Abstract:A 43-year-old woman was scheduled to undergo cardiac pacemaker implantation because of sick sinus syndrome. Following percutaneous access of the left subclavian vein, the guide wire traveled straight across to the right subclavian vein, and could not be turned acutely downward into the left persistent superior vena cava despite multiple attempts with lead reshaping. Injection of contrast medium showed the persistent left superior vena cava and absent right superior vena cava (Figure 1). A right subclavian approach was used to accomplish a bipolar lead with active fixation (Medtronic 5076-58, Minneapolis, MN) implantation in the right atrium, via the coronary sinus (Figure 2). The RV lead could not enter the right ventricle. A singlechamber pacing system (AAI) was selected, due to the normal atrioventricular conduction. Abstract:Cephalic vein puncture has been described as novel access for CIED implantation, promising high success rates and simplified handling. Implantation of at least one lead via CVP was successful in 90% of the patients. Total implantation and fluoroscopy times did not differ between cephalic and subclavian implantations. Pneumothorax only occurred after subclavian puncture. Introduction The establishment of venous access is one of the driving factors for complications during implantation of pacemakers and defibrillators (cardiac implantable electronic devices [CIED]). Recently, a novel approach of accessing the cephalic vein for CIED by cephalic vein puncture (CVP) using a modified Seldinger technique has been described, promising high success rates and simplified handling with steeper learning curves. In this single‐center registry, we analyzed the safety and efficiency of CVP to SVP access after defining CVP as the primary access route in our center. Methods A total of 229 consecutive patients receiving a CIED were included in the registry. Sixty‐one patients were implanted by primary or bail‐out SVP; 168 patients received primary cephalic preparation and CVP was performed when possible, using a hydrophilic transradial sheath. Results Implantation of at least one lead via CVP was successful in 151 of 168 patients (90%), and implantation of all leads was possible in 122 of 168 patients (72.6%). Total implantation times and fluoroscopy times and doses did not differ between CVP and SVP implantations. Pneumothorax occurred in 0/122 patients implanted via CVP alone, but 8/107 (7.5%) patients received at least one lead via SVP. Conclusion Our data confirms high success rates of the CVP for CIED implantation. Moreover, this method can be used without significantly prolonging the total procedure time or applying fluoroscopy dose compared to the highly efficient SVP while showing lower overall complication rates.

Supraclavicular vein approach to overcoming ipsilateral chronic subclavian vein obstruction during pacemaker-ICD lead revision or upgrading

Abstract:Aims: We report our experience with the supraclavicular vein approach of subclavian vein puncture to overcome ipsilateral chronic obstruction when implanting pacemaker or implantable cardioverter defibrillator leads. Methods and results: The subclavian vein obstruction was documented by venography. The skin was punctured with an 18-gauge needle, 1 cm lateral to the lateral head of the sternocleidomastoid muscle and 1 cm cranial to the clavicle. The needle was directed under and close to the clavicle pointing to the sternal notch. Once the vein was successfully punctured, medial to the obstruction, a 0.38 in. guidewire was inserted into the venous bed. A peel-away sheath was indwelled using the Seldinger technique. The leads were placed in the standard fashion; they were secured by suture to the subcutaneous tissue of the fossa supraclavicularis major using a protective sleeve. The proximal portion of the lead was tunnelled over the clavicle down to the device's prepectoral pocket. Lead insertion was performed in four patients (twice in one patient) with total left subclavian vein obstruction; the site of the obstruction was in the mid-segment of the left subclavian vein in two patients, in the axillary and distal segment of the subclavian vein in one patient, and in the distal segment of the subclavian vein in one patient. There were no complications with the surgical wound and the lead parameters remained stable. Conclusion: The supraclavicular approach of the subclavian vein puncture to overcome ipsilateral total occlusion is feasible and safe.

