used for quantitative determination of optimal dry weight and/or of the ultrafiltration to reach this weight. Practical bioimpedance measurement of ECV/TBW in a group of dialysis patients both pre- and post-dialysis confirmed both above assumptions, i.e. nearly exclusively extracellular origin of ultrafiltration as well as normalisation of the ECV/TBW ratio towards the end of dialysis. Supporting evidence of increasing ECV/TBW value with age was also found in literature. Although the suggested method needs detailed analysis of possible disturbing factors (ethnic "specificity" of the reference ECV/TBW vs. age characteristics in health, possible difference in "biological" and "physical" age of dialysis patient and others), the article is published at this early stage to enable wider testing of the proposed novel method by different investigators.

Abstract:Background/Aims: Evaluating the peritoneal fluid kinetics is of clinical importance in peritoneal dialysis treatment. We have previously developed a simple way to evaluate the peritoneal fluid transport characteristics in continuous ambulatory peritoneal dialysis patients which, however, cannot be applied to those patients on a fixed dialysis schedule. Therefore, in the present study, we tested the possibility to vary the peritoneal dwell time and tried to develop a more patient-friendly ultrafiltration (UF) collection protocol. Methods: The patients' UF volume was recorded for 10 days. All patients recruited were asked to perform their usual dialysis exchanges with, however, a special UF data collection protocol to improve the accuracy of computer simulation: at least one dwell of 2-4 h, one of 4-7 h, and one of more than 7 h. The fluid transport model was applied to the pooled UF volume for fluid kinetics simulation, and the data from the following day's UF records using the same glucose concentration and dwell time were used to evaluate reliability and accuracy of the simulated UF value. Results: Fifty-two chronic peritoneal dialysis patients were included in the present study. All of the UF data could be used in the computer simulation, and there was a significant negative correlation between fluid absorption rate (K-e; see text) and actual UF volume on the following day using a glucose concentration of 1.5% and a dwell time of 4 h (r = -0.336, p < 0.05). The estimated UF values correlated significantly with the actually measured UF values. The variability of the results, expressed by the width between the 95% limits of agreement, fell within -139.2 to 131.9 ml, while the mean difference was -3.7 ml. Conclusions: Our present study showed that varying the peritoneal dwell time was a patient-friendly UF data collection protocol for continuous ambulatory peritoneal dialysis patients on a fixed dialysis schedule. Abstract:Background: Ascertaining the total body water (V), usually obtained by the Watson formula or bioimpedance spectroscopy (BIS), is crucial for the calculation of Kt/V in patients with peritoneal dialysis (PD). The aim of our study was to compare two different methods of determining V and explore which one is suitable for clinical application. Methods: This was a retrospective observational study. V was determined using the Watson formula (Vwat) and BIS (Vbis). The differences between Vbis and Vwat and between Kt/Vbis and Kt/Vwat were assessed. The patients were allocated to different groups according to the Kt/Vwat and Kt/Vbis values. Clinical parameters were compared between these groups to investigate which method of obtaining the Kt/V value was more suitable. Results: 150 patients on PD were included. Vwat was significantly higher than Vbis, apart from in female patients with volume overload. Consequently, weekly Kt/Vwat was lower than Kt/Vbis in these patients. A significant negative correlation between mean Vwat-Vbis and overhydration values was also found. Moreover, through uniform manifold approximation and projection analysis, a clustering tendency between patients in the adequate group with both Kt/Vwat and Kt/Vbis > =1.7 and patients in the inconsistent group with Kt/Vwat =1.7 was identified, suggesting that their clinical features were similar.

Abstract:Background: Evaluating peritoneal fluid kinetics is of great value to the adjustment of peritoneal dialysis prescription. Therefore, in the present study, we developed a simple method, based on a membrane transport model, to evaluate the fluid transport characteristics of the peritoneal membrane in continuous ambulatory peritoneal dialysis (CAPD) patients. Methods: Patients’ peritoneal ultrafiltration (UF) volume was collected for 2 consecutive months. Membrane transport model and nonlinear least-squares regression analyses were applied to the pooled UF volume for the first month for fluid kinetic simulation and the data from the second month were used to evaluate the reliability and precision of simulated UF values. Results: Seventeen chronic peritoneal dialysis (PD) patients were selected for the present study. Simulated UF values based on the pooled UF volume in the first month correlated significantly with the actual measured UF values in the second month. By Bland-Atman plot, the width between the 95% limits of agreement fell within –133.3 to 154.2 ml with a mean difference of 10.5 ml. Conclusion: Our study showed that applying the fluid transport model and nonlinear least-squares regression analysis to the pooled actual UF value might be a good and simple way to predict peritoneal UF.
The regularity of intracellular and extracellular water changes in maintenance hemodialysis patients during dialysis

