Abstract:Abstract The polyetheretherketone (PEEK)-highly cross-linked polyethylene (XLPE), all-polymer knee prosthesis has excellent prospects for replacing the traditional metal/ceramic-polyethylene joint prosthesis, improving the service life of the joint prosthesis and the quality of patients’ life. The long-term wear mechanism of PEEK-XLPE knee joint prosthesis is comprehensively evaluated from wear amount, wear morphology, and wear debris compared to that of CoCrMo-XLPE joint prosthesis. After 5 million cycles of in vitro wear, the wear loss of XLPE in PEEK-XLPE (30.9±3.2 mg) is lower than that of XLPE in CoCrMo-XLPE (32.1±3.1 mg). Compared to the XLPE in CoCrMo-XLPE, the plastic deformation of XLPE in PEEK-XLPE is more severe in the early stage, and the adhesive peeling and adhesion are lighter in the later stage. The size distribution of XLPE wear debris in PEEK-XLPE is relatively dispersed, which in CoCrMo-XLPE is relatively concentrated. Wear debris is mainly flake and block debris, and the wear mechanism of XLPE was abrasive wear. The wear volume per unit area of PEEK femoral condyle (10.45×10 5 μm 3 /mm 2 ) is higher than that of CoCrMo (8.32×10 5 μm 3 /mm 2 ). The PEEK surface is mainly furrows and adhesions, while the CoCrMo surface is mainly furrows and corrosion spots. The PEEK wear debris is mainly in flakes and blocks, and the CoCrMo wear debris is mainly in the shape of rods and blocks. The wear mechanism of PEEK is abrasive wear and adhesion, and that of CoCrMo is abrasive wear and corrosion.

Abstract:Total cervical disc replacement aims at avoiding fusion for the treatment of degenerative disc disease. This paper investigates the wear performances of PEEK and CFRPEEK against UHMWPE for artificial cervical disc application on a knee wear simulator due to the unavailability of a spinal one. The obtained results showed that the average wear rates of PEEK-on-UHMWPE and CFRPEEK-on-UHMWPE were 3.82 mg/MC and 2.34 mg/MC, respectively. The edge area of the bearing surfaces displayed severer wear in comparison with the central zone and was dominated by arc-shaped wear tracks. The main wear mechanisms were abrasion, adhesion and fatigue wear. CFRPEEK-on-UHMWPE has demonstrated superior wear characteristics, and hence, is a potential material configuration for artificial cervical disc application.

Abstract:Particle generation from artificial knee prostheses is a leading cause of artificial joint failures. Vitamin E stabilized highly cross-linked polyethylene (VE/HXLPE) is known for its antioxidant properties and minimal wear characteristics. However, research on the wear behavior of VE/HXLPE is limited. This study aims to compare wear debris from VE/HXLPE with the conventional material Ultra High Molecular Weight Polyethylene (UHMWPE) under identical in vitro test conditions. Specifically, Principal Component Analysis (PCA) was employed to morphologically categorize particles, resulting in four distinct groups for VE/HXLPE and five for UHMWPE. Notably, 68% of VE/HXLPE particles exhibited small-sized spheroidal morphology, while UHMWPE mainly produced elongated bioactive particles. Key particle attributes were then assessed, including 'equivalent circle diameter (ECD)', 'aspect ratio (AR)', and 'roundness (R) ', and presented as frequency distributions. The morphological analysis revealed that VE/HXLPE particles displayed various shapes, such as spheroidal, flakes, bulks, rods, and fibrils, and their formation mechanism was discussed. Additionally, the Functional Biological Activity (FBA) of VE/HXLPE wear particles was calculated. The results demonstrated differences in particle sizes, shape distributions, and bioactivity between VE/HXLPE and UHMWPE, which indicates that VE/HXLPE particles had a lower likelihood of inducing osteolysis. This study holds promise for gaining deeper insights into the mechanical properties and the potential for bioactivity in VE/HXLPE as an artificial joint material.