Abstract:Ultrasound-guided axillary vein access is an effective alternative to conventional subclavian and cephalic access for cardiac implantable electronic device implantation. The aim of this study was to compare the safety, efficacy, and radiation exposure data of the ultrasound-guided axillary approach with other conventional access techniques. The study population included 130 consecutive patients, stratified as 65 (64% male; median age, 79 years) in the study group and 65 (66% male; median age, 81 years) in the control group. We performed a retrospective not-randomized analysis by comparing ultrasound-guided axillary vein puncture with subclavian and cephalic approaches in order to test the effect on X-ray exposure, total procedure time, and complications. Significant differences were observed in terms of radiation exposure, including fluoroscopy time (median, 95 s [study group] vs. 193 s [control group]; P < .001), air kerma (median, 29 mGy [study group] vs. 55.7 mGy [control group]; P < .001), and dose-area product (median, 8219 mGy·cm2 [study group] vs. 16736 mGy·cm2 [control group]; P < .001). The median procedure time was 45 min in the study group but 50 min in the control group (P < .05). Complications occurred in 6 control group patients (1 urticaria contrast medium-related, 3 pneumothorax, 2 subclavian artery puncture) and 2 study group patients (2 axillary artery puncture). We conclude that the ultrasound-guided axillary venous approach is a fast, feasible, and safe technique for cardiac lead implantation. It allows a significant reduction in fluoroscopy time without prolonging the procedural time. This approach offers direct visualization of the vessel during the puncture, so it can be useful in patients who cannot receive contrast medium, those who require "difficult" thoracic approaches (emphysema, too much or too little fat tissue), or those on anticoagulant therapy.

Abstract:Objective To discuss the application of implantation temporary cardiac pacing by balloon-tipped floating catheter for peroperative period patients. Method Thirty-three patients with severe bradyarrhythmia were received the balloon-tipped floating catheter to RV by Seldinger puncture technique.Meantime, the pacing signals of ECG were observed. The threshold and QRS waves to ensure the electrode position were determined. Result All of 33 cases were successfully punctured and 32 cases get paced, except 1 case had the electro - mechanical dissociation (EMD). Pacing threshold was less than 0. 9V, and mean remaining time was 12.47 ～ 161 (67. 05 ±34. 48) hours. Apical pacing was operated in 23 patients,and right ventricular outflow tract pacing in 10 patients. No severe complications were occurred. Conclusion The implantation method of temporary cardiac pacing by balloon-tipped floating catheter was convenient and safe. It could help the patients get safety in anesthesia and peroperative period.

Abstract:We reviewed the different approaches used for central vein access during insertion of cardiac implantable electronic devices. The benefits and hazards of each approach (cephalic vein cutdown, axillary vein cannulation using venography and ultrasound) are discussed. Each approach has its advantages and hazards that need to be considered for the individual patient and balanced against the skills of the operator. The benefits of ultrasound guided venous access in reducing radiation exposure to the patient and implanter, avoiding the need for angiographic contrast and in minimizing the risk of pneumothorax and inadvertent arterial puncture are highlighted. Trainees should be taught each approach to deal with patient variability. Ultrasound guidance should be considered as a mainstream option for most patients.

Abstract:Abstract Purpose The cephalic vein cutdown (CVC) and the subclavian puncture (SP) is the most common access for pacemaker implantation. The purpose of this study was to compare the peri-/postoperative complications of these approaches. Methods A retrospective analysis of the quality assurance data of the state of North Rhine-Westphalia was performed to evaluate the peri-/postoperative complications of first pacemaker implantation according to the venous access. The primary endpoint was defined as the occurrence of one of the following: asystole, ventricular fibrillation, pneumothorax, hemothorax, pericardial effusion, pocket hematoma, lead dislocation, lead dysfunction, postoperative wound infection or other complication requiring intervention. Descriptive analysis was done via absolute, relative frequencies and Odds Ratio. Fisher’s exact test was used for comparison of the both study groups. Results From 139,176 pacemaker implantations from 2010 to 2014, 15,483 cases were excluded due to other/double access. The median age was 78 years and the access used was CVC for 75,251 cases (60.8%) and SP for 48,442 cases (39.2%). The implanted devices were mainly dual-chamber pacemakers (73.9% in the CVC group and 78.4% in the SP group), followed by single-chamber pacemakers VVI (24.9% and 19.9% in the CVC and SP group respectively). There were significantly fewer peri/postoperative complications in the CVC group compared to the SP group (2.49% vs. 3.64%, p = 0.0001, OR 1.47; 95% CI 1.38–1.57). Conclusions CVC as venous access for pacemaker implantation has significantly fewer peri/postoperative complications than SP and appears to be an advantageous technique.

