Abstract:Seat pressure measurements in wheelchair users have been available for some time; however, repeated measurements from a commercially available pressure mat over 90 min did not differ in the pressure-loaded measurement area or the coordinates of the center of pressure, even in participants who were able to reposition themselves in the wheelchair. The question therefore arises: to what extent are there other parameters that reflect the activity of wheelchair users with the pressure mat? To investigate this, a commercial pressure mat (BodiTrak®) was used to perform the measurements of pressure of 33 adult wheelchair-dependent people with spinal cord injury after 30 and 90 min sitting on the cushion. In addition to the standard output of the pressure mat, graph-based surface analyses (calculation of the area of maximum pressure, calculation of the pressure-loaded measurement area, and pressure–area ratio) was performed retrospectively using Python 3.7. The analysis of the measurements after 30 and 90 min was performed by distinguishing the participants between those who could actively change their position (N = 24) and those who could not (N = 9). The parameters of the pressure mat and the graph-based analyses remained unchanged for active participants. In participants who were unable to actively change their position, the area of maximum pressure and the pressure–area ratio (ratio of maximum pressure area and total pressure-loaded area) increased. Significant differences between minutes 30 and 90 are only found for the pressure–area ratio. Thus, when measuring the seat pressure of wheelchair users, the pressure–area ratio should be taken into account as it reflects the daily relief activities of wheelchair users.

Radial Displacement Measurement of Hydrogen Storage Cylinders under Internal Pressure Based on Fringe Projection Profilometry

Abstract:Abstract Composite gas cylinders, as an important hydrogen storage device for hydrogen fuel cell vehicles, require a hydrostatic test to ensure safe usage and service. However, there is a lack of a non-contact and efficient method to monitor the deformation of the gas cylinders during the hydrostatic testing. Thus, this paper proposes a radial displacement measurement method based on binocular fringe projection profilometry (BFPP) for pressurized hydrogen storage cylinders. We use BFPP as a highly sensitive three-dimensional (3D) shape measuring method, where the shape information that encoded in the deformed light pattern is acquired based on phase demodulation and coordinate reconstruction. Combining with 3D morphology measurement technology, internal pressure loading tests were conducted on a composite overwrapped pressure vessel with 55 Litre and 35MPa design pressure. Firstly, a large-field BFPP system was constructed to collect 3D point cloud data of the surface of the cylinder at different internal pressures. Then, a cylinder fitting based on the least squares method was used to obtain the radius. Finally, the difference of radius between pressure load steps was calculated, which indicates the radial displacement of the gas cylinder at different internal pressures. The measured result shows a good agreement with FEA calculations, where the radial displacement increase linearly with the increment of inner pressure. The error ranges from 2% to 18%.

Abstract:Manual wheelchairs are essential for people with disabilities or limited mobility. However, manual propulsion causes biomechanical loads, including contact pressures on the palms of the hands. The hand rim design has received little attention over time, remaining almost unchanged since its creation. This study investigated how two different designs of such devices – one standard and another with a contoured design – influence the contact pressure on the surface of the hands. The procedures included a figure-of-eight shape propulsion task on a regular floor, using both models on a wheelchair. A pressure-mapping system coupled with a pair of fabric gloves recorded the data. The results show that the contoured hand rim provides lower pressure in most of the analysed regions. Considering that manual propulsion is performed during a considerable part of the day as a routine activity, improving the hand rim interface may benefit the user's comfort and safety during wheelchair use. <b>Practitioner summary:</b> The design of the hand rim used in wheelchair propulsion influences the contact pressure on the hands. Conventional round tube rims tend to concentrate high levels of pressure on the distal phalanges and metacarpal regions. Abstract:Background: Many frail elderly and disabled persons have to use a manual wheelchair to remain mobile and active. Apart from a well-fitted wheelchair, an appropriate wheelchair cushion is considered crucial for postural support and to prevent complications. Aim: To examined the effect of two types of seat cushions in two types of wheelchairs with regard to pelvic rotation, respiratory function, and interface pressure. Material and methods: Forty healthy elderly between 67 and 85 years of age participated. Two types of commonly used manual wheelchairs and two types of seat cushions were tested. Interface pressure, pelvic position, and respiratory function were measured using a sensor array mat, a study-specific inclinometer, and a spirometer. Results: Differences in interface pressure and pelvic rotation between cushions were found in both wheelchairs. Compared with sitting on a standard cushion, sitting on the positioning cushion increased peak pressure. The posterior pelvic tilt increased with the positioning cushion in the Etac Cross wheelchair and for both cushions in the Etac Cross wheelchair compared with the HD Balance wheelchair. No difference was observed in respiratory function. Conclusions: This study highlights the importance of a thorough evaluation of the effects of wheelchair interventions, especially for the elderly and disabled.

