Abstract:Telemedicine is the use of telecommunications technology to support health care at a distance. Technological advances have progressively increased the ability of clinicians to care for diverse patient populations in need of skin expertise. Dermatology relies on visual cues that are easily captured by imaging technologies, making it ideally suited for this care model. Moreover, there is a shortage of medical dermatologists in the United States, where skin disorders account for 1 in 8 primary care visits and specialists tend to congregate in urban areas. Even in regions where dermatologic expertise is readily accessible, teledermatology may serve as an alternative that streamlines health care delivery by triaging chief complaints and reducing unnecessary in-person visits. In addition, many patients in the developing world have no access to dermatologic expertise, rendering it possible for teledermatologists to make a significant contribution to patient health outcomes. Teledermatology also affords educational benefits to primary care providers and dermatologists, and enables patients to play a more active role in the health care process by promoting direct communication with dermatologists.

Abstract:Journal of Cutaneous Medicine and Surgery, Ahead of Print. Acne vulgaris is a common dermatological condition requiring individualized management. Teledermatology provides convenience and accessibility that is highly suitable for this dermatological condition. Herein, our review aimed to describe the current state of teledermatology in the context of acne management and to assess patient satisfaction, adherence to virtual care, and the experiences of health care providers in delivering remote dermatological services. A systematic search for articles was conducted in Medline, Embase, and PubMed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews. Title, abstract, full-text screening, and data abstraction were carried out in duplicate. One thousand one hundred three nonduplicate articles were screened based on title and abstract review. A total of 21 studies were included in the review. It was found that teledermatology is well-suited to the effective management of acne vulgaris. When compared to in-person care, teledermatology resulted in similar outcomes on several acne-grading scales. Additionally, patient satisfaction was comparable, with a large proportion of patients preferring virtual care to traditional in-person visits. However, compliance was found to be lower for virtual care. There are several secondary benefits to telemedicine, including time savings and greater accessibility to care for rural patients. Teledermatology is an evolving, promising medium for acne management for both clinicians and patients. Future research comparing the effectiveness of different teledermatology platforms, its limitations and pitfalls, and integration of patient and physician preferences to improve treatment outcomes is warranted.

Abstract:Background and aims: Telemedicine, with Teledermatology, has become a central component of modern medicine. Its importance, especially during the COVID-19 pandemic, underlines its potential to optimize access for dermatological needs. The study aims to assess the potential of teledermatology, understand the importance of linking digital and physical practices, and analyze the adoption of online services based on participants' demographic and experiential factors. Methods: This cross-sectional survey was conducted among users of the telemedicine platform from July 2022 to March 2023. The platform ("OnlineDoctor") allows users to contact dermatologists for remote dermatological consultations. The survey included questions about the participants' dermatological concerns, their reasons for using teledermatology, their satisfaction with the recommendations and their willingness to continue using telemedicine in the future. Data was collected via the RedCap online platform. Descriptive statistics and regression analyses were carried out. Results: Overall, 1141 people participated in the study (mean age 44.0 years [SD 14.6], 61.4% women). Results showed that 52.7% of participants with skin conditions had not consulted a dermatologist in the previous year. Shorter waiting times and the lack of face-to-face appointments were the main reasons for using the online platform. In total, 77.6% (n = 885) of participants indicated they would use teledermatology as their first choice if they had an upcoming skin condition. Age, gender, and satisfaction with previous consultations impacted the use of teledermatology as the first choice for future skin conditions. Conclusion: Teledermatology is characterized by various benefits, including reduced waiting times and improved accessibility to treatment. Nevertheless, the study underscores the importance of a hybrid care approach with direct interaction with the physician. Teledermatology can be transformative in meeting dermatologic needs, mainly when traditional face-to-face consultation is limited. A deep understanding of user preferences and widespread adoption of digital services can pave the way for the successful adoption of teledermatology platforms, improving healthcare accessibility and efficiency.
Virtual Grand Rounds in Dermatology: an 8-year experience in web-based teledermatology

