Abstract:The BJD has been publishing clinical practice guidelines (CPG) in dermatology for more than 20 years.1‐3 With the increasing adoption of gold‐standard guideline development methodology and reporting checklists, such as the GRADE4 approach and the AGREE II5 toolkit, recent CPGs published in the BJD are of high‐quality and are highly accessed, with CPGs making up half of the top 10 downloaded BJD articles in 2019. As a group of guideline developers and journal editors associated with the BJD, we were interested in how the readership perceives and uses CPGs. The objective of this study was to survey the BJD readership's demographics, utility of and attitudes towards CPGs.

Abstract:Abstract Keeping up with the influx of scientific research papers can feel overwhelming, especially alongside busy clinical responsibilities. A PubMed search of the term ‘dermatology’ produced 29 565 results for 2021 alone, and, consequently, clinicians face spending hours a week scanning journals just to stay up to date. This is coupled with increasing clinical demand due to rising waiting lists and skin cancer pressures. Innovative solutions to the problem include regular newsletters, which have been proven to be an effective tool in medical education in other specialities (Kolli A, Nguyen A, Portney D et al. The Lens: can a weekly newsletter improve medical student knowledge of the ophthalmology literature? J Surg Educ 2021; 78:1905–14). To address this challenge in dermatology, we created a newsletter to make dermatology research more accessible. Two consultant dermatologists created a system whereby clinically relevant articles are chosen following a review of ∼30 journals. These are condensed into short clinical summaries, containing relevant findings, limitations and applicability, emulating a journal club style. This was initially distributed within our local hospital, but the team and readership have since expanded. From January 2022, in addition to the written newsletter, a freely available podcast for each newsletter was created giving an opportunity to learn on the move (e.g. while commuting). At 12 months, the newsletter has 206 subscribers, and the podcast has achieved 810 total plays (with download locations ranging from the USA to India). More recently, we have reached new audiences via Instagram, with 1700 views and 123 likes of the content thus far. A survey was distributed to current subscribers to determine the usefulness of the newsletter as an educational resource. Eighteen responses were received, and 100% agreed that they felt more informed on recent publications in dermatology from reading the newsletter (83% strongly agreed; 17% agreed). In addition to this, 100% felt that the articles chosen were relevant to their clinical practice (78% strongly agreed; 22% agreed). Feedback included comments such as ‘it is a great way to keep informed on recent publications that may be of interest’ and ‘super useful bite-sized information’. The General Medical Council guidance on Good Medical Practice requires doctors to ‘keep professional knowledge and skills up to date’. We feel the interest in our grassroots journal club publication and the survey results (with all our respondents feeling more informed on dermatological research, particularly with articles relevant to their clinical practice) highlight an unmet need for accessible educational resources to enable busy dermatologists to become aware rapidly and efficiently of the latest clinical developments.

