Abstract:RATIONALE: The purpose of this study is to report evidence supporting the use of ACT for identifying adolescents at risk of poorly controlled asthma.METHODS: ACT was administered to 79 asthma patients (12-17 years) in asthma specialist's offices. ACT was scored on a scale from 5 (not controlled at all) to 25 (completely controlled). The specialist's global assessment of asthma control after spirometry was also collected for each patient. Internal consistency reliability was estimated using Cronbach's alpha. Analysis of variance (ANOVA) methods were used to test the discriminant validity of ACT scores across specialist's control ratings. Receiver Operating Characteristic (ROC) analysis was used to evaluate the effectiveness of ACT for use in screening for asthma control problems, as defined by the specialist control ratings.RESULTS: The internal consistency reliability of ACT scores was 0.82. In support of discriminant validity, mean ACT scores differed significantly (F=10.2, p<0.001) across groups of patients differing in specialist asthma control ratings. The mean ACT score of patients rated as “not controlled at all” was the lowest (12.7), followed by “poorly controlled” (14.9), followed by “somewhat controlled” (17.7). The mean ACT scores of patients rated as “well controlled” and “completely controlled” were 20.9 and 22.0, respectively. Using a cut-point score of ≤19 established in adults for screening of asthma control problems, ACT had a sensitivity and specificity of 70% and 77%, respectively, correctly classified 74% of patients, and an area under the ROC curve of 0.74.CONCLUSIONS: ACT is reliable and valid measure of asthma control with adolescent patients.