Abstract:Objective This study aimed to explore the regularity of intracellular and extracellular water changes in maintenance hemodialysis (MHD) patients during dialysis.Methods We selected 55 MHD patients as the subjects and measured their total body water (TBW),intracellular water (ICW) and extracellular water (ECW) before dialysis and every hour during dialysis,i.e.,a total of 5 measurements for a patient.Resuits ①Both ICW and ECW decreased during dialysis as compared with those before dialysis,and ICW decreased more than ECW.②The ratio of ICW/ECW was 2∶1 in the first two hours during the dialysis,without significant difference with that before dialysis (t=-5.620,P=0.580;t=-2.048,P=0.053);the ratio increased slightly to 2.1∶1 in the last two hours during the dialysis (t=-3.269,P=0.030;t=-4.696,P ＜0.001).③The ratios of △ICW/△ECW were 1.9,1.8,1.5 and 1.4 at the 1st,2nd,3rd and 4th hour respectively during the dialysis,and this ratio decreased significantly at the 4th hour as compared with that at the 1 st hour (t=2.279,P=0.036).④After dialysis,the reduction rate of ICW and ECW were 5.4％ and 8.4％ respectively (t=3.994,P＜0.001).Conclusion ①Dialysis by ultrafiltration decreased the water retention in both extracellular and intracellular spaces,and the intracellular water decreased more than the extracellular water,resulting in slight increase of ICW/ECW ratio at the end of dialysis.②The ratio of △ICW/△ECW decreased gradually along with the hour of the dialysis,and the reduction rate of ICW was less than that of ECW,suggesting that the excessive water in intracellular space moves to extracellular space through osmotic gradient and removes from the body with ECW during dialysis.

Abstract:To study the fluid distribution abnormalities in patients on hemodialysis and fluid shift between intracellular and extracellular compartment during hemodialysis (HD).Eighty stable end stage renal disease patients on maintenance hemodialysis were selected. Conventional low flux dialyses at dialysate sodium concentration of 138 mmoL/L were performed for all of the patients. Fresenius polysulphons or Gambro hemophon dialyzers were used. All of the patients showed no clinical signs of over ultrafiltration during HD sessions and dry body weights were considered adequate. Intracellular water (ICW), extracellular water (ECW) and total body water (TBW) were measured and standardized by body weight (nICW, nECW, nTBW) before and after HD sessions. Sixty-seven cases of sex, age and body weight matched normal individuals were used as controls. Bio-impedance spectrum (BIS) analysis (Xitron, Technologies, San Diego, CA, USA) was used in body fluid measurement. Pre and post HD nICW, nECW, and nTBW were compared with these of the controls.There were no differences in age, post-dialysis body weight and body mass index between patients on hemodialysis and the controls. Patients on hemodialysis had less nICW [male (0.28+/-0.05) vs (0.33+/-0.04), P<0.01; female (0.22+/-0.03) vs (0.27+/- 0.04), P<0.001], more nECW [male (0.28+/-0.02) vs (0.25+/-0.02), P<0.001; female (0.25+/-0.03) vs (0.23+/-0.01), P<0.05], and less nTBW [male (0.56+/-0.04) vs (0.58+/-0.05), P<0.05; female (0.47+/-0.05) vs (0.49+/-0.03), P<0.05] compared with the controls. The average ultra-filtration volume was (2.2+/-0.9)L. After dialysis, there was an increase in nICW [male (0.28+/-0.05) vs (0.30+/-0.05), P<0.001; female (0.22+/-0.03) vs (0.25+/-0.03), P<0.001), an decrease in nECW [male (0.28+/-0.02) vs (0.26+/-0.03), P<0.001; female (0.24+/-0.03) vs (0.21+/-0.03), P<0.001],and decrease of ECW/ICW ratio [male (1.06+/-0.30) vs (0.89+/-0.25) P<0.001; female (1.10+/-0.17) vs (0.88+/-0.17) P<0.001] which reached the levels of the normal individuals (0.79+/- 0.10 for male, 0.86+/-0.10 for female].Our results show that (1) Before HD sessions, patients on HD had less nICW and more nECW; (2) During conventional low flux HD sessions at dialysate sodium concentration of 138 mmoL/L, fluid shifted from extracellular space to intracellular space.(3) nECW and ECW/ICW ratio from normal population are reasonable for evaluation of dry weight in patients on hemodialysis, but nTBW and nICW are not.