Abstract:Abstract We identified important soft skills not specifically highlighted within the dermatology trainee Joint Royal Colleges of Physicians Training Board (JRCPTB) curriculum, from our journal club experience. We propose a ‘hidden generic curriculum’: a rapidly expanding list of important soft skills to be honed for enhancing trainee learning and competency, while facilitating the transition into consultant dermatologist. Initiated by trainees during the COVID pandemic, trainee-led and consultant/professor-facilitated journal club sessions were developed for in-person, virtual and hybrid formats. Sessions were designated for 08.00–09.00 h on Wednesdays, with voluntary attendance. Topics were chosen randomly and were mapped to the existing dermatology curriculum as ‘subdomains’. Learning points from articles were used to reflect on real-world clinical practice. Articles were discussed with PowerPoint aids, trainee-generated quizzes and podcasts as adjuncts to sessions. Our journal club format was utilized as a tool and curriculum adjunct to enhance soft skills important for practising dermatologists, leading to discussion around clinical cases. Common but perhaps overlooked important competencies used in everyday practice were identified, and a continually expanding list of skills has been generated, leading to comprehensive discussion between trainees and consultants. Group reflection followed, on when to apply learned skills in outpatients or during on-call settings, for example motivational interviewing in patients with chronic disease. We highlight our list of ‘top 10’ articles, recommended for trainees to reflect on in their journal clubs, allowing them to build on soft skills. This includes articles titled ‘dermatological games’, ‘SPIKES – a six-step protocol for delivering bad news: application to the patient with cancer’, ‘writing to patients’, ‘motivational interviewing in dermatology’, ‘suicide in dermatological patients’, ‘the difficult patient – the hateful patient revisited: relevance for 21st century medicine’, ‘achieving quality in clinical decision making: cognitive strategies and detection of bias’, ‘physician stress and burnout’, ‘compassionate care: enhancing physician–patient communication and education in dermatology’ and ‘how to train to discharge a dermatology outpatient: a review’. In a group, papers were summarized, learning points were noted, including why the article was important for trainees, and we proposed a hidden generic curriculum item or ‘subdomain’, later mapped to the JRCPTB curriculum. There are many other relevant articles beyond our top 10 list that have been used in subsequent journal club sessions. Exploring the ‘hidden generic curriculum’ may enhance the learning experience and trainee development. Abstract:BACKGROUND: Most clinical guidelines in dermatology are encyclopaedic, covering a disease and its aetiology, diagnosis, treatment and prevention in their entirety. The usability and uptake of guideline recommendations might be improved by guidelines that are more concise and address specific questions ranked by users according to their perceived importance.OBJECTIVE: To survey the largest association of dermatologists in Europe, identify which questions in their everyday practice they felt would benefit from short, evidence-based guidance and rank these systematically.METHODS: A two-phase online survey using a structured ranking approach and the members directory of the European Academy of Dermatology and Venereology (EADV).RESULTS: The first survey yielded 265 suggestions, indicating a response rate of 3.7%. We grouped all responses according to themes and subsequently combined these into a list of 35 broader topics. These were presented to all members of the EADV in the second survey (response rate: 9.7%), which yielded a list of the top 10 topics participants felt were most in need of guidance. The first three were 'Systemic drug treatment in dermatology during pregnancy and for women wishing to have children in the near future', 'alopecia areata' and 'interpretation of laboratory results in connective tissue diseases'.CONCLUSION: Our two-phase survey of EADV members and a structured ranking process were practical to implement and yielded a list of the top 10 topics in dermatology and venereology for guideline development. Guideline dissemination needs to be improved, and practical, more concise guidelines may facilitate efforts to do so.

Abstract:Abstract Advice and guidance (A&G) has become more prominent in medicine and has been used across multiple specialties. It has been implemented by most dermatology departments, which tackle the challenge in different ways. Why is it so challenging? What can we do to fix it? How can it be more manageable? These are common questions that are asked by clinicians, managers and referrers. We conducted a multicycle quality improvement project at our hospital collating feedback from clinicians, managers and referrers. As part of this we identified multiple challenges and also had a list of solutions, many of which led to the following improvement. We formed an A&G focus group within the department with a registrar chairing the group and discussing solutions at an annual strategy meeting. The first method was identifying the challenges. The challenges included a high number of referrals, multiple duplicate referrals that were pending, poor-quality photographs including inadequate dermoscopy, and, concerningly, referral of pigmented lesions including melanomas without review within primary care with senior clinicians. Having collated this list of challenges we emailed around asking for each clinician’s feedback from the team that actively takes part in A&G. These responses were collated and are shown below. They led to the following significant improvements in the numbers of pending A&G dermatology cases in the hospital. Over a 10-week period during this project there was a 33.6% reduction in the number of pending referrals from a peak of 1075 cases. There was a 99.4% reduction in the number of days taken to respond to urgent cases, a 35.7% reduction in the number of days taken to respond to paediatric cases and a 83.3% reduction in the number of days taken to respond to adult A&G referrals. The solutions involved (i) ensuring A&G staff were given clear dedicated time within job plans, with each clinician having a set number of cases to do per month; and (ii) feeding back response times via the local integrated care board during meetings and signposting to correct referral pathways during responses (as a tool to reduce overall duplication). Future plans include holding a circuit breaker week and sending out generic criteria for advice and guidance at the end of each response, highlighting current response time and the avoidance of 2-week-wait lesions. Our model can be used nationally within any medical or surgical specialty and has the potential to lead to significant improvements. It can help to ensure that referrers are clear of the pathways, while making A&G more manageable.

