Abstract:Abstract Keeping up with the influx of scientific research papers can feel overwhelming, especially alongside busy clinical responsibilities. A PubMed search of the term ‘dermatology’ produced 29 565 results for 2021 alone, and, consequently, clinicians face spending hours a week scanning journals just to stay up to date. This is coupled with increasing clinical demand due to rising waiting lists and skin cancer pressures. Innovative solutions to the problem include regular newsletters, which have been proven to be an effective tool in medical education in other specialities (Kolli A, Nguyen A, Portney D et al. The Lens: can a weekly newsletter improve medical student knowledge of the ophthalmology literature? J Surg Educ 2021; 78:1905–14). To address this challenge in dermatology, we created a newsletter to make dermatology research more accessible. Two consultant dermatologists created a system whereby clinically relevant articles are chosen following a review of ∼30 journals. These are condensed into short clinical summaries, containing relevant findings, limitations and applicability, emulating a journal club style. This was initially distributed within our local hospital, but the team and readership have since expanded. From January 2022, in addition to the written newsletter, a freely available podcast for each newsletter was created giving an opportunity to learn on the move (e.g. while commuting). At 12 months, the newsletter has 206 subscribers, and the podcast has achieved 810 total plays (with download locations ranging from the USA to India). More recently, we have reached new audiences via Instagram, with 1700 views and 123 likes of the content thus far. A survey was distributed to current subscribers to determine the usefulness of the newsletter as an educational resource. Eighteen responses were received, and 100% agreed that they felt more informed on recent publications in dermatology from reading the newsletter (83% strongly agreed; 17% agreed). In addition to this, 100% felt that the articles chosen were relevant to their clinical practice (78% strongly agreed; 22% agreed). Feedback included comments such as ‘it is a great way to keep informed on recent publications that may be of interest’ and ‘super useful bite-sized information’. The General Medical Council guidance on Good Medical Practice requires doctors to ‘keep professional knowledge and skills up to date’. We feel the interest in our grassroots journal club publication and the survey results (with all our respondents feeling more informed on dermatological research, particularly with articles relevant to their clinical practice) highlight an unmet need for accessible educational resources to enable busy dermatologists to become aware rapidly and efficiently of the latest clinical developments.

Abstract:The European Journal of Cardiovascular Nursing (EJCN), as part of the European Society of Cardiology (ESC) family of journals, is one of few journals globally publishing research generated by cardiovascular nurses and allied professionals. Since the new Editor in Chief, Philip Moons, has been in post, there have been a number of new initiatives launched. Among these is a move to engage patients living with cardiovascular disease much more in the journal. Working with the ESC Patient Forum, the journal has recruited Benoit Mores from the forum to sit on the EJCN Editorial Board. This was a first for journals within the ESC journal family. Having a patient representative on the Board serves as a reminder for other members to keep in mind the important perspective of the patient in what we do, whether it be research, teaching, or clinical practice.

Abstract:Abstract Background/Aims Patient involvement at every stage of service development is key to driving forward meaningful change. It can however, be difficult to engage a broad demographic. Focus groups can gain in depth information but are time consuming and will only involve a small number of people. Questionnaires are often poorly completed or have a low return rate. Asking a ‘question of the month’ is a simple method of collecting a large amount of data on a single topic. We have recently reinstated patient education sessions following a break in their provision due to Covid-19 and were keen to optimise these by finding out what patients wanted. Methods A poster was place in the waiting room asking ‘What was the most useful information you were given when you were diagnosed with arthritis’. Patients were given a post-it note and asked to write an answer on it and stick it onto the poster. The same question was asked in paediatric, adolescent and general adult clinics to all follow up patients with inflammatory arthritis during September 2023. The question of the month concept was developed by the Rubis QI Team as part of the JIALearn project. Thematic analysis was performed for the three age groups separately. Results Themes common to all age groups: 1. Knowing that there are a range of effective treatment options available. 2. Hope: “it is not life changing”, “expect a long journey and try and remain positive” In paediatrics, effective, accessible communication was a dominant theme in “any concerns are important and are listened to” In adolescents and young adults being given hope and knowing that there were treatment options were the most dominant themes: “it doesn’t have to affect your life”, “knowing I had different choices out there to help me” In adults the most dominant theme was around self management, particularly around regular exercise, diet and weight loss. “I can manage my problem”, “allows you to stay in control”. Some patients used this project as an opportunity to leave positive feedback about their care to date. Clinicians involved reported good clinical engagement and an improved therapeutic relationship with patients. These themes will be used to guide future patient education sessions targeted at specific age groups. The results of this project will be developed into a poster for the waiting room to demonstrate that comments have been listened to and acted upon. Conclusion Adults newly diagnosed with arthritis place a strong emphasis on self management. The themes of open communication and hope emerged in children and young adults. The comments provided will help to guide and shape future patient education sessions. Question of the month is a simple way of engaging with a broad number of patients. Disclosure R. Waller: None.