Abstract:Local shear is understood to be one of the principal risk factors for the development of pressure sores. There is a need for a small deformable sensor that can measure the shear force between skin and deformable materials without disturbing the shear phenomenon. In the present study a new shear sensor is introduced with a contact area of 4.05 cm2. A series of validation experiments was performed with ten healthy young subjects. It was demonstrated that with a forward-tilted seat, the sum of the local shear forces between skin and sensormat is equal to the resultant shear force measured with a force plate. This result serves as a validation of the new sensor. The shear values recorded are 4.8 kPa in the longitudinal direction and 8.5 kPa in the transversal direction while sitting in a wheelchair, and 5.6 kPa in the longitudinal direction and 3.1 kPa in the transversal direction on a mattress of a hospital bed, while in sitting position in bed.

Abstract:Objective: The main objective of the tests conducted was to analyze the position variability of the human body’s center of gravity during propelling the wheelchair, and to demonstrate the properties enabling the description of this variability by means of plane figures with a symmetry axis. A secondary objective was to show the impact of the used manual propulsion type and the wheelchair inclination angle in relation to the plane on the dimensions of the position variability areas of the center of gravity. Method and materials: Three patients participated in the research representing 50 centiles of anthropometric dimensions. Each patient carried out fifteen measurement tests on three wheelchairs for three inclination angles of the wheelchair frame in relation to the level. Each measurement test consisted of five propulsion cycles for which the positions of the center of gravity were determined with the sampling frequency of 100 Hz. The measured positions of the center of gravity were approximated with ellipses containing 95.4% of the measurements conducted, assuming their dimension scaling basis in the form of the double value of standard deviation defined based on the registered results. Results: Based on the measurements conducted, the average values of five ellipses parameters were determined for nine cases in which a variable was the type of wheelchair propulsion and its inclination angle in relation to the level. The area of the highest variability of the position of the center of gravity was measured for the wheelchair with a multispeed transmission. The average dimensions of the ellipse semi-axis amounted to 108.53 mm for the semi-axis a and 29.75 for the semi-axis b, the average position of the ellipse center amounted to x = 114.51 mm and y = −10.53 mm, and the average inclination angle of the ellipse α amounted to −6.92°. The area of the lowest variability of the position of the center of gravity was measured for the wheelchair with a hybrid transmission. In this case, the average dimensions of the ellipse semi-axis amounted to 64.07 mm for the semi-axis a and 33.85 for the semi-axis b; whereas, the average position of the ellipse center amounted to x = 245.13 mm and y = −28.24 mm, and the average inclination angle of the ellipse α amounted to −0.56°.
Evaluation of rolling resistance in manual wheelchair wheels and casters using drum-based testing

Abstract:Purpose: Rolling resistance is a drag force that increases the required propulsion force of manual wheelchair users (MWU) and increases the risk of upper extremity pain and injury. Materials and methods: To understand the influence of different design, environmental, and setup factors on rolling resistance (RR), a series of tests were performed on a range of wheels and casters using a drum-based equipment with the capability to measure RR forces. Independent factors were varied including load, camber, toe, speed, tire pressure, and surface, using ranges anticipated in the community. Combined factor testing of these factors was also completed to evaluate of RR changes due interactions of multiple factors. Results: A default reference trial was used to verify repeatability throughout the 924 rear wheel trials and 255 caster trials. Toe angle and tire pressure were found to have large and exponential relationships to RR. Tire/caster type and surfaces are significant influencers but have no specific relationship to RR. Load had a direct linear relationship to RR whereas camber and speed had a relatively small impact on RR. Pneumatic tires had lower rolling resistance compared to airless inserts, solid mag wheels and knobby tires. Combined factor testing revealed a linear additive effect of individual factors. Statistical analysis revealed that tire/caster type is a covariate to all of the results and statistical differences (p < 0.01) were found for toe, tire/caster type, tire pressure, surfaces and load. Conclusions: Factors act in a cumulative manner to impact RR and need to be monitored in device design, development, issuance, and maintenance.Implications for RehabilitationFirst comprehensive study of MWC RR showing the effects of individual and combined factors.Highlights the direct importance of tire and caster selection.