Abstract:The Global Guidelines in Dermatology Mapping Project (GUIDEMAP) assesses the methodological quality of clinical practice guidelines (CPGs) for high‐burden skin diseases. This review focuses on contact dermatitis. We searched MEDLINE, Embase, PubMed, Web of Science, Cochrane Library, Emcare, Epistemonikos, PsycINFO and Academic Search Premier for CPGs published between 1 November 2018 and 1 November 2023. Prespecified guideline resources were hand searched. Two authors independently undertook screening, data extraction and quality assessments. Instruments used were the Appraisal of Guidelines for Research and Evaluation (AGREE) II Reporting Checklist, the U.S. Institute of Medicine's (IOM) criteria of trustworthiness, The Agency for Healthcare Research and Quality's National Guideline Clearinghouse Extent Adherence to Trustworthy Standards (NEATS) Instrument and Lenzer's Red Flags. Twenty five CPGs were included, exhibiting heterogeneity in both the topics they addressed and their methodological quality. Whereas the CPGs on management of hand eczema from Denmark, Europe and the Netherlands scored best, most CPGs fell short of being clear, unbiased, trustworthy and evidence‐based. Disclosure of conflicts of interest scored well, and areas needing improvement include 'strength and wording of recommendations', 'applicability', 'updating' and 'external review'. Adhering to AGREE II and Grading of Recommendations, Assessment, Development and Evaluations (GRADE) enhances methodological quality.

Virtual Grand Rounds in Dermatology: an 8-year experience in web-based teledermatology

Abstract:Background: Teledermatology utilizes telecommunication technologies for the exchange of dermatologic information across distances. In 2000, we developed the Virtual Grand Rounds in Dermatology (VGRD) as a free, user-friendly platform for dermatologists to present complex dermatologic cases to the international community for clinical feedback, consultation, and continuing education. Methods: VGRD was designed using simple graphics to facilitate access in remote and developing geographic regions. Members present their case according to a recommended template and are encouraged to provide clinical and histopathologic photographs. Viewers' comments are posted on the site within 24-48 h following receipt. Results: VGRD's low-cost store-and-forward system allows easy accessibility to expertise for colleagues around the world. VGRD currently comprises a community of 344 clinicians across all dermatologic subspecialties, and links users to cases as well as a recent interactive addition for more urgent feedback: VGRD Blog. The Maine Virtual Skin Clinic is another sister site developed as an educational tool for physicians and residents in primary care. VGRD can be found at http://www.vgrd.org. Conclusions: With its easy-to-use platform for teleconsultation services, VGRD has the potential to enhance clinical outcomes and education. In the next few years, we hope to continue to expand VGRD, including launching a comprehensive search engine in VGRD to provide users with a "one-stop resource." With the growth of teledermatology, we will combine web consultations with web-based education into a single resource for dermatologists interested in new possibilities in clinical practice, patient care, and continuing education.