Abstract:The BJD has been publishing clinical practice guidelines (CPG) in dermatology for more than 20 years.1‐3 With the increasing adoption of gold‐standard guideline development methodology and reporting checklists, such as the GRADE4 approach and the AGREE II5 toolkit, recent CPGs published in the BJD are of high‐quality and are highly accessed, with CPGs making up half of the top 10 downloaded BJD articles in 2019. As a group of guideline developers and journal editors associated with the BJD, we were interested in how the readership perceives and uses CPGs. The objective of this study was to survey the BJD readership's demographics, utility of and attitudes towards CPGs.

Abstract:Abstract Recent developments in the understanding of the pathophysiology of inflammatory skin diseases have resulted in the emergence of new treatments. However, new therapies are frequently accompanied by new side effects. In the November issue of the Journal, we explore recent advances on how Cutibacterium acnes phylotypes are involved in biofilm formation and its significance in therapy resistance. The Janus kinase (JAK) inhibitor‐induced acneiform eruptions also raise intriguing questions about the complex relationship between JAK inhibition and acne pathophysiology. We also provide real‐world data on recent therapies for atopic dermatitis, hyperkeratotic hand and foot dermatitis, and generalized pustular psoriasis.

Matching researchers' needs and patients' contributions: practical tips for meaningful patient engagement from the field of rheumatology

Abstract:There is an increasing recognition of the importance of patient engagement and involvement in health research, specifically within the field of rheumatology. In general, researchers in this specialty appreciate the value of patients as partners in research. In practice, however, the majority of researchers does not involve patients on their research teams. Many researchers find it difficult to match their needs for patient engagement and the potential contributions from individuals living with rheumatic disease. In this Viewpoint, we provide researchers and patients practical tips for matching 'supply and demand,' based on our own experiences as patient engagement consultants and trainers in rheumatology research. All authors started as a 'naïve' patient or caregiver, an identity that evolved through a process of 'adversarial growth': positive changes that are experienced as a result of the struggle with highly challenging life circumstances. Here, we introduce four stages of adversarial growth in the context of research. We submit that all types of patients have their own experiences, qualities and skills, and can add specific input to research. The recommendations for engagement are not strict directives. They are meant as starting points for discussion or interview. Regardless of individual qualities and knowledge, we believe that all patients engaged in research have a single goal in common: to contribute to research that ultimately will change the lives of many other patients.

Abstract:Plain language summaries (PLSs) are being increasingly implemented and required by regulatory bodies to improve accessibility of scientific research for the general population. We assessed whether the British Journal of Dermatology (BJD) PLSs over 12 issues have been written at a suitable readability level for the general population, based on published guidelines. We found that authors may benefit from increased training or guidance in the production of these worthwhile texts, to further promote patient engagement with the BJD.

Abstract:The EBMT (European Blood and Marrow Transplantation Society) aims to connect patients, the scientific community, and other stakeholders to improve hematopoietic stem cell transplantation and cellular therapy outcomes. We performed a cross-sectional online survey to understand the perceptions regarding Patient Reported Outcomes (PROs) and Patient Active Involvement in Research (PAIR) in over 800 stakeholders ( n = 813). Patients ( n = 278) and health care professionals (HCPs) ( n = 351) were compared. We observed high openness for EBMT PRO collection ( n = 680, 84.5% across stakeholders' groups; patients n = 256, 93.1% versus HCPs n = 273, 78.4% [ p < 0.001]) and PAIR ( n = 702, 87.3% across stakeholder groups; patients n = 256, 92.4% versus HCPs n = 296, 85.8% [ p = 0.009]), with a significantly higher proportion of patients expressing interest compared to HCPs. Priority domains for PROs data-collection identified were the assessment of symptom experience, psychosocial and cognitive functioning. The most important issues for patients specifically were the data-collection of PROs reflecting cognitive function, the option of reporting data at home, the importance of identifying actionable targets to improve their recovery, and receiving feedback on their input when participating in research projects. Our multistakeholder approach suggests an added value to embracing patient engagement in the development of meaningful research and service design within the transplantation and cellular therapy community.