Abstract:Background: Teledermatology utilizes telecommunication technologies for the exchange of dermatologic information across distances. In 2000, we developed the Virtual Grand Rounds in Dermatology (VGRD) as a free, user-friendly platform for dermatologists to present complex dermatologic cases to the international community for clinical feedback, consultation, and continuing education. Methods: VGRD was designed using simple graphics to facilitate access in remote and developing geographic regions. Members present their case according to a recommended template and are encouraged to provide clinical and histopathologic photographs. Viewers' comments are posted on the site within 24-48 h following receipt. Results: VGRD's low-cost store-and-forward system allows easy accessibility to expertise for colleagues around the world. VGRD currently comprises a community of 344 clinicians across all dermatologic subspecialties, and links users to cases as well as a recent interactive addition for more urgent feedback: VGRD Blog. The Maine Virtual Skin Clinic is another sister site developed as an educational tool for physicians and residents in primary care. VGRD can be found at http://www.vgrd.org. Conclusions: With its easy-to-use platform for teleconsultation services, VGRD has the potential to enhance clinical outcomes and education. In the next few years, we hope to continue to expand VGRD, including launching a comprehensive search engine in VGRD to provide users with a "one-stop resource." With the growth of teledermatology, we will combine web consultations with web-based education into a single resource for dermatologists interested in new possibilities in clinical practice, patient care, and continuing education.

Abstract:Dermatology is a clinical and visual discipline, which makes it the quintessential medical specialty for spot diagnosis and telemedicine. The COVID-19 pandemic has led to an unprecedented worldwide renaissance of teledermatology (TD). It has helped deliver high-quality medical care, while protecting the medical personnel and vulnerable patients from potential infection. Examining a patient from a distance through digital photography has many drawbacks, including lack of physical touch, difficulties in performing full body examinations, and several legal and ethical issues. We summarize have summarized the more common pitfalls and highlight the key aspects of direct patient-to-physician TD. Basic practical advice includes the use of TD for obtaining patient history, examining patient-captured photographs for inflammatory skin disease, and skin cancer screening.

Abstract:BACKGROUND: During the global COVID-19 pandemic, dermatologists increasingly adopted teledermatology to facilitate patient care.OBJECTIVE: To identify differences in teledermatology platform usage and functionality among dermatologists as a means of understanding the potential effect on virtual healthcare access.METHODS: Results from a 2021 cross-sectional pre-validated survey distributed to actively practicing United States dermatologists were analyzed based on timepoint when teledermatology was adopted relative to COVID-19, previous/currently used platforms, self-reported platform functionality, and barriers to teledermatology implementation. Analysis was performed using chi-square and odds ratios (OR) with 95% confidence intervals (95% CI) for categorical data and single-factor analysis of variance (ANOVA) with post-hoc Tukey-Kramer for continuous data. P<.05 was considered significant.RESULTS: Early adopters (EAs) trialed significantly more (2.3 vs 1.9, P=0.02) platforms than (post) COVID adopters (CAs) before choosing their current platform. More EAs reported using platforms capable of uploading images (P=.002), required a mobile application (P=.006), and allowed staff to join patient encounters (P<.001). While poor image quality was the most cited barrier to implementation, CAs and non-adaptors (NAs) were materially more likely to cite it as their largest barrier to teledermatology.LIMITATIONS: The retrospective nature of the study and potential response bias.CONCLUSION: Dermatologists' use of teledermatology materially correlates with their teledermatology-adoption timepoint, and future usage may be materially impacted by the end of the COVID-19 public health emergency. Abstract:Introduction: Teledermatology utilises telecommunications technology to deliver dermatological care remotely, incorporating live video consultations, store-and-forward systems, and hybrid approaches. It is particularly valuable in underserved or remote areas with limited access to dermatologists. Reported benefits include reduced face-to-face consultations for benign lesions, leading to increased capacity for severe cases, improved access for rural patients, and enhanced satisfaction among clinicians and patients. The COVID-19 pandemic accelerated the adoption of teledermatology, integrating it into the National Health Service (NHS) framework for managing referrals and ensuring continuity of care. This study examines the outcomes of two-week wait referrals for suspected skin cancer, focusing on diagnostic concordance between teledermatology and histopathology. Materials and methods: The study was conducted at Addenbrooke's Hospital, part of Cambridge University Hospitals, via a retrospective review of patient records from November 2022 to May 2023. Inclusion criteria were all patients referred by their general practitioner (GP) under the two-week wait for suspected skin cancer pathway. Data collected included patient demographics, waiting times, clinical and histological diagnoses, and patient re-referrals for the same problem. The primary objective was to assess diagnostic concordance between the clinical diagnosis from teledermatology and histopathology. Secondary objectives included accuracy of lesion site description, patient waiting times, and computed time savings from the use of teledermatology. Results: The study covered 71 patients (34 males, 37 females) aged 19-87 years (mean: 59.63), with Fitzpatrick skin I-III predominating. A total of 110 individual lesions were assessed, and 46 required surgical management. Clinical and histological concordance was 62%, with 100% accuracy for basal cell carcinoma (BCC) and melanoma. The service saved 10 hours of consultant time and reduced the need for 62 initial face-to-face consultations. Lesion site documentation had a 73% correlation between GPs and dermatologists. Diagnoses varied widely between GPs and dermatologists, with a 31% concordance. Conclusion: Our study shows that teledermatology is a safe and effective method for managing two-week wait referrals for suspected skin cancer, reducing footfall, and saving time and costs for both clinicians and patients. While there are limitations, the usage of teledermatology allows increasingly limited capacity for face-to-face consultations to be reserved for high-risk patients. Further studies in different regions should explore teledermatology's utility across diverse demographics, particularly to address healthcare disparities for those with darker skin tones.