Abstract:This study aims at investigating the effects of three different wheelchair cushion technologies at the patient-wheelchair interface. To this end, eight participants were recruited to remain in an unrelieved seated position on a wheelchair successively equipped with three different cushions (foam, air-cell-based and gel), for a duration of 45 min. Interface pressure, temperature (measured with infrared thermography) and relative humidity were measured at the seat interface, at different timestamps. Experimental results show that foam cushion is significantly more efficient in reducing contact peak pressure (p < .01), while the gel cushion displays higher heat evacuation capabilities. In terms of relative humidity, no significant difference is observed among the three technologies (p > .29): all of them evacuate around only 10% of the total humidity compared to the reference situation (i.e., without cushion). Besides, a complementary numerical simulation corresponding to the steady state of the patient-wheelchair structure clearly highlights the temperature volume field at the underside of the seat, which acts like a thermal barrier and contributes to heat accumulation. Besides, an air flow at the underside of the chair in motion is shown to significantly reduce heat accumulation.
Real-Life Wheelchair Mobility Metrics from IMUs

Abstract:Daily wheelchair ambulation is seen as a risk factor for shoulder problems, which are prevalent in manual wheelchair users. To examine the long-term effect of shoulder load from daily wheelchair ambulation on shoulder problems, quantification is required in real-life settings. In this study, we describe and validate a comprehensive and unobtrusive methodology to derive clinically relevant wheelchair mobility metrics (WCMMs) from inertial measurement systems (IMUs) placed on the wheelchair frame and wheel in real-life settings. The set of WCMMs includes distance covered by the wheelchair, linear velocity of the wheelchair, number and duration of pushes, number and magnitude of turns and inclination of the wheelchair when on a slope. Data are collected from ten able-bodied participants, trained in wheelchair-related activities, who followed a 40 min course over the campus. The IMU-derived WCMMs are validated against accepted reference methods such as Smartwheel and video analysis. Intraclass correlation (ICC) is applied to test the reliability of the IMU method. IMU-derived push duration appeared to be less comparable with Smartwheel estimates, as it measures the effect of all energy applied to the wheelchair (including thorax and upper extremity movements), whereas the Smartwheel only measures forces and torques applied by the hand at the rim. All other WCMMs can be reliably estimated from real-life IMU data, with small errors and high ICCs, which opens the way to further examine real-life behavior in wheelchair ambulation with respect to shoulder loading. Moreover, WCMMs can be applied to other applications, including health tracking for individual interest or in therapy settings.

Temperature increases of railway wheels due to frictional braking on the running surface (wheel tread) can lead to maintenance challenges such as tread wear and thermal cracking. While heating within the anticipated design range should not adversely affect the structural safety of the wheels, excessive temperature rises resulting from brake system failures can induce tensile residual stresses in the rim, potentially leading to wheel fracture. Detecting wheels with abnormal residual stresses is therefore crucial for railway safety, and monitoring residual displacements in the rim due to changes in circumferential residual stresses is an implementable approach. However, the shape of the wheel web varies, and the displacement response to thermal input differs depending on the wheel geometries. This study aims to explore general deformation behaviours related to wheel shape and elucidate the relationship between shape and heat resistance. The elastic-plastic finite element analysis employing simplified intense thermal input revealed that residual deformation after excessive thermal loading consistently results in back gauge expansion, although the deformation and stress responses differ based on the web geometry. Differences in heat resistance are indicated by the variation in the temperature at which compressive yielding due to thermal stress occurs, depending on the web shape. The residual stress detectability of each wheel is determined based on the principle that greater residual displacement at lower stress indicates higher anomaly detectability.