Patient engagement in designing and publishing research in prostate cancer: a scoping review

Abstract:Patients with cancer have the unique ability of being able to offer valuable insights into how cancer therapeutics may impact the overall patient experience and improve clinical outcomes. Patient engagement could therefore contribute to tailoring treatment strategies and research design according to patient needs. This study evaluated patient engagement in prostate cancer research by identifying patient input in the prostate cancer literature. We performed a keyword cluster analysis of articles from multiple databases and congresses in which patients provided input on disease management or were involved in study design, manuscript authorship or presentation of results (patient voice). In total, 112 studies were included. Patients were involved in the design of 11 studies and were credited as authors in four studies. This review suggests a lack of meaningful patient involvement in prostate cancer research and publications. Patients with cancer have first-hand knowledge of what does and does not work for their care. Therefore, their voice is valuable to help improve treatment and guide research. Our goal was to find prostate cancer articles with patient input. We searched databases using keywords related to patient voice. We looked for articles involving patients in designing, writing or presenting the study. Only four out of the 112 articles we identified were published in journals focused on involving patients. Eleven articles involved patients in designing the study. Four articles involved patients in writing the published work. Overall, we did not find many articles where patients had a meaningful role in the study. Prostate cancer treatment and research will likely benefit from more patient input.

Patient journey and disease‐related digital media usage: A cross‐sectional study among dermatology patients across Germany

Abstract:Summary Background and Objectives This study aimed to fill the gap of evidence‐based data by examining the impact of health‐related digital media use's impact on the patient‐physician relationship and patient journey, with the goal of driving positive changes in office‐based dermatological practice. Patients and Methods German individuals with skin conditions completed a questionnaire assessing health‐related digital media use, eHealth literacy, impact of digital media use on the patient‐physician relationship and patient journey, and sociodemographic and health characteristics from June to November 2022, followed by descriptive analysis and linear regressions. Results Overall, 919 participants were analyzed (median age: 47.00, interquartile range [28.00; 61.00] years, female: 53.6%, health‐related digital media users: 56.8%). Google and online encyclopedias were commonly used before and after dermatological consultations. Online self‐help groups were used by 75% of non‐treated participants. Digital media use had no impact on the patient‐physician relationship and a positive impact on the patient journey. Positive impacts were associated with higher eHealth literacy, satisfaction with digital media quality, importance of digital media, rural residence, high education, and trust in dermatologists. Conclusions The potential of health‐related digital media in office‐based dermatological care necessitates the drive of an evidence‐based digital health platform to improve eHealth literacy and satisfaction with the quality of digital media among affected individuals.
Impact of Digital Media on the Patient Journey and Patient-Physician Relationship Among Dermatologists and Adult Patients With Skin Diseases: Qualitative Interview Study

Abstract:Background Digital media are easily accessible without time restrictions and are widely used for health- or disease-related purposes. However, their influence on the patient journey and the patient-physician relationship has not yet been sufficiently investigated. Objective This qualitative interview study was designed to explore dermatologists’ and patients’ experiences with digital media for medical purposes in the context of patient journeys and patient-physician relationships. Methods Twenty-eight semistructured video conference–based interviews were conducted and audiorecorded by experienced interviewers between November 2021 and June 2022 in Germany. Eligible patients were those who were aged ≥18 years, were affected by at least one physician-confirmed skin disease, and were fluent in the German language. The eligibility criterion for dermatologists was that they were currently practicing dermatology in an outpatient setting or in a hospital. Randomly selected dermatologists from the listing of the German National Association of Statutory Health Insurance Physicians and dermatologists from personal academic and professional networks were invited for participation via postal mail and asked to identify potential patient volunteers from their patient bases. All recorded data were pseudonymized, fully transcribed verbatim, and subsequently analyzed according to Mayring’s qualitative content analysis by 2 researchers, allowing for both a qualitative interview text analysis and a quantitative assessment of category assignments. Results In total, 28 participants were interviewed: 16 adult patients and 12 dermatologists. Eight main categories emerged as key areas of interest: (1) the search for diagnosis and symptom triggers, (2) preconsultation digital media use, (3) in-depth information and exchange with other patients, (4) self-treatment, (5) patient-physician interaction, (6) roles of dermatologists and patients, (7) patient eHealth literacy, and (8) opportunities and risks. Categories 1 and 2 were only coded for patients; the other categories were coded for both patients and dermatologists. Patients reported searches for diagnosis or treatment options were most frequently (8/16) caused by a mismatch of symptoms and diagnosis or dissatisfaction with current therapies. Concerns regarding a potentially severe diagnosis prompted searches for initial or in-depth information before or after dermatological consultations. However, the large volume of information of varying quality often confused patients, leading dermatologists to assume the role of evaluating information from preinformed patients. Dermatologists generally encouraged the use of digital media, considered teledermatology advantageous, and viewed big data and artificial intelligence as being potentially beneficial, particularly when searching for rare diagnoses. A single, easily accessible, and free-of-charge platform with high quality information in lay language was recommended by the dermatologists and desired by patients. Conclusions Digital media are widely accepted by both patients and dermatologists and can positively influence both the dermatological patient journey and patient-physician relationship. Digital media may therefore have great potential to improve specialized health care if patients and dermatologists embrace their new roles.