A thorough research of the current literature was carried out by using several databases (PubMed, Google Scholar, Cochrane Skin, Embase, EBSCO and MEDLINE) (until December 31, 2023) through the use of the following keywords: "telemedicine", "teledermatology", "artificial intelligence", "dermatology", "ChatGpt". In particular, articles regarding the use of teledermatology were screened. Meta-analyses, reviews, and real-life studies were considered, selecting the most relevant documents. Thus, the research was advanced by reviewing the abstracts and the texts of collected articles, as well as the references were also reviewed to include articles that could have been missed. Only English language manuscripts were considered. This article is based on previously conducted studies and does not contain any studies with human participants or animals performed by any of the authors. The first teledermatological experience was reported in Norway in 1993, in a manuscript describing the growing use of telemedicine in various specialties in this country. 5 However, the first publication about the use of teledermatology reported on PubMed date back to 1995. The first paper is from the United States (US), where the demand for dermatologists was particularly high. Abstract:Abstract Dermatology departments actively use digital solutions to provide services remotely, particularly to supplement overbooked face-to-face clinics. In coastal areas, there are additional challenges regarding provision of access to dermatology, with few centres offering dermatology appointments, and with patients not always having the means to travel. This paper discusses how the teledermatology service in one of the coastal ‘deserts of provision’ both widens access to secondary care and unmasks unmet need in these communities. The methods used involve direct comparison of the number of dermatology referrals from primary care, both before the establishment of the teledermatology service and afterwards. The teledermatology service examined has been established in coastal locations, offering photography and dermoscopic images of lesions, submitted with a standardized patient history form. The main focus of the service is to identify skin cancers and provide reassurance of benign lesions, without recourse to 2-week-wait appointments, although other skin conditions suitable for general clinics are also referred. Interestingly, the results show that the teledermatology service has had no impact on mitigating traditional referrals to routine or 2-week-wait clinics, which have continued to increase, on average by 18% in the 2-year period ending January 2024, across 24 surgeries in the region. For one surgery located in one of the most isolated areas, weekly referrals have increased by 142%, representing an increase from three to seven referrals per week. This centre also made an additional eight referrals to teledermatology per week, mainly for single lesions that general practitioners felt did not merit the 2 week wait, but that required timely dermatological opinion. Of 97 referrals examined across a 12-week period (November 2023 to January 2024), five lesions were listed directly for surgical excision, eight for biopsy and eight for further clinical evaluation. It seems evident that in providing this additional path of access, the teledermatology service has unmasked an unmet need in the coastal community. It offers a ‘dermatology department without walls’, where expert secondary care advice with consultant insight can be obtained locally, and surgical follow-up or further clinical management can be organized directly by the advising dermatology team. Further study is needed to confirm that this increased access also leads to improved patient outcomes, which is predicated on appropriate referrals to teledermatology by primary care, and assessment of the diagnostic accuracy of teledermatology compared with traditional in-person clinics. The hope is that effective dermatology specialty care is being brought to the patient more directly in coastal communities than has previously been possible.