Influence of Cross-Shear and Contact Pressure on Wear Mechanisms of PEEK and CFR-PEEK in Total Hip Joint Replacements

Abstract:With the increasing market demand for artificial hip joints, total hip joint replacement has gradually become an effective means of treating a series of hip joint diseases. In order to improve the service life of artificial hip joints, some new artificial hip joint materials, including polyetheretherketone (PEEK) and carbon fiber reinforced polyetheretherketone (CFR-PEEK), have been developed. In this paper, pin-on-plate wear tests under different cross-shear ratios and contact pressures were carried out to study the wear mechanism and worn surface topography of PEEK and CFR-PEEK. The experimental results showed that the wear of PEEK was associated with cross-shear, while CFR-PEEK was not. When the cross-shear ratio was 0.039 and contact pressure was 3.18 MPa, PEEK had poor wear resistance and its wear factor was about eight times that of ultra-high molecular weight polyethylene (UHMWPE). The wear resistance of CFR-PEEK had a significant advantage, since its wear factor was about 30% of that of PEEK. The wear factors of PEEK and CFR-PEEK increased as the contact pressure increased. The arithmetic average of the height amplitude of the surface, Sa, also increased gradually according to the topography of the worn surface. The wear mechanisms of PEEK and CFR-PEEK were scratching, plough cutting, and abrasion Since CFR-PEEK had good wear resistance and insensitivity to cross-shear motion, it is suitable for making artificial hip joints under low contact pressure condition.

Abstract:Polyether ether ketone (PEEK) has been considered as an alternative material to cobalt chrome in the femoral component of a total knee replacement. In this article, the experimental simulation of torsion wear for a PEEK and CoCr femoral head against an ultra-high-molecular-weight polyethylene (UHMWPE) tibial pad was carried out using a two-station knee simulator. The testing results showed that T-θ curves of torsion wear for PEEK-on-UHMWPE bearing couples showed three types of morphological characteristics, such as linear, elliptical, and parallelogram shapes. With an increase in torsion angular displacement, T-θ curves changed from the linear to the parallelogram type, and the relative motion state changed from the partial slip regime coordinated by the elastic deformation to the slip regime with plastic deformation. With an increase in normal load, the T-θ curve changed from the parallelogram to the elliptical type, and the relative motion state changed from the slip regime to the mixed slip regime. Under intermediate torsion wear conditions, the wear factors of UHMWPE were 11.25 ± 2.08 × 10−6 mm3/C and 14.65 ± 2.49 × 10−6 mm3/C against PEEK and CoCr. Under high torsion wear conditions, the wear factors were 20.51 ± 3.23 × 10−6 mm3/C and 23.26 ± 3.01 × 10−6 mm3/C, respectively. The wear surface of UHMWPE against PEEK showed slight adhesive wear and plastic deformation in the center, and abrasive wear with local fatigue spalling was found at the edges. The wear topographies presented characteristics similar to the CoCr bearing combination.

Abstract:Polyetheretherketone (PEEK) and its composite coatings are believed to be the potential candidates' bio-implant materials. However, these coatings have not yet been used on the surface of titanium-based orthopedics and joint products and very few investigations on the tribological characteristics could be found in the published literature till date. In this study, the wettabilities, composition and micro-hardness were characterized using contact angle measurement, scanning electron microscopy (SEM) and hardness tester. The tribological tests were conducted using a ball-on-disc contact pair under 25% newborn calf serum (NCS) lubricated condition. For comparison, bare Ti6Al4V was studied. The obtained results revealed that those PEEK/ZrO2 composite coatings could improve the tribological properties of Ti6Al4V significantly. Adhesive wear and mild abrasive wear might be the dominant wear and failure mechanisms for PEEK/ZrO2 composite coatings in NCS lubricated condition. After comprehensive evaluation in the present study, 5wt.% ZrO2 nanoparticles filled PEEK coating displayed the optimum tribological characteristics and could be taken as a potential candidate for the bearing material of artificial cervical disc.

Abstract:Artificial cervical disc is a key device for artificial cervical disc replacement (ACDR). In our previous study, we developed PEEK-on-UHMWPE and CFRPEEK-on-UHMWPE tribo-parings for artificial cervical disc application. To further evaluate the biotribological properties and potential inflammation responses of their by-products, the size distribution and morphological characteristics of collected post-test wear particles as well as the biological response of aggregated particles were studied. Our results indicated that the majority of these wear particles were in a size range of 0.05-25 mu m and the possibility of immunoreaction was small. Thus, we provide further evidence from the wear particles' point of view to suggest that CFRPEEK-on-UHMWPE can be considered as the potential paring for artificial cervical disc application.
Study on Biocompatibility, Tribological Property and Wear Debris Characterization of Ultra-Low-wear Polyethylene As Artificial Joint Materials.

