Abstract:The NHS is a world leader in using pagers and fax machines and should be ashamed of it, says a report by a Conservative MP, Alan Mak.1 New technology could transform the service, he concludes, but it relies instead on devices that were new in the 1980s. The NHS is the largest user of fax machines in the world and is home to 10% of the pagers in circulation worldwide. Abstract:I bet you're reading this on your phone, right? Maybe on the train or waiting for the kettle to boil. Some of you, go on, admit it, are reading this in the loo. OK, so it might not be a phone, but it will likely be some kind of tech: tablet, Kindle, maybe a computer. For many people, our tech is the gateway into the digital world that we use every single day, practically every minute that we are awake. Against this backdrop, the announcement of a digital patient passport, that carries our medical record with us, makes sense. It may even seem to be so obvious that it's odd the NHS hasn't done it yet. On the day of the announcement I told Wes Streeting, our Secretary of State for Health and Social Care, that I welcome his goal of more easily accessible medical records across trusts and for patients. But, before I could endorse it, I needed to know the answer to an important question: for their latest foray into the shift from analogue to digital, just who were the Government including in the consultation and how were they going to consult with them? I ask, because not everyone has a smart phone. Not everyone has a tablet or computer. Heck, lots of people in the UK don't even have regular access to the internet1,2 — and it's not just patients. Abstract:So much for decentralisation: England’s health secretary can’t keep his hands off the NHS It hasn’t taken long for former health secretary Andrew Lansley’s plan for a “hands-off” NHS to hit the rocks. His successor, Jeremy Hunt, is behaving as if the Health and Social Care Act 2012, with its careful separation of powers, had never passed into law. If there’s a problem—and there are plenty—it isn’t long before Hunt comes up with a new wheeze for dealing with it, as health secretaries always have. From pressure on emergency care to hospital quality or the supposed iniquities of the general practitioner contract, he’s there with an opinion and an initiative. His junior minister, Dan Poulter, has echoed his master’s voice with a plan of his own, suggesting with no great originality that the NHS could save money by better procurement.1 The merits of these interventions matter less than their frequency and specificity, which tell us that the Lansley plan is in tatters. Abstract:Abstract The National Programme for IT (NPfIT) was launched in 2005 to implement 7 nationwide IT services across the National Health Service (NHS). Despite the success of many of these designated “deliverables,” the establishment of a single nationwide electronic health record (EHR) system never fully materialized. As a result, NHS medical records are now stored using a diverse array of alternate EHR systems, which frequently restricts health care practitioners from accessing extensive portions of their patients’ notes. This not only limits their ability to make well-informed clinical decisions but also impacts the quality of care they are able to provide. This article assesses the medical, economic, and bureaucratic implications of an NHS-wide EHR system. Additionally, it explores how the shortcomings of the NPfIT should be addressed when attempting to introduce such a system in the future.

Abstract:Summary Objectives: This paper aims to describe and interpret the implementation of a hospital information system in a large UK hospital. Methods: The paper is based on a longitudinal case study over a three-year period in which a cross section of hospital staff involved with the information system were interviewed. Results and Conclusions: Ambitious government targets for the use of Information Technology in the UK National Health Service sit alongside a history of notable project failures. The decision by a UK hospital to install an advanced, integrated electronic patient record system therefore faced conflicting demands and expectations. This paper suggests that its simple categorisation as either a success or failure is problematic. Rather, the differing viewpoints that lead some clinicians to express “disappointment” with its performance, while others described its features as “tremendous” and managers suggested that the system had become “taken for granted” are explored. A number of broader phenomena relating to the organisational processes surrounding information systems implementation are also identified.

Abstract:Objective: Healthcare systems require transformation to meet societal challenges and projected health demands. Digital and computational tools and approaches are fundamental to this transformation, and hospitals have a key role to play in their development and implementation. This paper reports on a study with the objective of exploring the challenges encountered by hospital leaders and innovators as they implement a strategy to become a data-driven hospital organisation. In doing so, this paper provides guidance to future leaders and innovators seeking to build computational and digital capabilities in complex clinical settings. Methods: Interviews were undertaken with 42 participants associated with a large public hospital organisation within England's National Health Service. Using the concept of institutional readiness as an analytical framework, the paper explores participants' perspectives on the organisation's capacity to support the development of, and benefit from, digital and computational approaches. Results: Participants' accounts reveal a range of specific institutional readiness criteria relating to organisational vision, technical capability, organisational agility, and talent and skills that, when met, enhance the organisations' capacity to support the development and implementation of digital and computational tools. Participant accounts also reveal challenges relating to these criteria, such as unrealistic expectations and the necessary prioritisation of clinical work in resource-constrained settings. Conclusions: The paper identifies a general set of institutional readiness criteria that can guide future hospital leaders and innovators aiming to improve their organisation's digital and computational capability. The paper also illustrates the challenges of pursuing digital and computational innovation in resource-constrained hospital environments.