P031 Experience from a UK dermatology regional virtual biologics and high-cost drugs multidisciplinary team

Abstract:Abstract There are over 20 biologics and high-cost drugs (BHCD) in routine use in dermatology in the UK, each associated with NICE technology appraisals and local guidelines, and there are more emerging. This, coupled with increasing clinical complexity, can make individual decision making challenging. In January 2021 a fortnightly BHCD multidisciplinary team (MDT) was initiated to support clinical practice and audit. Meetings are held virtually on a fortnightly basis using Microsoft Teams and are attended by dermatology consultants, trainees, specialist nurses and a biologics coordinator. Regional colleagues join to discuss patients needing a supraspecialist view (e.g. fourth or more BHCD) and to support shared care service level agreements for NHSE specialized commissioned drugs (e.g. adalimumab for hidradenitis). We work to locally agreed pathways, incorporating NICE standards. Between January 2021 and December 2023 there were 632 case discussions (106 in 2021, 250 in 2022, 276 in 2023) comprising psoriasis (n = 318), atopic eczema (n = 246), hidradenitis suppurativa (HS; n = 64) and other (n = 4). Examining outputs and outcomes from 2022, we discussed 250 cases (133 psoriasis, 92 atopic eczema, 22 HS, 3 other) and demonstrated 100% compliance with local and national standards for the cases of eczema and psoriasis. There was 100% compliance with national standards and 93% compliance with local standards for HS. This collaborative approach to decision making in a virtual MDT for BHCD has supported clinical practice, improved adherence to guidelines and reduced the time needed to undertake audits for individual drugs. A regional ‘hub and spoke’ arrangement for NHSE-commissioned drugs has supported care closer to home. The MDT further supports cost-effective practice (e.g. drug choices, sequencing and Blueteq submission via the coordinator), research (e.g. recruitment into national registry studies), continuing professional development, teaching and training, and provides MDT support for individual funding applications.

Abstract:Abstract There is a growing trend towards paperless clinical practice. Apps and web-based tools can help to quickly capture useful clinical information to influence management. We reviewed free digital dermatology tools that are available for clinicians with the goal of promoting awareness of these resources. We conducted a PubMed literature search and searched the Apple App Store using key search terms. We found that web-based platforms or apps available with a dermatological theme that can aid clinical practice can be summarized and categorized into the following four groups. (i) Disease-specific severity assessment in alopecia areata (AASI), atopic eczema (eSCORAD, eEASI, POEM, vIGA_AD), melasma (MASI), psoriasis (ePASI), nail psoriasis (NAPPA-QOL), acne (ePGA), hidradenitis suppurativa (HS-PGA, IHS4, eSartorius, eHurley), chronic urticaria (UAS7, UCT), pressure ulcers (Braden scale), toxic epidermal necrolysis (eSCORTEN), symptom severity assessment in pruritus (PNRS, pruritus VAS), pain (ePain VAS, Neuropathic Pain Scale), obesity (body mass index calculator), leg ulcer (eWoundQoL score) and assessment of quality of life in any skin disorder (eDLQI). Also, staging of skin cancers like melanoma (eTNM) and squamous cell carcinoma (AJCC8, BWH), and estimating survival and risk prediction in melanoma [positive sentinel lymph node biopsy, recurrence, first primary, second primary, immunotherapy outcome prediction (MIA, SweMR)]. (ii) As an aid to diagnosis in Behçet disease, DRESS (RegiSCAR), psoriatic arthritis (DAPSA, CASPAR), scleroderma (RODNAN), systemic lupus erythematosus (EULAR-ACR, SLICC criteria, SLEDAI), necrotizing fasciitis (LRINEC score) and cellulitis (ALT-70); and for depression screening (ePHQ9), suicidality screening (ED-SAFE PSS-3), anxiety screening (GAD-7), excess alcohol consumption (AUDIT-C, CAGE), HIV risk screening (Denver HIV score), predicting life expectancy (Charlson Comorbidity Index), Clinical Frailty Score, mental state (AMT-10) and noninvasive estimate of liver cirrhosis (FIB-4 Index, NAFLD fibrosis score). (iii) To help inform decisions related to skin surgery: decision to stop or continue an anticoagulant before skin surgery (HASBLED), stroke risk in atrial fibrillation (CHA2DS2-VASs score), and a local anaesthetic dosing guide. (iv) For assistance in prescribing: electronic British National Formulary, systemic steroid conversion, hydroxychloroquine maximum daily dose, and Geneva score – inpatient venous thromboembolism prophylaxis. We found the following resources especially useful – MDApp, Dermascore, Dermatools, Dermavalue, MDCalc and the Melanoma Institute of Australia risk prediction tools; these require manual input or selection of relevant data from dropdown lists, enabling the program to calculate the final score. In our experience, use of digital resources facilitates processing of relevant clinical information quickly and efficiently, helping in patient engagement and thorough assessment. We propose that the British Association of Dermatologists could develop a ‘one-stop’ app with multiple scoring tools for dermatologists to use in clinical practice.