An Exploration of Dutch Dermatologists' Experience and Satisfaction With Teledermatology: Sociotechnical and Complex Adaptive System Perspective

Abstract:Background: Despite the global upscale of teledermatology during the COVID-19 pandemic, persistent barriers, such as the poor anamnesis and photo quality, hinder its effective use in practice. Understanding Dutch dermatologists' experiences and satisfaction with using the teledermatology system in the Dutch health care system is needed. A holistic evaluation may provide valuable insight to understand how barriers interrelate which is deemed necessary for the innovation of teledermatology in practice. Objective: Guided by a complex adaptive system perspective, this study aims to understand Dutch dermatologists' experience and satisfaction with their training, support communication, interaction, and usage of a teledermatology platform of a Dutch digital hospital during the COVID-19 pandemic, uncovering insights to improve teledermatology services for the future. Methods: A web-based questionnaire was sent in December 2021 to Dutch dermatologists who (1) had an active teledermatology platform account, and (2) responded to a teledermatology consultation between October 1, 2019, and September 30, 2021. The questionnaire consisted of the validated Store-and-Forward Telemedicine Service User-satisfaction Questionnaire (SAF-TSUQ) questionnaire, and new questions regarding; demographics of teledermatologists, the use of teledermatology during the COVID-19 pandemic, the performance of teledermatology by general practitioners (GP), and the role of dermatologists in the teledermatology process. The open-ended questions were analyzed by a grounded theory approach guided by a sociotechnical model and complemented by a complex adaptive system perspective. A panel discussion with 3 dermatologists was performed to provide additional insight into the responses to the questionnaire. Results: We obtained responses from 25 out of the 249 (10%) invited dermatologists. Overall, dermatologists had a positive experience with teledermatology. Interestingly, teledermatology use frequency remained unaffected by the COVID-19 pandemic. However, the insufficient quality and incompleteness of the clinical content (photos and anamneses information) of the teledermatology consultation impacted the efficiency of the teledermatology workflow. Dermatologists expressed the need for improvement to avoid time-consuming processes or physical referrals. The panel discussion enriched and confirmed the responses, suggesting solutions like mandatory fields for the GPs for a complete anamnesis. Conclusions: Dutch Dermatologists view teledermatology as a valuable tool to provide access to dermatology care. However, improvements regarding the quality and completeness of the provided clinical content are necessary for the effectiveness and efficiency of the complex teledermatology system in Dutch health care. This could increase both the dermatologists' satisfaction and the quality of teledermatology services. Managing trade-offs, such as time investments versus image quality, is crucial for teledermatology implementation and should be assessed from a complexity perspective to understand trade-offs and prevent unintended consequences.
Insights, Advantages, and Barriers of Teledermatology vs. Face-to-Face Dermatology for the Diagnosis and Follow-Up of Non-Melanoma Skin Cancer: A Systematic Review

Abstract:Background: Teledermatology is employed in the diagnosis and follow-up of skin cancer and its use was intensified during and after the COVID-19 pandemic. At the same time, demographic changes result in an overall increase in non-melanoma skin cancer and skin precancerous lesions. The aim of this study was to elucidate the role of teledermatology in comparison to conventional face-to-face dermatology for such lesions and determine the advantages and limitations of this workflow for patients and physicians. Methods: Research was performed using relevant keywords in MEDLINE and CENTRAL. Relevant articles were chosen following a predetermined standardized extraction form. Results: Diagnostic accuracy and interrater/intrarater agreement can be considered comparable—although lower—than in-person consultation. Improvement of particular features such as image quality, medical history availability, and teledermoscopy can further increase accuracy. Further aspects of limitations and advantages (mean time-to-assessment, time-to-treatment, cost-effectiveness) are discussed. Conclusions: Teledermatology has comparable diagnostic accuracy with face-to-face dermatology and can be utilized both for the effective triage of non-melanocytic epithelial tumors and precancerous lesions, as well as the follow-up. Easy access to dermatologic consultation with shorter mean times to diagnostic biopsy and/or treatment coupled with cost-effectiveness could compensate for the lower sensitivity of teledermatology and offer easier access to medical care to the affected populations.

Emerging Trends in Teledermatology Research: A Scientometric Analysis from 2002 to 2021