Abstract:Ultra-low-wear polyethylene (ULWPE) is a new type polyethylene made by experts who are from China petrochemical research institute, which is easy to process and implant. Preliminary test showed it was more resistant to wear than that of Ultra-high-molecular weight polyethylene (UHMWPE). The purpose of the research is to study biocompatibility, bio-tribological properties and debris characterization of ULWPE. Cytotoxicity test, hemolysis test, acute/chronic toxicity and muscular implantation test were conducted according to national standard GB/T-16886/ISO-10993 for evaluation requirements of medical surgical implants. We obtained that this novel material had good biocompatibility and biological safety. The wear performance of ULWPE and UHMWPE was evaluated in a pin-on-disc (POD) wear tester within two million cycles and a knee wear simulator within six million cycles. We found that the ULWPE was higher abrasion resistance than the UHMWPE, the wear rate of ULWPE by POD test and knee wear simulator was 0.4 mg/106cycles and (16.9 ± 1.8)mg/106cycles respectively, while that of UHMWPE was 1.8 mg/106cycles and (24.6 ± 2.4)mg/106cycles. The morphology of wear debris is also an important factor to evaluate artificial joint materials, this study showed that the ULWPE wear debris gotten from the simulator had various different shapes, including spherical, block, tear, etc. The morphology of worn surface and wear debris analysis showed that wear mechanisms of ULWPE were adhesion wear, abrasive wear and fatigue wear and other wear forms, which were consistent with that of UHMWPE. Thus we conclude that ULWPE is expected to be a lifetime implantation of artificial joint.

Abstract:Polyetheretherketone (PEEK) has been widely used in the medical field as an implant material, especially in bone tissue engineering and orthopedic surgery, in recent years. This material exhibits superior stability at high temperatures and is biosecured without harmful reactions. However, the chemical and biological inertness of PEEK still limits its applications. Recently, many approaches have been applied to improve its performance, including the modulation of physical morphology, chemical composition and antimicrobial agents, which advanced the osteointegration as well as antibacterial properties of PEEK materials. Based on the evolution of PEEK biomedical devices, many studies on the use of PEEK implants in spine surgery, joint surgery and trauma repair have been performed in the past few years, in most of which PEEK implants show better outcomes than traditional metal implants. This paper summarizes recent studies on the modification and application of biomedical PEEK materials, which provides further research directions for PEEK implants.

Abstract:Polyether-ether-ketone (PEEK) has been proposed as a biocompatible artificial joint material. Wear particles, generated by friction between artificial joints, lead to bone resorption, aseptic loosening, and ultimately, joint failure. The size and morphology of wear particles contain information of friction and wear. Aim to obtain the wear mechanism of PEEK under different loads, this study separated PEEK debris and investigated the mechanism of wear debris and the relationship between wear mechanism and PEEK-debris morphology. An experiment was carried out with a pin-on-plate testing apparatus under different load conditions, with PEEK sliding against XLPE under saline lubrication. A method of isolating PEEK and XLPE debris from 0.9% normal saline at the same time was investigated by low-speed centrifugation. The morphologies of worn surface and wear debris were obtained based on scanning electron microscopy. The results showed that the maximum friction coefficient and minimum wear loss were 0.115 and 0.223 mg at the load of 50 N. The friction coefficient decreased and the wear loss increased with the load increase. This debris-isolation method can effectively isolate PEEK and XLPE particles larger than 200 nm in diameter. More than 96% wear PEEK particles range from 0.1 μm to 10 μm. Compared with the debris generated under the lower load condition, 0.8% more large wear particles with irregular shapes were found at a load of 150 N. The morphology of wear particles is consistent with the wear mechanism.