Intra-pocket ultrasound-guided axillary vein puncture vs. cephalic vein cutdown for cardiac electronic device implantation: the ACCESS trial

Abstract:Background and aims: Intra-pocket ultrasound-guided axillary vein puncture (IPUS-AVP) for venous access in implantation of transvenous cardiac implantable electronic devices (CIED) is uncommon due to the lack of clinical evidence supporting this technique. This study investigated the efficacy and early complications of IPUS-AVP compared to the standard method using cephalic vein cutdown (CVC) for CIED implantation. Methods: ACCESS was an investigator-led, interventional, randomized (1:1 ratio), monocentric, controlled superiority trial. A total of 200 patients undergoing CIED implantation were randomized to IPUS-AVP (n = 101) or CVC (n = 99) as a first assigned route. The primary endpoint was the success rate of insertion of all leads using the first assigned venous access technique. The secondary endpoints were time to venous access, total procedure duration, fluoroscopy time, X-ray exposure, and complications. Complications were monitored during a follow-up period of three months after procedure. Results: IPUS-AVP was significantly superior to CVC for the primary endpoint with 100 (99.0%) vs. 86 (86.9%) procedural successes (P = .001). Cephalic vein cutdown followed by subclavian vein puncture was successful in a total of 95 (96.0%) patients, P = .21 vs. IPUS-AVP. All secondary endpoints were also significantly improved in the IPUS-AVP group with reduction in time to venous access [3.4 vs. 10.6 min, geometric mean ratio (GMR) 0.32 (95% confidence interval, CI, 0.28-0.36), P < .001], total procedure duration [33.8 vs. 46.9 min, GMR 0.72 (95% CI 0.67-0.78), P < .001], fluoroscopy time [2.4 vs. 3.3 min, GMR 0.74 (95% CI 0.63-0.86), P < .001], and X-ray exposure [1083 vs. 1423 mGy.cm², GMR 0.76 (95% CI 0.62-0.93), P = .009]. There was no significant difference in complication rates between groups (P = .68). Conclusions: IPUS-AVP is superior to CVC in terms of success rate, time to venous access, procedure duration, and radiation exposure. Complication rates were similar between the two groups. Intra-pocket ultrasound-guided axillary vein puncture should be a recommended venous access technique for CIED implantation.

Abstract:Placement of a dialysis catheter substantially increases the risk of central vein stenosis. 52-year-old female with end-stage renal disease and a right brachial-cephalic hemodialysis access presented with right arm swelling. The chronic total occlusion of right brachiocephalic vein was refractory to wire traversal. Sharp recanalization of the central venous occlusion was done with transseptal needle retrogradely. The track was balloon dilated and stented. When the conventional catheters and guide wires options fail, sharp recanalization technique may be used to salvage a precious dialysis access.