Abstract:Background: With advances in technology, teledermatology (TD) research has increased. However, an updated comprehensive quantitative analysis of TD research, especially one that identifies emerging trends of TD research in the coronavirus disease 2019 (COVID-19) era, is lacking. Objective: To conduct a scientometric analysis of TD research documents between 2002 and 2021 and explore the emerging trends. Methods: CiteSpace was used to perform scientometric analysis and yielded visualized network maps with corresponding metric values. Emerging trends were identified mainly through burst detection of keywords/terms, co-cited reference clustering analysis, and structural variability analysis (SVA). Results: A total of 932 documents, containing 27,958 cited references were identified from 2002 to 2021. Most TD research was published in journals from the "Dermatology" and "Health Care Sciences & Services" categories. American, Australian, and European researchers contributed the most research and formed close collaborations. Keywords/terms with strong burst values to date were "primary care," "historical perspective," "emerging technique," "improve access," "mobile teledermoscopy (TDS)," "access," "skin cancer," "telehealth," "recent finding," "artificial intelligence (AI)," "dermatological care," and "dermatological condition." Co-cited reference clustering analysis showed that the recently active cluster labels included "COVID-19 pandemic," "skin cancer," "deep neural network," and "underserved population." The SVA identified two reviews (Tognetti et al. and Mckoy et al.) that may be highly cited in the future. Conclusion: During and after the COVID-19 era, emerging trends in research on TD (especially mobile TDS) may be related to skin cancer and AI as well as further exploration of primary care in underserved areas.

Teledermatology in Atopic Dermatitis: A Systematic Review

Abstract:Journal of Cutaneous Medicine and Surgery, Ahead of Print. Telemedicine use has been increasing especially during the COVID-19 pandemic. Various studies have outlined benefits of telemedicine including improving health equity, reducing wait times, and cost-effectiveness. Skin diseases such as atopic dermatitis (AD) may potentially be managed via telemedicine. However, there are no evidence-based recommendations for best practices in telemedicine for assessing AD patients. The objective of this review is to assess and summarize current evidence on telemedicine modalities for AD. This review will assess patient outcomes from various telemedicine models for AD. A review protocol was developed according to the Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) statement. Two reviewers independently screened potential studies and extracted data. Studies were included if they evaluated any telemedicine assessment for AD. Of 2719 identified records, 5 reports were included. Two reports used the direct-access online model, 1 used web-based consultation, 1 used e-health through a personal eczema portal, and 1 used an online platform and mobile application. All models were variations of the asynchronous, store and forward model. In all the included reports, teledermatology for the follow-up of patients with AD was effective and equivalent when compared to in-person appointments or standard treatment for their respective key outcome measures. However, it is unclear what the most effective teledermatology model is due to significant heterogeneity between studies. Teledermatology may serve as an important tool for triaging and follow-up of patients with AD. More studies are needed to determine which teledermatology models are most effective for virtual assessment of AD.

Teledermatology in practice: Report of Mayo Clinic experience

Abstract:Background Delivery of dermatologic care through telemedicine was accelerated by the COVID-19 pandemic. We sought to analyze the teledermatology experience across Mayo Clinic's health care system to identify strengths and limitations of teledermatology. Methods Electronic health records of dermatology televisits were reviewed from multiple U.S. Mayo Clinic sites from January 2020 through January 2021. Results A total of 13,181 dermatology televisits were conducted in 6468 unique patients. Patients were primarily female (60.2%), and mean age of all patients was 34.1 years. Synchronous / live video conferencing visits were the most common (40.0%) telecare modality. Synchronous / live audio conferencing and asynchronous / store-and-forward visits comprised 33.0% and 27.0% of appointments. In total, 3944 televisits (29.9%) were successfully concluded via a single appointment. An in-person appointment was needed for 1693 patients (26.2%) after their initial televisit. For patients with a single televisit, synchronous / live video conferencing was the most common virtual modality (58.0% vs 32.2% of patients with multiple visits, p < 0.001). Patients needing in-person follow-up visits were slightly older than those who did not (mean [SD], 38.8 [22.3] vs 35.0 [23.6] years; p < 0.001) but without any sex-based difference. Around one-third of patients needed an in-person follow-up visit after their initial asynchronous / store-and-forward visit which was higher when compared with synchronous / live audio and video conferencing. Conclusion Single dermatology televisits effectively managed nearly one-third of patients who did not require in-person follow-up. An initial synchronous / live video conferencing was more likely to yield a single clinical encounter, whereas asynchronous / store-and-forward visits required more in-person follow-up. Future studies are required that focus on dermatology-specific cost, diagnoses, access, quality of care, and outcomes.