Abstract:The failure caused by fretting wear is a key issue in orthopedic applications as well as other engineering applications. In this study, fretting wear tests were conducted on poly (ether ether ketone) (PEEK), glass fiber reinforced PEEK (GFRPEEK) and carbon fiber reinforced PEEK (CFRPEEK), respectively. Surface characterizations of tested specimens were performed using XRD, microhardness tester, 3D white-light interfering profilometry, SEM and optical microscopy to analyze their wear features. The obtained results showed that the fibers increased the microhardness values and reduced the friction coefficients and wear rates of PEEK-based composites. The fretting regimes of PEEK, GFRPEEK and CFRPEEK were gross slip. The fretting wear mechanisms of those PEEK composites were dominated by abrasive wear, adhesive wear and delamination. CFRPEEK has demonstrated superior fretting wear characteristics, and hence, is a potential bio-implant material for applications such as artificial joints.

Abstract:The tribological behavior between orthopedic implants and cortical bone is important but usually neglected. Poly(ether-ether-ketone) (PEEK) is a promising material for orthopedic applications. To further understand and improve the interfacial tribological properties between PEEK implant and host bone tissue, a PEEK-cortical bone tribo-pair is designed and fabricated. The frictional and wear performance of such tribo-pair is investigated under different lubricants, i.e., simulated body fluid (SBF), calf serum (CS), hyaluronic acid (HA), and mucin (MUC). The results suggest that MUC solution can be utilized as an artificial natural synovial fluid to improve the tribological properties of PEEK-based implants.

Abstract:Understanding wear mechanisms is a key factor to prevent primary failures causing revision surgery in total hip replacement (THR) applications. This study introduces a wear prediction model of (Polyetheretherketone) PEEK-on-XLPE (cross-linked polyethylene) bearing couple utilized to investigate the wear mechanism under 3D-gait cycle loading over 5 million cycles (Mc). A 32-mm PEEK femoral head and 4-mm thick XLPE bearing liner with a 3-mm PEEK shell are modeled in a 3D explicit finite element modeling (FEM) program. The volumetric and linear wear rates of XLPE liner per every million cycles were predicted as 1.965 mm3/Mc, and 0.0032 mm/Mc respectively. These results are consistent with the literature. PEEK-on-XLPE bearing couple exhibits a promising wear performance used in THR application. The wear pattern evolution of the model is similar to that of conventional polyethylene liners. Therefore, PEEK could be proposed as an alternative material to the CoCr head, especially used in XLPE-bearing couples. The wear prediction model could be utilized to improve the design parameters with the aim of prolonging the life span of hip implants.

Abstract:The polyethylene (PE) for hip implants presents serious clinical problems; the production of debris may induce adverse tissue reactions that may lead to extensive bone loss around the implant and consequently osteolysis and implant loosening. Several attempts have been made to improve the wear properties of ultra-high molecular weight polyethylene (UHMWPE). More recently the attention of various researchers has been focused on cross-linked polyethylene (XLPE), due to its improved wear resistance with respect to conventional UHMWPE. This study was aimed at comparing the wear performances of clinically available acetabular liners (Zimmer Inc.) made of electron beam XLPE and conventional UHMWPE. To evaluate the influence of the material properties on wear, conventional UHMWPE and XLPE acetabular cups were tested against deliberately scratched CoCrMo femoral heads (Ra = 0.12-0.14 microm) in a hip joint wear simulator run for 3 million cycles with bovine calf serum as lubricant. Gravimetric measurements revealed significant differences between the wear behaviours of the two sets of acetabular cups: XLPE exhibited a wear rate about 40 times lower than conventional UHMWPE. Raman spectroscopy coupled to partial least-squares analysis was used to evaluate the possible crystallinity changes induced by mechanical stress (and thus the material wear resistance): only the UHMWPE cup which showed the highest weight loss displayed significant crystallinity changes. These results were correlated to the thickness of the plasticity-induced damage layer. The wear debris produced during the tests were isolated according to a validated protocol and imaged by scanning electron microscopy . The wear particles produced by XLPE were smaller than those produced by UHMWPE; the latter were observed as fibrillar and agglomerated particles. The mean equivalent circle diameter was 0.71 and 0.26 microm for UHMWPE and XLPE, respectively.