Abstract:The IMPELLA 5.5 (Abiomed Inc., Danvers, Massachusetts, United States) is a catheter-based, micro-axial blood pump designed to enhance organ perfusion in patients with cardiogenic shock. Despite its superior hemodynamic support, vascular complications are a significant concern, with many patients needing to discontinue IMPELLA therapy due to these issues. Patients may even require surgical intervention to address device-related vascular injuries. The IMPELLA 5.5 implantation in vessels with severe calcification is particularly associated with complications such as vascular calcification, stenosis, vascular tortuosity, and the use of larger sheaths are risk factors following endovascular therapy and IMPELLA implantation. In this report, we present a case of severe calcification in the right subclavian artery, in which the IMPELLA 5.5 was lodged. The calcifications protruded into the vascular lumen, becoming lodged between the IMPELLA motor and the cannula, complicating extraction despite the vessel having sufficient diameter. We successfully removed the device using a balloon dilation technique, ensuring safe extraction. No vascular complications such as pseudoaneurysm or dissection were observed in the right subclavian artery one month after extraction. This case highlights a potential approach for managing similar complications and vascular access for IMPELLA insertion.

Abstract:External jugular vein (EJV) cutdown for totally implantable venous access device (TIVAD) placement has been accepted as an alternative to the percutaneous subclavian vein approach. The aim of this work was to report a retrospective and monocentric study concerning the feasibility and complications of this surgical technique. Patients receiving a TIVAD from January 2010 to December 2015 were included in this study. Age, sex, surgical technique, disease, device used, length of the procedure, and morbidity were considered. There were 426 TIVADs placed in 418 patients: 149 males (35.6%) and 269 females (64.4%) aged 1 to 73 years. Of 425 patients, 172 underwent an EJV cut down on the first attempt, and 161 (93.6%) procedures were successful. Among the remaining 11 patients, seven (63.6%) underwent a TIVAD placement through the ipsilateral internal jugular vein and through the ipsilateral subclavian vein in the other four. Complications were identified in 11 ports in EJV group (6.25%) and in 35 ports in percutaneous puncture group (14.1%; P < .05). TIVAD placement by the EJV cutdown approach is safe and fast, and its success rate is very high. By avoiding the immediate complications associated with the percutaneous approach, the EJV cutdown has to be considered a valid, safe.
The Use of Subcostal Echocardiographic Views to Guide the Insertion of a Right Ventricular Temporary Transvenous Pacemaker-Description of the Technique

Abstract:The need for temporary cardiac pacing may occur in emergency and elective situations and may require transvenous right ventricular lead placement. The treatment of bradyarrhythmias presents the most common perioperative emergency indication. Intraoperatively, temporary rapid right ventricular pacing is accepted as a safe, titratable, and highly reliable method to achieve deliberate hypotension, and it has become a routine practice in the anesthetic management of cardiovascular interventions. The navigation of the lead into the right ventricle often requires fluoroscopy to guide placement and to confirm position. Ultrasound guidance has been accepted widely by perioperative physicians as a safe technique for central venous access. Basic ultrasound and transthoracic echocardiographic skills rapidly are becoming integral to anesthesiologists' practice. When used to guide transvenous pacemaker wire insertion, subcostal echocardiographic imaging offers attractive advantages over blind or fluoroscopic placement, including rapid deployment, avoidance of radiation, real-time visualization of the lead in relation to the cardiac structures, and early detection of potential complications, such as tamponade. Although several articles on echocardiographic guidance for transvenous pacing have been published in other acute care specialty fields in the last decade, this is the first description of the technique and of the recommended echocardiographic views in a perioperative context. In addition, a review of the current literature is presented, and the specific advantages and disadvantages of the approach are discussed in this article.