Abstract:Abstract The British Hair and Nail Society was formed in 2011. We currently have 133 members and invite them to submit challenging cases of hair disorders to our grand round (with relevant history, histopathology and photographs) for review by our expert hair panel. This comprises consultants with hair expertise from around the UK, and together we aim to address the referrers’ queries relating to diagnosis, treatment or both. We appreciate the complexity of hair disorders and the limited number of specialist hair clinics around the country, and therefore we have recently offered the opportunity for all British Association of Dermatologists (BAD) members to submit cases once a year. The aim of our review is to present the types of complex hair disorders that have been referred to our dedicated national hair-related grand round and showcase some of the most educational cases. The hair disorders grand round began in 2018 and since then a total of 26 hair cases (10 children and 16 adults) have been submitted from various regions. The main aims of referral include diagnostic expertise (n = 4), aid with treatment plans (n = 8) or both (n = 14). The median age of patients was 33 years (range 4–75). There were 14 female and 12 male cases. The regions of referral include Oxford, Brighton, Manchester and Scotland. There were equal numbers of scarring alopecia (n = 13) and nonscarring (n = 13) cases. The most common nonscarring hair condition was alopecia areata (n = 6), with the possibility of other concurrent hair conditions (n = 1) or an underlying genetic disorder (n = 3). The most common diagnosis of scarring alopecia was folliculitis decalvans (n = 6). The diagnosis was altered in 10 cases (38%). Examples of some of our rare diagnoses include keratosis follicularis spinulosa decalvans, genetic hypotrichosis and scalp dermatomyositis. We reviewed cases of recalcitrant disease such as folliculitis decalvans, and novel therapeutics were suggested such as the use of naltrexone, utilizing photodynamic therapy after removing crusts, and even the use of manuka honey and cannabinoid oil. Our grand round has recommended independent funding requests in three cases and supports applications from clinicians for their patients with complex conditions. We hope our presentation showcases to the wider BAD membership the value of our national hair disorders grand round in supporting complex diagnostic and therapeutic decisions, as well as offering support for clinicians in the form of individual funding request submissions or trialling novel therapies, which may have limited evidence. We welcome any future submissions.

Abstract:Plain language summaries (PLSs) are being increasingly implemented and required by regulatory bodies to improve accessibility of scientific research for the general population. We assessed whether the British Journal of Dermatology (BJD) PLSs over 12 issues have been written at a suitable readability level for the general population, based on published guidelines. We found that authors may benefit from increased training or guidance in the production of these worthwhile texts, to further promote patient engagement with the BJD.