Abstract:A current challenge in realising clean road transport is non-exhaust emissions. Important advances regarding measurement systems, including well-defined characterisation techniques, as well as regulation, will be made in the next few years. In this work, we present the detailed results of particle emission analyses, consisting of aerosol (size distribution, particle number (PN), and mass (PM)) and electron microscopy (EM) measurements, under different load conditions on a test bed for a wheel suspension and brakes. Standard tyres and brakes from serial production were tested with a high-load driving cycle, while particle measurements were conducted by gravimetric measurements and with a TSI SMPS, a TSI APS, and a GRIMM OPS. Furthermore, samples were analysed by electron microscopy. A bimodal particle size distribution (PSD) was obtained with an SMPS, with peaks at 20nm and around 400nm. The results of an EM analysis of >1400 single particles from the electrostatic sampler match the PSD results. The EM analysis also showed ultrafine particles, mainly containing O, Fe, Si, Ba, Mg, and S, and also fractal particles with high-C fractions. Our results suggest, in agreement with the previously published literature, that particulate emissions are related to the brake disc temperature and occur in significant amounts above a threshold temperature.

Abstract:Abstract The equipment for evaluating the propulsion of a wheelchair is very complex and expensive. To validate a new dynamometer prototype for assessing the propulsion capacity of wheelchairs, 21 healthy subjects (age: 20.9±2.4 yr; weight: 68.9±7.9 kg; height: 174.0±7.1 cm; BMI: 22.7±2.5 kg·m−2) who do not normally require wheelchairs performed a sprint protocol for 20 s after a 1-min warm-up. The power and rotation data acquired by the prototype (both right and left sides) were compared with those of a reference system via high-speed videography (240 fps). The results showed high levels of accordance (95% CI), excellent values for the intraclass correlation coefficient (ICC: .99; P<0.00), no significant differences in the rotation (P=0.91) and power (P=0.94) between the methods. The proposed equipment met the validation criteria and thus can be applied as a new tool for assessing wheelchair propulsion.

Abstract:Studies examining the stump/socket interface stresses have been restricted to unsupported stance and natural gait, i.e. walking at a comfortable speed on flat and straight walkway. However, the pressure behaviour as to the interface in unilateral transtibial amputees during walking on stairs, slope and non-flat road is unclear.Pressure distribution changes at multiple points, expressed as mean peak stump/socket interface pressure, mean pressure level over 90% of peak pressure, time in which pressure exceeded 90% of peak pressure and time-pressure integral at the period of sustained sub-maximal load, were measured during natural ambulating and walking on stairs, slope and non-flat road.Compared with natural gait, the mean peak pressure and sustained sub-maximal load increase notably over the patellar tendon during walking on stairs and non-flat road, and however decrease or change insignificantly at the patellar tendon on slope and over other measured areas in all conditions; moreover the time period of sustained sub-maximal load changes remarkably, except over the patellar tendon during walking up slope and over the popliteal area on non-flat road; finally, the time-pressure integral in the time period of sustained sub-maximal load changes considerably, except at the patellar tendon during walking up slope.The pressure characteristics during natural ambulating seem not to be highly predictive of what occurs in the conditions of walking on stairs, slope and non-flat road, which leads to significant increase in amplitude domain of tissue loading only at the patellar tendon, and however to remarkable changes in temporal sequences of tissue (un-)loading almost in all measured regions.

Abstract:About 1% of the world's population uses a wheelchair. Wheelchair use is a well-known risk of pressure injury. A connected pressure detection system could help to prevent this complication that is linked to long durations of sitting, provided that user expectations are understood. The aim of this study was to explore the needs of wheelchair users (WU) regarding connected pressure detection systems to prevent pressure injury. A cross-section survey-based study of WU was conducted, using an anonymous electronic questionnaire posted from July 2019 to June 2020. Eighty-eight people responded. The majority were power wheelchair users (72.7%); one third (33.0%) had already sustained a pressure injury; only 17.0% knew of the existence of pressure detection systems, nevertheless 78.4% believed that they could be useful in daily life. The feature that received the highest rating was a pressure warning alarm (4.2/5 points). The majority (71.6%) preferred reminder-alerts to be set according to their habits and not according to medical guidelines. In conclusion, pressure detecting systems were perceived as useful to prevent pressure injuries by both manual and power wheelchair users. Work is needed to inform potential users of the existence of such systems.