Abstract:Background: Cephalic vein cutdown (CVC) and subclavian vein puncture (SVP) are the most commonly used access sites for transvenous lead placement of cardiac implantable electronic devices (CIEDs). Limited knowledge exists about the long-term patency of the vascular lumen housing the leads. Methods: Among the 2703 patients who underwent CIED procedures between 2005 and 2013, we evaluated the phlebographies of 162 patients scheduled for an elective CIED replacement (median of 6.4 years after the first operation). The phlebographies were divided into four stenosis types: Type I = 0%, Type II = 1–69%, Type III = 70–99%, and Type IV = occlusion. Due to the fact that no standardized stenosis categorization exists, experienced physicians in consensus with the involved team made the applied distribution. The primary endpoint was the occurrence of stenosis Type III or IV in the CVC group and in the SVP group. Results: In total, 162 patients with venography were enrolled in this study. The prevalence of high-degree stenosis was significantly lower in the CVC group (7/89, 7.8%) than in the SVP group (15/73, 20.5%, p = 0.023). In the CVC group, venographies showed a lower median stenosis (33%) than in the SVP group (median 42%). Conclusions: The present study showed that the long-term patency of the subclavian vein is higher after CVC than after SVP for venous access in patients with CIED.

Two cases were postbilateral orthotopic lung transplant, without appropriate femoral or jugular access due to recent ECMO cannulation and jugular central venous catheters. In one case, there was strong patient preference for same‐day discharge. Stability testing confirmed adequate fixation and electrical testing confirmed stable parameters in all cases. All patients tolerated the procedure well without significant immediate complications. Conclusions We demonstrate the feasibility of transsubclavian implantation of the Aveir LCP.

Hybrid technique to remove the permanent pacemaker and ICD leads

Abstract:Objective To summarize the clinical use of hybrid technique extracting permanent pacemaker or ICD leads. Methods There were 4 patients treated by this kind of technique from January 1,2012 to December 31,2012.The surgical approach was minimal right anterior incision,minimal lower sterna incision,minimal left anterior incision and minimal medial incision according to different cases.Combined with direct heart surveillance and X-ray assistant,the complicated lead was extracted transvenously. Results All the 4 cases were successfully treated in this way.One case endured tricuspid valve rupture and server tricuspid valve regurgitation but was successfully repaired emergently.All the cases were fully recovered and discharged. Conclusions Extraction of permanent pacemaker lead by hybrid technique is feasible and can avoid cardiopulmonary bypass.It′s an alternative way of lead extraction for those high risk patients.
A real-time ultrasound-guided supraclavicular approach to the brachiocephalic vein to prevent Hickman catheter bending and occlusion

Abstract:The infraclavicular approach (subclavian approach) to the central vein is most often selected for implantation of a tunnelled Hickman/Broviac catheter because central venous catheter (CVC) insertion into the upper body is optimal considering patient comfort as well as the risk of infection [1] and intravenous thrombosis [2, 3]. However, the actual puncture site for a real-time ultrasound-guided infraclavicular axillary vein approach is at the level of the axillary vein in a much more lateral location than that for the traditional landmark infraclavicular approach to the subclavian vein. This is because an optimal view of the subclavian vein is difficult to obtain with a real-time ultrasound-guided technique, as the ultrasound beam is restricted by the clavicle [4]; the younger the patient, the greater would be its influence because of the width of the ultrasound probe. As a result, Hickman/Broviac catheter bending due to a sharply curved angle in the subcutaneous tunnel may occur when the subcutaneous tunnel exit needs to be created inside the patient’s nipple to obtain sufficient tunnel length, especially in young paediatric patients. When the internal jugular vein is chosen as an alternative puncture site, adverse events of catheter bending and occlusion may occur because the puncture direction to the target vein and the direction of the subcutaneous tunnel become very steep [5].

Abstract:We present a rare and interesting case of subclavian vein puncture-induced focal intraparenchymal lung hemorrhage and massive hemoptysis developed during CRT-D implantation. Clinical picture advanced to pulmonary edema in seconds. A noncontrast multiple-detector computed tomography scan revealed focal alveolar hemorrhage in the lung tissue right under the pacemaker pocket, but remarkably, there was no pneumothorax. This case shows that if cough and hemoptysis suddenly develop during subclavian puncture, injury of the adjoining lung because of parenchymal puncture should be considered as a complication.