The clinical utility of teledermoscopy in the era of telemedicine

Abstract:<p>Teledermoscopy is a novel diagnostic tool for the prevention, diagnosis, and treatment of skin disease when direct visualization of lesions is difficult. It is an economically viable option that can complement telehealth visits and that providers can utilize to identify melanocytic lesions and optimize care with diagnostic accuracy comparable to FTF diagnosis. Teledermoscopy is invaluable in monitoring chronic conditions that require frequent follow‐up and treatment optimization. Inclusion of clinical and dermoscopic images has been shown to improve the diagnostic accuracy of teledermatology services, thereby reducing healthcare costs. Teledermoscopy is also non‐discriminatory, as diagnostic accuracy is similar in lighter and darker skin types. It has been shown to improve patient access to specialty services and reduce the number of "no‐shows" at FTF clinics and length of surgery waiting times. Mobile teledermoscopy is user‐friendly, feasible, and economically viable, as inexpensive mobile dermatoscopes have emerged on the market to reduce consumer out‐of‐pocket costs. Research is limited on teledermoscopy's utility in diagnosing pre‐cancerous and cancerous skin lesions in adults, particularly complex pigmented lesions. Further research is recommended to investigate the role of dermoscopic expertise and artificial intelligence on the evaluation of teledermoscopic images.</p><p>This article is protected by copyright. Evolving teledermatology policy and reimbursement landscape in the United States

Abstract:Background: Policymakers and payers are reevaluating the temporary telehealth flexibilities granted during the COVID-19 public health emergency, which will shape future teledermatology utilization.Objective: To summarize the recently expanded telehealth flexibilities in the United States, projected changes, and corresponding implications for dermatologists.Methods: Narrative review of the literature, United States policies and regulations, and white paper reports.Results: Key telehealth flexibilities included expansion of payment parity, relaxation of originating site requirements, loosening of state licensure requirements, and HIPAA (Health Insurance Portability and Accountability Act of 1996) enforcement discretion. These changes enabled widespread accessibility and adoption of teledermatology, which enhanced high-quality and cost-effective dermatologic care. Most waivers will end 151 days following the end of the public health emergency declaration. Notably, asynchronous telehealth was not included in the reimbursement expansion.Limitations: Only policies and regulations through December 2022 are included.Conclusion: It will be important for the field of dermatology to stay abreast of the upcoming changes in telemedicine policies and reimbursement, to demonstrate teledermatology's value through evidence-based studies and to advocate for enduring policies that will promote the accessibility of teledermatology for patients.
Teledermatology Versus In-Person Visits for the Follow-Up of Atopic Dermatitis Patients

Abstract:Journal of Cutaneous Medicine and Surgery, Ahead of Print. Background:In recent years, teledermatology has rapidly emerged as a healthcare delivery method with potential implications for managing chronic inflammatory dermatoses like atopic dermatitis (AD).Objectives:This study assesses the utility of telemedicine in the management of AD by comparing virtual care with traditional in-office visits with the aim of identifying differences in clinical outcomes between these 2 healthcare delivery modalities.Methods:Patients of all ages with AD were recruited from 2 dermatology practices. Consecutive patients presenting to the clinics who met the inclusion criteria were invited to enrol in the study. Those who consented to participate were randomly assigned to the virtual or in-person arm of the study, with the opportunity to decline care in either study arm. The inclusion criteria required participants to have a confirmed diagnosis of AD. Exclusion criteria included significant comorbidity that might affect the course of treatment, inaccessibility to teleconsults such as not having a camera for video conferences, and self-declared limitations in operating Zoom. Patients were assessed at baseline (week 0), 4 to 6 weeks, and 8 to 12 weeks using 6 efficacy parameters.Results:In the virtual group, all 6 dermatological measures suggested improved outcomes. Average Body Surface Area scores decreased (β = −.07, 95% CI = −0.1, −0.3) over the course of follow-up. Virtual care patients had 80% lower odds of moderate-to-severe uncontrolled disease (OR = 0.2; 95% CI = 0.06, 0.5) and pruritus (OR = 0.2, 95% CI = 0.05, 0.7) over time.Conclusions:This study supports teledermatology as a feasible and effective option for providing follow-up care for atopic dermatitis patients of various demographic standings.