Abstract:In recent years, new approaches for optimal patient management of cancer have focused on patient-centered care, with integration of tumour-directed treatment and patient-directed supportive and palliative care throughout the disease journey from prevention through screening, diagnosis, treatment, and follow-up. In 2022, at the International Forum of Dermatology (IFD), a scientific session was entirely dedicated to highlight recent developments on patient-centered approaches in skin cancer. An international panel of different groups of participants involved in a patient's journey on the management of skin cancer presented and discussed challenges and barriers that persist in the field of skin cancer prevention and care pathways. Although primary prevention remains a crucial step in the prevention of melanoma, the different surveys performed during the last 20 years demonstrate that the use of sunscreen increases very slowly. Secondary prevention that includes skin screening and diagnostic measures may benefit from the development of digital tools. To improve adherence, patients need accurate, reliable information about their disease and the treatment options, and this type of content that can also be made available on digital tools. Shared decision-making is a hallmark of a patient-centered approach and requires health care providers who can communicate well to patients and their families, underscoring the pivotal role of health care professionals all through the patient journey. Health care providers have a crucial role in supporting patients through their journey in skin cancer. They will benefit from mobile apps and technologies that have been developed recently to address challenges in skin cancer prevention, detection and care, including those that are primarily directed to the patient. However, more peer-reviewed studies are needed as well as regulations to ensure that apps are accurate, reliable, and up to date.
BT39 Teledermatology assessment of community skin lesion referrals: insights, challenges and recommendations

Abstract:Abstract Consultations for skin lesions rank are a very common reason for general practitioner (GP) visits, with the urgent suspicion of cancer (USOC) referral pathway directing potentially cancerous lesions to dermatologists in our region. Despite the valuable role of clinical photographs alongside referrals, the variable quality poses a significant challenge. From March to April 2023, 100 GP referrals containing photos in our USOC pathway underwent analysis by two independent consultant dermatologists. They provided their top three diagnoses with free-text comments on image quality. Notably, prevalent image comments included ‘out of focus (60%)’ and ‘poor illumination’ (55%). Dermoscopy images were included in 15 community photos. Consultant 1 (G.G.) identified seborrhoeic keratosis as the primary diagnosis in 32%, while consultant 2 (R.J.) noted it as the top diagnosis in 34%. Consultant 1 included the GP’s diagnosis in their top three differentials 40% of the time, while consultant 2 did so 30% of the time. Existing guidance on teledermatology image quality is limited but emphasizes clear, well-lit photos with proper focus, taken from various angles and distances. Despite this, our data reveal a noticeable deviation from these practices. We propose that possible contributory factors leading to poor-quality images include time constraints during GP consultations, lack of awareness regarding guidance, and the use of devices with suboptimal image resolution. Notably, there are no local specific guidelines for community photos in our area. Based on our findings, we have created a local guideline, emphasizing the need for clear, focused images where the lesion is centrally framed, utilizing natural sunlight ideally at an angle to highlight skin texture. Additionally, an enlarged photo is recommended, balancing size with the maintenance of focus on the area of interest. Teledermatology provides broader access to expert advice, facilitates timely consultations, improves efficiency, and encourages global collaboration. However, the heterogeneity in photos can lead to misdiagnosis, inappropriate triage, and inefficient resource allocation. The implementation of guidance for referral photos can optimize resource use, enhance patient satisfaction, and streamline workflow.

Abstract:This article was migrated. The article was marked as recommended. Introduction Continuous Professional Development (CPD) is an important part of a physician's professional life. Yet, providing effective in-service training solutions is a persistent challenge for CPD planners. Methods Primary care physicians are frequently confronted with skin lesionsthey feel ill-prepared to manage. A dermatology Learning-by-concordance (LbC) online activity was developed and offered to family physicians for CPD credit. We were interested in finding out whether this online tool was suitable for CPD. Following a pilot phase, the on-line activity was launched and 45 geographically dispersed primary care physicians completed it. They participated in a telephone conference a week later with an expert to discuss outstanding questions. Evaluation was carried out by a survey that was available immediately after the last case. Results Participants found the on-line training tool user friendly and should be implemented on a larger scale. Participants found the dermatology concepts discussed allowed them to increase their knowledge and apply it to their practice. Discussion Among the strengths of LbC is that the learning task resemble those of a primary physician's daily practice. Finally, our study reveals that LbC is easily integrated in busy work schedules and thus is an effective learning solution for CPD.