Effect of Wheels, Casters and Forks on Vibration Attenuation and Propulsion Cost of Manual Wheelchairs

Abstract:Manual wheelchair users are exposed to whole-body vibrations as a direct result of using their wheelchair. Wheels, tires, and caster forks have been developed to reduce or attenuate the vibration that transmits through the frame and reaches the user. Five of these components with energy-absorbing characteristics were compared to standard pneumatic drive wheels and casters. This study used a robotic wheelchair propulsion system to repeatedly drive an ultra-lightweight wheelchair over four common indoor and outdoor surfaces: linoleum tile, decorative brick, poured concrete sidewalk, and expanded aluminum grates. Data from the propulsion system and a seat-mounted accelerometer were used to evaluate the energetic efficiency and vibration exposure of each configuration. Equivalence test results identified meaningful differences in both propulsion cost and seat vibration. LoopWheels and SoftWheels both increased propulsion costs by 12-16% over the default configuration without reducing vibration at the seat. Frog Legs suspension caster forks increased vibration exposure by 16-97% across all four surfaces. Softroll casters reduced vibration by 11% over metal grates. Wide pneumatic 'mountain' tires showed no difference from the default configuration. All vibration measurements were within acceptable ranges compared to health guidance standards. Out of the component options, softroll casters show the most promising results for ease of efficiency and effectiveness at reducing vibrations through the wheelchair frame and seat cushion. These results suggest some components with built-in suspension systems are ineffective at reducing vibration exposure beyond standard components, and often introduce mechanical inefficiencies that the user would have to overcome with every propulsion stroke.

Abstract:The article analyzes methods for solving problems on the drive systems' compacting effect on the support surface (soil). There are analyzed results of studies on the distribution of pressures and tensions in the contact surfaces depending on the propulsors' mechanical properties and the support surface's material. The article also includes an analysis of methods for formalizing contact surfaces and constructing mathematical models for determining the distribution of pressures, deformations, and tensions in the contact surfaces. Numerical methods for solving contact problems that have been used recently are not integral. To obtain such solutions, numerical FEM and DEM methods can be used for a specific problem, i.e. for a problem with a known geometric data source and specific mechanical properties of a contacting body. Based on the studies conducted, it was concluded that to understand the interaction of a deforming wheel with a deformable surface, it is necessary to use an analytical method for solving contact problems in a three-dimensional setting as the most general and productive. It’ll allow us to determine the geometrical dimensions and shapes of the contact spot, determine dependencies of the geometrical parameters of the wheel, rolling resistance, tension-strain state of the contacting surfaces depending on the loading conditions of the wheel, its mechanical properties, and the support surface.
The Design and Simulation of a 16-Sensors Plantar Pressure Insole Layout for Different Applications: From Sports to Clinics, a Pilot Study

Abstract:The quantification of plantar pressure distribution is widely done in the diagnosis of lower limbs deformities, gait analysis, footwear design, and sport applications. To date, a number of pressure insole layouts have been proposed, with different configurations according to their applications. The goal of this study is to assess the validity of a 16-sensors (1.5 × 1.5 cm) pressure insole to detect plantar pressure distribution during different tasks in the clinic and sport domains. The data of 39 healthy adults, acquired with a Pedar-X® system (Novel GmbH, Munich, Germany) during walking, weight lifting, and drop landing, were used to simulate the insole. The sensors were distributed by considering the location of the peak pressure on all trials: 4 on the hindfoot, 3 on the midfoot, and 9 on the forefoot. The following variables were computed with both systems and compared by estimating the Root Mean Square Error (RMSE): Peak/Mean Pressure, Ground Reaction Force (GRF), Center of Pressure (COP), the distance between COP and the origin, the Contact Area. The lowest (0.61%) and highest (82.4%) RMSE values were detected during gait on the medial-lateral COP and the GRF, respectively. This approach could be used for testing different layouts on various applications prior to production.