Keywords: teledermatology, resident training, research tool, dermatology, COVID-19, residency training In Romania, an emergency state has been declared, with a series of military ordinances being published during the previous months in order to clear hospital beds by only admitting urgent cases and limiting resident physicians' exposure to the circulating virus by implementing shift work with reduction in the total number of in-clinic hours. All the while, it remained possible that resident physicians could be called in to fulfill other necessary activities outside of our current module in order to help combat COVID infection. 1,2 This inherently impacts the clinical activity in Dermatological practices throughout all sectors. Our department, a subunit of the Clinical Infectious Disease hospital "Sfânta Cuvioasa Parascheva" in Galati, was moved to another hospital in order to facilitate the availability of beds for COVID patients. Initially, the hospital in question was the Railroad Hospital, but after this was also charged with providing intensive care for COVID patients, our department was once again relocated to another hospital, the Psychiatry Hospital, in a common ward with our colleagues from the infectious disease hospital who were treating non-COVID cases. Under these circumstances, our service took the decision of implementing teledermatology in order to act both as a filter for reducing in-hospital admittance or face-to-face interactions, and as a straightforward solution to solve the issues faced by patients and family doctors within our area. This was put into practice by using dedicated emails and Skype services, and we are currently instituting Zoom as an alternative method of communication to try and maximize patient access by diversifying the means through which we can be contacted. This embodies opportunities for resident physicians to consult and treat multiple clinical cases from a distance, without having the constraint of seeing only the typical hospital admittances. Live consultations rely on the resident physician's capacity to take a detailed patient history in the limited time allotted, while the subsequent case-discussion with the leading-physician has good didactic value. Positive feedback was received from the resident physicians, and by using this method, we were also able to meet with the residency program requirements. During the first 7 days, when our service was still rather unknown to the public, we registered 107 teleconsultations. In this timeframe, the legislation was changed in order to allow consultations to be given both to the insured and uninsured. We believe teledermatology is an important method, both for strengthening the collaboration with physicians outside of the dermatology area, and for accommodating efficient monitoring of the patients' evolution and treatment effectiveness. From our department's experience, the first 26 cases that have been consulted through teledermatology have had good treatment response, confirmed by patient feedback through emails. Two of these cases had difficulties accessing their family doctor, and they would certainly have been unable to access further specialist care (one patient was from the rural area, and the other has nosocomephobia). The advantage of this technique stems from the fact that most patients possess camera cell phones with internet access, facilitating access to the teledermatology platform. Abstract:Abstract Teledermatology has been shown to be accurate in the diagnosis of skin cancer. The number of skin cancer referrals to secondary care has surged and dermatology departments are under increasing pressure to meet cancer pathway targets. The aim of this study was to compare service outcomes for teledermatology vs. conventional face-to-face review. We performed a retrospective review of adult patients referred by general practitioners for suspected skin cancer via the 2-week-wait pathway from August 2022 to February 2023. Patients were assessed either by conventional face-to-face appointment or via teledermatology. Teledermatology patients completed a questionnaire and were seen by a photographer in the community who took regional, macroscopic and dermoscopic images. The primary outcomes were times from referral to first clinical appointment and to surgery (if applicable), surgical intervention rate, and histological diagnoses (if applicable). Statistical analysis was performed using Prism software (GraphPad Software, La Jolla, CA, USA). Comparisons between groups were performed with the Mann–Whitney U-test and Fisher exact, as appropriate. P-values < 0.05 were considered statistically significant. In total, 50 consecutive patients assessed by a single clinician via teledermatology were compared with 100 randomly selected patients assessed by the same clinician via face-to-face review. More than half of patients were female in both groups (52% teledermatology vs. 58% face-to-face). The teledermatology group was older ,with a mean age of 56.7 years (SD 18.5) vs. 40 years (SD 20.5), P < 0.001. The median waiting time for a clinical appointment was slightly longer in the teledermatology group [16.5 days (SD 8.5) vs. 14 days (SD 5.4) for face to face (P < 0.001)]. The median waiting time from receipt of referral to surgery was similar in both groups [38 days (SD 17.3) for teledermatology vs. 32 days (SD 12.5) for face to face (P = 0.4)]. The median waiting time from decision to treat to surgery was similar in both groups [21 days (SD 17.3) for teledermatology vs. 20 days (SD 11.7) for face to face (P = 0.9)]. There was no significant difference in the diagnostic biopsy/therapeutic excision rate (34% for teledermatology vs. 27% for face to face, P = 0.37), or in the proportion of benign, dysplastic and malignant lesions biopsied or excised (P = 0.75). Over one-third of patients reviewed by teledermatology were discharged without the need to attend hospital. In conclusion, this model of teledermatology offers a robust way to assess suspected cancer, with biopsy rates and eventual diagnoses comparable with conventional models. The use of professional medical photography may delay initial review, but higher-quality images are associated with improved diagnostic accuracy.