Abstract:Poly(ether-ether-ketone) (PEEK) is a type of biomaterial which may be used for modifying the surface of materials used in implants. Hence, in the present investigation, the potentiality of PEEK and its composites coatings has been explored for improving the friction and wear behavior of the Ti6Al4V to be used for cervical disks. The structural characteristics, micro-hardness, friction, and wear characteristics of PEEK/Al2O3 and PEEK/SiO2 composite coatings have been investigated and compared with pure PEEK coating and bare titanium alloy sample. According to the XRD analysis results, these coated samples were mainly orthorhombic crystalline form. The contact angle values of PEEK and its composite coatings were higher, while micro-hardness values of these samples decreased significantly. The thickness values of the three coated samples were all above 70 μm on average. The average friction coefficients with a counterface of ZrO2 ball decreased significantly, especially under NCS (newborn calf serum) lubricated condition. After comprehensive evaluation, the PEEK/Al2O3 coating demonstrated optimum tribological properties and could be applied as bearing materials for artificial cervical disk.

Abstract:Large bone defects caused by tumors and traumas are difficult clinical problems. For its treatment, autogenous bone transplantation is the "gold standard". However, there are some shortcomings of this treatment, such as limited supply and extra trauma from bone removal. In recent years, orthopedic implants and bone tissue engineering materials have developed rapidly, adding impetus to the solution of this problem. At present, the main orthopedic implants include medical metal materials, medical polymer materials, bone cement, ceramic materials, artificial bone materials, etc. Among which medical polymer materials have become the research hotspot and achieved outstanding results in recent years, especially the in-depth study of polyether-ether-ketone (PEEK) materials has great promising to solve this medical problem. PEEK materials have the advantages of non-toxicity, high-temperature resistance, corrosion resistance, abrasion resistance, high strength, high toughness, X-ray radiolucency, and excellent sterilization performance. PEEK materials have been successfully applied in clinical practice and have achieved excellent clinical efficacy and wide recognition. This review overviews the research progress of the performance requirements, material development, and material surface modification of PEEK as an orthopedic implant, and discusses prospects for the advance of medical PEEK materials.

Abstract:Polyetheretherketone (PEEK) is a kind of synthetic thermoplastic polymer, because of its good biological compatibility, chemical stability and radiolucency, it has been widely used in the field of medicine. Compared with the titanium, the elastic modulus of PEEK is closer to human cortical bone, PEEK could be a viable alternative material for dental implants. Nevertheless the inherent bio-inert nature and lower osteogenic activity limit its wider clinical applications, thus a considerable effort has been made to improve its biological properties. This article makes a summary on the research progress of the application of PEEK composites in dental implant.

Abstract:In order to investigate the biotribology behavior of a novel artificial joint pair composed of a carbon/carbon composite femoral head and an ultra-high molecular weight polyethylene (UHMWPE) acetabular cup, a hip joint simulator was employed to predict the clinical wear behavior with a constant load and a lubricant of newborn calf serum. The worn surface and the wear particles generated were analyzed by scanning electron microscopy and laser particle size analyzer. The results showed that the worn surface of UHMWPE had a ripple-like morphology with plentiful furrows. The wear particles generated had various morphologies with a size concentrated at about 15 μm.
A preclinical numerical assessment of a polyetheretherketone femoral component in total knee arthroplasty during gait

Abstract:BackgroundConventional total knee replacement designs show high success rates but in the long term, the stiff metal components may affect bone quality of the distal femur. In this study we introduce an all-polymer total knee replacement device containing a PEEK femoral component on an UHMWPE tibial implant and study its mechanical integrity, fixation, and stress shielding of the periprosthetic femur.MethodsThe implant was analysed in finite element simulations of level gait, adopted from the ISO 14243-1 standard. Mechanical integrity of the implant and underlying cement mantle were tested, and the fixation strength of the cement-implant interface was studied. Stress shielding was assessed based on strain energy density distributions in the distal femur. We compared PEEK and CoCr implants for mechanical performance and fixation, and compared both versions against an intact case to determine the change in bone strain energy density.ResultsThe mechanical integrity of the PEEK and CoCr components was similar in magnitude, but differences in stress patterns were found. Moreover, the cement mantle was loaded more heavily in the CoCr configuration. Under similar interface properties, the CoCr-cement interface was more at risk of failure than the PEEK-cement interface. The bone strain energy density distribution of the PEEK implant was similar to the intact case, while the CoCr implant showed signs of stress shielding, and a different distribution than the intact and PEEK models.ConclusionsDuring gait, the PEEK femoral component performed similarly to CoCr, with no added risk for the cement mantle. The reduction in stress shielding for PEEK was evident and confirms the potential reduction in long-term loss of bone stock for this all-polymer knee implant.
